Cereal–legume intercropping is promoted within semi-arid regions as an adaptation strategy to water scarcity and drought for low-input systems. Our objectives were firstly to evaluate the crop model APSIM for pearl millet (Pennisetum glaucum (L.))—cowpea (Vigna unguiculata (L.) Walp) intercropping—and secondly to investigate the hypothesis that intercropping provides complimentary yield under drought conditions. The APSIM model was evaluated against data from a two year on station field experiment during the dry season of a semi-arid environment in Patancheru, India, with severe, partial and no water deficit stress (well-watered); densities of 17 and 33 plants per m−2, and intercrop and sole crop production of pearl millet and cowpea. Overall, APSIM captured the dynamics of grain yields, indicated by the Willmott Index of Agreement (IA: 1 optimal, 0 the worst) 0.91 from 36 data points (n), total biomass (IA: 0.90, n = 144), leaf area index (LAI, IA = 0.77, n = 66), plant height (IA 0.96, n = 104 pearl millet) and cowpea (IA 0.81, n = 102), as well as soil water (IA 0.73, n = 126). Model accuracy was reasonable in absolute terms (RMSE pearl millet 469 kg/ha and cowpea 322 kg/ha). However, due to low observed values (observed mean yield pearl millet 1,280 kg/ha and cowpea 555 kg/ha), the relative error was high, a known aspect for simulation accuracy in low-yielding environments. The simulation experiment compared the effect of intercropping pearl millet and cowpea versus sole cropping under different plant densities and water supplies. A key finding was that intercropping pearl millet and cowpea resulted in similar total yields to the sole pearl millet. Both sole and intercrop systems responded strongly to increasing water supply, except sole cropped cowpea, which performed relatively better under low water supply. High plant density had a consistent effect, leading to lower yields under low water supply. Higher yields were achieved under high density, but only when water supply was high: absolute highest total intercrop yields were 4,000 (high density) and 3,500 kg/ha (low density). This confirms the suitability of the common practice among farmers who use the low planting density under water scarce conditions. Overall, this study confirms that intercropping is no silver bullet, i.e. not per se a way to achieve high yield production or reduce risk under drought. It does, however, provide an opportunity to diversify food production by additionally integrating protein rich crops, such as cowpea.

Preeclampsia is not fully understood; and few biomarkers, therapeutic targets, and therapeutic agents for its management have been identified. Original investigative findings suggest that abnormal placentation triggers preeclampsia and leads to hypertension, proteinuria, endothelial dysfunction, and inflammation, which are characteristics of the disease. Because of the regulatory roles that it plays in several metabolic processes, adiponectin has become a cytokine of interest in metabolic medicine. In this review, we have discussed the role of adiponectin in trophoblast proliferation, trophoblast differentiation, trophoblast invasion of the decidua, and decidual angiogenesis, which are the major phases of placentation. Also, we have highlighted the physiological profile of adiponectin in the course of normal pregnancy. Moreover, we have discussed the involvement of adiponectin in hypertension, endothelial dysfunction, inflammation, and proteinuria. Furthermore, we have summarized the reported relationship between the maternal serum adiponectin level and preeclampsia. The available evidence indicates that adiponectin level physiologically falls as pregnancy advances, regulates placentation, and exhibits protective effects against the symptoms of preeclampsia and that while hyperadiponectinemia is evident in normal-weight preeclamptic women, hypoadiponectinemia is evident in overweight and obese preeclamptic women. Therefore, the clinical use of adiponectin as a biomarker, therapeutic target, or therapeutic agent against the disease looks promising and should be considered.

Background: The distinction between ST elevation and nonST elevation infarcts is widely accepted and is employed as a guide to management. Aim: This is review of the world literature to assess the basis for this distinction, since the two studies on which it is based are seriously flawed in method and conclusions. Method: Pathologic and clinical studies were reviewed from the world literature. Finding: The pathology of the two subsets is identical as are the morbidity, mortality and clinical course. Non-ST elevation infarcts are likely to be subsequent, to occur in older patients and to involve the circumflex artery: this subset therefore includes a high-risk group. ST deviation in any part of the electric field of the heart will predictably be accompanied by reciprocal deviation if the entire field of the heart is mapped. Further, ST deviation of infarction is often transient, resolving in minutes so that infarcts will be predictably misclassified. ST deviation per se is therefore not a rational basis for classification of infarcts. In fact, invasive therapy is indicated in both subsets with identical results. Conclusion: The distinction between ST elevation and non-ST elevation infarcts is baseless. The high risk subgroup included in the non-ST elevation infarct set should not be denied the benefit of early invasive therapy. Ann Noninvasive Electrocardiol 2010;15(3):191–199

Diagnostic Cytology Laboratory, Inc., has an outpatient Fine Needle Biopsy Clinic, which evaluated 7,487 fine-needle biopsies (FNB) from January 1989 to February 1992. Two hundred eighty-eight (3.8%) of these specimens were collected from patients 19 years old or younger and this represents the largest study of this population in the scientific literature. The majority of these specimens were obtained from palpable masses in the head and neck region [lymph nodes (58.3%), thyroid (7.6%), and salivary gland (5.2%)] while a smaller number were collected from miscellaneous soft tissue (18.8%) and breast masses (10.1%). Thirteen (4.5%) (from 12 patients) were diagnosed as malignant by FNB and 275 (95.5%) (264 patients) were benign by FNB. This benign:malignant ratio (22:1) is significantly higher than has been reported from tertiary care institutions and is a reflection of the difference of this outpatient population. Two hundred nine of 276 patients (75.7%) had adequate follow-up, including 137 (49.6%) patients followed by observation, 48 (17.4%) patients followed by surgical biopsy, and 24 (8.7%) patients followed by some other modality: imaging studies, flow cytometry, or treatment. There was a single false positive diagnosis (a pilomatrixoma) and one (1) false negative interpretation (a cystic acinic cell carcinoma) resulting in a diagnostic sensitivity of 92.3% and a specificity of 99.6%. The positive predictive value was 92.3%, the negative predictive value was 99.6%, and the test efficiency was 99.3%. The utility, cost-effectiveness and uniqueness of the clinic population is discussed.

Most previous cost-benefit analyses of fluoridation programs have been retrospective in approach and have biased their results in favor of fluoridation. This study performed a prospective cost-benefit analysis of fluoridating a segment of the water supply for Houston, Texas, and explicitly introduced and evaluated the time pattern of the costs and benefits. It was shown that neglect of the time structure of the costs and benefits would significantly bias the results. A benefit-cost ratio of 1.51 and a net present value (or "social profit") of $1,102,970 were found. The results are biased downwards and should be considered a lower bound. Thus the results indicate an investment in a fluoridation program by Houston would be a socially profitable one.

Blood and breath acetaldehyde levels were measured following ethanol ingestion (0.5 ml/kg) in 11 boys familially at risk for alcoholism and 11 age-matched controls. No significant differences were found between groups for acetaldehyde, objective, or subjective measures of intoxication. Previous reports of acetaldehyde as a marker of risk for alcoholism were not confirmed. Baseline behavioral state predicted response to alcohol. Children tended to have a subjective response in a direction opposite from the baseline mood state.

Computerized tomographic dimensions of the foramen magnum of 63 achondroplastic individuals were compared to standards established for nonachondroplastic individuals. The size of the foramen magnum in patients with achondroplasia was small at all ages, particularly in those with serious neurologic problems. The data suggest that measurement of the foramen magnum may identify achondroplastic individuals at high risk of developing neurologic complications.
