We evaluated therapeutic plasma exchange (TPE) efficiency in treatment of a single relapse in steroid-refractory patients with neuromyelitis optica spectrum disorders (NMOSD) in a multi-ethnic resource-limited setting. This was a historical cohort study on the clinical outcomes post-TPE in a multiethnic cohort of 53 steroid-refractory NMOSD patients at a single Malaysian tertiary center. Primary outcomes, assessed both pre- and post-TPE, were Medical Research Council scale of muscle power, Modified Rankin Scale, Expanded Disability Status Scale (EDSS), and visual acuity. Secondary outcomes were ambulatory status and target neurological deficit (TND)-based TPE response. Significant improvements in Medical Research Council, Modified Rankin Scale, EDSS, and visual acuity (P 0.001) were observed at 1-month post-TPE with further improvement of EDSS at 6 months (EDSSΔ6) post-TPE (P 0.001). At 6 months post-TPE, 87% of patients has successful TND-based TPE response and 69.8% were ambulating without support. Patients with anti-aquaporin 4 seronegativity (P = 0.004), myelitis and brainstem features at first relapse (P = 0.004), longer cord lesions (P = 0.030), higher pre-TPE EDSS of ≥8 (P = 0.018) and delayed TPE initiation of14 days (P = 0.047) were significantly associated with improved EDSSΔ6. TND-based TPE response was significant in absence of cord atrophy (P = 0.030). TPE is an effective treatment for steroid-refractory acute relapses of NMOSD in a multiethnic Malaysian population despite its resource-limited setting. The predictive factors of EDSSΔ6 improvement were anti-aquaporin 4 seronegativity, longer cord lesions, and higher pre-TPE EDSS. Absence of cord atrophy was predictive of better TND-based TPE response. Unexpectedly, our study showed that delayed TPE initiation of more than 14 days and up to 60 days may also be beneficial.

Aims:\ud The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials.\ud \ud Methods and Results:\ud Adults with established HFrEF, New York Heart Association functional class (NYHA) ≥ II, EF ≤35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. 9. Cervical cancer prevented by screening: Long-term incidence trends by morphology in Norway

Both major morphologic types of cervical cancer, squamous cell carcinoma (SCC) and adenocarcinoma (AC), are causally related to persistent infection with high-risk human papillomavirus (hrHPV), but screening has primarily been effective at preventing SCC. We analysed incidence trends of cervical cancer in Norway stratified by morphologies over 55 years, and projected SCC incidence in the absence of screening by assessing the changes in the incidence rate of AC. The Cancer Registry of Norway was used to identify all 19,530 malignancies in the cervix diagnosed in the period 1956-2010. The majority of these (82.9%) were classified as SCCs, 10.5% as ACs and the remaining 6.6% were of other or undefined morphology. By joint-point analyses of a period of more than five decades, the average annual percentage change in the age-standardised incidence was -1.0 (95%CI: -2.1-0.1) for cervical SCC, 1.5 (95%CI:1.1-1.9) for cervical AC and -0.9 (95%CI: -1.4 to -0.3) for cervical cancers of other or undefined morphology. The projected age-standardised incidence rate of cervical SCC in Norway, assuming no screening, was 28.6 per 100,000 woman-years in 2010, which compared with the observed SCC rate of 7.3 corresponds to an estimated 74% reduction in SCC or a 68% reduction due to screening in the total cervical cancer burden. Cytology screening has impacted cervical cancer burden more than suggested by the overall observed cervical cancer incidence reduction since its peak in the mid-1970s. The simultaneous substantial increase in cervical adenocarcinoma in Norway is presumably indicative of an increase in exposure to HPV over time.

Summary Background Fixed airflow limitation can be found both in asthma and chronic obstructive pulmonary disease (COPD), posing a day-to-day diagnostic challenge. Objective We aimed to determine the external validity of metabolomic analysis of exhaled air by electronic nose for distinguishing asthma and COPD in patients with fixed airways obstruction. Methods One hundred patients were included in a cross-sectional design: 60 asthma patients: 21 with fixed airways obstruction (fixed asthma), 39 with reversible airways obstruction (classic asthma) and 40 COPD patients (GOLD stages II–III). Standardized sampling of exhaled breath was performed and volatile organic compounds were captured using an electronic nose resulting in breathprints. External validity in newly recruited patients (validation sets) was tested using a previous and independent training set. Breathprints were analysed by principal component and canonical discriminant analysis and area under the curve (AUC) of receiver operating characteristic curves. Results External validity of breathprints showed 88% accuracy for distinguishing fixed asthma from COPD (AUC 0.95, 95% CI 0.84–1.00, sensitivity 85%, specificity 90%) and 83% for classic asthma (AUC 0.93, 95% CI 0.87–1.00, sensitivity 91%, specificity 90%) (both P

Zusammenfassung Milchzusammensetzung und -leistung von hollandischen Warmblutstuten In der vorliegenden Untersuchung werden Angaben uber die Milchzusammensetzung von hollandischen Warmblutstuten wahrend der gesamten Laktation gegeben. Danach sinkt der Gehalt an Trockensubstanz, Eiweis, Fett, Calcium, Phosphor (P ≤ 0,01) und das Ca: P-Verhaltnis (P ≤ 0,05), wahrend der Gehalt an NfE (P ≤ 0,01) im Verlauf der Laktation ansteigt. Es wird auserdem vermutet, das eine Veranderung der Milchzusammensetzung wahrend der Fohlenrosse nicht als Ursache fur Durchfallerkrankungen der Fohlen verantwortlich ist. Durch Wiegen des Fohlens vor und nach dem Saugen uber 24 Stunden wurde die Milchleistung der Stuten gemessen. Die Stuten erbrachten eine durchschnittliche Milchleistung von 1617 ± 42,6 kg Milch in 100 Laktationstagen. Die hochste Milchleistung wurde in der 2. Halfte des 2. Laktationsmonats gemessen. Bei einem durchschnittlichen Geburtsgewicht von 57,2 kg verdoppelten die Fohlen das Geburtsgewicht in 37,5 Tagen.

15. Changes in gastric electrical activity in patients with severe post-operative nausea and vomiting

Gastric electrical activity in a group of patients with severe nausea and repeated vomiting in the post-operative phase following major abdominal surgery (Group A, n = 21), was compared to that in a control group of patients without symptoms after colonic surgery (Group B, n = 14). Physical signs related to the digestive tract were assessed and electrical activity of the stomach was measured non-invasively by means of electrogastrography (EGG) and impedance gastrography (IGG). Two EGG/IGG recordings were performed in each patient. In group A the first recording was carried out after the onset of nausea and/or vomiting and a second control recording after disappearance of these symptoms and resumption of a normal diet. In group B the post-operative recording was performed on the second post-operative day, the control EGG/IGG signals were recorded pre-operatively. EGG and IGG could be successfully recorded in the post-operative phase, even in nauseated and vomiting patients. During post-operative nausea and vomiting power and frequency of the normal gastric myoelectrical activity (0.04–0.06 Hz) were not significantly lower than in the control recordings. In the first post-operative recordings (in Group A during nausea and vomiting and in Group B on the second post-operative day) an increased incidence of abnormal gastric frequencies (tachygastrias, 0.10–0.18 Hz) was observed in both groups. No significant difference between the groups was found. After nausea and vomiting had subsided in Group A, the incidence of tachygastrias was persistently high.

This study of gastric ulcer patients examined whether characteristic abnormalities in the fasting and postprandial gastric myoelectrical activity could be discerned electrogastrographically and whether these abnormalities, if present, were related to ulcer characteristics and complaints of nausea and vomiting. A total of 61 recordings were made in 31 gastric ulcer patients, both at the time of diagnosis and after ulcer healing, and these were compared to 52 recordings made in 52 control subjects. Several types of abnormal gastric myoelectrical activity were found in patients with an active ulcer: 1) instability of both the fasting and postprandial gastric frequency; 2) absence of the normal postprandial amplitude increase in the electrogastrogram; 3) waveform changes in the electrogastrographic signal in both the fasting and postprandial states; and 4) tachygastrias. These abnormalities were predominantly found in ulcer patients with symptoms of nausea and vomiting. They disappeared with ulcer healing and resolution of nausea and vomiting. These findings indicate that the abnormalities in gastric myoelectrical activity found in gastric ulcer patients are not merely due to the presence of the ulcer itself, but are associated with the presence of nausea and vomiting.

We determined the value of galactomannan (GM) detection in computerized tomography (CT)-based broncho-alveolar lavage (BAL) fluid and serum for the diagnosis of invasive pulmonary aspergillosis (IPA) in haemato-oncological patients with neutropenia. CT of the thorax and BAL were performed systematically at predefined clinical indications. GM was determined by sandwich enzyme-linked immunosorbent assay; the clinicians were unaware of the results. Of 160 patients, 17 patients (10.6%) presented with proven, probable or suspected IPA. The sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of GM detection in CT-based BAL fluid were all 100%. For GM detection in serially sampled serum, the sensitivity was 47%, the specificity 93%, the PPV 73% and the NPV 82%. A non-blinded follow-up study was performed to validate these results. In this study, 22 of 198 patients (11.1%) presented with IPA, and the sensitivity, specificity, PPV and NPV of GM detection in CT-based BAL fluid were 85%, 100%, 100% and 88% respectively. None of BAL fluids obtained after antifungal treatment of 3 d or more were positive. These results indicate that, when CT is used systematically and at an early stage, GM detection in CT-based BAL fluid has a high PPV for diagnosing IPA early in untreated patients.

The mehanisms for loss of CO from benzaldoxime and acetophenone oxime have been investigated with the aid of both fluorine and deuterium labelling as well as field free region metastable peak shapes. Kinetic evergy release data and the metastable ion abundance raion test were used to obtain information about the product ions. It is proposed that loss of CO from both are generated via the nucleophilic attack of the oxygen lone pair eletrons at the ortho position of the phenyl ring as the first step in the reaction sequence.

The purpose of this study was to follow by i.r. analysis double bond shifts and chemical modification of natural rubber. Accurate determination of the original rubber spectrum is facilitated by the use of an internal standard. Isorubber, prepared by thermal dissociation of rubber hydrochloride, contains about 50% of double bonds in the side groups. Although a similar double bond shift is observed in the cyclization of rubber, part of the absorption, in the 11 μ region, may here be due to ring formation. Vulcanization with accelerated sulfur mixtures does not produce changes in the i.r. spectrum, unless the rubber is transformed into a derivative by overcuring with S2Cl2 or during hard rubber formation. At least three types of oxidation reactions can be distinguished spectroscopically: (1 ) degradation of the rubber (by oxidation of a rubber solution with air) with the ultimate formation of only volatile oxidation products; (2 ) gradual formation of oxygenated derivatives (oxidation with ozone), with a distinguishable structure in the spectrum; (3 ) gradual formation of an oxidized and more saturated rubber (oxidation of solid rubber with air), with less characteristic broad absorption bands.

It is shown that the elemental composition of a product ion generated from a multiply charged ion in a charge separation reaction can be determined with a double focusing instrument under conditions of high mass and energy resolution. Results for a number of charge separation reactions in benzylamine are briefly discussed.
