Within the last 20 years, tumour necrosis factor inhibitors have been proven to be effective in achieving and maintaining clinical and endoscopic remission in patients with Crohn's disease and ulcerative colitis. Since 2013, when infliximab originator lost its patent protection, patients with inflammatory bowel diseases (IBDs) in Poland have also been treated with biosimilar drugs. Biosimilars are drugs with high similarity to their reference products in terms of physicochemical properties, including structure, safety, and efficacy. Biosimilars are approved for use on the basis of the same rigorous quality standards as their reference products. In 2018, also biosimilars of adalimumab have become available. Studies published to date have shown that biosimilars do not differ from reference drugs in terms of the efficacy and safety. There are numerous data to confirm that a single switch of biological drugs (mainly from reference to biosimilar drugs) has no effect on therapy efficacy and safety. However, a significantly lower cost of therapy with biosimilars not only allows us to treat a much larger number of patients but may also necessitate multiple switches from reference drugs to biosimilars (including biosimilars produced by different manufacturers). Recently, the first results have been published concerning multiple switches in patients with psoriasis and rheumatoid arthritis. However, no such data are currently available for patients with IBDs.

This article describes the latest diagnostic and therapeutic recommendations in chronic pancreatitis, developed by the Working Group of the Polish Society of Gastroenterology and the Polish Pancreas Club. The recommendations refer to the diagnosis of chronic pancreatitis, autoimmune pancreatitis, conservative management, treatment of pain, and exocrine and endocrine pancreatic insufficiency, treatment of chronic pancreatitis by endoscopic and surgical methods, and oncological surveillance of chronic pancreatitis. This paper refers to the Polish recommendations published in 2011, which have been updated and supplemented. All recommendations were voted by experts of the Polish Society of Gastroenterology and the Polish Pancreas Club, who evaluated them each time on a five‑degree scale, where I meant full acceptance, II – acceptance with some reservation, III – acceptance with serious reservation, IV – rejection with some reservation and V – full rejection. The results of the voting, together with a brief commentary, have been included with each recommendation put to the vote. In addition, the expert group assessed the value of clinical studies on which the statements are based, on a scale where A means high (based on meta‑analyses and randomised clinical trials), B means medium (based on clinical trials and observational studies), and C means low (based mainly on expert opinion).

Introduction Small intestinal bacterial overgrowth (SIBO) may coexist with Helicobacter pylori infection, which can be the cause of chronic gastrointestinal complaints. Aim Evaluation of the therapeutic efficacy of amoxicillin and rifaximin in the treatment of these diseases. Material and methods The lactulose hydrogen breath test (LHBT) and the urea breath test (13C-UBT) were performed in 116 patients. In 62 patients the coexistence of small intestinal bacterial overgrowth and H. pylori infection was observed. Then, in group I (n = 30) pantoprazole (2 × 40 mg), amoxicillin (2 × 1000 mg) and metronidazole (2 × 500 mg) and in group II (n = 32) pantoprazole and amoxicillin at the above doses and rifaximin (3 × 400 mg) were administered for 10 days. After 6 weeks, both breath tests were repeated and the degree of remission of symptoms was measured using a 10-point visual analog scale (VAS). Results After the treatment the LHBT index decreased in group I from 61.2 ±19.4 ppm to 22.0 ±8.2 ppm (p 0.05). The decrease of pain below 3.0 points in the VAS was obtained in 64.8% of patients in group I and in 56.2% in group II. Conclusions Combination of amoxicillin and rifaximin may be effective in the treatment of patients with small intestinal bacterial overgrowth syndrome and concomitant H. pylori infection.

Introduction: Helicobacter pylori infection can induce disorders of myoelectric stomach functions and can have an influence on the frequency and grade of intensification of acid pathological gastroesophageal reflux (GER). The GER pathomechanism is also closely connected with stomach motor activity by affecting the tension in the lower oesophageal sphincter. Aim: The aim of the study was to try to answer whether there are significant disorders in myoelectric stomach functions in children and teenagers with gastritis and/or duodenitis coexisting with H. pylori infection and GERD (gastroesophageal reflux disease). Material and methods: One hundred and one patients over the age of 3 with dyspeptic symptoms who underwent endoscopic examination of the upper part of the digestive tract, pH-metric oesophagus test, and electrogastrographic test (EGG) were qualified for the examination. Helicobacter pylori infection was confirmed in the histopathological test and/or in the urease test and urea breathing test. Results: Among the examined patients in 50 cases the result of the pH-metric test was abnormal (group A [n = 50]), and in 51 patients the reflux index was below 4% (group B [n = 51]). In the analysis of the difference in percentage of normogastria, bradygastria, tachygastria and arrhythmia appearance before and after a meal in the EGG record in patients with confirmed H. pylori infection in group A (incorrect result of pH-metric test) and in the group of children with the correct result of the pH-metric test (group B) a statistically signi ficantly higher percentage of bradygastria was verified in Streszczenie Wstep: Zakazenie Helicobacter pylori moze indukowac zaburze nia czynności mioelektrycznej zolądka i wplywac na czestośc oraz stopien nasilenia kwaśnego patologicznego refluksu zolądkowo-przelykowego. Material i metody: Do badania zakwalifikowano 101 pacjentow powyzej 3. roku zycia z objawami dyspeptycznymi, u ktorych wykonano badanie endoskopowe gornego odcinka przewodu pokarmowego, badanie pH-metryczne przelyku oraz badanie elektrogastrograficzne (EGG). Zakazenie H. pylori potwierdzono w badaniu histopatologicznym i/lub w teście ureazowym oraz mocznikowym teście oddechowym. Wyniki: Wśrod 101 pacjentow u 50 chorych wynik badania pH-metrycznego byl nieprawidlowy [grupa A (n = 50)], u 51 pa cjentow indeks refluksowy wynosil ponizej 4% [grupa B (n = 51)]. Streszczenie Przewod pokarmowy plodu jest jalowy, a jego kolonizacja zaczyna sie natychmiast po urodzeniu i jest zalezna od wielu czynnikow, miedzy innymi od sposobu karmienia. U noworodkow i niemowląt karmionych naturalnie dominują bakterie rodzaju Bifidobacterium i Lactobacillus, natomiast u karmionych mieszankami modyfikowanymi dominują beztlenowce Bacteroides, Staphylococcus, Escherichia coli, Clostridia, a bifidobakterie stanowią mniejszy odsetek. Szczegolnie Bifidobacterium i Lactobacillus oddzialują korzystnie na blone śluzową oraz uklad immunologiczny przewodu pokarmowego. Ekosystem mikrobiontow przewodu pokarmowego pelni funkcje troficzne, metaboliczne i ochronne, miedzy innymi odpowiada za fermentacje niestrawionych weglowodanow do krotkolancuchowych kwasow tluszczowych, ktore są glownym źrodlem energii dla kolonocytow. Mikroorganizmy przewodu pokarmowego odgrywają role w produkcji witamin (szczegolnie witaminy K oraz witamin z grupy B), a takze absorpcji wapnia i magnezu. Stymulują rowniez uklad odpornościowy. Streszczenie Potwierdzenie skuteczności dzialania probiotykow w roznych jednostkach chorobowych powoduje ich coraz czestsze stosowanie w praktyce. Liczne badania randomizowane, a takze metaanalizy wskazują na duze bezpieczenstwo dostepnych preparatow, a obserwowane dzialania niepoządane wystepują bardzo rzadko. Nalezy jednak pamietac o doniesieniach o wystepowaniu bakteriemii lub fungemii w przebiegu stosowania probiotykow u pacjentow z zaburzeniami odporności czy chorobami przewleklymi.

Relationships Between Systolic Time Intervals and Heart Rate During Initial Response to Orthostatic Manoeuvre in Men of Different Age An analysis of transient changes in physiological parameters in response to the standardized tests could be used to evaluate the efficiency of the regulatory processes. Relationships between systolic time intervals and heart rate following the action of standing up from the supine position were investigated in 41 healthy men, aged 20 to 59 years, classified into three groups: (22 to 26 yrs, n=14), (33 to 49, yrs, n=13) and (51 to 59 yrs, n=14). The protocol consisted of the following sequence: laying down (20 minutes) - standing up (8 minutes). Ejection time, pre-ejection period, electromechanical systole, heart rate and the length of R-R intervals were continuously calculated using automatized impedance cardiography and electrocardiogram. The ratio of ejection time to pre-ejection period in young men was significantly higher in comparison with the other groups. The ratio of ejection time to the length of R-R interval increased with age in supine position and after standing up when R-R interval was maximal. It was suggested that changes of ejection time to pre-ejection period during the orthostatic manoeuvre are rather the result of balance between heart rate and hemodynamic factors, than solely related to heart rate.

Determination and Prediction of One Repetition Maximum (1RM): Safety Considerations Strength training is recommended for slowing age-dependent deterioration of muscular strength and for rehabilitating patients with muscle weakening illnesses. Reliable assessment of muscle strength is important for proper design of strength training regimes for prevention, rehabilitation, and sport. One repetition maximum (1RM) is an established measure of muscular strength and is defined as the value of resistance against which a given movement can be performed only once. Proper assessment of 1RM is time consuming, and may lead to muscle soreness as well as temporary deterioration of the function of the tested muscles. Attempts at indirect 1RM determination based on the maximum number of repetitions performed have predicted 1RM with a variable degree of accuracy. Cardiovascular safety has been neglected in 1RM determination, although arterial blood pressure increases considerably when exercising against maximal or near maximal resistance. From the perspective of cardiovascular safety, favorable 1RM measurement methods should avoid performance of repetitions until failure; movement against high resistance and muscle fatigue both increase blood pressure. Although such techniques are likely less accurate than the current methods, their prediction accuracy be sufficient for therapeutic strength training.

Zatrucia przypadkowe stanowią istotną przyczyne zachorowalności i śmiertelności dzieci i mlodziezy w wiekszości krajow świata. Celem pracy bylo porownanie przyczyn i okoliczności oraz przebiegu klinicznego zatruc przypadkowych u dzieci i mlodziezy w latach 1993–1995 oraz 2003–2005. Material pracy stanowila dokumentacja medyczna 176 pacjentow hospitalizowanych w Katedrze i Klinice Pediatrii, Alergologii i Gastroenterologii w Bydgoszczy oraz na Oddziale Dzieciecym Szpitala Powiatowego w Chelmnie z powodu zatruc przypadkowych. W pracy oceniano czestośc zatruc w stosunku do liczby dzieci hospitalizowanych, wiek, w ktorym najcześciej dochodzilo do zatruc, rodzaj substancji toksycznej. Oceniano takze przebieg kliniczny i czas hospitalizacji, miejsce zamieszkania, wiek rodzicow oraz warunki socjalne. Wyniki Analizą objeto 176 pacjentow (98 dziewczynek i 78 chlopcow) w wieku od 3 miesiecy do ukonczenia 18 roku zycia. W latach 1993–1995 najczestszą przyczyną zatruc byly leki, a zatrucia dotyczyly przede wszystkim dzieci w wieku pomiedzy 2 a 5 rokiem zycia. Natomiast w latach 2003–2005 najczestszą przyczyną zatruc przypadkowych byla ekspozycja na gaz pieprzowy, a zatrucia dotyczyly wszystkich grup wiekowych. Przebieg kliniczny zatruc w latach 1993–1995 byl ciezszy niz w latach 2003–2005. Skroceniu ulegl calkowity czas hospitalizacji. Wnioski Czestośc ostrych zatruc przypadkowych nie wykazuje tendencji spadkowej i dotyczą one wszystkich grup wiekowych. W ostatnich latach wyraźnie wzrosla czestośc zatruc przypadkowych wśrod mlodziezy, a zatrucia te są związane z eksperymentalną ekspozycją na latwo dostepne substancje toksyczne (gaz drazniący, rośliny halucynogenne i alkohol etylowy). Przebieg kliniczny ostrych zatruc jest lagodny i wydaje sie, ze w wielu przypadkach nalezaloby obserwowac pacjenta w warunkach ambulatoryjnych lub na Szpitalnym Oddziale Ratunkowym po wcześniejszej konsultacji z Regionalnym Ośrodkiem Toksykologicznym.

Profilaktyka pierwotna chorob ukladu sercowo-naczyniowego polegająca na wczesnej identyfikacji osob wysokiego ryzyka rozwoju miazdzycy i podejmowaniu dzialan zmierzających do modyfikacji tych czynnikow powinna obejmowac dzieci i mlodziez. Miazdzyca jest przewleklym procesem chorobowym zaczynającym sie w dziecinstwie, uwarunkowanym wplywem czynnikow genetycznych i środowiskowych. Początkowo dochodzi do uszkodzenia komorek środblonka, nastepnie do powstania blaszek miazdzycowych zawierających glownie cholesterol i jego estry. W patogenezie miazdzycy istotną role odgrywają ponadto zaburzenia krzepniecia, fibrynolizy oraz czynniki infekcyjne.
