Background: Operators in the obituary and necropsy sectors are exposed to various environmental hazards during specific tasks. Despite this exposure, occupational risks have often been underestimated, resulting in a lack of substantial evidence. The primary objectives of this study were to identify sources of chemical risk, establish procedures for monitoring and quantifying exposure during necropsy activities, and recommend adjustments to regulatory guidelines to protect the health of the operators. The study was conducted at the Legal Medicine Unit of the Umberto I General Hospital in Rome, focusing on the quantitative measurement of particulate matter (PM) exposure among at-risk operators during necropsy activities. Environmental levels of total suspended particles, PM10, PM4, PM2.5, and PM1 were assessed by evaluating the average, minimum, and maximum instantaneous indoor concentrations using an airborne analyzer. Results: The monitoring activities revealed that the PM concentrations were significantly lower than the recognized reference values. However, bone sawing, body removal, and cleaning were identified as high-risk maneuvers for dust suspension. Conclusions: Our study highlighted specific risks associated with necropsy activities, particularly concerning timing and certain maneuvers. These results may lead to interventions for improving current prevention procedures, implementing good practices, and developing specific guidelines to enhance operator safety. Balneotherapy includes practices and methods using medically and legally recognized mineral-medicinal waters, muds and natural gases from natural springs for therapeutic purposes. One of the most widely used method in balneotherapy is bathing with thermal mineral water. In the course of the years, scientific community has produced an increasing number of evidences that this practice is an effective method for treating signs and symptoms of several pathologies such as rheumatic, cardiovascular and dermatological diseases. This systematic review is aimed at evaluating the effect of balneotherapy with thermal water baths as a treatment to manage signs and symptoms of patients affected by all types of dermatological diseases. The systematic review was conducted according to the PRISMA Statement, and its protocol was registered on PROSPERO platform (CRD42022295913). The research was performed on the databases Pubmed, Scopus, Web of Science and Cochrane. We included clinical trials evaluating the effects of balneotherapy using thermal mineral water baths for managing dermatological diseases in humans, published in English and Italian language. Eight studies were included, seven of them enrolled adults affected by psoriasis and one studied atopic dermatitis patients. The common result of all the articles included was a clear improvement of signs and symptoms of psoriasis and eczematous diseases after use of thermal mineral water baths. These effects seem to be strictly related to physical and chemical properties of thermal water used for balneotherapy. However, studies in this field are still limited to support robust evidence of the effectiveness of balneotherapy using thermal mineral water baths and often their quality is low. Formaldehyde is a toxic and carcinogenic compound, still used in several occupational settings due to its properties. Thus, in these working scenarios, it is necessary to provide effective measures to reduce workers' exposure to formaldehyde. The aim of this systematic review is to provide a picture of the worldwide mitigation strategies implemented in occupational environments for minimizing the exposure to formaldehyde and which ones are the most effective for this purpose. The systematic review was performed according to PRISMA statement; the protocol was registered in PROSPERO (CRD42022302207). The search was performed on three electronic databases (PubMed, Scopus, and Web of Science). Studies were considered eligible if they describe strategies for mitigating formaldehyde occupational exposure and their efficacy. We included articles reporting observational studies, semi-experimental, and experimental studies and published in the English language, from the inception to March 26th, 2023. The quality assessment was performed using the Newcastle–Ottawa Quality Assessment Scale. In total, 28 articles were included in the review. The employment scenarios/activities studied were human and veterinary anatomy, autopsy, histopathology or pathology laboratories, embalming procedures, hospital, operating theaters, aquaculture, textile or foundry industries, industry using 3-D printers, offices, and firefighters' activities. Different methods have proven useful in mitigating formaldehyde exposure, such as the use of personal protective equipment, engineering control methods, organization methods, and technical strategies, with a reduction of airborne formaldehyde until to 99.6%. The highest reduction was obtained in an anatomy laboratory through locally exhausted dissection tables equipped with activated carbon filters. This systematic review is aimed to evaluate the effects of balneotherapy with thermal mineral water for managing the symptoms and signs of osteoarthritis located at any anatomical site. The systematic review was conducted according to the PRISMA Statement. The following databases were consulted: PubMed, Scopus, Web of Science, Cochrane Library, DOAJ and PEDro. We included clinical trials evaluating the effects of balneotherapy as a treatment for patients with osteoarthritis, published in English and Italian language, led on human subjects. The protocol was registered in PROSPERO. Overall, 17 studies have been included in the review. All of these studies were performed on adults or elderly patients suffering from osteoarthritis localized to knees, hips, hands or lumbar spine. The treatment assessed was always the balneotherapy with thermal mineral water. The outcomes evaluated were pain, palpation/pressure sensibility, articular tenderness, functional ability, quality of life, mobility, deambulation, ability to climb stairs, medical objective and patients' subjective evaluation, superoxide dismutase enzyme activity, serum levels of interleukin-2 receptors. The results of all the included studies agree and demonstrated an improvement of all the symptoms and signs investigated. In particular, pain and quality of life were the main symptoms evaluated and both improved after the treatment with thermal water in all the studies included in the review. These effects can be attributed to physical and chemical-physical properties of thermal mineral water used. However, the quality of many studies resulted not so high due and, consequently, it is necessary to perform new clinical trial in this field using more correct methods for conducting the study and for processing statistical data. Perfluorinated compounds (PFCs) are a wide class of emerging pollutants. In this study, we applied the US EPA method 533 for the determination of 21 PFCs in river water samples. In particular, this method was used to investigate the presence of the target PFCs in six rivers in central Italy during a 4-month-long monitoring campaign. In 73% of the analyzed samples, at least some of the target PFCs were detected at concentrations higher than the limit of detection (LOD). The sum of the 21 target analytes (∑21PFCs) ranged from 4.3 to 68.5 ng L−1, with the highest concentrations measured in the month of June, probably due to a minor river streamflow occurring in the warmer summer months. Considering the individual congeners, PFBA and PFPeA, followed by PFHxA and PFOA, were the predominantly detected compounds. Short- and medium-chain PFCs (C4–C9) prevail over the long-chain PFCs (C10–C18), likely due to the increased industrial use and the higher solubility of short-chain PFCs compared to long-chain PFCs. The ecological risk assessment, conducted by using the risk quotient method, highlighted that the risk for aquatic environments associated with PFBA, PFPeA, PFBS, PFHxA and PFOA was low or negligible. Only for PFOA, there was a medium level of risk in two rivers in the month of June. With regard to PFOS, 54% of the river water samples were classified as "high risk" for the aquatic environment. The aim of the study was to evaluate occupational exposure to 40 elements among a group of Eritrean adults employed at the Medeber metal market by analysing human scalp hair samples and by investigating the role of some predictors (gender, age, and body mass index—BMI) on their levels. Scalp hair samples were collected from 60 subjects (32 subjects working at the Medeber metal market and 28 subjects not working at Medeber and not employed in other industrial or artisanal activities) and were analysed through inductively coupled plasma mass spectrometry to determine the contents of 40 major and trace elements. Information on participants was collected via questionnaire. Significant differences were found in the hair levels of certain elements between subjects working at the Medeber metal market and subjects not exposed. In particular, working at the Medeber metal market increased hair levels of Be, B, Cu, Zn, Mo, Cd, Sb, Tl, Pb, and Fe from 25 to 100%, while reduced Na, K, and Rb levels from 30 to 50%. With multivariate adjustment for age, gender, and BMI, the results confirm that working at Medeber significantly increased the hair levels of Cu, Zn, Sb, Pb, and Fe. This finding reveals the need for more accurate and routine biomonitoring surveys and the necessity of health promotion campaigns both for local decision-makers and workers to increase health promotion and safety in this workplace. Purpose of Review: Modified risk products (MRP) are promoted as a safer alternative to traditional combustion cigarettes (TCC) in chronic smokers. Evidence for their lower hazardous profile is building, despite several controversies. Yet, it is unclear whether individual responses to MRP differ among consumers. We hypothesized that different clusters of subjects exist in terms of acute effects of MRP. Recent Findings: Pooling data from a total of 60 individuals, cluster analysis identified at least three clusters (labelled 1 to 3) of subjects with different electronic vaping cigarettes (EVC) effects and at least two clusters (labelled 4 to 5) of subjects with different heat-not-burn cigarettes (HNBC) effects. Specifically, oxidative stress, platelet aggregation, and endothelial dysfunction after EVC were significantly different cluster-wise (all p < 0.05), and oxidative stress and platelet aggregation after HNBC were significantly different (all p < 0.05). In particular, subjects belonging to Cluster 1 appeared to have less detrimental responses to EVC usage than subjects in Cluster 2 and 3, as shown by non-significant changes in flow-mediated dilation (FMD) and less marked increase in Nox2-derived peptide (NOX). Conversely, those assigned to Cluster 3 had the worst reaction in terms of changes in FMD, NOX, and P-selectin. Furthermore, individuals belonging to Cluster 4 responded unfavorably to both HNBC and EVC, whereas those in Cluster 5 interestingly showed less adverse results after using HNBC than EVC. Results for main analyses were consistent employing different clusters, tests, and bootstrap. Summary: Individual responses to MRP differ and smokers aiming at using EVC or HNBC as a risk reduction strategy should consider trying different MRP aiming at finding the one which is less detrimental, with subjects resembling those in Cluster 1 preferably using EVC and those resembling Cluster 5 preferably using HNBC. Phthalates and bisphenol-A are molecules widely used in packaging because they increase the plastic malleability and workability but they show the tendency to migrate or dissociate from plastics. Recently, researchers raised the alarm to have found their traces in water samples of plastic bottles left at high outdoor temperatures for a long time. The paper would like to show a simple, sensitive, and reproducible method for the simultaneous determination of phthalates and bisphenol-A in drinking water, based on the dispersive liquid-liquid microextraction followed by gas chromatography-mass spectrometry analysis. The dispersion is favored by means of ultrasonic bath and stirring magnetic plate, without dispersive solvent. The method presents an average R2 0.993 in the range 10–5000 ng mL-1, a limit of detection below 1.2 ng mL-1, and a limit of quantification below 7.7 ng mL-1. The release of such compounds from different beverage containers (6 plastic bottles, 6 canteens, and 3 newborn feeding bottles) has been analyzed. The release kinetics from the bottles are studied over 2 months, whereas over 6 h for the other containers. Only 2 compounds have been found in a plastic bottle and in a canteen bottle, with concentrations ranging between 24 and 117 ng mL-1. [ABSTRACT FROM AUTHOR]

Baby food is made specifically for babies between the ages of 4/6 months up to 2 years, and it comes in multiple varieties and tastes. This paper proposes a protocol for a rapid and reliable determination of 19 organophosphorus pesticides (OPs) by means of a method based on ultrasound-vortex-assisted liquid-liquid microextraction coupled with a gas chromatography-ion trap mass spectrometry (UVALLME-GC-IT/MS). The method development starts from the extraction solvent evaluation, i.e., n-heptane; the solution, held for 5 min on the vortex mixer and for 6 min in an ultrasonic bath to 100 W for favoring the solvent dispersion and the determinant extraction, is centrifuged at 4000 rpm for 30 min: 1 μL of the organic extract is injected into the GC-IT/MS. All the analytical parameters investigated are deeply discussed; LODs/LOQs (0.2-1.3 and 0.5-2.9 ng/g) and recoveries (81-109%) are compared with other papers dealing the determination of OPs in baby food matrix. A HS-SPME method coupled with GC-MS analysis has been developed for simultaneously measuring the concentration of 10 volatile organic compounds (VOCs) (benzene, toluene, ethylbenzene, o-, m-, and p-xylene, methyl tert-butyl ether, ethyl tert-butyl ether, 2-methyl-2-butyl methyl ether, and diisopropyl ether) in urine matrix as a biomonitoring tool for populations at low levels of exposure to such VOCs. These compounds, potentially toxic for human health, are common contaminants of both outdoor and indoor air, as they are released by autovehicular traffic; some of them are also present in environmental tobacco smoke (ETS). Thus, the exposure to these pollutants cannot be neglected and should be assessed. The low limits of detection and quantification (LODs and LOQs <6.5 and 7.5 ng L, respectively) and the high reproducibility (CVs <4 %) make the developed method suited for biomonitoring populations exposed at low levels such as children. Further, the method is cost-effective and low in time-consumption; therefore, it is useful for investigating large populations. The trace element composition of water intended for human consumption plays an important role for public health. Water is a significant source of potentially toxic and essential elements; it can provide the body by both direct recruitment and/or influencing the element concentration in agriculture, livestock products and food industry. The present study examines the concentrations of a great number of elements in the drinking waters distributed in the territory of Rome, whose water supply mainly comes from spring waters. 20 samples drawn from municipal potable facilities in Rome were analyzed by Instrumental Neutron Activation Analysis. The neutron irradiations were performed in the pneumatic channel and in the rotating rack of ENEA's Triga Mark II reactor at 1.3 × 10 and 2.6 × 10 n cm s, respectively. In total, 38 elements were determined in each sample. The results evidence that all the studied elements for all the aqueducts resulted well below the limits posed by the regulations on water for human consumption. Besides, considering the elements not regulated, the monitored waters resulted safe from a toxicological point of view, and adequate from nutritional aims. In addition, it was also evidenced that the elemental composition resulted quite similar to that of uncontaminated natural water. The ability of a transplanted lichen, Pseudovernia (P.) furfuracea, to act as a multi-tracer biomonitoring tool for As, Cd, Ni, Pb, 12 PAHs, 17 polychlorinated dibenzo- p-dioxins (PCDDs) and dibenzofurans (PCDFs) and 27 polychlorinated biphenyls (PCBs) was evaluated at six areas of varying risk (high, medium, negligible) of pollutant fallout from a municipal waste incinerator in central Italy. Transplanted P. furfuracea proved to be an useful tool to biomonitor PCDDs/Fs and PCBs. Concentrations of As, heavy metals, PAHs, PCDDs/Fs resulted similar for all monitored stations. Small differences in total PCBs (4378 and 4631 pg/g dw vs 3298, 4123, 3676 and 4022 pg/g dw) and dioxin-like PCBs (1235 and 1265 pg/g dw vs 794, 1069, 1106 and 1188 pg/g dw) were observed. The aim of this research is to evaluate the ability of transplanted lichen Pseudovernia ( P) . furfuracea to biomonitor and bioaccumulate in urban indoor environments. The elements As, Cd, Cr, Cu, Hg, Ni and Pb and 12 selected polycyclic aromatic hydrocarbons (PAHs) were used to assess P. furfuracea as a biomonitoring tool for the indoor air quality of school environments. To achieve this purpose, lichen samples were exposed for 2 months in the outdoor and indoor environments of five school settings located in urban and rural areas. The results demonstrated that transplanted lichen P. furfuracea is a suitable biomonitoring tool for metals and PAHs in indoor settings and can discriminate between different levels of air pollution related to urbanisation and indoor conditions, such as those characterised by school environments. A transplanted lichen biomonitoring strategy is cost-effective, 'green', educational for attending children and less 'invasive' than traditional air sampling methods. The feasibility of indoor monitoring by P. furfuracea is a relevant finding and could be a key tool to improve air quality monitoring programmes in school scenarios and thus focus on health prevention interventions for children, who are one of the most susceptible groups in the population. Objectives: To show the efficiency and safety of a surgical treatment in both acute and chronic acromio-clavicular junction (ACJ) dislocations. Design: Retrospective. Setting: Department of Orthopedics and Traumatology at IRCCS San Raffaele Hospital. Patients/participants: This study was performed on 37 subjects classified as Rockwood grade III-V or Tossy III ACJ dislocation. Procedure: The surgical treatment was carried out by placing a vascular graft (GorePropaten) between the tip of the coracoid process and the mid-lateral side of the clavicle with two temporary percutaneous K-wires positioned in a parallel manner in the AC ligament through the acromion and clavicle for 30 days to improve stability. Main outcome measurements: Outcomes were assessed using the Constant Shoulder (CS) score, DASH score, subjective satisfaction, and stability of AC joint at 24 months. UCLA scores taken preoperatively and at 15-month follow-up were used to perform statistical analysis significance using a control group treated conservatively. Postoperative X-rays were examined to assess joint stability after 24 months. Results: Preoperative CS score was 51 ± 12 in the acute group and 55 ± 15 in the chronic group. Follow-up mean CS score was 88 ± 11 in the acute group and 82 ± 20 in the chronic group at 24 months. Preoperative DASH score was 19 ± 3 in the acute group and 14 ± 5 in the chronic group. Mean DASH score was 3.60 ± 7 in the acute group and 6.42 ± 6 in the chronic group. Preoperative UCLA scores were 14.6 ± 2 and 15 ± 7 in the acute group and chronic group, respectively. At 15-month follow-up, UCLA scores of 28.1 ± 1.9 and 27.7 ± 1 in the acute and chronic group, respectively, showed a significant improvement ( p < 0.05) compared with UCLA scores taken preoperatively and at 15 months in the control group treated conservatively of 15.8 ± 0.9 and 16.2 ± 0.9, respectively. Results were good to excellent in 10 patients from the acute group and 23 patients from the chronic group who presented clinical stability of the AC joint without pain. Postoperative radiography showed anatomical repositioning of the ACJ and joint stability in 35 cases, but only two patients showed a partial re-dislocation at the 24-month X-ray follow-up. No infections, either deep or superficial, or nerve palsies were reported. Conclusions: Given the results obtained during the study and the response of the patients in both acute and chronic groups, the authors found that the employment of a vascular graft combined with temporary percutaneous K-wires is able to improve the patient's clinical outcome following an acromio-clavicular joint dislocation. We monitored heavy metal (As, Cd, Hg, and Pb) concentrations in surface water, sediments, and oligochaetes in four major rivers in Calabria (southern Italy) over the course of 1 year. As, Cd, and Pb showed accumulation factors of 10-10 for water to sediment and 1-10 for sediment to oligochaetes. Hg showed a water to sediment accumulation factor of 10-100. Finally, Hg concentrations exceeded the Italian quality standard for freshwater in all of the rivers, and As concentrations in sediments exceeded the respective Canadian standard. Selected polychlorinated dibenzo- p-dioxins (PCDDs) and dibenzofurans (PCDFs) were detected by gas-chromatography coupled to triple quadrupole mass spectrometry (GC-QqQ-MS/MS) in marine sediments collected in central Italy. Total PCDD/Fs concentrations ranged from 11.58 to 193.29 pg/g d.w., with PCDFs as the major contributors in most cases. The congener profiles were characterized by high OCDD (70.7 %-91.4 %). The PCDD/Fs values are within or close to the Environmental Canada sediment reference values (0.85 pg TEQ/g d.w.). GC-QqQ-MS/MS can be a suitable alternative to standard methods for PCDD/Fs determination in environmental samples. The ability of transplanted lichen Pseudevernia furfuracea to biomonitor specific airborne pollutants (heavy metals and polycyclic aromatic hydrocarbons—PAHs) was investigated at five stations with different traffic densities in Viterbo, Italy. Exposed lichen showed high levels of all analysed pollutants; greatest values were for Zn (147–252 μg/g dw), Pb (24.9–34.6 μg/g dw), fluoranthene (37–107 ng/g dw), pyrene (23–124 ng/g dw). Comparison between contaminants concentration in lichens before and after exposure showed “accumulation” or “severe accumulation” rates in more than 90% of each substance. This paper reports the results of a field study of occupational pesticide exposure (respiratory and dermal) among a group of Italian agricultural subcontractors. These workers consistently use pesticides during much of the year, thus resulting in a high exposure risk. Ten complete treatments were monitored during spring/summer. Pesticides that were applied included azinphos-methyl, dicamba, dimethoate, terbuthylazine, and alachlor. Several observations were made on worker operative modalities and the use of personal protective equipment (PPE) during work. Total potential and actual exposure ranged from 14 to 5700 µg and from 0.04 to 4600 µg, respectively. Dermal exposure contributed substantially more than inhalation to the total exposure (93.9–100%). Hand contamination ranged from 0.04 to 4600 µg and was the major contributor to dermal exposure. Penetration through specific protective garments was less than 2.4% in all cases, although penetration through general work clothing was as high as 26.8%. The risk evaluation, based on comparison between acceptable daily intake and total absorbed doses, demonstrates that it is presumable to expect possible health effects for workers regularly operating without PPE and improper tractors. Comparisons between exposure levels and operative modalities highlighted that complete PPE and properly equipped tractors contributed to a significant reduction in total exposure to pesticides during agricultural activities. In conclusion, monitored agricultural subcontractors presented very different levels of pesticide exposure due to the high variability of operative modalities and use of PPE. The aim of this study was to estimate the performance of different work clothing types for reducing skin exposure to five pesticides (azinphos-methyl, terbutylazine, alachlor, dimethoate, and dicamba) in field distribution by tractor equipped with boom sprayer. Performance was assessed by measuring the penetration factors of different types of work clothing. The results show that the protection offered by personal protective equipment (PPE) was always >97%, whereas the performance of cotton garments ranged from 84.1% to 92.5%. The different cotton garments differed significantly in their permeability, and the upper part of the body was the anatomical region showing the greatest values of the penetration factors. SARS-CoV-2 infection shows a wide-ranging clinical severity, requiring prognostic markers. We focused on S100B, a calcium-binding protein present in biological fluids, being a reliable biomarker in disorders having inflammatory processes as common basis and RAGE as main receptor. Since Covid-19 is characterized by a potent inflammatory response also involving RAGE, we tested if S100B serum levels were related to disease severity. Serum samples (n = 74) were collected from hospitalized SARS-CoV-2 positive patients admitted to Covid center. Illness severity was established by admission clinical criteria and Covid risk score. Treatment protocols followed WHO guidelines available at the time. Circulating S100B was determined by ELISA assay. Statistical analysis used Pearson's χ2 test, t-Test, and ANOVA, ANCOVA, Linear Regression. S100B was detected in serum from Covid-19 patients, significantly correlating with disease severity as shown both by the level of intensity of care (p < 0.006) as well by the value of Covid score (Multiple R-squared: 0.3751); the correlation between Covid-Score and S100B was 0.61 (p < 0.01). S100B concentration was associated with inflammation markers (Ferritin, C-Reactive Protein, Procalcitonin), and organ damage markers (Alanine Aminotransferase, Creatinine). Sediments from banks of the Sutlej River and roadside soils from vicinity of Ropar wetland (collected during pre- and post-monsoon seasons, 2013) were analysed to determine the spatiotemporal distribution of potentially toxic elements (PTEs, viz. arsenic, cadmium, cobalt, chromium, copper, iron, manganese, lead and zinc), which when present in high concentrations may pose health hazards and ecological risk. Contamination factor, degree of contamination, modified degree of contamination, metal pollution index, pollution load index, enrichment factor, geoaccumulation index and ecological risk index were also determined for these PTEs in the study area. Sediment and soil samples were found to be alkaline and non-saline (pH > 7.0; EC < 4500 μS cm−1) with sodium and potassium as major ions. Iron (mg kg−1) was found to be most abundant in sediments (1477.59–6512.45) and soils (922.64–12,455.00). Cadmium content in sediments exceeded the threshold value (0.99 mg kg−1) at 2 (pre-monsoon) and 3 (post-monsoon) sampling sites. In both seasons, cadmium (0.10–2.05) and cobalt (11.40–17.52) contents (mg kg−1) exceeded the threshold limits (0.06 and 8.00 respectively) in all roadside soils. Significant spatiotemporal variation (p ≤ 0.05) was observed for pH; EC; and calcium, magnesium, copper, iron and zinc contents. Low to moderate potential ecological risk was observed for both roadside soils (31.80–213.82) and sediments (41.47–236.73). Contamination factor, enrichment factor and geoaccumulation index for cadmium were highest in roadside soils (6.84, 46.91 and 2.19, respectively) and sediments (7.64, 167.46 and 2.35, respectively) due to settlement of coal fly ash released from the industrial setups, on sediments/soils of the study area.
