The Diabetes Textbook: Clinical Principles, Patient Management and Public Health Issues (2nd Edition) addresses diabetes from a comprehensive, multidisciplinary perspective. Diabetes has become a worldwide health problem, the global estimated prevalence approaches ten percent and the burden of this disease in terms of morbidity and mortality is unprecedented. The advances acquired through the knowledge of the mechanisms of the disease and the variety of therapeutic approaches contrast with the inability of private and public health systems in underdeveloped and even developed countries to achieve the goals of treatment. This paradox has been described in many sources: the surge of scientific advances contrast with an unprecedented amount of human suffering. Thus, a patient centered and an evidence based approach with the capacity to produce measurable clinical and economic outcomes is required. The purpose of this textbook is multiple: to offer a comprehensive resource covering all aspects of outpatient management; to address diabetes as a health problem from an epidemiological, economic and clinical perspective; to discuss the role of social determinants of health on the worldwide increase in diabetes; to highlight the challenges and obstacles in providing adequate care; and to outline a multidisciplinary approach to management in which medical visits retain their importance as part of a team comprising the patient, his or her family and a multidisciplinary group of health professionals who are able to move beyond the traditional approach of diabetes as a disease and greatly improve outcomes.

This volume celebrates the work of Petr Hájek on mathematical fuzzy logic and presents how his efforts have influenced prominent logicians who are continuing his work. The book opens with a discussion on Hájek's contribution to mathematical fuzzy logic and with a scientific biography of him, progresses to include two articles with a foundation flavour, that demonstrate some important aspects of Hájek's production, namely, a paper on the development of fuzzy sets and another paper on some fuzzy versions of set theory and arithmetic.Articles in the volume also focus on the treatment of vagueness, building connections between Hájek's favorite fuzzy logic and linguistic models of vagueness. Other articles introduce alternative notions of consequence relation, namely, the preservation of truth degrees, which is discussed in a general context, and the differential semantics. For the latter, a surprisingly strong standard completeness theorem is proved. Another contribution also looks at two principles valid in classical logic and characterize the three main t-norm logics in terms of these principles. Other articles, with an algebraic flavour, offer a summary of the applications of lattice ordered-groups to many-valued logic and to quantum logic, as well as an investigation of prelinearity in varieties of pointed lattice ordered algebras that satisfy a weak form of distributivity and have a very weak implication.The last part of the volume contains an article on possibilistic modal logics defined over MTL chains, a topic that Hájek discussed in his celebrated work, Metamathematics of Fuzzy Logic, and another one where the authors, besides offering unexpected premises such as proposing to call Hájek's basic fuzzy logic HL, instead of BL, propose a very weak system, called SL as a candidate for the role of the really basic fuzzy logic. The paper also provides a generalization of the prelinearity axiom, which was investigatedby Hájek in the context of fuzzy logic.

Logic has attained in our century a development incomparably greater than in any past age of its long history, and this has led to such an enrichment and proliferation of its aspects, that the problem of some kind of unified recom­ prehension of this discipline seems nowadays unavoidable. This splitting into several subdomains is the natural consequence of the fact that Logic has intended to adopt in our century the status of a science. This always implies that the general optics, under which a certain set of problems used to be con­ sidered, breaks into a lot of specialized sectors of inquiry, each of them being characterized by the introduction of specific viewpoints and of technical tools of its own. The first impression, that often accompanies the creation of one of such specialized branches in a diSCipline, is that one has succeeded in isolating the'scientific core'of it, by restricting the somehow vague and redundant generality of its original'philosophical'configuration. But, after a while, it appears that some of the discarded aspects are indeed important and a new specialized domain of investigation is created to explore them. By follOwing this procedure, one finally finds himself confronted with such a variety of independent fields of research, that one wonders whether the fact of labelling them under a common denomination be nothing but the contingent effect of a pure historical tradition.

This comprehensive reference work covers all aspects of growth plate fractures and their complications. Following general reviews of growth plate fractures, 21 chapters deal with each epiphyseal growth plate in the body. All of these chapters are constructed similarly for easy and quick retrieval of the required information. The main emphasis is on evaluation (diagnosis) and management (treatment). A separate section is devoted to premature partial physeal arrest, as this is by far the most common and feared complication of a growth plate fracture and its treatment is involved and controversial. The case studies included are often based on 20- to 30-year follow-ups, revealing cases that originally appeared to be quite satisfactory at the conclusion of growth but were found to have turned out quite poorly years later. This unique text is an essential and excellent resource for any pediatric orthopedic surgeon as well as all personnel in the emergency room.

Probabilistic Conditional Independence Structures provides the mathematical description of probabilistic conditional independence structures; the author uses non-graphical methods of their description, and takes an algebraic approach. The monograph presents the methods of structural imsets and supermodular functions, and deals with independence implication and equivalence of structural imsets. Motivation, mathematical foundations and areas of application are included, and a rough overview of graphical methods is also given. In particular, the author has been careful to use suitable terminology, and presents the work so that it will be understood by both statisticians, and by researchers in artificial intelligence. The necessary elementary mathematical notions are recalled in an appendix.
