In 2001, Collin and Murie[1] challenged the concept of endovascular repair (EVAR) for abdominal aortic aneurysm (AAA) in an editorial that remains controversial even now. EVAR offers minimally invasive treatment which can be done under local or regional anaesthesia, reducing postoperative pain, hospital stay and 30-day mortality compared with conventional open aortic aneurysm repair (OAR)[4]. In patients unfit for OAR (the EVAR 2 cohort), EVAR is pretty well tolerated and can minimize the risk of death from subsequent AAA rupture. Endovascular versus open repair of abdominal aortic aneurysm in 15-years' follow-up of the UK endovascular aneurysm repair trial 1 (EVAR trial 1): a randomised controlled trial.
