Background: Lung cancer still maintains the leading position among causes of death in the world; the process of early detection surely contributes to changes in the survival of patients. Standard diagnostic methods are grossly insensitive, especially in the early stages. In this paper, radiomic features are discussed that can assure improved diagnostic accuracy through automated lung cancer detection by considering the important feature categories, such as texture, shape, and intensity, originating from the CT DICOM images. Methods: We developed and compared the performance of two machine learning models—DenseNet-201 CNN and XGBoost—trained on radiomic features with the ability to identify malignant tumors from benign ones. Feature importance was analyzed using SHAP and techniques of permutation importance that enhance both the global and case-specific interpretability of the models. Results: A few features that reflect tumor heterogeneity and morphology include GLCM Entropy, shape compactness, and surface-area-to-volume ratio. These performed excellently in diagnosis, with DenseNet-201 producing an accuracy of 92.4% and XGBoost at 89.7%. The analysis of feature interpretability ascertains its potential in early detection and boosting diagnostic confidence. Conclusions: The current work identifies the most important radiomic features and quantifies their diagnostic significance through a properly conducted feature selection process reflecting stability analysis. This provides the blueprint for feature-driven model interpretability in clinical applications. Radiomics features have great value in the automated diagnosis of lung cancer, especially when combined with machine learning models. Although radiomics research has experienced rapid growth in recent years, with numerous studies dedicated to the automated extraction of diagnostic and prognostic information from various imaging modalities, such as CT, PET, and MRI, only a small fraction of these findings has successfully transitioned into clinical practice. This gap is primarily due to the significant methodological challenges involved in radiomics research, which emphasize the need for a rigorous evaluation of study quality. While many technical aspects may lie outside the expertise of most radiologists, having a foundational knowledge is essential for evaluating the quality of radiomics workflows and contributing, together with data scientists, to the development of models with a real-world clinical impact. This review is designed for the new generation of radiologists, who may not have specialized training in machine learning or radiomics, but will inevitably play a role in this evolving field. The paper has two primary objectives: first, to provide a clear, systematic guide to radiomics study pipeline, including study design, image preprocessing, feature selection, model training and validation, and performance evaluation. Furthermore, given the critical importance of evaluating the robustness of radiomics studies, this review offers a step-by-step guide to the application of the METhodological RadiomICs Score (METRICS, 2024)—a newly proposed tool for assessing the quality of radiomics studies. Background: Radiomics of salivary gland imaging can support clinical decisions in different clinical scenarios, such as tumors, radiation-induced xerostomia and sialadenitis. This review aims to evaluate the methodological quality of radiomics studies on salivary gland imaging. Material and Methods: A systematic search was performed, and the methodological quality was evaluated using the radiomics quality score (RQS). Subgroup analyses according to the first author's professional role (medical or not medical), journal type (radiological journal or other) and the year of publication (2021 or before) were performed. The correlation of RQS with the number of patients was calculated. Results: Twenty-three articles were included (mean RQS 11.34 ± 3.68). Most studies well-documented the imaging protocol (87%), while neither prospective validations nor cost-effectiveness analyses were performed. None of the included studies provided open-source data. A statistically significant difference in RQS according to the year of publication was found (p = 0.009), with papers published in 2021 having slightly higher RQSs than older ones. No differences according to journal type or the first author's professional role were demonstrated. A moderate relationship between the overall RQS and the number of patients was found. Conclusions: Radiomics application in salivary gland imaging is increasing. Although its current clinical applicability can be affected by the somewhat inadequate quality of the papers, a significant improvement in radiomics methodologies has been demonstrated in the last year. (1): Background: With the recent introduction of vesical imaging reporting and data system (VI-RADS), magnetic resonance imaging (MRI) has become the main imaging method used for the preoperative local staging of bladder cancer (BCa). However, the VI-RADS score is subject to interobserver variability and cannot provide information about tumor cellularity. These limitations may be overcome by using a quantitative approach, such as the new emerging domain of radiomics. (2) Aim: To systematically review published studies on the use of MRI-based radiomics in bladder cancer. (3) Materials and Methods: We performed literature research using the PubMed MEDLINE, Scopus, and Web of Science databases using PRISMA principles. A total of 1092 papers that addressed the use of radiomics for BC staging, grading, and treatment response were retrieved using the keywords "bladder cancer", "magnetic resonance imaging", "radiomics", and "textural analysis". (4) Results: 26 papers met the eligibility criteria and were included in the final review. The principal applications of radiomics were preoperative tumor staging (n = 13), preoperative prediction of tumor grade or molecular correlates (n = 9), and prediction of prognosis/response to neoadjuvant therapy (n = 4). Most of the developed radiomics models included second-order features mainly derived from filtered images. These models were validated in 16 studies. The average radiomics quality score was 11.7, ranging between 8.33% and 52.77%. (5) Conclusions: MRI-based radiomics holds promise as a quantitative imaging biomarker of BCa characterization and prognosis. However, there is still need for improving the standardization of image preprocessing, feature extraction, and external validation before applying radiomics models in the clinical setting. (1) Introduction: Multiparametric magnetic resonance imaging (mpMRI) is the main imagistic tool employed to assess patients suspected of harboring prostate cancer (PCa), setting the indication for targeted prostate biopsy. However, both mpMRI and targeted prostate biopsy are operator dependent. The past decade has been marked by the emerging domain of radiomics and artificial intelligence (AI), with extended application in medical diagnosis and treatment processes. (2) Aim: To present the current state of the art regarding decision support tools based on texture analysis and AI for the prediction of aggressiveness and biopsy assistance. (3) Materials and Methods: We performed literature research using PubMed MeSH, Scopus and WoS (Web of Science) databases and screened the retrieved papers using PRISMA principles. Articles that addressed PCa diagnosis and staging assisted by texture analysis and AI algorithms were included. (4) Results: 359 papers were retrieved using the keywords "prostate cancer", "MRI", "radiomics", "textural analysis", "artificial intelligence", "computer assisted diagnosis", out of which 35 were included in the final review. In total, 24 articles were presenting PCa diagnosis and prediction of aggressiveness, 7 addressed extracapsular extension assessment and 4 tackled computer-assisted targeted prostate biopsies. Simple Summary: Ovarian cancer is the second most frequent gynecological cancer in Western countries and the most common cause of death due to gynecological malignancies with an estimated five-year survival rate of 39%. The high aggressiveness and mortality are mainly related to the speed of abdominal spread: 70% of patients are diagnosed at an advanced stage of disease (stage III–IV FIGO) or in the presence of peritoneal carcinomatosis (PC), and about 60% of women will develop a recurrence. In this context, imaging plays an essential role for proper staging and follow-up and in selecting patients eligible for complete cytoreduction (CCR), the most important treatment and prognostic factor for patients. PC represents the most striking picture of the loco-regional spread of ovarian cancer, configuring stage III. In the last few years, many papers have evaluated the role of imaging and therapeutic management in patients with ovarian cancer and PC. This paper summed up the literature on traditional approaches to the imaging of peritoneal carcinomatosis in advanced ovarian cancer, presenting classification systems, most frequent patterns, routes of spread and sites that are difficult to identify. The role of imaging in diagnosis was investigated, with particular attention to the reported sensitivity and specificity data—computed tomography (CT), magnetic resonance imaging (MRI), positron emission tomography-CT (PET-CT)—and to the peritoneal cancer index (PCI). In addition, we explored the therapeutic possibilities and radiomics applications that can impact management of patients with ovarian cancer. Featured Application: This review reports the current evidence available regarding the role of radiomic features extracted from conventional imaging (MRI, CT and 18F-FDG PET scans) in assessing the status of Human Papillomavirus (HPV) in Oropharyngeal Squamous Cell Carcinoma (OPSCC) patients. The paper evaluates the possible future adoption of radiomic models in determining viral status in a non-invasive manner, especially for those not tolerating biopsy, when biopsy is not feasible or in case of discrepancies between immunohistochemistry and Polymerase Chain Reaction. This could also lead to more tailored treatment strategies.Background: Oropharyngeal Squamous Cell Carcinoma (OPSCC) is rapidly increasing due to the spread of Human Papillomavirus (HPV) infection. HPV-positive disease has unique characteristics, with better response to treatment and consequent better prognosis. HPV status is routinely assessed via p16 immunohistochemistry or HPV DNA Polymerase Chain Reaction. Radiomics is a quantitative approach to medical imaging which can overcome limitations due to its subjective interpretation and correlation with clinical data. The aim of this narrative review is to evaluate the impact of radiomic features on assessing HPV status in OPSCC patients. Methods: A narrative review was performed by synthesizing literature results from PUBMED. In the search strategy, Medical Subject Headings (MeSH) terms were used. Retrospective mono- or multicentric works assessing the correlation between radiomic features and HPV status prediction in OPSCC were included. Selected papers were in English and included studies on humans. The range of publication date was July 2015–April 2023. Results: Our research returned 23 published papers; the accuracy of radiomic models was evaluated by ROC curves and AUC values. MRI- and CT-based radiomic models proved of comparable efficacy. Also, metabolic imaging showed crucial importance in the determination of HPV status, albeit with lower AUC values. Conclusions: Radiomic features from conventional imaging can play a complementary role in the assessment of HPV status in OPSCC. Dual-energy CT (DECT) imaging has broadened the potential of CT imaging by offering multiple postprocessing datasets with a single acquisition at more than one energy level. DECT shows profound capabilities to improve diagnosis based on its superior material differentiation and its quantitative value. However, the potential of dual-energy imaging remains relatively untapped, possibly due to its intricate workflow and the intrinsic technical limitations of DECT. Knowing the clinical advantages of dual-energy imaging and recognizing its limitations and pitfalls is necessary for an appropriate clinical use. The aims of this paper are to review the physical and technical bases of DECT acquisition and analysis, to discuss the advantages and limitations of DECT in different clinical scenarios, to review the technical constraints in material labeling and quantification, and to evaluate the cutting-edge applications of DECT imaging, including artificial intelligence, qualitative and quantitative imaging biomarkers, and DECT-derived radiomics and radiogenomics. Our review explores AI's potential in this field, analyzing 89 recent studies from 2020 through 2023. Specifically, these studies included AI applications for the analysis of multi-omics data: radiomics, pathomics, clinical records, and lab data. Notably, eight studies combined diverse omics data types (genomics, transcriptomics, epigenomics, and proteomics). Integration of AI for the analysis of clinical and omics data contributes to a significant advancement and is essential for safe clinical implementation. Cancer is one of the leading causes of death, making timely diagnosis and prognosis very important. Utilization of AI (artificial intelligence) enables providers to organize and process patient data in a way that can lead to better overall outcomes. This review paper aims to look at the varying uses of AI for diagnosis and prognosis and clinical utility. PubMed and EBSCO databases were utilized for finding publications from 1 January 2020 to 22 December 2023. Articles were collected using key search terms such as "artificial intelligence" and "machine learning." Included in the collection were studies of the application of AI in determining cancer diagnosis and prognosis using multi-omics data, radiomics, pathomics, and clinical and laboratory data. The resulting 89 studies were categorized into eight sections based on the type of data utilized and then further subdivided into two subsections focusing on cancer diagnosis and prognosis, respectively. Eight studies integrated more than one form of omics, namely genomics, transcriptomics, epigenomics, and proteomics. Incorporating AI into cancer diagnosis and prognosis alongside omics and clinical data represents a significant advancement. Simple Summary: Among head–neck tumors, squamous cell carcinoma is the most frequent histotype and includes a range of malignancies with different sites of origin as well as different therapeutic strategies and clinical outcomes. In daily practice, patients with head–neck squamous cell carcinoma are seen in various clinical settings, requiring a multidisciplinary approach to therapeutic decisions and clinical care. 18F-FDG PET/CT plays a well-defined role in the management of these tumors for pre-treatment staging and radiotherapy planning as well as treatment-response assessment and post-therapy follow-up. This paper is an overview of the standard use of 18F-FDG PET/CT in the various clinical scenarios of head–neck squamous cell carcinoma. Also, emerging applications will be reviewed, including the use of radiopharmaceuticals other than 18F-FDG, PET/MRI implementation in clinical practice, and the use of radiomics and machine learning. This article provides an overview of the use of 18F-FDG PET/CT in various clinical scenarios of head–neck squamous cell carcinoma, ranging from initial staging to treatment-response assessment, and post-therapy follow-up, with a focus on the current evidence, debated issues, and innovative applications. Methodological aspects and the most frequent pitfalls in head–neck imaging interpretation are described. In the initial work-up, 18F-FDG PET/CT is recommended in patients with metastatic cervical lymphadenectomy and occult primary tumor; moreover, it is a well-established imaging tool for detecting cervical nodal involvement, distant metastases, and synchronous primary tumors. Various 18F-FDG pre-treatment parameters show prognostic value in terms of disease progression and overall survival. In this scenario, an emerging role is played by radiomics and machine learning. For radiation-treatment planning, 18F-FDG PET/CT provides an accurate delineation of target volumes and treatment adaptation. Due to its high negative predictive value, 18F-FDG PET/CT, performed at least 12 weeks after the completion of chemoradiotherapy, can prevent unnecessary neck dissections. In addition to radiomics and machine learning, emerging applications include PET/MRI, which combines the high soft-tissue contrast of MRI with the metabolic information of PET, and the use of PET radiopharmaceuticals other than 18F-FDG, which can answer specific clinical needs. Simple Summary: The 2021 WHO classification of central nervous system (CNS) tumors is challenging for neuroradiologists due to the central role of the molecular profile of tumors. We performed a scoping review of recent literature to assess the existing data on the power of novel data analysis tools to predict new tumor classes by imaging. We found room for performance improvement for subgroups with lower incidence (e.g., 1p/19q codeleted or IDH1/2 mutated gliomas) and patients with rare diagnoses (e.g., pediatric gliomas, midline gliomas). More data regarding functional MRI techniques need to be collected. Studies explicitly designed to assess the generalizability of AI-aided tools for predicting molecular tumor subgroups are lacking. The 2021 WHO classification of CNS tumors is a challenge for neuroradiologists due to the central role of the molecular profile of tumors. The potential of novel data analysis tools in neuroimaging must be harnessed to maintain its role in predicting tumor subgroups. We performed a scoping review to determine current evidence and research gaps. A comprehensive literature search was conducted regarding glioma subgroups according to the 2021 WHO classification and the use of MRI, radiomics, machine learning, and deep learning algorithms. Sixty-two original articles were included and analyzed by extracting data on the study design and results. Only 8% of the studies included pediatric patients. Low-grade gliomas and diffuse midline gliomas were represented in one-third of the research papers. Public datasets were utilized in 22% of the studies. Conventional imaging sequences prevailed; data on functional MRI (DWI, PWI, CEST, etc.) are underrepresented. Multiparametric MRI yielded the best prediction results. IDH mutation and 1p/19q codeletion status prediction remain in focus with limited data on other molecular subgroups. Reported AUC values range from 0.6 to 0.98. Studies designed to assess generalizability are scarce. Performance is worse for smaller subgroups (e.g., 1p/19q codeleted or IDH1/2 mutated gliomas). For decades, wavelet theory has attracted interest in several fields in dealing with signals. Nowadays, it is acknowledged that it is not very suitable to face aspects of multidimensional data like singularities and this has led to the development of other mathematical tools. A recent application of wavelet theory is in radiomics, an emerging field aiming to improve diagnostic, prognostic and predictive analysis of various cancer types through the analysis of features extracted from medical images. In this paper, for a radiomics study of prostate cancer with magnetic resonance (MR) images, we apply a similar but more sophisticated tool, namely the shearlet transform which, in contrast to the wavelet transform, allows us to examine variations along more orientations. In particular, we conduct a parallel radiomics analysis based on the two different transformations and highlight a better performance (evaluated in terms of statistical measures) in the use of the shearlet transform (in absolute value). Medical imaging plays an indispensable role in evaluating, predicting, and monitoring a range of medical conditions. Radiomics, a specialized branch of medical imaging, utilizes quantitative features extracted from medical images to describe underlying pathologies, genetic information, and prognostic indicators. The integration of radiomics with artificial intelligence presents innovative avenues for cancer diagnosis, prognosis evaluation, and therapeutic choices. In the context of oncology, radiomics offers significant potential. Feature selection emerges as a pivotal step, enhancing the clinical utility and precision of radiomics. It achieves this by purging superfluous and unrelated features, thereby augmenting model performance and generalizability. The goal of this review is to assess the fundamental radiomics process and the progress of feature selection methods, explore their applications and challenges in cancer research, and provide theoretical and methodological support for future investigations. Through an extensive literature survey, articles pertinent to radiomics and feature selection were garnered, synthesized, and appraised. The paper provides detailed descriptions of how radiomics is applied and challenged in different cancer types and their various stages. The review also offers comparative insights into various feature selection strategies, including filtering, packing, and embedding methodologies. The spleen, often referred to as the "forgotten organ", plays numerous important roles in various diseases. Recently, there has been an increased interest in the application of radiomics in different areas of medical imaging. This systematic review aims to assess the current state of the art and evaluate the methodological quality of radiomics applications in spleen imaging. A systematic search was conducted on PubMed, Scopus, and Web of Science. All the studies were analyzed, and several characteristics, such as year of publication, research objectives, and number of patients, were collected. The methodological quality was evaluated using the radiomics quality score (RQS). Fourteen articles were ultimately included in this review. The majority of these articles were published in non-radiological journals (78%), utilized computed tomography (CT) for extracting radiomic features (71%), and involved not only the spleen but also other organs for feature extraction (71%). Overall, the included papers achieved an average RQS total score of 9.71 ± 6.37, corresponding to an RQS percentage of 27.77 ± 16.04. In conclusion, radiomics applications in spleen imaging demonstrate promising results in various clinical scenarios. The use of radiomics and artificial intelligence applied for the diagnosis and monitoring of Alzheimer's disease has developed in recent years. However, this approach is not yet completely applicable in clinical practice. The aim of this paper is to provide a systematic analysis of the studies that have included the use of radiomics from different imaging techniques and artificial intelligence for the diagnosis and monitoring of Alzheimer's disease in order to improve the clinical outcomes and quality of life of older patients. A systematic review of the literature was conducted in February 2023, analyzing manuscripts and articles of the last 5 years from the PubMed, Scopus and Embase databases. All studies concerning discrimination among Alzheimer's disease, Mild Cognitive Impairment and healthy older people performing radiomics analysis through machine and deep learning were included. A total of 15 papers were included. The results showed a very good performance of this approach in the differentiating Alzheimer's disease patients—both at the dementia and pre-dementia phases of the disease—from healthy older people. In summary, radiomics and AI can be valuable tools for diagnosing and monitoring the progression of Alzheimer's disease, potentially leading to earlier and more accurate diagnosis and treatment. Simple Summary: In 2016, the World Health Organization (WHO) recommended the incorporation of molecular parameters, in addition to histology, for an optimal definition of the central nervous system (CNS) tumors. Gliomas, being among the most common types of CNS tumors, have distinct clinical outcomes and treatment strategies based on different tumor grades, isocitrate dehydrogenase (IDH) mutation, and 1p/19q codeletion statuses. This paper uses radiomics models to noninvasively predict the glioma subtype with clinical Magnetic Resonance Imaging (MRI) images. Different settings in the radiomics pipeline were investigated to achieve optimal performance, together with a better understanding of the exact role of each setting in the model performance. The characteristics of the radiomic features that best distinguish the glioma subtypes were also analyzed. This paper not only provides a radiomics pipeline which works well for predicting the glioma subtype, but it also contributes to the radiomics model development and interpretability. Gliomas are among the most common types of central nervous system (CNS) tumors. A prompt diagnosis of the glioma subtype is crucial to estimate the prognosis and personalize the treatment strategy. The objective of this study was to develop a radiomics pipeline based on the clinical Magnetic Resonance Imaging (MRI) scans to noninvasively predict the glioma subtype, as defined based on the tumor grade, isocitrate dehydrogenase (IDH) mutation status, and 1p/19q codeletion status. A total of 212 patients from the public retrospective The Cancer Genome Atlas Low Grade Glioma (TCGA-LGG) and The Cancer Genome Atlas Glioblastoma Multiforme (TCGA-GBM) datasets were used for the experiments and analyses. Different settings in the radiomics pipeline were investigated to improve the classification, including the Z-score normalization, the feature extraction strategy, the image filter applied to the MRI images, the introduction of clinical information, ComBat harmonization, the classifier chain strategy, etc. Based on numerous experiments, we finally reached an optimal pipeline for classifying the glioma tumors. We then tested this final radiomics pipeline on the hold-out test data with 51 randomly sampled random seeds for reliable and robust conclusions. The results showed that, after tuning the radiomics pipeline, the mean AUC improved from 0.8935 (±0.0351) to 0.9319 (±0.0386), from 0.8676 (±0.0421) to 0.9283 (±0.0333), and from 0.6473 (±0.1074) to 0.8196 (±0.0702) in the test data for predicting the tumor grade, IDH mutation, and 1p/19q codeletion status, respectively. The mean accuracy for predicting the five glioma subtypes also improved from 0.5772 (±0.0816) to 0.6716 (±0.0655). Pediatric sarcomas, rare malignancies of mesenchymal origin, pose diagnostic and therapeutic challenges. In this review, we explore the role of radiomics in reshaping our understanding of pediatric sarcomas, emphasizing methodological considerations and applications such as diagnostics and predictive modeling. A systematic review conducted up to November 2023 identified 72 papers on radiomics analysis in pediatric sarcoma from PubMed/MEDLINE, Web of Knowledge, and Scopus. Following inclusion and exclusion criteria, 10 reports were included in this review. Background: This paper offers an assessment of radiomics tools in the evaluation of intrahepatic cholangiocarcinoma. Methods: The PubMed database was searched for papers published in the English language no earlier than October 2022. Results: We found 236 studies, and 37 satisfied our research criteria. Several studies addressed multidisciplinary topics, especially diagnosis, prognosis, response to therapy, and prediction of staging (TNM) or pathomorphological patterns. In this review, we have covered diagnostic tools developed through machine learning, deep learning, and neural network for the recurrence and prediction of biological characteristics. Molecular characterization, categorization, prognosis and therapy recommendation are given special consideration. In a narrative manner, we outline the fundamental technological principles, the overall idea and usual workflow of radiomic analysis and what seem to be the present and potential challenges in normal clinical practice. Microvascular Doppler (MicroV) is a new-generation Doppler technique developed by Esaote (Esaote s.p.a., Genova, Italy), which is able to visualize small and low-flow vessels through a suppression of interfering signals. MicroV uses advanced filters that are able to differentiate tissue artifacts from low-speed blood flows; by exploiting the space–time coherence information, these filters can selectively suppress tissue components, preserving the signal coming from the microvascular flow. The aim of this paper is to develop a reproducible protocol for the recording and collection of MicroV intraoperative ultrasound images by the use of a capable intraoperative ultrasound machine and post-processing aimed at evaluation of brain-tumor microvascularization through the analysis of radiomic features. The proposed protocol has been internally validated on eight patients and will be firstly applied to patients affected by WHO grade IV astrocytoma (glioblastoma—GBM) candidates for craniotomy and lesion removal. In a further stage, it will be generally applied to patients with primary or metastatic brain tumors. IOUS is performed before durotomy. Tumor microvascularization is evaluated using the MicroV Doppler technique and IOUS images are recorded, stored, and post-processed. Three independent authors (GF, IM and ID) performed an independent review focusing on these issues. Key references were derived from a PubMed query. Hand searching and clinicaltrials.gov were also used, and grey literature was searched for further papers of interest. The final choice of papers included was discussed between all co-authors. Background: Pancreatic ductal adenocarcinoma (PDAC) is the seventh leading cause of cancer-related deaths worldwide. Surgical resection is the main driver to improving survival in resectable tumors, while neoadjuvant treatment based on chemotherapy (and radiotherapy) is the best option-treatment for a non-primally resectable disease. CT-based imaging has a central role in detecting, staging, and managing PDAC. As several authors have proposed radiomics for risk stratification in patients undergoing surgery for PADC, in this narrative review, we have explored the actual fields of interest of radiomics tools in PDAC built on pre-surgical imaging and clinical variables, to obtain more objective and reliable predictors. Methods: The PubMed database was searched for papers published in the English language no earlier than January 2018. Results: We found 301 studies, and 11 satisfied our research criteria. Of those included, four were on resectability status prediction, three on preoperative pancreatic fistula (POPF) prediction, and four on survival prediction. Most of the studies were retrospective. Neoadjuvant chemotherapy plus radical surgery could be a safe alternative to chemo-radiation in cervical cancer patients who are not willing to receive radiotherapy. The response to neoadjuvant chemotherapy is the main factor influencing the need for adjunctive treatments and survival. In the present paper we aim to develop a machine learning model based on cervix magnetic resonance imaging (MRI) images to stratify the single-subject risk of cervical cancer. We collected MRI images from 72 subjects. Among these subjects, 28 patients (38.9%) belonged to the "Not completely responding" class and 44 patients (61.1%) belonged to the 'Completely responding' class according to their response to treatment. This image set was used for the training and cross-validation of different machine learning models. A robust radiomic approach was applied, under the hypothesis that the radiomic features could be able to capture the disease heterogeneity among the two groups. Three models consisting of three ensembles of machine learning classifiers (random forests, support vector machines, and k-nearest neighbor classifiers) were developed for the binary classification task of interest ("Not completely responding" vs. "Completely responding"), based on supervised learning, using response to treatment as the reference standard. The best model showed an ROC-AUC (%) of 83 (majority vote), 82.3 (mean) [79.9–84.6], an accuracy (%) of 74, 74.1 [72.1–76.1], a sensitivity (%) of 71, 73.8 [68.7–78.9], and a specificity (%) of 75, 74.2 [71–77.5]. Artificial intelligence (AI) methods applied to healthcare problems have shown enormous potential to alleviate the burden of health services worldwide and to improve the accuracy and reproducibility of predictions. In particular, developments in computer vision are creating a paradigm shift in the analysis of radiological images, where AI tools are already capable of automatically detecting and precisely delineating tumours. However, such tools are generally developed in technical departments that continue to be siloed from where the real benefit would be achieved with their usage. Significant effort still needs to be made to make these advancements available, first in academic clinical research and ultimately in the clinical setting. The efforts toward standardization are examined, encompassing the establishment of reporting templates, use of common imaging lexicons, and integration of clinical decision support tools. Organizations such as the ACR, ESUR, RSNA, and ESR have developed standardized reporting systems, templates, and platforms to promote uniformity and collaboration. With the emergence of sentinel node technology, many patients can be staged histopathologically using lymphatic mapping and selective lymphadenectomy. Structural imaging by using US, CT and MR permits precise measurement of lymph node volume, which is strongly associated with neoplastic involvement. Sentinel lymph node detection has been an ideal field of application for nuclear medicine because anatomical data fails to represent the close connections between the lymphatic system and regional lymph nodes, or, more specifically, to identify the first draining lymph node. Artificial intelligence (AI) is rapidly integrating into diagnostic methods across many branches of medicine. Significant progress has been made in tumor assessment using AI algorithms, and research is underway on how image manipulation can provide information with diagnostic, prognostic and treatment impacts. Glioblastoma (GB) remains the most common primary malignant brain tumor, with a median survival of 15 months. This paper presents literature data on GB imaging and the contribution of AI to the characterization and tracking of GB, as well as recurrence. Furthermore, from an imaging point of view, the differential diagnosis of these tumors can be problematic. How can an AI algorithm help with differential diagnosis? The integration of clinical, radiomics and molecular markers via AI holds great potential as a tool for enhancing patient outcomes by distinguishing brain tumors from mimicking lesions, classifying and grading tumors, and evaluating them before and after treatment. Featured Application: The application of this work is in radiomics for medical imaging analysis. It addresses the question of how to establish if radiomic features are stable and reproducible. Radiomics involves the extraction of information from medical images that are not visible to the human eye. There is evidence that these features can be used for treatment stratification and outcome prediction. However, there is much discussion about the reproducibility of results between different studies. This paper studies the reproducibility of CT texture features used in radiomics, comparing two feature extraction implementations, namely the MATLAB toolkit and Pyradiomics, when applied to independent datasets of CT scans of patients: (i) the open access RIDER dataset containing a set of repeat CT scans taken 15 min apart for 31 patients (RIDER Scan 1 and Scan 2, respectively) treated for lung cancer; and (ii) the open access HN1 dataset containing 137 patients treated for head and neck cancer. Gross tumor volume (GTV), manually outlined by an experienced observer available on both datasets, was used. The 43 common radiomics features available in MATLAB and Pyradiomics were calculated using two intensity-level quantization methods with and without an intensity threshold. Cases were ranked for each feature for all combinations of quantization parameters, and the Spearman's rank coefficient, rs, calculated. Reproducibility was defined when a highly correlated feature in the RIDER dataset also correlated highly in the HN1 dataset, and vice versa. A total of 29 out of the 43 reported stable features were found to be highly reproducible between MATLAB and Pyradiomics implementations, having a consistently high correlation in rank ordering for RIDER Scan 1 and RIDER Scan 2 (rs > 0.8). 18/43 reported features were common in the RIDER and HN1 datasets, suggesting they may be agnostic to disease site. Useful radiomics features should be selected based on reproducibility. Hepatocellular carcinoma is the most common primary malignant hepatic tumor and occurs most often in the setting of chronic liver disease. Liver transplantation is a curative treatment option and is an ideal solution because it solves the chronic underlying liver disorder while removing the malignant lesion. However, due to organ shortages, this treatment can only be applied to carefully selected patients according to clinical guidelines. Artificial intelligence is an emerging technology with multiple applications in medicine with a predilection for domains that work with medical imaging, like radiology. With the help of these technologies, laborious tasks can be automated, and new lesion imaging criteria can be developed based on pixel-level analysis. Our objectives are to review the developing AI applications that could be implemented to better stratify liver transplant candidates. The papers analysed applied AI for liver segmentation, evaluation of steatosis, sarcopenia assessment, lesion detection, segmentation, and characterization. A liver transplant is an optimal treatment for patients with hepatocellular carcinoma in the setting of chronic liver disease. This paper focused on the problem of diagnosis of Alzheimer's disease via the combination of deep learning and radiomics methods. We proposed a classification model for Alzheimer's disease diagnosis based on improved convolution neural network models and image fusion method and compared it with existing network models. We collected 182 patients in the ADNI and PPMI database to classify Alzheimer's disease, and reached 0.906 AUC in training with single modality images, and 0.941 AUC in training with fusion images. This proved the proposed method has better performance in the fusion images. The research may promote the application of multimodal images in the diagnosis of Alzheimer's disease. Fusion images dataset based on multi-modality images has higher diagnosis accuracy than single modality images dataset. This paper investigates the effect of anisotropic resolution on the image textural features of pharmacokinetic (PK) parameters of a murine glioma model using dynamic contrast-enhanced (DCE) MR images acquired with an isotropic resolution at 7T with pre-contrast T1 mapping. The PK parameter maps of whole tumors at isotropic resolution were generated using the two-compartment exchange model combined with the three-site-two-exchange model. The textural features of these isotropic images were compared with those of simulated, thick-slice, anisotropic images to assess the influence of anisotropic voxel resolution on the textural features of tumors. The isotropic images and parameter maps captured distributions of high pixel intensity that were absent in the corresponding anisotropic images with thick slices. A significant difference was observed in 33% of the histogram and textural features extracted from anisotropic images and parameter maps, compared to those extracted from corresponding isotropic images. Anisotropic images in different orthogonal orientations demonstrated 42.1% of the histogram and textural features to be significantly different from those of isotropic images. This study demonstrates that the anisotropy of voxel resolution needs to be carefully considered when comparing the textual features of tumor PK parameters and contrast-enhanced images. Low-dose computed tomography-based screening, together with smoking cessation, is the only tool to fight lung cancer, as it has already been proven in the United States of America but also European randomized controlled trials. Screening requires a lot of well-organized specialized work, but it can be supported by artificial intelligence (AI). Simple Summary: The prognostic expectancies of patients affected by glioblastoma have remained almost unchanged during the last thirty years. Along with specific oncological research and surgical technical alternatives, corollary disciplines are requested to provide their contributions to improve patient management and outcomes. Technological improvements in radiology have led to the development of radiomics, a new discipline able to detect tumoral phenotypical features through the extraction and analysis of a large amount of data. Intuitively, the early foreseeing of glioma features may constitute a tremendous contribution to the management of patients. Background: The fifth edition of the WHO Classification of Tumors of the Central Nervous System (CNS), published in 2021, marks a step forward the future diagnostic approach to these neoplasms. Alongside this, radiomics has experienced rapid evolution over the last several years, allowing us to correlate tumor imaging heterogeneity with a wide range of tumor molecular and subcellular features. Radiomics is a translational field focused on decoding conventional imaging data to extrapolate the molecular and prognostic features of tumors such as gliomas. We herein analyze the state-of-the-art of radiomics applied to glioblastoma, with the goal to estimate its current clinical impact and potential perspectives in relation to well-rounded patient management, including the end-of-life stage. Methods: A literature review was performed on the PubMed, MEDLINE and Scopus databases using the following search items: "radiomics and glioma", "radiomics and glioblastoma", "radiomics and glioma and IDH", "radiomics and glioma and TERT promoter", "radiomics and glioma and EGFR", "radiomics and glioma and chromosome". Results: A total of 719 articles were screened. Further quantitative and qualitative analysis allowed us to finally include 11 papers. This analysis shows that radiomics is rapidly evolving towards a reliable tool. Conclusions: Further studies are necessary to adjust radiomics' potential to the newest molecular requirements pointed out by the 2021 WHO classification of CNS tumors. At a glance, its application in the clinical routine could be beneficial to achieve a timely diagnosis, especially for those patients not eligible for surgery and/or adjuvant therapies but still deserving palliative and supportive care. To completely comprehend neurodevelopment in healthy and congenitally abnormal fetuses, quantitative analysis of the human fetal brain is essential. This analysis requires the use of automatic multi-tissue fetal brain segmentation techniques. This paper proposes an end-to-end automatic yet effective method for a multi-tissue fetal brain segmentation model called IRMMNET. It includes a inception residual encoder block (EB) and a dense spatial attention (DSAM) block, which facilitate the extraction of multi-scale fetal-brain-tissue-relevant information from multi-view MRI images, enhance the feature reuse, and substantially reduce the number of parameters of the segmentation model. Additionally, we propose three methods for predicting gestational age (GA)—GA prediction by using a 3D autoencoder, GA prediction using radiomics features, and GA prediction using the IRMMNET segmentation model's encoder. Our experiments were performed on a dataset of 80 pathological and non-pathological magnetic resonance fetal brain volume reconstructions across a range of gestational ages (20 to 33 weeks) that were manually segmented into seven different tissue categories. The results showed that the proposed fetal brain segmentation model achieved a Dice score of 0.791 ± 0.18 , outperforming the state-of-the-art methods. The radiomics-based GA prediction methods achieved the best results (RMSE: 1.42 ). The Gleason score was originally formulated to represent the heterogeneity of prostate cancer and helps to stratify the risk of patients affected by this tumor. The Gleason score assigning represents an on H&E stain task performed by pathologists upon histopathological examination of needle biopsies or surgical specimens. In this paper, we propose an approach focused on the automatic Gleason score classification. We exploit a set of 18 radiomic features. The radiomic feature set is directly obtainable from segmented magnetic resonance images. We build several models considering supervised machine learning techniques, obtaining with the RandomForest classification algorithm a precision ranging from 0.803 to 0.888 and a recall from to 0.873 to 0.899. Preoperative, non-invasive, and accurate identification of the pathological subtypes of pulmonary ground glass nodules (GGNs) play an important role in the precise selection of clinical surgical operations and individualized treatment plans. Efforts have been made for the classification of pathological subtypes of GGNs, but most existing methods focus on benign or malignant diagnosis of GGNs by means of a one-time computed tomography image (CTI), which fails to capture the nodule development based on follow-up CTI. In this paper, a novel method for subtype classification based on follow-up CTIs is presented as a viable option to the existing one-time CTI-based approach. A total of 383 follow-up CTIs with GGNs from 146 patients was collected and retrospectively labeled via posterior surgical pathology. Feature extraction is performed individually to the follow-up CTIs. The extracted feature differences were represented as a vector, which was then used to construct a set of vectors for all the patients. Finally, a subspace K-nearest neighbor classifier was built to predict the pathological subtypes of GGNs. Experimental validation confirmed the efficacy of the new method over the existing method. Results showed that the accuracy of the new method could reach 72.5%, while the existing methods had an upper bound of 67.5% accuracy. Subsequent three-category comparison experiments were also performed to demonstrate that the new method could increase the accuracy up to 21.33% compared to the existing methods that use one-time CTI. Simple Summary: The extraction of quantitative data from standard-of-care imaging modalities offers opportunities to improve the relevance and salience of imaging biomarkers used in drug development. This review aims to identify the challenges and opportunities for discovering new imaging-based biomarkers based on radiomic and volumetric assessment in the single-site solid tumor sites: breast cancer, rectal cancer, lung cancer and glioblastoma. Developing approaches to harmonize three essential areas: segmentation, validation and data sharing may expedite regulatory approval and adoption of novel cancer imaging biomarkers. Clinical trials for oncology drug development have long relied on surrogate outcome biomarkers that assess changes in tumor burden to accelerate drug registration (i.e., Response Evaluation Criteria in Solid Tumors version 1.1 (RECIST v1.1) criteria). Drug-induced reduction in tumor size represents an imperfect surrogate marker for drug activity and yet a radiologically determined objective response rate is a widely used endpoint for Phase 2 trials. With the addition of therapies targeting complex biological systems such as immune system and DNA damage repair pathways, incorporation of integrative response and outcome biomarkers may add more predictive value. We performed a review of the relevant literature in four representative tumor types (breast cancer, rectal cancer, lung cancer and glioblastoma) to assess the preparedness of volumetric and radiomics metrics as clinical trial endpoints. We identified three key areas—segmentation, validation and data sharing strategies—where concerted efforts are required to enable progress of volumetric- and radiomics-based clinical trial endpoints for wider clinical implementation. Chronic obstructive pulmonary disease (COPD) is a preventable, treatable, progressive chronic disease characterized by persistent airflow limitation. Patients with COPD deserve special consideration regarding treatment in this fragile population for preclinical health management. Therefore, this paper proposes a novel lung radiomics combination vector generated by a generalized linear model (GLM) and Lasso algorithm for COPD stage classification based on an auto-metric graph neural network (AMGNN) with a meta-learning strategy. Firstly, the parenchyma images were segmented from chest high-resolution computed tomography (HRCT) images by ResU-Net. Second, lung radiomics features are extracted from the parenchyma images by PyRadiomics. Third, a novel lung radiomics combination vector (3 + 106) is constructed by the GLM and Lasso algorithm for determining the radiomics risk factors (K = 3) and radiomics node features (d = 106). Last, the COPD stage is classified based on the AMGNN. The results show that compared with the convolutional neural networks and machine learning models, the AMGNN based on constructed novel lung radiomics combination vector performs best, achieving an accuracy of 0.943, precision of 0.946, recall of 0.943, F1-score of 0.943, and ACU of 0.984. A portion of gastric cancer patients with negative lymph node metastasis at an early stage eventually die from tumor recurrence or advanced metastasis. Occult lymph node metastasis (OLNM] is a potential risk factor for the recurrence and metastasis in these patients, and it is highly important for clinical prognosis. Positron emission tomography (PET)/computed tomography (CT) is used to assess lymph node metastasis in gastric cancer due to its advantages in anatomical and functional imaging and non-invasive nature. Among the major metabolic parameters of PET, the maximum standardized uptake value (SUVmax) is commonly used for examining lymph node status. However, SUVmax is susceptible to interference by a variety of factors. In recent years, the exploration of new PET metabolic parameters, new PET imaging agents and radiomics, has become an active research topic. This paper aims to explore the feasibility and predict the effectiveness of using PET/CT to detect OLNM. The current landscape and future trends of primary metabolic parameters and new imaging agents of PET are reviewed. For gastric cancer patients, the possibility to detect OLNM non-invasively will help guide surgeons to choose the appropriate lymph node dissection area, thereby reducing unnecessary dissections and providing more reasonable, personalized and comprehensive treatments. Lung cancer is a deadly disease with a high mortality rate. Endobronchial ultrasonography (EBUS) is one of the methods for detecting pulmonary lesions. Computer-aided diagnosis of pulmonary lesions from images can help radiologists to classify lesions; however, most of the existing methods need a large volume of data to give good results. Thus, this paper proposes a novel pulmonary lesion classification framework for EBUS images that works well with small datasets. The proposed framework integrates the statistical results from three classification models using the weighted ensemble classification. The three classification models include the radiomics feature and patient data-based model, the single-image-based model, and the multi-patch-based model. The radiomics features are combined with the patient data to be used as input data for the random forest, whereas the EBUS images are used as input data to the other two CNN models. The performance of the proposed framework was evaluated on a set of 200 EBUS images consisting of 124 malignant lesions and 76 benign lesions. The experimental results show that the accuracy, sensitivity, specificity, positive predictive value, negative predictive value, and area under the curve are 95.00%, 100%, 86.67%, 92.59%, 100%, and 93.33%, respectively. Glioma is the most common primary malignant tumor of the adult central nervous system (CNS), which mostly shows invasive growth. In most cases, surgery is often difficult to completely remove, and the recurrence rate and mortality of patients are high. With the continuous development of molecular genetics and the great progress of molecular biology technology, more and more molecular biomarkers have been proved to have important guiding significance in the individualized diagnosis, treatment, and prognosis evaluation of glioma. With the updates of the World Health Organization (WHO) classification of tumors of the CNS in 2021, the diagnosis and treatment of glioma has entered the era of precision medicine in the true sense. Due to its ability to non-invasively achieve accurate identification of glioma from other intracranial tumors, and to predict the grade, genotyping, treatment response, and prognosis of glioma, which provides a scientific basis for the clinical application of individualized diagnosis and treatment model of glioma, radiomics has become a research hotspot in the field of precision medicine. Because of the intrinsic anatomic complexity of the brain structures, brain tumors have a high mortality and disability rate, and an early diagnosis is mandatory to contain damages. The commonly used biopsy is the diagnostic gold standard method, but it is invasive and, due to intratumoral heterogeneity, biopsies may lead to an incorrect result. Moreover, some tumors cannot be resectable if located in critical eloquent areas. On the other hand, medical imaging procedures can evaluate the entire tumor in a non-invasive and reproducible way. Radiomics is an emerging diagnosis technique based on quantitative medical image analyses, which makes use of data provided by non-invasive diagnosis techniques such as X-ray, computer-tomography (CT), magnetic resonance (MR), and proton emission tomography (PET). Radiomics techniques require the comprehensive analysis of huge numbers of medical images to extract a large and useful number of phenotypic features (usually called radiomics biomarkers). The goal is to explore and obtain the associations between features of tumors, diagnosis and patients' prognoses to choose the best treatments and maximize the patient's survival rate. Current radiomics techniques are not standardized in term of segmentation, feature extraction, and selection, moreover, the decision on suitable therapies still requires the supervision of an expert doctor. In this paper, we propose a semi-automatic methodology aimed to help the identification and segmentation of malignant tissues by using the combination of binary texture recognition, growing area algorithm, and machine learning techniques. In particular, the proposed method not only helps to better identify pathologic tissues but also permits to analyze in a fast way the huge amount of data, in Dicom format, provided by non-invasive diagnostic techniques. A preliminary experimental assessment has been conducted, considering a real MRI database of brain tumors. The method has been compared with the segmentation software's tools "slicer 3D". Breast cancer needs to be detected early to reduce mortality rate. Ultrasound imaging (US) could significantly enhance diagnosing cases with dense breasts. Most of the existing computer-aided diagnosis (CAD) systems employ a single ultrasound image for the breast tumor to extract features to classify it as benign or malignant. However, the accuracy of such CAD system is limited due to the large tumor size and shape variation, irregular and ambiguous tumor boundaries, and low signal-to-noise ratio in ultrasound images due to their noisy nature and the significant similarity between normal and abnormal tissues. To handle these issues, we propose a deep-learning-based radiomics method based on breast US sequences in this paper. The proposed approach involves three main components: radiomic features extraction based on a deep learning network, so-called ConvNeXt, a malignancy score pooling mechanism, and visual interpretations. Specifically, we employ the ConvNeXt network, a deep convolutional neural network (CNN) trained using the vision transformer style. We also propose an efficient pooling mechanism to fuse the malignancy scores of each breast US sequence frame based on image-quality statistics. Background: In recent decades, the application of machine learning technologies to medical imaging has opened up new perspectives in neuro-oncology, in the so-called radiomics field. Radiomics offer new insight into glioma, aiding in clinical decision-making and patients' prognosis evaluation. Although meningiomas represent the most common primary CNS tumor and the majority of them are benign and slow-growing tumors, a minor part of them show a more aggressive behavior with an increased proliferation rate and a tendency to recur. Therefore, their treatment may represent a challenge. Methods: According to PRISMA guidelines, a systematic literature review was performed. We included selected articles (meta-analysis, review, retrospective study, and case–control study) concerning the application of radiomics method in the preoperative diagnostic and prognostic algorithm, and planning for intracranial meningiomas. We also analyzed the contribution of radiomics in differentiating meningiomas from other CNS tumors with similar radiological features. Results: In the first research stage, 273 papers were identified. After a careful screening according to inclusion/exclusion criteria, 39 articles were included in this systematic review. Conclusions: Several preoperative features have been identified to increase preoperative intracranial meningioma assessment for guiding decision-making processes. Pediatric cancer, although rare, requires the most optimized treatment approach to obtain high survival rates and minimize serious long-term side effects in early adulthood. 18F-FDG PET/CT is most helpful and widely used in staging, recurrence detection, and response assessment in pediatric oncology. The well-known 18F-FDG PET metabolic indices of metabolic tumor volume (MTV) and tumor lesion glycolysis (TLG) have already revealed an independent significant prognostic value for survival in oncologic patients, although the corresponding cut-off values remain study-dependent and not validated for use in clinical practice. Advanced tumor "radiomic" analysis sheds new light into these indices. Numerous patterns of texture 18F-FDG uptake features can be extracted from segmented PET tumor images due to new powerful computational systems supporting complex "deep learning" algorithms. This high number of "quantitative" tumor imaging data, although not decrypted in their majority and once standardized for the different imaging systems and segmentation methods, could be used for the development of new "clinical" models for specific cancer types and, more interestingly, for specific age groups. In addition, data from novel techniques of tumor genome analysis could reveal new genes as biomarkers for prognosis and/or targeted therapies in childhood malignancies. Therefore, this ever-growing information of "radiogenomics", in which the underlying tumor "genetic profile" could be expressed in the tumor-imaging signature of "radiomics", possibly represents the next model for precision medicine in pediatric cancer management. The role of medical image computing in oncology is growing stronger, not least due to the unprecedented advancement of computational AI techniques, providing a technological bridge between radiology and oncology, which could significantly accelerate the advancement of precision medicine throughout the cancer care continuum. Medical image processing has been an active field of research for more than three decades, focusing initially on traditional image analysis tasks such as registration segmentation, fusion, and contrast optimization. However, with the advancement of model-based medical image processing, the field of imaging biomarker discovery has focused on transforming functional imaging data into meaningful biomarkers that are able to provide insight into a tumor's pathophysiology. More recently, the advancement of high-performance computing, in conjunction with the availability of large medical imaging datasets, has enabled the deployment of sophisticated machine learning techniques in the context of radiomics and deep learning modeling. This paper reviews and discusses the evolving role of image analysis and processing through the lens of the abovementioned developments, which hold promise for accelerating precision oncology, in the sense of improved diagnosis, prognosis, and treatment planning of cancer.
