症例は42歳，女性．心窩部から左季肋部への疼痛を主訴に当院を受診した．CTにて左横隔膜にヘルニアを認め，胸腔内へ腸管の脱出が認められた．左後方の横隔膜欠損からBochdalek孔ヘルニアと診断された．手術は胸腔鏡下ヘルニア根治術を行ったが，術中に腹腔内への大網の還納が困難と判断し，腹腔鏡下手術を追加した．ヘルニア門の閉鎖は胸腔鏡下に直接縫合にて行った．胸腔鏡，腹腔鏡を併用することで開胸開腹移行をすることなく，根治手術を行うことができた．術後経過は良好であり，8日目に退院となった．術後1年を経過した現在，再発を認めていない．成人Bochdalek孔ヘルニアに対する胸腔鏡－腹腔鏡併用手術はより安全で確実な方法と考えられる．

症例は65歳，男性．腹痛・高度の貧血を主訴に当院入院．血液生化学検査で貧血・CRPの上昇を認め，腫瘍マーカー（CA19-9）の上昇を認めた．腹部CT・腹部MRIで小腸壁肥厚を伴う腫瘤像を認め，両側副腎に転移を疑う腫瘤性病変を認めた．小腸腫瘤性病変からの出血が貧血の原因と診断し，緊急手術を施行した．開腹所見ではTreitz靱帯から20cmの空腸に下行結腸浸潤を伴う腫瘤と腸間膜リンパ節転移を認めたため，小腸・下行結腸部分切除術を施行した．両側副腎腫瘍は遠隔転移と診断し，根治的切除は困難と考え切除しなかった．病理組織学的には異型性の強い細胞を認め，免疫染色では上皮系マーカーのCAM5.2・AE1/AE3，間葉系マーカーのvimentinのみが陽性で，原発性小腸未分化癌と診断した．原発性小腸未分化癌の報告は少なく極めて稀である．今回われわれは，両側副腎転移を伴った原発性小腸未分化癌の1例を経験したので文献的考察を加えて報告する．

症例は52歳の女性．2000年6月にかかりつけ医で甲状腺右葉に34mm大の腫瘤を指摘されたが，以後腺腫様甲状腺腫の診断で経過観察となっていた．2008年7月，起床時に突然呼吸困難を自覚し，自ら救急車を要請．救急車内でチアノーゼと意識障害を認めたため気管内挿管を施行された．同日撮影した頸部CTで，上縦隔まで及ぶ巨大甲状腺腫による高度の気管狭窄を認められ，加療目的にて当院紹介．穿刺吸引細胞診では未分化癌が否定的であったため，手術を施行した．甲状腺腫瘍は圧排性の発育を示し，病理組織学的には被膜で覆われた濾胞性腫瘍で，被膜浸潤を認めたため微少浸潤型濾胞癌と診断した．術後経過は良好で，術後4日目に抜管した．医中誌で「甲状腺濾胞癌」「気管狭窄」「呼吸困難」等のkey wordで検索すると，同様の報告は4例であり比較的稀な疾患と思われる．甲状腺濾胞性腫瘍は高度の気管狭窄をきたすことがあり，注意が必要である．

症例は23歳，女性．生後5カ月時に左副腎神経芽腫に対し腫瘍切除，左腎・副腎合併切除術，術後放射線・化学療法が施行された．22歳時に発熱，左側腹部痛あり，近医受診．脾膿瘍と診断され，保存的治療で軽快したが，3カ月後に再発し，当科紹介となった．神経芽腫の再発はなく，脾膿瘍の原因は同定できなかった．原因精査，根治的治療として開腹手術を施行した．脾彎曲部結腸が脾臓・膵尾部・空腸と一塊となっており，同部を切除した．切除標本で結腸に腫瘍性病変を認め，病理組織学的検査で粘液癌，脾臓・膵尾部への直接浸潤と診断された．放射線照射野内に，10年以上の経過後に癌が発生していること，第二癌の病理組織像，発生臓器が第一癌と異なり，さらには結腸粘液癌であることより，放射線誘発大腸癌と考えられた．

症例1は53歳，女性．他院にて，1996年肝細胞癌（hepatocellular carcinoma，以下HCCと略記）に対して，肝切除術施行した．2001年S5再発に対して経皮的ラジオ波焼灼療法（radiofrequency ablation，以下RFAと略記）を施行した．2006年10月AFP値の増加を認め，CT・MRI検査にて胸壁腫瘍および肝S5，S6，S7に再発腫瘍を認めたため当科紹介された．同年12月，肝S5・6・7開腹下マイクロターゼ凝固術，胸壁腫瘤摘出術施行し，中分化型肝細胞癌であり，胸壁穿刺経路播種と診断した．2007年2月胸壁播種再々発を認め，胸壁腫瘍，横隔膜・助骨合併切除術を施行した．症例2は65歳，男性．2003年前医にてS5のHCCに対してRFAを3回施行したが腫瘍のコントロールつかず，2006年2月当科紹介された．同年2月，肝右葉切除術施行した．同年11月CTにて胸壁の第9肋間にRFA穿刺部に一致して腫瘍を指摘され，同年12月開胸下に肋間腫瘍摘出術を施行した．病理組織学的検査では，中分化型肝細胞癌であった．比較的稀な経皮的RFA後の穿刺経路播種をきたした2症例を経験したので文献的考察を加え報告する．

今回，術前温熱放射線化学療法が奏効し，根治術を施行することができた肛門管癌症例を経験したので報告する．症例は63歳，男性．肛門部腫瘤と疼痛にて来院した．受診時，肛門から臀部にかけて径5.5×7cm大の自壊を伴った腫瘍性病変を認めた，精査にて高分化型肛門管腺癌と診断された．ストーマ造設の上，術前温熱放射線化学療法を施行したところ，腫瘍は著明に縮小し，腹陰式直腸切断術と鼠径部リンパ節を含むリンパ節郭清術を施行し，大腿筋皮弁術および植皮術にて再建した．現在，肛門管癌に対する放射線化学療法は広く普及されている補助療法であるが，温熱療法を併用することにより腫瘍縮小効果はさらに高まると考えられた．肛門管外に巨大発育した肛門管癌に対する手術では会陰部切除が広範囲となり，術前の腫瘍縮小は再建においても非常に重要であると思われる．副作用の少ない温熱療法は術前補助療法として期待される治療法であると考えられた．

結腸癌の消化管内瘻形成は稀な病態である．当科で経験した3例について報告する．症例1は61歳の女性．S状結腸癌，結腸小腸瘻，肝転移の診断でS状結腸切除，小腸合併切除，肝部分切除術を施行した．開腹時に単発の腹膜播種を認めた．腫瘍は回腸と瘻孔を形成していた．術後3年9カ月で肝転移と腹膜播種再発を認めた．症例2は78歳の女性．S状結腸癌の診断で手術施行．腫瘍は空腸に浸潤しておりS状結腸切除，空腸合併切除を行った．摘出標本では腫瘍は空腸に瘻孔を形成していた．術後11カ月で肝・肺転移再発を認めた．症例3は71歳の女性．S状結腸癌，結腸十二指腸瘻と診断し，S状結腸切除，十二指腸合併切除術を施行した．術後3カ月で肝転移再発を認めた．瘻孔形成大腸癌は腹膜播種，肝転移，リンパ節転移の頻度が低く，根治切除で良好な予後が見込めるとする報告が多いが，経験した3例とも再発を認めており，進行度に準じた慎重な経験観察が必要である．

36例のgastrointestinal stromal tumorを臨床病理学的・免疫組織化学的に再検討した．発生部位は胃22例，十二指腸・小腸11例，直腸3例であった．組織学的には3例で上皮様成分が混在していた．免疫染色では全例でc-kit陽性だったが，1例では発現が弱く限局性であった．超低リスク（VL）群9例，低リスク（L）群10例，中リスク（I）群9例，高リスク（H）群8例に分類され，VL群／L群に転移はみられなかったが，I群／H群のうち10例に再発・転移を認めた．無再発生存率は，I群／H群はVL群／L群に比し，また，十二指腸・小腸・直腸原発例は胃原発例に比し，有意に不良であった．メチル酸イマチニブを投与された5例のうち4例は部分寛解が得られたが，c-kit発現が限局性だった残りの1例は効果が認められず死亡した．I群／H群，および，c-kit発現に乏しい例の予後改善が課題である．

Crohn病 (CD) に合併した大腸癌は比較的稀といわれているが, われわれは肛門管癌を合併した症例を経験したので報告する. 症例は74歳, 男性. 61歳時, 難治性腸瘻のため回盲部切除, S状結腸部分切除を施行された際に, 小腸大腸型CDと診断された. 平成17年10月, 下血を認め大腸内視鏡検査を施行. 肛門管に発赤隆起を認め, 生検で高分化腺癌と診断された. 肛門管癌の診断で腹会陰式直腸切断術を施行した. 病理組織学的所見は, P, 1型, 30×25mm, tub1, MP, N0, ly1, v0, H0, P0, M0, Stage Iであった. 直腸粘膜には少数の炎症細胞浸潤がみられたが, CDに特徴的な類上皮肉芽形成や, 全層性炎症症状はみられなかった. 経過は良好で, 術後26日目に退院となった.CD発症後の長期経過例で大腸に病変を有するものは, 潰瘍性大腸炎 (UC) と同様に, 通常と比べて癌の発生率は高い. そのためCDの長期経過例に対しては, UCと同様のサーベイランスが必要である.

症例は41歳の男性. 5年前に2型糖尿病の診断を受けていたが通院せず,放置していた.左前胸部と左腰部の疼痛・腫張を主訴に外来を受診し, CTにより同部の化膿性筋炎が疑われ,切開排膿術を施行した.インスリンによる血糖管理と抗生剤投与を開始したが炎症反応が持続するために再度CT検査を施行したところ,右腹部と右下肢にも膿瘍形成が確認された.同様に切開排膿し,症状は改善した. 化膿性筋炎の原因として宿主免疫能低下の関わっている例が多いとされているが,特に糖尿病患者が筋肉の腫張や疼痛を訴える場合には常に本症を念頭に置く必要がある.さらに病変部が多発している可能性を考慮し,早期に全身検索を行うことが肝要と思われた.

The role of peritoneal lavage cytology (PLC) in surgery for gastric cancer was investigated in terms of the correlations between PLC and clinicopathological findings of advanced gastric cancers. Subjects were 49 patients who were diagnosed as having gastric cancer with the depth of tumor invasion of more than T_??_on imaging methods preoperatively and underwent peritoneal lavage at laparotomy. Positive carcinoma cells determined by PLC was found in 38.8% of the patients and the rate of positive cytology was significantly correlated with the following factors: macroscopic peritoneal dissemination; the depth of tumor invasion, especially serosal invasion; and the infiltrating growth pattern of the tumor. Among patients without macroscopic peritoneal dissemination, five were found as positive in PLC. None of these patients survived from peritoneal recurrence within 2 years after surgery. These results suggest that PLC are useful for predicting the peritoneal cancer dissemination. For future treatment of gastric cancer, patients with positive PLC are expected to be targeted for intensive adjuvant therapies during and/or after surgery.

肝腫瘍患者24例(肝細胞癌16例,転移性肝腫瘍8例)に対しcomputed tomography during arterial portography (CTAP) とhelical-CTを施行し,両者における腫瘍検出率の差を比較検討した. 24例においてCTAPでは101個の腫瘍が検出されたが, helical-CTでは36個 (35.6%) にすぎなかった. 1cm未満の腫瘍はCTAPで20個検出されたが, helical-CTでは4個 (20%) にすぎなかった. helical-CTで手術可能とされたがCTAPで手術不能と判断された症例は24例中5例(肝細胞癌3例,転移性肝腫瘍2例)だった.これらの症例でCTAPでは63個の腫瘍が検出された. 1cm以上の腫瘍は47個検出されたが, helical-CTでは13個 (27.7%) にすぎなかった. 1cm未満の腫瘍は16個検出されたが, helical-CTでは4個 (25%) にすぎなかった.このように多発傾向にある肝腫瘍の局在診断においてhelical-CTでは不十分であり,手術適応の判断も含めCTAPは必要不可欠な検査法と思われる.

A case of ampullary carcinoma with a jumping metastasis to the paraaortic lymph node is presented. A 51-year-old woman was admitted to the hospital because of epigastrial discomfort. Liver dysfunction had been noticed since one year before without a history of jaundice. CA 19-9 level elevated. Intrahepatic bile duct dilatation was detected by ultrasound and computed tomography. Hypotonic duodenography showed a protruded mass, 4cm in size, which was well differentiated adenocarcinoma by endoscopic biopsy. There was no ascites, liver metastasis or peritoneal dissemination on laparotomy. An elastic hard tumor was palpable at the second portion of the duodenum without any invasion to the serosa. A pylorus-preserving pancreatico-duodenectomy was performed with extended lymph node dissection including paraaortic regions, and with intraoperative radiation. The mass was an exposed protruding type, invading the submucosa of the duodenum. The only one paraaortic lymph node was metastasized of all 55 dissected lymph nodes. We conclude that there is a direct stream of lymph from the papilla to paraaorta and that the paraaortic lymph node dissection would be mandatory for surgery of ampullary carcinoma even in an early stage.

A case of mucin hypersecreting invasive carcinoma of the pancreas penetrating into the stomach and duodenum in a 56-year-old Japanese man is reported. The patient survive for more than five years after a complete resection. It has been reported that the clinical course of patients with non-invasive mucin hypersecreting carcinoma of the pancreas is favorable, whereas that with invasive mucin hypersecreting carcinoma of the pancreas is as dismal as usual pancreatic carcinomas. In this case report, we recommend a complete resection even for invasive mucin hypersecreting carcinoma of the pancreas that can provide a long survival.

We experienced a case of apocrine carcinoma of the breast which was diagnosed preoperatively by needle aspiration biopsy cytlogy. A 63-year-old woman was seen at the hospital because of a tumor 3cm in diameter. Aspiration biopsy cytology (ABC) was performed preoperatively, and atypical cells with abundant fine eosinophilic granules were found. From findings of mammography, ultrasonography, and ABC, an apocrine carcinoma was strongly suspected. Modified radical mastectomy was performed, and the resected specimen showed that the tumor was invasive apocrine carcinoma with calcification. Apocrine carcinomas of the breast are rare, representing from 0.2% to 1.5% of all breast carcinomas. In Japan, one hundred and twenty one cases have been reported in a recent 14 year period. This report presents a review of 110 out of them.

A 21-year-old woman was found to have a developmental presacral cyst during pregnancy. The patient complained of low back pain. With digital examination of the rectum, a soft tumor was palpated in the posterior wall of the rectum. In making diagnosis ultrasonic examination, CT and magnetic resonant imaging were useful. The patient had a fever during observation of clinical course, and fistula was formed. Parts of the coccyx and sacrum were excised and the cyct was extirpated through dorsal approach. The extirpated cyst was 8×6×6cm, containing white cloud liquid. Histologically, the cyst lining consisted of squamous epithelium, and was diagnosed as a epidermoidcyst. There has been no recurrence, as of 2 years after the operation.

Recent experiance with 3 cases of catamenial pneumothorax is descrived. Patients were 40, 25 and 33-year-old wemen. Clinical diagnosis of catamenial pneumothorax was made based on their history and thoracotomy was performed in all 3 cases. The pneumothorax was localized on the right side in 2 cases and on the left side in another. In all cases, no lesions were observed on the diaphragmatic pleula, but on the viseral pleula adhesion with palietal pleula (2 cases) and numerous blebs (1 cases) were observed, and microscopic examination revealed the presense of endometrial tissue in these lesions. In Japan, 53 cases of catamenial pneumothorax, incruding these 3 cases, have been reported, but intra-thoracic endometriosis was histologically recognized in 25 cases. Out of these 25 cases, endometrial tissue was found on the diaphragma in 19 cases, and only in 6 cases found on the viseral pleula.
