Abstract: International audience; Muon neutrino and antineutrino disappearance probabilities are identical in the standard three-flavor neutrino oscillation framework, but CPT violation and non-standard interactions can violate this symmetry. In this work we report the measurements of $\sin^{2} \theta_{23}$ and $\Delta m_{32}^2$ independently for neutrinos and antineutrinos. The aforementioned symmetry violation would manifest as an inconsistency in the neutrino and antineutrino oscillation parameters. The analysis discussed here uses a total of 1.97$\times$10$^{21}$ and 1.63$\times$10$^{21}$ protons on target taken with a neutrino and antineutrino beam respectively, and benefits from improved flux and cross-section models, new near detector samples and more than double the data reducing the overall uncertainty of the result. No significant deviation is observed, consistent with the standard neutrino oscillation picture.

Abstract: Nanoviruses are plant multipartite viruses with a genome composed of six to eight circular single-stranded DNA segments. The distinct genome segments are encapsidated individually in icosahedral particles that measure ≈18 nm in diameter. Recent studies on the model speciesFaba bean necrotic stunt virus(FBNSV) revealed that complete sets of genomic segments rarely occur in infected plant cells and that the function encoded by a given viral segment can complement the others across neighbouring cells, presumably by translocation of the gene products through unknown molecular processes. This allows the viral genome to replicate, assemble into viral particles and infect anew, even with the distinct genome segments scattered in different cells. Here, we question the form under which the FBNSV genetic material propagates long distance within the vasculature of host plants and, in particular, whether viral particle assembly is required. Using structure-guided mutagenesis based on a 3.2 Å resolution cryogenic-electron-microscopy reconstruction of the FBNSV particles, we demonstrate that specific site-directed mutations preventing capsid formation systematically suppress FBNSV long-distance movement, and thus systemic infection of host plants, despite positive detection of the mutated coat protein when the corresponding segment is agroinfiltrated into plant leaves. These results strongly suggest that the viral genome does not propagate within the plant vascular system under the form of uncoated DNA molecules or DNA:coat-protein complexes, but rather moves long distance as assembled viral particles.

Abstract: To evaluate the proportion, baseline predictors, and outcomes of neovascular age-related macular degeneration (nAMD) patients treated with a high burden of vascular endothelial growth factor (VEGF) inhibitor intravitreal injections (IVT) at two years in routine clinical practice. European retrospective data analysis from a prospectively designed observational outcomes registry: the Fight Retinal Blindness! (FRB!) Project. Treatment-naïve eyes (one eye per patient) initiating VEGF inhibitors IVT from January 2017 to March 2020 with 24 months of follow-up, a minimum of 3 injections, and no gap between visits exceeding 365 days were tracked in the registry. Abstract: This work considers a radio-astronomy inverse problem of physical parameters inference from multispectral images. The forward model is a numerical simulation, and the observation model mixes different sources of noise. This results in a non-explicit non-log-concave likelihood function. We introduce a likelihood approximation with controlled error that allow the conception of a Monte Carlo Markov Chain (MCMC) method. The obtained sampler provides credibility intervals along with point estimates. We believe that the proposed approach is sufficiently generic to be applied to similar inverse problems., Ce travail considère un problème inverse en astrophysique, qui consiste à estimer un ensemble de paramètres physiques à partir d'images multispectrales. Le modèle direct sous-jacent est une simulation numérique et le modèle d'observation mélange différentes sources de bruit. Ces caractéristiques donnent lieu à une fonction de log-vraisemblance non explicite et non concave. Nous en définissons une approximation qui permet de concevoir une méthode de Monte Carlo par chaîne de Markov (MCMC). L'échantillonneur obtenu fournit estimations ponctuelles et intervalles de crédibilité associés. L'approche proposée est suffisamment générale pour être appliquée à des problèmes inverses similaires.

Abstract: International audience; Tuberculosis claims significantly more than one million lives each year. A feasible way to face the issue of drug resistance is the development of new antibiotics. Bacterial uridine 5’-monophosphate (UMP) kinase is a promis- ing target for novel antibiotic discovery as it is essential for bacterial sur- vival and has no counterpart in human cells. The UMP kinase from M. tuberculosis is also a model of particular interest for allosteric regula- tion with two effectors, GTP (positive) and UTP (negative). In this study, using X-ray crystallography and cryo-electron microscopy, we report for the first time a detailed description of the negative effector UTP-binding site of a typical Gram-positive behaving UMP kinase. Comparison between this snapshot of low affinity for Mg-ATP with our previous 3D-structure of the GTP-bound complex of high affinity for Mg-ATP led to a better understanding of the cooperative mechanism and the allosteric regulation of UMP kinase. Thermal shift assay and circular dichroism experiments corroborate our model of an inhibition by UTP linked to higher flexibility of the Mg-ATP-binding domain. These new structural insights provide valuable knowledge for future drug discovery strategies targeting bacterial UMP kinases.

Abstract: International audience; This work considers a challenging radio-astronomyinverse problem of physical parameter inference from multispec-tral observations. The forward model underlying this problem isa computationally expensive numerical simulation. In addition,the observation model mixes different sources of noise yieldinga non-concave log-likelihood function. To overcome these issues,we introduce a likelihood approximation with controlled error.Given the absence of ground truth, parameter inference isconducted with a Markov chain Monte Carlo (MCMC) algorithmto provide credibility intervals along with point estimates. To thisaim, we propose a new sampler that addresses the numericalchallenges induced by the observation model, in particular thenon-log-concavity of the posterior distribution. The efficiency ofthe proposed method is demonstrated on synthetic yet realisticastrophysical data. Abstract: The retinal vascular network fractal dimension (FD) could be a promising imaging biomarker. Our objective was to evaluate its repeatability and reproducibility in healthy eyes. A cross-sectional study was undertaken with young, healthy volunteers who had no reported cardiac risk factors or ocular disease history. For each participant, three SS-OCTA images (12 × 12 mm) were acquired using the Plex Elite 9000 (Carl Zeiss Meditec AG, Jena, Germany) by two ophthalmologists. Automated segmentation was obtained from both the superficial and deep capillary plexuses. FD was estimated by box counting. The intraclass correlation coefficients (ICC) were used as measures for repeatability and reproducibility. A total of 43 eyes of healthy volunteers were included. The mean ± standard deviation (SD) age was 30 ± 6.2 years. The results show good repeatability. The ICC was 0.722 (95% CI, 0.541–0.839) in the superficial capillary plexus and 0.828 (95% CI, 0.705–0.903) in the deep capillary plexus. For reproducibility, the ICC was 0.651 (95% CI, 0.439–0.795) and 0.363 (95% CI, 0.073–0.596) at the superficial and deep capillary plexus, respectively. In this study, the FD of the vascular network measured via SS-OCTA showed good repeatability and reproducibility in healthy participants.

Abstract: The bacterial Rho factor is a ring-shaped motor triggering genome-wide transcription termination and R-loop dissociation. Rho is essential in many species, including in Mycobacterium tuberculosis where rho gene inactivation leads to rapid death. Yet, the M. tuberculosis Rho [MtbRho] factor displays poor NTPase and helicase activities, and resistance to the natural Rho inhibitor bicyclomycin [BCM] that remain unexplained. To address these issues, we solved the cryo-EM structure of MtbRho at 3.3 Å resolution. The MtbRho hexamer is poised into a pre-catalytic, open-ring state wherein specific contacts stabilize ATP in intersubunit ATPase pockets, thereby explaining the cofactor preference of MtbRho. We reveal a leucine-to-methionine substitution that creates a steric bulk in BCM binding cavities near the positions of ATP γ-phosphates, and confers resistance to BCM at the expense of motor efficiency. Our work contributes to explain the unusual features of MtbRho and provides a framework for future antibiotic development.

Abstract: International audience; The gut microbiota is a complex ecosystem that inhabits the gastrointestinal tract and consists of archaea, fungi, viruses, and bacteria, with bacteria being dominant. From birth onwards, it coevolves dynamically together with the host. The composition of the gut microbiota is under the influence of a complex interplay between both host and environmental factors. Scientific advances in the past few decades have shown that it is essential in maintaining homeostasis and tipping the balance between health and disease. In addition to its role in food digestion, the gut microbiota is implicated in regulating multiple physiological processes in the host gut mucosa and in distant organs such as the brain. Persistent imbalance between gut microbial communities, termed "dysbiosis," has been associated with several inflammatory and metabolic diseases as well as with central nervous system disorders. In this review, we present the state of the art of current knowledge on an emerging concept, the microbiota-retina axis, and the potential role of its disturbance in the development of retinopathies. We also describe several microbiota-targeting strategies that could constitute preventive and therapeutic tools for retinopathies.

Abstract: The atomic-to-molecular hydrogen (H/H2) transition has been extensively studied as it controls the fraction of gas in a molecular state in an interstellar cloud. This fraction is linked to star-formation by the Schmidt-Kennicutt law. While theoretical estimates of the column density of the H I layer have been proposed for static photodissociation regions (PDRs), Herschel and well-resolved ALMA (Atacama Large Millimeter Array) observations have revealed dynamical effects in star forming regions, caused by the process of photoevaporation. We extend the analytic study of the H/H2 transition to include the effects of the propagation of the ionization front, in particular in the presence of photoevaporation at the walls of blister H II regions, and we find its consequences on the total atomic hydrogen column density at the surface of clouds in the presence of an ultraviolet field, and on the properties of the H/H2 transition. We solved semi-analytically the differential equation giving the H2 column density profile by taking into account H2 formation on grains, H2 photodissociation, and the ionization front propagation dynamics modeled as advection of the gas through the ionization front. Taking this advection into account reduces the width of the atomic region compared to static models. The atomic region may disappear if the ionization front velocity exceeds a certain value, leading the H/H2 transition and the ionization front to merge. For both dissociated and merged configurations, we provide analytical expressions to determine the total H I column density. Our results take the metallicity into account. Finally, we compared our results to observations of PDRs illuminated by O-stars, for which we conclude that the dynamical effects are strong, especially for low-excitation PDRs., Accepted by Astronomy & Astrophysics on September, 11th 2021. 18 pages, 16 figures

Abstract: The integration of a viral genome into the host genome has a major impact on the trajectory of the infected cell. Integration location and variation within the associated viral genome can influence both clonal expansion and persistence of infected cells. Methods based on short-read sequencing can identify viral insertion sites, but the sequence of the viral genomes within remains unobserved. We develop PCIP-seq, a method that leverages long reads to identify insertion sites and sequence their associated viral genome. Abstract: Physical review / D 101(3), 032006 (1-19) (2020). doi:10.1103/PhysRevD.101.032006, The ordering of the neutrino mass eigenstates is one of the fundamental open questions in neutrino physics. While current-generation neutrino oscillation experiments are able to produce moderate indications on this ordering, upcoming experiments of the next generation aim to provide conclusive evidence. In this paper we study the combined performance of the two future multi-purpose neutrino oscillation experiments JUNO and the IceCube Upgrade, which employ two very distinct and complementary routes towards the neutrino mass ordering. The approach pursued by the $20\,\mathrm{kt}$ medium-baseline reactor neutrino experiment JUNO consists of a careful investigation of the energy spectrum of oscillated $\bar{\nu}_e$ produced by ten nuclear reactor cores. The IceCube Upgrade, on the other hand, which consists of seven additional densely instrumented strings deployed in the center of IceCube DeepCore, will observe large numbers of atmospheric neutrinos that have undergone oscillations affected by Earth matter. In a joint fit with both approaches, tension occurs between their preferred mass-squared differences $ \Delta m_{31}^{2}=m_{3}^{2}-m_{1}^{2} $ within the wrong mass ordering. In the case of JUNO and the IceCube Upgrade, this allows to exclude the wrong ordering at $>5\sigma$ on a timescale of 3--7 years --- even under circumstances that are unfavorable to the experiments' individual sensitivities. For PINGU, a 26-string detector array designed as a potential low-energy extension to IceCube, the inverted ordering could be excluded within 1.5 years (3 years for the normal ordering) in a joint analysis., Published by Inst.302363, Melville, NY

Abstract: The Multimessenger Diversity Network (MDN), formed in 2018, extends the basic principle of multimessenger astronomy -- that working collaboratively with different approaches enhances understanding and enables previously impossible discoveries -- to equity, diversity, and inclusion (EDI) in science research collaborations. With support from the National Science Foundation INCLUDES program, the MDN focuses on increasing EDI by sharing knowledge, experiences, training, and resources among representatives from multimessenger science collaborations. Representatives to the MDN become engagement leads in their collaboration, extending the reach of the community of practice. Abstract: International audience; Purpose : To contribute to the WHO initiative, VISION 2020: The Right to Sight, an assessment of the causes of global vision impairment in 2020 and temporal change is needed. We aimed to extensively update estimates of global vision loss burden due to age-related macular degeneration (AMD), presenting estimates for 2020, temporal change since this initiative commenced, and distribution by sex and region.Methods : We did a systematic review and meta-analysis of population-based surveys of eye disease from January 1980 to October 2018. The data from these surveys are collated by the Vision Loss Expert Group in the Global Vision Database. We fitted hierarchical models to estimate prevalence (with 95% uncertainty intervals [UIs]) of moderate and severe vision impairment (MSVI; presenting visual acuity from -30%) in age-standardized AMD blindness rates between 2000 and 2020 was in Southeast Asia, East Asia, and Oceania (-34.1%, -32.8 to -36.1).Conclusions : Despite a decrease in the age-adjusted prevalence of AMD-related blindness, the increase in the age-adjusted prevalence of AMD-related MSVI and the increase and ageing of the population led to a marked increase in the number of individuals affected by AMD-related blindness and MSVI.

Abstract: The T2K experiment reports updated measurements of neutrino and antineutrino oscillations using both appearance and disappearance channels. This result comes from an exposure of $14.9~(16.4) \times 10^{20}$ protons on target in neutrino (antineutrino) mode. Significant improvements have been made to the neutrino interaction model and far detector reconstruction. An extensive set of simulated data studies have also been performed to quantify the effect interaction model uncertainties have on the T2K oscillation parameter sensitivity. T2K performs multiple oscillation analyses that present both frequentist and Bayesian intervals for the PMNS parameters. For fits including a constraint on \ssqthonethree from reactor data and assuming normal mass ordering T2K measures $\sin^2\theta_{23} = 0.53^{+0.03}_{-0.04}$ and $\Delta{}m^2_{32} = (2.45 \pm 0.07) \times 10^{-3}$ eV$^{2}$c$^{-4}$. The Bayesian analyses show a weak preference for normal mass ordering (89% posterior probability) and the upper $\sin^2\theta_{23}$ octant (80% posterior probability), with a uniform prior probability assumed in both cases. Abstract: International audience; Diet shapes the gut microbiota which impacts hepatic lipid metabolism. Modifications in liver fat content are associated with metabolic disorders. We investigated the extent of dietary fat and fiber-induced alterations in the composition of gut microbiota and hepatic fatty acids (FAs). Mice were fed a purified low-fat diet (LFD) or high-fat diet (HFD) containing non-soluble fiber cellulose or soluble fiber inulin. HFD induced hepatic decreases in the amounts of C14:0, C16:1n-7, C18:1n-7 and increases in the amounts of C17:0, C20:0, C16:1n-9, C22:5n-3, C20:2n-6, C20:3n-6, and C22:4n-6. When incorporated in a LFD, inulin poorly affected the profile of FAs. However, when incorporated in a HFD, it (i) specifically led to an increase in the amounts of hepatic C18:0, C22:0, total polyunsaturated FAs (PUFAs), total n-6 PUFAs, C18:3n-3, and C18:2n-6, (ii) exacerbated the HFD-induced increase in the amount of C17:0, and (iii) prevented the HFD-induced increases in C16:1n-9 and C20:3n-6. Importantly, the expression/activity of some elongases and desaturases, as well as the gut microbiota composition, were impacted by the dietary fat and fiber content. To conclude, inulin modulated gut microbiota and hepatic fatty acid composition, and further investigations will determine whether a causal relationship exists between these two parameters.

Abstract: International audience; OBJECTIVES: To assess the quality-of-life improvement after scleral lens fitting in patients with keratoconus. METHODS: We conducted a prospective observational study of all consecutive patients with keratoconus starting scleral lens fitting in 2019, from January 2 to December 27, at the Dijon University Hospital, France. Quality of life was assessed before and after wearing scleral lenses for 3 months through the National Eye Institute Visual Function Questionnaire 25 (NEI-VFQ 25) questionnaire. We investigated factors potentially influencing quality of life: final visual acuity, keratoconus stage, and maximum keratometry reading. RESULTS: In total, 41 eyes of 24 patients were successfully fitted with scleral lenses, 92.9% were 18 mm in diameter and 7.1% were 17 mm. Abstract: The current COVID-19 pandemic is an exceptional health situation including for drug use. As there was no known effective drug for COVID-19 at the beginning of the pandemic, different candidates were proposed. In this short article, we present the French public pharmacovigilance activities during this health crisis. Although COVID-19 is a confounding factor per se, owing to its potential for multi-organ damage including the heart and kidney, the quality of the transmitted data in adverse drug reaction reports, the timeliness of feedback from clinicians, and the real-time pharmacological and medical analysis by the French network of the regional pharmacovigilance centers made it possible to swiftly identify relevant safety signals. The French National Agency of Medicine was thus able to validate the data and convey their findings very early. This decentralized organization based on medical and pharmacological evaluation of case reports has proven to be efficient and responsive in this unique and challenging healthcare emergency.

Abstract: Resume La description du reseau microvasculaire retinien beneficie, depuis plus de 10 ans, du developpement de nouvelles techniques d’imagerie. Les logiciels automatiques d’analyse d’images, ainsi que l’OCT-angiographie (OCT-A) permettent de mettre en evidence des modifications subtiles et precoces de l’arbre vasculaire retinien grâce a de nombreuses donnees quantitatives microvasculaires. L’enjeu des recherches actuelles est de demontrer l’association entre ces modifications microvasculaires, le vieillissement vasculaire systemique et les pathologies cerebro et cardiovasculaires. En effet, un continuum physiopathologique existe entre alterations microvasculaires retiniennes et pathologies vasculaires systemiques. Dans l’etude de population Montrachet, nous avons montre qu’un reseau vasculaire suboptimal, evalue sur fond d’œil avec le logiciel Singapore I Vessel Assessment (SIVA), etait significativement associe a l’existence d’un diabete traite et d’un sur risque de mortalite cardiovasculaire. Par ailleurs, nous avons complete nos recherches sur le reseau vasculaire retinien avec l’utilisation de l’OCT-A. Dans l’etude EYE-MI, nous avons demontre le potentiel interet de caracteriser, de maniere quantitative, le reseau microvasculaire retinien en OCT-A afin d’evaluer le profil de risque cardiovasculaire des patients atteints de cardiopathies ischemiques. Un score de risque AHA (American Heart Association) eleve etait associe a une faible densite vasculaire retinienne independamment des modifications hemodynamiques systemiques. Ainsi, une meilleure comprehension du lien entre microvascularisation retinienne et pathologie macrovasculaire permettrait d’envisager son utilisation pour une identification precoce des patients a risque et de leur proposer un programme de prevention primaire personnalise. Le reseau vasculaire retinien pourrait donc etre un temoin de l’histoire vasculaire systemique mais aussi un biomarqueur predictif interessant d’evenements vasculaires systemiques.

Abstract: International audience; Purpose To evaluate the capacity of retinal nerve fibre layer (RNFL) thickness measured by SD-OCT to discriminate glaucoma patients from controls in an elderly population. Methods The MONTRACHET (Maculopathy, Optic Nerve, nuTRition, neurovAsCular and HEarT diseases) Study is a population-based study including participants aged 75 years and over. All participants underwent a complete eye examination with optic nerve photographs, visual field testing and OCT peripapillary RNFL thickness measurement. Glaucoma was defined according to the ISGEO (International Society for Epidemiologic and Geographical Ophthalmology) classification. Performance indicators were calculated including area under the receiver operating characteristics curves (AUC), likelihood ratios (LR) and diagnostic odds ratios (DOR). Results In total, 1061 participants were included in the study, of whom 89 were classified as having glaucoma and 972 were classified as normal. The mean (SD) age of the population was 82.3 (3.7) years. The average RNFL thickness was significantly lower in the glaucoma group than in controls 64.0 (14.9) mu m versus 88.9 (12.4) mu m, respectively, p < 0.001) and in all sectors compared with controls. The average RNFL thickness had the highest AUC (0.901) followed by the temporal-inferior (0.879) and temporal-superior sectors (0.862). When RNFL thickness was classified as abnormal by SD-OCT, the average RNFL thickness had the best sensitivity (83.75%) followed by the temporal-inferior sector (75.64%). The specificity for these two parameters was 87.34% and 91.08%, respectively. The highest DOR was 28.70 for average RNFL thickness and reached 34.84 when using the reference database of the OCT manufacturer. Conclusion This study confirms that SD-OCT could be useful as an additional test to discriminate glaucoma patients from controls in an elderly population.

Abstract: International audience; System requirements engineering is a key feature of complex systems engineering. This book proposes a method inspired by the best practices of system engineering standards: it favors a model-based systems engineering approach using the system modeling language SysML.System Requirements Engineering is divided into two parts. Part 1 defines the context, definitions and main concepts of requirements engineering as well as relevant activities before giving the proposed method, comprised of eight steps. Each step involves a discussion of the desired goal and how to achieve it using SysML.Part 2 illustrates the methods used by treating a case study and, with the use of detailed examples, to define on the one hand the needs of the stakeholders and on the other hand the system requirements. Finally, the use of the SysML supported requirements engineering method is discussed in conjunction with the Harmony, Arcadia and CESAM engineering methods.

Abstract: 27 pags., 19 figs., 4 tabs., Molecular hydrogen being unobservable in cold molecular clouds, the column density measurements of molecular gas currently rely either on dust emission observation in the far-IR or on star counting. (Sub-)millimeter observations of numerous trace molecules are effective from ground based telescopes, but the relationships between the emission of one molecular line and the H2 column density (NH2) is non-linear and sensitive to excitation conditions, optical depths, abundance variations due to the underlying physico-chemistry. We aim to use multi-molecule line emission to infer NH2 from radio observations. We propose a data-driven approach to determine NH2 from radio molecular line observations. We use supervised machine learning methods (Random Forests) on wide-field hyperspectral IRAM-30m observations of the Orion B molecular cloud to train a predictor of NH2, using a limited set of molecular lines as input, and the Herschel-based dust-derived NH2 as ground truth output. For conditions similar to the Orion B molecular cloud, we obtain predictions of NH2 within a typical factor of 1.2 from the Herschel-based estimates. An analysis of the contributions of the different lines to the predictions show that the most important lines are $^{13}$CO(1-0), $^{12}$CO(1-0), C$^{18}$O(1-0), and HCO$^+$(1-0). A detailed analysis distinguishing between diffuse, translucent, filamentary, and dense core conditions show that the importance of these four lines depends on the regime, and that it is recommended to add the N$_2$H$^+$(1-0) and CH$_3$OH(20-10) lines for the prediction of NH2 in dense core conditions. This article opens a promising avenue to directly infer important physical parameters from the molecular line emission in the millimeter domain. The next step will be to try to infer several parameters simultaneously (e.g., NH2 and far-UV illumination field) to further test the method. Abstract: International audience; Purpose Preservatives contained in glaucoma eye drops have been shown to have a deleterious impact on the ocular surface. We aimed to assess the association between preservative exposure and the occurrence of further glaucoma surgery among patients with glaucoma or ocular hypertension in France. Methods The study concerned all patients who first received glaucoma eye drop treatments in a French medical-administrative database (EGB) between 2008 and 2015. Three groups were created according to the level of preservative exposure during the whole follow-up: '0% preservatives', 'mixed' and '100% preservatives'. The occurrence of glaucoma surgery was estimated according to preservative exposure indicators in Cox multivariate models adjusted on age, sex, number of glaucoma eye drops simultaneously used, systemic antihypertensive treatment and duration of treatment. Results The sample consisted of 12 454 patients. The median (interquartile range) follow-up was 4.1 (1.7-6.1) years. A total of 231 (1.9%) patients underwent glaucoma surgery during follow-up. On multivariable analysis, the risk of glaucoma surgery was increased for the 'mixed' group (hazard ratio [HR] = 3.94 [95% CI, 1.54-10.05]) and for the '100% preservative' group (HR = 7.97 [95% CI, 3.07-20.67]) when compared with the 0% preservative group. Conclusion We found an association between exposure to glaucoma eye drop preservatives and the prevalence of further glaucoma surgery. While these data might be used to support the consideration of routine use of preservative-free drops, in the absence of a randomized clinical trial, they cannot prove a direct cause-and-effect relationship between preservative-free glaucoma eye drops and further glaucoma surgery.

Abstract: PURPOSE: To evaluate the efficacy of intravitreal anti-vascular endothelial growth factor (anti-VEGF) injections (IVT) in diabetic macular edema (DME) in real-life practice using the Save Sight Registries (SSR). MATERIAL AND METHODS: We conducted an observational, single-centre, retrospective study in the department of ophthalmology of the Dijon University Hospital. We included treatment-naive patients who presented with DME between January 2016 and December 2017. Demographic and clinical data, follow-up visits, and treatments administered were entered into the SSR, an international online ophthalmic registry. Primary endpoints were the change in best-corrected visual acuity (BCVA) and central subfield thickness (CST) from baseline to 12 and 24 months. RESULTS: Fifty-eight eyes of 43 patients with a mean [standard deviation (SD)] age of 67.1 [9.5] years were included. Forty-one eyes completed 12 months of follow-up, and 17 eyes completed 24 months of follow up. Median [SD] baseline BCVA was 56.1 [22.9] ETDRS letters and the median [95% confidence interval (95% CI)] baseline CST was 447.9 [161.0] micrometers (mum). Median [95% CI] improvement in BCVA from baseline to months 12 and 24 were respectively, +5.6 [+0.5; +10.7] ETDRS letters and +7.7 [-2.8; +18.2] ETDRS letters. The median [95% CI] decrease in CST from baseline to months 12 and 24 were respectively, -110.9 [-154.5; -67.3] mum and -125.5 [-198.0; -53.0] mum. CONCLUSION: Our clinical practice can be evaluated easily with the SSR system. In real life, anti-VEGF IVT are an effective treatment for DME, which result in improved BCVA and decreased CST.; Objectif Évaluer l’efficacité des injections d’anti-VEGF et nos pratiques dans l’œdème maculaire diabétique (OMD) en vraie-vie grâce à l’utilisation du Save Sight Registries (SSR). Matériels et méthodes Il s’agit d’une étude observationnelle, monocentrique, rétrospective réalisée dans le service d’ophtalmologie du CHU de Dijon incluant les patients traités pour un OMD entre janvier 2016 et décembre 2017. Les données cliniques et démographiques, les visites de suivi, les traitements administrés ont été implémentés sur la plateforme internet du SSR. Les critères de jugement principaux étaient l’évolution de la meilleure acuité visuelle corrigée (MAVC) et de l’épaisseur centrale fovéolaire (ECF) entre l’inclusion et les visites à 12 mois et 24 mois. Résultats Cinquante-huit yeux de 43 patients d’âge moyen [déviation standard] de 67,1 [5,9] ans ont été inclus. Quarante et un yeux ont poursuivi un suivi de 12 mois et 17 yeux de 24 mois. La médiane [DS] de la MAVC initiale était de 56,1 [9,22] lettres ETDRS et la médiane [intervalle de confiance 95% (IC 95%)] de l’ECF initiale était de 447,9 [161,0] micromètres (μm). Le gain médian [IC 95%] de la MAVC entre l’inclusion et le mois 12 et 24 était respectivement de +5,6 [+0,5; +10,7] et de +7,7 [−2,8; +18,2] lettres ETDRS. La diminution médiane [IC 95%] de l’ECF entre l’inclusion et le mois 12 et 24 était respectivement de −110,9 [−154,5; −67,3] μm et de −125,5 [−198,0; −53,0] μm. Abstract: International audience; Objectives The predominance of extramuscular manifestations (e.g., skin rash, arthralgia, interstitial lung disease [ILD]) as well as the low frequency of muscle signs in anti–melanoma differentiation-associated gene 5 antibody–positive (anti-MDA5+) dermatomyositis caused us to question the term myositis-specific antibody for the anti-MDA5 antibody, as well as the homogeneity of the disease. Methods To characterize the anti-MDA5+ phenotype, an unsupervised analysis was performed on anti-MDA5+ patients (n = 83/121) and compared to a group of patients with myositis without anti-MDA5 antibody (anti-MDA5−; n = 190/201) based on selected variables, collected retrospectively, without any missing data. Results Within anti-MDA5+ patients (n = 83), 3 subgroups were identified. One group (18.1%) corresponded to patients with a rapidly progressive ILD (93.3%; p < 0.0001 across all) and a very high mortality rate. The second subgroup (55.4%) corresponded to patients with pure dermato-rheumatologic symptoms (arthralgia; 82.6%; p < 0.01) and a good prognosis. The third corresponded to patients, mainly male (72.7%; p < 0.0001), with severe skin vasculopathy, frequent signs of myositis (proximal weakness: 68.2%; p < 0.0001), and an intermediate prognosis. Raynaud phenomenon, arthralgia/arthritis, and sex permit the cluster appurtenance (83.3% correct estimation). Nevertheless, an unsupervised analysis confirmed that anti-MDA5 antibody delineates an independent group of patients (e.g., dermatomyositis skin rash, skin ulcers, calcinosis, mechanic's hands, ILD, arthralgia/arthritis, and high mortality rate) distinct from anti-MDA5− patients with myositis. Conclusion Anti-MDA5+ patients have a systemic syndrome distinct from other patients with myositis. Three subgroups with different prognosis exist.

Abstract: Indexation en cours.; International audience; PURPOSE: Contact lens (CL)-related microbial keratitis (MK) has major public health implications, with about 300 million wearers worldwide, and certain potentially modifiable risk factors. This study aimed to identify the risk factors of CL-related MK. METHODS: A multicenter case-control study was conducted between 2014 and 2017. Cases presenting with CL-related MK were submitted to an anonymous 52-item questionnaire, which was also completed by healthy controls. Univariate followed by multivariate logistic regression analysis was performed. Risk factors for CL-related MK were given as odds ratio (OR) with 95% confidence interval and P-value. RESULTS: The study included a total of 2267 patients (1198 cases and 1069 controls). The MK risk factors for the daily disposable lenses group were exceeding the lens renewal period (OR = 9.16, P = 0.008) and occasionally wearing CL when sleeping (OR = 15.83, P = 0.035). The most important risk factors in the nondaily disposable lenses group were lens cleaning solution distributed by eye care brands (OR = 3.50, P < 0.001) and failure to renew lens cases (OR = 3.39, P = 0.001). Statistically and clinically significant variables were used to establish the MK risk equation for CL wearers, allowing an individual calculation of the risk of MK under lenses. CONCLUSIONS: The MK risk equation is a valuable tool for educating patients about the risks associated with wearing CL. It allows the patient to be informed about their overall risk of infection while detailing the precipitating elements of the infectious risk with the aim of modifying risk behavior.

Abstract: International audience; PURPOSE:To report the incidence of acute postoperative endophthalmitis (POE) after macular surgery in France between 2006 and 2016 and to identify associated factors.METHODS:This retrospective database study included all hospital discharge records involving a surgical procedure for an epiretinal membrane or a macular hole in France from January 2006 to October 2016. Acute POE was identified by two codes in the tenth edition of the International Classification of Diseases within 42 days of a macular surgical procedure in the French national administrative database.RESULTS:In France, 152 034 macular surgical procedures for epiretinal membranes or macular holes were recorded from 1 January 2006 to 31 October 2016. Suspected acute POE was reported in 381 cases. The incidence of POE was 0.25% overall, 0.30% for epiretinal membrane surgery and 0.14% for macular hole surgery. In multivariable Poisson regression analysis, epiretinal membrane surgery was associated with POE [incidence rate ratio (IRR), 2.24; 95% CI, 1.62-3.11; p < 0.001]. For epiretinal membrane surgery, the 2010-2011 period was significantly associated with a higher risk of POE (IRR, 1.66; 95% CI, 1.13-2.42; p = 0.03).CONCLUSION:The incidence of POE after macular surgery was 0.25% overall in France between 2006 and 2016 and twice higher for epiretinal membrane surgery than for macular hole surgery. For epiretinal surgery only, the incidence of POE was higher in 2010-2011 (period of the switch to transconjunctival vitrectomy) than in the rest of the study period.

Abstract: Introduction: The rate of unknown glaucoma is around 50% in industrialized countries. The purpose of our study was to estimate the prevalence of unknown cases of ocular hypertension, glaucoma suspects, and glaucoma in patients consulting for refractive disorders in France. Methods: A retrospective study in the Point Vision ophthalmology center was led in Toulouse, France. All participants consulting for refractive disorders between June 2015 and June 2017 in the ophthalmology center were included. The cases were identified by the assessment of intraocular pressure, optic nerve head structure, and visual field. Ocular hypertension was defined as an intraocular pressure >21 mm Hg. Glaucoma was defined as the association of a glaucomatous papilla and two successive pathological visual fields. Glaucoma suspect was defined as the association of a glaucomatous papilla without visual field defect. The primary endpoint was the prevalence of unknown ocular hypertension, glaucoma suspects, and glaucoma in patients seen in an ophthalmology center. Results: A total of 66,068 patients (mean age = 37 years) consulted for a refraction visual assessment during the study period. Among them, 234 had a visual field and a retinal nerve fiber layer assessment for ocular hypertension and/or suspicious papilla. The prevalence of unknown cases of ocular hypertension, glaucoma suspect, and glaucoma was 2.6, 0.8, and 0.5 per 1,000 consultants, respectively. Median age at diagnosis of ocular hypertension, glaucoma suspect, and glaucoma was 52, 53, and 65 years, respectively. Conclusion: The present study highlights the importance of glaucoma screening in people over 40 years old with the measurement of intraocular pressure and an optic nerve head assessment.

Abstract: International audience; Purpose : Although rare, endophthalmitis is a severe complication of ophthalmological procedures. The incidence of endophthalmitis following cataract, glaucoma, vitreoretinal surgeries, and intravitreal injections has previously been described in large-scale studies, but no data is available on the proportion of causative procedures. We aimed to describe longitudinally the causes of endophthalmitis at a national scale using the French medico-administrative database.Methods : The SNIIRAM (French national inter-scheme insurance database) collects all data of medical expenditures and procedures performed in France. We extracted acute endophthalmitis occurring 42 days after an ophthalmological procedure, by means of hospital discharge code (CIM10 H440, H441) and procedures codes from 2009 to 2018.Results : From 2009 to 2018, 7 776 acute endophthalmitis followed ophthalmological procedures, in 7 754 patients. Age at diagnosis was 71.7 ± 12.9 years) and 51.02% of the patients were women. Most endophthalmitis were caused by non-combined cataract surgery (4 957 cases, 63.75%), followed by IVTs (1 392, 17.90%), vitreoretinal surgery (761, 9.79%), anterior segment surgery (226, 2.91%), combined cataract and vitreoretinal surgery (154, 1.98%), cornea surgery (131, 1.68%), glaucoma surgery (84, 1.08%). The frequency of causative procedures remained stable over the years for all procedures except for cataract surgery which decreased from 2009 (763, 79.98% of the year’s etiologies) to 2018 (432, 51.92%) with a rapid decrease in 2012, and for IVT which increased in the same time period (from 70, 7.34% to 258, 31.01%). Patients with acute endophthalmitis following IVTs were the oldest (75.2 ± 12.3 years old), Endophthalmitis following Intravitreal procedures, IVTs, and vitreoretinal surgeries occurred sooner than after other procedures (median (IQR); 4.0 days (3.0-7.0) and 4.0 (2.0-8.0), respectively) versus 6.0 days (4.0-11.0) days in cataract surgery.Conclusions : We observe that in more than 1.3 million ophthalmological procedures per year in France, the occurrence of acute endophthalmitis is low. This data shows a shift in causative factors with a decrease of endophthalmitis following cataract surgery and an increasing number after IVT.

Abstract: International audience; MAIN OBJECTIVE: To prospectively assess the cost-consequence of a standardized diagnostic strategy as to compared to an open one for the etiological diagnosis of uveitis. DESIGN: This was a prospective, non-inferiority, multicentre, randomized controlled trial. METHODS: We included all consecutive patients with uveitis who had visited at least one of the Departments of Ophthalmology. In the standardized group, patients had a minimal work-up regardless of the type of uveitis (including evaluation of the CBC, ESR, C-reactive protein, tuberculin skin test, syphilis serology and chest X-ray). Depending on ophthalmological findings, further investigations could be performed. In the open strategy, ophthalmologists were free to order any kind of investigation. The main outcome was the mean cost per patient of each strategy. RESULTS: 903 uveitis patients were included from January, 2010 to May, 2013. The mean cost per patient of the standardized strategy was 182.97 euros [CI 95% (173.14; 192.80)], and the mean cost per patient of the open strategy was 251.75 euros [CI 95% (229.24; 274.25)]. Abstract: International audience; PURPOSE: To characterize the phenotype of retinal vessels using central retinal artery equivalent (CRAE), central retinal vein equivalent (CRVE), tortuosity and fractal dimension (FD) in primary open-angle glaucoma (POAG) subjects. METHODS: This prospective case-control multicentre study included 61 POAG subjects and 61 controls matched for age, systemic hypertension and body mass index. Fundus images of the right eye were acquired using a non-mydriatic camera. Central retinal artery equivalent (CRAE), CRVE, arteriole-to-venule ratio, FD and tortuosity of the vascular network were measured using VAMPIRE software (Vessel Assessment and Measurement Platform for Images of the Retina). Primary open-angle glaucoma (POAG) patients underwent 24.2 sita-standard visual field and peri-papillary optical coherence tomography (OCT) examinations. Data were expressed as median and interquartile range (75-25th percentiles). RESULTS: The control group was comparable to the POAG group for sex ratio, refraction and intraocular pressure. The mean CRAE and the mean CRVE were significantly lower in the POAG group than in the control group [150.5 (137.9; 157.1) mum versus 161.3 (154.0; 168.4) mum and 204.8 (190.1; 218.1) mum versus 233.5 (222.3; 246.9) mum, respectively; p < 0.001] and for fractal parameters as well. No significant difference was found for tortuosity between the two groups. There was a significant correlation between CRAE and retinal nerve fibre layer (RNFL) thickness (r = 0.27; p = 0.03). VAMPIRE parameters were not correlated with visual field indices. CONCLUSION: Primary open-angle glaucoma (POAG) was associated with a narrowing of arterial and venous retinal vessels, a higher arteriole-to-venule ratio and lower values of FD. The relationship between CRAE and RNFL thickness needs further investigation.

Abstract: La neoglucogenese intestinale (NGI) est une fonction cle dans le maintien de l’homeostasie energetique. Chez la souris, son induction par un regime riche en fibres diminue le poids et la masse grasse, suggerant sa capacite a moduler le metabolisme tissu adipeux (TA). Nous proposons que la NGI serait capable de proteger des alterations du metabolisme du TA induites par un regime high-fat/high-sucrose (HF/HS). Nous avons etudie l’adiposite, la thermogenese et la production d’adipokines de souris qui sur-expriment dans l’intestin le gene codant pour la glucose-6-phosphatase (souris I.G6pcsurexp), l’enzyme cle de la production endogene de glucose. L’induction de la NGI diminue le poids et la masse grasse, sans modifier la prise alimentaire et les lipides circulants. Les souris I.G6pcsurexp presentent une elevation de la temperature rectale et de la chaleur produite par le TA brun, associee a des augmentations de l’expression des marqueurs de thermogenese, de la taille des mitochondries et de leur capacite respiratoire. Dans le TA blanc, nous montrons une augmentation de l’expression des marqueurs de l’oxydation des acides gras, de la thermogenese et du recepteur adrenergique-β3, suggerant une activation du systeme nerveux sympathique. La NGI ameliore le profil de secretion des adipokines en diminuant les adipokines pro-inflammatoires et en augmentant la forme de haut-poids moleculaire de l’adiponectine, qui exerce de nombreux effets metaboliques benefiques. Ainsi, la NGI protege le TA des modifications metaboliques deleteres induites par un regime HF/HS, en limitant son expansion, en activant la thermogenese et en ameliorant le profil de secretion des adipokines.

Abstract: Neutrino detectors participate in the indirect search for the fundamental constituents of dark matter (DM) in form of weakly interacting massive particles (WIMPs). In WIMP scenarios, candidate DM particles can pair-annihilate into Standard Model products, yielding considerable fluxes of high-energy neutrinos. A detector like ANTARES, located in the Northern Hemisphere, is able to perform a complementary search looking towards the Galactic Centre, where a high density of dark matter is thought to accumulate. Both this directional information and the spectral features of annihilating DM pairs are entered into an unbinned likelihood method to scan the data set in search for DM-like signals in ANTARES data. Results obtained upon unblinding 3170 days of data reconstructed with updated methods are presented, which provides a larger, and more accurate, data set than a previously published result using 2101 days. Abstract: International audience; INTRODUCTION: To describe the one-year functional outcomes of treatment-naive neovascular age-related macular degeneration (nAMD) treated with anti-VEGF agents at the Dijon University Hospital Ophthalmology Department. METHODS: Real-life interventional study including all treatment-naive nAMD patients from January 2016 to December 2018 in the Ophthalmology Department of Dijon University Hospital. Data were retrospectively collected from the Fight Retinal Blindness! (FRB!) registry. At baseline, medical history, visual acuity (VA), type of lesion and activity on angiography and optical coherence tomography (OCT), and treatment were recorded. On follow-up, VA, lesion activity and treatment were recorded. RESULTS: Three-hundred twenty eyes of 259 patients were included, of which 65.6% were female and with a mean age of 80.1+/-11.1 years. Mean VA (standard deviation, SD) at baseline was 53.2 ETDRS letters (25.3). All patients received anti-VEGF injections, of which 164 eyes (51.2%), 152 eyes (47.5%) and 4 eyes (1.2%) were treated with aflibercept, ranibizumab and bevacizumab, respectively. A total of 198 eyes of 169 patients completed the 12-month follow-up, with a median (first quartile, third quartile) of 12 visits (10, 13). At one year (n=198), the overall mean VA gain [95% CI] was +3.3 ETDRS letters [0.7, 5.9] and 173 (87.4%) of the treated eyes did not lose 15 or more letters. We found no statistically significant difference in mean VA gain between aflibercept and ranibizumab. CONCLUSION: This real-world study confirmed the efficacy of anti-VEGF agents in nAMD and the feasibility of analyzing data in an international registry.

Abstract: BackgroundWithin a surveillance of the prevalence and causes of vision impairment in high-income regions and Central/Eastern Europe, we update figures through 2015 and forecast expected values in 2020.MethodsBased on a systematic review of medical literature, prevalence of blindness, moderate and severe vision impairment (MSVI), mild vision impairment and presbyopia was estimated for 1990, 2010, 2015, and 2020.ResultsAge-standardised prevalence of blindness and MSVI for all ages decreased from 1990 to 2015 from 0.26% (0.10–0.46) to 0.15% (0.06–0.26) and from 1.74% (0.76–2.94) to 1.27% (0.55–2.17), respectively. In 2015, the number of individuals affected by blindness, MSVI and mild vision impairment ranged from 70 000, 630 000 and 610 000, respectively, in Australasia to 980 000, 7.46 million and 7.25 million, respectively, in North America and 1.16 million, 9.61 million and 9.47 million, respectively, in Western Europe. In 2015, cataract was the most common cause for blindness, followed by age-related macular degeneration (AMD), glaucoma, uncorrected refractive error, diabetic retinopathy and cornea-related disorders, with declining burden from cataract and AMD over time. Uncorrected refractive error was the leading cause of MSVI.ConclusionsWhile continuing to advance control of cataract and AMD as the leading causes of blindness remains a high priority, overcoming barriers to uptake of refractive error services would address approximately half of the MSVI burden. New data on burden of presbyopia identify this entity as an important public health problem in this population. Additional research on better treatments, better implementation with existing tools and ongoing surveillance of the problem is needed.

Abstract: (abridged) The far-UV wavelength range (912-2000A) provides access to atomic and molecular transitions of many species the interstellar medium (ISM), circumgalactic medium (CGM), and intergalactic medium, within phases spanning a wide range of ionization, density, temperature, and molecular gas fraction. Far-UV space telescopes have enabled detailed studies of the ISM in the Milky Way thanks to absorption features appearing in the UV spectra of hot stars and yielding fundamental insights into the composition and physical characteristics of all phases of the ISM along with the processes that influence them. However, we have yet to design a spectrometer able to observe the full UV domain at resolving power R>10^5 with a signal-to-noise ratio SNR>500. Such a resolution is necessary to resolve lines from both the cold molecular hydrogen and the warm metal ions with a turbulent velocity of about 1 km s-1, and to differentiate distinct velocity components. Future UV spectroscopic studies of the Milky Way ISM must revolutionize our understanding of the ISM as a dynamical, unstable, and magnetized medium, and rise to the challenge brought forward by current theories. Another interesting prospect is to transpose the same level of details that has been reached for the Milky Way to the ISM in external galaxies, in particular in metal-poor galaxies, where the ISM chemical composition, physical conditions, and topology change dramatically, with significant consequences on the star-formation properties. Finally, we need to be able to perform statistical analyses of background quasar lines of sight intersecting the CGM of galaxies at various redshifts and to comprehend the role of gas exchanges and flows for galaxy evolution., This ESA white paper draws from and expands on the following white papers previously submitted for the Astro 2020 Decadal Survey, Gry et al.
