6. Intoxication au plomb par polycriblage de particules de plomb : étude de cas

Abstract

Objectif L’objectif etait de discuter les schemas therapeutiques de chelation du plomb en cas d’exposition chronique suite a un criblage par particules de plomb. Contexte Mme E.Z., et sa fille Mme G.R., ont ete criblees de plomb en septembre 2018 apres un accident domestique dans le cadre de pratiques culturelles d’exorcisme consistant en l’utilisation de vapeurs de plomb. Un contenant rempli de plomb a ete pose sur une plaque chauffante, causant sa deflagration. Mme E.Z. a recu de tres nombreuses particules de plomb au contact des articulations thoraciques mais aussi de maniere tres profonde, notamment jusqu’a la plevre. La premiere plombemie mesuree etait de 411 μg/L apres l’accident. Mme E.Z. a beneficie 2 chelations par EDTA IV puis elle a eu 4 protocoles de chelation par succimer (DMSA) 600 mg 3 fois par jour, ce qui represente 16 semaines de chelation discontinue, avec chaque semaine 5 jours de traitements, soit 9000 mg par semaine, et un total de 144 000 mg de DMSA sur 12 mois, afin de maintenir la plombemie en dessous de 200 μg/L, ce qui n’a pas ete obtenu malgre ce traitement. Il n’a pas ete possible d’extraire les eclats de plombs en contact avec les sereuses ni avec les articulations. La derniere plombemie mesuree en janvier 2020 est de 387 μg/L. En janvier 2020 il a ete propose a la patiente de suivre un nouveau protocole de 6 semaines de chelation discontinue par DMSA 600 mg 3 fois par jour, soit un total de 54 000 mg de DMSA. Sa fille Mme G.R n’a pas necessite de chelation mais a beneficie de chirurgie pour extraire certains eclats de plomb, notamment situe pres des articulations des mains. Il persiste malgre tout de nombreux eclats au niveau abdominal et surtout thoracique. La premiere plombemie mesuree en octobre 2018 etait de 417 μg/L, et la derniere plombemie mesuree en septembre 2019 est de 244 μg/L. La question d’une chelation au long cours se pose devant l’impossibilite d’une chirurgie d’exerese du plomb, et devant la persistance d’une plombemie de ce niveau. Discussion Le succimer est utilise per os pour le traitement des intoxications par le plomb et le mercure voire pour l’arsenic. Le succimer augmente l’elimination urinaire des metaux lourds, en formant des complexes stables hydrosolubles. Il n’a pas ete retrouve dans la litterature medicale de protocole de chelation pour les intoxications chroniques au plomb pour lesquelles la source de plomb ne peut pas etre eliminee (cas du polycriblage). Un premier article datant de 2007 [1] a etudie l’effet a long terme de la chelation sur 48 mois mais les patients n’ont recu au maximum que 4 semaines de chelation. Une etude de cas de 2011 [2] rapporte l’utilisation de succimer chez des patients ayant recu des balles en plomb qui n’ont pas pu etre retirees. Le 1er cas a ete traite initialement par succimer 500 mg 2 fois par jour pendant 2 semaines, puis succimer 100 mg 2 fois par jour pendant 1 mois, puis a la dose de 100 mg 1 fois par jour sur une periode de 24 mois en continu, soit un total de 42 000 mg de DMSA sur 25,5 mois, permettant de faire baisser la plombemie initiale de 230 μg/L a 150 μg/L. Des symptomes a type de nausees et asthenie ont persiste, sans qu’il n’ait ete possible d’imputer la cause au traitement par succimer, a la plombemie, ou a une autre etiologie. Le second cas a ete traite initialement par succimer 1000 mg 2 fois par jour pendant 2 semaines, puis 100 mg 2 fois par jour pendant 1 mois, puis par succimer a 100 mg 3 fois par jour sur une periode d’un an en continu, soit un total de 102 000 mg de DMSA sur 13,5 mois permettant une baisse de la plombemie initiale de 290 μg/L a 90 μg/L. Il n’y a pas eu de symptomes rapportes. Conclusion La chelation sur une longue duree a donne de bons resultats dans ces deux observations, mais les modalites exactes d’utilisation du succimer au long cours n’ont pas ete formalisees dans la litterature.

7. Devenir à l’âge de 3ans d’une cohorte d’enfants nés à moins de 26 semaines d’aménorrhée

Abstract

Resume Introduction Les progres de la medecine perinatale ont permis la survie de nouveau-nes de plus en plus immatures. Cependant, l’extreme prematurite est a haut risque d’anomalies du developpement. Nous avons etudie le devenir neurocognitif a 3 ans d’une cohorte d’enfants nes avant 26 semaines d’amenorrhee (SA). Methode Il s’est agi d’une etude observationnelle descriptive d’une cohorte d’enfants nes entre 23 + 0 SA et 25 + 6 SA, dans la region Provence-Alpes-Cote-d’Azur (PACA) Ouest–Corse du Sud, entre 2007 et 2011. Les donnees du suivi du reseau d’aval « Naitre et devenir » ont permis de classer les enfants en trois groupes : absence de deficience, deficience intermediaire et deficience severe. A l’aide d’une regression logistique, nous avons recherche les facteurs perinatals predictifs d’un devenir favorable a 3 ans. Resultats Cent soixante-deux enfants avaient ete admis en neonatologie. Parmi les 101 survivants (62 %), 66 avaient effectue leur visite a 3 ans. Le taux de survie augmentait avec l’âge gestationnel : 33 % a 23 SA, 57 % a 24 SA et 68 % a 25 SA. Les caracteristiques perinatales des perdus de vue etaient comparables a celles des enfants suivis. Aucune deficience n’a ete depistee chez 56 % et une deficience severe etait presente chez 6 %. Une paralysie cerebrale etait presente chez 13 % des enfants. La faible duree de ventilation mecanique (odd ratio [OR] = 0,96 [0,93–0,99] ; p = 0,03) et la presence d’une corticotherapie antenatale complete (OR = 4,7 [1–7,7–17] ; p = 0,02) etaient associees a l’absence de deficience a 3 ans. Conclusion Ameliorer la prise en charge perinatale de ces nouveau-nes reste un enjeu majeur des equipes soignantes. L’evaluation neurocognitive doit se poursuivre jusqu’a la petite enfance afin de depister les troubles des apprentissages.

8. Radiothérapie par modulation d’intensité des cancers du sein sur Halcyon : résultats préliminaires

Abstract

Objectif de l’etude Nous avons revu les donnees cliniques et dosimetriques de nos premiers patients traites avec modulation d’intensite sur HalcyonTM pour un carcinome mammaire localise ou localement avance. Materiel et methode Trente patientes ont ete prises en charge dans notre centre pour un carcinome mammaire localise (n = 21) ou localement evolue (n = 9). L’âge median etait de 67,5 ans. La lateralite de l’irradiation etait droite (n = 14), gauche (n = 16). Le traitement a ete realise avec modulation d’intensite en utilisant des photons 6 MV sans filtre compensateur (FFF) sur HalcyonTM (Varian) soit en arctherapie ou en radiotherapie conformationnelle avec modulation d’intensite (RCMI) par faisceaux dynamiques modules ou technique hybride RCMI/arctherapie. La planification dosimetrique a ete effectuee sur Eclipse v15.5. Le repositionnement a ete realise par imagerie planaire de haute energie ou conique de haute energie, dont la contribution de dose a ete incluse dans parametres dosimetriques. Resultats Le temps d’irradiation moyen etait de 50 s. La toxicite aigue a ete marquee par une radiodermite de grade 1 (n = 24) ou 2 (n = 6), une œsophagite de grade 1 (n = 5). Pour les cancers du sein localises, les volumes medians pulmonaires homolateraux V20 Gy, V5 Gy (V × Gy : volume recevant × Gy) etaient respectivement de 8,8 % [1,8–15 %], 24 % [14–28,8 %]. Les doses moyennes medianes dans le poumon homolateral et le cœur etaient respectivement de 6,36 Gy [3,8–8,3 Gy] et 1,9 Gy [1–4,2 Gy]. Pour les cancers du sein localement evolues, les volumes medians pulmonaires homolateraux V20 Gy, V5 Gy etaient respectivement de 22 % [7,5–28 %] et 66 % [22,8–89 %]. Les doses moyennes medianes dans le poumon homolateral et au cœur etaient respectivement de 12,5 Gy [5,9–15,4 Gy] et 1,9 Gy [1,3–6,2 Gy]. Conclusion La radiotherapie avec modulation d’intensite des carcinomes mammaires sous HalcyonTM permet d’obtenir un gain dosimetrique et de temps d’irradiation comparativement aux techniques classiques, notamment pour des balistiques complexes sans augmenter la toxicite aigue precoce. La comptabilisation de la dose delivree par l’imagerie dans les parametres dosimetriques finaux semble un atout.

9. Gestion des changements morphologiques lors des traitements avec modulation d’intensité pour les cancers du sein sur Halcyon™ guidés par l’imagerie conique quotidienne de basse ou de haute énergie

Abstract

Resume Introduction et but de l’etude Les modalites de guidage par imagerie de la radiotherapie mammaire ne sont pas consensuelles. Nous avons evalue nos pratiques et l’influence des changements morphologiques du sein pendant la radiotherapie sur Halcyon™. Materiel et methodes Parmi plus de 1000 patientes traitees, 128 dossiers ont ete tires au sort et analyses retrospectivement. Il s’agissait d’une radiotherapie conformationnelle avec modulation d’intensite (RCMI, n = 104) ou d’une arctherapie volumetrique avec modulation d’intensite (n = 24) en utilisant respectivement trois a quatre faisceaux modules ou trois a cinq arcs partiels de photons de 6 MV en mode sans egalisateur de filtre (flattening filter-free, FFF). L’optimisation dosimetrique utilisait un bolus virtuel de 1,5 cm en arctherapie volumetrique modulee ou une extension de fluence (skin flash) de 1,5 cm en RCMI afin de tenir compte notamment des mouvements respiratoires. Avant chaque seance, une tomographie conique de basse ou de haute energie a ete realisee. Une scanographie dosimetrique a ete effectuee avant et a mi-parcours du traitement, voire plus precocement selon les modifications anatomiques. Les changements morphologiques etaient notes et rapportes aux medecins, physiciens par les manipulateurs d’electroradiologie medicale. Le cas echeant, un recalage semiautomatique rigide suivi d’une deformation elastique et verification medicale des contours etaient realises. En cas de modification majeure, une nouvelle dosimetrie etait recalculee. Resultats et analyse statistique Chez 22 patientes il y a eu une modification morphologique, avec un œdeme radio-induit (n = 6), une diminution de lymphocele/œdeme (n = 16). Hormis les difficultes de repositionnement, les modifications anatomiques induisaient un impact dosimetrique faible sur la couverture de la glande mammaire avec des zones de surdosage ou de sous-dosage des volumes cibles du sein/de la paroi de moins de 3 % (doses maximales, moyennes et minimales). L’impact dosimetrique sur les aires ganglionnaires et les organes a risque etait negligeable. Conclusion Cette etude valide nos pratiques de controle quotidienne par tomographie conique et confirme la robustesse de nos techniques d’optimisation dosimetrique. Une etude est en cours pour confirmer ces resultats et estimer precisement l’impact dosimetrique quotidien a partir de la tomographie conique ainsi que l’apport du repositionnement surfacique.

Abstract

Introduction A new Halcyon linear accelerator had been installed at clinique Charcot in Sainte-Foy-Les-Lyon. It’s the first one in France. The accelerator head is mounted on a ring gantry. There is no jaws and, for each bank, two MLC (proximal and distal) of 29 and 28 leaves of 1 cm width at isocenter with an offset of 5 mm to minimize the transmission. The maximum speed of the leaves is 5 cm/s. The maximum field size is 28 × 28 cm2. Halcyon system has one unique energy of 6XFFF and a maximum dose rate of 800 UM/min. The MV imaging panel size is 43 × 43 cm2, fxed at a distance source to imager of 154 cm and its resolution is 0.22 mm at isocenter. Currently, Halcyon system has only MV imaging (MV/MV or MV CBCT). Imaging dose is calculated by TPS Eclipse (Varian) and taken into account during optimization. Beam model of Halcyon is already preinstalled and fully modelled in Eclipse. Methods All the preinstalled data in Eclipse were measured in a water tank Blue Phantom2 (IBA) using CC13 (IBA), EDGE (SunNuclear) et Farmer (PTW) detectors in accordance with Varian measurements. PDD, profiles, FOC, transmission factor and DLG measurements were compared with preinstalled beam model. Calculated dose distribution with AAA algorithm for PDD and profiles were compared with our measurements using tolerances from SFPM report n°27. End-to-end and RapidArc commissioning were performed. Imaging dose from clinical radiotherapy plans were calculated from several anatomical regions and measured in an anthropomorphic phantom CIRS with a semiflex (PTW) ionisation chamber. Results A very good superposition of preinstalled data with our measurements was observed for every data. Largest variations were observed for small field profiles 2 × 2 cm2. Mean and maximum variations for FOC measurements were −0.03% and 0.71%. Leaf transmission measured through both banks is 0.005%. Beam quality index (PDD(10) with SSD = 100 cm) is 63.5% (Varian specification 63% ± 1). Maximum depth dose for a 10x10cm2 field size at SSD = 100 cm is 1.28 cm (Varian specification 1.3 cm ± 0.2). For every field sizes 100% of the points for PDD and profiles pass the tolerances from SFPM report n°27. Gantry isocentricity measured with Drum phantom (Varian) is 0.65 mm. Absolute dose difference at isocenter for a VMAT plan measured during an End-to-End test performed in the IMRT Thorax Phantom (CIRS) is 1%. Conclusions An excellent agreement between Varian preinstalled beam model and our measures were observed and therefore no changes to the preinstalled beam model have been made.

Abstract

The gastric mucosa and the mucosa of the right and left dorsal colon were biopsied in each of the 15 horses, and a total of 45 samples were collected. Mucosal samples were cultured using a Lactobacillus enrichment broth. While numerous Lactobacillus strains were identified, Lactobacillus reuteri was the most common organism identified. Sixteen strains of Lactobacillus reuteri were selected for antimicrobial testing. Salmonella antimicrobial activity was identified in six out of 16 strains tested. Organisms with Salmonella antimicrobial activity were cultured from the stratified squamous epithelium of the stomach and the mucosa of the right and left dorsal colon.

Abstract

A sample of 4148 four-jet events observed in the ALEPH-detector at LEP in 1989 and 1990 is used to test the underlying gauge group of strong interactions. A fit to the ratios of the "colour factors" C(F), N(C) and T(F), which determine the differential cross sections, yields N(C)/C(F) = 2.24 +/- 0.32stat +/- 0.24syst and T(F)/C(F) = 0.58 +/- 0.17stat +/- 0.23syst. This is in agreement with the values expected from QCD: N(C)/C(F) = 2.25 and T(F)/C(F) = 0.375. The non-zero value of N(C)/C(F) constitutes direct evidence for the existence of the triple-gluon coupling and excludes any abelian gauge theory by more than five standard deviations.

Abstract

From more than 175 000 hadronic Z decays observed with the ALEPH detector at LEP, we select 823 events with pairs of leptons in the final state. From these we measure χ , the probability thata b hadron which is observed to decay originated as a b hadron. We find χ =0.132 −0.026 +0.027 .

Abstract

The average lifetime of B hadrons has been measured by the ALEPH experiment at LEP. Events containing B hadrons are selected by the identification of leptons with high transverse momentum in hadronic Z decays, and the lifetime is extracted from a fit to the impact parameter distribution of the lepton tracks. From a sample of 1.7×105 hadronic Z decays a lifetime of 1.29±0.06±0.10 ps is measured.

Resume Cette etude porte sur les projets de restructuration des services de sterilisation de l'hopital Haut-Leveque du CHU de Bordeaux. Elle avait comme objectif la mise en conformite au sens de la circulaire du 20 octobre 1997 concernant la sterilisation par le biais d'une centralisation des moyens de lavage des instruments, au niveau d'une sterilisation centrale. La demarche s'est articulee autour de trois phases. Tout d'abord, nous avons etabli un bilan de l'existant et, plus particulierement, un bilan de l'activite afin d'estimer les volumes traites et leur exploitation (etudes de charge). Ensuite, nous avons mis en place la methodologie d'Analyse Preliminaire des Risques qui a permis de reveler les risques du process pour le patient et pour le personnel ainsi que les defaillances de ce processus. A ce stade, la conformite par rapport aux Bonnes Pratiques de Sterilisation a egalement eteevaluee. Enfin, pour construire le projet de restructuration a Bordeaux, nous avons du prendre en compte plusieurs parametres qui doivent obeir a des regles specifiques pour un service de sterilisation.

17. Résistance aux œstrogènes due à une nouvelle mutation homozygote d’ESR1

Abstract

Une resistance aux œstrogenes liee a une mutation du recepteur α des œstrogenes (ERα) a ete decrite a ce jour uniquement chez deux individus, un homme en 1994 (Smith, NEJM) et une femme en 2013 (Quaynor, NEJM). Nous presentons deux sœurs de 24 et 19 ans, issues d’une famille consanguine. Elles ont consulte pour une amenorrhee primaire avec un impuberisme. Le bilan hormonal du cas index a montre a quatre reprises des taux de 17β-estradiol fluctuants entre 8000 et 9400 pmol/L (N : 200–500), avec une FSH et une LH respectivement a 13,3 UI/L (N : 2–10) et 24,1 UI/L (N : 2–10). L’echographie pelvienne a mis en evidence des kystes ovariens bilateraux. Le caryotype des deux sœurs est 46,XX. L’analyse genetique a identifie une substitution nucleotidique homozygote (c.1181G>A) dans le 5e exon codant du gene ESR1. Les parents et une sœur indemne sont heterozygotes pour cette mutation. Cette variation entraine la mutation faux-sens p.Arg394His qui affecte un acide amine essentiel pour la liaison de l’estradiol a son recepteur ERα. Elle est localisee dans l’helice H5 du domaine de liaison a l’hormone. Les analyses fonctionnelles demontrent une expression normale du recepteur mute, mais confirment une baisse d’environ 100 fois de l’affinite de l’estradiol pour le mutant R394H. Aucun ligand œstrogenique agoniste (Ethinyl-estradiol, DES) ou SERM (Clomiphene, Raloxifene, Tamoxifene) ne restaure le defaut d’activite du recepteur mute in vitro. Cette etude a permis d’identifier et de caracteriser la premiere mutation familiale perte de fonction du gene ESR1.

Resume Les para-osteo-arthropathies sont des complications classiques des affections neurologiques centrales, surtout dans les contextes traumatiques. Elles surviennent principalement au voisinage des grosses articulations. Leur physiopathologie exacte reste inconnue malgre de tres nombreux travaux et cas rapportes. Il semble que leur survenue au decours d'affections neurologiques peripheriques et que leur localisation a proximite de petites articulations soient exceptionnelles. Nous presentons le cas de para-osteo-arthropathies des deux mains compliquant un syndrome de Guillain-Barre.

Abstract

We investigate neutral pion electroproduction off protons in the framework of heavy baryon chiral perturbation theory. The chiral expansion of the S–wave multipoles E0+ and L0+ is carried out to three orders. There appear several undetermined low– energy constants. Three are taken from a recent study of the new TAPS threshold � 0 photoproduction data, one is fixed from the proton Dirac form factor and the novel S–wave constants appearing at orders q 4 and q 5 are determined from a best fit (constrained by resonance exchange) to the recent NIKHEF and MAMI data at photon momentum transfer squared k 2 = −0.1 GeV 2 . The inclusion of a particular set of dimension five operators is forced upon the fit by a soft–pion theorem which severely constrains the momentum dependence of the longitudinal S–wave multipole L0+ at order q 4 . We give predictions for lower photon virtualities for the various multipoles and differential cross sections. Further improvements are briefly touched upon.

20. Comparaison du phénotype gonadotrope chez 265 hommes avec hypogonadisme hypogonadotrophique congénital (HHC) et 404 avec hypogonadisme hypogonadotrophique acquis (HHA) : étude monocentrique

Contexte L’HHC est caracterise par un deficit gonadotrope present depuis la vie fœtale, alors que l’HHA apparait le plus souvent apres la puberte. La fonction gonadotrope dans ces deux entites nosologiques n’a jamais ete comparee sur des grandes series. Patients Deux cent soixante-cinq HHC et 404 HHA ont ete evalues. Les volumes testiculaires (VT) et les concentrations de gonadotrophines, steroides sexuels, inhibine B (IB), AMH et INSL3 ont ete mesures. Resultats Le VT etait superieur chez les HHA (16,7 ± 6 mL) que chez les HHC (3,1 ± 1,9 mL, p Conclusion Les hommes avec HHA ont un deficit gonadotrope moins severe que les HHC. La conservation de l’activation de l’axe gonadotrope pendant la periode fœtale, neonatale et pubertaire dans les HHA rend probablement compte de ces differences. L’atteinte gonadotrope chez les patients avec craniopharyngiome est plus profonde que dans les autres causes de HHA.

We consider the chiral expansion for the reaction $\pi N \to \pi \pi N$ in heavy baryon chiral perturbation theory. To order $M_\pi$ we derive novel low--energy theorems that compare favorably with recent determinations of the total cross sections for $\pi^+ p \to \pi^+ \pi^+ n$ and $\pi^- p \to \pi^0 \pi^0 n$.

23. R-spondin 2, un acteur de la physiologie ovarienne : recherche de variants dans une cohorte de patientes atteintes d’insuffisance ovarienne primaire

La folliculogenese est un phenomene complexe continu, marque par une succession d’etapes mal comprises, un defaut de l’une d’entre elles peut conduire a l’Insuffisance Ovarienne Primaire (IOP). Une des multiples causes d’IOP est l’identification de mutations du gene codant Newborn Ovary Homeobox (NOBOX). Son invalidation chez la souris conduit a une infertilite due a la repression de plusieurs genes de la folliculogenese dont le ligand R-spondin 2 (Rspo2). Rspo2 active la voie Wnt et joue un role cle dans la maturation ovocytaire. Les souris Rspo2–/– meurent a la naissance mais les Rspo2+/– deviennent infertiles vers l’âge de 4 mois representant un bon modele d’IOP. Nous avons identifie in silico trois elements putatifs de liaison de NOBOX dans le promoteur humain de RSPO2 que nous avons clone. Afin de valider RSPO2 comme une nouvelle cible de NOBOX des essais transcriptionnels sont en cours. Par ailleurs, nous avons recherche des mutations de RSPO2 dans une cohorte de 108 patientes presentant une IOP idiopathique, sans premutation de FMR1 ou d’anomalie du caryotype. Seuls des polymorphismes connus ont ete identifies et retrouves dans les cohortes controles du 1000 Genomes et de l’Exome Variant Server a des frequences elevees. Malgre l’haploinsuffisance de Rspo2 responsable de l’infertilite chez la souris, nos donnees suggerent que les mutations de ce facteur ne sont pas une cause frequente de la pathologie humaine. Il serait souhaitable de poursuivre la recherche de mutations potentielles de ce gene sur de plus grandes cohortes.

Introduction Les prolactinomes sont les tumeurs hypophysaires secretantes les plus frequentes. A ce jour, aucun gene de predisposition specifique a ces tumeurs n’a ete identifie. Recemment a ete rapportee la premiere mutation inactivatrice du recepteur de la prolactine (PRLR) dans le cadre d’une hyperprolactinemie familiale. De plus les souris invalidees pour le PRLR developpent des prolactinomes. Nous avons donc teste l’hypothese de l’implication de mutations de ce gene dans la survenue des prolactinomes chez l’homme. Objectif Evaluer la prevalence de mutations germinales du PRLR dans une serie de patients avec prolactinome. Patients et methodes Quatre-vingt-huit patients avec prolactinome sporadique ont ete inclus. Leur âge median au diagnostic etait de 24 ans (11–55). La majorite des patients presentaient un macroprolactinome (taille > 10 mm). Nous avons exclu de cette analyse les patients ayant une mutation des genes MEN1 et AIP. Les 10 exons codant la forme longue et active en transcription du PRLR ont ete sequences. Resultats Nous avons identifie 4 variations faux-sens du PRLR chez 16 patients, 14 a l’etat heterozygote et 2 a l’etat homozygote. Parmi ces variants, 2 sont recurrents (c.298A > G, c.508A > C) et correspondent a des polymorphismes. Le variant c.1732G > C est predit deletere. Enfin, le variant c.394G > A est rapporte ici pour la premiere fois, sa caracterisation fonctionnelle est en cours. Conclusion Malgre les donnees tres recentes indiquant que le PRLR pourrait etre un gene candidat du prolactinome, la presence d’une mutation germinale de ce gene ne semble que rarement impliquee dans la survenue des tumeurs lactotropes chez l’homme.

27. PO46 - Baisse de la concentration de cobalamine chez les patients diabétiques de type 2 traités par metformine

Introduction Des etudes recentes ont fait etat d’une diminution de vitamine B12 plasmatique chez les patients traites par metformine. On ne sait pas les consequences de cette baisse, en particulier sur l’hematopoiese. Nous avons analyse les parametres sanguins et le dosage de vitamine B12 chez des patients traites par metformine. Patients et methodes Etude de cohorte sur tous les patients hospitalises sur une periode de 6 semaines. Dosage de vitamine B12, NFS, calcul du MDRD. Resultats Cent six patients, âges de 62 ± 11 ans, duree de diabete 13 ± 9 ans. 64 (60 %) traites par metformine, duree moyenne 10 ± 8 ans. Les patients traites par metformine ne differaient des autres ni pour l’âge ni pour le MDRD. La concentration de cobalamine etait plus basse chez les patients traites metformine (415 ± 293 vs 560 ± 204 pg/mL p = 0,0054). En analyse multivariee (âge, sexe, fonction renale et prise de metformine), seule la prise de metformine etait associee a la concentration de vitamine B12 (p = 0,0009). Cette concentration etait correlee a la duree de traitement (p = 0,0092). Il n’y avait pourtant pas plus de carence en vitamine B12 (cobalamine Discussion Nous confirmons la baisse de la concentration de cobalamine chez les patients traites par metformine. Celle-ci est correlee a la duree de traitement. Nous n’avons pourtant pas observe de carence en vitamine B12, ni de repercussion sur les parametres de la numeration. Cependant il parait necessaire d’evaluer d’autres consequences de cette baisse, en particulier sur la concentration d’homocysteine. Cette deuxieme partie est en cours.

Abstract

The onset of fast fission was investigated with the Cl + Au system. The mass distribution of the fragments and the fusion cross sections were determined at six different bombarding energies. It turns out that the width of the symmetric component of the mass distribution increases strongly with the critical angular momentum. This could be related with the disappearance of the fission barrier due to centrifugal effects. A semi-static model is developed and the results are compared with the experimental data. In this framework, the observed broadening of the mass distribution can be attributed to the presence of the fast fission phenomenon.

Abstract

In a previous paper a model has been presented which treats the pion as a collective superposition of quark-antiquark pairs. The pion wave function has strong admixtures of many-quark-many-antiquark components. This wave function is used to investigate empirical weak and electromagnetic-decay properties of the pion. The results so obtained are consistent with a small intrinsic pion radius, substantially smaller than the measured pion charge radius.

Abstract

Using 106 000 hadronic events obtained with the ALEPH detector at LEP at energies close to the Z resonance peak, the strong coupling constant αs is measured by an analysis of energy-energy correlations (EEC) and the global event shape variables thrust, C-parameter and oblateness. It is shown that the theoretical uncertainties can be significantly reduced if the final state particles are first combined in clusters using a minimum scaled invariant mass cut, Ycut, before these variables are computed. The combined result from all shape variables of pre-clustered events is αs(MZ2 = 0.117±0.005 for a renormalization scale μ= 1 2 M Z . For μ values between MZ and the b-quark mass, the result changes by −0.009+0.006.

Abstract

From a data sample of 450 000 hadronic events recorded with the ALEPH detector at LEP, 92 +/- 10 events are observed containing a J/psi meson decaying to mu+mu- or e+e-. From these data the measured inclusive branching fraction for a b flavoured hadron to decay to a J/psi is BR(b --> J/psiX) = (1.21 +/- 0.13 (stat.) +/- 0.08 (syst.))%, and the average b hadron lifetime in the events tagged with a J/psi is tau(b) = 1.35(-0.17)+0.19 +/- 0.05 ps. Five events are observed consistent with the exclusive decay B+/- --> J/psiK+/- and from these events the exclusive branching fraction is measured to be BR(B+/- --> J/psiK+/-) = (0.22 +/- 0.10 +/- 0.02)%. Upper limits for other exclusive branching ratios are given.
