Abstract

International audience; Background: Crohn's disease (CD) is a chronic disorder requiring long-term treatment. However, up to 20% of patients interrupt temporarily or permanently anti-TNFα. Primary aim was to identify internal and external factors influencing patient's motivation to pursue anti-TNFα in active CD.Methods: This was a French, multicentre, prospective study enrolling CD patients on anti-TNFα therapy since more than 3 months. Patients completed the Satisfaction of Patients with Crohn's Disease questionnaire (SPACE-Q) and other patient-reported-outcome tools at inclusion visit, and after 6 and 12 months.Results: A total of 274 patients were included: 146 (53.3%) received adalimumab, while 128 (46.7%) infliximab. Abstract

In Europe, the number of cases of Campylobacter enteritis and their quinolone resistance is increasing. The aims of this work were to evaluate: (1) the hospital epidemiology of bacterial enteritis between 2010 and 2015. (2) The proportion of Campylobacter and Salmonella enteritis. (3) Resistance to quinolones in adult and paediatric populations. (4) To investigate possible regional epidemiological and bacteriological disparities.This is a multicentric study carried out in 21 general hospitals (CHG) representing 14 French regions with a prospective collection of the results of coprocultures from 2010 to 2015 in adult and paediatric populations (children 15 years old not exposed to quinolones). The epidemiological and bacteriological data were collected from software laboratory for positive stool cultures for Campylobacter and Salmonella. The results were compared year by year and by a period of 2 years.In adults, Campylobacter enteritis was each year significantly more frequent than Salmonella (P 0.001), with a significant increase from 2010 to 2015 (P 0.05). In children, there was also a significant and stable predominance of Campylobacter enteritis over the study period (P = 0.002). The quinolone resistance of Campylobacter was greater than 50% on the whole territory, with no North-South difference over the three periods studied. It increased significantly from 2012 to 2015 in adults (48% to 55%, P 0.05) and in children (54% to 61%, P = 0.04).Our results confirm the increase in the prevalence of Campylobacter enteritis compared to Salmonella between 2010 and 2015. The quinolone resistance of Campylobacter is greater than 50% on the whole territory, stable between 2010 and 2015 in adults and significantly increased in children.

Abstract

Summary Aims To determinate the topographical distribution of key diagnostic histological features of lymphocytic colitis (LC) and collagenous colitis (CC) and to establish what correlations may exist between the histological findings and the causes and severity of MC. Patients and methods Patients with MC were included in a prospective multicentre French study from September 2010 to October 2012. MC was diagnosed by performing total colonoscopy with multiple biopsies of the rectum and colon collected in separate jars and analyzed separately for each site (descending and sigmoid colon, transverse colon, ascending colon). CC was defined as a subepithelial collagen layer > 10 μm thick and LC as an intraepithelial lymphocyte (IEL) count > 20 lymphocytes per 100 epithelial cells without any associated thickening of the subepithelial collagen. Results Ninety-five patients, 69 with LC 26 and with CC, were included in the analysis. The sensitivity of the biopsies for diagnosing MC was maximum in the transverse colon and minimum in the rectum. Rectal and left colonic biopsies resulted in the diagnosis of CC and CL in 93% and 94% of cases, respectively. All the remaining cases of MC were diagnosed by performing additional biopsies beyond the splenic flexure. In patients with LC, a higher rate of IELs was associated with the absence of abdominal pain (P = 0.01) and a shorter duration of diarrhea (P = 0.001). In patients with CC, a lower level of collagen thickness in the basement membrane was associated with the presence of an autoimmune disease (P = 0.02). Conclusion More than 90% of cases of microscopic colitis were diagnosed in this study by performing rectal and left colonic biopsies.

Abstract

Summary Background and objective Mortality of upper gastrointestinal bleeding seems declining. Whether practice guidelines for the management of peptic ulcer bleeding are followed is unknown. We aimed to update epidemiology of peptic ulcer bleeding and to assess the adherence to guidelines in the French community. Methods Between March, 2005 and February, 2006, a prospective multicenter study was conducted including all patients with communautary upper gastrointestinal bleeding. Data from patients with peptic ulcer bleeding were extracted and analyzed. Results Out of 3203 analyzable patients included, 1140 (35.6%) had a peptic ulcer bleeding and 965 of them a duodenal and/or gastric ulcer. Seven hundred and thirty-five were male (64.5%) and mean age was 66.4 years (±18.8). Overall, 699 patients (61.3%) were taking medication inducing upper gastrointestinal bleeding. Two-hundred and sixty-eight (23.5%) patients had endoscopic therapy, 190 (70.9%) of whom had epinephrine injection alone. Among the 349 patients with high risk stigmata on endoscopy (Forrest IA, IB, IIA), 209 (59.9%) underwent endoscopic therapy. One thousand one hundred and seven patients (97.1%) were given proton-pump inhibitors. One hundred and thirty-four patients (11.8%) experienced haemorrhagic recurrence. Forty-eight patients (4.2%) underwent surgery and 61 (5.4%) died. Conclusions Consistently with previous studies, mortality of upper gastrointestinal bleeding seems declining. Further progress lies above all in prevention but also probably in better adherence to therapeutic guidelines and management of comorbidities.

Abstract

Background & Aims Little is known about the magnitude of the risk of anal and rectal cancer in patients with anal and/or perineal Crohn's disease. We aimed to assess the risk of anal and rectal cancer in patients with Crohn's perianal disease followed up in the Cancers Et Surrisque Associe aux Maladies Inflammatoires Intestinales En France (CESAME) cohort. Methods We collected data from 19,486 patients with inflammatory bowel disease (IBD) enrolled in the observational CESAME study in France, from May 2004 through June 2005; 14.9% of participants had past or current anal and/or perianal Crohn's disease. Subjects were followed up for a median time of 35 months (interquartile range, 29–40 mo). To identify risk factors for anal cancer in the total CESAME population, we performed a case-control study in which participants were matched for age and sex. Results Among the total IBD population, 8 patients developed anal cancer and 14 patients developed rectal cancer. In the subgroup of 2911 patients with past or current anal and/or perianal Crohn's lesions at cohort entry, 2 developed anal squamous-cell carcinoma, 3 developed perianal fistula–related adenocarcinoma, and 6 developed rectal cancer. The corresponding incidence rates were 0.26 per 1000 patient-years for anal squamous-cell carcinoma, 0.38 per 1000 patient-years for perianal fistula–related adenocarcinoma, and 0.77 per 1000 patient-years for rectal cancer. Among the 16,575 patients with ulcerative colitis or Crohn's disease without anal or perianal lesions, the incidence rate of anal cancer was 0.08 per 1000 patient-years and of rectal cancer was 0.21 per 1000 patient-years. Among factors tested by univariate conditional regression (IBD subtype, disease duration, exposure to immune-suppressive therapy, presence of past or current anal and/or perianal lesions), the presence of past or current anal and/or perianal lesions at cohort entry was the only factor significantly associated with development of anal cancer (odds ratio, 11.2; 95% CI, 1.18-551.51; P = .03). Conclusions In an analysis of data from the CESAME cohort in France, patients with anal and/or perianal Crohn's disease have a high risk of anal cancer, including perianal fistula–related cancer, and a high risk of rectal cancer.

Abstract

Summary Iron-deficiency anemia remains unexplained after upper and lower endoscopies in about 35% of the cases. Association between iron-deficiency anemia and atrophic gastritis has been recognized for many years. As early as 1993, well-documented clinical cases, mainly of children and adolescents, have shown that iron-deficiency anemia (in particular, iron-resistant anemia) could be corrected by eradication of Helicobacter pylori, even without associated iron supplementation. It seems that the main mechanism is iron malabsorption. Indeed, absorption of iron necessitates a reduction of the ferric to ferrous form, which is dependent upon the pH of gastric juice and ascorbic acid. Recent studies have shown that patients with an association of pangastritis (inflammatory rather than atrophic) and unexplained iron-deficiency anemia had increased intragastric pH and lower intragastric and plasma levels of ascorbic acid compared to patients with unexplained iron-deficiency anemia without H. pylori-associated gastritis and patients with H. pylori-associated gastritis but without anemia. Based on the available evidence, testing and treatment of persons with pangrastritis and unexplained iron-deficiency anemia for H. pylori infection was recommended by the experts at the latest Maastricht III European consensus conference.

Abstract

Liver disease is exceptional in patients with inflammatory bowel disease. The most common manifestation, sclerosing cholangitis, characterized by inflammation and fibrosis of the intra- and\or extrahepatic bile ducts, is unusual in patients with inflammatory bowel disease. Conversely, inflammatory bowel disease (mainly chronic ulcerative colitis) is not infrequent in patients with sclerosing cholangitis. Gallstone disease, portal vein thrombosis, and hepatic abscesses are complications directly related to inflammatory bowel disease. Drugs prescribed for the treatment of inflammatory bowel disease can be the cause of rare but potentially serious hepatic manifestations which must be recognized and detected early. Recent studies have demonstrated the role of purine analogues in the development of nodular regenerative hyperplasia. Because of the poor prognosis, patients taking purine analogues should be monitored regularly to search for inaugural signs such as an elevation of serum alkaline phosphatase or low platelet counts (which may not necessarily reach thrombopenia). The risk of methotrexate-induced fibrosis is exceptional in inflammatory bowel disease. Patients should be monitored with non-invasive tests to recognize the development of fibrosis. Finally, because of the risk of viral reactivation, patients should be screened for hepatitis B virus surface antigen before introducing infliximab; chronic carriers should be given preventive treatment with nucleoside or nucleotide analogues.

Abstract

Resume Les steatopathies hepatiques non alcooliques (NAFLD) regroupent plusieurs entites anatomopathologiques allant de la simple steatose a la fibrose hepatique severe, voire la cirrhose. Ces lesions sont semblables a celles de la maladie alcoolique du foie, mais surviennent en dehors d'une consommation excessive d'alcool. L'association d'une steatose hepatique a des lesions histologiques inflammatoires, necrotiques et fibrosantes d'intensite variable definit la steatohepatite non alcoolique (NASH). Les NAFLD representent une des causes les plus frequentes de perturbation des tests hepatiques. La NASH est la cause de la plupart des cirrhoses cryptogenetiques et des carcinomes hepatocellulaires qui les compliquent. La prevalence des NAFLD dans la population generale occidentale est comprise entre 10 et 24 % ; parmi ces patients, environ 3 % ont une NASH. L'insulinoresistance est la cause principale des NAFLD. Plusieurs mecanismes physiopathologiques pourraient etre a l'origine des NASH ; l'apport excessif de graisses dans les hepatocytes, une diminution de l'oxydation intracellulaire des acides gras et l'augmentation de la peroxydation lipidique sont les principaux. Les NAFLD sont souvent associees a d'autres pathologies, le plus souvent, un diabete de type 2, une surcharge ponderale ou une hyperlipidemie. Une fois le diagnostic de NAFLD pose, deux questions restent encore sans reponse : Quelle est sa cause ? et Quel est son pronostic ? Meme si pour la plupart les patients ont une maladie benigne non evolutive, certains developpent une fibrose extensive, voire une cirrhose. Il n'existe pas de traitement codifie des NAFLD. Une reduction ponderale, en cas de surpoids, est toujours preconisee. Les indications d'un traitement medicamenteux sont encore a definir.

Resume L'hepatite alcoolique aigue est une maladie frequente dont la mortalite a 1 an, toutes formes confondues, est d'environ 20 %. L'abstinence, bien que toujours recommandee, n'est pas suffisante chez certains patients pour ameliorer leur survie et permettre leur guerison. C'est particulierement le cas des patients presentant une hepatite alcoolique aigue grave. Plusieurs molecules ont ete experimentees comme traitement de l'hepatite et de la cirrhose alcooliques. Leur mode d'action est centre sur les principaux processus physiopathologiques de l'hepatique alcoolique, en particulier l'hypercatabolisme, l'inflammation, les anomalies des cytokines, le stress oxydatif. La corticotherapie est le traitement le plus efficace chez les patients ayant une hepatite alcoolique aigue grave. Cependant, ses resultats ne sont pas homogenes et la realite de son efficacite clinique demeure controversee. Meme si les corticosteroides sont benefiques en cas d'hepatite alcoolique aigue grave, ils ne peuvent pas etre utilises chez tous les patients. Dans cette optique, d'autres therapeutiques ont ete testees : la pentoxifylline et la nutrition enterale a base de triglycerides a chaine moyenne semblent efficaces. D'autres recherches s'orientent vers des molecules diminuant les lesions histologiques inflammatoires d'hepatite chronique ou de cirrhose alcooliques, en particulier les agents antioxydants ou les anti- tumour necrosis factor alpha. La transplantation hepatique est parfois le seul traitement des hepatites alcooliques aigues severes.

Resume Le desir de grossesse est une preoccupation frequente chez les patientes atteintes de maladies inflammatoires chroniques de l’intestin. Les taux de fertilite, d'avortement spontane et d'anomalies congenitales ne semblent pas plus eleves au cours des maladies inflammatoires chroniques de l’intestin (MICI). En revanche, les taux de prematurite et d'hypotrophie fœtale seraient plus importants au cours de la maladie de Crohn (MC) en activite. La grossesse ne semble avoir d'influence sur l’histoire naturelle des maladies inflammatoires chroniques de l’intestin. Une periode de quiescence de la maladie d'au moins 3 mois est recommandee avant d’envisager une grossesse afin de minimiser le risque de complication maternelle et fœtale. Les medicaments autorises pour le traitement des poussees sont les derives salicyles (a posologie reduite) et les corticoides. L'azathioprine est habituellement contre-indiquee. Un accouchement par voie basse est la regle en dehors d'une localisation actuelle ou ancienne de la maladie a la region anoperineale. L’allaitement est possible au cours des MICI, cependant, certains medicaments sont contre-indiques.

The aim of this study was to analyze the differences in the therapeutical strategy in the 2011 ECCO-EpiCom inception cohort within the first year after diagnosis. Methods: Fourteen European (9 Western and 5 Eastern European centers) and one Australian center with 258 CD, 380 UC and 71 IBDU patients (65% from Western, 25% from Eastern Europe, 10% from Australia; female/male: 326/383; mean age at diagnosis: 40.9 years, SD: 17.3 years) participated in the one-year followup. Patients' data regarding disease characteristics and medical therapy were registered and entered in the web-based ECCOEpiCom database every third month during the first 12 months after diagnosis. The aim of this study was to describe the characteristics and identify the prognostic factors of UGIB associated with MWS. Patients and methods: A subgroup of patients with UGIB secondary to MWS was isolated from the data recorded on UGIB at the French ANGH observatory in 2005 to 2006. The epidemiological characteristics and prognostic factors of the patients in the MWS subgroup were compared with those of patients with peptic ulcer bleeding (n 965). Results: The subgroup studied here consisted of 148 men and 56 women (mean age 52.9 21.3 years), accounting for 6.4% of the all the patients recorded at this observatory. Endoscopic treatment, which was performed in 20.6% of cases (corresponding to 34% of active bleeding), consisted in performing a sclerosing injection alone in 28 cases, associated with a clip in 4 cases, or with a ligature in one case and with argon plasma coagulation in 2 cases. Ligation alone was performed in one case and a clip alone in 5 cases. Patients with MWS were younger (52.9 21.4 vs. 66.6 18.7, p 0.0001), more frequently male (72.5% vs. 64.9%, p 0.04), and tended more frequently to have cirrhosis (17.6% vs. 8.9%, p 0 .0001), to have fewer transfusions (32.5% vs. 71.5%, p 0 .0001), a lower rate of recurrent bleeding (3.9% vs. 12.7%, p 0 .0001), surgery (0.5% vs. 5.3%, p 0.002) and a shorter hospital stay (5.85 4 vs. 9 8.6, p 0.0001). No significant differences were observed between the rates of active bleeding (59.8% vs. 64.8%, p 0.2), endoscopic treatment (20.6% vs 25.4%, p 0.2) or mortality (2.9% vs. 5.7%, p 0.1) in the two groups. The 6 deaths were not directly due to the bleeding but to comorbid conditions. Conclusion: UGIB secondary to MWS is less severe than that due to peptic ulcers and the mortality is mainly due to comorbidities.
