1. Opioid Use and Associated Patient Satisfaction With Pain Control After Endoscopic Surgery and Robotic-Assisted Radical Prostatectomy

Abstract

To evaluate patient reported measures in patients undergoing endourologic procedures and robotic assisted radical prostatectomy (RARP) to demonstrate the efficacy of non-opioid postoperative pain management strategies.A prospective cohort study performed at an academic medical center included a patient telephone questionnaire and chart review. Opioid prescriptions, opioid use, and patient reported outcomes were recorded. Bivariate analyses were used to compare patients who did and did not use opioids in the RARP cohort while overall trends were reported for the endourologic procedures.Of the 68 patients undergoing endoscopic intervention, 14 (21%) were prescribed an opioid and 6 (9%) reported any opioid use. 58 (85%) reported their pain was very well or well controlled while 9 reported their pain was poorly controlled. 59 (87%) were satisfied or very satisfied with their pain control. Fifty-three (93%) of the 57 patients undergoing RARP received an opioid prescription and only 23 reported any opioid use. All but 1 patient reported that their pain was well or very well controlled and almost all (54) of the patients were satisfied with their level of pain control. 36 (63%) reported their pain was less than expected while only 7 (12%) reported it was more than expected.Most patients undergoing endourologic procedures do not use postoperative opioids and report favorable outcomes regarding their pain control. Similarly, after RARP, most patients do not use opioids even when they are prescribed and are satisfied with their pain control.

Abstract

When an elastic block is squeezed in contact with a substrate the contact will in general consist of many asperity contact regions. During sliding the long-range elastic coupling between the contact regions have a big influence on the sliding dynamics: when the loading force increases the density of contact regions increases, which affects the lateral coupling between the contact region, and introduces a dependence of the friction force on the loading force. Here we present a full three-dimensional model study of the role of the elastic coupling on the friction force. The theory is applied to rubber friction, but the mechanism for the dependence of the friction coefficient on the load is relevant for non-rubber materials as well.

Abstract

The stability of the gain of the photomultiplier (PMT) camera for the Fluorescence Detector (FD) of the Telescope Array experiment was monitored using an 241 Am loaded scintillator pulsers (YAP) and a diffused xenon flasher (TXF) for a selected set of 35 PMT-readout channels. From the monitoring of YAP pulses over four years of FD operation, we found slow monotonic drifts of PMT gains at a rate of −1.7~+1.7%/year. An average of the PMT gains over the 35 channels stayed nearly constant with a rate of change measured at −0.01±0.31(stat)±0.21(sys)%/year. No systematic decrease of the PMT gain caused by the night sky background was observed. Monitoring by the TXF also tracked the PMT gain drift of the YAP at 0.88±0.14(stat)%/year.

Abstract

Hydrogen embrittlement of austenitic stainless steels has been examined with respect to deformation microstructures and lattice defects created during plastic deformation. Two types of austenitic stainless steels, SUS 304 and SUS 316L, uniformly hydrogen-precharged to 30 mass ppm in a high-pressure hydrogen environment, are subjected to tensile straining at room temperature. A substantial reduction of tensile ductility appears in hydrogen-charged SUS 304 and the onset of fracture is likely due to plastic instability. Fractographic features show involvement of plasticity throughout the crack path, implying the degradation of the austenitic phase. Electron backscatter diffraction analyses revealed prominent strain localization enhanced by hydrogen in SUS 304. Deformation microstructures of hydrogen-charged SUS 304 were characterized by the formation of high densities of fine stacking faults and e-martensite, while tangled dislocations prevailed in SUS 316L. Positron lifetime measurements have revealed for the first time hydrogen-enhanced creation of strain-induced vacancies rather than dislocations in the austenitic phase and more clustering of vacancies in SUS 304 than in SUS 316L. Embrittlement and its mechanism are ascribed to the decrease in stacking fault energies resulting in strain localization and hydrogen-enhanced creation of strain-induced vacancies, leading to premature fracture in a similar way to that proposed for ferritic steels.

Abstract

We present a measurement of the energy spectrum of ultra-high-energy cosmic rays performed by the Telescope Array experiment using monocular observations from its two new FADC-based fluorescence detectors. After a short description of the experiment, we describe the data analysis and event reconstruction procedures. Since the aperture of the experiment must be calculated by Monte Carlo simulation, we describe this calculation and the comparisons of simulated and real data used to verify the validity of the aperture calculation. Finally, we present the energy spectrum calculated from the merged monocular data sets of the two FADC-based detectors, and also the combination of this merged spectrum with an independent, previously published monocular spectrum measurement performed by Telescope Array’s third fluorescence detector [T. Abstract

A Zr 55 Cu 30 Al 10 Ni 5 bulk metallic glass (BMG) plate with thickness of 2 mm was successfully spot friction stir welded to a commercial pure Cu plate. Microstructural characterization revealed that some serrations formed at the edge of the BMG plate and some nano-sized nucleuses were dispersed in the BMG side along the Cu/BMG joint interface. In the upper Cu side close to the keyhole, some blocky and strip-like BMG fragments were embedded in the Cu matrix. As a result, significantly elongated Cu grains with width of about 200 nm were formed between the strip-like BMG fragments. Shear tensile tests showed that the Cu/BMG spot joints fractured at a maximum load of about 2300 N through the stir zone pullout fracture mode.

Abstract

Background Radiofrequency ablation (RFA) is considered to be the most promising non-surgical ablation technique for the treatment of small breast cancer. However, few data are available regarding long-term follow-up of patients treated with this modality. Methods Since 2005, we have performed RFA and sentinel lymph node (SLN) biopsy in 19 cases. Axillary lymph node dissection (ALND) was performed in patients with positive SLNs. From 24 to 202 days after RFA, the ablated tumour tissue was excised by mammotome biopsy and examined histologically or immunohistochemically with H&E staining, nicotinamide adenine dinucleotide (NADH)-diaphorase staining, and single-stranded (ss) DNA staining. All cases were followed-up after breast radiation and systemic therapies. Results Although complete response was histologically confirmed in only 8 cases, NADH-diaphorase and ssDNA staining did not demonstrate any viable tumour cells in the ablated lesions. At a mean follow-up of 60 months (follow-up range, 37–82 months), there were no cases of in-breast recurrence, although one patient died due to hepatic metastases. Cosmesis of the conserved breast was excellent or good in all of the cases, but a hard lump was persistent after RFA in half of the cases. Conclusions The long-term outcome of patients treated with RFA is encouraging with regard to cosmesis and local control. Because a persisted lump may cause patient discomfort, anxiety and fear, however, further studies are needed to establish the optimal technique. Moreover, a prospective study will be required to determine the equivalency in local recurrence rates between the RFA therapy and conventional breast-conserving treatment.

Abstract

The electron slowing-down in polytetrafluoroethylene (PTFE) was investigated on the basis of a proposed physical model considering the elastic and inelastic electron scattering on bound fluorine and carbon atoms, electron–phonon scattering and electron trapping. The resonance dissociative and non-dissociative electron attachment cross sections for с –C 4 F 8 molecule were suggested as a basis set to describe the electron trapping in PTFE. Modification of the basis set was carried out by comparison of the calculated total secondary electron yield (TSEY) from PTFE with its experimental values for electron beam energies of 100 eV–5 keV. The electron trajectories were simulated on the base of Monte Carlo technique up to the electron dissociative or non-dissociative attachment. It was shown the calculated positions of electron traps because of dissociative attachment to give the distribution of the broken C–F bonds which cause the (–CF 2 –) chain scission and appearance of vinyl groups, and, consequently, the PTFE degradation. The calculated TSEY as a function of the large (>80°) incident electron angles demonstrated a sharp drop for high energy electron beams that has never been published anywhere.

Abstract

The Telescope Array(TA) experiment located at western desert in Utah USA (N39.3,W112.9) is designed for observation of air shower from extreme high energy cosmic rays. The TA detector consists of 2 types of detector to enable a cross check on systematic difference from the two main methods of observation for the energy region. One is a Fluorescence detector (FD) for detecting fluorescence light from air shower and another is surface detector (SD) array for detecting air shower particles at ground level. Each SD consists of 2 layers of plastic scintillator with 3m 2 of surface and more sensitive to electromagnetic component in air shower. The full operation using 3FD stations and full SD array has started. Here we present the updated status of Telescope Array experiment.

Abstract

In this study, we evaluated the influence of photoresist–water contact time on the quantity of the photoacid generator (PAG) leached from photoresists into pure water and alteration of the photoresist composition using LC–MS, XPS, and TOF-SIMS, by employing exposed and unexposed photoresists. As a result, the quantity of PAG leached into pure water increased as the contact time elapsed. Then, it was observed by TOF-SIMS that the quantity of the PAG on the photoresist surface decreased as the contact time elapsed. Regarding the ratio of the functional groups on the photoresist surface, the methyladamantyl group decreased but the carboxyl group increased because of exposure, respectively. On the exposed photoresist surface, the methyladamantyl group increased as the contact time elapsed. This was strongly related to the phenomenon that the quantity of methyladamantyl group was different between the inside and surface of photoresist.

Abstract

Dichloromethane, ethanol, and boiling water extracts of the brown seaweeds Sargassum fulvellum and Sargassum thunbergii were examined for antipyretic, analgesic, and anti-inflammatory activities in mice. The activities were evaluated against yeast-induced pyrexia, tail-flick test, and phorbol myristate acetate-induced inflammation (edema, erythema, and blood flow). The dichloromethane extract (0.4 mg/ear) of Sargassum fulvellum inhibited an inflammatory symptom of mouse ear edema by 79.1%. The ethanol extract (0.4 mg/ear) of Sargassum thunbergii also inhibited edema by 72.1%. No acute toxicity was observed after p.o. administration of each extract (5 g/kg bw). These findings are consistent with various claims that these seaweeds can be used as remedies for inflammation-related symptoms.

The observatory will measure the energy spectrum, anisotropy and composition of ultra-high energy cosmic rays. The surface detectors are being deployed and the array should be complete by the end of February, 2007. We will soon be collecting hybrid data at the Telecope Array.

Abstract

The purpose of this study was to develop a numerical tool to simulate the performance of lugged wheels designed for a lunar microrover. The performance was analyzed using a Discrete Element Method (DEM) whose accuracy was validated for interactions between lugged wheels and soil. DEM analysis indicated that, on flat horizontal lunar surfaces, wheels with 18 10-mm-high lugs would provide less net traction than would wheels with 36 5-mm-high lugs.

Abstract

A new silicon vertex detector for the Belle experiment has been in operation at the high-luminosity asymmetric energy electron–positron collider KEKB since October 2003. It provides a larger polar angle acceptance, a better vertex resolution and a higher radiation tolerance than the previous one. The obtained performances indicate that the SVD2 works reliably, in very good agreement with expectations.

Abstract

The old silicon vertex detector (SVD1) of the Belle detector, operating at the high-luminosity asymmetric energy e+e− collider KEKB, was replaced by an upgraded version (SVD2) in 2003. The new detector has modified geometry and redesigned readout electronics, providing a larger polar angle acceptance, better vertex resolution, improved radiation hardness and reduced dead time. The operation of the detector started successfully in October 2003. The basic concepts of the SVD2 design as well as its performance after 10 months of data taking are described in this paper.

Abstract

Tool wear is a key issue for the friction stir welding (FSW) of aluminum matrix composites (AMCs), especially when the volume fraction of reinforcing particulates is relatively high. In this study, a threaded tool of WC–Co hard alloy is used to weld a cast AMC composed of Al–Si matrix and 30 vol%SiC particulates so as to examine the wear characteristics of the tool. Experimental results indicate that the tool wear is appreciable although the shoulder size and pin length are changed slightly. The radial wear of the pin is very different at different locations of the pin, and the maximum wear is finally produced at a location of about one-third pin length from the pin root. The welding speed has a decisive effect on radial wear rate of the pin, and the maximum wear rate is produced in the initial welding.

Abstract

A major upgrade of the Silicon Vertex Detector (SVD 2.0) of the Belle experiment at the KEKB factory was installed along with new front-end and back-end electronics systems during the summer shutdown period in 2003 to cope with higher particle rates, improve the track resolution and meet the increasing requirements of radiation tolerance. The SVD 2.0 detector modules are read out by VA1TA chips which provide “fast or” (hit) signals that are combined by the back-end FADCTF modules to coarse, but immediate level 0 track trigger signals at rates of several tens of a kHz. Moreover, the digitized detector signals are compared to threshold lookup tables in the FADCTFs to pass on hit information on a single strip basis to the subsequent level 1.5 trigger system, which reduces the rate below the kHz range. Both FADCTF and level 1.5 electronics make use of parallel real-time processing in Field Programmable Gate Arrays (FPGAs), while further data acquisition and event building is done by PC farms running Linux. The new readout system hardware is described and the first results obtained with cosmics are shown.

Abstract

Crystalline α-Al 2 O 3 thin films have been deposited using reactive magnetron sputtering at the deposition rate of 0.5–0.7 μm/h above 700 °C in a commercial production scale PVD system. In the experiments, using Si wafers and CrN-coated cemented carbides as substrates, an oxidization pre-treatment was done, and then deposition of Al 2 O 3 films was carried out by varying the process temperature and the substrate bias voltage. An investigation of the correlation between characteristics of these films and deposition conditions was done. XRD study revealed that the under-layer of the Al 2 O 3 films and the substrate temperature for deposition remarkably influenced the film structure. Although only γ-Al 2 O 3 was observed in the films on the Si wafer, Al 2 O 3 film mostly consisting of α-phase was obtained on the CrN under-layer at 700 °C, and pure α-Al 2 O 3 film was deposited at 750 °C without applying bias voltage. TEM observation demonstrated that the α-Cr 2 O 3 interlayer formed by the oxidization pre-treatment of the CrN film played an important role in forming α-Al 2 O 3 . The hardness of the α-Al 2 O 3 film measured by nanoindentation was 23 GPa, and was enhanced to 27 GPa by applying bias.

33. Catalytic effect of 3d transition metals on hydrogen storage properties in mechanically milled graphite

Abstract

In this paper, we examined the catalytic effect of 3d transition metals on hydrogen storage properties in nanostructural graphite prepared by ball milling under hydrogen atmosphere. The Fe-doped nanostructured graphite shows the most marked hydrogen storage properties among the Fe-, Co-, Ni- and Cu-catalyzed graphite systems. The absorbed hydrogen concentration reaches up to ∼4 wt% by mechanically milling for 32 h (∼7 wt% for 80 h), and two peaks of hydrogen (mass number=2) around 730 and 1050 K were observed in the thermal desorption mass spectra (TDS). The starting temperature for hydrogen desorption was ∼600 K. On the other hand, the Co-doped graphite indicates that absorbed hydrogen concentrations reaches up to ∼2 wt% by mechanically milling for 32 h. The TDS spectrum showed only a broad peak around 1100 K, but the starting point for hydrogen desorption lowered down to ∼500 K. The Ni- and Cu-doped graphites did not show any significant improvement for hydrogen storage. These results suggest that the catalytic effect on hydrogen storage properties strongly depends on the affinity of graphite and doped metals.

Abstract

Multispectral satellite data sets and site surveyed data was utilized for water quality assessment. Lake Inbanuma represents a complex situation of major optical constituents and emergent spectral signals for remote sensing analysis. As such it provided a good test of the combined radiometric correction methods developed for optically sensed satellite data. Based upon results of repetitive laboratory experiments we examined this region by several field campaigns synchronized date and time with satellite observations. As results, first, in proportion to increased chlorophyll content reflectance absorption appeared in the portion between 625 and 675 nm whereas near infrared peaked at approximately 720 nm. Second, the spectral ratio of red and near infrared reflectance was diagnostically useful for assessing high chlorophyll-a levels in regional water. Third, the chlorophyll-a based trophic state index, established by Carlson in 1977, was used to calculate the results from the field campaign data. Based upon those factors, two transformed Landsat Thematic Mapper data sets were digitally analyzed. The complicated pattern and direction of eutrophic/ hypertrophic lake water flow resulting from the ‘‘open-and-shut operation’’, was analyzed. Regional variances in trophic state index values were observed. Also, the over all change in the trophic state of Lake Inbanuma indicated a specific efficacy in the ‘‘open-and-shut operation’’. The accumulation of these digitally analyzed data will aid in further understanding the appropriate intervals of operation and will contribute to more accurate regional environmental assessments. 2003 COSPAR. Abstract

Attenuation of the singlet quark--anti-quark bound state passing through a random external SU(N) color field at high energy is studied within the eikonal framework as a model for the absorption of a quarkonium by nuclear target. For a thick target of length L the penetration probability is suppressed non-exponentially and behaves asymptotically $\propto 1/LN^2$ with 1/N^2 indicating the equi-partitioning in the color space. An approximate rational expression for the penetration of the small Gaussian bound state is found which allows the explicit L and N dependences, and shows the attenuation is in the powers of L although the approach to the asymptote is rather slow., Comment: 8 pages, 2 figures

Abstract

This paper describes a study of B meson decays to the pseudoscalar-vector final state \rho\pi using 31.9\times 10^6 B\bar{B} events collected with the Belle detector at KEKB. The branching fractions B(B^+ \to \rho^0\pi^+) = (8.0^{+2.3+0.7}_{-2.0-0.7}) \times 10^{-6} and B(B^0 -> \rho^{+-} \pi^{-+}) = (20.8^{+6.0+2.8}_{-6.3-3.1}) \times 10^{-6} are obtained. In addition, a 90% confidence level upper limit of B(B^0 \to \rho^0\pi^0) < 5.3 \times 10^{-6}is reported.

Abstract

We present a measurement of the branching fraction for the semileptonic B decay B 0 →D + l − ν , where l− can be either an electron or a muon. We find Γ( B 0 →D + l − ν )=(13.79±0.76±2.51) ns −1 , and the resulting branching fraction B ( B 0 →D + l − ν )=(2.13±0.12±0.39)% , where the first error is statistical and the second systematic. We also investigate the B 0 →D + l − ν form factor and the implications of the result for |Vcb|. From a fit to the differential decay distribution we obtain the rate normalization |Vcb|FD(1)=(4.11±0.44±0.52)×10−2. Using a theoretical calculation of FD(1), the Cabibbo–Kobayashi–Maskawa matrix element |Vcb|=(4.19±0.45±0.53±0.30)×10−2 is obtained, where the last error comes from the theoretical uncertainty of FD(1). The results are based on a data sample of 10.2fb−1 recorded at the ϒ(4S) resonance with the Belle detector at the KEKB e+e− collider.

Abstract

We present a measurement of the Cabibbo–Kobayashi–Maskawa (CKM) matrix element |Vcb| using a 10.2 fb−1 data sample recorded at the ϒ(4S) resonance with the Belle detector at the KEKB asymmetric e+e− storage ring. By extrapolating the differential decay width of the B 0 →D ∗+ e − ν decay to the kinematic limit at which the D ∗+ is at rest with respect to the B 0 , we extract the product of |Vcb| with the normalization of the decay form factor F(1), |Vcb|F(1)=(3.54±0.19±0.18)×10−2, where the first error is statistical and the second is systematic. A value of |Vcb|=(3.88±0.21±0.20±0.19)×10−2 is obtained using a theoretical calculation of F(1), where the third error is due to the theoretical uncertainty in the value of F(1). The branching fraction B ( B 0 →D ∗+ e − ν ) is measured to be (4.59±0.23±0.40)×10−2.

Abstract

Thin zirconia films doped with semiconductor nanocrystals x ZrO 2 ·100− x CdS : Mn 2+ and Eu 3+ ( x =20, 50, 80) were prepared by sol–gel method to classify emission centers by measurements of luminescence, time-resolved luminescence, excitation spectra and decay profiles in the temperature range between 10 and 300 K. The broad emission band over the 24–13×10 3 cm −1 is found to be composed of four spectral components due to band-edge emission (CdS), ligand to metal charge transfer (LMCT) center (ZrO 2 ), surface-bound deep centers, and lattice-bound d–d (Mn 2+ ) or f–f transition (Eu 3+ ) centers. Luminescence of Mn 2+ and Eu 3+ showed x -composition-dependent spectral profiles and shortening of lifetimes to 5–50 μs at 10 K. Ultra-fast decay components of 0.5–1.5 ns were also detected in thin films at room temperature. Luminescence properties of doped-CdS nanocrystals are characterized by dependence on annealing temperatures, very fast decay times and low thermal activation energy of 1300 cm −1 in comparison with bulk crystals.

Abstract

The authors had earlier proposed the concept of an SLB (step–lock–bolt), which has some steps on the lead helix, with an objective of developing an anti-loosening screw fastener. The SLB produced had eight steps in a pitch and 45% flat part in which the lead angle is nil. It was found that the SLB does not easily loosen when it was fastened with a clamping force of 16±4 kN , because it suppresses the bolt torsion which is one of the causes of loosening. In this paper, a newly produced SLB, which has 10 steps and 50% flat part, is reported. The aim of developing this screw fastener is to prevent loosening even when a high clamping force cannot be applied, which is the case when wooden or rubber plates are fastened. The importance of the number of steps in a pitch and the ratio of the flat part in the anti-loosening performance of the SLB are discussed. The matching of the hardness of bolt and nut for preventing loosening is also discussed. The contact state between the bolt screw thread and nut is investigated carefully. As a result, the newly produced SLB prevents loosening even when the applied clamping force is 4 kN. The higher ratio of the flat part makes the SLB exhibit better performance.

Abstract

We report measurements for two-body charmless B decays with an eta' meson in the final state. Using 11.1X10^6 BBbar pairs collected with the Belle detector, we find BF(B^+ ->eta'K^+)=(79^+12_-11 +-9)x10^-6 and BF(B^0 -> eta'K^0)=(55^+19_-16 +-8)x10^-6, where the first and second errors are statistical and systematic, respectively. No signal is observed in the mode B^+ -> eta' pi^+, and we set a 90% confidence level upper limit of BF(B^+-> eta'pi^+) eta'K^+- decays is investigated and a limit at 90% confidence level of -0.20

49. Methanogenic activities on rice roots and plant residue and their contributions to methanogenesis in wetland rice field soil

Abstract

Changes in potential of methanogenic activities of three fractions (soil, living rice roots and plant residue) in wetland rice field soil during the cropping season of rice were determined by incubating them anaerobically in diluent in tubes. The soil fraction was prepared by removing various materials such as rice roots and plant residue mixed in core soil samples of the plow layer. Rice roots and plant residue (mainly rice straw plowed into soil) collected were washed in anoxic diluent to remove adhering soil and cut to pieces before the incubation. The methanogenic activity of the soil fraction was rather low from mid-May, when rice seedlings were transplanted, until late-June. Thereafter, the activity rapidly increased and at early-July it reached the maximum level of the activity in the year. The high activity was kept for about one month and then, during the period of intermittent irrigation, it dropped rapidly to much lower levels. The overall pattern of changes in the activity of the rice roots fraction resembled to that of the soil fraction, however, the activity on a dry weight basis was much higher than that of the soil fraction on almost all the days of determination. For the plant residue fraction, the pattern of seasonal changes was much different from those of the other two fractions and the activity increased soon after the start of flooding of the field. At the early-cropping season, the activity on a dry weight basis was much higher (up to 150 times) than that of the soil fraction of the same day. Changes in the weights of the three fractions in the rice field were measured by using hill plots, each of which was applied with rice straw and transplanted with rice seedlings. From the changes in the methanogenic activities and the weights of the three fractions in the field, the seasonal variations in the contribution of methanogenic activity of each fraction to the total methanogenic activity in the soil ecosystem were estimated. It was shown that plant residue should significantly enhance the methanogenic activity in the first half of the cropping season, and in contrast, rice roots should enhance it in the latter half.

Abstract

We built a data acquisition system for the BELLE experiment. The system was designed to cope with the average trigger rate up to 500 Hz at the typical event size of 30 kB. This system has five components: (1) the readout sequence controller, (2) the FASTBUS-TDC readout systems using charge-to-time conversion, (3) the barrel shifter event builder, (4) the parallel online computing farm, and (5) the data transfer system to the mass storage. This system has been in operation for physics data taking since June 1999 without serious problems.
