Statement of problem A consensus on the clinical performance of implant-supported removable partial dentures (ISRPDs) is lacking. Purpose The purpose of this systematic review and meta-analysis was to evaluate the clinical performance of ISRPDs in terms of the implant survival rates, marginal bone loss, and patient-reported outcome measures (PROMs). Material and methods Four electronic databases (MEDLINE/PubMed, EMBASE, Web of Science, and Cochrane Library) were independently searched by 2 reviewers for articles published up to December 2020. A single-arm meta-analysis was performed to evaluate the implant survival rates and marginal bone loss by using the R program. The Cochrane collaboration tool was used to evaluate the risk of bias of randomized controlled trials (RCTs), and the Newcastle-Ottawa scale for non-RCT studies. Results Sixteen studies were included, with a total of 334 participants and a mean age of 58 years. The participants received a total of 581 dental implants, 475 conventional and 106 mini-implants. All included studies reported implant survival rate above 90% (range: 92% to 100%). Meta-analysis indicated a high proportion of implant survival rates of 3% (95% confidence interval [CI]: 2% to 5%) and a low mean raw score of marginal bone loss 0.98 mm (CI: 0.61 to 1.36 mm). Compared with conventional RPDs, improved patient quality of life and satisfaction were reported by studies that evaluated ISRPDs. RCT studies exhibited a low risk of bias for most domains, while most non-RCT studies were classified as good quality. Conclusions ISRPDs exhibited high implant survival rates and acceptable bone loss with improvement in the quality of life and satisfaction of patients when compared with conventional RPDs. Therefore, they can be considered suitable for the rehabilitation of partially edentulous patients.

Statement of problem The development of polymethyl methacrylate (PMMA) computer-aided design and computer-aided manufacturing (CAD-CAM) resin blocks with reported improved mechanical properties has simplified complete denture production. However, whether the objective of improved mechanical properties has been achieved compared with conventional heat-polymerized PMMA is not yet clear. Purpose The purpose of this systematic review and meta-analysis was to evaluate the mechanical properties of denture base resins manufactured by conventional heat-polymerization and by CAD-CAM in terms of flexural strength, flexural modulus, and surface roughness. Material and methods Electronic databases (PubMed/MEDLINE, Scopus, Web of Science) were independently searched by 2 researchers for relevant studies published up to November 2020. The population, intervention, comparison, and outcome (PICO) question was, “Does the conventionally manufactured, heat-polymerized PMMA resin, as a denture base, demonstrate the same mechanical properties as the CAD-CAM resin block?” In addition, a meta-analysis was based on the inverse variance method. Flexural strength, flexural modulus, and surface roughness were analyzed through the continuous outcome evaluated by mean difference and standard deviation, with 95% confidence intervals. To evaluated heterogeneity, the I2 value (≤25%=low, ≥50%=moderate and ≥75%=high) and the P value were considered. P Results Thirteen in vitro studies were included in the analysis. A total of 507 specimens were evaluated, 222 conventional and 285 CAD-CAM. In terms of flexural strength, the data showed no significant difference when conventional heat-polymerized PMMA was compared with CAD-CAM PMMA resins (P=.06; mean difference=18.28; 95% confidence interval:-0.42 to 36.97). In terms of flexural modulus, there was a significant difference for the CAD-CAM PMMA group (P=.01; mean difference=589.22; 95% confidence interval: 117.95 to 1060.48). In terms of surface roughness, a significant difference was observed between the groups (P=.02; mean difference=-0.53; 95% confidence interval: -0.97 to -0.09) with the conventional heat-polymerized PMMA resin having higher surface roughness values. Conclusions The mechanical properties of CAD-CAM PMMA resins were generally improved when compared with heat-polymerized polymethyl methacrylate resin.

The objective of this study was to compare, through a systematic review with a meta-analysis, the relative risks of arthroscopy and arthrocentesis in the temporomandibular joint. MEDLINE/PUBMED, EMBASE, Cochrane Library (CENTRAL), Web of Science, SCOPUS were the researched databases, as well as grey literature and manual searches. The search results showed 656 studies, but only five met the eligibility criteria. The evaluation included 194 joints (104 patients): 101 were arthroscopy and 93 arthrocentesis. Complications were observed in four patients undergoing arthroscopy (two with temporary facial paralysis and two with prolonged cervical oedema) and in three patients undergoing arthrocentesis (two with severe bradycardia and one with prolonged cervical oedema). The meta-analysis demonstrated a relative risk of 0.99 for complications after arthroscopy compared with arthrocentesis, but the results showed no statistical differences. In conclusion, this systematic review suggests that there is no increased risk of complications with arthroscopy than arthrocentesis. When complications were present, they were temporary.

Objective The objective of this systematic review was to evaluate the efficacy of non-surgical periodontal therapy on rheumatoid arthritis activity. Material and methods Articles published until April 2019 were electronically searched and screened using PubMed / MEDLINE, Scopus, and Cochrane databases according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines (PRISMA). This study was recorded in the international PROSPERO database (CRD42019132205). The PICO question (population, intervention, comparison, results) was: in adult patients with rheumatoid arthritis and periodontitis (P), does non-surgical periodontal treatment (I), as compared to no treatment (C), provides better outcomes in rheumatoid arthritis activity (O). Results After searching the databases, seven articles were selected for qualitative and five for quantitative analysis. The total number of participants included was 292, with an average age of 50.5 years. All patients had rheumatoid arthritis and periodontal disease. Non-surgical periodontal treatment significantly reduced Disease Activity Score 28 (P = 0.004; I2 = 92%) and erythrocyte sedimentation rate (P = 0.01; I2 = 78%), but with no significant effect on C-reactive protein (P = 0.34; I2 = 92%). Conclusions It can be concluded that non-surgical periodontal treatment can benefit patients with rheumatoid arthritis.

Statement of problem One-step dental implant surgery with a 1-piece implant has been introduced with the aim of simplifying and increasing the effectiveness of treatment and providing greater comfort for the patient. Whether these goals have been met compared with conventional 2-piece implants remains unclear. Purpose The purpose of this systematic review and meta-analysis was to compare the use of 1-piece versus 2-piece implants in terms of marginal bone loss and implant survival rate. Material and methods This systematic review and meta-analysis adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and was recorded in the International prospective register of systematic reviews (PROSPERO) database (CRD 42018095721). A literature search was conducted in the PubMed/MEDLINE, Web of Science, and Cochrane Library databases for articles published up to May 2018. The population, intervention, comparison, outcome (PICO) question was, “Do patients who received 1-piece implants show similar marginal bone loss, survival rates, and complications as those who receive 2-piece implants?” Results The meta-analysis was based on the Mantel-Haenszel and inverse variance methods. Implant survival rate was analyzed by using a dichotomous outcome, measured according to risk ratio (RR) and marginal bone loss by continuously evaluating the outcomes according to the mean difference (MD), both with a corresponding 95% confidence interval (CI). Five studies, including 270 participants with a mean age of 54.70 years and receiving 434 dental implants, were included. The mean follow-up period was 4 years. Meta-analysis did not reveal a significant difference in relation to implant survival rate (P=.85; RR: −0.89; CI: −0.27 to 2.98), as well as to marginal bone loss (P=.85; MD: −0.43; CI: −0.43 to −0.52). Conclusions One- and 2-piece implants demonstrated effectiveness in the rehabilitation of patients requiring dental implants.

Photodynamic therapy is widely used in dentistry, but limited evidence exists regarding its effectiveness in treating denture stomatitis. High resistance to antifungals has been reported, and photodynamic therapy could be an alternative treatment.The purpose of this systematic review and meta-analysis was to evaluate whether photodynamic therapy is effective in reducing denture stomatitis.A systematic review was conducted based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) checklist and recorded in the prospective register of systematic reviews (PROSPERO) (CRD42020205589) to answer the population, intervention, control, outcome (PICO) question: "Is photodynamic therapy effective in the treatment of denture stomatitis when compared with the use of antifungal agents?" Electronic searches were performed in databases PubMed/MEDLINE, Cochrane library, and Web of Science for articles published until February 2021 by using the following terms: (denture stomatitis OR oral candidiasis) AND (low-level light therapy OR laser therapy OR lasers OR photodynamic therapies OR photochemotherapy) AND (antifungal drugs OR antifungal agents OR antimicrobial OR treatment). Clinical trials and randomized clinical trials, studies in the English language, and studies comparing antifungal agents with photodynamic therapy were included.In total, 5 articles were selected for the qualitative analysis and 3 for the meta-analysis. No significant difference was detected between antifungal therapy and photodynamic therapy in the reduction of colony-forming units on the palate. In a subgroup analysis, a significant difference was found in the reduction of colony-forming units on the palate at 15 days and at the denture surface at 30 days.Photodynamic therapy is effective in the treatment of denture stomatitis, but after 30 days and 15 days, the antifungals demonstrated better performance.

The aim of the present study was to perform a systematic review and meta-analysis to assess the influence of bariatric surgery on the clinical periodontal conditions in patients with obesity. This review was based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses and registered at the International Prospective Registry of Systematic Reviews (CRD42018099313). A search was conducted by 2 investigators in the PubMed/MEDLINE, Web of Science, and Cochrane Library databases for relevant articles published up to May 2018. The inclusion criteria were randomized controlled trials, prospective and retrospective studies, observational studies, longitudinal studies, and cohort studies with at least 3 months of follow-up. No language restrictions were imposed. The exclusion criteria were studies that did not evaluate or report the periodontal measurements, cross-sectional studies (without follow-up after surgery), studies that performed periodontal treatment, and those with insufficient periodontal data. The meta-analysis was based on the Mantel-Haenszel method and inverse variance. The quantitative analysis revealed no statistically significant differences with regard to bleeding on probing (P = .9; mean deviation: −.70; confidence interval = −11.43 to 10.04) or probing pocket depth (P = .41; mean deviation: −.46; confidence interval = −1.55 to .63) before and after intervention. Clinical attachment loss showed a statistically significant difference (P = .0002; mean deviation: .18; confidence interval = .07–.30). It can be concluded that bariatric surgery does not influence bleeding on probing or probing pocket depth, but leads to a worsening of clinical attachment loss.

However, clinical evidence to help clinicians make this decision is lacking. Purpose: The purpose of this systematic review and meta-analysis was to assess the marginal bone loss, implant survival rate, and prosthetic complications of splinted and nonsplinted implant restorations. Material and methods: This study was designed according to the Cochrane criteria for elaborating a systematic review and meta-analysis and adopted the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. Also, this review was registered at the International Prospective Register of Systematic Reviews (PROSPERO) (CRD42017080162). An electronic search in the PubMed/MEDLINE, Cochrane Library, and Scopus databases was conducted up to November 2017. A specific clinical question was structured according to the population, intervention, comparison, outcome (PICO) approach. The addressed focused question was “Should the restoration of adjacent implants be splinted or nonsplinted?” The meta-analysis was based on the Mantel-Haenszel and inverse variance methods to assess the marginal bone loss, implant survival, and prosthetic complications of splinted and nonsplinted implant restorations. Results: Nineteen studies were selected for qualitative and quantitative analyses. A total of 4215 implants were placed in 2185 patients (splinted, 2768; nonsplinted, 1447); the mean follow-up was 87.8 months (range=12-264 months). Quantitative analysis found no significant differences between splinted and nonsplinted restorations for marginal bone loss. The assessed studies reported that 75 implants failed (3.4%), of which 24 were splinted (99.1% of survival rate) and 51 were nonsplinted (96.5% of survival rate). Quantitative analysis of all studies showed statistically significant higher survival rates for splinted restorations than for nonsplinted restorations. Ceramic chipping, screw loosening, abutment screw breakage, and soft tissue inflammation were reported in the selected studies. The quantitative analysis found no statistically significant difference in the prosthetic complications of splinted and nonsplinted restorations. Conclusions: Within the limitations of this systematic review and meta-analysis, it was concluded that there was no difference in the marginal bone loss and prosthetic complications of splinted and nonsplinted implant restorations; this is especially true for restorations in the posterior region. However, splinted restorations were associated with decreased implant failure. No consensus has been reached on the use of dental implants in human immunodeficiency virus (HIV)-positive patients. This systematic review evaluated dental implants in HIV-positive patients in terms of implant survival and success rates, marginal bone loss, and complications. The review was conducted according to the PRISMA checklist. Two independent reviewers performed a comprehensive search of the PubMed/MEDLINE, Scopus, and Cochrane Library databases for studies published until October 2017. Six studies were selected for review. In total, 821 implants were placed: 493 in 169 HIV-positive patients, and 328 in 135 HIV-negative patients. The mean duration of follow-up was 47.9 months. Weighted mean survival rate, success rate, and marginal bone loss values were calculated for the HIV-positive patients. Mean survival and success rates at the patient level (according to the number of patients) were 94.76% and 93.81%, respectively; when calculated at the implant level (according to the number of implants), these rates were 94.53% and 90.37%, respectively. Mean marginal bone loss was 0.83mm at the patient level and 0.99mm at the implant level. Thus, dental implants are suitable for the rehabilitation of HIV-positive patients with controlled risk factors and normal CD4+ cell counts.

No. of bitstreams: 0 Previous issue date: 2018-09-01 Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) Conselho Nacional de Desenvolvimento Científico e Tecnológico (CNPq) The purpose of this study was to analyze the stress distribution of bone tissue around implants with different implant-abutment interfaces: platform switching (PSW); external hexagon (EH) and Morse taper (MT) with different diameters (regular: Ø 4 mm and wide: Ø 5 mm), bone types (I–IV) and subjected to axial and oblique load conditions using three-dimensional finite element analysis (3D-FEA). Sixteen 3D models of various configurations were simulated using InVesalius, Rhinoceros 3D 4.0, and SolidWorks 2011 software, and processed using Femap 11.2 and NeiNastran 11.0 programs. Axial and oblique forces of 200 N and 100 N, respectively, applied at the occlusal surface of prostheses. Maximum principal stress values were obtained from the peri-implant cortical bone of each model. Statistical analyses were performed using ANOVA and Tukey's test for maximum principal stress values. Oblique loading showed higher tensile stress than axial loading (P < 0.001). Wide-diameter implants showed lower stress concentration rather than regular-diameter implants, regardless of both connection and bone type (P < 0.001). Under axial loading, wide-diameter EH implants with regular platforms showed more favorable stress distribution than PSW implants for axial loading (P < 0.001); however, under oblique loading, PSW implants exhibited lower stress concentrations (P < 0.001). Regular-diameter MT implants showed lower stress than EH implants (P < 0.001). Bone type IV showed higher stress in the cortical region than bone types I and II (P < 0.001), but no significant difference when compared with bone type III (P > 0.05). The conclusion drawn from this in silico is that MT implants should be considered for use in situations that preclude the placement of wide-diameter implants, particularly where bone types III and IV are concerned. The aim of this systematic review and meta-analysis was to compare the survival rate of the implants and the peri-implant tissue changes associated with implants inserted in fresh extraction sockets and those inserted in healed sockets. This review has been registered at PROSPERO under the number CRD42016043309. A systematic search was conducted by two reviewers independently in the databases PubMed/MEDLINE, Embase, and the Cochrane Library using different search terms; articles published until November 2016 were searched for. The searches identified 30 eligible studies. A total of 3,049 implants were installed in a total of 1,435 patients with a mean age of 46.68 years and a minimum of 6 months of follow-up. The survival rate of delayed implants (98.38%) was significantly greater than immediate implants (95.21%) ( p =.001). For the marginal bone loss ( p =.32), implant stability quotients values ( p =.44), and pocket probing depth ( p =.94) there was no significant difference between the analysed groups. The immediate implants placed in fresh sockets should be performed with caution because of the significantly lower survival rates than delayed implants inserted in healed sockets.

Purpose: The purpose of this 3D finite element analysis (FEA) study was to evaluate the biomechanical behavior of 3-unit implant-supported prostheses under parafunctional forces with and without an occlusal device. Materials and methods: Eight 3D models consisting of a posterior (type IV) maxillary bone block with 3 external hexagon implants (Ø4.0×7.0 mm) and 3-unit implant-supported prostheses with different crown connections (splinted or unsplinted) and an occlusal device under functional and parafunctional loading were simulated. The abutment screws were evaluated by von Mises stress maps, and the bone tissue by maximum principal stress and microstrain maps by using a finite element software program. Results: An occlusal device improved the biomechanical behavior of the prostheses by reducing stress in the abutment screws and stress and strain in the bone tissue. However, the use of an occlusal device was not sufficiently effective to negate the biomechanical benefit of splinting. Conclusions: The use of splinted crowns in the posterior maxillary region with an occlusal device was the most effective method of reducing stress in the abutment screws and stress and strain in the bone tissue when parafunction was modeled. This systematic review evaluated the efficacy of the morning fasting serum C-terminal telopeptide (CTX) test in predicting the development of bisphosphonate-related osteonecrosis of the jaw (BRONJ). A comprehensive search of studies published up to March 2016, and listed in the PubMed/MEDLINE, Web of Science, and Cochrane Library databases, was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. This review has been registered in the PROSPERO international prospective register of systematic reviews (CRD42016036717). The search identified 542 publications; eight studies were finally deemed eligible for inclusion according to the study criteria. These studies included a total 1442 patients (mean age 66.7 years). The most prescribed drug was alendronate, with osteoporosis being the most frequent indication for the prescription of bisphosphonates. Tooth extraction was the most common trigger for BRONJ. Of all patients evaluated after bisphosphonate treatment, only 24 (1.7%) developed BRONJ. All eight of the selected studies found that CTX levels were not predictive of the development of BRONJ. In conclusion, this systematic review indicates that the CTX test has no predictive value in determining the risk of osteonecrosis in patients taking bisphosphonates.

Statement of problem No consensus has been reached on which retention system, cement- or screw-retained, is best to avoid bone loss around the implant of a fixed implant-supported restoration. Purpose The purpose of this systematic review and meta-analysis was to compare cement- and screw-retained retention systems in fixed implant-supported restorations in terms of marginal bone loss, implant survival, and prosthetic complications. Material and methods A comprehensive search of studies published from January 1995 to March 2015 and listed in the PubMed/MEDLINE, Embase, Scopus and the Cochrane Library databases was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. The meta-analysis was based on the Mantel-Haenszel and inverse variance methods. Marginal bone loss was the continuous outcome measure evaluated by mean difference (MD), and implant survival and prosthetic complications were the dichotomous outcome measures evaluated by risk ratio (RR), both with corresponding 95% confidence intervals (CI). Results The 20 studies selected for review evaluated 2139 participants, whose mean age was 47.14 years and who had received 8989 dental implants. The mean follow-up was 65.4 months (range: 12-180 months). Results of the MD for marginal bone loss showed statistically significant differences in favor of the cement-retained prosthesis ( P =.04; MD: −0.19; CI: −0.37 to −0.01). The implant survival rate was higher for the cement-retained prosthesis ( P =.01; RR: 0.49; CI: 0.28 to 0.85), and the prosthetic complication rate was higher for the screw-retained prosthesis ( P =.04; RR: 0.52; CI: 0.28 to 0.98). Additional analysis of the mean plaque index did not show differences between retention systems ( P =.58; MD: 0.13; CI: −0.32 to 0.57). Conclusions The current meta-analysis indicated that cement-retained, fixed implant-supported restorations showed less marginal bone loss than screw-retained, fixed implant-supported restorations during the follow-up period, which ranged from 12 to 180 months. However, the small difference between the mean values may not show clinical significance. The rates of prosthetic complication and implant survival also compared favorably with cement-retained prostheses.

This systematic review evaluated the effect on bone formation and implant survival of combining platelet-rich plasma (PRP) with bone grafts in maxillary augmentation. A comprehensive review of articles listed in the PubMed/MEDLINE, Embase, and Cochrane Library databases covering the period January 2000 to January 2015 was performed. The meta-analysis was based on bone formation for which the mean difference (MD, in millimetres) was calculated. Implant survival was assessed as a dichotomous outcome and evaluated using the risk ratio (RR) with 95% confidence interval (CI). The search identified 3303 references. After inclusion and exclusion criteria were applied, 17 studies were selected for qualitative analysis and 13 for quantitative analysis. A total of 369 patients (mean age 51.67 years) and 621 maxillary sinus augmentations were evaluated. After the data analysis, additional analyses were performed of the implant stability quotient, marginal bone loss, and alveolar bone height measured by MD. The results showed no significant difference in implant stability (P=0.32, MD 1.00, 95% CI -0.98 to 2.98), marginal bone loss (P=0.31, MD 0.06, 95% CI -0.05 to 0.16), alveolar bone height (P=0.10, MD -0.72, 95% CI -1.59 to 0.14), implant survival (P=0.22, RR 1.95, 95% CI 0.67-5.69), or bone formation (P=0.81, MD -0.63, 95% CI -5.91 to 4.65). In conclusion, the meta-analysis indicates no influence of PRP with bone graft on bone formation and implant survival in maxillary sinus augmentation.

No. of bitstreams: 0 Previous issue date: 2016-03-01 The aim of this study was to perform a systematic review and meta-analysis to evaluate the possible benefits of platform-switching (PSW) implants when compared to regular platform (RP) implants in the categories of bone preservation and longevity. This systematic review and meta-analysis was performed in accordance with the PRISMA statement, PICO question, and Jadad scale. The relative risk (RR) of failure and the mean difference for marginal bone loss were calculated considering a confidence interval (CI) of 95%. Heterogeneity and subgroup analyses were performed, and funnel plots drawn. Twenty-five studies (17 randomized controlled trials (RCTs) and eight prospective studies) involving 1098 patients and 2310 implants were analysed. The meta-analysis revealed a significant reduction in crestal bone loss for PSW implants compared with RP implants (-0.41 mm, 95% CI -0.52 to -0.29, P < 0.00001). However, there was no statistically significant difference in implant failure (RR 1.10, 95% CI 0.6-2.02, P = 0.75). A reduction in bone loss with PSW implants was observed for the following subgroups: RCTs only, implants in the maxilla, and implants in the mandible. PSW implants presented lower bone resorption compared with RP implants. RCTs should be done to explain the possible biases. MATERIAL AND METHODS: Thirty-nine patients were analyzed in 3 call centers to determine anxiety levels, pain levels, and preoperative and postoperative histories using the State–Trait (STAI) questionnaire. RESULTS: A total of 93 dental implants were installed, with a success rate of 100%. The most frequently reported systemic disease was hypertension. There was a significantly higher rate of effective clamping (torque) to the mandibular bone than to the maxillary bone. The association between postoperative surgical complications and longer operative time was not significant, but there was a significant correlation between the alteration of mouth opening and daily routine activities and a significant decrease in anxiety levels between the day of surgery and the postoperative time point (p=0.006). CONCLUSION: A longer surgical time was associated with surgery-related complications and with a higher anxiety index on the preoperative evaluation. Clinicians should consider parafunctional occlusal load when planning treatment. Prosthetic connections can reduce the stress distribution implant-supported prosthesis. Purpose. The purpose of this 3-dimensional finite element study was to parafunctional loading and prosthetic connections on stress distribution. Material and methods. Computer-aided design software was used to construct 3 models. Each model was composed of a bone and an implant (external hexagon, internal hexagon, or Morse taper) with a crown. Finite element analysis software was used to generate the finite element mesh and establish the loading and boundary conditions. A normal force (200-N axial load and 100-N oblique load) and parafunctional force (1000-N axial and 500-N oblique load) were applied. Results were visualized as the maximum principal stress. Three-way analysis of variance and Tukey test were performed, and the percentage of contribution of each variable to the stress concentration was calculated from sum-of squares-analysis. Results. The aim of this study was to analyze the stress distribution of short implants supporting single unit or splinted crowns by the photo-elasticity method. Four photo-elastic models were produced: A (3.75×7 mm); B (3.75×7 mm, 3.75×7 mm and 3.75×7 mm); C (3.75×10 mm, 3.75×7 mm and 3.75×7 mm); D (3.75×13 mm, 3.75×7 mm and 3.75×7 mm). The prostheses were made with Ni-Cr alloy. A load of 100 N in the axial and oblique directions was applied, totaling 380 applications, individually capturing their images in each model. The data were randomized and analyzed qualitatively and quantitatively by 2 examiners. The oblique loading was significantly more damaging. The increase in length was favorable for stress distribution (p0.05). The splinting was beneficial for the transmission of stresses mainly (p0.05). The splinting of the crowns, as well as increasing the length of the first implant and axial loading was most beneficial in the stress distribution. Short splinted implants behaved better than single unit implants. Increasing of the length of the first implant significantly improved the stress distribution in all analyzed situations.

The study of short implants is relevant to the biomechanics of dental implants, and research on crown increase has implications for the daily clinic. The aim of this study was to analyze the biomechanical interactions of a singular implant-supported prosthesis of different crown heights under vertical and oblique force, using the 3-D finite element method. Six 3-D models were designed with Invesalius 3.0, Rhinoceros 3D 4.0, and Solidworks 2010 software. Each model was constructed with a mandibular segment of bone block, including an implant supporting a screwed metal-ceramic crown. The crown height was set at 10, 12.5, and 15 mm. The applied force was 200 N (axial) and 100 N (oblique). We performed an ANOVA statistical test and Tukey tests; p0.05 was considered statistically significant. The increase of crown height did not influence the stress distribution on screw prosthetic (p0.05) under axial load. However, crown heights of 12.5 and 15 mm caused statistically significant damage to the stress distribution of screws and to the cortical bone (p0.001) under oblique load. High crown to implant (C/I) ratio harmed microstrain distribution on bone tissue under axial and oblique loads (p0.001). Crown increase was a possible deleterious factor to the screws and to the different regions of bone tissue.

The objective of this systematic review is to evaluate the effect of the incorporation of zirconia (ZrO2) particles on the mechanical properties of PMMA (polymethyl methacrylate), and to establish which characteristics of this material yield the best results aiming their biomedical applicability. This study was carried out in accordance with the Items of Preferred Reports for Systematic Reviews and Meta-Analyzes (PRISMA) and is registered in PROSPERO under registration number: CRD42018095801. The searches were carried out in the PubMed, MEDLINE, Scopus and Cochrane Library databases for articles published up until April 2018. After the different stages of the article selection process, eight articles were selected for qualitative and quantitative analysis. All were in vitro studies, totaling 536 evaluated samples. The concentrations of zirconia ranged from 0.5% to 20% and the particle sizes were between 15 nm and 10 μm. The incorporation of zirconia particles did not increase the flexural strength of PMMA in only one study. While the concentration of zirconia influences PMMA, the type of acrylic resin, size, and silanization of zirconia particles did not influence the results. Thus, the addition of zirconia particles showed a positive effect on PMMA enhancing their use in the medical and dental field, especially when certain anatomical areas requires higher strength of the materials, providing longevity for the rehabilitation.

The aim of this study was to evaluate stress distribution in the fixation screws and bone tissue around implants in single-implant supported prostheses with crowns of different heights (10, 12.5, 15 mm - crown-to-implant ratio 1:1, 1.25:1, 1.5:1, respectively). It was designed using three 3-D models. Each model was developed with a mandibular segment of bone block including an internal hexagon implant supporting a screw-retained, single metal-ceramic crown. The crown height was set at 10, 12.5, and 15 mm with crown-to-implant ratio of 1:1, 1.25:1, 1.5:1, respectively. The applied forces were 200N (axial) and 100 N (oblique). The increase of crown height showed differences with the oblique load in some situations. By von Mises' criterion, a high stress area was concentrated at the implant/fixation screw and abutment/implant interfaces at crown-to-implant ratio of 1:1, 1.25:1, 1.5:1, respectively. Using the maximum principal criteria, the buccal regions showed higher traction stress intensity, whereas the distal regions showed the largest compressive stress in all models. The increase of C/I ratio must be carefully evaluated by the dentist since the increase of this C/I ratio is proportional to the increase of average stress for both screw fixation (C/I 1:1 to 1:1.25 ratio=30.1% and C/I 1:1 to 1:1.5 ratio=46.3%) and bone tissue (C/I 1:1 to 1:1.25 ratio=30% and C/I 1:1 to 1:1.5 ratio=51.5%).

The purpose of this systematic review was to evaluate clinical studies on the follow-up survival of implants inserted in the zygomatic bone for maxillary rehabilitation. A comprehensive search of studies published from 2000 to July 2012 and listed in the PubMed/MEDLINE, Embase, and Cochrane Library databases was performed in accordance with the PRISMA statement. Relevant studies were selected according to predetermined inclusion and exclusion criteria. The initial database search yielded 751 titles. After filtering, 313 abstracts were selected, culminating in 42 full text articles. Application of eligibility criteria led to the elimination of 17 articles. Hence 25 full-text articles were considered clinically relevant and were included. Calculations of the interval survival rates and cumulative survival rates of implants could be carried out on the data extracted from the final list of included studies for the different time intervals. These studies reported the insertion of a total of 1541 zygomatic implants and 33 implant failures. Failure generally occurred during the first year interval and was related to clinical complications, such as recurrent acute and chronic sinusitis. After a 36-month follow-up, the survival rate was 97.86%. Additional studies with longer follow-up periods, including the number of zygomatic implants inserted and details of the variations in the surgical techniques used and the impact of the maxillary morphology are still required.

Statement of problem There is insufficient evidence to recommend the restorative material for implant-supported prostheses. Purpose The purpose of this systematic review and meta-analysis was to evaluate studies that compared ceramic and metal-ceramic restorations for implant-supported prostheses (within the same study to avoid indirect comparison) in terms of the mechanical and biological complication rates, prosthesis survival rate, and marginal bone loss. Material and methods Two independent reviewers performed a comprehensive search in databases (PubMed/MEDLINE, Web of Science, and Cochrane Library) for articles indexed until March 31, 2018. The search was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement and methods were registered in the International Prospective Register of Systematic Reviews (PROSPERO). The focused question was “Do ceramic restorations have mechanical/biological complication rates, prosthesis survival rates, and marginal bone loss similar to those of metal-ceramic restorations?” Results The search identified 949 references. The interinvestigator agreement using kappa values was 0.87 for PubMed/MEDLINE, 0.93 for Scopus, and 1.0 for the Cochrane Library. After analysis, 12 studies were selected for qualitative and quantitative analysis. The mechanical complication rate did not differ between ceramic and metal-ceramic restorations (P=.89), independent of the type of prostheses (single crown: P=.63; fixed partial denture: P=.65). The biological complication rate was also not significantly different between ceramic and metal-ceramic restorations (P=.21). The prosthesis survival rate showed no significant differences between the 2 types of restorations (P=.56). Marginal bone loss was also similar for both types of restorations (P=.12). Conclusions This systematic review indicated that ceramic and metal-ceramic implant-supported prostheses have similar mechanical and biological complication rates, prosthesis survival rates, and marginal bone loss. Thus, both treatments are appropriate options for long-term rehabilitation treatment.

Purpose The purpose of this study was to investigate color change of irises obtained by both printed digital image and painted with gouache, acrylic and oil paints, after polymerization and accelerated aging. Materials and methods Eight samples simulating ocular prostheses were fabricated. Each sample was constituted by one disc of N1 colored acrylic resin and one disc of colorless acrylic resin with the iris interposed between the discs. The irises in brown and blue color were obtained by painting or by digital image. The specimens’ colors were measured with a spectrophotometer using the CIE L*a*b* system, at baseline (B), after polymerization (P), and after 504 h (A1) and 1008 h (A2) of accelerated aging. The data were evaluated by 2-way repeated-measures ANOVA and the Tukey's HSD test (α = .05). Results Color change was observed in all samples both after polymerization and after accelerated aging. The different periods of accelerated aging did not influence on color change for the irises painted with blue oil paint. The polymerization promoted a statistical significant effect on the iris color stability for all painting techniques in comparison to all accelerated aging periods. Conclusion After accelerated aging the oil painting technique showed the greatest color stability while the printed digital images technique the worst.

Purpose The aim of this systematic review was to evaluate the effect of corticosteroid (CS) administration on edema and neural regeneration in orthognathic surgery. Materials and Methods A systematic literature search was conducted using the PubMed (Medline), Cochrane Library, and Scopus databases. The PICO approach was used, in which patients with skeletal dentofacial deformity composed the population; uni- or bimaxillary orthognathic surgery composed the intervention; administration versus no administration of CSs composed the comparison; and decrease in postoperative edema and neurosensory disorders composed the outcome. Thirty of 240 articles were selected and evaluated for their titles and abstracts in relation to the inclusion and exclusion criteria. After duplicate references were eliminated, 8 articles remained. Results Patients who used CSs had lower rates of edema. In fact, after 4 months, there were no remarkable edema rates. These results suggest that neurosensory disorders resolved after 3 months. In addition, in the early and late periods, administration of CSs did not influence the regression of neurosensory disorders. Conclusions Administering CSs in orthognathic surgery improved the regression of facial edema independent of the dosage used but did not influence neurosensory disorders.

Statement of problem Different factors can influence marginal bone loss around dental implants, including the type of internal and external connection between the implant and the abutment. The evidence needed to evaluate these factors is unclear. Purpose The purpose of this systematic review was to evaluate marginal bone loss by radiographic analysis around dental implants with internal or external connections. Material and methods A systematic review was conducted following the criteria defined by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Initially, a population, intervention, comparison, and outcome(s) (PICO) question was defined: does the connection type (internal or external) influence marginal bone loss in patients undergoing implantation? An electronic search of PubMed/MEDLINE and Scopus databases was performed for studies in English language published between January 2000 and December 2014 by 2 independent reviewers, who analyzed the marginal bone loss of dental implants with an internal and/or external connection. Results From an initial screening yield of 595 references and after considering inclusion and exclusion criteria, 17 articles were selected for this review. Among them, 10 studies compared groups of implants with internal and external connections; 1 study evaluated external connections; and 6 studies analyzed internal connections. A total of 2708 implants were placed in 864 patients. Regarding the connection type, 2347 implants had internal connections, and 361 implants had external connections. Most studies showed lower marginal bone loss values for internal connection implants than for external connection implants. Conclusions Osseointegrated dental implants with internal connections exhibited lower marginal bone loss than implants with external connections. This finding is mainly the result of the platform switching concept, which is more frequently found in implants with internal connections.
