Abstract

Biofortification of staple crops with higher levels of micronutrients via traditional breeding methods is a sustainable strategy and can possibly complement fortification and other interventions to target micronutrient deficiencies in low resource settings, particularly among vulnerable populations such as children. We aimed to determine if iron- and zinc-biofortified pearl millet (FeZnPM, Dhanashakti, ICTP-8203Fe)-based complementary feeding improves nutritional status, including iron biomarkers and growth, in children living in urban slums of Mumbai.We conducted a randomized controlled trial of FeZnPM among 223 children aged 12-18 months who were not severely anemic at baseline (hemoglobin ≥9.0 g/dL). Children were randomized to receive either FeZnPM or conventional non-biofortified pearl millet (CPM) daily for 9 months. Iron status (hemoglobin, serum ferritin), plasma zinc, and anthropometric indicators (length, weight, mid-upper arm circumference, triceps and subscapular skinfolds) were evaluated at enrollment and throughout the trial. World Health Organization (WHO) anthropometric z-scores were calculated using WHO growth standards. Primary outcomes were hemoglobin and serum ferritin concentrations, and growth, defined as WHO z-scores. An intent to treat approach was used for analyses. We used the Hodges-Lehmann-Sen test to assess the change in primary outcomes between baseline and the last visit and report corresponding 95% confidence intervals.At baseline, 67.7% of children were anemic (hemoglobin11.0 g/dL) and 59.6% were iron deficient (serum ferritin12.0 μg/L). FeZnPM did not significantly increase iron biomarkers or improve growth, compared to CPM. In subgroup analyses, FeZnPM improved hemoglobin concentrations in male children, and in children with iron deficiency or iron depletion (serum ferritin25.0 μg/L) at baseline, relative to CPM.Daily consumption of FeZnPM-based complementary foods did not significantly impact iron and zinc status or growth in children living in Mumbai's urban slums. 2. A Randomized Trial of Quadruple-Fortified Salt for Anemia and Birth Defects Prevention in Southern India: Protocol Design and Methods

Abstract

BACKGROUND: Women of reproductive age are at an increased risk of anemia and micronutrient deficiencies. Evidence supports the role of periconceptional nutrition in the development of neural tube defects (NTDs) and other pregnancy complications. Vitamin B(12) deficiency is a risk factor for NTDs and may modify folate biomarkers that predict NTD risk at the population level. There is an interest in mandatory fortification with vitamin B(12) and folic acid for anemia and birth defect prevention. However, there are limited population-representative data needed to inform policy and guidelines. OBJECTIVES: This randomized trial will be conducted to evaluate the efficacy of quadruple-fortified salt (QFS; iron, iodine, folic acid, vitamin B(12)) in 1,000 households in Southern India. METHODS: Women 18 to 49 y who are not pregnant or lactating and reside within the catchment area of our community-based research site in Southern India will be screened and invited to participate in the trial. After informed consent, women and their households will be randomized to receive one of the following 4 interventions: 1) double-fortified salt (DFS; iron, iodine), 2) DFS + folic acid (iron, iodine, folic acid), 3) DFS + vitamin B(12) (iron, iodine, vitamin B(12)), or 4) DFS + folic acid and vitamin B(12) (QFS; iron, iodine, folic acid, vitamin B(12)) for 12 mo. Structured interviews will be conducted by trained nurse enumerators to collect sociodemographic, anthropometric, dietary, health, and reproductive history data. Biological samples will be collected at baseline, midpoint, and endpoint. Whole blood will be analyzed for hemoglobin using Coulter Counter. Total vitamin B(12) will be measured by chemiluminescence; red blood cell folate and serum folate will be evaluated using the World Health Organization-recommended microbiologic assay. CONCLUSIONS: The results of this randomized trial will help to evaluate the efficacy of QFS to prevent anemia and micronutrient deficiencies. Abstract

BackgroundThe use of opioids is increasing globally, but data from low- and middle-income countries on opioid-related mental and behavioural disorders (hereafter referred to as opioid-related disorders) are scarce. This study examines the incidence of opioid-related disorders, opioid agonist use, and excess mortality among persons with opioid-related disorders in South Africa’s private healthcare sector.MethodsWe analysed longitudinal data of beneficiaries (≥ 11 years) of a South African medical insurance scheme using reimbursement claims from Jan 1, 2011, to Jul 1, 2020. Beneficiaries were classified as having an opioid-related disorder if they received an opioid agonist (buprenorphine or methadone) or an ICD-10 diagnosis for harmful opioid use (F11.1), opioid dependence or withdrawal (F11.2-4), or an unspecified or other opioid-related disorder (F11.0, F11.5-9). We calculated adjusted hazard ratios (aHR) for factors associated with opioid-related disorders, estimated the cumulative incidence of opioid agonist use after receiving an ICD-10 diagnosis for opioid dependence or withdrawal, and examined excess mortality among beneficiaries with opioid-related disorders.ResultsOf 1,251,458 beneficiaries, 1,286 (0.1%) had opioid-related disorders. Between 2011 and 2020, the incidence of opioid-related disorders increased by 12% (95% CI 9%-15%) per year. Men, young adults in their twenties, and beneficiaries with co-morbid mental health or other substance use disorders were at increased risk of opioid-related disorders. The cumulative incidence of opioid agonist use among beneficiaries who received an ICD-10 diagnosis for opioid dependence or withdrawal was 18.0% (95% CI 14.0-22.4) 3 years after diagnosis. After adjusting for age, sex, year, medical insurance coverage, and population group, opioid-related disorders were associated with an increased risk of mortality (aHR 2.28, 95% CI 1.84-2.82). Opioid-related disorders were associated with a 7.8-year shorter life expectancy.ConclusionsThe incidence of patients diagnosed with or treated for an opioid-related disorder in the private sector is increasing rapidly. People with opioid-related disorders are a vulnerable population with substantial psychiatric comorbidity who often die prematurely. Evidence-based management of opioid-related disorders is urgently needed to improve the health outcomes of people with opioid-related disorders.

6. Cancer treatment and survival among cervical cancer patients living with or without HIV in South Africa

Abstract

Objective To compare cancer treatment and all-cause mortality between HIV-positive and HIV-negative cervical cancer patients in South Africa. Methods We assessed cancer treatment and all-cause mortality in HIV-positive and HIV-negative cervical cancer patients who received cancer treatment within 180 days of diagnosis using reimbursement claims data from a private medical insurance scheme in South Africa between 01/2011 and 07/2020. We assessed treatment provision using logistic regression and factors associated with all-cause mortality using Cox regression. We assigned missing values for histology and ethnicity using multiple imputation. Results Of 483 included women, 136 (28 %) were HIV-positive at cancer diagnosis (median age: 45.7 years), and 347 (72 %) were HIV-negative (median age: 54.1 years). Among 285 patients with available ICD-O-3 morphology claims codes, the proportion with cervical adenocarcinoma was substantially lower in HIV-positive (4 %) than in HIV-negative patients (26 %). Most HIV-positive patients (67 %) were on antiretroviral therapy at cancer diagnosis. HIV-positive patients were more likely to receive radiotherapy (adjusted odds ratio [aOR] 1.90, 95 % confidence interval [CI] 1.05-3.45) or chemotherapy (aOR 2.02, 95 %CI 0.92-4.43) and less likely to undergo surgery (aOR 0.53, 95 %CI 0.31-0.90) than HIV-negative patients. HIV-positive patients were at a higher risk of death from all causes than HIV-negative patients (adjusted hazard ratio 1.52, 95 %CI 1.06-2.19). Other factors associated with higher all-cause mortality included age > 60 years and metastases at diagnosis. Conclusions HIV-positive cervical cancer patients in South Africa had higher all-cause mortality than HIV-negative patients which could be explained by differences in tumour progression, clinical care, and HIV-specific mortality.

Abstract

In this review, the influence of seasonal variation on undernutrition during the first 1000 days of life of life in rural South Asia is conceptualized using a modified framework developed under the “Tackling the Agriculture and Nutrition Disconnect in India” project. Evidence for the existence and extent of seasonality is summarized from 14 studies reporting on six proximal determinants of undernutrition. A limited number of studies examine seasonal variation in risk factors for this age group. All available studies, however, report a compelling finding of significant seasonal variation for at least one determinant of undernutrition. Research to clarify mechanisms for potentially adverse effects of seasonal variation on health and nutritional status during the first 1000 days of life is needed.

8. Robotic SBRT in Prostate Cancer Patients Younger Than 50 Years Old

Abstract

PURPOSE/OBJECTIVE(S) Stereotactic Body Radiation Therapy (SBRT) is a standard therapeutic option for men with prostate adenocarcinoma. The median age of prostate cancer in the US is 66 but patients as young as 35 have been reported. Many younger patients will have surgery rather than SBRT for localized prostate cancer but some will be treated with SBRT. There is a paucity of data on the outcomes of this younger subset. This study reports outcomes on patients younger than 50 treated with SBRT at a single institution and compares outcomes to older patients. MATERIALS/METHODS Between April 2006 and December 2020, 3626 patients with prostate cancer were treated with inhomogeneous-dosed SBRT using a robotic linear accelerator and followed at an academic institution. 3173 (87.51%) of patients were treated with a median dose of 3500cGY (3500-3625) delivered over 5 consecutive fractions prescribed to the 83-85% isodose line, and the remaining 453 (12.49%) other patients receiving a median dose of 4500cGY (4500-5400) to the pelvis in conventional fractionation followed by a 3 fraction SBRT boost of 2100 cGY (1950-2100) over 3 consecutive fractions. Androgen deprivation Therapy (ADT) was prescribed in 865 (23.86%) of these cases. The mean age was 67.3 years old. 47 patients were younger than 50 years old (mean age 46.6). 3,579 patients were 50 or older. Patients were divided into prognostic D'Amico risk groups with 44.68%, 48.94%, 6.38% of patients falling in the low, intermediate, and high-risk stratifications in the younger cohort and 24.76%, 56.83%, 18.41% in the older cohort respectively. Pretreatment PSA was 1.72 - 43.2 (median: 5.4) in the younger group and 0.3 - 661 (median: 6.5) in the older group. In the younger group, Gleason scores were 6 in 48.94%, 7 in 46.81%, and 8-10 in 4.25%. 44 younger patients were treated with SBRT alone. 3 patients also received supplemental external beam radiation (median dose 4500cGY) and 5 patients (10.6%) received Androgen Deprivation Therapy (ADT) as part of their treatment regimen. In the older group, Gleason scores were 6 in 30.57%, 7 in 54.06%, and 8-10 in 15.37%. 3129 were treated with SBRT alone. 450 patients also received supplemental external beam radiation (median dose 4500cGY) and 860 patients (24.03%) received Androgen Deprivation Therapy (ADT) as part of their treatment regimen. RESULTS At 64.8 months (range 7 months - 177 months) the 5-year biochemical relapse free survival was 98% in younger patients compared to 99% in older patients using the Phoenix definition of biochemical failure. The 5-year median post treatment PSA was 0.15 in the younger patients and 0.20 in the older patients. There were no significant differences in biochemical relapse free survival between the groups. CONCLUSION This represents the largest series evaluating outcomes in very young patients treated with definitive SBRT for prostate cancer. With 5-year follow up, SBRT is an effective treatment for this younger subset of patients. Continued follow up will be required to see if these results remain durable.

Abstract

BACKGROUND Iron deficiency remains the most prevalent micronutrient deficiency globally, but few studies have examined how iron status relates to cognition in adolescents. Iron biofortification of staple food crops is being scaled up, yet it is unknown whether consuming biofortified crops can benefit cognition. Objective Our objective was to determine the efficacy of iron-biofortified pearl millet in improving attention and memory in Indian school-going adolescents. Methods A double-blind, randomized, intervention study was conducted in 140 Indian boys and girls, aged 12-16 y, who were assigned to consume iron-biofortified [Fe = 86 parts per million (ppm)] or conventional (Fe = 21-52 ppm) pearl millet. Hemoglobin, ferritin, and transferrin receptor (TfR) were measured and body iron (BI) was calculated at baseline and after 4 and 6 mo. Five measures of cognitive function were obtained at baseline and 6 mo: simple reaction time (SRT), Go/No-Go (GNG) task, Attentional Network Task (ANT), Composite Face Effect (CFE) task, and Cued Recognition Task (CRT). Intention-to-treat analysis was used. Results Daily iron intake from pearl millet was higher in those consuming biofortified compared with conventional pearl millet (19.6 compared with 4.8 mg/d). 10. Stereotactic Body Radiation Therapy for Ultra-Large (> 100 cc) Prostate Glands: Oncologic, Toxicity and Patient-Reported Outcomes

Abstract

Purpose/Objective(s) Historically, caution has been warranted when irradiating large target volumes particularly those in close proximity to organs at risk. Prior literature has demonstrated an increased incidence of GI and GU toxicity when men with large prostates were treated with conventionally fractionated radiation therapy. However, there is very limited data regarding the clinical outcomes when SBRT is used as a definitive treatment modality. We explore the long term oncologic, toxicity, and patient reported outcomes of men treated with definitive SBRT with ultra-large prostate glands (≥ 100 cc.) Materials/Methods From 2006 to 2020, a total of 3,393 patients with low and intermediate risk prostate cancer were treated with definitive robotic-SBRT. We performed a retrospective review to identify all patients in this cohort with pre-treatment prostate volumes ≥ 100 cc. Prostate volume was measured at the time of treatment regardless of ADT incorporation. All patients were treated to a total dose of 35-36.25 Gy in 5 fractions. All patients had a minimum of 2 years follow-up and were given pre- and post-treatment EPIC questionnaires at defined intervals. Biochemical control was assessed using the Phoenix definition. Late toxicity was defined using CTCAE version 5.0 and was characterized as occurring ≥ 6 months post treatment. Results A total of 67 patients were identified with ≥ 100 cc prostate glands. Of these, 18 patients received ADT prior to treatment. Overall, the median prostate volume was 139.37 cc (range 100.1 – 227 cc). The D'Amico risk classification was low (n = 19) and intermediate (n = 48). The median age was 70 years (range 54 – 87 years) and the median pretreatment PSA was 8.7 ng/ml. The mean pre-treatment EPIC bowel and urinary scores were 87.8 and 79.7, respectively. One-month following SBRT, mean EPIC bowel and urinary scores worsened to 83.6 and 76.5, respectively. Three months following SBRT, mean epic bowel and urinary scores continued to decline to 83.2 and 77.1, respectively. However, bowel and bladder symptomatology improved by 1 year to 86.16 and 77.19, and by 2 years improved above baseline to 90.00 and 85.78, respectively. There were no high grade (3+) GI toxicities observed, though one grade 3 urinary retention was identified. Excellent oncologic outcomes were observed with a 5-year median PSA nadir of 0.6 ng/mL and a biochemical relapse free survival (bRFS) of 100% at 5 years. Conclusion SBRT has been demonstrated to be oncologically effective with minimal toxicity, and has become a more ubiquitous radiation option in men with localized prostate cancer. Although there is a historical reticence for treatment of men with large glands, we report excellent clinical outcomes. Five-year bRFS was 100% and grade 3+ urinary toxicity was 2%. Although EPIC scores transiently dropped at 1 and 3 months following SBRT, resolution was seen by 1 year following treatment. The use of SBRT for the treatment of localized prostate cancer in men with ultra-large prostate gland is feasible with minimal toxicity.

Abstract

OBJECTIVES: To examine the associations between micronutrient status and child growth, as part of the National Health and Nutrition Survey in Mexico. METHODS: Data from the National Health and Nutrition Survey (ENSANUT 2018) were analyzed to examine the association between micronutrient status and child growth in 1,656 children 36 to 59 months of age. Abstract

Background The literature on correlates of nutrition has seen an increase in studies focused on functional consequences at the levels of neural, perceptual, and cognitive functioning. A range of measurement methodologies have been used in these studies, and investigators and funding agencies have raised the questions of how and if these various methodologies are at all comparable. Objective The aim was to determine the extent to which 3 different sets of cognitive measures provide comparable information across 2 subsamples that shared culture and language but differed in terms of socioeconomic status (SES) and academic preparation. Methods A total of 216 participants were recruited at 2 US universities. Each participant completed 3 sets of cognitive measures: 1 custom-designed set based on well-understood laboratory measures of cognition [cognitive task battery (COGTASKS)] and 2 normed batteries [Cambridge Neuropsychological Test Automated Battery (CANTAB), Weschler Adult Intelligence Scale, fourth edition (WAIS-IV)] designed for assessing general cognitive function. Results The 3 sets differed with respect to the extent to which SES and educational preparation affected the results, with COGTASKS showing no differences due to testing location and WAIS-IV showing substantial differences. There were, at best, weak correlations among tasks sharing the same name or claiming to measure the same construct. Conclusions Comparability of measures of cognition cannot be assumed, even if measures have the same name or claim to assess the same construct. In selecting and evaluating different measures, construct validity and underlying biological mechanisms need to be at least as important as population norms and the ability to connect with existing literatures.

Abstract

Sediment yield estimates, combined with information regarding sediment sources and partitioning (bedload vs. suspended load), can provide important insights into geomorphic character of the landscape of interest. This study reports such an analysis, conducted in a forested mountain basin in inland Pacific Northwest using reservoir deposits, ground surveys, and repeat LiDAR mapping. Our research generated three key conclusions. First, we estimated mean specific yield of clastic sediment from the study basin during the last century as ~51 Mg km−2 a−1. In the context of a data compilation from mountain basins in northwestern North America, this value was among the highest for inland basins, and among the lowest when compared with coastal basins. Second, a sediment source analysis provided valuable clues regarding the relative importance of various geomorphic processes operating in the system under study. These findings were consistent with our hypothesis that anthropogenic disturbances may be an important factor that conditions sediment yield from the basin. A mix of quantitative and qualitative evidence suggested that the estimated sediment yield value reflects a transient increase associated with past timber harvest, road construction, and large wood removal. Legacies of these disturbances appear to have the opposite effect on contemporary processes, limiting lateral activity of the channel as well as hillslope-channel connectivity. Third, our partitioning procedure revealed that bedload constituted approximately a third of the total clastic load exported from the basin. This finding suggests that, in this and similar fluvial systems, sediment yield recovery following major geomorphic disturbances could be protracted, as a considerable portion of mobilized sediment that moves as bedload is routed and evacuated from the basin for years or decades after rapid flushing of suspended material. Moreover, this finding indicates that the common assumption that bedload constitutes 10–20% of the total load can lead to underestimation in mountain basins.

Abstract

Summary Background The number of patients in need of second-line antiretroviral drugs is increasing in sub-Saharan Africa. We aimed to project the need of second-line antiretroviral therapy in adults in sub-Saharan Africa up to 2030. Methods We developed a simulation model for HIV and applied it to each sub-Saharan African country. We used the WHO country intelligence database to estimate the number of adult patients receiving antiretroviral therapy from 2005 to 2014. We fitted the number of adult patients receiving antiretroviral therapy to observed estimates, and predicted first-line and second-line needs between 2015 and 2030. We present results for sub-Saharan Africa, and eight selected countries. We present 18 scenarios, combining the availability of viral load monitoring, speed of antiretroviral scale-up, and rates of retention and switching to second-line. HIV transmission was not included. Findings Depending on the scenario, 8·7–25·6 million people are expected to receive antiretroviral therapy in 2020, of whom 0·5–3·0 million will be receiving second-line antiretroviral therapy. The proportion of patients on treatment receiving second-line therapy was highest (15·6%) in the scenario with perfect retention and immediate switching, no further scale-up, and universal routine viral load monitoring. In 2030, the estimated range of patients receiving antiretroviral therapy will remain constant, but the number of patients receiving second-line antiretroviral therapy will increase to 0·8–4·6 million (6·6–19·6%). The need for second-line antiretroviral therapy was two to three times higher if routine viral load monitoring was implemented throughout the region, compared with a scenario of no further viral load monitoring scale-up. For each monitoring strategy, the future proportion of patients receiving second-line antiretroviral therapy differed only minimally between countries. Interpretation Donors and countries in sub-Saharan Africa should prepare for a substantial increase in the need for second-line drugs during the next few years as access to viral load monitoring improves. OBJECTIVES: To determine a) the prevalence of protective measles IgG titers in young children living in urban slums of Mumbai, and b) the association of nutritional status assessed through a combination of anthropometry and micronutrient biomarker concentrations with these titers at baseline and after 9 months of follow-up. METHODS: Anthropometry and serum concentrations of vitamins A, B12, D; ferritin, and zinc, and anti-measles immunoglobulin G (IgG) were measured among 12–24-month-old children in urban slums of Mumbai. Demographic data and health history were also collected. The association between baseline predictors and measles titers was assessed using linear regression for continuous measles titer and change in measles titer, and binomial regression for the dichotomous outcome of having a protective titer at baseline and at endpoint (>16.5 AU/mL). RESULTS: At baseline (n = 122, median age 14.5 months), 71% of children reported receiving dose 1 of the measles vaccine typically administered at 9 months of age, while 57% of children had protective measles IgG titers (>16.5 AU/mL). Abstract

The rates at which sediment from the landscape is recruited to channel networks reflect lateral geomorphic connectivity and have significant implications for biophysical river systems. By strongly influencing sediment supply regime, such inputs control the dynamics and characteristics of the channel as well as the associated habitat and biota. The main objective of this research was to disentangle the relative importance of geomorphic and biological factors which define spatial patterns in lateral connectivity by promoting or restricting bank erosion, avulsions, and slope failures. Focusing on a forested, formerly glaciated mountain basin in the interior Pacific Northwest, we combined extensive field surveys with remote sensing data (LiDAR, aerial imagery) to obtain metrics representing this complex suite of landscape characteristics. The interpretation of evidence yielded by correlations between these variables, a regime model, and qualitative observations, suggest that, at a multi-decadal timescale between major landscape disturbances, lateral connectivity patterns in Sullivan Creek may be, to a large extent, governed by a combination of biotic factors and glacial legacies. In particular, forest canopy metrics were key predictors of hillslope stability as well as bank and valley floor resistance, suggesting a pivotal influence of root reinforcement. On the other hand, instream large wood abundance was closely related to channel splitting, which reflects its role in promoting channel avulsions. More extensive bank erosion in reaches dominated by drift deposits appeared to indicate glacial legacies. Furthermore, a strong association between bank erosion and colluvial inputs suggested a close coupling within the channel-hillslope system. Lateral confinement – defined as the proportion of the channel length abutting against bedrock, terraces, fans and roads – was negatively associated with channel braiding; the relationships linking confinement with bank erosion and the magnitude of colluvial inputs (negative and positive association, respectively) were relatively weak. Taken together, findings emerging from this research suggest that better understanding of lateral geomorphic connectivity patterns in mountain river basins similar to our study site requires that both channel processes and broader, biophysical landscape system be considered.

Background: Iron deficiency is the most widespread nutritional deficiency in the world. Objective: The objective of this randomized efficacy trial was to determine the effects of iron-biofortified pearl millet (FePM) on iron status compared with control pearl millet (Control-PM). Abstract

OBJECTIVES: a) To examine the mean nutrient intake among young children 12–18 months of age in urban slums of Mumbai, b) to determine the adequacy of micronutrient intake in their diet as per the National Institute of Nutrition (NIN) recommendations for optimum growth and development, and c) to determine if there is a difference in the nutrient intake of those randomized vs not randomized to the trial involving feeding of iron- and zinc-biofortified pearl millet complementary foods. METHODS: A total of 407 children were screened of which dietary data was available for 359 children. Of the total randomized (n = 223), dietary data was available for 216 children who were randomized to one of the two study arms (Arm 1: n = 107, Arm 2: n = 109). A 24-hour dietary recall was performed, and nutrient intake was calculated using CS dietary software using the NIN food composition tables. RESULTS: The average age of children was 14.81 months; 51% were males and 49% females. Majority of the children (81.64%) breastfed the previous day. The mean intake per day of calories, protein, iron, and zinc from complementary foods was found to be 598 kcal, 19.9 g, 3.25 mg, and 1.85 mg, respectively. On average, complementary feeding contributed to 89.6% of RDA for energy, 82.4% of calcium, 57.9% of iron, 58.8% of zinc, 51.9% vitamin A, 7.2% of vitamin B12, and 83.3% of dietary folate. All children had adequate caloric and protein intake. However, > 90% of children consumed inadequate iron, zinc, and folate according to the nutrient density of the complementary food, and > 50% of children consumed inadequate vitamin A and calcium. Overall, children randomized to the intervention were found to have a significantly higher intake of calories, protein, folate, and zinc. CONCLUSIONS: Majority of children living in the urban slums of Mumbai consumed diet inadequate for most micronutrients. Abstract

The absolute muon flux between 20 and 3000 GeV is measured with the L3 magnetic muon spectrometer for zenith angles ranging from 0°to 58°. Due to the large exposure of about 150 m2sr d, and the excellent momentum resolution of the L3 muon chambers, a precision of 2.3% at 150 GeV in the vertical direction is achieved.The ratio of positive to negative muons is studied between 20 and 500 GeV, and the average vertical muon charge ratio is found to be 1.285±0.003(stat.)±0.019(syst.)., Physics Letters B, 598 (1-2), ISSN:0370-2693, ISSN:0031-9163, ISSN:1873-2445

Abstract

A prototype detector based on thin-walled segmented tubes has been developed and its parameters have been studied. The detector contains 360 registration channels with a straw diameter of 4 mm. The prototype’ granularity is 4 cm 2 and the length of insensitive region due to straw internal elements is less than 5% of its full sensitive area. Deterioration of the spatial resolution near these elements is observed for 1.0% of the detector sensitive area. The time and spatial parameters of the detector do not differ from those of conventional tracking detectors based on drift tubes. & 2011 Published by Elsevier B.V.

Abstract

Biofortification is the breeding of crops to increase their nutritional value, including increased contents of micronutrients or their precursors. Biofortification aims to increase nutrient levels in crops during plant growth rather than during processing of the crops into foods. Emerging research from 8 human trials conducted in the past decade with staple food crops that have been biofortified by traditional plant breeding methods were presented in this symposium. Specifically, data from 6 efficacy and 2 effectiveness trials were discussed to assess the effects of regular consumption of these enhanced staple crops on improving population vitamin A and iron status and reducing the burden of micronutrient deficiencies in targeted populations living in South Asia, Sub-Saharan Africa, and Latin America. Biofortified food crops appear to have a positive impact on nutritional and functional health outcomes, as the results from the trials suggest. Additional implementation research will be needed to ensure maximization of the beneficial impact of this intervention and a smooth scaling up to make biofortification a sustainable intervention in public health. The challenge for the global health community remains how to take this efficacious intervention and implement at large scale in the real world.

Abstract

The hard X-ray polarimeter POLAR aims to measure the linear polarization of the 50-500 keV photons arriving from the prompt emission of gamma-ray bursts (GRBs). The position in the sky of the detected GRBs is needed to determine their level of polarization. We present here a method by which, despite of the polarimeter incapability of taking images, GRBs can be roughly localized using POLAR alone. For this purpose scalers are attached to the output of the 25 multi-anode photomultipliers (MAPMs) that collect the light from the POLAR scintillator target. Each scaler measures how many GRB photons produce at least one energy deposition above 50 keV in the corresponding MAPM. Simulations show that the relative outputs of the 25 scalers depend on the GRB position. A database of very strong GRBs simulated at 10201 positions has been produced. When a GRB is detected, its location is calculated searching the minimum of the chi2 obtained in the comparison between the measured scaler pattern and the database. This GRB localization technique brings enough accuracy so that the error transmitted to the 100% modulation factor is kept below 10% for GRBs with fluence Ftot \geq 10^(-5) erg cm^(-2) . The POLAR localization capability will be useful for those cases where no other instruments are simultaneously observing the same field of view., Comment: 13 pages, 10 figures

Abstract

Summary Mice that lack interleukin-23 (IL-23) are resistant to T cell-mediated autoimmunity. Although IL-23 is a maturation factor for T helper 17 (Th17) cells, a subset of γδ T cells expresses the IL-23 receptor (IL-23R) constitutively. Using IL-23R reporter mice, we showed that γδ T cells were the first cells to respond to IL-23 during experimental autoimmune encephalomyelitis (EAE). Although γδ T cells produced Th17 cell-associated cytokines in response to IL-23, their major function was to prevent the development of regulatory T (Treg) cell responses. IL-23-activated γδ T cells rendered αβ effector T cells refractory to the suppressive activity of Treg cells and also prevented the conversion of conventional T cells into Foxp3 + Treg cells in vivo. Thus, IL-23, which by itself has no direct effect on Treg cells, is able to disarm Treg cell responses and promote antigen-specific effector T cell responses via activating γδ T cells.

Abstract

The Alpha Magnetic Spectrometer is a large acceptance cosmic-ray detector ( 0.5 m 2 sr ) designed to operate at an altitude of 400 km on the International Space Station. The AMS-02 silicon tracker contains 2264 silicon microstrip sensors (total active area 6.75 m 2 ). The internal alignment parameters of the assembled tracker have been determined on the ground with cosmic-ray muons. The alignment procedure is described and results for the alignment precision and position resolution are reported.

Abstract

The TopoTomo bending magnet beamline at the ANKA synchrotron facility in Karlsruhe (Germany) operates in the hard X-ray regime (above 6 keV). Recently, an X-ray micro-imaging station has been installed at TopoTomo. For typical imaging applications, a filtered white beam or from 2009 on a double-multilayer monochromator is used. In order to optimize the field of view and the resolution of the available indirect pixel detectors, different optical systems have been installed, adapted, respectively, to a large field of view (macroscope) and to high spatial resolution (microscope). They can be combined with different camera systems, ranging from 14-bit dynamic range CCDs to fast CMOS cameras. The spatial resolution can be brought substantially beyond the micrometer limit by using a Bragg magnifier. Due to the moderate flux of the beamline compared to insertion-device beamlines on third generation light sources, special emphasis has been put on the efficiency of the detectors via a dedicated scintillator concept. The layout of the beamline optics makes optimal use of the coherence properties. Thus, absorption contrast, phase-contrast and analyzer-based imaging can be applied. Additionally, white beam synchrotron topography is performed, using digital indirect X-ray pixel detectors as well as X-ray film.

Abstract

Objective. To investigate geographic differences in obesity related metabolic abnormalities in middle-aged and older Chinese people with different levels of adiposity. Method. Data from 3289 individuals aged 50-70 years who participated in a cross-sectional survey in 2005 (1641 from northern [Beijing] and 1648 from southern [Shanghai] China) were analyzed. Overweight or obesity and abdominal obesity were classified as body mass index >= 24 kg/m(2) and waist circumference >= 90 cm for men and >= 80 cm for women. Metabolic abnormalities (metabolic syndrome, dyslipidemia, hypertension and diabetes) were defined using the NCEP/ATPIII criteria. Results. The overall prevalence of overweight and obesity was 53.4%, and the prevalence of abdominal obesity was 48.0%, respectively. The prevalence of the metabolic abnormalities was significantly higher among obese individuals than their normal weight counterparts. The association between obesity and the higher prevalence of the metabolic abnormalities was more pronounced among people who were overweight or obese in the north, especially for those living in rural areas. Conclusion. The prevalence of obesity and associated metabolic abnormalities varies substantially among Chinese people in different geographic region. Individuals who were overweight or obese living in rural areas in the north contribute importantly to the north-south differences in obesity-related metabolic abnormalities among Chinese populations. Abstract

Within the EUROTRANS Integrated Project, Forschungszentrum Karlsruhe (FZK) and the Institute for Transuranium Elements (ITU) are joining their efforts to study the behavior of Mo-based CERMET non-uranium fuel for the ADS. Contributions include core safety calculations, and fuel property measurements and irradiation experiments. Safety studies for optimized EFIT core designs have concluded that, for the new low power cores of EFIT with a power class of ∼400 MWth and a fuel power density of ∼250 MW/m3, the CERMET-loaded cores behave favorably and the design limits of the fuels were not violated. Mo-based CERMET fuel pellets and pins loaded with Pu and Am were fabricated for irradiation programmes which will start by mid-2007 in PHENIX (France) and HFR-Petten (The Netherlands). The thermal diffusivity and specific heat of the CERMET fuels (loaded with Pu and Am) were the main properties measured, and the thermal conductivity was deduced. The results were used to prepare the safety report for the irradiation experiments.

Abstract

An overview of current nuclear power generation and fuel cycle strategies in Europe is presented, with an emphasis on options for the management of separated plutonium in the medium to long term. Countries which have opted for reprocessing of spent fuel have had to contend with increasing inventories of separated plutonium. Of the various potential options for utilisation or disposition of these stockpiles, only light water reactor (LWR) mixed-oxide (MOX) fuel programmes are sufficiently technologically mature to be fully operational in several European countries at present. Such reprocessing-recycling programmes allow for a stabilisation of the overall separated plutonium stocks, but not for a significant reduction in the stockpile. Moreover, the quality of recycled plutonium decreases at each potential step of re-irradiation. Therefore, optimised or new ways of managing the plutonium stocks in the medium to long term are required. In the present overview we consider the most promising options for reactor utilisation of plutonium in both near-term future reactor and Generation IV systems.

Abstract

Within the EFTTRA-T4 project a MgAl 2 O 4 based inert matrix fuel for 241 Am transmutation was produced in 1996. Am L 3 -edge EXAFS of the annealed material indicate that Am is present as AmAlO 3 . During 10 years of storage the material accumulated self-irradiation damage and no AmAlO 3 X-ray diffraction features can be observed, indicating amorphization of the aged material by the corresponding cumulative α-damage of 2.9 × 10 19 g −1 AmAlO 3 . The Am L 3 -edge EXAFS of the amorphous AmAlO 3 shows that structural order is limited to the first oxygen shell around Am, where the rhombohedral splitting in three sub-shells is still present. Evidence for a single Al atom in the next shell is found at an Am–Al distance of 3.28 A.

Abstract

The impact of Pu and Am, two elements that potentially pose a long term hazard for the disposal of spent nuclear fuel, can be abated by their reintroduction into the fuel cycle for transmutation. Such transmutation targets can be fabricated by a sol gel method for the production of porous inactive beads, which are then infiltrated by Am solutions. Following calcination, compaction into pellets and sintering, the product is obtained. At its heart, the sol gel process relies on an ammonia precipitation, so that it is not universally applicable. Therefore, an alternative is sought not just to overcome this chemical limitation, but also to simplify the process and reduce waste streams. The new concept utilises powder metallurgy routes (compaction, crushing and sieving) to produce porous, almost, dust free granules, which are infiltrated with the actinide nitrate. The method has been developed using yttria stabilised zirconia and alumina, and has been demonstrated for the production of Al2O3–AmO2 targets for neutron capture investigations. The results are very promising and meet light water reactor fuel specifications. In addition, the process is ideally suited for the production of ceramic matrices for conditioning actinides for geological disposal.

Abstract

The sub-critical Accelerator Driven System (ADS) is now being considered as a potential means to burn long-lived transuranium nuclides. The preferred fuel for such a fast neutron reactor is uranium-free, highly enriched with plutonium and minor actinides. Requirements for ADS transmuter fuels are linked with the core design and safety parameters, the fuel properties and the ease of reprocessing. This study concerns the properties of metals as matrices, with the particular case of Mo. To improve the neutronic characteristics, enriched molybdenum (Mo-92) is required. To overcome the high enrichment cost, it is proposed to recover the matrix by pellet dissolution, and to recycle it for further use. Irradiation programmes are also planned to examine the in-reactor properties of the material. Based on the current status of the research, the results are promising, but irradiation results are still missing.

Abstract

The e+e− → e+e−hadrons reaction, where one of the two electrons is detected in a low polar-angle calorimeter, is analysed in order to measure the hadronic photon structure function F2γ. The full high-energy and high-luminosity data set, collected with the L3 detector at centre-of-mass energies 189 GeV ≼ √s ≼ 209 GeV, corresponding to an integrated luminosity of 608 pb−1 is used. The Q2 range 11 GeV2 ≼ Q2 ≼ 34 GeV2 and the x range 0.006 ≼ x ≼ 0.556 are considered. The data are compared with recent parton density functions., Facultad de Ciencias Exactas

Abstract

Acute mu and kappa opioids activate the ERK/MAPK phosphorylation cascade that represents an integral part of the signaling pathway of growth factors in astrocytes. By this cross-talk, opioids may impact neural development and plasticity among other basic neurobiological processes in vivo. The mu agonist, [D-ala2,mephe4,glyol5]enkephalin (DAMGO), induces a transient stimulation of ERK phosphorylation, whereas kappa agonist, U69,593, engenders sustained ERK activation. Here we demonstrate that acute U69,593 and DAMGO stimulate ERK phosphorylation by utilization of different secondary messengers and protein kinase C (PKC) isoforms upstream of the growth factor pathway. Immortalized astrocytes transfected with either antisense calmodulin (CaM), a mutant mu opioid receptor that binds CaM poorly or a dominant negative mutant of PKCepsilon were used as a model system to study mu signaling. Evidence was gained to implicate CaM and PKCepsilon in DAMGO stimulation of ERK. DAMGO activation of PKCepsilon and/or ERK was insensitive to selective inhibitors of Ca2+ mobilization, but it was blocked upon phospholipase C inhibition. These results suggest a novel mechanism wherein, upon DAMGO binding, CaM is released from the mu receptor and activates phospholipase C. Subsequently, phospholipase C generates diacylglycerides that activate PKCepsilon. In contrast, U69,593 appears to act via phosphoinositide 3-kinase, PKCzeta, and Ca2+ mobilization. These signaling components were implicated based on studies with specific inhibitors and a dominant negative mutant of PKCzeta. Collectively, our findings on acute opioid effects suggest that differences in their mechanism of signaling may contribute to the distinct outcomes on ERK modulation induced by chronic mu and kappa opioids.

Abstract

Neutral-current four-fermion production, e + e − → f f ¯ f ′ f ¯ ′ , is studied in 0.7 fb −1 of data collected with the L3 detector at LEP at centre-of-mass energies s = 183 – 209 GeV . Four final states are considered: q q ¯ ν ν ¯ , q q ¯ l + l − , l + l − l ′ + l ′ − and l + l − ν ν ¯ , where l denotes either an electron or a muon. Their cross sections are measured and found to agree with the Standard Model predictions. In addition, the e + e − → Z γ ∗ → f f ¯ f ′ f ¯ ′ process is studied and its total cross section at the average centre-of-mass energy 〈 s 〉 = 196.6 GeV is found to be 0.29 ± 0.05 ± 0.03 pb , where the first uncertainty is statistical and the second systematic, in agreement with the Standard Model prediction of 0.22 pb. Finally, the mass spectra of the q q ¯ l + l − final states are analysed to search for the possible production of a new neutral heavy particle, for which no evidence is found.

Abstract

The process e+e-→Zγ where the Z boson decays into hadrons or neutrinos, is studied with data collected with the L3 detector at LEP at centre-of-mass energies from 189 up to 209 GeV. The cross sections are measured and found to be in agreement with the Standard Model predictions. Limits on triple neutral-gauge-boson couplings, forbidden in the Standard Model at tree level, are derived. Limits on the energy scales at which the anomalous couplings could be manifest are set. They range from 0.3 to 2.3 TeV depending on the new physics effect under consideration. © 2004 Published by Elsevier B.V.

Abstract

Maternal nutritional status is accepted as an important factor in fetal growth in developing countries. The prevalence of stunting at birth (defined as birth length for gestational age below the 10th percentile) may be ashigh as 50%. This study examined the relationship between maternal weight gain at various stages of pregnancy and linear fetal growth. All pregnant women 19 to 34 years of age who lived in one of four rural villages in Guatemala were asked to participate, and 200 of them agreed to do so: Ultrasound studies were done at 15-24 and at 28-32 weeks' gestation. The findings were compared with maternal weight gain from the first to the second, and from the second to the third trimester. The 98 male and 102 female infants had an average birth weight approximately 0.5 standard deviation (SD) below the reference mean, and birth length averaged 1 SD below the reference mean. Intrauterine growth retardation was diagnosed in 18.5% of infants, and low birth weight (less than 2500 gm) in 8%. More than a third of infants (36.5) were stunted at birth. These infants had lower birth weights than the others, but mean gestational ages did not differ significantly. Mothers of stunted infants had had significantly lower mean weight and height before pregnancy, and lower rates of weight gain from the second to the third trimester. A significant shortened femur length was noted in the third trimester in infants born stunted, but there was no difference in the rate of tibial growth from the second to the third trimester. The rate of maternal weight gain from the first to the second trimester significantly predicted both femur and tibia length in the second and third trimesters. Weight gain from the second to the third trimester did not predict bone length in the third trimester. The rate of maternal weight gain from the first to the second trimester did significantly predict infant length at birth. These observations suggest that linear fetal growth is dependent on events taking place early in pregnancy.

Abstract

Using video fluoroscopy, 10 subjects having a mobile-bearing posterior cruciate-retaining total knee arthroplasty were analyzed to determine their in vivo kinematic patterns. Under weight-bearing conditions, while in extension, the average contact position was posterior to the mid-tibia sagittal plane with posterior translation of both condyles to 60 degrees of flexion, followed by anterior translation to 120 degrees of flexion. Under non-weight-bearing conditions, the average condylar contact positions were significantly more anterior from full extension to 90 degrees of knee flexion (P=.01). The average range of motion was 129 degrees under non-weight-bearing conditions and 119 degrees during weight-bearing. Although subjects in this study exhibited variable motion patterns, they are accommodated by the unconstrained optimized articulation of this highly conforming mobile-bearing implant.

Abstract

The work presented is an example of how large area compositional mapping (≥1 mm 2 ) can be used to provide quantitative information on element distribution and specimen homogeneity. High-resolution was accomplished by producing a collage of X-ray maps acquired using classical conditions; magnification ×400, spatial resolution 256×256 pixels. The individual images, each measuring roughly 250×250 μm, were converted to quantitative maps using the Himax ® software package and the Xmas ® matrix correction from SAMx. The quantitative gray-level large area X-ray picture was pieced together using the ‘Multiple Image Alignment’ function of the AnalySIS ® image processing software. This software was also used to convert the gray-level pictures to false color images. The specimens investigated were transverse sections of MOX fuel pellets. Results are presented for the distribution of Pu by area fraction and cumulative area fraction, the size distribution of regions of high Pu concentration and average separation of these regions.

Abstract

The cross section of the process e + e − → Z γ γ → q q ¯ γ γ is measured with 215 pb−1 of data collected with the L3 detector during the final LEP run at centre-of-mass energies around 205 GeV and 207 GeV. No deviation from the Standard Model expectation is observed. The full data sample of 713 pb−1, collected above the Z resonance, is used to constrain the coefficients of anomalous quartic gauge boson couplings to: − 0.02 GeV − 2 a 0 / Λ 2 0.03 GeV − 2 , − 0.07 GeV − 2 a c / Λ 2 0.05 GeV − 2 , at 95% confidence level.

Abstract

Results are presented from a study of the structure of high energy hadronic events recorded by the L3 detector at s ⩾192 GeV. The distributions of several event shape variables are compared to resummed O (α s 2 ) QCD calculations. We determine the strong coupling constant at three average centre-of-mass energies: 194.4, 200.2 and 206.2 GeV. These measurements, combined with previous L3 measurements at lower energies, demonstrate the running of α s as expected in QCD and yield α s ( m Z )=0.1227±0.0012±0.0058, where the first uncertainty is experimental and the second is theoretical.

Abstract

The process e + e − → Z γγ→ q q γγ is studied in 0.5 fb−1 of data collected with the L3 detector at centre-of-mass energies between 130.1 GeV and 201.7 GeV. Cross sections are measured and found to be consistent with the Standard Model expectations. The study of the least energetic photon constrains the quartic gauge boson couplings to −0.008 GeV−2

Abstract

Excited leptons are searched for using the L3 detector at LEP. The data collected at centre-of-mass energies in the range from 192 up to 202 GeV correspond to a total luminosity of 233 pb−1. No evidence of either pair production of excited leptons, nor of single production is found. From the searches for pair produced excited leptons, lower mass limits close to the kinematic limit are set. From the searches for singly produced excited leptons, upper limits on their couplings are derived in the mass range up to 200 GeV.
