Abstract

International audience; Marketing messages are most effective if they reach the right customers. Deciding which customers to contact is an important task in campaign planning. The paper focuses on empirical targeting models. We argue that common practices to develop such models do not account sufficiently for business goals. To remedy this, we propose profit-conscious ensemble selection, a modeling framework that integrates statistical learning principles and business objectives in the form of campaign profit maximization. Studying the interplay between data-driven learning methods and their business value in real-world application contexts, the paper contributes to the emerging field of profit analytics and provides original insights how to implement profit analytics in marketing. The paper also estimates the degree to which profit-concious modeling adds to the bottom line. The results of a comprehensive empirical study confirm the business value of the proposed ensemble learning framework in that it recommends substantially more profitable target groups than several benchmarks.

Abstract

Rationale Adhesion molecules are key elements in stroke-induced brain injury by regulating the migration of effector immune cells from the circulation to the lesion site. Platelet endothelial cell adhesion molecule-1 (PECAM-1) is an adhesion molecule highly expressed on endothelial cells and leukocytes, which controls the final steps of trans-endothelial migration. A functional role for PECAM-1 in post-ischemic brain injury has not yet been demonstrated. Objective Using genetic Pecam-1 depletion and PECAM-1 blockade using a neutralizing anti-PECAM-1 antibody, we evaluated the role of PECAM-1 mediated trans-endothelial immune cell migration for ischemic injury, delayed brain atrophy, and brain immune cell infiltrates. Trans-endothelial immune cell migration was furthermore evaluated in cultured human cerebral microvascular endothelial cells. Methods and results Transient middle cerebral artery occlusion (tMCAO) was induced in 10–12-week-old male Pecam-1−/− and Pecam-1+/+ wildtype mice. PECAM-1 levels increased in the ischemic brain tissue due to the infiltration of PECAM-1+ leukocytes. Using magnetic resonance imaging, we observed smaller infarct volume, less edema formation, and less brain atrophy in Pecam-1−/− compared with Pecam-1+/+ wildtype mice. The transmigration of leukocytes, specifical neutrophils, was selectively reduced by Pecam-1−/−, as shown by immune fluorescence and flow cytometry in vivo and transmigration assays in vitro. Importantly, inhibition with an anti-PECAM-1 antibody in wildtype mice decreased neutrophil brain influx and infarct. Conclusion PECAM-1 controls the trans-endothelial migration of neutrophils in a mouse model of ischemic stroke. Antibody blockade of PECAM-1 after stroke onset ameliorates stroke severity in mice, making PECAM-1 an interesting target to dampen post-stroke neuroinflammation, reduce ischemic brain injury, and enhance post-ischemic brain remodeling.

Abstract

Resumen Antecedentes y objetivo La enfermedad de Hansen o lepra es una enfermedad causada por Mycobacterium leprae y Mycobacterium lepromatosis. Estas bacterias ingresan al organismo por las vias aereas superiores, por lo que generan manifestaciones clinicas en la mucosa nasal y en la cavidad bucal. El objetivo del estudio es caracterizar las lesiones bucales de los pacientes con una enfermedad de Hansen. Materiales y metodos Se diseno un estudio observacional, transversal, de 100 pacientes con diagnostico de lepra. Se examino la cavidad bucal y se registraron los hallazgos clinicos en una ficha epidemiologica. Ademas, se incluyo el estudio histopatologico de las lesiones que requirieron de una biopsia. Los casos sugestivos de lepra fueron tenidos y analizados con hematoxilina y eosina, Ziehl-Neelsen y Fite-Faraco. Las variables fueron analizadas y comparadas mediante las pruebas Chi2, correlacion de Spearman y U de Mann-Whitney. Resultados El grupo de estudio estuvo conformado por 67 hombres y 33 mujeres, con una media (± desviacion estandar) de 48,1±16,4 anos de edad. Fueron observadas lesiones bucales en 34 pacientes. Las lesiones orales relacionadas con lepra, presentes en 9 pacientes, se ubicaron en el paladar duro y el labio superior. Estas fueron significativamente mas frecuentes en la lepra lepromatosa, la borderline lepromatosa (p=0,029) y en el eritema nudoso leproso (p=0,031). Conclusiones Las lesiones bucales asociadas a lepra ocurren con una frecuencia baja, manifestandose como lepromas y placas leprosas en el paladar duro y el labio superior.

Abstract

Background and objective Hansen disease, or leprosy, is caused by Mycobacterium leprae or Mycobacterium lepromatosis. Because these bacteria enter the body via the upper airways, they generate clinical manifestations in the nasal mucous membranes and the mouth. We aimed to describe the characteristics of oral lesions in patients with Hansen disease. Material and methods Cross-sectional observational study of 100 patients diagnosed with Hansen disease. We examined the oral cavity and recorded clinical findings on a disease reporting form for each patient. We also included the histopathologic findings for lesions that required a biopsy. Samples suggestive of Hansen disease were processed with hematoxylin-eosin, Ziehl–Neelsen, and Fite–Faraco staining. Variables were analyzed, as appropriate, with the χ2 test, the Mann-Whitney U test, or the Spearman correlation coefficient. Results Sixty-seven men and 33 women were included. The mean (SD) age was 48.1 (16.4) years. Thirty-four patients had oral lesions. Lesions related to Hansen disease were found in 9 patients. The locations were the hard palate and upper lip. Oral lesions were significantly more frequent in patients with lepromatous leprosy, borderline lepromatous leprosy (P = .029), and erythema nodosum leprosum (P = .031). Conclusions The frequency of oral lesions is low in Hansen disease. Such lesions present as lepromas and leprous plaques on the hard palate and upper lip.

Abstract

In order to assess risks associated with establishing relationships with corporate partners such as clients, suppliers, debtors or contractors, decision makers often turn to business failure prediction models. While a large body of literature has focused on optimizing and evaluating novel methods in terms of classification accuracy, recent research has acknowledged the existence of asymmetric misclassification costs associated with prediction errors and thus, advocates the usage of alternative evaluation metrics. However, these papers often assume a misclassification cost matrix to be known and fixed for both the training and the evaluation of models, whereas in reality these costs are often uncertain. This paper presents a methodological framework based upon heterogeneous ensemble selection and multi-objective optimization for cost-sensitive business failure prediction that accommodates uncertainty at the level of misclassification costs. The framework assumes unknown costs during model training and accommodates varying degrees of uncertainty during model deployment. Specifically, NSGA-II is deployed to optimize cost space resulting in a set of pareto-optimal ensemble classifiers where every learner minimizes expected misclassification cost for a specific range of cost ratios. An extensive set of experiments evaluates the method on multiple data sets and for different scenarios that reflect the extent to which cost ratios are known during model deployment. Results clearly demonstrate the ability of our method to minimize cost under the absence of exact knowledge of misclassification costs.

Abstract

Resumen Introduccion Aunque las guias de practica clinica recomiendan un ajuste continuado del tratamiento del asma, reduciendo la medicacion de mantenimiento cuando se alcanza el control (step-down), existen pocos estudios de practica clinica habitual orientados a recabar informacion de los factores que condicionan su fracaso. Objetivo Determinar los factores que condicionan en la practica clinica habitual el fracaso del step-down en los pacientes asmaticos moderados-graves controlados con una combinacion de glucocorticoides inhalados/agonistas beta-2 adrenergicos de accion larga. Metodo Estudio multicentrico retrospectivo sobre 374 pacientes con asma moderada-grave controlada con glucocorticoides inhalados/agonistas beta-2 adrenergicos de accion larga en quienes el facultativo indico en 2016 un step-down. Resultados El step-down fracaso en el 41,7%. Los factores relacionados con el fracaso fueron: la mayor edad del paciente (p = 0,006), la presencia de 2 o mas comorbilidades (p = 0,016), el mayor nivel de gravedad (persistente grave vs. persistente moderada) (p 40 anos) (p = 0,045), cuanto mas alto es el escalon terapeutico previo (p = 0,003) y posterior al cambio (p Conclusion En condiciones de practica clinica habitual el step-down fracasa en un porcentaje alto de pacientes y se recomienda indicarlo cuando el paciente lleve mas de 6 meses controlado.

Abstract

Although the clinical practice guidelines recommend continuous adjustment of asthma treatment and reducing the maintenance drugs when achieving control (step-down), there are few studies of standard clinical practice aimed at collecting information on the factors that determine step-down failure.To determine the factors that determine step-down failure in standard clinical practice of patients with moderate-severe asthma controlled by a combination of inhaled glucocorticoids and long-acting beta agonists.A multicentre retrospective study included 374 patients with moderate-severe asthma controlled with inhaled glucocorticoids and long-acting beta agonists for whom the physician indicated a step-down in 2016.The step-down failed in 41.7% of the patients. The following factors were related to failure: greater patient age (P=.006), presence of at least 2 comorbidities (P=.016), greater severity level (severe persistent vs. moderate persistent) (P.001), greater age at diagnosis (40 years) (P=.045), the higher the therapeutic step before (P=.003) and after the change (P.001), the shorter the time of improvement/control prior to the change (P=.019), lower FEVIn standard clinical practice conditions, step-down fails in a high percentage of patients, and the suggestion is to indicate step-down when the patient has had more than 6 months of disease control.
