13 figures, 5 tables., The expected increase of hydrogen fuel cell vehicles has motivated the emergence of a significant number of studies on Hydrogen Refuelling Stations (HRS). Some of the main HRS topics are sizing, location, design optimization, and optimal operation. On-site green HRS, where hydrogen is produced locally from green renewable energy sources, have received special attention due to their contribution to decarbonization. This kind of HRS are complex systems whose hydraulic and electric linked topologies include renewable energy sources, electrolyzers, buffer hydrogen tanks, compressors and batteries, among other components. This paper develops a linear model of a real on-site green HRS that is set to be built in Zaragoza, Spain. This plant can produce hydrogen either from solar energy or from the utility grid and is designed for three different types of services: light-duty and heavy-duty fuel cell vehicles and gas containers. In the literature, there is a lack of online control solutions developed for HRS, even more in the form of optimal online control. Hence, for the HRS operation, a Model Predictive Controller (MPC) is designed to solve a weighted multi-objective online optimization problem taking into account the plant dynamics and constraints as well as the disturbances prediction. Performance is analysed throughout 210 individual month-long simulations and the effect of the multi-objective weighting, prediction horizon, and hydrogen selling price is discussed. With the simulation results, this work shows the suitability of MPC for HRS control and its significant economic advantage compared to the rule-based control solution. In all simulations, the MPC operation fulfils all required services. Resumen Introduccion La eficacia de la trombectomia mecanica en territorio carotideo en las primeras 24 horas se ha probado con trabajos publicados recientemente. Metodos Revision retrospectiva a partir de un registro prospectivo en nuestro centro de referencia de ictus para valorar la eficacia y seguridad del tratamiento endovascular realizado mas alla de las 6 horas de evolucion de los sintomas en pacientes con ictus isquemico agudo y oclusion de gran vaso en territorio carotideo, entre noviembre de 2016 y abril de 2019. Resultados Se recopilaron datos de 59 pacientes (55,9% mujeres, mediana de edad 71 anos). Treinta y tres pacientes fueron detectados al despertar. El 57,6% de los casos fueron traslados secundarios. La mediana de NIHSS basal fue 16. La mediana del ASPECTS fue 8 y el 94,9% de los pacientes presento > 50% de tejido salvable. El 88,1% de los pacientes logro una recanalizacion satisfactoria, en 5 pacientes despues de 24 horas de evolucion. El 67,8% de los casos logro la independencia funcional a los 90 dias de seguimiento. Los pacientes que no lograron la independencia funcional presentaban mayor edad, mayor proporcion de fibrilacion auricular, mayor tiempo puncion-recanalizacion y mayor puntuacion NIHSS, tanto basal como al alta. Conclusion En nuestra experiencia la trombectomia mecanica despues de las 6 horas se asocio con buenos resultados de funcionalidad a los 90 dias. La edad, la puntuacion NIHSS, el tiempo puncion-recanalizacion y la prevalencia de fibrilacion auricular fueron factores determinantes en el pronostico funcional. La eficacia de este tratamiento por encima de las 24 horas merece ser estudiada.

Introduction Thrombectomy in the carotid artery territory was recently shown to be effective up to 24 hours after symptoms onset. Methods We conducted a retrospective review of a prospective registry of patients treated at our stroke reference centre between November 2016 and April 2019 in order to assess the safety and effectiveness of mechanical thrombectomy performed beyond 6 hours after symptoms onset in patients with acute ischaemic stroke and large vessel occlusion in the carotid artery territory. Results Data were gathered from 59 patients (55.9% women; median age, 71 years). In 33 cases, stroke was detected upon awakening; 57.6% of patients were transferred from another hospital. Median baseline NIHSS score was 16, and median ASPECTS score was 8, with 94.9% of patients presenting > 50% of salvageable tissue. Satisfactory recanalisation was achieved in 88.1% of patients, beyond 24 hours after onset in 5 cases. At 90 days of follow-up, 67.8% were functionally independent; those who were not were older and presented higher prevalence of atrial fibrillation, greater puncture-to-recanalisation time, and higher NIHSS scores, both at baseline and at discharge. Conclusion In our experience, mechanical thrombectomy beyond 6 hours was associated with good 90-day functional outcomes. Age, NIHSS score, puncture-to-recanalisation time, and presence of atrial fibrillation affected functional prognosis. The efficacy of the treatment beyond 24 hours after onset merits study.

Telehealth has emerged as an alternative to conventional, face-to-face visits, and the COVID pandemic has hastened its introduction. Telephone appointments make use of an easy-to-use and accessible technology.To investigate the usability of telephone-based telehealth in a women's health outpatient clinic and whether this may be affected by the severity of the COVID pandemic.A telephone survey was prepared to explore two usability domains: interaction quality (4 items) and satisfaction, preference and future use (6 items). Women were selected from two periods during the COVID pandemic when the infection rates were high and low.The survey was completed by 106 women (60 when the prevalence of COVID was high, mean age 53.58 years, and 46 when it was low, mean age 48.59 years) out of the 153 women who had a telephone appointment. The severity of the COVID pandemic showed an effect on responses. Women were less enthusiastic about using the telephone during the period of low COVID prevalence, as shown by lower scores on 3 of the 4 items of the first domain [I had enough time; I would have understood better in person; I would have expressed myself better in person (p 0.001 for comparison between groups on each of the 3 items)], and on 4 of the 6 items in the second domain [satisfied with quality of care (p 0.001), or with the information received (p = 0.018); use of telephone in future (p 0.001); preference to try other technologies in future (p 0.001)]. Overall, women expressed a preference for in-person visits regardless of COVID prevalence rates.Telephone calls were a feasible alternative to face-to-face visits in a women's health outpatient clinic, but the pandemic pressure modified usability parameters. Respondents preferred in-person visits at any pandemic stage.

[EN] Experience is an important requirement to apply for a qualified job. While employers need to find workers with the required skills, education institutions must design curricula which provide the skills demanded by the job market. Besides, along their careers, workers need to make good choices on continuing education courses. Current Bachelor and Master degrees follow programs which attempt to offer a practical perspective, but still from the academia point of view. To bridge the gap between academia and industry, University Extension Diploma in Food Technology (DEUTA) deepens into the Food sector seeking professional capacitation of par-ticipants. This is achieved by both first-hand know-how of food sector professionals and academics, along with an internship period in a food company. This paper reports the experience for more than fifteen years of this Diploma. Curriculum, students background, food sectors where internships are taken, student's opinion on the course management as well as on issues related to employability, development of professional skills and life-long learning are discussed. The analysis demonstrates how collaborative courses strengthen academia-industry bonds, and employability is boosted thanks to internships and the network created. This experience may be extrapolated to many sectors other than food, such as the chemical, environmental or biotechnological, and may help lecturers and institutions organize similar courses or diplomas to prepare competent and efficient technical workers, as well as help students and professionals orient their training and continuing education choices., Funding for open access charge: CRUE - Universitat Politècnica de València.

During the 1900s, tacrolimus became the mainstay immunosuppressive agent to prevent rejection after kidney transplant. Subsequently, an extended-release tacrolimus (ER-Tac) formulation was developed to improve adherence, and its generic version has been marketed over the last years. This study examines the differences in efficacy and safety between the generic ER-Tac (Conferoport) and the reference brand-name drug (Advagraf).Prospective, randomized and parallel single-center study (May 2020 to June 2021) with 52 kidney transplant recipients who were randomly assigned to 1 of the following groups: study group (Conferoport, n = 31) and control group (Advagraf, n = 21). The variables of interest were collected and analyzed to compare tacrolimus efficacy and safety between them. Demographic characteristics of the patients and clinical donor data were homogeneous in both groups (P.05).No statistically significant differences were found among treatments regarding dosage used, levels, creatinine, and proteinuria (P.05), with these variables presenting a downward trend during follow-up and, consequently, the improvement of graft function. Analyses also revealed the absence of differences concerning the incidence of acute rejection and intrapatient variability (coefficient of variation) throughout the first year of evolution between both formulations (P.05). A total of 5 graft losses occurred, 2 resulting from patient death.In our experience, we found no significant differences between the measured parameters in relation to the efficacy and safety profile of both drugs, with generic ER-Tac being an alternative comparable with the reference brand-name ER-Tac.
