Very few cross-national studies on body image focus on Black African women. In this study, a comparative analytical approach and sociocultural theory framework was utilized in examining measures of body image among 209 Kenyan and Nigerian women aged 18–69 (M = 34.4, SD = 7.81). We examined both weight-related and non-weight-related aspects of appearance, including hair texture, skin tone, and other racialized features. The regional samples were compared on scores of internalized Eurocentric beauty ideals, skin color satisfaction, objectified body consciousness, racialized body image satisfaction, and other demographic metrics. Kenyan and Nigerian women showed differences across multiple body image measures. Nigerian women showed thinner body ideals compared to Kenyan women and also endorsed higher skin color satisfaction, racialized body dissatisfaction, and internalized Eurocentric beauty idealization. Kenyan women showed higher levels of objectified body consciousness. An indirect effect of nationality was observed with internalized Eurocentric beauty ideals mediating differences in racialized body dissatisfaction and objectified body shame. Background & Aims: Hepatocellular carcinoma (HCC) is a leading cause of cancer-related mortality worldwide. Several studies have shown that statins could have chemopreventive effects on HCC. We performed a systematic review and meta-analysis of studies that evaluated the effects of statins on the risk of HCC. Methods: We conducted a systematic search of MEDLINE, Embase, and Web of Science through May 2012 and manually reviewed the literature. Studies were included if they evaluated and clearly defined exposure to statins, reported the incidence of HCC, and reported relative risks or odds ratios (ORs) or provided data for their estimation. Ten studies reporting 4298 cases of HCC in 1,459,417 patients were analyzed. Summary OR estimates with 95% confidence intervals (CIs) were calculated using the random effects model. Statistical heterogeneity was assessed with the Cochran''s Q statistic and I2 statistic. Results: Statin users were less likely to develop HCC than statin nonusers (adjusted OR, 0.63; 95% CI, 0.52–0.76), although the results were heterogeneous (P = .01, I2 = 59%). This heterogeneity could be accounted for by study location (Asian population [n = 4]: adjusted OR, 0.52; 95% CI, 0.42–0.64; Western population [n = 6]: adjusted OR, 0.67; 95% CI, 0.53–0.85) and design (observational studies [n = 7]: adjusted OR, 0.60; 95% CI, 0.49–0.73; clinical trials [n = 3]: adjusted OR, 0.95; 95% CI, 0.62–1.45). Conclusions: Based on meta-analysis, statin use is associated with a reduced risk of HCC, most strongly in Asian but also in Western populations. Randomized clinical trials in populations at high risk for HCC (especially in Asian populations with hepatitis B) are warranted. [Copyright &y& Elsevier]

Background: Hilar cholangiocarcinoma presents both diagnostic and therapeutic challenges. While establishing a diagnosis is important for patients considering aggressive treatment, a transperitoneal fine needle aspiration (FNA) may lead to seeding of the tumour. The aim of the present study was to determine whether patients who have undergone transperitoneal FNA of the primary tumour have a higher incidence of metastases. Patients and Methods: Outcomes of 191 patients enrolled in a neoadjuvant chemoradiotherapy followed by liver transplantation (LT) from 1 October 1992 to 1 January 2010 were analysed. The incidence of metastases was compared between those who did or did not undergo a transperitoneal FNA biopsy of the primary tumour. Results: A total of 16 patients underwent FNA biopsy. There were six patients with biopsies positive for adenocarcinoma and 5/6 (83%) had peritoneal metastases at operative staging. Nine patients had biopsies, which did not demonstrate a tumour, and had no evidence of metastasis. One patient had an equivocal biopsy. Of those who did not undergo a transperitoneal biopsy, the incidence of peritoneal metastasis was 8% (14/175), P= 0.0097 vs. positive staging (83%) in those with a diagnostic transperitoneal FNA. Survival at 5 years for those who underwent LT was 74%. Conclusion: Transperitoneal biopsy of hilar cholangiocarcinoma is associated with a higher rate of peritoneal metastases, and it should not be performed if a curative approach such as LT is available. Background & Aims: Information about malignancies that arise in patients after liver transplantation comes from volunteer registry databases and single-center retrospective studies. We analyzed a multicenter, prospectively obtained database to assess the probabilities of and risk factors for de novo malignancies in patients after liver transplantation. Methods: We analyzed the National Institute of Diabetes and Digestive and Kidney Diseases'' liver transplantation database of 798 adults who received transplants from April 1990 to June 1994 and long-term follow-up data through January 2003. In this patient population, 171 adult patients developed 271 de novo malignancies. Of these malignancies, 147 were skin-related, 29 were hematologic, and 95 were solid organ cancers; we focused on nonskin malignancies. Results: The probability of developing any nonskin malignancy was highest in patients with primary sclerosing cholangitis (PSC; 22% at 10 years) or alcohol-related liver disease (ALD; 18% at 10 years); all other diagnoses had a 10% probability. Multivariate analysis indicated that increased age by decade (hazard ratio [HR] = 1.33, P = .01), a history of smoking (HR = 1.6, P = .046), PSC (HR = 2.5, P = .001), and ALD (HR = 2.1, P = .01) were associated with development of solid malignancies after liver transplantation. The probabilities of death after diagnosis of hematologic and solid malignancy were 44.0% and 38.0% at 1 year and 57.6% and 53.1% at 5 years, respectively. Conclusions: De novo malignancy primarily affects patients with PSC or ALD, compared to other transplant recipients, with a significant impact on long-term survival. [Copyright &y& Elsevier]

Dietary supplements containing usnic acid are marketed for weight loss and have been associated with hepatotoxicity. The specific Ingredient responsible for the hepatotoxicity is currently unknown. We describe 2 patients who developed severe hepato- toxicity within 3 months of taking a dietary supplement containing usnic acid. One patient developed fulminant hepatic failure requiring emergency liver transplantation; the other developed submassive hepatic necrosis but did not require transplantation. Thorough investigation, including histopathological examination of the liver, revealed no other cause of acute liver injury. Usnic acid hepatotoxicity should be considered as a possible etiologic factor in patients presenting with fulminant hepatic failure, especially if they have been taking dietary supplements for weight reduction. Detection and characterization of focal liver lesions (FLLs) is key for optimizing treatment for patients who may have a primary hepatic cancer or metastatic disease to the liver. This is the first study to develop an EUS-based convolutional neural network (CNN) model for the purpose of identifying and classifying FLLs. A prospective EUS database comprising cases of FLLs visualized and sampled via EUS was reviewed. Relevant still images and videos of liver parenchyma and FLLs were extracted. Patient data were then randomly distributed for the purpose of CNN model training and testing. Once a final model was created, occlusion heatmap analysis was performed to assess the ability of the EUS-CNN model to autonomously identify FLLs. The performance of the EUS-CNN for differentiating benign and malignant FLLs was also analyzed. A total of 210,685 unique EUS images from 256 patients were used to train, validate, and test the CNN model. Occlusion heatmap analyses demonstrated that the EUS-CNN model was successful in autonomously locating FLLs in 92.0% of EUS video assets. When evaluating any random still image extracted from videos or physician-captured images, the CNN model was 90% sensitive and 71% specific (area under the receiver operating characteristic [AUROC], 0.861) for classifying malignant FLLs. When evaluating full-length video assets, the EUS-CNN model was 100% sensitive and 80% specific (AUROC, 0.904) for classifying malignant FLLs. This study demonstrated the capability of an EUS-CNN model to autonomously identify FLLs and to accurately classify them as either malignant or benign lesions. Magnesium (Mg) isotopes in carbonate minerals are a useful proxy for paleoclimate studies, but interpretations are often limited by an inadequate understanding of the various factors controlling Mg isotopic fractionation during carbonate formation. Previous work has studied a number of parameters including aqueous chemistry, mineralogy, temperature, and precipitation rate. However, little is known about the impact of solid/solution ratio, calcite growth mechanism, and crystal morphology on isotope fractionation. In this work, two groups of seeded chemo-stat calcite precipitation experiments were conducted at 25 °C to explore the potential impact of crystal growth and morphology on the fractionation of Mg isotopes. Group-1 experiments (G1) contained nine individual runs that were performed under identical physicochemical conditions, except for solid/solution ratio and the length of an experiment. The isotope fractionation between precipitated calcite and aqueous solution is limited, with Δ26Mg cal-sol ranging from −2.58 to −2.40‰ and an average of −2.49 ± 0.12‰ (2SD, n = 9). The Group-2 experiments (G2) contained 3 paired runs with solution Mg/Ca molar ratios of 0.5, 2.0, and 5.0, and yielded Δ26Mg cal-sol values that ranged from −2.69 to −2.36‰ with an average of −2.62 ± 0.25‰ (2SD, n = 6). The average Δ26Mg cal-sol value for both sets of experiments is −2.54 ± 0.22‰ (2SD, n = 15), and it is independent of precipitation rate, solution Mg/Ca molar ratio, solid/solution ratio, amount of overgrowth, and mol% Mg content in overgrowth. The form and texture of the calcite overgrowths in our experiments range from {104} rhombohedra with smooth crystal faces containing few macrosteps to {104} rhombohedra containing extensive evidence for 2-D nucleation on crystal faces, to more steeply sided rhombohedra {0 kl }. While significant changes in crystal morphology are related to solid/solution ratio and solution composition in the G1 and G2 experiments, respectively, there was no difference in Mg isotope systematics, suggesting that crystal morphology does not affect the Mg isotopic composition of calcite within the range of features investigated and 2-D nucleation may be less affected by calcite growth kinetics than a spiral growth mechanism. Integrating our results with previous published values, an equilibrium isotopic fractionation factor of −2.47 ± 0.09‰ (weighted average ± weighted 2SD, n = 70) between calcite and aqueous solutions is derived at room temperature. Methylphenidate is a stimulant used to treat attention deficit and hyperactivity disorder (ADHD). In the last decade, illicit use of methylphenidate has increased among healthy young adults, who consume the drug under the assumption that it will improve cognitive performance. However, the studies that aimed to assess the methylphenidate effects on memory are not consistent. Here, we tested whether the effect of methylphenidate on a spatial memory task can be explained as a motivational and/or a reward effect. We tested the effects of acute and chronic i.p. administration of 0.3, 1 or 3 mg/kg of methylphenidate on motivation, learning and memory by using the 8-arm radial maze task. Adult male Wistar rats learned that 3 of the 8 arms of the maze were consistently baited with 1, 3, or 6 sucrose pellets, and the number of entries and reentries into reinforced and non-reinforced arms of the maze were scored. Neither acute nor chronic (20 days) methylphenidate treatment affected the number of entries in the non-baited arms. However, chronic, but not acute, 1–3 mg/kg methylphenidate increased the number of reentries in the higher reward arms, which suggests a motivational/rewarding effect rather than a working memory deficit. In agreement with this hypothesis, the methylphenidate treatment also decreased the approach latency to the higher reward arms, increased the approach latency to the low reward arm, and increased the time spent in the high, but not low, reward arm. These findings suggest that methylphenidate may act more as a motivational enhancer rather than a cognitive enhancer in healthy people. • Chronic methylphenidate increases motivation and/or reward preference for high-value rewards. • Chronic methylphenidate decreased motivation and/or reward preference for low-value rewards. • Methylphenidate did not affect reference and working memory in the 8-arm radial maze.
