Conserving natural resources and halting biodiversity loss are key environmental objectives of the European Union. High Nature Value farmland and forest (HNVFF) areas are associated with high biodiversity and other public goods. Currently, there are few dedicated HNV farmland and no HNV forest quality and quantity monitoring systems implemented in Europe, and none are in use in the Republic of Ireland. This study presents a conceptual framework to monitor quality, quantity, and change of HNVFF areas in the Republic of Ireland. The methods were tested using a set of monad sampling cells distributed across a range of representative landscapes. High resolution sampling cell habitat cover maps were developed using a national spatial database and recent orthoimagery, and a subsequent revision of the cell maps was performed using historical orthoimagery. This enabled a simulation of a 13-year monitoring cycle to compare trends in HNV farmland and forest quality, quantity, connectivity, and habitat categorical change. A contrasting trend of decreasing HNV farmland and increasing HNV forest quantity was observed over the monitoring period.Categorical habitat change detection identified a range of HNV habitat loss and gain, driven predominantly by land use intensification, but also by ecological succession and technogenic pressures. To upscale the monitoring to national scale, a sampling strategy using a recent landscape classification is proposed. The national monitoring approach aims to meet the needs of evolving policy requirements. This provides a useful example for countries seeking to establish their HNVFF and other land cover monitoring systems. Given the CV benefit noted in the CVOTs, GLP-1RAs and SGLT-2is are given preference in T2DM guidelines. While guidelines do not report potential gender difference, those differences exist. On restricting the CVOTs results to women with increased CV risk or established ASCVD, GLP-1RAs significantly reduced MACE while SGLT-2is resulted in a non-significant reduction. Reduction of enteric pathogens in animal manures before field application is essential for mitigating the risk of foodbome illness associated with produce. Aerobic composting of manures has been advocated as an effective treatment for reducing pathogen populations, and heat is a major factor contributing to pathogen inactivation. This study was initiated to determine the potential contribution of both thermal and nonthermal (pH, volatile acids, and ammonia) factors to pathogen inactivation during aerobic composting in bioreactors for mixtures containing manure from various sources (dairy, chicken, and swine). The test mixtures were formulated with an initial moisture content of 60% and a C:N ratio of 20:1, using straw and cottonseed meal as amendments. Mixtures were then inoculated with Salmonella and Listeria monocytogenes labeled with green fluorescent protein at initial populations of ca. 107 CFU/g. Three replicate trials of each treatment were conducted. Temperatures within the bioreactors were recorded at 30-min intervals, and duplicate samples were withdrawn daily from two sampling locations within the bioreactor. Significant regression models were derived relating decreases in pathogen populations to the degree of heat generated in the mixture (cumulative heat) and the pH of the mixture on the day before the pathogen losses were calculated (P < 0.0002). Although pathogens in swine manure compost mixtures were inactivated by the third day of composting, very little heat was generated in these mixtures, which were characterized by significantly higher levels of volatile acids compared with the other two compost mixtures. Therefore, volatile acids could help achieve pathogen inactivation when temperatures are too low such as when heat is lost too quickly at the surface of static compost piles or during winter composting. Abstract: Background: The extent to which low medication adherence in hypertensive individuals contributes to disparities in stroke and transient ischemic attack (TIA) risk is poorly understood. Methods: Investigators examined the relationship between self-reported medication adherence and blood pressure (BP) control (<140/90 mm Hg), Framingham Stroke Risk Score, and physician-adjudicated stroke/TIA incidence in treated hypertensive subjects (n = 15,071; 51% black; 57% in Stroke Belt) over 4.9 years in the national population-based REGARDS cohort study. Results: Mean systolic BP varied from 130.8 ± 16.2 mm Hg in those reporting high adherence to 137.8 ± 19.5 mm Hg in those reporting low adherence (P for trend < .0001). In logistic regression models, each level of worsening medication adherence was associated with significant and increasing odds of inadequately controlled BP (≥140/90 mm Hg; score = 1, odds ratio [95% confidence interval], 1.20 [1.09–1.30]; score = 2, 1.27 [1.08–1.49]; score = 3 or 4, 2.21 [1.75–2.78]). In hazard models using systolic BP as a mediator, those reporting low medication adherence had 1.08 (1.04–1.14) times greater risk of stroke and 1.08 (1.03–1.12) times greater risk of stroke or TIA. Campylobacter is an important human pathogen, and consumption of undercooked poultry has been linked to significant human illnesses. To reduce human illness, intervention strategies targeting Campylobacter reduction in poultry are in development. For more than a decade, there has been an ongoing national and international controversy about whether Campylobacter can pass from one generation of poultry to the next via the fertile egg. We recognize that there are numerous sources of Campylobacter entry into flocks of commercial poultry (including egg transmission), yet the environment is often cited as the only source. There has been an abundance of published research globally that refutes this contention, and this article lists and discusses many of them. along with other studies that support environment as the sole or primary source. One must remember that egg passage can mean more than vertical, transovarian transmission. Fecal bacteria, including Campylobacter, can contaminate the shell, shell membranes, and albumen of freshly laid fertile eggs. This contamination is drawn through the shell by temperature differential, aided by the presence of moisture (the "sweating" of the egg); then, when the chick emerges from the egg, it can ingest bacteria such as Campylobacter, become colonized, and spread this contamination to flock mates in the grow house. Improvements in cultural laboratory methods continue to advance our knowledge of the ecology of Campylobacter, and in the not-so-distant future, egg passage will not be a subject continuously debated but will be embraced, thus allowing the development and implementation of more effective intervention strategies. A cross-sectional study was performed to estimate the prevalence of Salmonella on retail market chicken carcasses in Colombia. A total of 1,003 broiler chicken carcasses from 23 departments (one city per department) were collected via a stratified sampling method. Carcass rinses were tested for the presence of Salmonella by conventional culture methods. Salmonella strains were isolated from 27% of the carcasses sampled. Logistic regression analysis was used to determine potential risk factors for Salmonella contamination associated with the chicken production system (conventional versus free-range), storage condition (chilled versus frozen), retail store type (supermarket, independent, and wet market), poultry company (integrated company versus nonintegrated company), and socioeconomic stratum. Chickens from a nonintegrated poultry company were associated with a significantly (P < 0.05) greater risk of Salmonella contamination (odds ratio, 2.0) than were chickens from an integrated company. Chilled chickens had a significantly (P < 0.05) higher risk of Salmonella contamination (odds ratio, 4.3) than did frozen chicken carcasses. [ABSTRACT FROM AUTHOR]

Sampling protocols for detecting Salmonella on poultry differ among various countries. In the United States, the U.S. Department of Agriculture Food Safety and Inspection Service dictates that whole broiler carcasses should be rinsed with 400 ml of 1% buffered peptone water, whereas in the European Union 25-g samples composed of neck skin from three carcasses are evaluated. The purpose of this study was to evaluate a whole carcass rinse (WCR) and a neck skin excision (NS) procedure for Salmonella and Escherichia coli isolation from the same broiler carcass. Carcasses were obtained from three broiler processing plants. The skin around the neck area was aseptically removed and bagged separately from the carcass, and microbiological analysis was performed. The corresponding carcass was bagged and a WCR sample was evaluated. No significant difference (α ≥ 0.05) in Salmonella prevalence was found between the samples processed by the two methods, but both procedures produced many false-negative Salmonella results. Prechill, 37% (66 carcasses), 28% (50 carcasses), and 51% (91 carcasses) of the 180 carcasses examined were positive for Salmonella by WCR, NS, and both procedures combined, respectively. Postchill, 3% (5 carcasses), 7% (12 carcasses), and 10% (17 carcasses) of the 177 carcasses examined were positive for Salmonella by the WCR, NS, and combination of both procedures, respectively. Prechill, E. coli plus coliform counts were 3.0 and 2.6 log CFU/ml by the WCR and NS methods, respectively. Several foodborne pathogens, including Salmonella species and campylobacters, are common contaminants in poultry and livestock. Typically, these pathogens are carried in the animal's intestinal tract asymptomatically; however, they can be shed in feces in large populations and be transmitted by other vectors from feces to animals, produce, or humans. A wide array of interventions has been developed to reduce the carriage of foodborne pathogens in poultry and livestock, including genetic selection of animals resistant to colonization, treatments to prevent vertical transmission of enteric pathogens, sanitation practices to prevent contamination on the farm and during transportation, elimination of pathogens from feed and water, feed and water additives that create an adverse environment for colonization by the pathogen, and biological treatments that directly or indirectly inactivate the pathogen within the host. An estimated 2,500 cases of listeriosis occur annually in the United States. Listeriosis is particularly severe among pregnant women and immunocompromised individuals. Little is known regarding the effect of the food matrix on the ability of L. monocytogenes to survive in the gastrointestinal tract and cause systemic infection. Mice were inoculated with various doses of L. monocytogenes in skim milk, Half & Half, or whipping cream to determine whether differences in milk fat content influence the ability of L. monocytogenes to survive passage through the gut and infect the liver or spleen. The number of fecal samples positive for L. monocytogenes increased with increasing doses of L. monocytogenes for all three vehicles. The number of L. monocytogenes cells isolated from liver or spleen of mice dosed with L. monocytogenes was not significantly different among treatment vehicles. Dose-response models revealed that as the dosage of L. monocytogenes was increased in different milk vehicles, the number of L. monocytogenes cells in liver or spleen also increased. Although fat content of food had no dose-dependent effect on L. monocytogenes infection in the murine gastrointestinal tract, we cannot discount the possibility that it may be a factor in L. monocytogenes infections of humans because of differences in the physiology of gastrointestinal tracts of mice and humans. Objective: We investigated whether the number of cigarettes smoked by women who had mildly abnormal cytologic study results could identify those at risk for high-grade dysplasia.Study Design: This was a prospective study of all women who were referred for colposcopy with a mildly abnormal cervical smear over a 4-year period. A detailed questionnaire was completed that evaluated sociodemographic characteristics including smoking history. Colposcopy then determined the degree of disease.Results: One hundred seventy-three women were referred with a mildly abnormal cervical smear. There was a significant relationship between the numbers of cigarettes smoked and the risk of high grade disease (P = .007). Once the number of cigarettes smoked daily exceeded 20, the risk of high grade cervical intraepithelial neoplasia was increased fivefold (odds ratio 5.85 [95% confidence interval 1.92 to 17.80]).Conclusion: Cigarette smoking is associated with a dose-dependent increased risk of cervical intraepithelial neoplasia grade 2 or 3 among women who have mildly abnormal cervical smears. There is no general agreement among paediatric urologists on how infants with spina bifida (SB) should be investigated after birth. Recently the EUA/ESPU guidelines have been published recommending a baseline DMSA scan in the first year of life and a Voiding Cystourethrogram (VCUG) or Videourodynamic (VUD) between the second and third month of life. The aim of this study was to evaluate the outcome of renal investigations in the first year of life in infants with SB to verify if an early DMSA scan is indicated in the management of this group of patients. All renal imaging, Renal and Bladder Ultrasound (RBUS), VCUGs, VUDs and DMSA were reviewed by two independent assessors to evaluate outcome. Seventy patients with spina bifida (40 girls) were enrolled between June 2015 and February 2020. An early VUD detected vesico-ureteral reflux (VUR) in 8/49 (16%) of patients. An early VUD also gave additional information on detrusor under or over activity, bladder trabeculation, end filling detrusor pressure (EFDP) and sphincteric incompetence. DMSA scan detected renal scarring in 4/68 (6%) patients. Three of these 4 patients had significant history of febrile UTIs while the fourth patient had grade 2 left sided VUR. The initial assessment of a newborn with myelodysplasia includes a Renal and Bladder Ultrasound during birth hospitalization. This study confirms the recently published EUA/ESPU guidelines on the management of neurogenic bladder in children and adolescents, which recommend a VUD or VCUG & Cystomanometry with Electromyogram (CMG) (if VUD not available) in the first 6–12 weeks of life. A selective approach to DMSA scan only in infants with SB who either had a febrile UTI or vesico-ureteric reflux would not have missed any scarring or dysplasia and would have saved 58 unnecessary nuclear scans. Data regarding Salmonella on raw poultry are very limited in China. The objective of this study was to determine the prevalence of Salmonel la on raw poultry at the retail level in six provinces and two national cities in China. Whole chicken carcasses (n = 1,152) were collected from three types of retail markets (large, small, and wet). All samples were analyzed for the presence of Salmonel la by using the U.S. Department of Agriculture, Food Safety Inspection Service method. Of 1,152 chicken samples, overall Salmonel la prevalence was 52.2%. The highest prevalence was observed in Guangxi Province (65.3%), next in Guangdong Province (64.6%), and then in Beijing (63.9%), Shaanxi Province (50.7%), Henan Province (47.9%), Shanghai (44.4%), and Fujian Province (42.4%), and lowest prevalence was observed in Sichuan Province (38.9%). Salmonel la prevalence was significantly different among the six provinces and two national cities. Salmonella prevalence was highest in the wet markets (54.4%) compared with the large markets (50.3%) and the small markets (52.1%), but differences were not significant (P > 0.05). Good manufacturing practices, good agricultural practices, and hazard analysis critical control point systems for Salmonel la control in poultry production at the farm, processing, and retail level should be implemented. The aim of this study was to gain a better understanding of the impact of rhizosphere/burrow oxidation by wetland plants and burrowing invertebrates on the biogeochemistry of metals and metalloids in salt marsh ecosystems. It was hypothesised that salt marsh plants and burrowing invertebrates could considerably affect the retention capacity of wetlandsfor metals through oxidation of the rhizosphere/burrow wall. Various soil, plant and porewater samples were collected from areasdominated by the plant species Spartina townsendii and Atriplex portulacoides and by the lugworm Arenicola marina, and from correspondingnearby unvegetated/uninhabited sites at North Bull Island salt marsh, Dublin Bay, Ireland. Samples were analysed for total Fe, Zn and As.The organic matter content (LOI), bulk density, water content and dry/fresh weight ratio of rhizosphere, burrow wall and bulk soil was measured for each species. DCB-extractable Fe, Zn and As, associated with the iron plaque on the roots of the two plant species were also determined. The presence of vegetation and, to a lesser extent, burrowing organisms were shown to have a significant effect on the concentration and accumulation of heavy metals in salt marsh soils. Iron and arsenic concentrations were significantly higher in vegetated/inhabited soils. compared to nearby unvegetated/uninhabited areas. Zinc showed the same trend but the difference was not statistically significant. The concentrations of Fe and As were also significantly higher in the rhizosphere soil around the plant roots and in the burrow walls ofArenicola compared to the bulk soil. For zinc, the same pattern was significant only for S. townsendii-dominated soils.
