25. Injektionstherapien bei Tendinopathien im Sport.

Abstract

Sehnenbeschwerden sind im sportorthopädischen Alltag einer der häufigsten Konsultationsgründe. Vermehrte sportliche Betätigung in der Freizeit, höhere Anforderungen im Leistungssport und neue Trendsportarten führten zu einer Zunahme der Inzidenz von Überlastungsschäden, insbesondere Tendinopathien und Sehnenrupturen. In den vergangenen Jahren haben sich jedoch auch das Verständnis der Pathogenese sowie die Diagnostik und Behandlungsmethoden enorm weiterentwickelt. Unter anderem stehen zahlreiche wirkungsvolle Injektionstherapien mit zunehmend guter Evidenz aus hochwertigen klinischen Studien zur Verfügung. Es ist unbestritten, dass Tendinopathien selten primär eine Entzündung zu Grunde liegt. Vielmehr führt ein multifaktorielles Geschehen zur Degeneration des Sehnengewebes. Die Therapie beruht somit auf der Analyse der auslösenden und unterhaltenden Faktoren, welche dann im Einzelnen adressiert werden müssen, um einen langfristigen Behandlungserfolg zu gewährleisten. Injektionstherapie spielen unterstützend eine immer wichtigere Rolle insbesondere vor dem Hintergrund des oft langwierigen Verlaufs und der besseren Verfügbarkeit, wirkungsvoller, kortison-freier Substanzen. In diesem Artikel werden die unterschieddlichen Präparate und Methoden vorgestellt und hinsichtlich Evidenzlage, Indikationen, Wirkungen und Nebenwirkungsprofilen analysiert. The number of tendon injuries has risen significantly in recent years. Increased leisure time sporting activity, higher demands in competitive sports as well as new trend sports led to an high incidence of overuse injuries in sports. In parallel, the understanding of the pathogenesis of tendinopathies improved and the diagnostic and treatment modalities expanded. It has become evident that tendinopathies are not primarily based on inflammation processes. Treatment needs to be individualized and is based on the analysis of the triggering and maintaining factors, that need to be addressed in order to ensure long-term success. Injection therapy is playing a rising important role. The various injection methods as well at the injectable substances are evaluated in this article with regard to indication and effect. 32. Das Deutsche Arthroskopieregister DART und „Big Data" in der Sportorthopädie.

Abstract

Unter dem Aspekt „Big Data" rückte die sportorthopädische Registerforschung in den letzten Jahren zunehmend in den Vordergrund. Register basieren dabei entweder auf klinischen Daten oder Versicherungs-/Verwaltungsdaten. Klinische Register messen den Erfolg von Prozeduren vorwiegend anhand patientenbezogener Outcome-Parameter – oft über mehrere Jahre. Im Deutschen Arthroskopieregister (DART), welches 2017 gegründet wurde und aktuell operative Eingriffe zu Knie-, Sprung-, Schulter- und Hüftgelenk erfasst, konnten bereits mehr als 8.000 (sport-)orthopädische Eingriffe und deren Ergebnisse erfassen. Zuletzt wurde das Sehnenregister International Tendon Registry (InTeRest) integriert, welches sowohl Daten zur konservativen als auch operativen Therapie von Sehnenverletzungen erfasst. Aktuelle Bestrebungen zielen darauf ab, die Patientenzahl weiter zu erhöhen und künstliche Intelligenz, insbesondere maschinelles Lernen, zur Auswertung von DART zu nutzen, um die Patientenversorgung mit innovativen Methoden zu verbessern. The use of orthopaedic registries to collect and analyze "big data" has gained popularity in recent years. Registries either collect clinical data or administrative claims. In clinical registries, the success of procedures is often assessed using patient-reported outcome measures – with often long follow-up time frames. The German Arthroscopy Registry (DART), which was initiated in 2017, has thus far registered over 8,000 knee, ankle, shoulder and hip joint procedures as well as their outcome. The International Tendon Registry (InTeRest) was newly added, which collects on both the conservative as well as the surgical treatment of tendon pathologies. Today, the aim of DART is to increase the amount of registered procedures and to utilize artificial intelligence, specifically machine learning, to analyze the data and ultimately, improve patient care. 34. Hyperpolarized Metabolic and Parametric CMR Imaging of Longitudinal Metabolic-Structural Changes in Experimental Chronic Infarction.

Abstract

Prolonged ischemia and myocardial infarction are followed by a series of dynamic processes that determine the fate of the affected myocardium toward recovery or necrosis. Metabolic adaptions are considered to play a vital role in the recovery of salvageable myocardium in the context of stunned and hibernating myocardium. The potential of hyperpolarized pyruvate cardiac magnetic resonance (CMR) alongside functional and parametric CMR as a tool to study the complex metabolic-structural interplay in a longitudinal study of chronic myocardial infarction in an experimental pig model is investigated. Metabolic imaging using hyperpolarized [1-13C] pyruvate and proton-based CMR including cine, T 1 /T 2 relaxometry, dynamic contrast-enhanced, and late gadolinium enhanced imaging were performed on clinical 3.0-T and 1.5-T MR systems before infarction and at 6 days and 5 and 9 weeks postinfarction in a longitudinal study design. Chronic myocardial infarction in pigs was induced using catheter-based occlusion and compared with healthy controls. Metabolic image data revealed temporarily elevated lactate-to-bicarbonate ratios at day 6 in the infarcted relative to remote myocardium. The temporal changes of lactate-to-bicarbonate ratios were found to correlate with changes in T 2 and impaired local contractility. Assessment of pyruvate dehydrogenase flux via the hyperpolarized [13C] bicarbonate signal revealed recovery of aerobic cellular respiration in the hibernating myocardium, which correlated with recovery of local radial strain. This study demonstrates the potential of hyperpolarized CMR to longitudinally detect metabolic changes after cardiac infarction over days to weeks.
