Background: Liver disease is an increasing cause of morbidity and mortality in the United Kingdom and can be challenging to live with in the advanced stages. There has been little research exploring the healthcare experiences of UK individuals with decompensated disease when the liver cannot carry out its functions properly. A PhD research project was developed with people who have liver disease to explore care experiences in decompensated advanced liver disease. Public involvement (PI) is an essential aspect of meaningful health research, and this paper reports on the progression of our PI approach in this ongoing study. Objective: To embed PI throughout the research project to ensure that the study is meaningful to individuals with liver disease and the people who support them. Methods: The research adopts a Constructivist Grounded Theory methodology to develop a theory of care experience. Various PI approaches were considered in developing the PI strategy for this qualitative study. Initially, Embedded consultation was the preferred model, which has evolved to include aspects of collaboration and coproduction. A PI group was set up to oversee the project through the national public engagement website VOICE, and reflections on PI from three members of the group are included in this paper to illuminate the PI process. Results: Six individuals with liver disease and three carers from across the United Kingdom are part of an ongoing PI group. Their role includes commenting on the findings of the systematic literature review for this project and contributing to decisions about recruitment, data collection and data analysis. Additionally, they had a direct impact on changing the focus of the research. The PI group will continue involvement until the completion of the project. Conclusion: Successfully embedding PI into doctoral research, as demonstrated in this project, requires commitment, planning and dedication to reciprocal working for the benefit of PI contributors as well as the research. This approach could be adopted by other postgraduate researchers. Background: This paper describes a UK-based study, SPICES-Sussex, which aimed to co-produce and implement a community-based cardiovascular disease (CVD) risk assessment and reduction intervention to support underserved populations at moderate risk of CVD. The objectives were to enhance stakeholder engagement; to implement the intervention in four research sites and to evaluate the use of Voluntary and Community and Social Enterprises (VCSE) and Community Health Worker (CHW) partnerships in health interventions. Methods: A type three hybrid implementation study design was used with mixed methods data. This paper represents the process evaluation of the implementation of the SPICES-Sussex Project. The evaluation was conducted using the RE-AIM framework. Results: Reach: 381 individuals took part in the risk profiling questionnaire and forty-one women, and five men participated in the coaching intervention. Effectiveness: quantitative results from intervention participants showed significant improvements in CVD behavioural risk factors across several measures. Qualitative data indicated high acceptability, with the holistic, personalised, and person-centred approach being valued by participants. Adoption: 50% of VCSEs approached took part in the SPICES programme, The CHWs felt empowered to deliver high-quality and mutually beneficial coaching within a strong project infrastructure that made use of VCSE partnerships. Implementation: Co-design meetings resulted in local adaptations being made to the intervention. 29 (63%) of participants completed the intervention. Practical issues concerned how to embed CHWs in a health service context, how to keep engaging participants, and tensions between research integrity and the needs and expectations of those in the voluntary sector. Maintenance: Several VCSEs expressed an interest in continuing the intervention after the end of the SPICES programme. Conclusion: Community-engagement approaches have the potential to have positively impact the health and wellbeing of certain groups. Furthermore, VCSEs and CHWs represent a significant untapped resource in the UK. However, more work needs to be done to understand how links between the sectors can be bridged to deliver evidence-based effective alternative preventative healthcare. Reaching vulnerable populations remains a challenge despite partnerships with VCSEs which are embedded in the community. Purpose: This paper aims to investigate the association of COVID-19 confirmed cases and deaths with mental health, unemployment and financial markets-related search terms for the USA, the UK, India and worldwide using Google Trends. Design/methodology/approach: The authors use Spearman's rank correlation coefficients to assess the relationship between relative search volumes (RSVs) and mental health, unemployment and financial markets-related search terms, with the total confirmed COVID-19 cases as well as deaths in the USA, UK, India and worldwide. The sample period starts from the day 100 cases were reported for the first time, which is 7 March 2020, 13 March 2020, 23 March 2020 and 28 January 2020 for the US, the UK, India and worldwide, respectively, and ends on 25 June 2020. Findings: The results indicate a significant increase in anxiety, depression and stress leading to sleeping disorders or insomnia, further deteriorating mental health. The RSVs of employment are negatively significant, implying that people are hesitant to search for new jobs due to being susceptible to exposure, imposed lockdown and social distancing measures and changing employment patterns. The RSVs for financial terms exhibit the varying associations of COVID-19 cases and deaths with the stock market, loans, rent, etc. Research limitations/implications: This study has implications for the policymakers, health experts and the government. The state governments must provide proper medical facilities and holistic care to the affected population. It may be noted that the findings of this study only lead us to conclude about the relationship between COVID-19 cases and deaths and Google Trends searches, and do not as such indicate the effect on actual behaviour. Originality/value: To the best of the authors' knowledge, this is the first attempt to investigate the relationship between the number of COVID-19 cases and deaths in the USA, UK and India and at the global level and RSVs for mental health-related, job-related and financial keywords. Background: Treatment fidelity refers to the degree to which an intervention is implemented as intended. Promoting treatment fidelity is important to achieve a valid comparison in intervention research. However, it is often underreported: few studies detail the use and development of fidelity measures. This study aims to promote the treatment fidelity of a modified version of the Derbyshire Language Scheme (M‐DLS), a manualised intervention for children with language difficulties, by exploring participants' opinions on training and intervention delivery. Results inform development of a checklist and scoring system to monitor and promote treatment fidelity in a comparison trial. Method: Ten student speech and language therapists (SLTs) and two research assistants (RAs) participated in the study. All received training on the M‐DLS, and 10 were video‐recorded completing role‐plays of an M‐DLS session in small groups. Feedback was gathered after training and role‐plays in focus groups and interviews. Feedback was interpreted using the constructs of the Theoretical Domains Framework (TDF). A treatment fidelity checklist was then developed using the feedback. The first author and two RAs rated role‐play videos using the checklist to trial it to inform amendments and to promote interrater reliability. Interrater agreement was calculated using Spearman's test of correlation. Results: Participants discussed the importance of having clear materials and time to practise sessions. They suggested amendments to the materials and training to promote treatment fidelity. The checklist and scoring system accounted for participants' suggestions, with amendments detailed in a log. Spearman's correlation results suggested agreement between the raters was strong. Conclusions: Results emphasise the importance of training quality, practice and reflective opportunities and clear materials to promote treatment fidelity. The construction of the checklist and scoring system was described in detail, informing the development of future checklists. After further trialling, the checklist can be used to ensure the M‐DLS is delivered with high treatment fidelity in the comparison trial. What this paper adds: What is already known on this subject: Treatment fidelity is an essential component of intervention effectiveness and efficacy studies, ensuring the intervention is delivered as intended. It is also an essential component of evidence-based clinical practice. However, few research studies report the treatment fidelity process or publish the checklists used, depriving clinicians of useful information for implementation. What this study adds: This study describes in detail the iterative process of treatment fidelity checklist development, engaging those implementing the intervention in development. This ensured clarity and interrater reliability of the checklist. Furthermore, a novel scoring system was developed so that accuracy of implementation can be easily compared across users and across practice attempts. Background: Very young children from lower socioeconomic status (SES) backgrounds often show poorer language development. Whilst there have been attempts to provide early intervention programmes, these sometimes miss the most disadvantaged groups. Aims: This report presents preliminary feasibility and effectiveness data for a novel language intervention designed for parents of toddlers in the United Kingdom. Methods and Procedures: In total, 43 UK families of 2–4‐year‐olds were recruited to the study, half of whom completed an 8‐week course (Tots Talking) focussed on parent interaction, and half of whom acted as wait‐list controls. Results and Outcomes: Results suggest that such programmes are feasible for families with 86% staying in the intervention. In addition, greater changes in underlying communication skills such as joint attention and gesture were evident compared to wait‐list controls. Conclusions and Implications: We conclude that pre‐verbal skills may be more important to measure as initial outcomes than language or vocabulary change in this population. AI-tools such as ChatGPT can assist researchers to improve the performance of the research process. This paper examines whether researchers could apply ChatGPT to develop and empirically validate new research scales. The study describes a process how to prompt ChatGPT to assist the scale development of a new construct, using the example of the construct of perceived value of ChatGPT-supported consumer behavior. The paper reports four main empirical studies (US: N = 148; Australia: N = 317; UK: N = 108; Germany: N = 51) that have been employed to validate the newly developed scale. The first study purifies the scale. The following studies confirm the adjusted factorial validity of the reduced scale. Although the empirical data imply a simplification of the initial multi-dimensional scale, the final three-dimensional operationalization is highly reliable and valid. Background: Selection into UK-based GP training has used the Multi-Specialty Recruitment Assessment (MSRA) and a face-to-face selection centre (SC). The MSRA comprises of a situational judgement test and clinical problem-solving test. The SC was suspended during the COVID-19 pandemic. Evidence is needed to guide national and international selection policy. Aim: To evaluate the validity of GP training selection. Design & setting: A retrospective cohort study using data from UK-based national recruitment to GP training, from 2015–2021. Method: Data were available for 32 215 GP training applicants. The ability of scores from the specialty selection process to predict subsequent performance in the Clinical Skills Assessment (CSA) of the Membership of the Royal College of General Practitioners examination was modelled using path analysis. The effect sizes for sex, professional family background, and world region of qualification were estimated. Results: All component scores of the selection process demonstrated statistically significant independent relationships with CSA performance (P<0.001), thus establishing their predictive validity. All were sensitive to demographic factors. The SC scores had the weakest relationship with future CSA performance. However, for candidates with MSRA scores below the lowest quartile, the relative contribution of the SC scores to predicting CSA performance was similar to that observed for MSRA components. Conclusion: The MSRA has predictive validity in this context. Re-instituting an SC for those with relatively low MSRA scores should be considered. This qualitative study investigates the factors that affect the job satisfaction of home caregivers. It draws upon a thematic analysis of data from in-depth interviews with home caregivers in the UK and finds that both visible and invisible factors have an effect. Overall factors pertaining to the way the caregiving agencies are run and behave have a significant influence on caregivers' job satisfaction. Identifying these factors will assist both policymakers and professional care agencies to be aware of the needs of caregivers in practice. These experiences can be used as references to handle the challenges of an ageing society in other countries with similar care industry characteristics. [ABSTRACT FROM AUTHOR]

(1) Background: Preterm premature rupture of membranes (PPROM) has been associated with increased perinatal morbidity, but the effect of PPROM on respiratory disease has not been previously quantified. We hypothesised that PPROM would be associated with a higher incidence of invasive ventilation. (2) Methods: A retrospective cohort study at the Neonatal Unit at King's College Hospital NHS Foundation Trust, London, UK, was conducted on infants born before 37 weeks of gestation. PPROM was defined as the rupture of membranes for >48 h. (3) Results: We reviewed 1901 infants (434 with PPROM) with a median (IQR) gestational age of 32.4 (28.7–35.0) weeks. The median (IQR) duration of rupture of membranes in the infants with PPROM was 129 (78–293) h. The incidence of invasive ventilation was 56% in the infants with PPROM and 46% in the infants without PPROM (p < 0.001). Following regression analysis, PPROM was significantly related to a higher incidence of invasive ventilation (odds ratio: 1.48; 95% CI: 1.13–1.92, adjusted p = 0.004) after adjusting for birth weight [odds ratio = 0.34; 95% CI: 0.33–0.43, adjusted p < 0.001], Apgar score at 10 min [odds ratio =0.61; 95% CI: 0.56–0.66, adjusted p < 0.001] and antenatal corticosteroid use (adjusted p = 0.939). Objectives: The aim of this clinical audit was to assess patient demographics, outcomes and experience with care in patients who received acupuncture in a private practice setting in the United Kingdom. Methods: Demographic and clinical data were extracted from patients' records over a 7-year period. The Measure Yourself Medical Outcomes Profile (MYMOP) questionnaire and an adapted Patient Global Impression of Change (PGIC) scale were used routinely to monitor patient outcomes over an 18-month period. Finally, a retrospective questionnaire was used to assess patient beliefs regarding treatment effectiveness, adverse events and overall experience with care. Patients not providing consent or known to be deceased were excluded. Results: Data were collected for 306 patients presenting with 376 separate health complaints, 58% of which were musculoskeletal. Follow-up outcomes (MYMOP scores (n = 51) and PGIC scale responses (n = 50)) showed a clinically significant improvement compared to baseline for the majority of health complaints (93% of PGIC scores were 'improved' and 79% MYMOP demonstrated > 1 point change). Total mean MYMOP severity scores were reduced by almost 50% (p < 0.001) after 1–4 weeks, and this was sustained in the medium-to-long term. There was a strong negative correlation (r = −0.767, p < 0.001) between the MYMOP and PGIC scores. A total of 118 health complaints were reported by 85/255 patients who responded to a retrospective questionnaire. Over 84% of patients believed that the treatments they received were 'effective' at addressing their health complaints. Seven minor adverse events were reported and four patients experienced negative treatment outcomes. Conclusions: Although musculoskeletal conditions were the most common, this audit found that patients sought treatment for a wide range of predominantly chronic health complaints, for many of which there is a currently a lack of quality evidence to support the use of acupuncture. Video gamers can play to negate the psychological impact of stress, which may become problematic when users over-rely on the stress relief potential of gaming. This study used a repeated measures experimental design to investigate the relationships between stress, video gaming, and problematic video gaming behaviours in a convenience sample of 40 students at a UK university. The results indicated that positive affect increased and negative affect decreased, whilst a biological stress measure (instantaneous pulse rate) also decreased after a short video gaming session (t(36) = 4.82, p < 0.001, d = 0.79). The results also suggested that video gaming can act as a short-term buffer against the physiological impact of stress. Further research should focus on testing individuals who have been tested for gaming disorder, as opposed to the general population. Research could also utilise variations of the methodological framework used in this study to examine the intensity of a stress relief effect under different social situations. Purpose: The number of university students exhibiting mental health concerns have surged considerably in the past decade. Amongst a number of potential contributing factors, this study aims to assess the role of a broader societal phenomenon; the shift in emphasis in our interactions from the physical to the virtual environments. Specifically, a decrease in nature contact and a contrasting increase in smartphone use are identified as two pathways in which this shift may impact negatively on mental health. Previous research evidences both facets as consistent correlates of depression, although limited research extends these associations to the student population or attempts to establish an interaction between the two. Design/methodology/approach: The current study recruited a sample of 380 first-year undergraduate students, via an online survey, to assess if problematic smartphone use (SAS-SV) and nature contact (NCQ) were significant predictors of depression (PHQ-9). Findings: Nature contact frequency and smartphone use were significant predictors of depression. Simple Summary: There are around 11 million pet dogs in the United Kingdom, yet there is more to learn about how best to ensure their welfare needs are met. We know that owners have an impact on their dog's well-being and studies have suggested that what people do before they get their dog can subsequently impact their dog's welfare. We used data collected by the People's Dispensary for Sick Animals (PDSA) for the 2017 PDSA Animal Wellbeing (PAW) Report, the primary nationally representative means of monitoring the well-being of pets in the UK. We investigated associations between how much research people did before they got their dog and what they did or thought about. We found that owners who did more research before getting their dog were likely to be younger or have a higher education level. They were also more likely to have heard of the 'five welfare needs', have a realistic idea of the cost of owning a dog, and know the best place to go for help with their dog's behaviour. These findings help us understand what influences dog owners and suggest that doing more research before getting a dog is linked to improved knowledge, understanding, and behaviours. Despite many dogs living in homes in the UK, there is still more to know about the welfare of those individual animals. Past research has shown that owners' thoughts and behaviours have a substantial impact on their dog's welfare. This study aimed to better understand owners' pre-purchase behaviours, knowledge and understanding, and ownership practices, and explore any associations between these factors and their dog's welfare. We conducted further analysis of the data collected by People's Dispensary for Sick Animals (PDSA) for their 2017 PDSA Animal Welfare (PAW) Report (n = 1814). We extracted variables to describe owner demographics (7), pre-purchase behaviours (1), knowledge and understanding (4), welfare indicators, (2) and ownership practices (4), and we tested for an association between these variables. We found more pre-purchase research was carried out by younger respondents and those with a higher education level. Also, more research was associated with feeling more informed about the five welfare needs and knowing to seek help for behaviour from appropriate sources. The COVID-19 pandemic has had a significant impact on countries worldwide, including the United Kingdom (UK). The UK has faced numerous challenges, but its response, including the rapid vaccination campaign, has been noteworthy. While progress has been made, the study of the pandemic is important to enable us to properly prepare for future epidemics. Collaboration, vigilance, and continued adherence to public health measures will be crucial in navigating the path to recovery and building resilience for the future. In this article, we propose an overview of the COVID-19 situation in the UK using both mathematical (a nonlinear differential equation model) and statistical (time series modeling on a moving window) models on the transmission dynamics of the COVID-19 virus from the beginning of the pandemic up until July 2022. This is achieved by integrating a hybrid model and daily empirical case and death data from the UK. We partition this dataset into before and after vaccination started in the UK to understand the influence of vaccination on disease dynamics. We used the mathematical model to present some mathematical analyses and the calculation of the basic reproduction number ( R 0 ). Following the sensitivity analysis index, we deduce that an increase in the rate of vaccination will decrease R 0 . Also, the model was fitted to the data from the UK to validate the mathematical model with real data, and we used the data to calculate time-varying R 0 . The homotopy perturbation method (HPM) was used for the numerical simulation to demonstrate the dynamics of the disease with varying parameters and the importance of vaccination. Furthermore, we used statistical modeling to validate our model by performing principal component analysis (PCA) to predict the evolution of the spread of the COVID-19 outbreak in the UK on some statistical predictor indicators from time series modeling on a 14-day moving window for detecting which of these indicators capture the dynamics of the disease spread across the epidemic curve. The results of the PCA, the index of dispersion, the fitted mathematical model, and the mathematical model simulation are all in agreement with the dynamics of the disease in the UK before and after vaccination started. Conclusively, our approach has been able to capture the dynamics of the pandemic at different phases of the disease outbreak, and the result presented will be useful to understand the evolution of the disease in the UK and future and emerging epidemics. Population-based prospective studies, such as UK Biobank, are valuable for generating and testing hypotheses about the potential causes of human disease. We describe how UK Biobank's study design, data access policies, and approaches to statistical analysis can help to minimize error and improve the interpretability of research findings, with implications for other population-based prospective studies being established worldwide.
