This paper describes five attacks of Kaposi's varicelliform eruption which occurred during a period of 18 months in a woman with Darier's disease. Herpes simplex virus type 1 was isolated from the lesions in three of the four episodes which were studied in the laboratory and Coxsackievirus type A16 from the remaining attack. It is suggested that Coxsackievirus A16 may be a hitherto overlooked cause of Kaposi's varicelliform eruption. Infection with any one of a number of viruses may be accompanied by vesicular lesions of the skin. Herpes simplex virus and vaccinia virus arc the two most frequently found affecting established skin disease, especially atopic eczema, when the term Kaposi's varicelliform eruption is commonly used to describe the condition. Hyperkeratosis follicularis, Darier's disease, is an uncommon dyskeratosis with a characteristic clinical and histological picture and may be complicated by secondary viral infection. In most of the published reports of Kaposi's varicelliform eruption in Darier's disease, herpes simplex virus has either been assumed to be the infecting agent (Bleiberg & Kiosk, 1951; Kesten, 1952; Hitselberger & Burns, 1961) or actually proven to be by virus isolation (von Weiss, Kibrick & Lever, 19655 Doeglas & Moolhuysen, 1969; Izumi & Goldschmidt, 1970). Infection with vaccinia virus has also been recorded (Gerstcin & Shelley, 1960). Elimination of flour and milk from the diet of an adult atopic patient caused remission of symptoms on two occasions. Relapses followed a return to full diet. Investigations showed hypoglycaemia, hypercholesterolaemia and a low serum lipase.

Immunosuppressive agents have been used extensively in the treatment of the nephrotic syndrome, but many problems regarding dosages, appropriate agents, and short- and long-term toxicity require resolution before their use can become generalized. The only group of patients clearly demonstrated to have had benefit from immunosuppressive therapy are the frequent relapsers. At this time, it appears reasonable to recommend that a child with nephrosis who is being considered for immunosuppressive therapy ought to be referred to an appropriate treatment center and a course of therapy decided upon jointly by both the referring and the consultant pediatricians. The article focuses on the research paper "Hydrocortisone Ointment in the Eczemas," by Ronald Church, which was published in a 1955 issue of the "British Medical Journal." It describes the one hundred and five cases of eczema which were treated with hydrocortisone ointment. Improvement was registered in 76% of the cases. Response was observed to be poorer in lichenified eczemas. There were also chances of relapse in certain cases.

The results of a follow-up study of 200 males committed under the NARA Act have been compared with general findings of other such studies. It was found that, if relapse is defined as any re-use of narcotics, the observed relapse rate of 87 per cent is equivalent to the 80-90 per cent relapse rates reported in other studies. Variables which have been related to relapse in other studies were also examined in this project. Some support was found for Winick's "maturation hypothesis". Although no relationship war found between length of addiction and relapse, age was found to be related to relapse. Specifically, it was found that patients under 30 years of age used narcotics regularly or became readdicted at much higher rates than those over 30. Similarly, the rates of total abstinence or occasional use of narcotics were higher for those over 30. These findings indicate that Winick's hypothesis cannot be completely rejected. Employment was also found to be related to relapse in that those who found jobs relapsed less often than those unable to obtain employment. Most of the problems patients encountered in seeking a job seemed to be related more to their own personality problems than to other factors. Two other variables, ethnicity and education, were generally unrelated to relapse. Both the counselors and patients cited three general factors which led to relapse. One factor was use of narcotics to alleviate interpersonal stress. The second factor was the patient's craving or enjoyment of the euphoric effects and the "magnetic" pull of the addict subculture. Inability to cope with his own problems and frustration constituted the third general factor which helped to explain the relapse phenomenon. In citing reasons for abstinence, counselors anti patients agreed that the patient's desire to stay clean, effectiveness of therapy, emotional support of the family and fear of the consequences of continued involvement in the drug subculture were the mast important factors in a patient remaining drug free. . , 453-457. . During the seven-year period 1965 to 1972 mitral valvotomy for mitral stenosis was undertaken in 72 children aged 15 years or less. Excellent results were achieved from closed transventricular instrumental valvotomy unless the valve was calcified, there was a jet of mitral incompetence or a second valvotomy was being undertaken. Severe pulmonary arterial hypertension was present before operation and regressed after a successful valvotomy. There was a low incidence of overt antecedent rheumatic fever. A case of pityriasis rubra pilaris with concomitant ascorbic acid deficiency is presented in an elderly Negro woman. Vitamin in shortage was incidental but treatment with vitamin C yielded favourable results; spontaneous resolution seemed unlikely. Addition of vitamin A in less than optimal dose checked a mild cutaneous relapse awl enhanced further the involuting process. Conjoint administration of vitamins A and C may prove ideal in management of pityriasis rubra pilaris. The therapeutic value of ascorbic and in this dermatosis warrants further study. Cobalt 60 teletherapy was given to 121 patients with complicated peptic ulcer as a supplement to medical treatment. There has been a minimum follow-up of 4 years in 79 patients. Therapy was well tolerated with minimum reactions. A total of 629 gastric secretion studies were done. A significant (greater than 50 per cent decrease or achlorhydria) decrease in basal gastric secretion in the first 6 months was obtained by 90 per cent of the patients, while only 63 per cent obtained a significant decrease after 6 months. With Histalog-stimulated acid secretion, 55 per cent obtained a significant decrease within 6 months, and 20 per cent had a significant decrease after 6 months. Cobalt 60 teletherapy usually decreased gastric acid secretion, but there was a return of hyperacidity in some patients 6 months or longer after therapy. A good response, without any evidence of recurrent ulcer, was obtained in 87 patients. There was a questionable recurrence of ulcer in 13 patients, and definite recurrent ulcers in 21 patients. Sixteen patients underwent surgery. Irradiation with Cobalt 60 does not replace medical therapy, but may help many with complicated or "surgical" ulcers as a supplement to medical treatment. Such therapy may be particularly indicated if there are other conditions, such as cardiac lesions, emphysema, hypertension, that contraindicate surgery. Our experience and that of others has shown that regional enteritis may periodically resolve itself without effective medical treatment or following a by-pass operation, and there may be no recurrence in 35 to 50 per cent of the cases. Also, recurrence developed after any form of treatment (other than with crude liver extract) in 50 to 65 per cent of those cases observed for ten years or more. Furthermore, there was the probability that every case would not respond to treatment with crude liver extract as our cases have responded. Certainly this should be true in the neglected cases when the fibrous changes in the intestinal wall had become mature and irreversible. Under these conditions our series of cases was too small to arrive at positive conclusions. We believed that further trial with crude liver extract and research directed to other medical methods were indicated by these experiences because: 1. No operations that involved resection or by-passing were done. 2. Relief of symptoms and the disappearance of roentgen findings occurred too promptly to be coincidental. 3. Recurrent attacks have responded promptly to the same treatment and the recurrence rate appeared to be lower. 4. A case complicated by fecal fistulas, a condition believed to be a definite indication for resection, was resolved without operation. The quantity of the extract to be given any one case was on an empirical basis. It was our belief that regional enteritis was a deficiency state. The "curative" agent was found in the complex crude liver substance. Pathological tissue diagnosis was uncertain in the early stages and in these instances the combined observations of pathologist and surgeon were necessary to arrive at an accurate diagnosis. Roentgen ray examinations in the early stages were of diagnostic value only when correlated with the clinical signs. The article presents a case report of relapse of porphyria cutanea tarda. The patient had severe colic and neurological disturbances. Porphobilinogen and uroporphyrin were detected in the urine. Electromyography indicated gross denervation of the type found in polyneuritis.
