Contains fulltext : 244604.pdf (Publisher’s version ) (Closed access) BACKGROUND: The aim was to evaluate self-reported reproductive characteristics and markers of ovarian function in a nationwide cohort of female survivors of childhood acute lymphoblastic leukemia (ALL), because prior investigations have produced conflicting data. PROCEDURE: Self-reported reproductive characteristics were assessed by questionnaire among 357 adult 5-year survivors, treated between 1964 and 2002, and 836 controls. Ovarian function was assessed by serum levels of anti-Müllerian hormone (AMH), follicle-stimulating hormone (FSH), and inhibin B and by antral follicle count (AFC). Differences between controls and (subgroups of) survivors (total group, chemotherapy [CT]-only group, CT and radiotherapy [RT] group) were analyzed. Abstract

Background: Cancer therapy-related cardiac dysfunction and heart failure are major problems in long-term childhood cancer survivors (CCS). We hypothesize that assessment of more sensitive echo- and electrocardiographic measurements, and/or biomarkers will allow for improved recognition of patients with cardiac dysfunction before heart failure develops, and may also identify patients at lower risk for heart failure. Objective: To describe the methodology of the Dutch LATER cardiology study (LATER CARD). Methods: The LATER CARD study is a cross-sectional study in long-term CCS treated with (potentially) cardiotoxic cancer therapies and sibling controls. We will evaluate 1) the prevalence and associated (treatment related) risk factors of subclinical cardiac dysfunction in CCS compared to sibling controls and 2) the diagnostic value of echocardiography including myocardial strain and diastolic function parameters, blood biomarkers for cardiomyocyte apoptosis, oxidative stress, cardiac remodeling and inflammation and ECG or combinations of them in the surveillance for cancer therapy-related cardiac dysfunction. 16. Cardiovascular disease after treatment for childhood cancer

Abstract

Een op de acht overlevenden van kinderkanker die bestraald zijn én behandeld werden met anthracyclines, heeft dertig jaar later een ernstige hartaandoening. Om kinderen met kanker te blijven volgen en controleren op late bijwerkingen, riep het Emma Kinderziekenhuis AMC in 1996 de Polikliniek Late Effecten Kindertumoren (PLEK) in het leven. Daarnaast beschikken onderzoekers over een database met gegevens over diagnose en behandeling van alle kinderen met kanker die in het EKZ/AMC opgenomen waren. Heleen van der Pal deed onderzoek in een cohort van 1362 patiënten die minstens vijf jaar na de behandeling nog in leven zijn. Ze vond 50 ernstige hartaandoeningen bij 42 patiënten. Meer dan de helft kampte met hartfalen, gevolgd door hartklepafwijkingen, ischemische hartziekten (zoals een infarct) en ritmestoornissen. Dertig jaar na de behandeling was de kans op een ernstige hartaandoening het grootst in de groep die zowel radiotherapie als anthracyclines kreeg. Een op de acht patiënten loopt dat risico. Uit echo’s van het hart bij jongvolwassenen die in hun kindertijd met middelen behandeld werden die potentieel schadelijk zijn voor het orgaan, bleek dat 27 procent van de 525 personen een verminderde hartfunctie had. Van der Pal denkt dat zij gaandeweg hartaandoeningen ontwikkelen die klachten kunnen geven. Sommige patiënten krijgen daarom preventief medicijnen. Ook wordt er bij een aantal van hen vaker een echo gemaakt.

20. The usefulness of growth hormone treatment for psychological status in young adult survivors of childhood leukaemia; an open-label study

Abstract

Background: To reduce the risk of brain damage children with acute lymphoblastic leukaemia (ALL) are nowadays mainly treated with intrathecal chemotherapy (ITC) instead of central nervous system (CNS) radiation therapy (CRT) to prevent CNS relapse. However, chemotherapy may also lead to cognitive deficits. As growth hormone deficiency (GHD) or impaired growth hormone secretion are frequently found in ALL patients treated with cranial radiation therapy and/or chemotherapy, we hypothesized that GH therapy may reduce cognitive deficits in these patients. Methods: Twenty young adult survivors of childhood ALL with reduced bone mineral density (<-1 SD) and/or low IGF-I SD-scores (<-1 SD) were included in the study. A final group of 13 patients (9 males and 4 females), mean age 23.7 ± 2.9 years (range 20 - 29.7) completed a 2-year treatment with GH. IQ and neuropsychological performance were assessed at pre-treatment (T1) and after one (T2) and two (T3) years. ANOVA was performed with assessment at T1, T2 and T3 as repeated measurements factor. Relations between test score changes and changes of IGF-I levels were determined by calculating the Pearson correlation coefficient. Results: Scores on the cognitive tests were in the normal range. Verbal short- and long-term memory performance decreased between T1 and T2, and increased between T2 and T3. Performance at T3 was not significantly different from that at T1. Performance for sustained attention improved from T1 to T2 and from T1 to T3. Visual-spatial memory was improved after one year of GH treatment. A significant positive correlation was found for Δ IGF-I (T2-T1) with difference scores of visual-spatial memory (T2-T1 and T3-T1), indicating that IGF-I increase after one year of GH treatment is associated with increase in cognitive-perceptual performance at month 12 and 24. Abstract

Long term adverse side events among childhood cancer survivors (CCS) can negatively impact quality of life. A concern are late effects among CCS treated with radiotherapy directed to the upper body (i.e. brain, neck, and thorax, including spinal radiotherapy and total body irradiation). Damage to these areas might induce cerebrovascular events or subsequent central nervous system (CNS) tumors, including meningiomas. To improve quality of late effects care for survivors, clinical research studying long term adverse events is crucial. Evidence-based guidelines and recommendations are warranted to permit deliberate choices in clinical decision-making. The objectives of this thesis are: a) to increase knowledge on risk factors and clinical characteristics of late effects in CCS after treatment with upper body radiotherapy; b) to provide guidance on surveillance of Childhood Adolescent Young Adult cancer survivors (CAYA cancer survivors) with respect to subsequent CNS tumors and concerning COVID-19. In the first part of this thesis two retrospective clinical research studies are presented concerning cerebrovascular events (CVE) and meningioma as long-term adverse events after upper body radiotherapy.
