2. Exploring Physiological Responses in Virtual Reality-based Interventions for Autism Spectrum Disorder: A Data-Driven Investigation

Virtual Reality (VR) has emerged as a promising tool for enhancing social skills and emotional well-being in individuals with Autism Spectrum Disorder (ASD). Through a technical exploration, this study employs a multiplayer serious gaming environment within VR, engaging 34 individuals diagnosed with ASD and employing high-precision biosensors for a comprehensive view of the participants' arousal and responses during the VR sessions. Participants were subjected to a series of 3 virtual scenarios designed in collaboration with stakeholders and clinical experts to promote socio-cognitive skills and emotional regulation in a controlled and structured virtual environment. We combined the framework with wearable non-invasive sensors for bio-signal acquisition, focusing on the collection of heart rate variability, and respiratory patterns to monitor participants behaviors. Further, behavioral assessments were conducted using observation and semi-structured interviews, with the data analyzed in conjunction with physiological measures to identify correlations and explore digital-intervention efficacy. Preliminary analysis revealed significant correlations between physiological responses and behavioral outcomes, indicating the potential of physiological feedback to enhance VR-based interventions for ASD. The study demonstrated the feasibility of using real-time data to adapt virtual scenarios, suggesting a promising avenue to support personalized therapy. The integration of quantitative physiological feedback into digital platforms represents a forward step in the personalized intervention for ASD. 7. Effects of discrete energy and helicity conservation in numerical simulations of helical turbulence

Helicity is the scalar product between velocity and vorticity and, just like energy, its integral is an in-viscid invariant of the three-dimensional incompressible Navier-Stokes equations. However, space-and time-discretization methods typically corrupt this property, leading to violation of the inviscid conservation principles. This work investigates the discrete helicity conservation properties of spectral and finite-differencing methods, in relation to the form employed for the convective term. Effects due to Runge-Kutta time-advancement schemes are also taken into consideration in the analysis. The theoretical results are proved against inviscid numerical simulations, while a scale-dependent analysis of energy, helicity and their non-linear transfers is performed to further characterize the discretization errors of the different forms in forced helical turbulence simulations.

8. Small-scale anisotropy induced by spectral forcing and by rotation in non-helical and helical turbulence

We study the effect of large-scale spectral forcing on the scale-dependent anisotropy of the velocity field in direct numerical simulations of homogeneous incompressible turbulence. Two forcing methods are considered: the steady ABC single wavenumber scheme and the unsteady non-helical or helical Euler scheme. The results are also compared with high resolution data obtained with the negative viscosity scheme. A fine-grained characterization of anisotropy, consisting in measuring some quantities related to the two-point velocity correlations, is used: we perform a modal decomposition of the spectral velocity tensor into energy, helicity and polarization spectra. Moreover, we include the explicit dependence of these three spectra on the wavevector direction. The conditions that allow anisotropy to develop in the small scales due to forcing alone are clearly identified. It is shown that, in turbulent flows expected to be isotropic, the ABC forcing yields significant energy and helicity directional anisotropy down to the smallest resolved scales, like the helical Euler scheme when an unfavourable forcing scale is used. The direction-and scale-dependent anisotropy is then studied in rotating turbulence. It is first shown that, in the ABC-forced simulations the slope of the energy spectrum is altered and the level of anisotropy is similar to that obtained at lower Rossby number in Euler-forced runs, a result due both to the nature of the forcing itself and to the fact that it allows an inverse cascade to develop. Second, we show that, even at low rotation rate, the natural anisotropy induced by the Coriolis force is visible at all scales. Finally, we identify two different wavenumber ranges in which anisotropy behaves differently, and show that if the Rossby number is not too low the characteristic lenghscale separating them is the one at which rotation and dissipation effects balance.

14. Use of wound infusion catheters for postoperative local anaesthetic administration in cats.

- Abstract
Objectives: The present study aimed to document the use of the wound infusion catheter (WIC) following a variety of surgical procedures in cats, investigating complications and risk factors associated with catheter placement or local anaesthetic (LA) administration. Methods: A retrospective, multicentric study was performed. Medical databases of eight veterinary referral hospitals from 2010 to 2021 were searched to identify records of cats where WICs were used. Information regarding signalment, type of surgery, size and type of WIC placed, and LA protocol used, as well as postoperative complications, were retrieved. Results: One hundred and sixty-six cases fulfilled the inclusion criteria. Feline injection site sarcoma resection was the most common surgery. Overall complications were identified in 22/166 cats (13.2%). Thirteen cats (7.8%) experienced wound-related complications, whereas nine cats (5.4%) experienced drug-delivery complications. The only factor associated with an increased risk of complications was the amount of a single dose of LA delivered through the catheter (P <0.001). An amount higher than 2.5 ml of LA delivered at each administration was associated with an increased risk of complications. All complications were minor and self-limiting. Conclusions and relevance: WICs were used for a large variety of surgical procedures with different protocols of LA administration as part of a multimodal analgesic plan in cats. The risk of complications was relatively low and self-limiting, suggesting its safe use in cats. Further prospective studies are required to evaluate efficacy of postoperative analgesia and to determine the suitable protocol for WIC handling and maintenance. [ABSTRACT FROM AUTHOR]

19. Clinical characteristics and long-term outcome of lung lobe torsions in cats: a review of 10 cases (2000–2021).

Case series summary: Lung lobe torsion is rare in cats. The aim of this multi-institution retrospective study was to describe clinical and diagnostic findings, treatments and outcomes of lung lobe torsion (LLT) in 10 cats. Dyspnoea and tachypnoea were the most common clinical signs. Pleural effusion was present in nine cats at presentation. Fluid analysis confirmed chylothorax in three cats. Nine cats underwent CT and five cats had thoracic radiographs taken. A diagnosis was made preoperatively in six cats, while in the other four cats it was made at exploratory thoracotomy. Affected lung lobes were the right cranial (n = 4/11), left cranial (n = 4/11) and right middle (n = 3/11). One cat had a concurrent torsion of two lung lobes. Lung lobectomy was successfully performed in all cases. Based on clinical, diagnostic and lung histopathology findings, three cats had idiopathic and seven cats secondary LLT. Intraoperative complications included hypotension and hypothermia in four and five cats, respectively. Postoperative complications occurred in six cats and lead to euthanasia or death in four cats, whereas complications resolved in the other two cats. Three cats were euthanased within 5 weeks of discharge. For the three cats surviving long term, including one euthanased at 252 days postoperatively, owner-described outcomes and quality of life were considered good to excellent. Relevance and novel information: Secondary LLT associated with underlying thoracic pathology was associated with high complication rates and poor outcomes. Long-term outcomes of cats undergoing surgery for LLT and surviving the perioperative period were deemed good to excellent. [ABSTRACT FROM AUTHOR]

Objectives: The aim of this retrospective observational study was to describe the clinical presentation, treatment and outcome of cats with sialocoele. Methods: Clinical records from seven referral hospitals were retrospectively searched to identify cats with sialocoele between 2007 and 2021. Results: Nineteen cats were identified. The most common clinical signs were ptyalism, dysphagia and anorexia. Localisation of the sialocoele was cervical (n = 6), sublingual (n = 6), cervical/sublingual (n = 3), facial (n = 2), cervical/pharyngeal (n = 1) and zygomatic (n = 1). The affected salivary glands were mandibular–sublingual (n = 15), mandibular–sublingual/parotid (n = 1), zygomatic/parotid (n = 1) and parotid (n = 2). The aetiology of the sialocoele was traumatic in two cases, neoplastic in one cat and unknown in 16 cats. Sialoadenectomy was performed in 11 cats. Other treatments included ranula marsupialisation (n = 3), needle drainage (n = 2), single stab incision (n = 2) and parotid duct ligation (n = 1). The median follow-up time was 399 days (range 15–1460). Postoperative seroma was the only complication observed in one cat. No recurrence was reported. Conclusions and relevance: Feline salivary sialocoele are relatively rare and have a good prognosis. They can be managed successfully with sialoadenectomy; however, a more conservative approach can be used with appropriate case selection. [ABSTRACT FROM AUTHOR]

- Abstract
Background The best treatment option for patients with type 2 diabetes in whom treatment with metformin alone fails to achieve adequate glycaemic control is debated. We aimed to compare the long-term effects of pioglitazone versus sulfonylureas, given in addition to metformin, on cardiovascular events in patients with type 2 diabetes. Methods TOSCA.IT was a multicentre, randomised, pragmatic clinical trial, in which patients aged 50–75 years with type 2 diabetes inadequately controlled with metformin monotherapy (2–3 g per day) were recruited from 57 diabetes clinics in Italy. Patients were randomly assigned (1:1), by permuted blocks randomisation (block size 10), stratified by site and previous cardiovascular events, to add-on pioglitazone (15–45 mg) or a sulfonylurea (5–15 mg glibenclamide, 2–6 mg glimepiride, or 30–120 mg gliclazide, in accordance with local practice). The trial was unblinded, but event adjudicators were unaware of treatment assignment. The primary outcome, assessed with a Cox proportional-hazards model, was a composite of first occurrence of all-cause death, non-fatal myocardial infarction, non-fatal stroke, or urgent coronary revascularisation, assessed in the modified intention-to-treat population (all randomly assigned participants with baseline data available and without any protocol violations in relation to inclusion or exclusion criteria). This study is registered with ClinicalTrials.gov, number NCT00700856. Findings Between Sept 18, 2008, and Jan 15, 2014, 3028 patients were randomly assigned and included in the analyses. 1535 were assigned to pioglitazone and 1493 to sulfonylureas (glibenclamide 24 [2%], glimepiride 723 [48%], gliclazide 745 [50%]). At baseline, 335 (11%) participants had a previous cardiovascular event. The study was stopped early on the basis of a futility analysis after a median follow-up of 57·3 months. Habitat diversity plays a crucial role in shaping the structure and distribution of benthic macroinvertebrate communities. However, few studies have investigated the concurrent effects of such drivers operating at different spatial scales in mountain streams. Here, we assessed which environmental factors at different spatial scales, related to stream/river type and substrate, influence the diversity of benthic macroinvertebrate assemblages in mountain streams and rivers. Within the framework of the aquatic investigations of the Biodiversity Monitoring South Tyrol (BMS) – a newly-established, regional-based, long-term biodiversity monitoring program – benthic macroinvertebrates and a set of environmental factors describing water quality, stream/river hydromorphology and in-stream habitat characteristics were collected and analysed. The variation in total density, taxonomic richness, Shannon evenness and % Ephemeroptera–Plecoptera–Trichoptera taxa of stream benthic macroinvertebrates was assessed at multi-scale habitat levels, in 48 sampling sites across the mountainous region of the Autonomous Province of Bolzano/Bozen (Italy). The effects of stream/river type, substrates and different environmental factors on community composition of benthic macroinvertebrates were quantified, using a stepfoward Redundancy Analysis (RDA) and generalized additive models (GAMs). In addition, we performed an Indicator Value analysis to detect taxa that were significantly associated with a specific substrate and/or stream/river type. Overall, we observed significant differences in benthic biological metrics among both stream/river and substrate types. We also found at least one indicator taxa for each stream/river type, with environmental factors acting at reach and large-spatial scale – temperature, stream channel stability, elevation – being the key drivers in shaping macroinvertebrate distribution patterns in the analysed mountain rivers. While significant differences in macroinvertebrate community structures were found among substrates, the influence of stream/river type conditions was particularly evident. Apart from ranking environmental variables worth including in a long-term aquatic biodiversity monitoring program in mountain areas, our study provides a baseline for practical applications. For instance, depending on the desired outcome of habitat heterogeneity restoration, it can suggest the optimal scale (landscape vs. reach vs. patch) for prioritizing in-field interventions. Background and aims Metformin is the first-line therapy in type 2 diabetes. In patients inadequately controlled with metformin, the addition of a sulfonylurea or pioglitazone are equally plausible options to improve glycemic control. However, these drugs have profound differences in their mechanism of action, side effects, and impact on cardiovascular risk factors. A formal comparison of these two therapies in terms of cardiovascular morbidity and mortality is lacking. The TOSCA.IT study was designed to explore the effects of adding pioglitazone or a sulfonylurea on cardiovascular events in type 2 diabetic patients inadequately controlled with metformin. Methods Multicentre, randomized, open label, parallel group trial of 48 month duration. Type 2 diabetic subjects, 50–75 years, BMI 20–45 Kg/m 2 , on secondary failure to metformin monotherapy will be randomized to add-on a sulfonylurea or pioglitazone. The primary efficacy outcome is a composite endpoint of all-cause mortality, nonfatal myocardial infarction, nonfatal stroke, and unplanned coronary revascularization. Principal secondary outcome is a composite ischemic endpoint of sudden death, fatal and non-fatal myocardial infarction and stroke, endovascular or surgical intervention on the coronary, leg or carotid arteries, major amputations. Side effects, quality of life and economic costs will also be evaluated. Efficacy, safety, tolerability, and study conduct will be monitored by an independent Data Safety Monitoring Board. End points will be adjudicated by an independent external committee. Conclusions TOSCA.IT is the first on-going study investigating the head-to-head comparison of adding a sulfonylurea or pioglitazone to existing metformin treatment in terms of hard cardiovascular outcomes. - Abstract
A marine sediment core from the western Mediterranean provides a new high-resolution 4500 year record of palaeomagnetic secular variation and relative palaeointensity. In 2013, the 7.1 m C5 core was recovered from the Tyrrhenian Sea as part of the NextData climate data project. The coring site, 15 km offshore from the Volturno river mouth, is well located to record combined marine and terrestrial palaeoclimatic influences, and the fine-grained, rapidly deposited sediments are effective palaeomagnetic recorders. We investigate the palaeomagnetic field direction and strength recorded in the core, which provide a valuable high-resolution record of Holocene geomagnetic variation in the area. Using rock magnetic techniques, we constrain the magnetic mineralogy of the studied sediments and confirm their suitability for palaeomagnetic analysis. Palaeomagnetic declination and inclination records were determined by stepwise alternating-field demagnetization, and relative palaeointensity estimates were obtained based on normalization to anhysterestic and isothermal remanent magnetization and to magnetic susceptibility. The age of the core is well constrained with a tephra and biostratigraphic age model, and its magnetic records are compared with relevant core and model data for the region, demonstrating that our record is compatible with previous results from the area. An automated curve matching approach is applied to assess the compatibility of our data with the existing secular variation path for the Mediterranean area.

- Abstract
Ureteral obstruction secondary to ureterolithiasis in cats is a challenging situation. Ureteral stenting has recently been introduced to prevent complications that often occurred after ureterotomy or other invasive surgeries. The purpose of this study is to describe the stenting technique and perioperative difficulties, as well as long-term outcome and complications with ureteral stenting in 12 cats with ureteroliths. Fifteen 2.5 Fr soft double pigtail multi-fenestrated ureteral stents were placed in an anterograde fashion under open surgical approaches and with fluoroscopic guidance in 12 cats. Nine cats received a unilateral stent and three received bilateral stents. Ureterotomy or ureteral resection and end-to-end anastomosis were performed in three and four cases, respectively. In six cats, papillotomy was performed to facilitate dilatator and stent placement. All cats recovered well from the surgical procedure, except one cat, which died during the anaesthesia recovery period. Postoperative complications included dysuria (three cases, diagnosed at 15 days, 1 month and 3 months, respectively), urinary tract infection (one case, 1 month after surgery), stent migration requiring stent replacement (one case, 19 months after surgery) and stent obstruction requiring stent removal (three cases with previously end-to-end anastomosis between 2 and 8 months after surgery). Nine cats (75%) were alive at a mean follow-up of 453 ± 194 (123–720) days. The median survival time was >415 days. Stent placement appeared to be a valuable and safe option for treating ureteral obstruction in cats. However, periodic and long-term monitoring of stents is warranted. [ABSTRACT FROM PUBLISHER]

Abstract: The Marsili Seamount (MS) is an about 3200m high volcanic complex measuring 70×30km with the top at ~500mb.s.l. MS is interpreted as the ridge of the 2Ma old Marsili back-arc basin belonging to the Calabrian Arc–Ionian Sea subduction system (Southern Tyrrhenian Sea, Italy). Previous studies indicate that the MS activity developed between 1 and 0.1Ma through effusions of lava flows. Here, new stratigraphic, textural, geochemical, and
14 C geochronological data from a 95cm long gravity core (COR02) recovered at 839mbsl in the MS central sector are presented. COR02 contains mud and two tephras consisting of 98 to 100 area% of volcanic ash. The thickness of the upper tephra (TEPH01) is 15cm, and that of the lower tephra (TEPH02) is 60cm. The tephras have poor to moderate sorting, loose to partly welded levels, and erosive contacts, which imply a short distance source of the pyroclastics.14 C dating on fossils above and below TEPH01 gives an age of 3kaBP. Calculations of the sedimentation rates from the mud sediments above and between the tephras suggest that a formation of TEPH02 at 5kaBP MS ashes has a high-K calcalkaline affinity with 53wt.%2 <68wt.%, and their composition overlaps that of the MS lava flows. The trace element pattern is consistent with fractional crystallization from a common, OIB-like basalt. The source area of ashes is the central sector of MS and not a subaerial volcano of the Campanian and/or Aeolian Quaternary volcanic districts. Submarine, explosive eruptions occurred at MS in historical times: this is the first evidence of explosive volcanic activity at a significant (500–800mbsl) water depth in the Mediterranean Sea. Abstract: A 9-month-old, chemically spayed female ferret (Mustela putorius furo) presented after a fall (5.5m) from the first floor of an apartment building. The animal was nonambulatory, with pain localized in the pelvic region. Radiographic examination revealed a bilateral fracture of the iliac body with medial displacement of the left hemipelvis and narrowing of the pelvic canal. Rib fractures, pulmonary contusions, and pleural effusion were also present on thoracic radiographic images. After emergency stabilization of the patient, the fracture of the left iliac body was successfully repaired using a composite internal fixation system with K-wires and polymethylmethacrylate. The implants allowed fracture healing despite loosening of one of the K-wires. Bony healing was achieved within 25 days, including the restoration of pelvic rigidity and maintenance of the diameter of the pelvic canal. Orthopedic examination was unremarkable 4 months following the surgical procedure. To the authors’ knowledge, this is the first described open reduction of a pelvic fracture in a ferret. A 3-year-old neutered male Bengal cat with a history of chronic mucopurulent bilateral nasal discharge and sneezing was diagnosed with severe fungal rhinosinusitis. A diagnosis was obtained after computer tomography imaging, histopathological examination and fungal culture. The mold Scedosporium apiospermum was identified as the aetiological agent. To our knowledge, this case is the first description of a rhinitis or sinusitis caused by this agent in a cat. Aggressive surgical debridement combined with topical and systemic antifungal therapy was performed. Unfortunately, the treatment resulted only in a partial remission of signs. [ABSTRACT FROM AUTHOR]

Heterophilic antibodies can interfere with laboratory tests causing erroneous results. We report the case of a 55-year-old man with Mantle cell myeloma treated with Rituximab that caused false positive results of HIV serologic test. The cause of this interference was investigated, and very high levels (741 ng/ml) of human anti-mouse antibodies (HAMA) were demonstrated. Polyethylene glycol (PEG) precipitation of intact immunoglobulins was used to cross-check the antibody-related interference. The PEG treatment negativized the HIV test outcome, confirming that the cause of the false positive serologic HIV result was due to interference caused by HAMA. In conclusion, we suggest to cautiously interpreting laboratory findings in patients treated with Rituximab because of the possibility that HAMA may cause interference in serological assays.

- Abstract
Autosomal dominant dehydrated hereditary stomatocytosis (DHSt) usually presents as a compensated hemolytic anemia with macrocytosis and abnormally shaped red blood cells (RBCs). DHSt is part of a pleiotropic syndrome that may also exhibit pseudohyperkalemia and perinatal edema. We identified PIEZO1as the disease gene for pleiotropic DHSt in a large kindred by exome sequencing analysis within the previously mapped 16q23-q24 interval. In 26 affected individuals among 7 multigenerational DHSt families with the pleiotropic syndrome, 11 heterozygous PIEZO1missense mutations cosegregated with disease. PIEZO1 is expressed in the plasma membranes of RBCs and its messenger RNA, and protein levels increase during in vitro erythroid differentiation of CD34+cells. PIEZO1 is also expressed in liver and bone marrow during human and mouse development. We suggest for the first time a correlation between a PIEZO1mutation and perinatal edema. DHSt patient red cells with the R2456H mutation exhibit increased ion-channel activity. Functional studies of PIEZO1mutant R2488Q expressed in Xenopusoocytes demonstrated changes in ion-channel activity consistent with the altered cation content of DHSt patient red cells. Our findings provide direct evidence that R2456H and R2488Q mutations in PIEZO1alter mechanosensitive channel regulation, leading to increased cation transport in erythroid cells.

45. A Novel Murine Femoral Segmental Critical-Sized Defect Model Stabilized by Plate Osteosynthesis for Bone Tissue Engineering Purposes

- Abstract
Mouse models are invaluable tools for mechanistic and efficacy studies of the healing process of large bone defects resulting in atrophic nonunions, a severe medical problem and a financial health-care-related burden. Models of atrophic nonunions are usually achieved by providing a highly stable biomechanical environment. For this purpose, external fixators have been investigated, but plate osteosynthesis, despite its high clinical relevance, has not yet been considered in mice. We hereby proposed and investigated the use of an internal osteosynthesis for stabilizing large bone defects. To this aim, a 3.5-mm-long segmental bone defect was induced in the mid-shaft of the femur using a Gigli saw and a jig. Bone fixation was performed using a titanium microlocking plate with four locking screws. The bone defect was either left empty or filled with a syngenic bone graft or filled with a coralline scaffold. Healing was monitored using radiographs. The healing process was further assessed using microcomputed tomography and histology 10 weeks after surgery. With the exception of one mouse that died during the surgical procedure, no complications were observed. A stable and reproducible bone fixation as well as a reproducible fixation of the implanted materials with full weight bearing was obtained in all animals tested. Nonunion was consistently observed in the group in which the defects were left empty. Bone union was obtained with the syngenic bone grafts, providing evidence that, although such defects were of critical size, bone healing was possible when the gold-standard material was used to fill the defect. Although new bone formation was greater in the coralline scaffold group than in the left-empty animal group, it remained limited and localized close to the bony edges, a consequence of the critical size of such bone defect. Our study established a reproducible, clinically relevant, femoral, atrophic nonunion, critical-sized defect, low morbidity mouse model. The present study was successful in designing and testing in a small animal model, a novel surgical method for the assessment of bone repair; this model has the potential to facilitate investigations of the molecular and cellular events involved in bone regeneration in load-bearing, segmental-bone defects.

Feline parotid salivary duct sialocele is an uncommon disorder that has been previously reported in association with traumatic rupture of the duct in only two cats. Both cases were successfully treated by proximal duct ligation. We describe the successful surgical treatment of a parotid duct sialocele, secondary to spontaneous salivary duct stenosis, in an adult domestic shorthair cat. The cat was referred for assessment of a recurrent fluid-filled swelling on the left side of the face. Cytology of the aspirated fluid was consistent with serous saliva. The anatomical localisation of the lesion and the nature of the fluid were indicative of parotid gland/duct involvement. Retrograde sialography by parotid duct cannulation was unsuccessful because the left parotid duct opening was stenosed and obstructed by scar tissue. Surgical exploration revealed a parotid salivary duct sialocele, which was completely removed along with the parotid gland without complications. [ABSTRACT FROM AUTHOR]
