Six cases of congenital subscalp nodule associated with underlying cranium bifidum are reported. A plain skull roentgenogram showed a midline bone defect in the parieto-occipital region near the lambda. CT scan demonstrated neither brain malformation nor ventricular deformity except for the high position of the straight sinus. Cerebral angiography revealed an elongation of the vein of Galen and anomalous upward course of the straight sinus. At surgery, the tumor was solid and connected to a cord which extended intracranially via the cranium bifidum and blended with thickened arachnoid membrane either on the dorsal aspect of the midbrain or at the surface of the anterior vermis. Histologically, the tumor consisted in all cases of arachnoid cells and fibrous tissue with immature glial cells in one case. Multistep pH-peak-focusing liquid chromatography with a column packed with a hydrophilic polymer gel (a cross-linked hydroxylated methacrylic polymer gel) was developed for separation of rare earth metal ions. Metal ions in a sample solution introduced to the column are chromatographically extracted into the stationary gel phase at the top of the column equilibrated with a basic solution used as the first mobile phase containing acetylacetone and 1,10-phenanthroline by synergistic extraction effect. After the sample solution is introduced, the mobile phases are delivered into the column by stepwise gradient elution in order of decreasing pH. Each metal ion is concentrated at a pH border formed between the zones of different pH in the column and moves toward the outlet of the column with the pH border. Mutual separation of La(III), Ce(III), Nd(III), Eu(III), Y(III), Tb(III), and Yb(III) was achieved by the present method for an 1-mL sample injection with the column of which the inner volume is 11.8 mL. The carcinostatic activity has been studied for fatty acids of diverse species but scarcely for fatty alcohols. Three unsaturated fatty alcohols at 35-50 microM inhibited DNA synthesis and the proliferation of tumor cells by a combination with hyperthermia to greater extents in the order: oleyl (C18:1)-> linoleyl (C18:2)-> alpha-linolenyl (C18:3) alcohol, which is an order inverse to that known for the corresponding fatty acids (4). In contrast, two saturated fatty alcohols, palmityl (C16:0)- and stearyl (C18:0) alcohols, did not inhibit at the same concentrations. At 100 microM, palmityl alcohol inhibited, whereas stearyl alcohol did not. The effective fatty alcohols appreciably permeated the cells. The inhibition of the unsaturated fatty alcohols on DNA synthesis and proliferation was nearly proportional to the amount of their intercellular accumulation at 37 degrees C or 42 degrees C; the most inhibitory, oleyl alcohol, was the most membrane-permeable, whilst inversely the least inhibitory, alpha-linolenyl alcohol, was the least permeable. A proportional correlation was not observed for saturated fatty alcohols; palmityl alcohol underwent an approximate 2-fold more abundant accumulation than other fatty alcohols, but was weakly inhibitory. Thus, oleyl alcohol may exert an antitumor action via appropriately efficient transmembrane permeation and a combination with hyperthermia.

Two patients with ovarian cancer who had received multiple courses of cisplatin without complications experienced hypersensitivity reactions to cisplatin: one, involving intrahepatic artery infusion, manifested general erythema, dyspnea, and hypotension; the other, involving intravenous infusion, manifested abdominal pain, general erythema, and fever. When platinum-containing agents are administered, patients should be closely monitored for hypersensitivity reactions, even in previously uncomplicated cases.

Cranial malignant fibrous histiocytomas are rare tumors. Most are hypervascular, destructive masses that are similar to other malignant lesions and to malignant fibrous histiocytomas found elsewhere in the body. We describe a myxoid malignant fibrous histiocytoma of the temporal bone, possibly of dural origin, with features that more closely resembled a meningioma at CT, MR imaging, and angiography.

Purpose: The in vivo pharmacokinetics and sensitizing effects of PR-350, a newly developed, highly water-soluble radiosensitizer were examined., Materials and Methods: C3H/HeJ mice bearing SCC-VII tumor cells were used in the experiments. Results were compared with those of PR-28 and SR-2508., Results: The intratumoral concentration of PR-350 reached the maximum value of 77.7 mg/kg 20 min after intravenous injection. The concentration in the brain was below the detection limit. The enhancement ratios of PR-350 calculated using the growth delay method were 0.9 for PR-350 at 50 mg/kg and 1.4 for PR-350 at 100 mg/kg, compared with 1.4 for SR-2508 at 100 mg/kg and 1.0 for PR-28 at 100 mg/kg., Conclusion: PR-350 and SR-2508 revealed similar sensitizing effects. The side effects of PR-350 are expected to be equal to or less than those of SR-2508. PR-350 seems to be a promising radiosensitizer.

A 30-year-old primigravida, delivering by cesarean section, went into shock with cardiac arrest due to pulmonary thromboembolism. Thrombolytic therapy with recombinant-tissue-type plasminogen activator (rt-PA) induced a rapid improvement in both the hemodynamic and respiratory conditions. This case shows the marked efficacy of rt-PA in treating massive pulmonary thromboembolism. Close attention must be given to the patient in such a case, however, because the risk of hemorrhagic complication exists.

Effects of 6-O-palmitoyl ascorbate (ascorbate) developed to increase the antitumour activity of ascorbic acid on DNA synthesis and proliferation of Ehrlich ascites tumour cells were investigated. Treatment of the cells with the acylated ascorbate at 25-50 microM for 1 h resulted in no effect on DNA synthesis, assayed by pulse incorporation of [3H]thymidine after a culture period of 20 h, but led to 49%-87% enhanced DNA synthesis after 4 days, suggesting that long-term culture is required for promotion by ascorbate to occur. At a dose as high as 75 microM acylated ascorbate, however, cellular DNA synthesis was 64% inhibited after 20 h and 99% after 4 days. The results suggest that acylated ascorbate exhibits a dual action on DNA synthesis: promotion at low doses and inhibition at high doses, both of which are potentiated in a time-dependent manner. In contrast to the above-mentioned results at 37 degrees C, acylated ascorbate at 25-75 microM inhibited but did not promote DNA synthesis at 42 degrees C whatever the culture period. Similar results were exhibited when proliferation of cells cultured for a long period was investigated. At 37 degrees C, 50 microM acylated ascorbate increased the number of the cells to 3.6 times the control values after 8 days and to 1.9 times after 11 days; in contrast, a 75-microM dose decreased the cell number considerably. Combination with hyperthermia (42 degrees C) suppressed the increase and cell growth was completely inhibited at 75 microM.

We present two cases of central giant cell granuloma in the temporal bone. CT showed an enhancing tumor causing a smoothly margined temporal squamosa and floor of the middle fossa. External carotid angiograms showed tumor stain mainly supplied by the temporal branches of the internal maxillary artery. In one case, MR images showed a hypointense mass on both T1- and T2-weighted images that was thought to reflect the pathologic character of this lesion.

Nine patients with hepatoma, 5 with massive ascites, 3 with rupture of esophageal varices and 1 with hematoemesis, successfully underwent TIPS without technical complications. Two of the patients died 3 and 5 months, respectively, after TIPS due to hepatic failure and/or tumor extension. Another patient in whom hepatoma ruptured 1 month after TIPS was experienced. It is concluded that TIPS is a useful and safety treatment for portal hypertension even in patients with hepatoma which was not existed through the puncture route. However TIPS should be performed in patients in whom hepatoma has been controlled by hepatic embolization and/or ethanol injection therapy.

An artificial neural network approach was applied to assess ventricular size from computed tomograms. Three layer, feed-forward neural networks with a back propagation algorithm were designed to distinguish between three degrees of enlargement of the ventricles on the basis of patient's age and six items of computed tomographic information. Data for training and testing the neural network were created with computed tomograms of the brains selected at random from daily examinations. Four radiologists decided by mutual consent subjectively based on their experience whether the ventricles were within normal limits, slightly enlarged, or enlarged for the patient's age. The data for training was obtained from 38 patients. The data for testing was obtained from 47 other patients. The performance of the neural network trained using the data for training was evaluated by the rate of correct answers to the data for testing. The valid solution ratio to response of the test data obtained from the trained neural networks was more than 90% for all conditions in this study. The solutions were completely valid in the neural networks with two or three units at the hidden layer with 2,200 learning iterations, and with two units at the hidden layer with 11,000 learning iterations. The squared error decreased remarkably in the range from 0 to 500 learning iterations, and was close to constant over two thousand learning iterations. The neural network with a hidden layer having two or three units showed high decision performance. The preliminary results strongly suggest that the neural network approach has potential utility in computer-aided estimation of enlargement of the ventricles.
