International audience; In this context of lockdown linked to the Covid-19 health crisis, a survey was offered to daily cannabis users in order to study the impact of lockdown on their use and their health. The specific objectives of this article are to describe this population of daily cannabis users, the changes in use before and during the lockdown, and the link with certain sociodemographic, behavioural and health characteristics.; Contexte : Les consommateurs de cannabis représentent une population hétérogène en constante augmentation. Même si le cannabis présente un pouvoir addictogène avec des effets néfastes pour la santé, cette substance est également connue pour ses effets thérapeutiques sur de nombreuses conditions et notamment sur la douleur et la dépendance à d’autres substances. Le confinement lié à l’épidémie de Covid-19 a provoqué dès l’annonce des réactions de la part des usagers et des associations de réduction des risques, préoccupés par les situations à risque (stress et sevrage). Dans ce contexte, une enquête a été proposée aux consommateurs quotidiens de cannabis afin d’étudier l’impact du confinement sur leurs usages et leur santé. Les objectifs spécifiques de cet article sont de décrire cette population de consommateurs quotidiens de cannabis, les évolutions d’usage avant et pendant le confinement et le lien avec certaines caractéristiques sociodémographiques, comportementales et sanitaires.Méthodes : L’enquête a consisté en un questionnaire en ligne rempli pendant la période de confinement et diffusé sur différents sites fréquentés par les usagers de cannabis. Ce questionnaire était composé de 3 parties : profil sociodémographique, pratiques de consommation avant le confinement et pendant le confinement. Des données plus précises sur la santé des usagers ont été recueillies telles que les symptômes anxieux, dépressifs, la douleur et les troubles du sommeil.Résultats : Au total, 4 279 questionnaires ont été remplis de manière complète. La population est essentiellement composée d’hommes (75 %), de jeunes (médiane à 27 ans), de personnes diplômées (84 % ont le baccalauréat) et vivant dans des grandes unités urbaines (62 %). L’étude de l’évolution de la consommation de cannabis avant et pendant le confinement permet de distinguer 4 profils : ceux qui ont arrêté (7 %), ceux qui ont diminué (28 %), ceux qui n’ont pas changé (29 %) et ceux qui ont augmenté (36 %). L’augmentation est plus fréquente chez les femmes, pour les personnes habitant dans des grandes villes et celles qui ont stocké du cannabis en prévision du confinement. L’arrêt est associé à un état de santé dégradé (anxiété, dépression, douleur et troubles du sommeil). Ceux qui ont maintenu une consommation stable pendant le confinement semblent avoir eu moins d’effets négatifs sur leur santé.Conclusion : Notre enquête CANNAVID a permis de recruter un échantillon suffisamment important pour pouvoir décrire une partie de la population des consommateurs quotidiens de cannabis et les effets du confinement sur leurs usages et leur santé. Ces données préliminaires montrent que le confinement a provoqué des réactions et des effets qui ont été délétères surtout chez ceux qui ont arrêté ou augmenté leur consommation.

Introduction Dans l’asthme, la formation de bouchons muqueux dans les voies aeriennes impliquerait la retention du mucus a la surface de l’epithelium bronchique mais les mecanismes de cette stase sont mal connus. Dans l’epithelium reconstitue in vitro d’asthmatiques severes, nous avons observe une hyperexpression de la transglutaminase 2 (TG2), une enzyme capable de reticuler les mucines intestinales. Nous avons emis l’hypothese que la reticulation des mucines respiratoires par la TG2 contribuerait a leur piegeage a la surface de l’epithelium bronchique dans l’asthme. Methodes L’epithelium bronchique d’asthmatiques severes et de temoins a ete reconstitue in vitro par culture a l’interface air-liquide (culture ALI) de cellules issues de biopsies. La retention de MUC5AC par l’epithelium bronchique a ete modelisee par exposition a l’IL-13 de cultures ALI de temoins. L’expression de la TG2 et de MUC5AC a ete quantifiee par RT-qPCR, western-blot et immunohistochimie/fluorescence. La modificatio des mucines par la TG2 a ete mise en evidence par l’incorporation d’une amine biotinylee en western blot. Le role de la TG2 a ete evalue a l’aide de l’inhibiteur pharmacologique ZDON. Resultats La TG2 et son produit catalytique etaient exprimes a la surface de l’epithelium bronchique dans l’asthme severe, et etaient localises au voisinage de MUC5AC. Dans les cultures ALI des sujets sains et asthmatiques, l’expression de la TG2 etait correlee a celle de MUC5AC. La TG2 etait capable de modifier les mucines respiratoires in vitro. L’inhibition de la TG2 par ZDON favorisait l’elimination de MUC5AC apres un lavage apical a la surface des epithelia hyspersecretoires. Discussion Nos resultats suggerent que l’activite enzymatique de la TG2 participe a la retention du mucus a la surface de l’epithelium bronchique hypersecretoire. La validation de ces observations dans les epithelia reconstitues d’asthmatiques severes est en cours. Conclusion La modulation pharmacologique de la TG2 pourrait permettre de cibler les manifestations liees a la stase muqueuse dans l’asthme.

18. Tumeurs pulmonaires abcédées : prise en charge diagnostique et thérapeutique

Introduction Les abces pulmonaires peuvent etre presents des le diagnostic de cancer pulmonaire, ou en etre une complication. Il n’existe que peu de consensus concernant la prise en charge therapeutique de ces tumeurs abcedees. La prise en charge de ces patients peut devenir tres delicate, opposant l’instauration d’une chimiotherapie anti-infectieuse et la poursuite du traitement anti-cancereux. Methodes Nous avons mene une etude monocentrique retrospective et inclus tous les patients hospitalises au CHU de Rouen pour prise en charge d’une tumeur pulmonaire abcedee, entre 2005 et 2019. Resultats Nous avons inclus 52 patients. Il s’agissait principalement d’hommes (73 %), âges en moyenne de 58,5 [52–62,25] ans. Tous etaient fumeurs ou anciens fumeurs. Vingt-huit patients (53 %) ont developpe un abces pulmonaire au cours du traitement de leur cancer. Des prelevements microbiologiques respiratoires ont ete realises pour chacun des patients. L’identification microbiologique a ete possible pour 29 patients (56 %). Les germes les plus frequemment identifies sont decrits dans le Tableau 1 . Les abces etaient le plus souvent polymicrobiens (n :13 ; 45 %). Lors du diagnostic de l’abces, la duree mediane de sejour etait de 28,5[14,8–42,0] jours. L’ensemble des patients a ete traite par d’une a 6 lignes d’antibiotherapie. Les associations amoxicilline-acide clavulanique (n = 26), piperacilline-tazobactam (n = 9) et la ceftriaxone (n = 8) etaient les traitements utilises en premiere ligne. La duree mediane de l’antibiotherapie etait de 14 [7–28] jours. La survenue d’un abces pulmonaire a conduit a une prise en charge neoplasique palliative chez 24 patients (46 %), et retarde la chimiotherapie pour 12 patients (23 %). Le pronostic de ces patients etait sombre, avec une survie mediane de 6,5 mois apres le diagnostic de l’abces pulmonaire. Conclusion La survenue d’un abces pulmonaire conduit a l’interruption des traitements pour pres de la moitie des patients et s’accompagne d’un pronostic sombre. L’antibiotherapie initiale doit etre large compte-tenu du caractere generalement microbien des abces.

In 1987 the Australasian Law Teachers Association (ALTA) Law Teaching Workshop celebrated its tenth anniversary. For over a decade, it has conducted residential workshops for law teachers in the Australasian region annually, and in 1994 it sponsored the first advanced workshop for participants of earlier workshops. Although the workshop has evolved since its introduction into Australia from Canada by Professors Neil Gold and Mary Gerace, its main aim has remained the same — to professionalise the teaching of law in tertiary institutions in the Australasian region. It is, therefore, somewhat fitting that, by chance, ten years on, workshops were held in York, Western Australia and in Bangalore, India.

Introduction La prevalence de la ventilation non invasive (VNI) a domicile est croissante dans les pays developpes mais les effets secondaires lies a ce traitement au long cours ont ete rarement etudies dans une vaste population de patients. Notre objectif etait de determiner l’incidence des effets secondaires de la VNI de domicile, leur severite et leurs consequences sur la qualite de vie et l’observance des patients suivis dans notre centre. Methodes Cette etude a inclus tous les patients appareilles par VNI a domicile et admis pour bilan pneumologique programme entre mai 2017 et janvier 2018. Onze effets secondaires ont ete identifies de maniere systematique a l’aide d’une grille d’interrogatoire et quantifies par les patients de 0 (absence d’effet secondaire) a 4 (effet secondaire intolerable). Le score total correspondait a la somme des scores de chaque effet secondaire. Tous les patients repondaient egalement au questionnaire de qualite de vie liee a la sante Severe Respiratory Insufficiency questionnaire (SRI). L’observance de la VNI etait evaluee via le recueil des donnees logicielles du ventilateur. Resultats Parmi les 490 patients evalues, 437 (89 %) avaient rempli a la fois leur questionnaire SRI et le questionnaire des effets secondaires de la VNI ( Tableau 1 ). Les pathologies respiratoires ayant motive l’instauration de la VNI etaient : syndrome obesite-hypoventilation (n = 142, 32 %), association BPCO-SAOS (n = 96, 22 %), BPCO (n = 81, 19 %), maladie neuromusculaire (n = 78, 18 %), deformation thoracique (n = 29, 7 %), syndrome d’apnees du sommeil (n = 11, 2 %). Vingt (5 %) patients ne rapportaient aucun effet secondaire. L’incidence de chaque effet secondaire est reportee dans le Tableau 1 . Le score total des effets secondaires etait de 5 [quartile : 3–8] sur 44. Le score des effets secondaires etait inversement correle au SRI et a l’observance de la VNI : rho : −0,419 (p Conclusion Les effets secondaires sont tres frequents chez les patients appareilles par VNI a domicile et ont un impact negatif sur la qualite de vie et l’observance.

Introduction Les complications pulmonaires sont frequentes chez les patients transplantes renaux (TR) principalement du fait de l’immunosuppression a laquelle ils sont soumis. Notre etude avait pour but d’evaluer le rendement diagnostique et la securite de la realisation des bronchoscopies chez les patients transplantes renaux. Methodes Une etude retrospective concernant tous les patients TR ayant eu une bronchoscopie entre le 1er octobre 2011 et le 1er juin 2017 au CHU de Rouen a ete menee. Dans une population totale de 59 TR ayant eu une bronchoscopie, 39 (66 %) patients ayant eu leur bronchoscopie apres la TR ont ete inclus. Pour chaque patient, les donnees cliniques, biologiques, microbiologiques et radiologiques ainsi les resultats de la bronchoscopie ont ete recueillies. Les complications de la bronchoscopie ont ete egalement recueillies. Chaque resume clinique accompagne des donnees radiologiques a ete soumis a un panel de pneumologues ignorant si le patient avait ou non eu une bronchoscopie. Leur hypothese diagnostique et leur recommandation quant a la pratique d’une bronchoscopie etaient recueillies pour chaque patient. Resultats Les indications a la bronchoscopie etaient cliniques (fievre, n = 18, 46 %) et/ou radiologiques (infiltrats, n = 17, 44 %). Les etiologies etaient principalement d’origine infectieuse, plutot de nature bacterienne que virale ou fongique. Le rendement diagnostique global observe de la bronchoscopie etait de 41 % mais il etait nettement plus eleve en cas de cause infectieuse (77 %). La bronchoscopie a induit une modification therapeutique dans 51,3 % des cas. Le consensus entre les differents membres du panel relatif a la realisation d’une endoscopie etait faible : κ : 0,251 (p Conclusion La realisation d’une bronchoscopie chez les patients TR a un rendement et un profil de securite satisfaisants. La complexite de ces patients justifie une prise en charge multidisciplinaire.

We report the chemical and isotopic composition of fumarole gas discharges collected at Poás Volcano, Costa Rica from 2001 to 2014, covering a period during which the volcano experienced a series of phreatic eruptions (March 2006 to October 2014). The relative abundances of Poás C–S–H–O gas species are controlled by reactions involving the SO2–H2S and So–SO2 gas buffers indicating magmatic temperatures of up to 800 °C. Although fumarole outlet temperatures are < 120 °C for most samples, SO2 is the dominant sulfur gas and HCl contents are relatively high. Gas compositional changes within the magma-lake-hydrothermal system likely result from a combination of several processes, including: 1) The injection of new and undegassed magma in late 2000–early 2001, 2) the heating of the hydrothermal system, accompanied by gas pressure build-up, and 3) hydrofracturing through 2006. These processes culminated in the phreatic eruptions of 2006 and 2008. Since 2005 the lake level has declined and is now (January 2014) at the lowest level (10 m) since the last periods that it dried out completely (April 1984 and April 1994). The most recent data of 2014 show high level of degassing from the dome fumaroles and the release of HCl-rich and CO2-poor gases implies that the magma injected in late 2000 continues to supply volatiles. Our data show that time series sampling of fumarole gases provides important insights to better understand magmatic and hydrothermal processes at active volcanoes and also to potentially forecast phreatic eruptions. International audience; Legionella bacteria are ubiquitous in aquatic environments. Members of the species Legionella pneumophila are responsible for more than 98% of cases of Legionnaires' disease in France. Our objective was to validate a molecular typing method called infrequent restriction site PCR (IRS PCR), applied to the study of the ecology of Legionella and to compare this method with reference typing methods, pulsed-field gel electrophoresis (PFGE) and sequence-based Typing (SBT). PFGE and SBT are considered as gold methods for the epidemiological typing of Leg. pneumophila strains. However, these methods are not suitable to an ecological monitoring of Legionella in natural environments where a large number of strains has to be typed. Validation of IRS PCR method was performed by the identification of 45 Leg. pneumophila isolates from cooling circuits of thermal power plants by IRS PCR, PFGE and SBT. The parameters of each method were measured and compared to evaluate the effectiveness of IRS PCR. The results of this study showed that IRS PCR has a discriminating power similar or better than that of the reference methods and thus that, by its speed and low cost represents an appropriate tool for the study of the ecology of Legionella in cooling circuits.

Reducing the area occupied by microwave monolithic integrated circuits is a necessity to decrease the cost of these circuits. A reduction in area can be obtained with circuit configurations comprising a minimum of inductances and by using spiral inductors. When a circuit is compacted, the coupling between radiating elements such as the inductances has also to be taken into account. An amplifier working from 2 to 10GHz with a measured gain of 5.8 +- 0.4dB is presented. The efforts to reduce the area have led to a surface of 0.16mm/sup 2/ and therefore an integration density of 36dB/mm/sup 2/.

This paper describes the performance of a GaAs monolithic X-Band SPDT switch with integrated BFL driver TTL compatible. A single TTL command voltage is needed to switch the RF signal from one channel to the other. From 8 to 12 GHz, the insertion loss is less than 1.6 dB and the isolation of the disconnected channel greater than 21 dB. The response time of the switch, which was not optimized in the design, was found to be 2 ns. To our knowledge this is the first fully monolithic mixed microwave and digital SPDT switch to be published.

- Abstract
Identification of therapeutic targets other than asthma can guide the choice of biologics in cases of severe asthma. Some of the allergic diseases (atopic dermatitis, food allergies, allergic rhinoconjunctivitis) that may be associated with asthma can be treated with biologics. In this review, we aim to assess the effectiveness of these biologic therapies on the allergic comorbidities of asthma. In the treatment of atopic dermatitis, only Dupilumab, an anti-IL4Rα, has proven its effectiveness and has received reimbursement authorization for this indication. In patients presenting with allergic rhinoconjunctivitis, Omalizumab has shown effectiveness, but has not been approved for this indication. Data from post-hoc analyses of studies on severe asthma likewise suggest the effectiveness of Dupilumab regarding allergic rhinitis. While these two biologic therapies have shown positive signals, inducing oral food tolerance, the relevant data are not robust. Biologic therapies targeting IL-5 or its receptor (Mepolizumab, Benralizumab) have seldom been evaluated in allergic comorbidities, excepting atopic dermatitis, for which their effectiveness has not been proven. Lastly, there are interesting data on the combination of biologic therapy and allergen immunotherapy in cases of allergic rhinitis and food allergies, but they need to be confirmed by randomized studies., (Copyright © 2024 SPLF. The term "littératie" is derived from the English word "literacy", which refers to knowledge and skills in the fields of reading, writing, speech (or other means of communication) and calculation that allow people to be efficient and integrated into society. Health literacy is a recent concept that relies on the ability to find, understand, evaluate and communicate information in ways that promote, maintain and improve the health of the individual in various settings over the course of life. The objectives of this review are first of all to realize an overview on the health literacy of populations in Europe. Then, we propose to study the link between health literacy and health status (risk behaviors, chronic diseases, morbi-mortality, adherence to care and medical monitoring) and to study its medico-economic impact. We also analyzed the association between personalized therapeutic education and health literacy. Introduction: In France, opioids are responsible for the majority of overdose deaths in France. The antidote naloxone is available in France in take home formulations since 2016. Addiction specialized centres are in front-line for naloxone diffusion. The objective was to provide an overview of professional practices, difficulties and needs regarding overdose prevention and naloxone diffusion in these centres of the Provence-Alpes-Côte d'Azur (PACA) region., Methods: The POP programme "Prevention and harm reduction of opioid overdoses in PACA region" aims to improve the care of patients at risk of overdose and facilitate naloxone diffusion. The 75 addiction specialized centres of the PACA region were invited to respond to a semi-structured interview or a telephone questionnaire. Data collected included 2020 centres' activity and professionals' perceptions of overdose risk in their active file, their practices, difficulties and needs., Results: Overall, 33 centres answered. Among them, 22 were dispensing naloxone with a mean of 20 kits dispensed in 2020 (min-max: 1-100). Two strategies were pointed out: systematically offering of naloxone to all opioids users or targeting people considered at risk. Several difficulties limiting naloxone diffusion were expressed: a lack of knowledge from opioids users, refusal from people not feeling concerned or refusing the injectable formulation, a lack of training of some professionals to be comfortable with the tool or reglementary or time constraints., Conclusion: Naloxone diffusion is incrementally getting into common practices. However barriers are persisting. - Abstract
Patients with autosomal dominant (AD) hyper-IgE syndrome (HIES) suffer from a constellation of manifestations including recurrent bacterial and fungal infections, severe atopy, and skeletal abnormalities. This condition is typically caused by monoallelic dominant-negative (DN) STAT3 variants. In 2020, we described 12 patients from eight kindreds with DN IL6ST variants resulting in a new form of AD HIES. These variants encoded truncated GP130 receptors, with intact extracellular and transmembrane domains, but lacking the intracellular recycling motif and the four STAT3-binding residues, resulting in an inability to recycle and activate STAT3. We report here two new DN variants of IL6ST in three unrelated families with HIES-AD. The biochemical and clinical impacts of these variants are different from those of the previously reported variants. The p.(Ser731Valfs*8) variant, identified in seven patients from two families, lacks the recycling motif and all the STAT3-binding residues, but its levels on the cell surface are only slightly increased and it underlies mild biological phenotypes with variable clinical expressivity. The p.(Arg768*) variant, identified in a single patient, lacks the recycling motif and the three most distal STAT3-binding residues. This variant accumulates at the cell surface and underlies severe biological and clinical phenotypes. The p.(Ser731Valfs*8) variant shows that a DN GP130 expressed at near normal levels on the cell surface can underlie heterogeneous clinical presentations, ranging from mild to severe. Background: At the time of the SARS-CoV-2 emergence, asthma patients were initially considered vulnerable because respiratory viruses, especially influenza, are associated with asthma exacerbations, increased risk of hospitalization and more severe disease course. We aimed to compare the asthma prevalence in patients hospitalized for COVID-19 or influenza and risk factors associated with poor prognosis with the diseases., Methods: This retrospective cohort study used the Paris university hospitals clinical data warehouse to identify adults hospitalized for COVID-19 (January to June 2020) or influenza (November 2017 to March 2018 for the 2017-2018 influenza period and November 2018 to March 2019 for the 2018-2019 period). Asthma patients were identified with J45 and J46 ICD-10 codes. Poor outcomes were defined as admission in intensive care or death., Results: Asthma prevalence was significantly higher among influenza than COVID-19 patients (n = 283/3 119, 9.1%, 95% CI [8.1-10.1] in 2017-2018 and n = 309/3 266, 9.5%, 95% CI [8.5-10.5] in 2018-2019 versus n = 402/9 009, 4.5%, 95% CI [4.0-4.9]). For asthma patients, 31% with COVID-19 were admitted to an intensive care unit versus 23% and 21% with influenza. Obesity was a risk factor for the 2017-2018 influenza period, smoking and heart failure for the 2018-2019 period. Among COVID-19 patients with asthma, smoking and obesity were risk factors for the severe form., Conclusions: In this study, patients with an asthma ICD-10 code were less represented among COVID-19 patients than among influenza-infected ones. However, outcomes were poorer for COVID-19 than influenza patients, both with asthma. - Abstract
Background: Lockdown measures during the first wave of the COVID-19 pandemic in France led to serious public health concerns over people who use illicit drugs, especially in terms of mental health. We assessed changes in cannabis use during the first lockdown in France among daily cannabis users and associated correlates., Methods: CANNAVID is a French, national, cross-sectional web-based survey, conducted from 17 April to 11 May 2020. Daily cannabis users aged ≥ 18 years and living in France were invited to participate through advertisements. Respondents completed an ad hoc questionnaire on a dedicated online platform. We analyzed changes in cannabis use during the first lockdown (i.e., stopped, decreased, unchanged, or increased) and performed a multinomial logistic regression analysis to evaluate correlates of these changes., Results: Of the 4019 participants, 74.0% were men. Median age was 27 years (interquartile range: 22-37). With regard to cannabis use, 293 (7.3%) persons stopped, 1153 (28.7%) decreased, 1146 (28.5%) did not change, and 1427 (35.5%) increased their use during the lockdown. A multinomial logistic regression model revealed several sociodemographic, behavioral and health-related factors associated with changes in cannabis use. Compared with participants with an unchanged level of cannabis use during the lockdown, those who increased and those who stopped cannabis use were more likely to have increased tobacco and alcohol use and to have experienced depression and sleep disorders intensification. Those who stopped cannabis use were also more likely to have increased benzodiazepine use and to have experienced pain increase during lockdown., Conclusions: France's first COVID-19-related lockdown had a differential impact on daily cannabis users' consumption patterns. Most study respondents reported changes to their cannabis consumption pattern. Those who reported a stable cannabis use were more likely to report fewer negative changes. Specific interventions are needed for this population, as well as research to assess the long-term impacts of these changes., (© 2022. The Author(s).)

- Abstract
Exacerbations of asthma are still responsible for 900 deaths per year in France, most of which are considered preventable. They have a major impact on the quality of life of patients and are responsible for a large part of the socio-economic cost of asthma in France. Exacerbations of asthma can be partly prevented by inhaled corticosteroid treatment as controller therapy, at a dose adapted to the level of control of symptoms and the management of aggravating factors. Every patient should benefit from a written action plan that allows them to start oral corticosteroid therapy as soon as possible during an exacerbation. The treatment combines short-acting bronchodilators with systemic corticosteroids. Systemic antibiotic therapy has no place in the treatment of exacerbations. - Abstract
Global biodiversity in freshwater and the oceans is declining at high rates. Reliable tools for assessing and monitoring aquatic biodiversity, especially for rare and secretive species, are important for efficient and timely management. Recent advances in DNA sequencing have provided a new tool for species detection from DNA present in the environment. In this study, we tested whether an environmental DNA (eDNA) metabarcoding approach, using water samples, can be used for addressing significant questions in ecology and conservation. Two key aquatic vertebrate groups were targeted: amphibians and bony fish. The reliability of this method was cautiously validated in silico, in vitro and in situ. When compared with traditional surveys or historical data, eDNA metabarcoding showed a much better detection probability overall. For amphibians, the detection probability with eDNA metabarcoding was 0.97 (CI = 0.90-0.99) vs. 0.58 (CI = 0.50-0.63) for traditional surveys. For fish, in 89% of the studied sites, the number of taxa detected using the eDNA metabarcoding approach was higher or identical to the number detected using traditional methods. We argue that the proposed DNA-based approach has the potential to become the next-generation tool for ecological studies and standardized biodiversity monitoring in a wide range of aquatic ecosystems., (© 2015 John Wiley & Sons Ltd.)

Members of the pathogenic Legionella genus encounter suitable growth conditions in nuclear power plant cooling circuits. To limit its proliferation and ensure that levels remain below regulatory thresholds, chemical treatment with monochloramine can be used in continuous or sequential conditions. The aim of this study was to determine the impact of monochloramine on L. pneumophila subpopulations in the cooling circuits of a nuclear power plant. The chosen procedure involved monitoring the diversity and dynamics of L. pneumophila subpopulations every month over the course of a year in a nuclear power plant cooling circuit, which was treated for 2 months during the period under study. This study confirmed the effectiveness of monochloramine to limit L. pneumophila concentrations in cooling circuits. The culturable L. pneumophila community was strongly affected by the injection of monochloramine. Several subpopulations persisted during treatment at low concentrations (below the detection limit of standard methods), suggesting that the susceptibility of L. pneumophila is strain dependent. Although the composition of the subpopulations was not similar, the resilience of the community structure was observed. Indeed, the community eventually returned to its initial structure and presented a similar pattern of richness, diversity and uniformity to that seen before treatment., (© 2013 Federation of European Microbiological Societies. - Abstract
Legionella bacteria are ubiquitous in aquatic environments. Members of the species Legionella pneumophila are responsible for more than 98% of cases of Legionnaires' disease in France. Our objective was to validate a molecular typing method called infrequent restriction site PCR (IRS PCR), applied to the study of the ecology of Legionella and to compare this method with reference typing methods, pulsed-field gel electrophoresis (PFGE) and sequence-based Typing (SBT). PFGE and SBT are considered as gold methods for the epidemiological typing of Leg. pneumophila strains. However, these methods are not suitable to an ecological monitoring of Legionella in natural environments where a large number of strains has to be typed. Validation of IRS PCR method was performed by the identification of 45 Leg. pneumophila isolates from cooling circuits of thermal power plants by IRS PCR, PFGE and SBT. The parameters of each method were measured and compared to evaluate the effectiveness of IRS PCR. The results of this study showed that IRS PCR has a discriminating power similar or better than that of the reference methods and thus that, by its speed and low cost represents an appropriate tool for the study of the ecology of Legionella in cooling circuits., (© 2012 The Society for Applied Microbiology.)

Members of the Legionella genus find suitable conditions for their growth and survival in nuclear power plant cooling circuits. To limit the proliferation of Legionella pathogenic bacteria in nuclear power plant cooling circuits, and ensure that levels remain below regulatory thresholds, monochloramine treatment can be used. Although the treatment is highly effective, i.e. it reduces Legionella numbers by over 99%, Legionella bacteria can still be detected at low concentrations and rapid re-colonisation of circuits can occur after the treatment has ceased. The aim of this study was to develop an in vitro methodology for determining the intrinsic susceptibility of L. pneumophila strains, collected from various nuclear power plant cooling circuits subjected to different treatment conditions. The methodology was developed by using an original approach based on response surface methodology (RSM) combined with a multifactorial experimental design. The susceptibility was evaluated by the Ct factor. The susceptibility of environmental strains varies widely and is, for some strains, greater than that of known tolerant species; however, strain susceptibility was not related to treatment conditions. Selection pressure induced by monochloramine use did not result in the selection of more tolerant Legionella strains and did not explain the detection of Legionella during treatment or the rapid re-colonisation of cooling circuits after disinfection has ceased.

- Abstract
Array-CGH enables the detection of submicroscopic chromosomal deletions and duplications and leads to an accurate delineation of the imbalances, raising the possibility of genotype to phenotype and mapping minimal critical regions associated with particular patterns of clinical features. We report here on four patients sharing common clinical features (psychomotor retardation, coarse facies and ocular anomalies), with proximal 5q deletions identified by oligo array-CGH. The deletions range from 5.75 to 17.26-Mb in size and occurred de novo. A common 2.63-Mb region between the deletions described here can be defined in 5q12.1 (59,390,122-62,021,754 bp from 5pter, hg18) and includes 12 genes. Among them, KIF2A, which encodes a kinesin superfamily protein, is a particularly interesting candidate for the phenotype, as it suppresses the growth of axonal collateral branches and is involved in normal brain development. Ocular defects, albeit unspecific, seem to be common in the 5q12.1 deletion. Identification of additional cases of deletions involving the 5q12.1 region will allow more accurate genotype-phenotype correlations., (Copyright © 2011 Wiley-Liss, Inc.)

- Abstract
While numerous detection methods exist for environmental heavy metal monitoring, easy-to-use technologies combining rapidity with in vivo measurements are lacking. Multiwell systems exploiting transgenic tadpoles are ideal but require time-consuming placement of individuals in wells. We developed a real-time flow-through system, based on Fountain Flow cytometry, which measures in situ contaminant-induced fluorescence in transgenic amphibian larvae immersed in water samples. The system maintains the advantages of transgenic amphibians, but requires minimal human intervention. Portable and self-contained, it allows on-site measurements. Optimization exploited a transgenic Xenopus laevis bearing a chimeric gene with metal responsive elements fused to eGFP. The transgene was selectively induced by 1 microM Zn(2+). Using this tadpole we show the continuous flow method to be as rapid and sensitive as image analysis. Flow-through readings thus accelerate the overall process of data acquisition and render fluorescent monitoring of tadpoles suitable for on-site tracking of heavy metal pollution.
