The immunoperoxidase stain for estrogen and progesterone receptor content in endometrial adenocarcinoma was correlated with the grade and stage, level of myometrial invasion, age and survival of the patients. Anti-estrogen and anti-progesteone receptor monoclonal antibodies were applied to paraffin-embedded tissue from hysterectomy specimens of 100 patients with adenocarcinoma of the endometrium. In 34 of the cases the receptors were studied in the endometrium adjacent to the tumor and compared to the nuclear receptor content in the carcinoma. There was a high inverse correlation between the estrogen receptor status and the grade of tumor (R= − 0.45,P= 0.006). The estrogen receptor measured in the endometrium near the tumor showed a negative correlation with the grade of the tumor (R= −0.42,P= 0.013). The estrogen, but not the progesterone, receptor content, was positively related to the age of the patient (P< 0.05). No significant correlation of the receptor status with the depth of myometrial invasion was found, despite the obvious interdependence between the grade and myometrial invasion. The progesterone receptor staining index appeared to be a distinct independent prognostic factor in endometrial cancer. The immunohistochemical analysis of the steroid hormone status in endometrial cancer therefore offers an alternative to the quantitative ligand-binding assay.

Dermatofibrosarcoma protuberans is an uncommon skin tumor with a high potential for local recurrence if not adequately excised. There are only two reports in the literature describing dermatofibrosarcoma protuberans that enlarged considerably during pregnancy. On the other hand, the new development or enlargement of dermatofibromas in pregnancy is well documented. We present 2 additional patients in whom a dermatofibrosarcoma protuberans appeared and grew rapidly during pregnancy. Immunohistochemical studies were negative for estrogen and progesterone receptors. The possible pathophysiology and surgical management are presented and discussed.

This study deals with the pattern of estrogen receptors in 52 cases of invasive breast carcinoma by comparing the ligand-binding assays with dextran-coated charcoal (DCC) to an immunoperoxidase assay (IPE) technique in paraffin-embedded material from the same tumor. A modification of the IPE by adding cobalt chloride in the final reaction and counter-staining with eosin instead of hematoxylin was introduced. A significantly high correlation was found between the two compared methods (80.8% positivity by the DCC method and 80% by the IPE method). The correlation was very significant in cases where the immunohistochemical grading was higher than 4 (according to our semi-quantitative evaluation scale of 0-8), and for these cases we suggest the IPE method as highly specific and precise.

The histological findings in the first diagnostic biopsy of 50 patients with Kaposi's sarcoma (KS) were classified and were related to the clinical course. Forty-one patients had the "classic KS," of whom 31 had a benign course and 10 an aggressive disease. Nine patients had iatrogenically induced KS. Histologically the following subtypes were found: mixed type in 18 patients, spindle type in 16, early type in 14, and hemangiomatous and lymphangiomatous types in one each. We did not find any statistical relationship between the histological subtypes, degree of nuclear atypia, number of mitoses, and density of the inflammatory cell infiltrates in the KS lesions, and the clinical types and course of the disease. The results of this study do not support the possibility of a prognostic importance regarding the histological features of the early lesions in patients with classic or iatrogenic Kaposi's sarcoma.

We studied 10 biopsies of early (1 week) localized granuloma annulare (LGA) lesions and 10 specimens of the normal-looking skin adjacent to actively spreading LGA lesions for signs of vasculitis, using histological and direct immunofluorescent (DIF) techniques. In the early LGA lesions, the collagen fibers showed various forms of alterations, with hyalinization and fragmentation being the most common; in half of these lesions, neutrophils and nuclear fragments in various numbers and densities were found among the altered collagen fibers. Some of the small blood vessels in the areas of granulomatous inflammation showed endothelial cell hypertrophy in four cases, and in one case also endothelial cell proliferation and luminal occlusion. In only one case, a single small blood vessel, which was situated in the center of a palisading granuloma, showed fibrinoid necrosis of its walls. The DIF study of all 20 specimens and the histological study of the 10 specimens of normal-looking adjacent skin did not reveal immune deposits in the vessels' walls, or histological evidence of vasculitis, respectively. We believe that these findings do not support a role of vasculitis in the formation of LGA lesions, but suggest that neutrophils might play a primary or secondary role in their pathogenesis.

Amelanotic malignant melanoma can present a diagnostic problem in histopathology, especially when it presents in an extracutaneous location. In such cases electron microscopy and/or immunohistochemistry are invaluable for diagnosis. A 63-year-old women with rectal bleeding was found to have an amelanotic malignant melanoma of the anal canal, with compatible clinical and gross pathology and light and electron microscopic findings. Tumor cells showed positive staining with antibodies to vimentin, S 100 protein, and HMB-45 (melanoma-specific antibody), but also with polyclonal antibody to carcinoembryonic antigen (CEA). Tumor cells failed to stain with monoclonal antibody to CEA. The nature and significance of CEA reactivity in malignant melanoma are discussed.

To emphasize the importance of early diagnosis in cases of müllerian adenosarcoma that appeared as benign-looking cervical polyps.We examined seven cases of müllerian adenosarcoma of the uterus in patients 14-63 years of age (median 39 years). Tissue protruding from the external os and an initial diagnosis of a cervical polyp were common findings for all patients. On repeated examination, all lesions were interpreted as müllerian adenosarcomas.Histologic examination demonstrated benign glands with a sarcomatous stroma, which typically formed periglandular cuffs of increased cellularity. The sarcomatous stroma was homologous in four cases and contained heterologous elements such as striated muscle, lipoblast, and cartilage in three cases; one patient had a sarcomatous overgrowth of stromal elements. The question of a müllerian adenofibroma versus adenosarcoma was raised in three cases with the general appearance of slit-like glands surrounded by a stroma with fibrosis and a low mitotic rate. Using the criteria of stromal cellularity--marked stromal atypia and a mitotic index of two figures per ten high-power fields--the cases were classified as adenosarcomas. The sarcomatous overgrowth, the presence of heterologous elements, and a high mitotic rate seem to be important prognostically.Gynecologists and pathologists should be aware of the difficulties and delay in the diagnosis of müllerian adenosarcoma when the tumor presents as a benign-looking cervical polyp.

The peroxidase-antiperoxidase staining technique for the antigens S-100, neuron-specific enolase, and alpha 1-antichymotrypsin (alpha 1ACT) was applied to 57 intraocular tumors: 46 malignant melanomas of the uvea, seven retinoblastomas, and four tumors metastasizing to the eye. The staining characteristics of the different intraocular tumors were compared. Staining for S-100 in a fine-needle aspiration biopsy sample taken from a malignant melanoma of the choroid before enucleation of the globe was attempted. The positive staining of a few cells thus obtained suggested that this technique may be helpful in the diagnosis of melanomas. The alpha 1ACT stain used in this study has not been used previously in ophthalmology to our knowledge. We found 60% of malignant melanomas of the choroid stained positively. Another finding was the staining of the retinal pigment epithelium with alpha 1ACT in 30% of eyes with malignant melanoma of the uvea.

A post mortem examination of a 70 year old man, who died three years after a poorly differentiated adenocarcinoma of prostate had been diagnosed, showed widespread signet-ring cell carcinoma, with an associated linitis plastica. The signet-ring cells stained positively with prostatic specific antigen and with prostatic specific acid phosphatase, but failed to react with mucopolysaccharide staining. The electron microscopic appearance of the signet-ring cell tumour was due to the presence of large cytoplasmic vacuoles. This case emphasises the possibility that cases of metastatic signet-ring cell carcinoma may be prostatic in origin. This can be confirmed by specific immunohistochemical studies.

No detailed information about the ultrastructure of Tenon's capsule has been published. The purpose of the present study was to compare the ultrastructural features of collagen fibrils from Tenon's capsule in a nonstrabismic control group (seven children) to those in an infantile esotropic group (10 children). Small biopsy specimens from Tenon's capsule were taken during various operations to be examined by electron microscopy. On electron microscopy, the capsule was found to be composed of groups of collagen fibrils arranged irregularly in different orientations, forming a three-dimensional network that provides tissue resistance to stress. The cross-sectioned collagen fibrils were studied by an image analyzer. In both study groups, all fibrils had a round, regular contour. In the esotropic group, the Tenon's collagen fibrils were thicker, as reflected by their significantly greater mean diameter: 101 +/- 5 nm (mean +/- standard deviation) compared to 86 +/- 5 nm in the control group. Also, significantly greater heterogeneity was found in the collagen fibril thickness of each individual in the esotropic group compared to the control group. Moreover, the mean number of collagen fibrils per unit area was significantly higher in the esotropic group: 98 +/- 13 fibrils per 10(6) nm2 compared to 73 +/- 5 fibrils per 10(6) nm2 in the control group. These ultrastructural changes may be stress-induced secondary alterations of the Tenon's collagen fibrils resulting from prolonged deviation of the eye in infantile esotropia. The significantly denser collagen fibrils may cause a decrease in the elasticity of Tenon's capsule in infantile esotropia.

Fibrous hamartoma of infancy is an uncommon lesion of uncertain histogenesis. Three cases were studied by light microscopy, electron microscopy, and immunohistochemistry. Two histologic variants are presented. Ultrastructurally, fibroblasts and myofibroblasts were found in the fascicular-fibroblastic areas while primitive mesenchymal cells were found in the immature-appearing regions. Vimentin positivity was noted in both areas, whereas desmin and actin positivity was found mainly in the fascicular-fibroblastic regions. The lesion appears to fulfill the criteria for its characterization as “hamartoma”.

Thirty-three children with idiopathic nephrotic syndrome who underwent kidney needle biopsy were reevaluated. The male to female ratio was 2:1, and a preponderance of North-African Jewish and Arab origin over Ashkenazi Jewish origin was noted. There was a positive correlation between the severity of glomerular changes and prognosis among the 10 cases with minimal change disease (MCD) and the 23 with focal segmental glomerulosclerosis (FSGS). On long-term follow-up (mean over 11 years) chronic renal failure developed in none of 10 MCD patients, 1 of 12 FSGS patients with mild glomerular sclerosis, 1 of 7 FSGS patients with moderate glomerular sclerosis and 3 of 4 FSGS patients with severe glomerular sclerosis. Prognosis of patients with mild glomerular sclerotic lesion on light microscopy was substantially not worse than the prognosis of patients with mild glomerular alterations only on the electron microscopic study (MCD-B). Thus, both pathologically and prognostically, there was a continuous spectrum from 'pure' MCD (MCD-A) to FSGS with severe glomerular sclerosis. Glomerular changes confined to the origin of the proximal tubule ('tip' changes) were seen only in 4 patients and did not have a distinct prognostic significance. No case of peripheral location of the sclerotic segment within the glomerulus was found in our series of FSGS, and therefore no correlation between location of segmental sclerosis and prognosis was feasible.

The fibrotic lateral rectus muscle and the medial rectus muscle of 4 patients with esotropia and myopia who underwent surgical correction were studied ultrastructurally. All patients revealed underactivity of the lateral rectus muscle. In 3 patients the underactive lateral rectus was completely fibrotic; in 1 patient there was 80% fibrosis, and the rest of the muscle showed different stages of degeneration. The fibrotic lateral recti were composed of collagen fibrils and remnants of degenerated organelles. The collagen fibrils of the lateral recti were of different sizes, and the mean diameter varied from 65 to 92 nm. On the other hand, the interstitial collagen fibrils of nonfibrotic medial rectus muscles of these patients and also of control muscles showed less variation in size, and the mean diameter was smaller as compared with the fibrotic lateral rectus. It is suggested that the collagen fibrils that compose the fibrotic lateral rectus muscle probably develop under conditions which differ from the normal development of collagen fibrils.

A case of oat-cell lung carcinoma metastatic to renal oncocytoma is described. The pertinent literature is reviewed.

Fechtner syndrome, a variant of Alport syndrome, was first reported by Peterson et al. [1985]. It is characterized by nephritis, hearing loss, eye abnormalities, macrothrombocytopenia, and leucocyte inclusions, present in varying combinations in several members of the same family. This is the second family reported; 16 relatives are affected. The clinical manifestations of the syndrome are delineated. The pattern of inheritance is autosomal dominant. The hematologic abnormalities are similar to those detected in May Hegglin anomaly. They are present in every affected relative and may be present at birth. The feasibility of prenatal diagnosis is discussed.

Summary The combination of systemic lupus erythematosus, Sjögren's disease and severe diffuse glomerular nephritis has only rarely been reported. A 14-year-old girl is described with lupus nephritis in whom co-existent clinical and histological features of Sjögren's syndrome were found. These include bilateral parotid enlargement, xerostomia, increased serum amylase, reduced salivary secretion and lymphocyte infiltration of both salivary glands and kidneys. The co-existence of systemic lupus erythematosus with Sjögren's syndrome is discussed together with a consideration of pathogenesis.

Four children from two related families are described with a clinical picture characterized by the onset of asymptomatic proteinuria with subsequent nephrotic syndrome in infancy, and progression to renal failure and death before the age of three years. The clinical picture and the renal pathological studies were consistent with the entity described by Habib as infantile mesangial sclerosis. We propose that this entity possibly represents a genetic disorder which is transmitted as an autosomal recessive trait.

SUMMARY Mast cells in the upper dermis and mucosal subepithelial layer were counted in thirty consecutive light microscopy oil-immersion fields of the following: thirty-four skin and oral specimens from lesions in patients with active Behcet's disease, eight specimens of apparently uninvolved skin of the same patients, and 102 lesions of a variety of other skin diseases. The results revealed a significant increase in the number of mast cells in Behcet's lesions. Furthermore, the histamine content of five reactive lesions showed a two-fold increase compared with that in apparently uninvolved skin of patients with active Behcet's disease. It is suggested that mechanisms similar to cutaneous basophil hypersensitivity may be involved in the production of Behcet's lesions.

A series of 120 breast biopsies and mastectomy specimens originally diagnosed as carcinoma was reviewed in order to emphasize the differences between lobular carcinoma in situ and cancerization of lobules. Criteria for differential diagnosis between the two types of lobular lesion are proposed. In the reviewed material 15 (12.5%) cases of lobular carcinoma in situ and 64 (53.3%) of lobular cancerization were found. The carcinoma in situ usually co-existed with other types of breast carcinoma, such as intraductal carcinoma, cancerization and invasive carcinoma of different types.

SUMMARY The mechanism of skin hyperreactivity (pathergy) in Behcet's disease is unknown. It has been suggested that the response is due to an Arthus-like reaction. In 18 patients with active Behcet's disease, pathergy was induced and the skin biopsied after 24 h. The histological reaction was that of mild to intense round cell infiltration, perivascular in type. Direct immunofluorescence failed in all 18 patients to demonstrate the local presence of immunoglobulins and complement at the site of lesion. A marked increase in the number of mast cells was observed at the site of reaction and scattered throughout the dermis, the role of which is to be further investigated. The study failed to demonstrate a humoral mechanism in the production of the cutaneous hyperreactivity.

D-penicillamine is not generally considered to cause hepatic damage. Cholestatic jaundice developed in a patient with rheumatoid arthritis 4 weeks after penicillamine was added to his regimen, and he died in acute renal failure. The probability that penicillamine caused the cholestasis is discussed.

Postoperative jaundice is an occasional complication of major surgery. Four illustrative cases are described and a mechanism of development is suggested. The jaundice may seem to be biochemically of the obstructive type. Most of the bilirubin is of the conjugated type. The disturbance of liver function can be caused by several factors, including massive transfusion, hematoma in tissues, shock, heart failure, anoxia, infection and perhaps medications. Postoperative jaundice by itself is not of cardinal importance in deciding the prognosis of the patient.

Four initially misdiagnosed cases of thyroid cancer or purported thyroideal malignancy are described. In each, aspirative cytology was instrumental in establishing the true nature of the lesion.

While immigrants from Morocco and Romania make up 7 and 6%, respectively, of the Israeli population, one-third of all postnecrotic cirrhosis patients come from Morocco and another third from Romania. The rate of hepatitis B virus (HBV) infection, as expressed by serum hepatitis B surface antigen, anti-HBs, and anti-HBc, was compared in immigrant patients with postnecrotic cirrhosis and in age and sex matched controls from the same countries. Among 19 Moroccan immigrants with postnecrotic cirrhosis 74% showed evidence of HBV infection. This was not significantly different from the prevalence of HBV infection (64%) in a matched control group of 61 Moroccan individuals. In the 26 Romanian cirrhotics, however, a significantly higher prevalance of HBV infection than in the 60 matched controls was observed (54 and 23%, respectively, p less than 0.01). The difference was detected only when anti-HBc was looked for in addition to hepatitis B surface antigen and anti-HBs. These results indicate that: 1) HBV infection and postnecrotic cirrhosis are associated in Romanian immigrants to Israel. 2) Association between HBV infection and cirrhosis in Moroccan immigrants cannot be shown because of the high infection rate in the control population. 3) Anti-HBc is often the only marker of HBV infection in cirrhotic patients, and should be examined in addition to hepatitis B surface antigen and anti-HBs in order to get meaningful results.

The capillaries of overacting inferior oblique muscles from children with strabismus and of dog inferior oblique muscles were ultrastructurally described. Biopsy material was used in all instances. The mean diameter of the capillary lumen was 2.03 microns for the human inferior oblique and 2.76 microns for the dog inferior oblique. Quantitative estimates of capillary basement membrane width were obtained by two different methods of measurement. The capillaries of human and dog inferior oblique muscles were compared with capillaries of human gastrocnemius muscle and of dog quadriceps muscle. The data were statistically evaluated. It was found that the mean basement membrane width of capillaries of overacting inferior oblique muscles is quite similar to that of the dog normal inferior oblique muscles, showing 2,049 A in overacting muscles and 1,976 A in normal muscles. With both methods of measurement, the mean basement membrane width of inferior oblique muscles appeared thicker than the mean basement membrane of capillaries of the leg muscles in man and in dog as well. It is suggested that the thickening of the capillary basement membrane of inferior oblique muscles represents a regional characteristic. Marked variation in the thickness of the basement membrane was encountered in all muscles.

A rare case of malignant melanoma metastasizing to the kidney is described. Five other cases were found in the English literature, this being the sixth. A review of the subject is brought up.

Light microscopy findings in early Wilson's disease (hepatolenticular degeneration) may be normal even when special cytochemical stains are used. We present a case of Wilson's disease in which light microscopy was negative, while electron microscopy showed the characteristic changes in the hepatocytes. Since low serum ceruloplasmin levels and high urinary copper excretion are not by themselves definite proof of Wilson's disease, and since copper content of the liver is not universally measured, electron microscopy examination of liver tissue appears to be a worthwhile additional tool for the early diagnosis of Wilson's disease.

The assumption that fresh cow's milk may have a direct effect on jejunal mucosal macromolecular permeability was tested in 14-day-old suckling guinea pigs, by in situ luminal perfusion of a proximal jejunal segment with horseradish peroxidase administered either in 0.9% NaCl or in cow's milk. For positive control, a group of guinea pigs previously sensitized to beta-lactoglobulin was similarly perfused with horseradish peroxidase in 0.9% NaCl or in cow's milk at 14 days of age. A segment of the wall of the perfused jejunal loop was processed for peroxidase staining and for examination of the extent and routes of macromolecular absorption by light and electron microscopy. Mucosal exposure to cow's milk was associated with penetration of tracer material across the jejunal epithelium only in the lactoglobulin-sensitized guinea pigs. In guinea pigs with a first-time exposure to cow's milk, no entry of tracer material beyond the brush border surface of the enterocytes was seen. Thus, whereas we could observe absorption of bystander antigens during antigenic challenge of the mucosa in sensitized animals, we found no evidence of a presumptive "permeability factor" in cow's milk that can disrupt the mucosal macromolecular barrier by an acute direct action.

In order to assess the role of mast cells in the pathogenesis of mucocuta-neous lesions of Behçet’s disease, the following investigations were performed: (1) a comparative study of ultrastructural features of mast cells in sopntaneous and reactive (pa-thergic) lesions of the process and in apparently uninvolved skin of patients in the active stage of the disease; (2) comparative estimation of the histamine content at the site of cutaneous hyperreactivity lesions and in apparently uninvolved skin of the same patients. Various forms and degrees of degranulation were revealed among the mast cells. These forms were obviously more frequent in spontaneous and reactive lesions than in the uninvolved skin, 66 and 56% as compared with 7.7%. Also the histamine content was twofold more in the reactive lesions than in the unaffected skin of the same patient, 43 ng/mg tissue as compared with 21 ng/mg tissue. The results of our study seem suggestive of an active role of mast cells in this process. The role is probably similar to that of basophils in cutaneous basophilic hypersensitivity.

Distant bone metastases in cases of intracranial meningiomas are uncommon. Two patients with meningiomas showing malignant histological features are described, in both of whom bone metastases were observed many years after cranial surgery.

The relationship of structure and function of the intact myocardium during the early minutes of experimental ischemia was studied in the anesthetized, open-chest pig. Regional contraction of ischemic and normal myocardium was measured, using isometric strain gauges on the anterior and lateral left ventricle. Biopsies for electron microscopy were taken consecutively 5, 10, 15, and 20 minutes after anterior descending artery occlusion-each followed by 10 minutes of reperfusion. Contraction of the ischemic area fell significantly within 15 to 30 seconds following occlusion, returning to 68, 50, 40, and 28%, respectively, after 5, 10, 15, and 20 minutes fo ischemia. In contrast, slight morphological changes were detected only after 5 and 10 minutes of ischemia, becoming more clear after 15 minutes, and unequivocal after 20 minutes-when return of contraction was minimal. The morphologic alterations in the ischemic area as seen by electron microscopy were: reduction of glycogen granules and specific changes in the mitochondria. The latter, consisting of swelling, disruption of cristae, and reduction of matrix, began to appear at 5 minutes and were more evident after 10 minutes of ischemia. These changes, although present, were not always proportional to sequential ischemic periods. This apparent lack of correlation between contraction and morphology could be due to sampling problems or to the focal nature of the ischemic process. However, it could imply that the changes seen by electron microscopy are a "late" phenomenon as compared to the loss of function. Morphologic corroboration of the immediate decrease of contraction of ischemic myocardium still escapes the perception of current morphological techniques. It is hoped that more refined stereological measurements will help bridge the present gap between function and morphology in early myocardial ischemia.

Salivary composition and lip biopsies were analysed in 35 xerostomic patients; 23 of the patients suffered from Sjogren's syndrome. A highly significant correlation was found between lip biopsies and sialochemistry: r = 0.382, p r = 0.56, p

The clinical picture and histopathological findings in 11 infants with infantile nephrotic syndrome are presented. Three infants, who presented in the perinatal period, had congenital nephrotic syndrome (Finnish type); six of eight infants who presented in the 1st yr of life had mesangial sclerosis nephrotic syndrome, while the remaining two had minimal change nephrotic syndrome. Four of the six infants with mesangial sclerosis nephrotic syndrome were from one family. All patients died within 2 mo to 2 yr after presentation.

Application of the hematoxylin basic fuchsin-picric acid method to human postmortem myocardium specimens failed to show reproducible results. Therefore, controlled animal studies utilizing the method were undertaken. Myocardial ischemia was induced in swine by ligation of the anterior descending coronary artery for periods of 30 minutes to six hours. Electron microscopy was used to monitor normal and ischemic myocardium. Reproducible results were not obtained by staining successive slides of the same block at the same time or at different times, whether stained in the same reagent bath or after changing the reagent solutions. Fresh, frozen, unfixed material also yielded nonreproducible results. The method in its present form is unreliable for routine use in the diagnosis of early myocardial ischemia.

Regional contraction of ischemic anterior and normal lateral left ventricular myocardium was measured with isometric force gauges after 5, 10, 15, and 20 minutes of anterior descending coronary artery occlusion-each followed by 10 minutes of reperfusion. Multiple myocardial biopsies of both regions were taken at these same intervals and examined by electron microscopic techniques. Mean contraction of the ischemic area fell significantly in 15 to 30 seconds and returned to an average of 68, 51, 40, and 28 per cent, respectively, after 5, 10, 15, and 20 minutes of ischemia. Simultaneously, focal morphologic changes were detected after 5 and 10 minutes, were more clear and widespread at 15 minutes, and diffuse and unequivocal at 20 minutes, when return of local contraction was minimal. The changes of myocardial morphology in the ischemic area as seen by electron microscopy were: reduced content of glycogen granules and mitochondrial changes. The latter began to appear at 5 minutes and consisted of swelling, disruption of cristae, and reduction of matrix. This study indicates a qualitative correlation between ultrastructural changes in regionally ischemic myocardium and diminished regional function in the intact heart. At 5 and 10 minutes the mitochondrial changes were focal, requiring multiple samples, while at 15 and 20 minutes they became more widespread, making the occasional sample more representative.

Two patients in whom various immunological phenomena obscured and delayed the diagnosis of subacute bacterial endocarditis (SBE) are described. Immunological abnormalities usually correlate well with the chronicity and severity of the disease, as is demonstrated here. The mechanisms underlying the various immunological phenomena are discussed. The importance of alertness to the possibility of SBE in cases of disease manifesting itself as an immunological disorder, thus achieving early diagnosis and early antimicrobial treatment, is stressed.

A comparative cytologic-histologic study of 107 lesions clinically suspected of being basal-cell carcinomas was conducted. Good correlation was found in 94.3% of the cases. This study indicated that needle aspiration cytology could be utilized for the preoperative and follow-up diagnosis of skin lesions clinically suspected of being basal-cell carcinoma. Cellular as well as structural diagnostic criteria are suggested.

Ascites induced by hybridoma cells following their injection into pristane-primed peritoneal cavities of BALB/c mice is often associated with the formation of intra-abdominal tumors related to the transplanted cells. These tumors and adjacent abdominal and extra-abdominal organs were excised and examined by electron microscopy. Clusters of viral particles were seen in nearly all cells composing the hybridoma tumors. The viral particles were usually present in rough ER cisternae of the transplanted tumor cells and at times were observed budding from the cisternal membranes. Individual viral units were doughnut shaped, 80-85 nm in diameter, with two concentric shells of which the outer one was more electron dense. No exogenous viral particles were detected in the peritoneally growing hybridoma cells. All non-tumor tissues examined such as liver, spleen, lung, brain and circulating WBC were free of virus. These findings provide further evidence of viral contamination of hybridomas and demonstrate the mode of persistence of such contamination when cells are placed in-vivo.

GIANT cell tumour of tendon sheath is a relatively common lesion and is also referred to as pigmented villonodular synovitis. The tumour consists of lymphcytes, foamy macrophages, haemosiderin and fibrous tissue but lacks the characteristic spindle cell stroma of a giant cell tumour of soft tissue which is relatively rare (Salm and Sissons, 1972). It presents morphologically as an encapsulated firm lesion palpable as an irregular subcutaneous nodule. It arises from a tendon sheath and may extend locally over or under the sheath, alongside the bone or neurovascular bundle (Boyes, 1972). In the following case the tumour appeared as a fungating mass which eroded the overlying skin and protruded about I cm above its surface. To the best of our knowledge this clinical appearance of a giant cell tumour arising from a tendon sheath has not previously been reported.

Inbred X/Gf mice are reported to have a very low incidence of spontaneous and induced tumors. Experiments were performed to determine whether the cells of the X/Gf mouse are themselves resistant to in vitro oncogenesis or the organism as a whole is surveyed by a homeostatic mechanism that prevents the expression of potential neoplasia. Cultures of X/Gf or C57BL/6J embryonic fibroblasts were exposed to the chemical carcinogens 7,12-dimethylbenz[a]anthracene and benzo[a]pyrene to test whether the cells of the X/Gf mouse are themselves resistant to oncogenesis. The chemical carcinogens caused transformation of both X/Gf and C57BL/6 fibroblasts into cells that demonstrated altered growth patterns in liquid culture and grew in a clonal fashion in soft agar. Cloned transformed cells were shown to be malignant and resulted in the growth of sarcomas in vivo. X/Gf mice were decidedly more resistant than were C57BL/6 mice to their respective clones of transformed cells. The resistance to neoplastic growth demonstrated by the X/Gf mice was inherited by (C57BL/6J X X/Gf)F1 mice that were resistant to the growth of X/Gf-transformed and C57BL/6-transformed cells at cell doses at which the parental X/Gf mice had survived but at which the C57BL/6 had succumbed to progressively growing tumors. It has thus been established that the tumor resistance of the X/Gf mice is not based on the resistance of isolated cells from these animals to malignant transformation, but rather is attributable to an in vivo homeostatic mechanism that prevents tumor growth.

A case of immunoblastic lymphadenopathy and systemic amyloidosis in a 21-year-old man is presented. The basic defect in immunoblastic lymphadenopathy is believed to be an abnormal immune reaction, most probably of the B cell system. Although systemic amyloidosis is common in some disorders of the B cell system, no cases of systemic amyloidosis complicating immunoblastic lymphadenopathy have been previously reported. Immunoblastic lymphadenopathy includes elements known to be associated with amyloidosis and we believe that this association is not merely coincidental.

Quantitative light microscopic and ultrastructural kidney changes were studied in young and old mice subjected to moderate exercise. While there was little or no change in the glomeruli of the young animals, the older age-group showed progressive and statistically significant mesangial sclerosis with obliteration of capillary lumina. Electron microscopy also showed electron dense deposits in both the mesangium and subendothelium. Kidney weight in the young mice subjected to exercise was decreased when compared with the controls, while old animals showed a higher kidney weight after 6 weeks of running exercise. Our results of progressive kidney changes in old exercising animals, together with previous studies on myocardial and skeletal muscle showing parallel changes, stress the need for careful clinical studies on the possible damage of these organs in older people who engage in jogging and heavy exercise.

The effect of short-term endurance exercise on the kidneys of C57BL/6J mice of various ages was studied. While as a function of age there was a slight increase in the ratio of kidney weight to body weight, after 6 weeks of training there was a significant hypertrophy of kidneys of old mice compared to young ones. In addition to these gross anatomical studies, light- and electron-microscopic studies were performed. Using light microscopy, semiquantitative morphological assessment of kidney damage revealed that in young animals about 66% of total glomeruli were characterized as normal. 32% of the glomeruli were singled out as slightly damaged but still functioning. These values remained basically the same for kidneys of young animals trained for 6 weeks. In old mice, on the other hand, only 37% of total glomeruli were characterized as normal morphologically, and over 60% showed various degrees of kidney sclerosis. Exercise for 6 weeks reduced the proportion of normal-appearing glomeruli to 6.25%, while the damaged glomeruli of all types increased to over 90%. Electron-microscopic pictures revealed an increased amount of mesangial damage and severe obliteration of capillary lumens in aging animals. These changes were magnified in old running mice. The results may indicate that while kidneys of young animals were not affected by 6 weeks of exercise, kidneys of old animals were adversely affected by this type of exercise regimen.
