OBJECTIVE: In recent years, an overwhelming association between Pediatric Type 1 Diabetes Mellitus (T1DM) and autoimmune diseases has been largely reported. The current study was designed to determine a possible association between autoimmune thyroiditis (AIT), celiac disease (CD) - associated autoantibodies, and Parvovirus B19 infection among pediatric T1DM cases in the southwestern region of Saudi Arabia. PATIENTS AND METHODS: Blood samples from age groups 1-18 years attending the Diabetic Clinic were collected over a period of 12 months. Serum anti-thyroid peroxidase (TPO), anti-thyroglobulin (TG), anti-tissue transglutaminase immunoglobulin A (TG-IgA), endomysial IgA (EMA-IgA), Parvovirus B19-IgG and IgM antibodies were detected by standard methods. RESULTS: The results showed the prevalence of autoantibodies against thyroid and CD among pediatric T1DM patients to be 44 (25%) and 25 (14.4%), respectively. The prevalence of antibodies against B19 was 70 (40%). Further determination of the prevalence of Parvovirus B19-IgG antibodies and thyroid antibodies among T1DM pediatric patients revealed that there was a signifi- cant association between them with a p<0.0491. CONCLUSIONS: The prevalence of autoantibodies against the thyroid was higher among the seropositive Parvovirus B19 children with T1DM. A positive association between the prevalence of autoantibodies against thyroid disease and the increase in the duration of diabetes was also noted. Hence, periodic screening of T1DM patients for B19 antibodies and autoantibodies for thyroid is crucial. [ABSTRACT FROM AUTHOR]

Parvovirus B19, one of the most common human pathogens, is a small DNA virus that belongs to the Parvoviridae. As a result of previous infections, antibodies to B19 are present in most adults. B19 has a strong tropism to erythroid progenitor cells and is able to cause a series of medical conditions including fifth disease, arthritis, myocarditis, hydrops fetalis and aplastic crisis. No approved vaccine is currently available for B19 and there is a lack of structural characterization of any B19 epitopes. Here we present the first cryo-EM structure of a B19 virus-like particle (VLP) complexed with the antigen-binding fragment (Fab) of a human neutralizing antibody, 860-55D. A model was built into the 3.2Å resolution map and the antigenic residues on the surface of B19 capsid were identified. Antibody 860-55D bridges the capsid of B19 by binding to a quaternary structure epitope formed by residues from three neighboring VP2 capsid proteins. [ABSTRACT FROM AUTHOR]

Human parvovirus B19 (B19V) causes various human diseases, ranging from childhood benign infection to arthropathies, severe anemia and fetal hydrops, depending on the health state and hematological status of the patient. To counteract B19V blood-borne contamination, evaluation of B19 DNA in plasma pools and viral inactivation/removal steps are performed, but nucleic acid testing does not correctly reflect B19V infectivity. There is currently no appropriate cellular model for detection of infectious units of B19V. We describe here an improved cell-based method for detecting B19V infectious units by evaluating its host transcription. We evaluated the ability of various cell lines to support B19V infection. Of all tested, UT7/Epo cell line, UT7/Epo-STI, showed the greatest sensitivity to B19 infection combined with ease of performance. We generated stable clones by limiting dilution on the UT7/Epo-STI cell line with graduated permissiveness for B19V and demonstrated a direct correlation between infectivity and S/G2/M cell cycle stage. Two of the clones tested, B12 and E2, reached sensitivity levels higher than those of UT7/Epo-S1 and CD36+ erythroid progenitor cells. These findings highlight the importance of cell cycle status for sensitivity to B19V, and we propose a promising new straightforward cell-based method for quantifying B19V infectious units.

Despite continuous improvement in safety and purity of blood products for individuals with haemophilia, transmissible agents continue to affect individuals with haemophilia. This chapter addresses three viral pathogens with significant clinical impact: HIV, hepatitis C and parvovirus B19. Hepatitis C is the leading cause of chronic hepatitis and the major co-morbid complication of haemophilia treatment. Clinically, asymptomatic intermittent alanine aminotransferase elevation is typical, with biopsy evidence of advanced fibrosis currently in 25%. Current treatment is effective in up to 70%, and many new agents are in development. For those progressing to end-stage liver disease, liver transplantation outcomes are similar to those in non-haemophilia subjects, although pretransplant mortality is higher. HIV infection, the second leading co-morbid condition in haemophilia, is managed as a chronic infection with highly active antiretroviral therapy (HAART). HAART also slows hepatitis C virus (HCV) progression in those with HIV/HCV co-infection. Viral inactivation and recombinant technologies have effectively prevented transfusion-transmitted viral pathogens in haemophilia. Human parvovirus B19 infection, typically associated with anaemia or, rarely severe aplastic crisis, is a non-lipid enveloped virus, for which standard inactivation techniques are ineffective. Thus, nucleic acid testing (NAT) to screen the blood supply for B19 DNA is currently under consideration by the Food and Drug Administration. To the extent, viral inactivation, recombinant, and NAT technologies are available worldwide, and the lifespan for those with haemophilia is approaching that of the normal population. Abstract: Since 1998, the Aquitaine-Limousin branch of the French Blood Institute has set up a parvovirus B19 (PV B19) systematic screening on each unit of plasma to be treated by solvent–detergent procedure for virus inactivation. Parvovirus B19 nucleic acid systematic testing in plasma pools became mandatory since 2005 (European monograph “Human plasma” – pooled and treated for virus inactivation). The French competent state authority (AFSSAPS) has decided to introduce this test as a part of the external quality control of labile blood products. This process is related to the harmonization of quality control practice realised on blood products in Europe even if the human plasma pooled and treated for virus inactivation by solvent–detergent is considered in France as a blood labile component. Implementation of this test required a validation step and a close cooperation between AFSSAPS and Aquitaine-Limousin blood transfusion centre. Validation consisted in perfecting a semi-quantitative, real-time nucleic acid testing method with automated extraction. This collaborative study leads us to control 1642 plasma pools. All the results were under the threshold of 10,0 IU/μL. AFSSAPS''s results were in agreement with those of Aquitaine-Limousin''s blood transfusion center who carry out the parvovirus B19 screening both on fresh frozen plasma units composing the pool and on plasma pools. Abstract: The B19 Parvovirus (B19V) has for a long time been considered as the unique human virus belonging to the genus Erythrovirus. The genetic diversity of B19V isolates has been shown to be very low. The isolation of a variant (V9 strain), with a sequence markedly distinct from that of B19V which led to attributing this classification to this family of viruses. Phylogenetic analysis of sequences of V9-related isolates indicates an organization into three well-individualized genotypes. The B19V infection can be transmitted by transfusion. In immunocompetent recipients, B19V exposure by transfusion is most often inconsequential, since a large proportion is immunized. Such an infection may have serious clinical outcome in not immunized patients with shortened red cell survival, seronegative pregnant women and immunocompromised patients. In cellular products, viral DNA detection is not performed, but a preventive strategy could be discussed for at-risk recipients. Whereas in plasma derivatives, B19V screening is performed with a threshold of 104 IU/ml using molecular assays. With recent data of a new classification of three genotypes within human erythrovirus, nucleic acid testing assays would be validated in accordance with the genetic variability, in order to guarantee optimal safety. Recently, a new human parvovirus (PARV4) has been discovered. The consequences on blood transfusion of this blood-borne agent and its pathogenicity are still unknown. [Copyright &y& Elsevier]

Introduction. Parvovirus B19 (B19V) is the causative agent of the relatively benign childhood disease erythema infectiosum (fifth disease). Maternal B19V infection can lead to serious fetal complications during pregnancy. Up to 50% of women are nonimmune and susceptible to B19V infection. Methodology. We studied the literature and we search for the most frequent complications that were reported in the European Union regarding the parvovirus B19 infection. Results. Infection may result in anemia, spontaneous abortion and/or hydrops fetalis. The early diagnosis of B19V infection will identify those at risk and may allow for early intervention therapy, thereby improving fetal survival. The virus is spread via aerosol droplets through the respiratory route. It can be spread transplacentally to the fetus during active maternal infection (33% transmission rate across the placenta). Conclusions. Most pregnant women are asymptomatic but, for an appropriate patient management, there is a need for an accurate B19V diagnosis. [ABSTRACT FROM AUTHOR]

Abstract: B19 Parvovirus (B19V) has been considered for a long period of time as the unique human virus belonging to the genus Erythrovirus. The genetic diversity of B19V isolates has been shown to be very low (<2% nucleotide divergence). The isolation of a variant (V9 strain), with a sequence markedly distinct from that of B19V (>13% nucleotide divergence) led to specify the classification of this virus family. Phylogenetic analysis of partial sequences of V9-related isolates combined with Erythrovirus sequences in sequences banks indicates an organization into three well-individualized genotypes. Analysis of the nearly full-length genome sequences show an ancient separation between the three genotypes lineages. Genotype 3 (the most ancient lineage) could have originated in Africa. The functional regions of major proteins are conserved in the three genotypes. The frequency of these genotypes is various according to studies. Genotype 1 is predominant, except in Ghana where all the described isolates were genotype 3. A prospective French study performed between 1999 and 2001 indicated that genotypes 2 and 3 viruses circulated with a significant frequency (10%). Parvovirus B19 (B19) causes many clinical disorders, of which the most common are erythema infectiosum, aplastic crisis complicating chronic hemolytic anemia, and hydrops fetalis. In young adults, the skin eruption caused by B19 is accompanied with polyarthritis and polyarthralgia in 60% of the cases. The joint abnormalities predominate in the hands and feet and usually resolve within a week (range 2–21 d). Serological tests show IgM antibodies against B19, confirming the diagnosis of recent infection. Protracted polyarthritis occurs in some patients and seems associated with the DR4 histocompatibility alleles. Rheumatoid factors can be produced transiently in these patients. Other autoantibodies produced in the wake of B19 infection include anti-nuclear antibodies, anti-DNA, anti-SSA/SSB, and anti-phospholipids. Acute B19 infection can simulate early rheumatoid arthritis (RA) or systemic lupus erythematosus (SLE) (lupus-like eruption over the cheeks, cytopenia, etc.). In addition, there have been a few reports of erosive RA or SLE developing shortly after a B19 infection, with positive PCR tests for B19 DNA in synovial tissue or blood cells. Studies in large series indicate that B19 is probably an extremely rare cause of RA or SLE. Vasculitides affecting the small vessels (Henoch-Schonlein purpura, Wegener’s granulomatosis), medium-sized vessels (periarteritis nodosa), and large vessels (giant cell arteritis) can occur after B19 infection. Recent clinical observations support the hypothesis that persistent parvovirus B19 is a triggering factor of rheumatoid arthritis (RA) in certain genetically predisposed individuals. If this hypothesis is correct, a number of RA patients may exhibit parvovirus B19 DNA in their synovial membranes. We tested the synovial tissue and peripheral blood leukocytes of 20 patients with RA, 24 patients with other arthritides or osteoarthritis (non-RA), and 34 healthy blood donors for the presence of parvovirus B19 DNA using specific DNA amplification by polymerase chain reaction (PCR). Using this technique, parvovirus B19 DNA was demonstrated in the synovial biopsies of 75% of patients with RA but in those of only 16.7% of patients with non-RA. In autologous peripheral blood mononuclear cells the percentage of PCR-positive patients was about 15% in both RA and non-RA groups and did not differ from that in healthy controls. When the PCR data were correlated with the presence of anti-parvovirus B19 IgG antibodies in serum and synovia all patients with parvovirus B19 DNA in peripheral blood alone or in both peripheral blood and synovial membrane were seropositive. In contrast, about 40% of patients with parvovirus B19 DNA restricted to the synovial membrane were seronegative. The broad spectrum of clinical manifestations associated with B19 parvovirus often allows the infection to go unrecognized. We tested for the presence of B19 parvovirus and the specific serological response in serum from hospitalized patients submitted for viral investigations without any specific indications for B19 parvovirus. We diagnosed human parvovirus B19 infection in four paediatric patients showing different clinical manifestations. The patients, aged between 5 and 8 years, were admitted to hospital for: (1) petechial rash; (2) mononucleosis-like syndrome; (3) neurological syndrome; and (4) respiratory disease (in an immunodeficient patient). A spontaneous remission occurred after 3 years of regular transfusions in a cortico resistant Diamond Blackfan anaemia. However after 1 year of transfusion independence an apparent relapse was observed which was transient and could be attributed to a B19 parvovirus primary infection. The discovery of the human parvovirus B19 was done by Yvonne Cossart in 1975. In addition to fifth disease and aplastic crisis numerous clinical manifestations have been associated with B19 infections. Routine virological diagnosis is made by the detection of specific IgM and sometimes by the detection of viral DNA. Some clinicians are perplexed by the persistence of B19 DNAemia after acute infections. In this point of view we try to discuss this problematic.

A letter to the editor is presented in response to the article about a 32-year-old woman presenting with a bathing-trunk eruption associated with primary infection to human parvovirus B19 which was published in the previous issue.

The frequency of each genotype depends on geographic origin and population. Human erythroviruses infection can be transmitted by transfusion.
