Background: The 2022/23 influenza season in the United Kingdom saw the return of influenza to prepandemic levels following two seasons with low influenza activity. The early season was dominated by A(H3N2), with cocirculation of A(H1N1), reaching a peak late December 2022, while influenza B circulated at low levels during the latter part of the season. From September to March 2022/23, influenza vaccines were offered, free of charge, to all aged 2–13 (and 14–15 in Scotland and Wales), adults up to 49 years of age with clinical risk conditions and adults aged 50 and above across the mainland United Kingdom. Methods: End‐of‐season adjusted vaccine effectiveness (VE) estimates against sentinel primary‐care attendance for influenza‐like illness, where influenza infection was laboratory confirmed, were calculated using the test negative design, adjusting for potential confounders. Methods: Results In the mainland United Kingdom, end‐of‐season VE against all laboratory‐confirmed influenza for all those > 65 years of age, most of whom received adjuvanted quadrivalent vaccines, was 30% (95% CI: −6% to 54%). VE for those aged 18–64, who largely received cell‐based vaccines, was 47% (95% CI: 37%–56%). Overall VE for 2–17 year olds, predominantly receiving live attenuated vaccines, was 66% (95% CI: 53%–76%). Conclusion: The paper provides evidence of moderate influenza VE in 2022/23. Simple Summary: Existing cross-cultural research on animal welfare often overlooks the way that policy and law are not the exclusive domain of central government. This can result in an over-simplification or misrepresentation of the true situation. The political culture and institutional arrangements for governing the modern state are more complex than a "one-size-fits-all" approach. It is argued that cross-cultural research needs to give greater attention to differences within as well as between unitary states. Specifically, it needs to examine developments in constituent nations and territories. Here we illustrate this by drawing on new research in the United Kingdom, and examine how 'devolution'—or decentralized government for Wales and Scotland—is providing contrasting opportunities for NGOs, campaigners, and the public to lobby to improve animal welfare policy based on local practices, beliefs, and demands (collectively known as the "political culture"). Our findings show how this is important because it results in contrasting animal welfare policies and laws in the constituent nations of the UK. It is argued that extant cross-cultural research on animal welfare often overlooks or gives insufficient attention to new governance theory, civil society, politics, and the realities of devolved or (quasi-)federal, multi-level governance in the modern state. This paper synthesizes relevant social theory and draws on new empirical findings of civil society accounts of campaigning on animal welfare policies and law in the United Kingdom. It is presented as a corrective to arguably reductive, earlier unitary state-based analyses. Our core, evidence-based argument is that cognizance of civil society activism and the contrasting institutional governance structures and political cultures of constituent nations in unitary states—such as the UK—are providing opportunities for the territorialization of legally grounded animal welfare regimes, and culturally distinctive practices. British supermarket-panel data suggest no increases in overall sales and purchases of alcohol following COVID-19 lockdowns, yet survey and mortality data suggest otherwise. This paper attempts to unravel the paradox. Based on purchase data of 79,417 British households from Kantar Worldpanel, we undertake controlled interrupted time series analysis of the impact of COVID-19 confinement introduced on 23rd March 2020, and variably applied during 2020, compared to purchases during 2015 to 2019 as controls. We also undertook Poisson regression analyses to estimate if changes in purchases differed by household socio-demographic and economic factors. Excess off-trade household alcohol purchases (expressed as grams of ethanol) following the introduction of confinement, were 29.2% higher (95% CI = 25.8% to 32.5%) for the post-confinement months of 2020, being larger until mid-July 2020 (37.5%, 95%CI = 33.9 to 41.26%) when pubs re-opened with restrictions, and smaller (24.6%, 95%CI = 21.6 to 27.7) thereafter. During the time of complete pub closures, and fully adjusting for no on-trade purchases, household purchases of alcohol did not change when compared with the same time period during 2015–2019 (coefficient = -0.9%, 95%CI = -5.6 to 3.8). Excess purchases from 23rd March to 31st December 2020 varied by region of Great Britain, being higher in the north of England, and lower in Scotland and Wales. Excess purchases were greater in the most deprived households, compared with the least deprived households. Excess purchases increased substantially as the amount of alcohol normally purchased by a household increased, with the top one fifth of households that normally bought the most alcohol increasing their purchases more than 17 times than the bottom one fifth of households that bought the least alcohol. That the heaviest buyers of alcohol increased their purchases the most, with some independent impact of socio-economic disadvantage, might explain why reported alcohol problems and recent alcohol-related death rates might have increased. A conclusion of this is that alcohol policy to reduce high consumption of alcohol, and the availability of help and treatment to reduce alcohol consumption become more important during extraordinary times, such as COVID lockdowns. Deemed consent for organ donation has long been discussed as a potential solution to the shortage of organs for transplantation, with several countries having implemented it. In Great Britain, Wales was the first nation to introduce such a system, having done so in 2015. Now, the other two nations are following suit. In this paper, I compare the approaches of England and Scotland in moving to systems of deemed consent for organ donation. After outlining both sets of legislation, I focus on three points on which the two nations differ. First, the role of those close to the deceased in the consent process and the extent to which clinicians are required to consult them ahead of consent being deemed. Second, the role of government ministers in ensuring widespread public awareness. Third, the ways in which the two nations responded to the challenge of the COVID-19 pandemic in relation to the implementation of deemed consent. A geodemographic classification aims to describe the most salient characteristics of a small area zonal geography. However, such representations are influenced by the methodological choices made during their construction. Of particular debate are the choice and specification of input variables, with the objective of identifying inputs that add value but also aim for model parsimony. Within this context, our paper introduces a principal component analysis (PCA)-based automated variable selection methodology that has the objective of identifying candidate inputs to a geodemographic classification from a collection of variables. The proposed methodology is exemplified in the context of variables from the UK 2011 Census, and its output compared to the Office for National Statistics 2011 Output Area Classification (2011 OAC). Through the implementation of the proposed methodology, the quality of the cluster assignment was improved relative to 2011 OAC, manifested by a lower total within-cluster sum of square score. Across the UK, more than 70.2% of the Output Areas (OAs) occupied by the newly created classification (i.e. AVS-OAC) outperform the 2011 OAC, with particularly strong performance within Scotland and Wales. Debates surrounding the ostensibly ‘transformative’ potential of personalisation for social work services, and service users, have variously illustrated the risks and opportunities this presents, although the implications for criminal justice social work services have received comparatively limited attention. By extending the concept of ‘service user’ to include not only offenders, but wider stakeholders (victims and communities), this paper considers the practical application of theories of personalisation and co-production by reviewing proposed and extant strategies for maximising stakeholder involvement in criminal justice services. It is argued that, in progressing beyond the more individualistic interpretations of this somewhat controversial reform agenda—in prioritising not only the individual, their rights, strengths and subjective identities, but locating the individual in situ, in the concrete realities and textures of their lives and communities—the strength of the personalisation agenda rests in its potential to develop and strengthen the collective organisation of service users, service providers and communities in a co-productive endeavour. It is argued that both this reading and the principles underpinning it resonate more widely with the empirical and theoretical literature on just and effective penal practices and, in so doing, this paper exposes the complexities that lie behind the apparent simplicity of this argument. Background Secondhand smoke (SHS) exposure is higher among lower socioeconomic status (SES) children. Legislation restricting smoking in public places has been associated with reduced childhood SHS exposure and increased smoke-free homes. This paper examines socioeconomic patterning in these changes. Methods Repeated cross-sectional survey of 10 867 schoolchildren in 304 primary schools in Scotland, Wales and Northern Ireland. Children provided saliva for cotinine assay, completing questionnaires before and 12 months after legislation. Results SHS exposure was highest, and private smoking restrictions least frequently reported, among lower SES children. Proportions of saliva samples containing <0.1 ng/ml (i.e. undetectable) cotinine increased from 31.0 to 41.0%. Although across the whole SES spectrum, there was no evidence of displacement of smoking into the home or increased SHS exposure, socioeconomic inequality in the likelihood of samples containing detectable levels of cotinine increased. Among children from the poorest families, 96.9% of post-legislation samples contained detectable cotinine, compared with 38.2% among the most affluent. Socioeconomic gradients at higher exposure levels remained unchanged. Among children from the poorest families, one in three samples contained >3 ng/ml cotinine. Smoking restrictions in homes and cars increased, although socioeconomic patterning remained. Conclusions Urgent action is needed to reduce inequalities in SHS exposure. Do regional boundaries defined by governments respect the more natural ways that people interact across space? This paper proposes a novel, fine-grained approach to regional delineation, based on analyzing networks of billions of individual human transactions. Given a geographical area and some measure of the strength of links between its inhabitants, we show how to partition the area into smaller, non-overlapping regions while minimizing the disruption to each person's links. We tested our method on the largest non-Internet human network, inferred from a large telecommunications database in Great Britain. Our partitioning algorithm yields geographically cohesive regions that correspond remarkably well with administrative regions, while unveiling unexpected spatial structures that had previously only been hypothesized in the literature. We also quantify the effects of partitioning, showing for instance that the effects of a possible secession of Wales from Great Britain would be twice as disruptive for the human network than that of Scotland. This study presents a comprehensive review of building materials, construction methods, and building regulations on the U.K. mainland. This provides valuable insights into the historical progression and transformation of the construction industry through a comprehensive analysis of both traditional and modern building construction materials and methods and categorising their evolutionary trajectory. Current building regulations in England, Wales, and Scotland are compared, highlighting differences in fire safety, noise safety, energy conservation, and sustainability. For example, fire safety regulations are analysed in detail, including fire resistance duration, wall cladding combustibility, and limitations on unprotected areas. Advances in knowledge and technology have led to increasingly sophisticated and energy-dependent methods, materials, and regulations. This study showcases the vast array of building construction materials spanning centuries, each possessing unique properties and performances. The selected methods and materials represent those currently employed or widely utilised in the U.K. construction industry, affirming their relevance and applicability in modern construction practices. Limitations in construction practices primarily stem from a lack of knowledge and tools rather than material scarcity. Enhancing knowledge and access to advanced tools is crucial to overcoming these limitations and driving advancements in the field. This study provides insights into the evolution of building materials, construction methods, and building regulations that can inform future developments in sustainable building practices. The findings have significant implications for policymakers, building designers, and constructors, particularly in terms of adopting sustainable materials and construction methods that comply with building regulations while reducing the environmental impact of the built environment. Background: One of the many challenges faced by mental health services during the COVID-19 pandemic was how to deliver care during lockdown. In community and crisis services, this often meant rapidly adopting or expanding the use of telemental health technologies, including phone and video calls. The aim of this study is to explore variations in use and report staff views of such technologies during the early stages of the pandemic. The primary analysis compared rates of use between professions, demographic groups, genders, regions, and crisis and community services. Methods: We used data from an online survey conducted by the Mental Health Policy Research Unit in Spring 2020 regarding the impact of the pandemic on mental healthcare in the United Kingdom. We included quantitative data from all professional groups working in community or crisis services providing care to working age adults, including general and specialist services. Our outcome of interest was the percentage of clients whom clinicians primarily interacted with via videocall. We also collected demographics and professional characteristics such as the type of mental health service respondents worked in. In addition, we explored respondents' views and experiences of telemental health as a medium for providing care. Results: 978 participants were included in the primary analysis (834 provided outcome data for community services, 193 for crisis services). In community services, virtually all staff reported stopping some or all face-to-face appointments following the onset of the pandemic, with a large majority using video or phone call appointments where possible instead. Telemental health use was higher in community than in crisis services, and amongst professionals who mainly provided psychotherapy or peer support than in other groups. There was also evidence of use being lower in regions in Northern England, Scotland, and Wales than elsewhere. There was no evidence of an association with staff gender, age, or ethnicity. Staff were generally positive about telemental health and intended to make more use of technologies following the pandemic. However, significant barriers to its use were also reported, often involving skills and available infrastructure. Conclusions: Despite its rapid implementation, telemental health was viewed positively by clinicians who saw it as an effective alternative to face-to-face appointments in some contexts, including during the pandemic. The British forestry sector lacks reliable dynamic growth models for stands of improved Sitka spruce, the most important commercial forest type in Great Britain. The aim of this study is to fill this gap by trialling a new modelling framework and to lay the foundations of a future dynamic growth simulator for that forest type. First, we present single tree diameter and height increment models that are climate sensitive and include explicit competition effects. The predictions from the increment models are pooled to project diameter and height at a given age. These projections are then used as inputs to an integrated taper model from which stochastic tree volume predictions are obtained. Retrospective data from over 1400 trees collected in two extensive genetic trials in Scotland and Wales were used for the purposes of this study. Diameter increment and height increment predictions were highly accurate and diameter and height projections proved consistent. The predicted volume at the time of harvesting also exhibited a high degree of accuracy, which shows the robustness of our approach. Further data will be needed in the future to recalibrate the present models and extend their range of validity to the whole of Great Britain. [ABSTRACT FROM AUTHOR]

Background: Access to high-quality palliative care is inadequate for most people living and dying with serious illness. Policies aimed at optimising delivery of palliative and end of life care are an important mechanism to improve quality of care for the dying. The extent to which palliative care is included in national health policies is unknown. We aimed to identify priorities and opportunities for palliative and end of life care in national health policies in the UK. Methods: Documentary analysis consisting of 1) summative content analysis to describe the extent to which palliative and end of life care is referred to and/or prioritised in national health and social care policies, and 2) thematic analysis to explore health policy priorities that are opportunities to widen access to palliative and end of life care for people with serious illness. Relevant national policy documents were identified through web searches of key government and other organisations, and through expert consultation. Documents included were UK-wide or devolved (i.e. England, Scotland, Northern Ireland, Wales), health and social care government strategies published from 2010 onwards. Results: Fifteen policy documents were included in the final analysis. Twelve referred to palliative or end of life care, but details about what should improve, or mechanisms to achieve this, were sparse. Policy priorities that are opportunities to widen palliative and end of life care access comprised three inter-related themes: (1) integrated care – conceptualised as reorganisation of services as a way to enable improvement; (2) personalised care – conceptualised as allowing people to shape and manage their own care; and (3) support for unpaid carers – conceptualised as enabling unpaid carers to live a more independent lifestyle and balance caring with their own needs. Conclusions: Although information on palliative and end of life care in UK health and social care policies was sparse, improving palliative care may provide an evidence-based approach to achieve the stated policy priorities of integrated care, personalised care, and support for unpaid carers. Aligning existing evidence of the benefits of palliative care with the three priorities identified may be an effective mechanism to both strengthen policy and improve care for people who are dying. The decarbonisation of heating in the United Kingdom is likely to entail both the mass adoption of heat pumps and widespread development of district heating infrastructure. Estimation of the spatially disaggregated heat demand is needed for both electrical distribution network with electrified heating and for the development of district heating. The temporal variation of heat demand is important when considering the operation of district heating, thermal energy storage and electrical grid storage. The difference between the national and urban heat demands profiles will vary due to the type and occupancy of buildings leading to temporal variations which have not been widely surveyed. This paper develops a high-resolution spatiotemporal heat load model for Great Britain (GB: England, Scotland a Wales) by identifying the appropriate datasets, archetype segmentation and characterisation for the domestic and nondomestic building stock. This is applied to a thermal model and calibrated on the local scale using gas consumption statistics. The annual GB heat demand was in close agreement with other estimates and the peak demand was 219 GWth. The urban heat demand was found to have a lower peak to trough ratio than the average national demand profile. Background: The Obstetric Anal Sphincter Injury (OASI) Care Bundle comprises four primary and secondary prevention practices that target the rising rates of severe perineal tearing during childbirth, which can have severe debilitating consequences for women. The OASI Care Bundle was implemented in 16 maternity units in Britain in the OASI1 project (2017-2018), which demonstrated the care bundle's effectiveness in reducing OASI rates. In OASI2, the care bundle will be scaled up to 20 additional National Health Service (NHS) maternity units in a hybrid effectiveness-implementation study that will examine the effectiveness of strategies used to introduce, implement and sustain the care bundle.Methods: OASI2 is a two-arm cluster-randomised control trial (C-RCT) of maternity units in England, Scotland and Wales, with an additional non-randomised study arm. C-RCT arm 1 (peer support, n = 10 units) will be supported by 'buddy' units to implement the OASI Care Bundle. C-RCT arm 2 (lean implementation, n = 10 units) will implement without external support. The additional study arm (sustainability, n = 10 units) will include some original OASI1 units to evaluate the care bundle's sustainability and OASI rates over time, from before OASI1 and through the end of OASI2. Units in all three study arms will receive an Implementation Toolkit with training resources and implementation support. The C-RCT arms will be compared in terms of OASI rate reduction (primary effectiveness outcome) and clinicians' adoption of the care bundle (primary implementation outcome). Clinical data will be collated from maternity information systems; implementation data will be collected through validated surveys with women and clinicians, supplemented by qualitative methods. Descriptive statistics and regression modelling will be used for analysis. Emergent themes from the qualitative data will be assessed using framework analysis.Discussion: OASI2 will study the impact of various implementation strategies used to introduce and sustain the OASI Care Bundle, and how these strategies affect the bundle's clinical effectiveness. The study will generate insights into how to effectively scale-up and sustain uptake and coverage of similar interventions in maternity units. The article presents a response to a paper by Mok et al. on the suicide rate for men and women in Wales, Scotland and England. The suicide rate in Scotland and Northern Ireland is higher than those for Wales. The paper also shows that there is a high incidence of frowning in Scotland, which is attributed to the presence of 90% standing freshwater in all of Great Britain.

Background: Stratifying patient populations by risk of adverse events was believed to support preventive care for those identified, but recent evidence does not support this. Emergency admission risk stratification (EARS) tools have been widely promoted in UK policy and GP contracts.Aim: To describe availability and use of EARS tools across the UK, and identify factors perceived to influence implementation.Design and Setting: Cross-sectional survey in UK.Method: Online survey of 235 organisations responsible for UK primary care: 209 clinical commissioning groups (CCGs) in England; 14 health boards in Scotland; seven health boards in Wales; and five local commissioning groups (LCGs) in Northern Ireland. Analysis results are presented using descriptive statistics for closed questions and by theme for open questions.Results: Responses were analysed from 171 (72.8%) organisations, of which 148 (86.5%) reported that risk tools were available in their areas. Organisations identified 39 different EARS tools in use. Promotion by NHS commissioners, involvement of clinical leaders, and engagement of practice managers were identified as the most important factors in encouraging use of tools by general practices. High staff workloads and information governance were identified as important barriers. Tools were most frequently used to identify individual patients, but also for service planning. Nearly 40% of areas using EARS tools reported introducing or realigning services as a result, but relatively few reported use for service evaluation.Conclusion: EARS tools are widely available across the UK, although there is variation by region. Introduction: The most abundant and widespread tick species in Great Britain, Ixodes ricinus, is responsible for the transmission of a range of pathogens that cause disease in livestock. Empirical data on tick distribution and prevalence are required to inform farm management strategies. However, such data are largely unavailable; previous surveys have been rare and are usually relatively localised. Methods: A retrospective questionnaire survey of farmers was used to assess the reported prevalence of ticks on livestock across Great Britain. Spatial scan statistics and kernel density maps were used to assess spatial clustering and identify areas of significantly elevated risk, independent of the underlying distribution of respondents. Logistic regression models were used to identify risk factors for tick presence. Results: Tick infection risk to livestock is shown to be spatially aggregated, with areas of significantly elevated risk in north Wales, northwest England and western Scotland. Overall, the prevalence of farms reporting tick presence was 13% for sheep farms and 6% for cattle farms, but in "hot spot" clusters prevalence ranged between 48–100%. The prevalence of farms reporting tick-borne disease overall was 6% for sheep and 2% for cattle, but on farms reporting ticks, prevalence was 44% and 33% for sheep and cattle farms, respectively. Upland farming, larger flock sizes, region and the presence of sheep on cattle farms were all significant risk factors for tick presence. Conclusions: These data have important implications for assessing both the risk of tick-borne disease in livestock and optimising approaches to disease management. In particular, the study highlights the need for effective livestock tick control in upland regions and the southwest, and provides evidence for the importance of sheep as tick maintenance hosts. Background Giving children the best start in life is critical for their future health and wellbeing. Political devolution in the UK provides a natural experiment to explore how public health systems contribute to children's early developmental outcomes across four countries. Method A systematic literature review and input from a stakeholder group was used to develop a public health systems framework. This framework then informed analysis of public health policy approaches to early child development. Results A total of 118 studies met the inclusion criteria. All national policies championed a 'prevention approach' to early child development. Political factors shaped divergence, with variation in national conceptualizations of child development ('preparing for life' versus 'preparing for school') and pre-school provision ('universal entitlement' or 'earned benefit'). Poverty and resourcing were identified as key system factors that influenced outcomes. Scotland and Wales have enacted distinctive legislation focusing on wider determinants. However, this is limited by the extent of devolved powers. Conclusion The systems framework clarifies policy complexity relating to early child development. The divergence of child development policies in the four countries and, particularly, the explicit recognition in Scottish and Welsh policy of wider determinants, creates scope for this topic to be a tracer area to compare UK public health systems longer term. Devolution has increased the divergence in law and policy, and this has impacted on the social work services as part of the welfare state. It has led to debate about the changing nature of social citizenship within the UK. Many adults who come to the attention of social work services already have conditional citizenship by virtue of poverty, environment, illness and disability. This article demonstrates how this can be further compounded by the welfare agencies themselves by comparing the mental health law frameworks for Scotland and England. It argues that differences in participation ultimately led to the different civil and social rights being enshrined in law. However, law is just one ingredient that defines citizenship; the political ethos and the public service culture are two other key ingredients. This article will demonstrate that these have also been significant in creating divergence. Finally, the article will explore whether this social citizenship model for mental health has a wider relevance for all people who use social work services. [ABSTRACT FROM PUBLISHER]

Background: Completeness of ascertainment is a very important aspect of cancer registration. There is no recent published estimate for childhood cancer in Britain.Methods: We estimated completeness of ascertainment by the National Registry of Childhood Tumours for cancer diagnosed under age 15 years in residents of Britain during 2003-04. Stratified two-source capture-recapture was applied to notifications from general cancer registries (CRs) and specialist clinicians. Variation in notification patterns was assessed by logistic regression. Results were verified by cross-checking with Hospital Episode Statistics for leukaemia patients from England born in 1998 and diagnosed before 2005.Results: CRs notified 92-96% of registrations, and specialist clinicians 93%. Notification patterns varied slightly according to registry region, age at diagnosis, diagnostic group, socioeconomic status, and whether the patient had died. Irrespective of stratification by these factors, the overall completeness estimate was 99-100% (assuming independence of sources). Estimated completeness was at least 99% within all subgroups, except for one region (Thames 98-99%) and two small diagnostic groups (germ-cell and gonadal cancer 98-99%, melanoma and non-skin cancer 97-98%).Interpretation: The independence assumption cannot be fully justified, as both sources used records from treatment centres. This article explores judicial perceptions of child witnesses. It considers the impact of recent legislation in England and Wales as well as in Scotland which classified all child witnesses as vulnerable and introduced a series of special measures to facilitate children's evidence. The article reports the findings of an empirical research study conducted with the judiciary in Scotland which suggests that judicial perceptions of child witnesses extend across a complex spectrum where a child may be viewed as vulnerable but is also likely to be seen as suggestible, reliable or resilient. The article advances two propositions. First, that the statutory conceptualization of children as invariably vulnerable has not displaced established beliefs concerning children's suggestibility and therefore has made little difference to perceptions of their ability to produce reliable testimony. Second, that focusing on children's potential for resilience rather than their vulnerability may prove a more productive conceptualization of children, one which could better support their capability as witnesses. The article reassesses the importance of local factors in shaping policy for Scotland and Wales, and explores their subsequent incorporation into the Conservative Party's wider discourse of decentralisation. It is noted that the party's new policies for Scotland and Wales formed part of a distinctive strategic response to the new political world that opened up at home and abroad after I945. The nationalist pressure on rank and file Conservatives in Wales and Scotland was an important stimulus to the creation of policy, but the party's leaders championed the proposals as part of a genuine alternative to Labour Party's model of the centralised State.

The devolution of legislative and executive powers to the different parts of the United Kingdom has meant that, because of the asymmetric arrangements made, there has been an increase in policy divergence from one part to another. Some of this has been intended, some unintended. With reference, in particular, to Scotland and Wales this article focuses on the asymmetric consequences of devolution which derive specifically from the jurisdictional or ‘legal system’ differences across the United Kingdom. These affect the conditions under which law-making powers may be devolved, the ‘management’ of devolution and the rights enjoyed by citizens. Background: Health policy in the UK has rapidly diverged since devolution in 1999. However, there is relatively little comparative data available to examine the impact of this natural experiment in the four UK countries. The Quality and Outcomes Framework of the 2004 General Medical Services Contract provides a new and potentially rich source of comparable clinical quality data through which we compare quality of primary medical care for coronary heart disease (CHD), stroke, hypertension and diabetes across the four UK countries.Methods: A cross-sectional analysis was undertaken involving 10,064 general practices in England, Scotland, Wales and Northern Ireland. The main outcome measures were prevalence rates for CHD, stroke, hypertension and diabetes. Achievement on 14 simple process, 3 complex process, 9 intermediate outcome and 5 treatment indicators for the four clinical areas.Results: Prevalence varies by up to 28% between the four UK countries, which is not reflected in resource distribution between countries, and penalises practices in the high prevalence countries (Wales and Scotland). Differences in simple process measures across countries are small. Larger differences are found for complex process, intermediate outcome and treatment measures, most notably for Wales, which has consistently lower quality of care. Scotland has generally higher quality than England and Northern Ireland is most consistently the highest quality.Conclusion: Previously identified weaknesses in Wales related to waiting times appear to reflect a more general quality problem within NHS Wales. Identifying explanations for the observed differences is limited by the lack of comparable data on practice resources and organisation. Maximising the value of cross-jurisdictional comparisons of the ongoing natural experiment of health policy divergence within the UK requires more detailed examination of resource and organisational differences. In recent years, UK public health messages about the risks of sunlight exposure (skin cancer) have been increasingly balanced by messages about its benefits (vitamin D production). Currently, data about the effects of this shift on public knowledge, awareness, and behaviour are scant. Thus, the objective of this paper is to report the findings of the first large-scale and representative survey of the awareness, knowledge, and behaviour of adults in Great Britain (England, Scotland, and Wales) ( n = 2024) with respect to sunlight exposure, vitamin D, and sunburn and skin cancer. The findings suggest that the public in Great Britain is much more aware of public promotion of the risks of sunlight exposure than its benefits. That said, knowledge about sunlight exposure and vitamin D is fairly strong, though not with respect to the detail of the 'little and often' approach. However, the survey also suggests that levels of sunlight exposure among the public are often excessive. The survey indicates that knowledge and behaviour are both less satisfactory among men and people in lower socio-economic groups. The paper concludes with recommendations for public health communications and for research in this area.

The threat to public health in the United Kingdom of infections with Vero cytotoxin producing Escherichia coli (VTEC), and E. coli O157 in particular, has led the PHLS to develop guidance for control. Published and unpublished data have been reviewed by a subcommittee of the PHLS working group on VTEC. This paper summarises the review and presents interim guidelines for the control of VTEC infections.
