Inuit face worse cancer survival rates and outcomes than the general Canadian population. Persistent health disparities cannot be understood without examining the structural factors that create inequities and continue to impact the health and well-being of Inuit. This scoping review aims to synthesise the available published and grey literature on the structural factors that influence cancer care experienced by Inuit in Canada. Guided by Inuit input from Pauktuutit Inuit Women of Canada as well as the Joanna Briggs Institute scoping review methodology, a comprehensive electronic search along with hand-searching of grey literature and relevant journals was conducted. A total of 30 papers were included for analysis and assessment of relevance. Findings were organised into five categories as defined in the a priori framework related to colonisation, as well as health systems, social, economic, and political structures. The study results highlight interconnections between racism and colonialism, the lack of health service information on urban Inuit, as well as the need for system-wide efforts to address the structural barriers in cancer care. Historically, there is a documented lack of support for women leaving their communities to give birth. Due to an over-emphasis on risk discourse (no distinction between low- or high-risk pregnancies with options to birth at home, the large geographical distances to regional centres within circumpolar regions, remoteness of communities, lack of qualified staff, and limited resources), women are forced to travel great distances to give birth. In Canada until 2017, women travelled alone or bore the burden of paying out of pocket for someone to travel with them for support with the births of their babies. A recent policy change has allowed for one paid escort to accompany a pregnant woman. The purpose of this paper is to provide an outcome assessment of the perinatal transport and non-medical escort policy implemented by the federal government in Canada. The paper utilises a narrative literature review along with preliminary Indigenous photovoice results to illuminate the perspectives of Indigenous families. There are numerous challenges women face when travelling for birth including feelings of loneliness and fear, stress and separation from their children, lack of community connection, financial concerns, and a loss of self-determination. Women are given an opportunity in decision-making about who will travel with them as an escort. This does little to address the disparity of delivering their babies away from their families and communities, the burden of figuring out who cares for the children at home, and the impact their absences have on health of their families. Although, additional support has been provided for birthing women in northern Canada, there continues to be little effort to return to community birthing creating safety for Indigenous families through traditional practices, Indigenous midwifery, and community ceremonies and changing the disproportionate burden of poor outcomes experienced by Indigenous women. Background: Parents seek support on behalf of their transgender and gender nonconforming (TGNC) children. Previous qualitative studies explore the types of support parents seek inside and outside of healthcare settings. Healthcare providers often remain unprepared to effectively provide gender-affirming services to TGNC children and their accompanying parents and may benefit from learning about the support seeking experiences of parents with TGNC children. Aims: This paper summarized qualitative research studies that address the topic of parents seeking support on behalf of their TGNC children. We produced this report for healthcare providers to review to enhance gender-affirming services for parents and TGNC children. Methods: This paper outlines a qualitative metasummary of studies from the United States or Canada with data collected from parents of TGNC children. Data collection included the steps of journal runs, database searches, reference checks, and area scans. Data analysis involved the steps of extracting, editing, grouping, abstracting, and calculating the intensity and frequency effect sizes for finding statements from qualitative research study articles. Results: The results of this metasummary yielded two primary themes, six subthemes, and 24 total findings. The first primary theme of seeking guidance had three subthemes: educational resources, community networks, and advocacy efforts. The second primary theme of seeking healthcare had three subthemes: healthcare providers, mental healthcare, and general healthcare. Discussion: These findings provide information healthcare providers can use to inform their practice. These findings also highlight the importance of providers working collaboratively with parents when serving TGNC children. This article concludes with practical tips for providers. The present article explores the structure of and recent developments in research activities in the polar regions. Based on a bibliographic study of published papers indexed in the ISI Web of Science during the period 1981-2007, we have analyzed trends in publication, scientific disciplines and subdisciplines, coauthorship, and international collaboration within the field of polar research. We have uncovered several rather striking trends. Scientific output in terms of refereed publications has increased far more rapidly in polar research compared to science in general, quadrupling rather than doubling over the surveyed period. There is a nearly 1:1 ratio between papers covering the Arctic relative to the Antarctic, with the vast majority within either the geosciences (40%) or biology (33%). There has been particularly a steep rise in the number of climate-related papers. The U.S.A. is by far the largest contributor to polar research on both the Arctic and the Antarctic, followed by Canada, the U.K., Germany, Norway, and Russia. The number of coauthored papers has grown markedly, reflecting geopolitical shifts and changing national and international funding priorities during the period. We believe our publication-based survey reveals interesting developments in scientific activities and international cooperation in general, and in polar science strategies and priorities in particular. Objectives. To examine if Aboriginal health research conducted within the field of social sciences reflects the population and geographic diversity of the Aboriginal population. Study Design. Review. Methods. We searched the Web of Science Social Science Citation Index, the Arts and Humanities Citation Index and Scholars Portal for the time period 1995-2005 using search terms to reflect different names used to refer to Canada's Aboriginal peoples. Citations that did not focus on health or Canada were eliminated. Each paper was coded according to 7 broad categories: Aboriginal identity group; geography; age; health status; health determinants; health services; and methods. Results. Based on the 96 papers reviewed, the results show an under-representation of Métis and urban Aboriginal peoples. Most of the papers are on health status and non-medical determinants of health, with a particular focus on chronic conditions and life-style behaviours. Only 6 papers examined traditional approaches to healing and/or access to traditional healers/medicines. A small number involved the use of community-based research methods. Conclusions. Further research is required to address gaps in the current body of literature. Community-based research studies are necessary to address gaps that are most relevant to Aboriginal peoples. Purpose: This research examines the implementation of the Icelandic Prevention Model (IPM) in Canada to identify opportunities revealed by the COVID-19 pandemic to re-design our social eco-system to promote wellbeing. This paper has two objectives: 1) to provide a conceptual review of research that applies the bioecological model to youth substance use prevention with a focus on the concepts of time and physical space use and 2) to describe a case study that examines the implementation of the IPM in Canada within the context of the COVID-19 pandemic. Method: Study data were collected through semistructured qualitative interviews with key stakeholders involved in implementing the IPM. Results: Findings are organized within three over-arching themes derived from a thematic analysis: 1) Issues that influence time and space use patterns and youth substance use, 2) Family and community cohesion and influences on developmental context and time use and 3) Opportunities presented by the pandemic that can promote youth wellbeing. Conclusion: We apply the findings to research on the IPM as well as the pandemic to examine opportunities that may support primary prevention and overall youth wellbeing. Many Indigenous communities in Canada experience endemic tuberculosis with superimposed periodic epidemic outbreaks. Failures in outbreak management have resulted in the "seeding" of future infection and disease. In this paper we present a model that may be used in planning, implementation and review of tuberculosis outbreak management in Cree Indigenous communities in Canada, based on the Medicine Wheel, a paradigm for holistic living. In the context of tuberculosis management, the Medicine Wheel provides a path for the establishment of respectful cross-cultural relationships, the expression of values through action, true community engagement and partnership, and the establishment of culture-based processes of transparency, accountability and change. Inuit Canadians are on average about 20 years younger and have a 10-year lower life expectancy than other Canadians. While there have been improvements in Inuit health status over time, significant health disparities still remain. This paper will review the peer-reviewed literature related to Inuit child, youth, and maternal health between 2000 and 2010, investigate which thematic areas were examined, and determine what proportion of the research is related to each group. Establishing areas of research concentrations and scarcities may help direct future research where it is needed. We followed a systematic literature review and employed peer-reviewed research literature on child, youth, and maternal health which were selected from 3 sources, MEDLINE, CINAHL, and the Circumpolar Health Bibliographic Database. The resulting references were read, and summarized according to population group and thematic area. The thematic areas that emerged by frequency were: infectious disease; environment/environmental exposures; nutrition; birth outcomes; tobacco; chronic disease; health care; policy, human resources; interventions/ programming; social determinants of health; mental health and wellbeing; genetics; injury; and dental health. The 72 papers that met the inclusion criteria were not mutually exclusive with respect to group studied. Fiftynine papers (82%) concerned child health, 24 papers (33%) youth health, and 58 papers (81%) maternal health. The review documented high incidences of illness and significant public health problems; however, in the context of these issues, opportunities to develop research that could directly enhance health outcomes are explored. Background: In 2019, more than $34.5 billion was spent on prescription drugs in Canada. However, little is known about the distribution of this spending across medications and settings (outpatient and inpatient) over time. The objective of this paper is to describe the largest expenditures by medication class over time in inpatient and outpatient settings. This information can help to guide policies to control prescription medication expenditures. Methods: IQVIA's Canadian Drugstore and Hospital Purchases Audit data from January 1, 2001, to December 31, 2020, were used. In this dataset, purchasing was stratified by outpatient drugstore and inpatient hospital. Spending trajectories in both settings were compared to total expenditure over time. Total expenditure of the 25 medications with the largest expenditure were compared over time, stratified by setting. Nominal costs were used for all analysis. Results: In 2001, spending in the outpatient and inpatient settings was greatest on atorvastatin ($467.0 million) and erythropoietin alpha ($91.2 million), respectively. In 2020, spending was greatest on infliximab at $1.2 billion (outpatient) and pembrolizumab at $361.6 million (inpatient). Annual outpatient spending, although increasing, has been growing at a slower rate (5.3%) than inpatient spending (7.0%). In both settings, spending for the top 25 medications has become increasingly concentrated on biologic agents, with a reduction in the diversity of therapeutic classes of agents over time. Discussion: Identification of the concentration on spending on biologic agents is a key step in managing costs of prescription medications in Canada. Given the increases in spending on biologic agents over the last 20 years, current cost-control mechanisms may be insufficient. Future research efforts should focus on examining the effectiveness of current cost-control mechanisms and identifying new approaches to cost control for biologic agents. [ABSTRACT FROM AUTHOR]

This paper examines educational policies toward indigenous minorities in Japan and Canada during the period of nation-building, from the latter half of the nineteenth century to the first half of the twentieth century. Both Japan and Canada first segregated indigenous children into separate educational institutions and then tried to assimilate them into mainstream society. Beneath these broad policy similarities, however, lie different rationales, with substantially different implications for education and social policy in diverse societies. In Japan, national integration was promoted through a cultural or ethnic rationale, a socially coherent approach that nonetheless allows little room for minorities. Canada approached national integration using a notion of citizenship that both allows considerable space for minorities but is challenged by unity. These two strategies can be seen in two polar models of the state - a civic-assimilationist approach of the 'French model' and an ethnocultural exclusionist model of the formation of the German state. Etuaptmumk or Two-Eyed Seeing (E/TES) is foundational in ensuring that Indigenous ways of knowing are respected, honoured, and acknowledged in health research practices with Indigenous Peoples of Canada. This paper will outline new knowledge gleaned from the Canadian Institute of Health Research and Chronic Pain Network funded Aboriginal Children's Hurt & Healing (ACHH) Initiative that embraces E/TES for respectful research. We share the ACHH exemplar to show how Indigenous community partners take the lead to address their health priorities by integrating cultural values of kinship and interconnectedness as essential components to enhance the process of community-led research. E/TES is conceptualised into eight essential considerations to know in conducting Indigenous health research shared from a L'nuwey (Mi'kmaw) perspective. L'nu knowledge underscores the importance of working from an Indigenous perspective or specifically from a L'nuwey perspective. L'nuwey perspectives are a strength of E/TES. The ACHH Initiative grew from one community and evolved into collective community knowledge about pain perspectives and the process of understanding community-led practices, health perspectives, and research protocols that can only be understood through the Two-Eyed Seeing approach. Background: Few studies have been published about specialty-specific fourth-year medical student leadership in any discipline. This paper provides insight from pediatric educators about the current status and recommendations for pediatric-specific fourth-year leaders. Objective: To identify the prevalence of pediatric fourth-year medical student directors across the US and Canada and to compare current and ideal responsibilities for this role. Methods: Five multi-part questions were written and submitted for the 2019 Council on Medical Student Education in Pediatrics (COMSEP) Annual Survey and subsequently disseminated to all COMSEP member physicians. Anonymous responses were collected and results analyzed. The study was IRB exempt. Results: The program-level survey response rate was 79%. Of 115 respondent medical schools, 37% reported having a pediatric fourth-year director separate from the clerkship director, with an average of 9.8% full-time equivalent (FTE) protected time for the role. In contrast, individuals indicated 20% FTE would be ideal for fourth-year director responsibilities. The most common role identified for pediatric fourth-year directors was directing sub-internships. Respondents indicated it would be ideal for pediatric fourth-year directors to have an increased level of involvement in all areas queried in the survey, especially directing a pediatric residency preparatory course/boot camp, faculty development for educators of fourth-year students, and remediating fourth-year students. Conclusions: As specialty-specific experiences have grown in the fourth year of medical school, there is an increasing demand for faculty leadership separate from direction of the pediatric clerkship. In this national survey, pediatric educators expressed a need for additional protected time to lead fourth-year specific activities. Similar findings in other disciplines would support advocating for more protected time and expanded roles for specialty-specific fourth-year directors nationally. [ABSTRACT FROM AUTHOR]

Hydroelectric stations are a renewable source of electricity, but social and environmental impacts have been hindering social acceptance of new projects. The hydroelectricity sector has been investing heavily in sustainability marketing and social responsibility to improve the sector perceptions among shareholders and minimize public scrutiny. This paper examines corporate social responsibility (CSR) reporting of the hydroelectric sector focusing on Canadian corporations. In addition, this paper reveals that although CSR is addressed in either the websites or environmental reports, the type of framework adopted, the completeness, and frequency of reporting is not equally implemented. Overall, financial accountability persists driving the reports attention and playing a major role in the CSR view. This work also attempts to link CSR and social impact assessment to improve sustainability reporting. Several recommendations are presented throughout the text. [ABSTRACT FROM AUTHOR]

Environmental assessment (EA) is an important entry point to the development of renewable energy projects, yet the role of EA in the renewable energy sector remains poorly understood in comparison to its role in assessing and managing the potential impacts of traditional fossil fuel-based energy developments. This paper examines the requirements and provisions of EA for renewable energy development, specifically wind energy development, across Canada and the implications for renewable energy transition. Results show considerable variability in EA requirements and provisions for wind energy, including such factors as EA timelines, screening approaches, proponent responsibilities for consultation, and whether a proposed wind energy development even triggers an EA review. Differences in EA systems and procedures for wind energy projects may have implications for the predictability of EA and the relative attractiveness of certain jurisdictions for wind energy developers. As countries transition to renewable energy technologies to meet climate change targets and to deliver sustainable energy to remote and developing regions, the role of EA in balancing good environmental reviews with the pressing need to build and operate renewable power production requires further attention. The illicit tobacco trade accounts for 10% of the global cigarette market and results in US$31 billion in lost tax revenues annually. Despite legal prosecution of tobacco companies, and the introduction of new policy responses, the trade has reached an all-time high. Previous research documents how transnational tobacco companies have sought to influence government responses to the illicit trade in various countries through multiple means, including influencing of news media framing. This paper extends this analysis to Canada where the illicit trade is particularly problematic in scale and political complexity. Articles in Canadian newspapers, published from 2010–2015, were systematically searched (n = 177) and analysed to identify dominant frames, frame sponsors and policy positions related to the illicit tobacco trade. The results show that the most common frames present the issue in ways favourable to the industry. The most common non-governmental sponsors of these frames frequently have links to the tobacco industry, which are rarely disclosed. Findings indicate the need for Canadian media to be critical in its use of data sources amid industry efforts to shape public policy, and the importance of reframing policy discussions in public health terms based on independent evidence. Background: A surgical readiness assessment is a major step in the medical journey of trans people seeking gender-affirming surgery. Much of the peer-reviewed literature surrounding surgical readiness assessments emphasizes the perspectives of academics and clinicians, leaving the voices of trans and gender diverse patients largely unheard. Aims: This paper foregrounds patient experiences with surgery readiness assessments to discuss the tensions, challenges and opportunities they generate. Methods: We conducted a thematic analysis of 35 in-depth interviews with trans people who accessed or were seeking to access gender-affirming surgery in British Columbia. Results: We developed three main themes to capture participants' narratives of their surgical assessment experiences. The first, assessments as gatekeeping, explores the stories of people who described their assessments as outdated and even discriminatory processes. The second, assessments as a barrier to care, discusses the informational missteps, bureaucratic regulations, economic issues, and geographic concerns that made assessments difficult to access. The third, assessments as useful, includes positive stories about assessments that often involved feeling supported by an assessor and feeling prepared for the next steps. Discussion: These narratives demonstrate how much variation exists among people's experiences of readiness assessments for gender-affirming surgery. No matter how their actual assessment turned out, many participants approached their appointments with a great deal of anxiety and trepidation. We attributed this stress was to challenges ranging from lengthy wait times, arbitrary medical gatekeeping, a lack of access to knowledgeable and supportive providers, unclear or changing administrative processes, and insufficient communication. To address these challenges, it is crucial for the medical system to create more accessible pathways with centralized, up-to-date information for people trying to access assessments. Providing reliable estimates of subpopulation/area parameters has attracted increased attention due to their importance in applications such as policymaking. Due to low or even no samples from some areas, we must adopt indirect model approaches. Existing indirect small area estimation methods often assume that a single nested error regression model is suitable for all the small areas. In particular, the effects of the auxiliary variables are either fixed or have a single attraction center. In some applications, it can be more appropriate to cluster the small areas so that the effects of the auxiliary variables are fixed but have multiple centers in the nested error regression model. In this paper, we examine an extended nested error regression model in which the auxiliary variables have mixed effects with multiple centers. We use a penalty approach to identify these centers and estimate the model parameters simultaneously. We then propose two new small area mean estimators and construct estimators of their mean square errors. Simulations based on artificial and realistic finite populations show that the new estimators can be efficient. Furthermore, the confidence intervals based on the new methods have accurate coverage probabilities. The article reports on the International Evidence-Based Librarianship conference held in Brisbane between October 16-19, 2005. Nearly 200 participants attended the conference, from many countries around the world. The Great Britain, Canada, U.S., Australia and New Zealand were all represented, with a very stimulating mix of practitioners and researchers/academics. The focus was on developments in evidence: linking research with practice. A range of workshops and presentations throughout the event elicited much greater interaction and questioning than generally occurs in U.S. conferences. At the conference a \vide range of research projects in library and information science were reported and discussed. During the conference the emphasis was led on the point that libraries have a very active interest in understanding what their users needs are, what services they should be delivering and how they should be developing their services to better meet these needs. Workshops included case studies of generalised library activities and services to develop research questions which were not limited to the health sectors.

The article discusses various issues related to immigration, population, and the Canadian future. Unlike the American and Australian studies, the Canadian Immigration and Population Study otherwise known as the "Green Paper" on immigration, was undertaken by a small group of bureaucrats in the Canada's Department of Manpower and Immigration and not by an independent commission. A number of reports on specific questions relating population and immigration were prepared by outside consultants, and some of these have been published as supplementary reports to the Green Paper, which was released early in 1975. Public discussion of the Green Paper on immigration has tended to emphasize the ethnic aspects and to be polarized around the question of whether Canadian immigration policy and the Green Paper itself-is "racist." Representatives of the black and Asian groups are afraid that the adoption of quotas, or other measures in the future will result in the restoration of de facto racial discrimination. Right wing groups advocating "white supremacy" consider that Canada has already gone too far in the direction of a liberal "open door" policy and should revert to the practice of restricting non-white immigration. Between these two extremes are the advocates of "zero population growth," who claim not to favor discrimination, but who argue instead in favor of a substantial reduction in total immigration. Given the recentness of immigration from Third World countries, such a policy would, in practice, have the same effect as reinstituting racial discrimination or imposing quotas proportional to the present ethnic composition of the Canadian population.

This paper examines the consideration of climate change in environmental assessment (EA) in the liquefied natural gas (LNG) sector, British Columbia, Canada. Based on an analysis of recent EA applications, results show that climate change is considered, to some extent, in all phases of EA for most LNG projects. However, stakeholders indicate a dissatisfaction with practice – often based on expectations about EA that exceed what it can deliver as a project-based tool, and sometimes based on an incomplete understanding of existing climate change legislation and targets. Results also indicate inconsistent application of existing climate change requirements across project EAs. Notwithstanding proponents often addressing climate change in their EA applications, climate change tends to receive little attention in project decision and approval conditions. The paper concludes with recommendations for better practice climate change consideration in EA that is commensurate with the scope and scale of project-level issues, complemented by more strategic EA and economic instruments. The higher religiosity of women in the Western Christian societies is one of the best documented findings in the religious scholarship. In spite of the recent vibrancy of secular movements in North America, the higher religiosity of women appears persistent. As a result, the gender ratio is greatly skewed in the secular groups in favour of males. For instance, for every atheist female in North America, there are at least three males. Using the Canadian General Social Surveys of 1985–2014, this paper examines how the gender religiosity gap has evolved among the Canadian-born. Throughout the period, Canadian-born women are found less likely to be unaffiliated and show a greater frequency of religious attendance. The religious attendance gap is found to be closing. The unaffiliation gap, on the other hand, seems to have widened in the 21st century. Limiting the analyses to the gainfully employed respondents only reduces the religious attendance gap. For the high earners, the attendance gap effectively disappears, while a large unaffiliation gap persists into the 2010s. This pattern is best explained by the recent literature asserting that men and women are differentially socially sanctioned for the adoption of a secularized identity. The alleged sexism of the new secular movements is also noted as a potential explanation. The examination of the recent Canadian data on perceived religious and gender discrimination produces evidence congruent with both of these potential explanations. [ABSTRACT FROM AUTHOR]

In cold region environments, any alteration in the hydro-climatic regime can have profound impacts on river ice processes. This paper studies the implications of hydro-climatic trends on river ice processes, particularly on the freeze-up and ice-cover breakup along the Athabasca River in Fort McMurray in western Canada, which is an area very prone to ice-jam flooding. Using a stochastic approach in a one-dimensional hydrodynamic river ice model, a relationship between overbank flow and breakup discharge is established. Furthermore, the likelihood of ice-jam flooding in the future (2041–2070 period) is assessed by forcing a hydrological model with meteorological inputs from the Canadian regional climate model driven by two atmospheric–ocean general circulation climate models. Our results show that the probability of ice-jam flooding for the town of Fort McMurray in the future will be lower, but extreme ice-jam flood events are still probable. [ABSTRACT FROM AUTHOR]

The Green Chemistry Initiative (GCI) is a student-led group founded in 2012 with the primary mission of promoting green chemistry education at the University of Toronto. In order to achieve this, the GCI's activities have included undergraduate curriculum development, arrangement of an external speaker seminar series, and organization of an annual three-day symposium along with biweekly trivia challenges. To broaden education beyond the Department of Chemistry, a successful YouTube video campaign articulating the Twelve Principles of Green Chemistry in an accessible manner has also been undertaken (acquiring over 40,000 views), in addition to monthly blog posts and conference/outreach presentations. Descriptions of these activities are discussed in this paper, along with the resulting impact they have had. Through such efforts, undergraduate and graduate students are showing a growing understanding of the relevance of green chemistry in today's world, with the GCI serving as a platform for similar groups to build upon across Canada. This paper examines the media habits of light TV viewers in Canada to determine how best to reach them. Light viewers cannot be selectively reached by manipulating TV schedules because no type of programming attracts more than its fair share of light viewers. English light viewers spend more time with TV than any other medium, while French light viewers spend the most time with radio. This refutes the Media Imperative concept that magazines are the alternative to TV. Reach analysis reveals that light viewers are reached by TV at even moderate weight levels. Background: This paper describes the creation of a theatrical commons that aimed to broaden and deepen democratic engagement between diverse citizens in one public school community in Manitoba, Canada. Purpose and method: The researchers considered how Forum and Image Theatre provided former refugee youth, guardians, parents and the general public with the opportunity to name and invent different possibilities to address complex school community challenges at an individual and at a policy level. The study utilised critical participatory action research that drew heavily on the theatrical imaginations of Augusto Boal and David Diamond. Main arguments and conclusions: Three recommendations were derived from the findings as they related to the research questions and literature: (1) that Image and Forum Theatre can be used to connect public education to the pursuit of transformative democracy; (2) that Image and Forum Theatre would be a useful means of informing system planning and reflection, and; (3) that a theatrical commons has value for creating space for marginalised voices to influence decision-making. Using two recent nationally representative Canadian surveys, this paper investigates how secularization affects civic participation, inclusive of political engagement and philanthropy. Three mutually exclusive categories of secularized individuals are considered. The analysis suggests that Canadian secular groups are relatively less engaged with politics and volunteer fewer hours, compared with the actively religious. In this paper we contend neo-liberal education policy which supports the creation of schooling choices in public education systems is reshaping, conflating and branding ethnicity. We make these points in reference to school choice in Toronto, Canada, and the establishment of an Africentric ethno-centric school. We argue that one of the registers within which education and ethnicity in Toronto operates relates to the conflation of commodification, ethnicity and geography, and that this conflation indicates one of the limits of school choice as a possible way to redress Black student disadvantage. We suggest education policy, which enables the establishment of ethno-centric schools, enters the realm of other debates about race, equity and difference that include the practices of marketing and branding. Objectives. The number of new health technologies has risen over the past decade. These new technologies usually are more effective but they also cost more compared to existing ones. In a publicly funded health care system such as Canada, the aim is to maximize the health of the population within the resources available. As a result, it is unavoidable that choices and trade-offs have to be made because there will always be more treatment options than resources will allow (i.e., scarcity of resources) as well as alternative uses for those resources (i.e., opportunity costs). The objective of this paper is to provide an overview of economic evaluations and how these tools can be used to help inform payers of the best use of scarce health care resources. Study design. This descriptive paper includes a summary of key consepts and definitions in economic appraisal and draws upon recently published papers as illustrations. Methods. Background on the necessity and role of economic evaluations is provided, followed by a description of the approaches for, and types of, economic evaluations. Two illustrative examples are used and some implications for rural, remote and circumpolar communities are discussed. Results. There are 2 main approaches for conducting an economic evaluation (trial- and model-based) and 3 types of evaluations which can be considered to inform payers of the best use of health care resources (cost-effectiveness, cost-utility and cost-benefit analyses). Conclusions. Techniques of economic evaluation are useful tools and an important input into the decision- making process. Although these techniques have universal application, there are issues specific to rural, remote and circumpolar communities which can affect the results of economic appraisals. This paper is a brief overview of the concept of the transnational archive as a counterpoint to the idea that a national archive is necessarily a locus of a static idea of nation. The Canadian national archives is used as a case study of an archives that was transnational in its inception, and one that has continued to change in its mandate and materials as a response to patterns in migration and changing notions of multiculturalism as a Canadian federal policy. Drawing upon transnational multi-sited research analysing sending and receiving aspects of migration flows and the shifting priorities of neoliberal citizenship regimes, this article highlights the class complexity of Philippine gendered migration pathways to Canada. Migrant agency and class complexity are linked to neoliberal immigration and labour export policies that privilege the acquisition of capital serving the interests of sending and receiving countries. Sometimes this benefits elite migrants but it also exacerbates gendered class cleavages between migrants and within Philippine society. The histories of Philippine internal and overseas migration have contributed to a culture of migration whereby Filipinos exhibit flexibility to draw advantage from subtle shifts in Canadian immigration policy. This paper examines state power and cultural marginalisation among Aboriginal women in a relatively remote area of north-western British Columbia, Canada. In this paper, I argue that intersecting state powers explain intra-generational ‘ethnic mobility’, that is, shifts in individual ethnic identity among Aboriginal women in this area. In order to understand these shifting identities I have used the idea of ‘structural violence’ (Farmer 2003, 8) to describe how power inequalities between people and the state affect impoverished and marginalised people in different ways, and how decisions made by others constrain their survival strategies . Canada is not a peripheral country by most standards. However, since the 1960s, Canadian cities have sought to change the somewhat provincial image they have historically had, and they have used mega-events such as Olympic Games, World Expositions and other 'second order' international sports events to try to reposition themselves on the world stage. This paper examines the place promotion agendas of the two Canadian cities that have hosted Olympic Games, and will also comment on the aspirations of Vancouver, which will host the Winter Olympics in 2010. The core of the paper will concentrate on different aspects of 're-maging' the city: image building, signalling and identity transformation. Here I argue that an important but widely overlooked aspect of increasing the stature of a city involves changing the ways that citizens of that city think about themselves. The next section will examine whether the exposure and the opportunities for civic promotion afforded by a Games actually translate into economic growth. I will argue that the evidence for this is mixed at best Finally, the paper argues that the public money spent on Olympics typically brings the greatest benefits to local and regional elites, despite popular rhetoric that such events are good for the 'community as a whole'. This paper presents a case study from the Canadian Arctic, in which the community context of an archaeological project has led to a re-thinking of a fundamental aspect of archaeological interpretation. Archaeologists are constantly confronted with the problem of identifying appropriate analogues for the societies whose material remains they study. In the Arctic, a particularly rich ethnographic record exists relating to recent Inuit lifeways; however, it remains difficult to determine when, if ever, it should be used to interpret the Palaeo-Eskimo archaeological record which pre-dates 1,000 BP. This issue will be explored within the context of the Iqaluktuuq Project, a new program of field research which aims to combine the traditional knowledge of modern Inuit elders with the Palaeo- and Neo-Eskimo archaeological records in the Ekalluk River region of southeastern Victoria Island, Nunavut. Ultimately, the social engagement of archaeologists with elders has led to a reconsideration of the process of analogical inference, resulting in a more robust use of recent Inuit lifeways as models for Palaeo-Eskimos than would have occurred based on purely 'academic' considerations. The paper seeks to identify common features in the fertility patterns of the English-speaking world and provide a model basis for comparison of fertility between countries and over time. Attention is focused on the heterogeneity within the fertility patterns of Australia, Canada, New Zealand, and the United States, similar to that reported earlier for the UK and the Irish Republic. The recent age-specific fertility patterns of these countries display a marked 'bulge' in fertility of women under age 25. A mixture model with two-component: Hadwiger functions provides a suitable fit. The heterogeneity thus suggested is shown to be related to differences in the timing of births by marital status, and its magnitude is related to the proportion of births outside marriage. Additionally, there is some evidence that, in the United States, and to a lesser extent in New Zealand, this heterogeneity in fertility patterns may be explained by ethnic differences in the timing and number of births. This paper presents ten examples from the Province of Quebec, where firms have increased their profitability while protecting the environment. These examples contain new ideas and arguments that can help environment-conscious managers to convince their colleagues to become proactive in this area. The examples are preceded by a discussion of the general context of the relationship between environmental quality and profitability. Specifically, the link between improved environmental quality and increased profitability is elucidated. We then discuss ten examples grouped by industrial sector. Three of the cases are presented in detail, to better shed light on the motivations leading to the implementation of these measures, along with the implementation process. Subarctic ecosystems are experiencing rapid changes as a result of climate warming and more frequent and severe disturbances. There is considerable uncertainty regarding ecological trajectories following disturbance in forested ecosystems underlain by permafrost because their structure and function is controlled by feedbacks among soil conditions, vegetation, and ground thermal regime. In this paper, we studied post-disturbance ecosystem recovery in an area of discontinuous permafrost 32 years after construction and abandonment of a winter access road in Nahanni National Park Reserve (NNPR). Ecosystem recovery was examined by comparing disturbed (road) and undisturbed (adjacent to the road) sites in the following terrain types: spruce peatland, black spruce parkland, deciduous forest, and alpine treeline terrain. Our field data show that disturbances to discontinuous permafrost terrain can lead to large and persistent changes to ecosystem composition and structure. Our findings indicate that the ecological response of discontinuous permafrost to disturbance and climate warming will depend on interactions between soil conditions and vegetation communities. In instances where disturbance to discontinuous permafrost fundamentally disrupts stabilizing interactions between soil conditions and vegetation communities, we should expect lasting changes to ecosystem structure and function. The identification of homogeneous precipitation regions is essential in the planning, design and management of water resources systems. Regions are identified using a technique that partitions climate sites into groups based on the similarity of their attributes; the procedure is known as regionalization. In this paper the ability of four attribute sets to form large, coherent precipitation zones is assessed in terms of the regional homogeneity of precipitation statistics and computational efficiency. The outcomes provide guidance for effective attribute selection for future studies in Canada. The attributes under consideration include location parameters (latitude, longitude), distance to major water bodies, site elevation and atmospheric variables modelled at different pressure levels. The analysis is conducted in two diverse climate regions within Canada including the Prairie and the Great Lakes–St Lawrence lowlands regions. The method consists of four main steps: (i) formation of the attribute sets; (ii) determination of the preferred number of regions (selection of thec-value) into which the sites are partitioned; (iii) regionalization of climate sites using the fuzzyc-means clustering algorithm; and (iv) validation of regional homogeneity usingL-moment statistics. The results of the attribute formation,c-value selection, regionalization and validation processes are presented and discussed in a comparative analysis. Based on the results it is recommended for both regions to use location parameters including latitude, longitude and distance to water bodies (in the Great Lakes region) to form precipitation regions and to consider atmospheric variables for future (climate change) applications of the regionalization procedure. Income tax reform has become a hot topic in both the United States and Canada. Over the past few years, a variety of proposals have been advanced for the replacement of the current income tax system and most proposals involve a compression of the multi-rate structure into a single rate and a shift to some form of consumption tax base. Flat taxes are advocated on the belief that they will provide a strong stimulus to investment, employment and output. Their supporters are convinced that the economic benefits are sufficiently large to make everyone better off, therefore there is no need to be concerned about the distributional effects of flat taxes. However, the claims about potentially large efficiency gains from flat taxes are not supported by research. Evaluating the effects of a consumption-base flat tax of the type proposed by Hall and Rabushka is one of the main purposes of the paper. Using a microdata set for Canada, which allows identifying taxpayers by both income level and family type, we show that flat taxes not only increase income inequality but also have important horizontal equity implications. We argue that a full debate on income tax reform requires a detailed evaluation of both polar alternatives to the current hybrid income tax: a move to a consumption base and a move to a comprehensive income tax. Toward that end, we have performed a simulation which estimates the distributional effects of a comprehensive income base with across the board rate reductions in order to maintain revenue-neutrality. We show that this option has advantages over the consumption-base flat tax in terms of both vertical and horizontal equity. [ABSTRACT FROM AUTHOR]

Using field data collected in northern Canadian communities between 1991 and 1995, this paper explores recent attempts by Canadian aboriginal peoples to determine and re-discover, on their own terms, what is meant by the expressions "traditional life" and "traditional culture." Faced with the infiltration into the north of rationalized and commercial "southern" sports and the values they imply, aboriginal Canadians have attempted to consolidate their cultural integrity through the staging of games-festivals. An examination of these games-festivals demonstrates a number of tensions or colliding identities not only between "outside" and aboriginal communities but also within aboriginal communities over the place and meaning of traditional forms of sport, games and other contests. This paper examines the effects of parents an ethnic language retention by children in five Chinese communities in urban Canada Parents' direct promotion of ethnic language is found to explain 44 percent of the total variance of children's language retention, indicating that the family is an important language retaining agent. Parents' direct promotion is related positively to their ethnic community involvement and negatively to their knowledge of English/French. Surprisingly, parents' ethnic identification and their length of time in Canada have no significant relationships with the above variables, suggesting that, among Canadian-Chinese parents, ethnic identification is not a determinant of ethnic life, and their levels of assimilation have remained rather stable.
