Background: Many centers worldwide are shifting from laparoscopic to robotic minimally invasive hepato-pancreato-biliary resections (MIS-HPB) but large single center series assessing this process are lacking. We hypothesized that the introduction of robot-assisted surgery was safe and feasible in a high-volume center. Methods: Single center, post-hoc assessment of prospectively collected data including all consecutive MIS-HPB resections (January 2010–February 2022). As of December 2018, all MIS pancreatoduodenectomy and liver resections were robot-assisted. All surgeons had participated in dedicated training programs for laparoscopic and robotic MIS-HPB. Primary outcomes were in-hospital/30-day mortality and Clavien-Dindo ≥ 3 complications. Results: Among 1875 pancreatic and liver resections, 600 (32%) were MIS-HPB resections. The overall rate of conversion was 4.3%, Clavien-Dindo ≥ 3 complications 25.7%, and in-hospital/30-day mortality 1.8% (n = 11). When comparing the period before and after the introduction of robotic MIS-HPB (Dec 2018), the overall use of MIS-HPB increased from 25.3 to 43.8% (P < 0.001) and blood loss decreased from 250 ml [IQR 100–500] to 150 ml [IQR 50–300] (P < 0.001). The 291 MIS pancreatic resections included 163 MIS pancreatoduodenectomies (52 laparoscopic, 111 robotic) with 4.3% conversion rate. The implementation of robotic pancreatoduodenectomy was associated with reduced operation time (450 vs 361 min; P < 0.001), reduced blood loss (350 vs 200 ml; P < 0.001), and a decreased rate of delayed gastric emptying (28.8% vs 9.9%; P = 0.009). The 309 MIS liver resections included 198 laparoscopic and 111 robotic procedures with a 3.6% conversion rate. The implementation of robotic liver resection was associated with less overall complications (24.7% vs 10.8%; P = 0.003) and shorter hospital stay (4 vs 3 days; P < 0.001). Conclusion: The introduction of robotic surgery was associated with greater implementation of MIS-HPB in up to nearly half of all pancreatic and liver resections. Although mortality and major morbidity were not affected, robotic surgery was associated with improvements in some selected outcomes. Ultimately, randomized studies and high-quality registries should determine its added value. Graphical Abstract: [Figure not available: see fulltext.]

Archeologie & Cultuurhistorie heeft een archeologisch bureau- en een inventariserend veldonderzoek door middel van verkennende boringen uitgevoerd voor een plangebied aan de Hugo van der Goeslaan te Eindhoven, gemeente Eindhoven (bekend als het Campinaterrein, afbeelding 1, kaart 1). Het Campinaterrein ligt binnen het bedrijventerrein de Kade. De locatie wordt begrensd door de Dirk Boutslaan (zuiden), het bestaand bedrijventerrein de Kade (westen), de Kanaaldijk Zuid (noorden) en de Ring (oosten). De locatie ligt in stadsdeel Stratum, in de wijk Oud-Stratum, in de Irisbuurt. Het plangebied heeft een oppervlakte van ca. 3,3 ha, en is momenteel vrijwel geheel bebouwd en verhard. Binnen het plangebied is nieuwbouw van woningen voorzien. Op afbeelding 2 is een impressie opgenomen van de toekomstige situatie. Zoals op de luchtfoto te zien is wordt voor de realisatie een groot deel van de bestaande bebouwing gesloopt. Op de afbeeldingen 3 en 4 is dit schematische weergegeven. Daarnaast zal ook binnen het plangebied nieuwe riolering worden aangelegd. Deze zijn weergegeven op afbeelding 5 met de huidige kabels en leidingen. Voorafgaand aan de ingrepen dient in kaart te worden gebracht of bij de voorgenomen ontwikkeling archeologische waarden in het geding kunnen raken. Gezien de aard van de ingrepen (nieuwbouw, aanleg infra etc. zullen de geplande ingrepen mogelijk tot in het relevante archeologische niveau reiken. Het plangebied had op basis van het bureauonderzoek in principe een hoge archeologische verwachting voor het aantreffen van resten vanaf de Steentijd tot aan de Late Middeleeuwen/Nieuwe tijd; gezien de verstoringen binnen het plangebied waarschijnlijk bij te stellen tot ‘middelhoge tot hoge archeologische verwachting’. Het plangebied ligt volgens de concept gemeentelijke archeologische beleidskaart binnen een buffer rond het historisch bebouwingslint van Poeijers waar mogelijk resten van bewoning voorkomen vanaf de Late Middeleeuwen/Nieuwe tijd. Er zijn op basis van historisch kaartmateriaal geen aanwijzingen aangetroffen voor bewoning van het plangebied in de Late Middeleeuwen/Nieuwe tijd, maar het kan gezien de aanwezigheid van historische wegen ook niet geheel worden uitgesloten (middelhoge archeologische verwachting). Om deze verwachting te toetsen is op 21 februari 2022 een inventariserend veldonderzoek uitgevoerd. Tijdens dit veldonderzoek bleek het plangebied grotendeels verstoord. In het zuid/zuidwesten van het plangebied werden echter wel nog intacte profielen aangetroffen. Advies Op basis van de bevindingen van het booronderzoek bestaat voor een deel van het plangebied nog een hoge archeologische verwachting voor het aantreffen van archeologische resten uit alle perioden, met inbegrip van een historische weg. Dit is het gedeelte van het plangebied dat op de gemeentelijke beleidskaart binnen de buffer ligt voor historische bebouwing van het gehucht Poeijers. Hier adviseert Vestigia nog vervolgonderzoek uit te voeren (afbeelding 20). Het zetten van karterende of waarderende boringen is niet de meest geëigende prospectiemethode voor het opsporen van historische bebouwing of wegen uit de Late Middeleeuwen/Nieuwe tijd. Geadviseerd wordt vervolgonderzoek uit te voeren in de vorm van een archeologische begeleiding bij het uitgraven van de bouwkuip, of een profsleuvenonderzoek al dan niet met doorstart naar een opgraving. Hiervoor dient eerst een Programma van Eisen (PvE) te worden opgesteld dat de goedkeuring behoeft van het bevoegd gezag, de gemeente Eindhoven. Zonder een goedgekeurd en getekend PvE kan niet met de werkzaamheden worden begonnen. Op basis van het booronderzoek kan de archeologische verwachting binnen de rest van het plangebied worden bijgesteld naar laag. De kans dat daar met de geplande werkzaamheden een intacte archeologische vindplaats wordt geschaad moet als klein worden ingeschat. Vestigia Archeologie & Cultuurhistorie adviseert daarom ook geen verdere stappen in het kader van de cyclus Archeologische Monumentenzorg (AMZ). Het bevoegd gezag, de Gemeente Eindhoven, dient eerst over het advies in dit rapport een besluit te nemen. Wanneer het bevoegd gezag besluit dat vervolgonderzoek niet noodzakelijk is en het plangebied wordt vrijgegeven voor de voorgenomen ontwikkelingen, blijft de meldingsplicht archeologische toevalsvondst of waarneming van kracht (Erfgoedwet, artikel 5.10 Archeologische toevalsvondst). Aangezien het nooit volledig is uit te sluiten dat tijdens eventueel grondverzet een archeologische ‘toevalsvondst’ wordt gedaan, is het wenselijk de uitvoerder van het grondwerk te wijzen op de plicht om hiervan zo spoedig mogelijk melding te doen bij het bevoegd gezag, de Gemeente Eindhoven, en de Rijksdienst voor het Cultureel Erfgoed. Background Preoperative portal vein embolization (PVE) is frequently used to improve future liver remnant volume (FLRV) and to reduce the risk of liver failure after major liver resection. Objective This paper aimed to assess postoperative outcomes after PVE and resection for suspected perihilar cholangiocarcinoma (PHC) in an international, multicentric cohort. Methods Patients undergoing resection for suspected PHC across 20 centers worldwide, from the year 2000, were included. Liver failure, biliary leakage, and hemorrhage were classified according to the respective International Study Group of Liver Surgery criteria. Using propensity scoring, two equal cohorts were generated using matching parameters, i.e. age, sex, American Society of Anesthesiologists classification, jaundice, type of biliary drainage, baseline FLRV, resection type, and portal vein resection. Results A total of 1667 patients were treated for suspected PHC during the study period. In 298 patients who underwent preoperative PVE, the overall incidence of liver failure and 90-day mortality was 27% and 18%, respectively, as opposed to 14% and 12%, respectively, in patients without PVE (p p = 0.005). After propensity score matching, 98 patients were enrolled in each cohort, resulting in similar baseline and operative characteristics. Liver failure was lower in the PVE group (8% vs. 36%, p p p = 0.01), and 90-day mortality (7% vs. 18%, p = 0.03). Conclusion PVE before major liver resection for PHC is associated with a lower incidence of liver failure, biliary leakage, abscess formation, and mortality. These results demonstrate the importance of PVE as an integral component in the surgical treatment of PHC.

Background: Limited evidence exists to guide the management of patients with liver metastases from squamous cell carcinoma (SCC). The aim of this retrospective multicentre cohort study was to describe patterns of disease recurrence after liver resection/ablation for SCC liver metastases and factors associated with recurrence-free survival (RFS) and overall survival (OS). Method: Members of the European-African Hepato-Pancreato-Biliary Association were invited to include all consecutive patients undergoing liver resection/ablation for SCC liver metastases between 2002 and 2019. Patient, tumour and perioperative characteristics were analysed with regard to RFS and OS. Results: Among the 102 patients included from 24 European centres, 56 patients had anal cancer, and 46 patients had SCC from other origin. RFS in patients with anal cancer and non-anal cancer was 16 and 9 months, respectively (P = 0.134). A positive resection margin significantly influenced RFS for both anal cancer and non-anal cancer liver metastases (hazard ratio 6.82, 95 per cent c.i. 2.40 to 19.35, for the entire cohort). Median survival duration and 5-year OS rate among patients with anal cancer and non-anal cancer were 50 months and 45 per cent and 21 months and 25 per cent, respectively. For the entire cohort, only non-radical resection was associated with worse overall survival (hazard ratio 3.21, 95 per cent c.i. 1.24 to 8.30). Conclusion: Liver resection/ablation of liver metastases from SCC can result in long-term survival. Survival was superior in treated patients with liver metastases from anal versus non-anal cancer. A negative resection margin is paramount for acceptable outcome.

Otto Lanz (1865-1935) was a Swiss surgeon trained by Theodore Kocher in Berne. In 1902, he was invited to become professor of surgery at the Binnengasthuis hospital in Amsterdam. He was an outstanding surgeon who was well known for his surgery of the thyroid gland and the appendix. He introduced the eponymous Lanz's point in acute appendicitis and also the girdle incision for appendectomy. He was a strong proponent of 'early' appendectomy, as opposed to the conservative approach, at that time the treatment of choice. He was a great connoisseur and passionate collector of Italian Renaissance art. His Italian works of art have been exhibited in a special 'Lanz Hall' at the Rijksmuseum in Amsterdam. During World War II, his collection was acquired for Hitler's proposed Führermuseum but was returned to Amsterdam after the war. A portrait by Jan Toorop and Lanz's former villa on the Museum Square in Amsterdam remind us of this colourful surgeon.

Background Distal pancreatectomy with celiac axis resection (DP-CAR) is a treatment option for selected patients with pancreatic cancer involving the celiac axis. A recent multicenter European study reported a 90-day mortality rate of 16%, highlighting the importance of patient selection. The authors constructed a risk score to predict 90-day mortality and assessed oncologic outcomes. Methods This multicenter retrospective cohort study investigated patients undergoing DP-CAR at 20 European centers from 12 countries (model design 2000–2016) and three very-high-volume international centers in the United States and Japan (model validation 2004–2017). The area under receiver operator curve (AUC) and calibration plots were used for validation of the 90-day mortality risk model. Secondary outcomes included resection margin status, adjuvant therapy, and survival. Results For 191 DP-CAR patients, the 90-day mortality rate was 5.5% (95 confidence interval [CI], 2.2–11%) at 5 high-volume (≥ 1 DP-CAR/year) and 18% (95 CI, 9–30%) at 18 low-volume DP-CAR centers (P = 0.015). A risk score with age, sex, body mass index (BMI), American Society of Anesthesiologists (ASA) score, multivisceral resection, open versus minimally invasive surgery, and low- versus high-volume center performed well in both the design and validation cohorts (AUC, 0.79 vs 0.74; P = 0.642). For 174 patients with pancreatic ductal adenocarcinoma, the R0 resection rate was 60%, neoadjuvant and adjuvant therapies were applied for respectively 69% and 67% of the patients, and the median overall survival period was 19 months (95 CI, 15–25 months). Conclusions When performed for selected patients at high-volume centers, DP-CAR is associated with acceptable 90-day mortality and overall survival. The authors propose a 90-day mortality risk score to improve patient selection and outcomes, with DP-CAR volume as the dominant predictor. Biliary tract carcinoma (BTC) comprises gallbladder and intra-/extrahepatic cholangiocarcinoma (GBC, ICC, EHC), which are currently classified by anatomical origin. Better understanding of the mutational profile of BTCs might refine classification and improve treatment. We performed a systematic review of studies reporting on mutational profiling of BTC. We included articles reporting on whole-exome/whole-genome-sequencing (WES/WGS) and targeted sequencing (TS) of BTC, published between 2000-2017. Pooled mutation proportions were calculated, stratified by anatomical region and sequencing technique. A total of 25 studies with 1806 patients were included. Overall, TP53 was the most commonly mutated gene in BTC. GBC was associated with mutations in PFKFB3, PLXN2 and PGAP1. Mutations in IDH1, IDH2 and FGFR fusions almost exclusively occurred in ICC patients. Mutations in APC, GNAS and TGFBR2 occurred exclusively in EHC patients. In conclusion, subtypes of BTCs exhibit minor differences in mutational profile, which is likely influenced by the cell of origin.
