Aim: To evaluate the 10-year treatment outcomes and cost-effectiveness of adolescents' mental health treatment initiated under the social network-oriented open dialogue (OD) approach. -- Methods: This longitudinal register-based study included all persons who, for the first time, received psychiatric treatment in Finland during the period 1 January 2003–31 December 2008, and who were aged 13–20 at onset (n = 44 868). The OD group included all persons whose treatment commenced in the Western Lapland catchment area (n = 780), this being the only region in Finland where OD covered the entire mental healthcare service at the time of inclusion. The comparison group (CG) included the rest of Finland (n = 44 088). The primary outcome variables were psychiatric treatment and/or disability allowances at the end of the 10-year follow-up, or death. The secondary outcome variables were treatment and disability expenses. Generalized linear models weighted by inverse probability of treatment were used to study the association between OD and the primary outcomes. Population proportions were used to evaluate the cost-effectiveness of the approaches. -- Results: Treatment that commenced outside OD was associated with higher odds of continuing to receive treatment (adjusted odds ratio [aOR] 1.4; 95%CI 1.2–1.6) and disability allowances (aOR 1.6; 95%CI 1.2–2.1) at the end of the 10-year follow-up. No significant difference in the mortality ratio emerged. The cumulative 10-year expenses per capita were lower under OD. -- Conclusions: OD associated with favourable long-term outcomes, but due the observational design and possible residual confounding, further studies with a more robust research design are required. peerReviewed

PurposeThis article analyzes the cognitive microfoundations, conflicting institutional logics and professional hybridization in a case characterized by conflict.Design/methodology/approachIn contrast to the majority of earlier studies focusing on special health care, the study was conducted in a Finnish basic health care organization. The empirical data include 36 interviews, accounting reports, budgets, newspaper articles and meeting notes collected 2013–2018.FindingsThe use of accounting techniques in this case did not offer professionals sufficient support under conditions of conflict. The authors suggest that this perceived lack of support intensified the negative emotions toward accounting techniques. These negative emotions aggregated into incompatible professional-level institutional logics, which contributed to the lack of hybridization between such logics. The authors highlight the importance of the cognitive microfoundations, that is, the individual-level interpretations and emotional responses, in the analysis of conflicting institutional logics.Practical implicationsManagerial attention needs to be directed to accounting practices perceived as frustrating or threatening, a perception that can prevent the use of accounting techniques in the creation of professional hybrids. The Finnish basic health care context involves inconsistent political decision-making, multiple tasks, three institutional logics and individual interpretations and emotions in various decision-making situations.Originality/valueThis study develops microfoundational accounting research by illustrating how individual-level cognitive microfoundations such as dissatisfaction with budgeting, aggregate into professional-level institutional logics, and in our case, prevent professional hybridization in a basic health care setting characterized by conflict and three separate institutional logics.

Liikenteen sähköistyminen on globaali ilmiö, joka vaikuttaa myös Suomeen. Suomessa arvioidaan olevan vuoteen 2030 mennessä jopa 700 000 sähköautoa. Sähköautojen lataus tapahtuu tutkimusten mukaan pääasiassa kodeissa ja työpaikoilla. Latauspisteiden määrälle sähköautoa kohden käytetään kerrointa 1,6. Sähköautojen määrän lisääntymisen sekä lainsäädännön vuoksi Suomeen tarvitaan tulevina vuosina satoja tuhansia uusia latausvalmiuksia ja -pisteitä. Latauspisteiden ja -valmiuksien rakentaminen on edelleen tuore ilmiö eikä toteutusta suoraan ohjaavia standardeja ole. Tämä aiheuttaa kovan kysynnän lisäksi riskin, että markkinoilla on toimijoita, joilla ei ole kokemusta latausjärjestelmien toteuttamisesta, mikä voi johtaa jopa riskeihin latausjärjestelmien toimivuudelle. Tämän diplomityön toimeksiantaja oli sähköyhtiö Oomi Oy. Työn tarkoituksena oli selvittää, mitä vaiheita latausjärjestelmien toteutusprojekteihin kuuluu, keitä sidosryhmiä niissä on mukana ja mitä huomioitavaa etenkin suunnittelussa ja mitoituksessa yleensä on. Lisäksi etsittiin kirjallisuuden ja asiantuntijahaastattelujen avulla laatumittareita, joilla latausjärjestelmien toteutusprojektien laatua voitaisiin mitata ja vertailla. Tutkimus toteutettiin niin kirjallisuustutkimuksena, haastattelututkimuksena sekä toimeksiantajan sisäisten kustannus- ja projektitietojen perustella. Työssä oli tutkimusaineistona mukana kolme Oomin toimittamaa latausjärjestelmää eri puolille Suomea. Kohteiden latausjärjestelmät sisälsivät kymmeniä ja satoja latausvalmiuksia. Case-esimerkkikohteille laadittiin kustannusanalyysi ja kohteita arvioitiin valittujen laatumittareiden perusteella Oomin sisäisten projektimateriaalien avulla. Lopuksi selvitettiin työn tulosten ja asiantuntijahaastattelujen tulosten kautta johtopäätöksenä keinoja kehittää yleisesti latausjärjestelmien toteutusprojektien laatua ja kustannustehokkuutta. Latausjärjestelmän toteutusprojekti voidaan jakaa muiden rakennusprojektien tapaan määrittely-, suunnittelu-, toteutus- ja lopetusvaiheeseen, joissa voi olla mukana tilaajan ja pääurakoitsijan lisäksi aliurakoitsijoita, konsultteja ja ulkopuolelta hankittu suunnittelija. Latausjärjestelmien toteutusprojektien kustannuksiin vaikuttavat merkittävästi kohteen sijainti, latausjärjestelmän tyyppi ja maatöiden tarve. Laatumittareina laadun arvioimiseen voidaan käyttää esimerkiksi arvioidun ja toteutuneen toimitusajan toteutumista, reklamaatioiden määrää sekä arvioitujen kustannusten suhdetta toteutuneisiin kustannuksiin. Case-esimerkkikohteiden perusteella asennustyö, maatyöt, sähkökeskukset sekä asennustarvikkeet muodostivat suurimmat kustannukset niin tarjouslaskentavaiheessa kuin toteutuneissakin kustannuksissa. Lisäksi etenkin älykkäissä latausjärjestelmissä latauslaitteet ovat olennainen kustannustekijä, mutta niiden kustannukset ovat helposti arvioitavissa. Näiden huomioiminen tarjouslaskentavaiheessa on kriittistä projektin budjetissa pysymiseen. Takuuajan reklamaatioista valtaosa oli liittynyt latauslaitteiden takuukorjauksiin. Lisäkustannuksia oli syntynyt etenkin materiaalihankinnoista, tietoliikenneyhteyksien järjestämisestä ja maatöistä. Johtopäätöksenä voidaan todeta, että tarjouslaskennan ja suunnittelun rooli on projektissa aivan keskeinen. Sillä on vaikutusta niin kustannustehokkuuteen kuin laatumittarien mukaisen laadun toteutumiseen. Rakennusvaiheessa erittäin tärkeää on myös tiedonjako eri sidosryhmien välillä sekä urakoitsijan puolella myös asennusresurssien tehokas koordinointi. Etenkin taloyhtiöpuolella korostuu tarve asiantuntijuudelle latausjärjestelmää hankittaessa, jolloin ulkopuolisen konsultin tai asiantuntijan käyttäminen apuna tarjouspyynnön laatimisessa sekä toimitusprojektin aikana helpottaa usein jokaista eri sidosryhmää.

Julkisen koulurakentamisen vallitseva trendi on tehdä suuria, satojen oppilaiden yhtenäiskouluja. Tavoitteena on tuottaa oppimista tukevia tiloja ja mahdollistaa oppilaille eheä oppipolku koko peruskoulun ajaksi. Keskustelussa esiintyy usein myös olettamia suurempien yksiköiden teknistaloudellisista skaalaeduista. Tässä tutkimuksessa analysoitiin yli 350 peruskouluyksikön otoksella, liittykö koulukoon kasvattamiseen skaalaetuja tilankäytön näkökulmasta. Tutkimuksen tulokset kyseenalaistavat käsityksen, että suurten koulujen oppilaskohtaiset tilakustannukset olisivat pääsääntöisesti pieniä kouluja edullisempia. peerReviewed

This paper systematically reviews the literature on how to reduce nutrient emissions to the Baltic Sea cost-effectively and considerations for allocating these costs fairly among countries. The literature shows conclusively that the reduction targets of the Baltic Sea Action Plan (BSAP) could be achieved at considerably lower cost, if countries would cooperate to implement the least costly abatement plan. Focusing on phosphorus abatement could be prudent as the often recommended measures—wastewater treatment and wetlands—abate nitrogen too. An implication of our review is that the potential for restoring the Baltic Sea to good health is undermined by an abatement strategy that is more costly than necessary and likely to be perceived as unfair by several countries. Neither the BSAP nor the cost-effective solution meet the surveyed criteria for fairness, implying a need for side-payments.

Sähköasemarakentamiseen investoidaan enemmän kuin koskaan aiemmin. Tämä johtuu osittain käynnissä olevasta energiamurroksesta, mutta myös jatkuvasti ikääntyvän verkko-omaisuuden saneeraustarpeesta. Projektien kustannustehokkaan toteutuksen varmistaminen on sähköverkon haltijoille hyvin ajankohtainen aihe. Tässä Caruna Oy:lle tehdyssä diplomityössä selvitettiin vaikutusmahdollisuuksia 110/20 kV sähköasemaprojektin kustannustehokkuuden lisäämiseksi. Selvitys aloitettiin sähköasemaprojektin kustannusrakennetutkimuksella. Tutkimuksen lopputuloksena tunnistettiin kolmen esimerkkiprojektin avulla keskeisimmät sähköasemaprojektissa esiintyvät kustannuskomponentit. Tunnistettujen kustannuskomponenttien pohjalta määriteltiin missä kohtaa projektia kukin komponentti toteutuu ja milloin sen suuruuteen on mahdollista vaikuttaa. Selvitystä täydennettiin haastattelututkimuksella, jossa tarkoituksena oli avata, miten kustannusrakennetutkimuksessa tunnistettujen kustannuskomponenttien suuruuteen on mahdollista vaikuttaa projektin aikana. Diplomityötä varten haastateltiin yhteensä 17 henkilöä. Haastateltavat muodostuivat Carunan SJ projektit, hankinta, kunnossapito ja yleissuunnittelu tiimien edustajista sekä ulkoisista sidosryhmistä kuten esisuunnittelu yhteistyökumppanista ja urakoitsijoista. Tutkimuksen lopputuloksena tunnistettiin useita kehitysehdotuksia kustannustehokkuuden lisäämiseksi Carunan sähköasemaprojekteissa. Työn tutkimuksia pohjustettiin sähköasema ja projektinhallinta teoriaan keskittyvällä kirjallisuusselvityksellä. Tämän lisäksi työn loppuun koottiin tiivis katsaus sähköverkkojärjestelmän ja sähköasemarakentamisen tulevaisuudennäkymistä sekä haastattelu- että kirjallisuuslähteitä hyödyntämällä. Diplomityön keskeinen päätelmä on, ettei kustannustehokkuutta ole mahdollista saavuttaa tekemällä jokin yksittäinen asiaa oikein. Kustannustehokas projekti on kustannustehokkaasti toteutettujen projektin elinkaarenvaiheiden muodostama kokonaisuus. Kustannustehokas 110/20 kV sähköasema rakennetaan laadukkaasti ja yhteistyöllä toteutetun suunnitelman pohjalta, mahdollisuuksien mukaan tarkoin valitulle tontille, laadukkailla ja luotettavilla laitteilla, jotka ovat turvallisia käyttää ja mahdollisimman huoltovapaita sekä alusta loppuun proaktiivisesti johdettuna. Tulevaisuuden sähköasemarakentamisessa tullaan huomioimaan entistä enemmän pitkäaikainen kustannustehokkuus ja elinkaariajattelu sekä ympäristölliset asiat. Moderni sähköasema on SF6-kaasu vapaa ja digitaalinen. Substation construction is being invested into more than ever before. This is partly due to the ongoing energy revolution, but also to the need to restructure aging network assets. Ensuring the cost-effective implementation of projects is a very topical issue for distribution system operators. In this thesis done for Caruna Oy, the possibilities for increasing the cost-effectiveness of the 110/20 kV substation project were investigated. The study was started with a cost structure study of the substation project. As a result of the study, the most important cost components in the substation project were identified with the help of three example projects. Based on the identified cost components, it was determined at which point in the project each component will be implemented and when its amount can be influenced. The study was supplemented by an interview study, which aimed to reveal how the amount of the cost components identified in the cost structure study can be influenced during the project. A total of 17 people were interviewed for the thesis. The interviewees consisted of Caruna’s HV projects, procurement, maintenance, and general design team representatives as well as external stakeholders such as pre-design partner and contractors. As a result of the study, several development proposals were identified to increase cost efficiency in Caruna’s substation projects. The research of the work was primed with a literature review focusing on the substation and project management theory. In addition, a concise overview of the prospects of the electricity grid system and substation construction was compiled by utilizing both interview and literature sources. The key conclusion of the thesis is that it is not possible to achieve cost efficiency by doing some individual part correctly. A cost-effective project is an entity formed by the cost-effective stages of a project's life cycle. A cost-effective 110/20 kV substation is built on a high-quality and collaborative plan, on a carefully selected site where possible, with high-quality and reliable equipment that is safe to use and as maintenance-free as possible, and proactively managed from start to finish. Long-term cost-effectiveness and life cycle thinking, as well as environmental issues, will be considered even more in the construction of future substations. The modern substation is SF6 gas free and digital.

The rates of ecosystem degradation and biodiversity loss are alarming and current conservation efforts are not sufficient to stop them. The need for new tools is urgent. One approach is biodiversity offsetting: a developer causing habitat degradation provides an improvement in biodiversity so that the lost ecological value is compensated for. Accurate and ecologically meaningful measurement of losses and estimation of gains are essential in reaching the no net loss goal or any other desired outcome of biodiversity offsetting. The chosen calculation method strongly influences biodiversity outcomes. We compare a multiplicative method, which is based on a habitat condition index developed for measuring the state of ecosystems in Finland to two alternative approaches for building a calculation method: an additive function and a simpler matrix tool. We examine the different logic of each method by comparing the resulting trade ratios and examine the costs of offsetting for developers, which allows us to compare the cost-effectiveness of different types of offsets. The results show that the outcomes of the calculation methods differ in many aspects. The matrix approach is not able to consider small changes in the ecological state. The additive method gives always higher biodiversity values compared to the multiplicative method. The multiplicative method tends to require larger trade ratios than the additive method when trade ratios are larger than one. Using scoring intervals instead of using continuous components may increase the difference between the methods. In addition, the calculation methods have differences in dealing with the issue of substitutability.

Global deterioration of marine ecosystems, together with increasing pressure to use them, has created a demand for new, more efficient and cost-efficient monitoring tools that enable assessing changes in the status of marine ecosystems. However, demonstrating the cost-efficiency of a monitoring method is not straightforward as there are no generally applicable guidelines. Our study provides a systematic literature mapping of methods and criteria that have been proposed or used since the year 2000 to evaluate the cost-efficiency of marine monitoring methods. We aimed to investigate these methods but discovered that examples of actual cost-efficiency assessments in literature were rare, contradicting the prevalent use of the term “cost-efficiency.” We identified five different ways to compare the cost-efficiency of a marine monitoring method: (1) the cost–benefit ratio, (2) comparative studies based on an experiment, (3) comparative studies based on a literature review, (4) comparisons with other methods based on literature, and (5) subjective comparisons with other methods based on experience or intuition. Because of the observed high frequency of insufficient cost–benefit assessments, we strongly advise that more attention is paid to the coverage of both cost and efficiency parameters when evaluating the actual cost-efficiency of novel methods. Our results emphasize the need to improve the reliability and comparability of cost-efficiency assessments. We provide guidelines for future initiatives to develop a cost-efficiency assessment framework and suggestions for more unified cost-efficiency criteria. My aim in this chapter is to inspect how neoliberal economic policy and rationale are enmeshed with conceptions of body, gender and health in the contemporary (primarily Western) cultural sphere and how they have been addressed in research literature, particularly concerning fatness and the fat body. In recent years, the fat body has been and is a target of intensifying biopolitical control. My point of departure is that different elements of neoliberal culture have found something to latch onto and exploit in the concept of the fat body. Social institutions such as health care policies; or structures, such as health care systems; not to mention social, moral, and political orders of the day, all contribute to the conceptual (as well as physical) shaping of fat bodies. In the age of neoliberalism, even biopolitical control is neoliberally attuned (Lemke, 2001). I will present how neoliberalism has come to inform and steer our understanding of bodies, how we live in them, and the relationship we are supposed to enjoy with them. My goal here is twofold. I will look at the ways in which the fat body has been made intelligible in the broader context of neoliberal culture. Secondly, I am interested in the role of neoliberalism in moulding “acceptable” and “unacceptable”, healthy and unhealthy gendered bodies and subjects. peerReviewed

Objectives: Frailty increases the risks of hospitalization, institutionalization, and death. Our objective was to study the effects of home-based physical exercise on the number of days spent at home among pre frail and frail persons, versus usual care. In addition, utilization and costs of health care and social services, cost-effectiveness, and health-related quality-of-life (HRQoL) were explored. Design: Randomized controlled trial, with year-long supervised exercise for 60 minutes twice a week versus usual care. Follow-up for 24 months after randomization. Setting and Participants: A sample of 299 home-dwelling persons in South Karelia, Finland. Main inclusion criteria: >65 years, meeting at least 1 of the frailty phenotype criteria, Mini-Mental State Examination score >17. Methods: Primary outcome, days spent at home over 24 months, was calculated deducting days in inpatient care, in nursing homes, and days after death. HRQoL was assessed (15D questionnaire) at baseline and at 3, 6, and 12 months. Utilization data were retrieved from medical records. Results: The participants' mean age was 82.5 (SD 6.3), 75% were women, 61% were pre-frail and 39% frail. After 24 months, there was no difference between groups in days spent at home [incidence rate ratio 1.03; 95% confidence interval (CI) 0.98-1.09]. After 12 months, the costs per person-year were 1.60-fold in the exercise group (95% CI 1.23-1.98), and after 24 months, 1.23-fold (95% CI 0.95-1.50) versus usual care. Over 12 months, the exercise group gained 0.04 quality-adjusted life-years and maintained the baseline 15D level, while the score in the usual care group deteriorated (P for group Conclusions and Implications: Physical exercise did not increase the number of days spent at home. Exercise prevented deterioration of HRQoL, and in the frail subgroup, all intervention costs were compensated with decreased utilization of other health care and social services over 24 months. Maailmanlaajuinen kilpailu pakottaa yrityksiä pyrkimään kustannustehokkuuteen ja ketteryyteen kaikissa prosesseissa, myös taloushallinnon osalta. Taloushallinnon tulisi osoittaa olevansa toiminnoiltaan tehokas; virheettömyys ja täsmällisyys eivät enää riitä. Samalla tulisi toimia entistä nopeammin ja kustannustehokkaammin; tukien koko yrityksen kilpailukykyä. Ostolaskujen käsittelyä on yleisesti pidetty taloushallinnon eniten resursseja vievänä prosessina ja siksi sen automatisoinnista ja tehostamisesta voidaan saada merkittävää hyötyä. Keinoälyn ja robotiikan kehitys on jo vaikuttanut ja tulee yhä vaikuttamaan taloushallinnon kehitykseen Suomessa lähitulevaisuudessa. Tämän vuoksi on aiheellista tutkia, kuinka laskujenkäsittelyprosessia voidaan nykyään kehittää monimuotoisessa projektiliiketoiminnassa. Tämän työn tavoitteena oli luoda käytännön toimenpide-ehdotuksia kohdeyrityksen laskujenkäsittelyprosessin tehostamiseksi ja selvittää prosessissa liikkuvan tiedon merkitystä projektiliiketoiminnalle. Työn tutkimuksessa käytettiin laadullista tutkimusotetta ja tutkimusmenetelmäksi valittiin osallistuva toimintatutkimus. Alan kirjallisuudesta löydettiin tälle työlle teoreettinen viitekehys laskujenkäsittelyprosessin tehostamisesta projektiliiketoiminnassa, jota voitiin hyödyntää tutkimuksessa. Työn tutkimuksen tuloksissa korostuvat alan kirjallisuudessakin havaittujen projektiliiketoiminnan erityispiirteiden vaikutukset laskujenkäsittelyprosessiin. Tutkimustuloksina kuvattiin laskujenkäsittelyprosessin nykytila ja havaittiin useita mahdollisuuksia parantaa prosessin tehokkuutta. Työn tuloksia kohdeyritys voi käyttää päätöksenteon tukena suunniteltaessa uutta toimintatapaa ja mahdollisia järjestelmäinvestointeja. Käytännön toimenpide-ehdotusten lisäksi tässä diplomityössä dokumentoidaan ainutlaatuista tietoa laskujenkäsittelyprosessin kehittämisestä projektiliiketoimintaa harjoittavassa suomalaisessa yrityksessä.

Hip fracture causes substantial burden for individual and society, with increased mortality and loss of function. The purpose of this study was to (1) examine the effects of specialized (physical and geriatric) rehabilitation on home-dwelling hip fracture patients 50 years or older on recovery one year after the fracture, (2) to evaluate the costs and cost-effectiveness of specialized rehabilitation modalities, (3) to study the recovery after hip fracture between home-dwelling male and female patients and (4) to determine recommendations for hip fracture rehabilitation concerning the general rehabilitation practices in Finland. A total of 538 consecutively, independently living patients with non-pathological hip fracture treated in Oulu University hospital, were randomized to one of the three rehabilitation modalities: privately-based rehabilitation unit (physical rehabilitation), geriatric department (geriatric rehabilitation) and healthcare centre hospital (control group). Patients were evaluated on admission, at 4 and 12 months for social status, residential status, walking ability, use of walking aids, pain in the hip, activities of daily living and mortality. Costs were evaluated by recording the use of healthcare service and the prices were obtained from Diagnosis Related Group (DRG) price list for the hospital and from a publication of the National Research and Development Centre for Welfare and Health. Mortality was lower in the physical rehabilitation group 4 and 12 months after the fracture compared to geriatric and the control rehabilitation group. Physical and geriatric rehabilitation improved the ability of independent living after 4 months, but this effect could not be seen after 12 months. The rehabilitation costs were higher in the physical rehabilitation than in the control group, but the total healthcare-related costs one year after the fracture were lower in the physical rehabilitation group than in the control. Male and female patients recovered similarly after hip fracture. Age, poor functional status before the fracture and high ASA-score increased the mortality risk. This thesis suggests that intensive mobilization and rehabilitation is a recommended practice after the hip fracture, resulting in better functional recovery, survival and lower economic costs compared to routine treatment. Poor recovery was predicted not by sex, but by prefracture function and morbidity. Tiivistelmä Lonkkamurtuma on yksi merkittävimmistä toimintakyvyn laskua ja kuolleisuutta aiheuttavista vammoista. Tämän väitöskirjatutkimuksen tarkoituksena oli (1) tutkia tehostetun (fysikaalisen ja geriatrisen) kuntoutuksen vaikutusta itsenäisesti asuvien, vähintään 50-vuotiaiden lonkkamurtumapotilaiden kuntoutumiseen murtumaa seuraavan vuoden aikana, (2) arvioida lonkkamurtuman hoidon kustannuksia ja kustannusvaikuttavuutta tehostetussa kuntoutusyksikössä, (3) tutkia lonkkamurtuman kuntoutumisen eroja mies- ja naispotilaiden välillä sekä (4) tarkentaa lonkkamurtuman hoitoketjun yleisiä suosituksia Suomessa. Tutkimuksessa seurattiin 538 lonkkamurtumapotilasta, jotka hoidettiin kirurgisesti Oulun yliopistollisessa sairaalassa. Valintakriteereihin kuului vähintään 50 vuoden ikä sekä kyky itsenäiseen asumiseen joko omassa kodissa tai kodinomaisessa ympäristössä ennen murtumaa. Lonkkaleikkauksen jälkeen potilaat satunnaistettiin yhteen kolmesta kuntoutusryhmästä: fysikaaliseen kuntoutusryhmään (Oulun Diakonissalaitos), geriatriseen kuntoutusryhmään (Oulun kaupunginsairaalan kuntoutusosasto) sekä kontrolliryhmään (terveyskeskussairaalassa tapahtuva kuntoutus). Seuranta-aika oli yksi vuosi. Potilaat haastateltiin ennen kuntoutusta, sekä neljän ja 12 kuukauden kuluttua murtumasta, joiden yhteydessä aineisto kerättiin koskien yleistä terveydentilaa, toiminta- ja kävelykykyä, asumismuotoa ja kuolleisuutta. Kustannukset arvioitiin terveyspalveluiden käytöstä ja näiden hintoina käytettiin sairaalan yksikköhintoja (DRG) ja sosiaali- ja terveysalan tutkimus- ja kehittämiskeskuksen (STAKES) yksikköhintoja. Fysikaalinen kuntoutus vähensi merkittävästi potilaiden kuolleisuutta neljän ja 12 kuukauden seurannoissa verrattuna geriatriseen kuntoutukseen sekä kontrolliryhmään. Lisäksi fysikaalinen ja geriatrinen kuntoutus lisäsivät lyhytaikaisesti potilaiden kykyä itsenäiseen asumiseen verrattuna kontrolliryhmään. Taloudellisten vaikutusten osalta fysikaalinen kuntoutusjakso oli huomattavasti tavanomaista kuntoutusta kalliimpi, mutta fysikaalisen kuntoutusryhmän kokonaiskustannukset vuoden aikana olivat pienemmät kuin kontrolliryhmän potilailla. Sukupuolella ei ollut vaikutusta potilaiden toimintakykyyn, laitostumiseen tai kuolleisuusriskiin murtuman jälkeen. Merkittäviksi kuolleisuutta ennustaviksi tekijöiksi osoittautuivat ikä, toimintakyky ennen murtumaa sekä leikkauskelpoisuusluokitus. Tulokset puoltavat tehostetun, erikoistuneessa kuntoutusyksikössä suoritetun hoidon käyttöä lonkkamurtumapotilailla sekä terveydellisten että taloudellisten syiden osalta.

The emergence of cloud computing induces opportunities for software vendors and for organizations with great storage needs. Indeed, software companies have begun to adopt cloud technologies and offer their software as a service to customers instead of selling software products. As a result, their revenue models have changed, and the diversity and complexity of the resulting pricing models have created both opportunities and impediments to competition in the market. On the other hand, due to the exponentially growing volume of digital content, organizations have an increasing demand for storage and need to choose between private, public, and hybrid storage solutions. This decision requires consideration of various determinants; cost is one of the most vital factors. This dissertation investigates the impact of cloud computing technologies on the IT industry, with a focus on the financial aspects of business models. That is, this research uses quantitative, qualitative, and analytical-mathematical research methods, as well as simulations, to find ways to improve companies’ financial performance by enhancing their pricing models and reducing their storage costs. In particular, this research contributes to the cloud pricing literature by proposing a pricing framework developed specifically for the cloud industry, by providing insights into the relationship between software architecture and pricing, and by providing a view on how cloud technology adoption has changed software pricing models. In addition, this research contributes to the cloud cost literature and to the concurrent sourcing literature by providing analytical models that capture the effects that various cost determinants have on the cost- efficiency of private versus public storage and the cost of hybrid storage. This research indicates that, in the case of exponentially growing storage demand, reestimating future needs and acquiring the necessary in-house infrastructure usually makes the private cloud more cost-effective than the public cloud; the total cost of hybrid storage decreases as well. In addition, this research sheds light on various details regarding storage cost reduction, such as the roles of volume uncertainty and volume variability in hybrid storage costs, the time point when reestimating hybrid storage needs results in the greatest cost benefits, and the magnitude of these achievable cost benefits.

The European Union's Water Framework Directive requires that all water bodies should achieve good ecological status at latest by 2027. A spreadsheet tool was developed to identify cost-effective solutions at a catchment scale in Finland. The tool is designed to meet the needs of regional river basin management planning in Finland. It can be used to compare individual measures by their cost-effectiveness or by their achievable phosphorus reduction rate. In addition to this, combinations of measures can be built and compared by their costs and reductions. Uncertainty analysis of the tool is carried out with the Monte Carlo simulation. The tool has been piloted in eight different Finnish river basins during 2011–2013. The case studies have shown that the tool can support the allocation of financial resources better; improve communication between experts, authorities and local stakeholders; and add realism in river basin management planning by giving estimates of the achievable loading reductions.

Harvesting branches, unmerchantable tree tops and stumps for bioenergy reduces the carbon stock and the sink capacity of forest. We analyzed forest management changes that are financially viable for a forest owner to compensate for carbon loss resulting from the forest harvest residue extraction, and thus lead to truly carbon-neutral forest bioenergy. The management options studied included forest fertilization, elongated rotation periods, varying the type of forest residues extracted, and leaving high stumps. The costs of carbon loss compensation varied widely from 5 to 4000 € ha − 1 between the management options. The lowest costs resulted from harvesting quickly decomposing branches combined with low levels of fertilization. Harvesting all residues and applying intensive fertilization regimes or postponing final felling generated the highest costs. A requirement for fast carbon loss compensation increased the costs. The results indicated that changes in the forest management improve the carbon benefits of forest bioenergy, and some of these changes are inexpensive for the forest owner. The optimization results suggested that the longer time period was allowed for the carbon loss compensation, the fewer cost-effective silvicultural measures existed in the optimal combination of management regimes for the compensation.

Recent advances in wireless communications coupled with the widespread of bandwidthhungry and high-power consuming wireless services as well as the rapid penetration of multimedia-enabled smart handheld devices generate a drastic growth in mobile traffic volume. With these trends, the goals of 5G and beyond wireless communications include energy efficiency, low latency, and low service cost from the user’ perspective as well as spectrum efficiency, revenue increasing, and lower capital and operational cost from the network operators’ point of view. This thesis studies the potential of mobile clouds for improving energy, spectral, and cost efficiency towards the primary goals of 5G networks. Mobile clouds define a promising 5G integrated technology which combines the conventional cellular and shortrange networks into a novel and powerful communication architecture. Mobile clouds can be seen as a flexible and efficient platform for cooperative content delivery, cooperation, resource sharing, and social networks. Research work in this thesis is conducted in two main areas, namely fundamental and applied research on mobile clouds. Fundamental research focuses on cloud formation strategies and cloud maintenance mechanisms. In the applied area, the potential of mobile clouds for content distribution is investigated in both licensed and unlicensed bands. For content distribution using unlicensed band, several cooperative strategies are designed and their efficiency is compared to traditional multicast strategy in terms of energy efficiency, content distribution latency, and users’ quality of experience (QoE). In content distribution over licensed band, spectrum sharing and resource allocation algorithms are considered for multi pair and full-duplex device-to-device (D2D) communications, aiming at improving spectral efficiency and network performance as well as quality of service (QoS) provisioning. In addition, a business model is designed to consider how mobile clouds can reduce the cost of users and operators while improving revenue of operators and increasing users’ satisfaction. The results of this thesis show that mobile clouds technology creates a flexible and efficient platform which can efficiently enhance spectral efficiency, significantly improve energy efficiency and network performance while guaranteeing cost efficiency for users and network operators simultaneously. Tiivistelmä Langattomassa viestinnässä on viime aikoina saavutettu useita edistysaskeleita samalla kun suurta kaistanleveyttä edellyttävien ja paljon tehoa kuluttavien langattomien palveluiden käyttö on yleistynyt ja multimediatoiminnoilla varustetut älylaitteet ovat vallanneet nopeasti markkinoita. Tämän seurauksena mobiililiikenne on lisääntynyt huomattavasti. Näiden suuntausten valossa langattoman 5G-viestinnän ja sitä seuraavien sukupolvien langattoman viestinnän kehitystavoitteita ovat muun muassa seuraavat: käyttäjän näkökulmasta energiatehokkuus, mahdollisimman lyhyet viiveajat ja palvelun käytöstä aiheutuvien kustannusten edullisuus sekä verkko-operaattorin näkökulmasta spektritehokkuus, tulojen kasvattaminen ja aiempaa matalammat pääoma- ja käyttökustannukset. Tässä väitöskirjassa tarkastellaan millaisia mahdollisuuksia mobiilipilvet tarjoavat energia-, spektri- ja kustannustehokkuuden parantamiseen ja siten 5G-verkkojen päätavoitteiden saavuttamiseen. Mobiilipilvi on lupaava osa 5G-teknologiaa, jossa perinteiset solukkoverkot ja lyhyen kantaman verkot yhdistyvät aivan uudenlaiseen ja tehokkaaseen tietoliikennearkkitehtuuriin. Mobiilipilvet voidaan nähdä joustavana ja tehokkaana alustana sisällön yhteisjakelulle, yhteistyölle, resurssien jakamiselle ja toiminnalle sosiaalisissa verkoissa. Tätä väitöskirjaa varten on tehty tutkimusta sekä mobiilipilviä koskevan perustutkimuksen että soveltavan tutkimuksen parissa. Perustutkimuksessa keskitytään tarkastelemaan pilvien muodostamisessa käytettäviä strategioita ja pilvien ylläpidossa käytettäviä mekanismeja. Soveltavan tutkimuksen alueella mobiilipilvien tarjoamia sisällön jakelun mahdollisuuksia tarkastellaan sekä luvanvaraisilla että vapaasti käytettävillä kaistoilla. Vapaasti käytettävillä kaistoilla toteutettavaa jakelua varten työssä kehitetään useita yhteistoimintastrategioita, joiden tehokkuutta verrataan perinteiseen monilähetysstrategiaan energiatehokkuuden, sisällön jakelun viiveiden ja käyttäjien kokemuksen laadun (Quality of Experience, QoE) osalta. Luvanvaraisilla kaistoilla toteutettavan sisällön jakelun osalta tarkastellaan moniparisessa ja samanaikaisessa molemminsuuntaisessa laitteiden välisessä viestinnässä (D2D-kommunikaatiossa) käytettäviä spektrin jakamisen ja resurssien allokoimisen algoritmeja, jotta spektritehokkuutta ja verkon suorituskykyä sekä palvelun laatua (Quality of Service, QoS) pystyttäisiin parantamaan. Tämän lisäksi työssä kehitellään liiketoimintamallia, jonka avulla voidaan pohtia, miten mobiilipilvillä voidaan vähentää käyttäjien ja operaattorien kustannuksia ja samalla kasvattaa operaattorien tuloja ja parantaa käyttäjien tyytyväisyyttä. Tämän työn tulokset osoittavat, että mobiilipilviteknologia on joustava ja tehokas alusta, jolla voidaan lisätä olennaisesti spektritehokkuutta, parantaa merkittävästi energiatehokkuutta ja verkon suorituskykyä sekä taata kustannustehokkuus sekä käyttäjien että verkko-operaattorien näkökulmasta.

Julkisilla hankinnoilla voidaan edistää ympäristön ja talouden kestävyystavoitteiden saavuttamista. Cleantech hankinnat tarkoittavat sellaisia teknologisia ratkaisuja, jotka merkittävästi vähentävät hankinnan elinkaarenaikaisia ympäristövaikutuksia, edistävät uusiutuvan energian käyttöä ja materiaalitehokkuutta sekä hyödyntävät digitaalista tiedonhallintaa. Parhaimmillaan cleantech hankinta vastaa hankkijan tarpeisiin tarjoamalla laadukkaan, kustannustehokkaan ja ympäristönäkökulmasta selvästi paremman ratkaisun. Laajimmillaan kyse on innovatiivisten ratkaisujen ja uusien toimintatapojen synnyttämisestä, joilla on välillisesti myös myönteisiä alueellisia ja elinkeinopoliittisia vaikutuksia. Tässä raportissa tarkastellaan, mitä cleantech tarkoittaa julkisen hankkijan näkökulmasta ja mitä cleantech hankinnat ovat eri sektoreilla. Tutkimuksen mukaan julkiset investoinnit suuntautuvat lähitulevaisuudessa pääosin rakentamiseen ja peruskorjaukseen, vesihuollon ja teiden infrastruktuuriin, sekä energiantuotantoon ja jakeluun, joissa kaikissa avautuvat suuret mahdollisuudet cleantech hankinnoille. Myös palveluhankinnoissa cleantechilla on monia mahdollisuuksia. Onnistuneen cleantech hankinnan avaintekijöitä ovat avoin ja laaja markkinavuoropuhelu, verkostojen hyödyntäminen, huolellinen ja riittävän pitkä valmisteluaika, joustava rahoitusratkaisu sekä johdon strateginen tuki. Lisäksi tarvitaan ahkera ja määrätietoinen hankkija. Cleantech ratkaisujen edistäminen hankinnoissa voi käytännössä vaatia hankintakulttuurin muutosta, jotta näkökulma tulee osaksi kaikkien hankintojen suunnittelua. Tutkimushankkeen aikana vahvistui käsitys, että hankkijoiden tarve uusiin ideoihin, niiden käytännön toteuttamiskeinoihin ja muiden kokemuksiin cleantech hankinnoista on suuri. Hankkeessa käytiin läpi onnistuneita esimerkkejä toteutuneista cleantech hankinnoista, haastateltiin suomalaisia hankkijoita sekä edistettiin hankintoja käytännössä. Esimerkit ja haastattelut koottiin tämän raportin lisäksi avoimeen nettiportaaliin: Cleantech Hankintamappi (www.ymparisto.fi/hankintamappi) johon hankkijat voivat myös itse lisätä tekemiään hankintoja.

Epidemiological studies can often be designed in several ways, some of which may be more optimal than others. Possible designs may differ in the required resources or the ability to provide reliable answers to the questions under study. In addition, once the data are collected, the selected modeling approach may affect how efficiently the data are utilized. The purpose of this dissertation is to investigate efficient designs and analysis meth ods in follow-up studies with longitudinal measurements. A key question is how to select optimally a subcohort for a new longitudinal covariate measurement if we cannot afford to measure the entire cohort. Another key question we consider is how to determine the reasonable number of longitudinal measurements. Different ways to utilize longitudinal covariate measurements in modeling cardiovascular disease (CVD) mortality are also studied. Follow-up data are modeled using parametric or semiparametric proportional haz ards models. Subcohort selections are carried out using optimality criteria initially developed for optimal experimental design. Measures of model discrimination are ap plied to plan the number of longitudinal measurements. The topics are studied using simulations and the East–West data, which are Finnish part of an international follow- up study in the field of cardiovascular epidemiology, the Seven Countries Study. This work demonstrates that the cost-efficiency of follow-up designs can be improved by careful planning. The proposed method for selecting optimal subcohorts is shown to outperform simple random sampling and it is demonstrated how the number of longi tudinal measurements can be determined using simulated data and data from previous similar studies. The results also indicate that individual-level changes and cumulative averages of classical risk factors are good predictors of CVD mortality.

Suomen valtion talousahdingon, alati kallistuvan puolustusmateriaalin ja Euroopan kiristyneen turvallisuustilanteen ristipaineessa on puolustushallinto joutunut tekemään suuria muutoksia toimintansa tehostamiseksi. Puolustusvoimauudistuksessa vuosina 2012–2014 toteutettiin yksi Puolustusvoimien suurimmista rauhanajan organisaatiomuutoksista. Muutoksella tavoiteltiin säästöjä toiminnan menoihin, jotka ovat edustaneet perinteisesti kolmasosaa Suomen puolustusbudjetista. Tehostamistoimien jälkeen herää kysymys, miten tehokkaasti loppuosa rahoituksesta käytetään. Tässä tutkielmassa arvioidaan vallitsevaa Puolustusvoimien tilannetta taloudelliselta kannalta ja pyritään tunnistamaan puolustushallinnon materiaalihankintoihin liittyviä tehostamiskohteita. Tutkielman alkupuolella esitellään Suomessa käytetyn sotatalouden käsitteen sisältöä ja maailmalla vakiintuneen, mutta Suomessa kohtuullisen tuntemattoman maanpuolustuksen taloustieteen sovelluksia sekä näiden kahden käsitteen välistä suhdetta. Talousteoreettisista ajatuksista käsitellään tarkemmin organisaation tehokkuuteen vaikuttavia ilmiöitä, periaatteita ja kustannustehokkuuden arviointiin soveltuvia menetelmiä. Empiriaa edustavassa tutkielman osassa tuodaan julki puolustushallinnon ajattelu- ja menettelytapoja. Niitä hallitsevat paitsi byrokraattinen käyttäytyminen, myös vaikeus nähdä puolustushallinnon materiaalihankkeita investointeina. Ajattelua rajoittaa puolustusvalmiuden abstraktius julkishyödykkeenä. Hyötyjä ei tunnisteta eikä niiden suuruutta osata arvioida saati mitata. Puolustushallinnon materiaalihankintojen kilpailutukset edellyttävät erityisen huolellista valmistelua hankintaorganisaatiolta. Se realisoituu raskaana byrokraattisena toimintana, jossa henkilöstön työajan käytöllä ei tunnisteta olevan mitään kustannusta. Merkittävässä roolissa tehottomuuksien takana voidaan nähdä olevan se, että varsinkin Puolustusvoimissa taloudelliset suunnitelmat ja päätökset tehdään maallikoiden eli talouskoulutusta saamattomien henkilöstöryhmien toimesta. Pääsääntöisesti tämä tarkoittaa upseereita, jotka eivät saa urallaan kaupallista saati taloustieteellistä opetusta. Tutkielman johtopäätöksinä esitetään vastaus tutkimuskysymykseen, millaista tehostamisvaraa puolustushallinnon materiaalihankinnoissa on taloustieteellisten tehokkuusperiaatteiden valossa tarkasteltuna. Havainnot jalostuvat suosituksiksi, joilla tähdätään kustannustehokkuuden kasvattamiseen.

This paper investigates the nature of quality costs and the likelihood of their having financial impacts in the public sector. An illustrative quality development case project in an accounting department of a Finnish city administration is presented. The empirical data includes answers to a qualitative project question sheet, workshop material and participative observation. Quality problems are classified according to their quality cost category and the extent to which they are avoidable. Some of the quality costs proved unavoidable and some are likely to lack financial impact, exhibiting sticky cost behaviour. The study indicates that certain prevention and appraisal costs in the public sector might not deliver the desired outcomes because of such sticky behaviour of costs. Traditional quality thinking holds that increased prevention and appraisal investments decrease failure costs. However, the extent to which these costs are avoidable has not been analysed in previous studies. peerReviewed
