Abstract Purpose Obstructive sleep apnea (OSA) is a common sleep-breathing disorder. Numerous investigations have found a strong inherent relationship between OSA and hypothyroidism. Studies suggest that lymphocytes may be involved in the development of hypothyroidism in patients with OSA. This study aimed to assess the association between lymphocytes and hypothyroidism in OSA patients. Patients and methods This study involved 920 patients with OSA who underwent nocturnal sleep monitoring, thyroid function testing, and routine blood tests. In patients with OSA, logistic regression analysis indicated independent predictors of hypothyroidism. The cutoff level of lymphocyte count was determined using a receiver operating characteristic (ROC) analysis to predict the occurrence of hypothyroidism in individuals with OSA. Results This study comprised 920 OSA patients (617 males and 303 women), 879 with normal thyroid function, and 41 with hypothyroidism, with a hypothyroidism incidence of 4.46%. In the entire OSA population and male OSA patients, the number of lymphocytes was significantly higher in the hypothyroid group than in the control group (p = 0.002 and 0.020, respectively). In addition, among the OSA population younger than 60 years old and patients with mild to moderate OSA, lymphocytes were found to be considerably more in the hypothyroid group than in the euthyroid group. Lymphocyte count, ESS, and sex were all independent predictors of hypothyroidism development in OSA patients. According to ROC curve analysis, the risk of hypothyroidism increases with increasing lymphocyte count in the total patient population, with an optimal diagnostic cutoff point of 2.5 (× 10*9/L). Conclusions The prevalence of hypothyroidism in patients with OSA increases as the number of lymphocytes increases. Lymphocyte count can be used as an independent predictor of the occurrence of hypothyroidism, and it has a diagnostic value for OSA combined with hypothyroidism.

BackgroundThere is a considerable amount of controversy regarding the treatment and prognosis of adult patients with Chiari malformation type I (CM-I) at home and abroad; furthermore, no large-sample, long-term, follow-up studies have examined CM-I patients with syringomyelia (SM) comparing posterior fossa decompression with resection of tonsils (PFDRT) vs. posterior fossa decompression with duraplasty (PFDD).ObjectiveThis study retrospectively analyzed the factors affecting the treatment and long-term prognosis of adults with CM-I combined with SM.MethodsWe retrospectively analyzed data from 158 adult CM-I patients combined with SM who underwent PFDRT or PFDD, including 68 patients in group PFDRT and 90 patients in group PFDD. Abstract Background Ferroptosis is iron-dependent non-apoptotic cell death, that is characterized by the excessive accumulation of lipid peroxides. Ferroptosis-inducing therapy also shows promise in the treatment of cancers. However, ferroptosis-inducing therapy for glioblastoma multiforme (GBM) is still in the exploratory stage. Methods We identified the differentially expressed ferroptosis regulators using Mann–Whitney U test in the proteome data from Clinical Proteomic Tumor Analysis Consortium (CPTAC). We next analyzed the effect of mutation on protein abundance. A multivariate Cox model was constructed to identify the prognostic signature. Results In this study, we systemically portrayed the proteogenomic landscape of ferroptosis regulators in GBM. We observed that some mutation-specific ferroptosis regulators, such as down-regulated ACSL4 in EGFR-mutated patients and up-regulated FADS2 in IDH1-mutated patients, were linked to the inhibited ferroptosis activity in GBM. To interrogate the valuable treatment targets, we performed the survival analysis and identified five ferroptosis regulators (ACSL3, HSPB1, ELAVL1, IL33, and GPX4) as the prognostic biomarkers. We also validated their efficiency in external validation cohorts. Notably, we found overexpressed protein and phosphorylation abundances of HSPB1 were poor prognosis markers for overall survival of GBM to inhibit ferroptosis activity. Alternatively, HSPB1 showed a significant association with macrophage infiltration levels. Macrophage-secreted SPP1 could be a potential activator for HSPB1 in glioma cells. Finally, we recognized that ipatasertib, a novel pan-Akt inhibitor, could be a potential drug for suppressing HSPB1 phosphorylation, inducing ferroptosis of glioma cells. Conclusion In summary, our study characterized the proteogenomic landscape of ferroptosis regulators and identified that HSPB1 could be a candidate target for ferroptosis-inducing therapy strategy for GBM.

Regions in high latitudes are experiencing greater climate change than other regions. In this study, the impact of temperature and precipitation on the yield of fifteen cultivars (eight barley [Hordeum vulgare], four oat [Avena sativa], and three wheat [Triticum aestivum]) from a small grain field experiment in Fairbanks and Delta Junction, Alaska, from 1978 to 2018 were analyzed to evaluate their sensitivity to variation in climate that occurred during this period. BackgroundThe best surgical treatment of Chiari malformation patients with syringomyelia remains controversial, and whether cerebellar tonsillectomy should be performed has not been decided.ObjectiveTo evaluate the efficacy of posterior fossa decompression with duraplasty (PFDD) and Posterior fossa decompression with resection of tonsils (PFDRT) in patients of Chiari malformation type I (CM-I) with syringomyelia and explore relevant factors affecting prognosis.Patients and methodsWe retrospectively analyzed 182 adult patients of CM-I with syringomyelia who underwent PFDD or PFDRT over a 6-year period, and analyzed their clinical manifestations, imaging features, and follow-up data. Clinical outcomes were assessed using the Chicago Chiari Outcome Scale (CCOS), and imaging outcomes were assessed using the syrinx remission rate. Difference comparisons were performed to compare the differences between different surgical groups. Influencing factors associated with outcome were investigated using bivariate analysis and multiple linear regression analysis.ResultsThere were statistically significant differences in CCOS score (p = 0.034) and syrinx remission rates (p = 0.046) between the PFDRT group and the PFDD group after surgery. Regression analysis showed that preoperative motor dysfunction, cerebellar-related symptoms and different surgical methods may have influenced the CCOS score and that brainstem-related symptoms and age may have influenced the syrinx remission rates in the total patient group (p < 0.05). Regression analysis showed that the duration of symptoms, cerebellar-related symptoms and preoperative syrinx diameter may have influenced the CCOS score and that the preoperative cerebellar tonsillar hernia distance may have influenced the postoperative syrinx remission rate in the PFDRT group (p < 0.05). Age and length of hospital stay may have influenced the CCOS score, and brainstem-related symptoms and age may have influenced the syrinx remission rates in the PFDD group (p < 0.05).ConclusionThis study showed that the CCOS score in the PFDRT group was better than that in the PFDD group. Preoperative motor dysfunction, cerebellar-related symptoms, and different surgical methods in patients of CM-I with syringomyelia affected postoperative CCOS score. Both the duration of symptoms and the age of the patients should be actively considered as factors influencing prognosis. Symptomatic CM-I patients with syringomyelia should undergo surgical treatment as early as possible.

BackgroundHypothyroidism could cause obstructive sleep apnea (OSA), however, the specific association of them remained unclear. This cross-sectional study aimed to determine the prevalence of hypothyroidism among patients with OSA, and the characteristics and predictors of hypothyroidism associated with OSA.MethodsA total of 573 patients with OSA were included in the study. Serum levels of thyroid stimulating hormone (TSH), free triiodothyronine (FT3) and free thyroxine (FT4) were measured in all participants. Univariate and binary logistic regression analysis were performed to assess the association of OSA with hypothyroidism while controlling for potential confounders. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the OSA effect in the distinction between euthyroid and hypothyroidism.ResultsThe prevalence of hypothyroidism was 6.75%、5.12%、10.38% in the total, men, and women cohort, respectively, and the prevalence rate in women OSA patients was significantly higher than that in men OSA patients (P=0.018). The men OSA patients with hypothyroidism had a higher Epworth sleepiness scale (ESS) than women OSA patients with hypothyroidism (P=0.022). Additionally, the ESS was significantly higher in men OSA patients with hypothyroidism than those with euthyroid (P=0.042), while women OSA patients had no such difference (P=0.822). In men patients with OSA, ROC curve analyses revealed that the risk of hypothyroidism increased in accordance with increasing ESS after adjustment for potential confounders, and the optimal cutoff value was 10 score. Craniopharyngiomas (CPs) are histologically benign tumors located in the sellar–suprasellar region. Although the transcriptome development in recent years have deepened our knowledge to the tumorigenesis process of adamantinomatous craniopharyngioma (ACP), the peritumoral immune infiltration of tumor is still not well understood. In this study, weighted gene coexpression network analysis (WGCNA) was applied to identify different gene modules based on clinical characteristics and gene expression, and then, the protein–protein interaction (PPI) network with the Cytohubba plug-in were performed to screen pivotal genes. In addition, immune cell infiltration (ICI) analysis was used to evaluate the immune microenvironment of ACP patients. In total, 8,568 differential expression genes were identified based on our datasets and two microarray profiles from the public database. The functional enrichment analysis revealed that upregulated genes were mainly enriched in immune-related pathways while downregulated genes were shown in the hormone and transduction of signaling pathways. The WGCNA investigated the most relevant modules, and 1,858 hub genes was detected, from which the PPI network identified 14 pivotal genes, and the Hypoxia-inducible factor 1-alpha (HIF-1α) pathway including four critical genes may be involved in the development of ACP. Moreover, naïve CD4+ and CD8+ T cells were decreased while specific subtypes of T cells were significantly increased in ACP patients according to ICI analysis. Validation by immunofluorescence staining revealed a higher expression of HIF-1α in ACP (ACP vs. control) and adult-subtype (adult vs. children), suggesting a possible state of immune system activation. Notably, children with low HIF-1α scores were related to the hypothalamus involvement and hydrocephalus symptoms. In this study, we successfully identified HIF-1α as a key role in the tumorigenesis and development of ACP through comprehensive integrated analyses and systematically investigated the potential relationship with immune cells in ACP. The results may provide valuable resources for understanding the underlying mechanisms of ACP and strengthen HIF-1α as a potential immunotherapeutic target in clinical application.

Objective. The aim of the study is to explore the characteristics of systolic blood pressure (SBP) and heart rate (HR) changes in patients undergoing percutaneous balloon compression (PBC) for trigeminal neuralgia and analyze the factors that influence the formation of the symbolic pear shape of the balloon, which signifies successful compression. Methods. We retrospectively analyzed the changes in systolic blood pressure (SBP) and heart rate (HR) in 103 consecutive patients with trigeminal neuralgia (TN). Fifty-five patients who underwent operations with intermittent inflating cuff blood pressure (IICBP) monitoring were classified into the IICBP group. The other 48 patients who underwent continuous intra-arterial blood pressure (CIABP) monitoring were classified into the CIABP group. Results. Among all the patients, there were more women than men and the patients in both the groups more commonly had TN on the right side and involving branch III. First, the balloon appeared as “pear-shaped” when the compression was effective, and the SBP increased an average of 59.04% in the CIABP group. CIABP provided us the precise range of the increase. Older patients had higher SBPs, especially patients with hypertension. Second, SBP was more sensitive and lasted much longer than HR in the process of puncturing the foramen ovale and compressing the ganglion. SBP fluctuated much more in the CIABP group than in the IICBP group. Third, the median time taken in the CIABP group was less than the IICBP group, and the prognosis of the CIABP group was better than the IICBP group. Conclusion. Effective ganglion compression significantly increased SBP. CIABP can monitor SBP in real time and can also safeguard the compression process. CIABP is a safe and effective method in the PBC process that is worthy of promotion and application.

We compared the effects of adjuvant RT after surgery, gross total resection (GTR), and subtotal resection (STR) on progression-free survival (PFS) and overall survival (OS) in patients with AMs, respectively.MethodsWe performed a systematic review and meta-analysis of the literature published in PubMed, Embase, and the Cochrane Library from inception to February 1, 2021, to identify articles comparing the PFS and OS of patients receiving postoperative RT after surgery, GTR and STR.ResultsWe identified 2307 unique studies; 24 articles including 3078 patients met the inclusion criteria. Craniopharyngiomas (CPs) are benign tumors arising from the sellar region. However, little is known about their clinical features and long-term recurrence due to low morbidity and the lack of large cohort studies. Thus, we aimed to develop nomograms to accurately predict the extent of resection and tumor recurrence using clinical parameters. A total of 545 patients diagnosed with CP between 2009 and 2019 were examined: 381 in the development cohort and 164 in the validation cohort. Least absolute shrinkage and selection operator (LASSO) and Cox regression analyses were performed to establish two nomograms. Receiver operating characteristic (ROC) curves, calibration curves, decision curve analysis (DCA) and Kaplan-Meier (KM) curves were used to evaluate their predictive performance and discriminative power, respectively, in the two cohorts. In addition, the EORTC QLQ-BN20 questionnaire was used to assess neuropsychological status in the follow-up. In the development cohort, the area under the curve (AUC) and C-index were 0.760 and 0.758, respectively, for predicting the extent of resection and 0.78 and 0.75, respectively, for predicting 3-year progression-free survival (PFS) and 5-year PFS. Additionally, the model had a predictive accuracy of 0.785. Both nomograms showed acceptable discrimination in the two cohorts. Moreover, DCA demonstrated excellent clinical benefits from the two nomograms. Finally, participants were classified into two distinct risk groups according to the risk score, and an online calculator was created for convenient clinical use. During long term follow-up, hypothyroidism (77.61%) and hypocortisolism (76.70%) were the most common endocrine dysfunction after surgery and significant deficits were observed concerning visual disorder, motor dysfunction and seizures in the recurrent groups. In particular, better quality of life was associated with gross total resection (GTR), postoperative radiation, anterior interhemispheric (AI) approach and transsphenoidal approach. To our knowledge, these are the first nomograms based on a very large cohort of patients with CP that show potential benefits for guiding treatment decisions and long-term surveillance. The current study demonstrated the online calculator serve as the practical tool for individual strategies based on the patient’s baseline characteristics to achieve a better prognosis.

Agriculture in the boreal and Arctic regions is perceived as marginal, low intensity and inadequate to satisfy the needs of local communities, but another perspective is that northern agriculture has untapped potential to increase the local supply of food and even contribute to the global food system. Policies across northern jurisdictions target the expansion and intensification of agriculture, contextualized for the diverse social settings and market foci in the north. However, the rapid pace of climate change means that traditional methods of adapting cropping systems and developing infrastructure and regulations for this region cannot keep up with climate change impacts. Moreover, the anticipated conversion of northern cold-climate natural lands to agriculture risks a loss of up to 76% of the carbon stored in vegetation and soils, leading to further environmental impacts. The sustainable development of northern agriculture requires local solutions supported by locally relevant policies. There is an obvious need for the rapid development of a transdisciplinary, cross-jurisdictional, long-term knowledge development, and dissemination program to best serve food needs and an agricultural economy in the boreal and Arctic regions while minimizing the risks to global climate, northern ecosystems and communities.

Recent accelerations of climate warmings can open agricultural opportunities in the region of Interior Alaska. In this paper, a simulation of spring wheat growth forced with projected climate scenarios was conducted by the Decision Support System for Agrotechnology Transfer (DSSAT) crop model. The model was calibrated and validated using experimental data.Using an Alaskan cultivar (cv.) Ingal and a baseline covering 1989–2018, projected changes in days to maturity and yield were simulated following the Representative Concentration Pathways (RCP) 4.5 (medium–low emissions) and RCP8.5 (high emissions) climate change scenarios. For each RCP scenario, spring wheat growth was simulated in the time series covering 2020–2049 (indicated as 2035 s), 2050–2079 (2065 s), and 2080–2099 (2090 s). The baseline value of days to maturity was 69 and yield resulted in 1956 kg ha−1. Results show that under RCP4.5 and RCP8.5 2035 s, 2065 s, and 2090 s scenarios, days to maturity decrease, ranging from 64 to 55 days, and changes in yield range from a 3% increase to a 6% decrease.Adaptation by increasing the cultivar’s growing degree day requirement resulted in 69 and 68 days to maturity in RCP4.5 2035 s and RCP8.5 2035 s, respectively, which in turn increased yields 5% and 7%, respectively. Increased soil water at planting from 80 to 85% field capacity, due to increased annual precipitation, resulted in additional yield increases. This indicates that selecting spring wheat varieties to maintain similar baseline days to maturity and agronomic practices that store fall/winter precipitation are of importance to materialize future spring wheat yield increases of Interior Alaska.

Barley heading date has advanced in Fairbanks (64.83° N, 147.77° W), Alaska, USA. However, it is unclear if this advance coincidently causes weather pattern changes around heading and leads to yield loss. Using the Variety Trial and weather data in Fairbanks and Delta Junction (64.05° N, 145.60° W) from 1991 to 2018, two barley cultivars were selected to analyze the yield and weather trends, the yield variation explained by weather, and the effect of extreme weather on yield. The results showed that the heading date of ‘Otal’ significantly advanced and yield significantly declined in Fairbanks while there were no heading and yield changes of ‘Otal’ in Delta Junction and of ‘Thual’ in both Fairbanks and Delta Junction. The weather pattern changed around heading due to advanced heading of ‘Otal’ in Fairbanks. The climate factors at 7–10 days around heading explained over 50% of ‘Otal’ yield variation in Fairbanks. The results suggest that ‘Otal’ can still be good to plant in Delta Junction but not in Fairbanks. To cope with the climate change in Alaska, the farmers should increase the diversity of barley cultivars, select non-photoperiod sensitive cultivars and cultivars with longer duration from planting to heading, and sow late to avoid the impact on heading and yield.

This work evaluated the level and nature of water-extractable soil organic matter (WEOM) from a 6-yr potato ( L.) crop rotation field experiment. The content of WEOM was higher in continuous potato soils than in the 2-yr and 3-yr crop rotation soils except for those crop rotation soils with soil improvement management by composted manure. Irrigation increased the level of WEOM in crop rotation soils. Ultraviolet–visible and fluorescence parameters indicated that WEOM in continuous potato soil possessed a high degree of humification and that crop rotation increased the aromatic and low molecular mass portions. In most irrigated soils, WEOM contained less aromatic but higher humified components. Characterization of the WEOM samples with crop rotation and irrigation suggests that these management practices stimulate the decomposition of the humic fraction in soil organic matter pools, implying healthier soil conditions with these management practices than with continuous potato growth.

As the acreages of agricultural lands increase, changes in surface energetics and evapotranspiration (ET) rates may arise consequently affecting regional climate regimes. The objective of this study was to evaluate summertime ET dynamics and surface energy processes in a subarctic agricultural farm in Interior Alaska. The study includes micrometeorological and hydrological data. Results covering the period from June to September 2012 and 2013 indicated consistent energy fractions: LE/Rnet (67%), G/Rnet (6%), H/Rnet (27%) where LE is latent heat flux, Rnet is the surface net radiation, G is ground heat flux and H is the sensible heat flux. Additionally actual surface evapotranspiration from potential evaporation was found to be in the range of 59 to 66%. After comparing these rates with those of most prominent high latitude ecosystems it is argued here that if agroecosystem in high latitudes become an emerging feature in the land-use, the regional surface energy balance will significantly shift in comparison to existing Arctic natural ecosystems.

Sequential fractionation coupled with phosphatase hydrolysis allows a greater understanding of the effects of animal manure on the chemical distribution of soil P. Concentrations of specific soil P fractions were determined after long-term (>10 yr) poultry litter application at rates of 4.5, 6.7, 9.0, 11.2, and 13.4 Mg manure ha-1 yr-1 to watershed-scale plots (cultivated and grazed-ungrazed pasture) on a calcareous Texas Blackland Vertisol. Soil total extractable P (Pt) and inorganic P (Pi) were quantified following sequential extraction with H2O, NaHCO3, NaOH, and HCl. Hydrolyzable organic P (Pe) and non-hydrolyzable organic P (Pne) were determined in the extracted fractions following enzymatic hydrolysis. Litter application increased Pt regardless of land-use type compared with the control. Concentrations of H2O-extractable Pi in litter-amended plots increased by 9 to 34% (cultivated) and 7 to 30% (pasture) over the control, indicating substantial risk of soluble P runoff. Labile organic P (Po) extracted with H3O and NaHCO3 decreased in the order monoester > nucleic acid > phytate > Pne. An average of 68% of Pt was extractable with HCl. Organic P comprised the majority (95%) of HCl-Pt; however, only trace levels of HCl-Po were hydrolyzable, and litter application increased HCl-Pne up to 217%. Thus, litter application increased levels of both soluble Pi and stable Pne, but the specific response varied with application rate and land management. This study increased understanding of P chemical distribution with time in litter-amended soil with high clay and CaCO3 contents under differing land-use scenarios. [ABSTRACT FROM AUTHOR]

Finally, we recognized that ipatasertib, a novel pan-Akt inhibitor, could be a potential drug for suppressing HSPB1 phosphorylation, inducing ferroptosis of glioma cells. Water extractable organic matter (WEOM) is critical for soil fertility because of its role as a readily available C substrate for microbial metabolism. In the semi‐arid Texas High Plains, dryland cropping systems are common because of limited precipitation and irrigation water. Little is known about the long‐term effects of dryland cropping on soil WEOM content and composition, and how these differ from native grassland. The objective of this study was to use soils collected near Amarillo, TX, in 1977 and 2013 to examine the long‐term (72 yr total cropping length) effects of dryland cropping systems [continuous wheat and wheat–fallow rotations] and tillage methods (disk plow, stubble‐mulch tillage, and delayed stubble‐mulch tillage) on WEOM characteristics, compared with native grassland. In cropped plots, water‐extractable organic carbon (WEOC) concentrations, a measure of WEOM content, were depleted regardless of tillage or crop, with reductions up to 13% in 1977 and 60% in 2013, compared with native grassland in 2013. Other parameters of WEOM changed over time, where cropped soils from 2013 had proportionally more WEOM in aromatic, phenolic, and lignin‐like forms, attributed to recently incorporated crop residues. In contrast, WEOM in cropped soils from 1977 and native grassland (2013) contained more functional groups (e.g., aliphatic, aromatic, polysaccharide, phenol, etc.) and humic‐like substances than cropped soils in 2013. This suggests that long‐term dryland cropping led to depletion of stable, humic‐like WEOM. Enhancement of WEOM stocks and long‐term sustainability in the Texas High Plains may require more vigorous conservation systems than currently used. Previous studies have shown a strong correlation between topographic/soil features and agricultural production; however, linkages between these features and agricultural insurance products are scarce. Agricultural insurance is an ever‐growing means of governmental support for producers globally. However, failure to set insurance premiums that accurately reflect risk exposure can lead to low participation rates and/or adverse selection. The U.S. federal crop insurance program partly guards against this at the farm level by inducing pricing heterogeneity via a rate multiplier curve, which does not consider topographic/soil information. We develop a method for econometrically incorporating this information into existing rating procedures used by the Risk Management Agency (RMA). The empirical application leverages 149,267 farm‐level observations of Kansas producers across four dryland crops (corn, soybean, sorghum, and wheat), spanning forty‐six years, and matched to fine‐scale topographic/soil features. The results suggest that incorporating these features does improve the prediction accuracy of yield losses and can, in general, improve rating performance. However, these improvements are specific to farms with limited yield histories, as there are no improvements for farms with the commonly used yield history of ten years. This suggests substantial rating improvements for new farms or those with limited histories for a particular crop, but more general improvements for the program are not likely to occur given a large number of current participants with a full ten‐year yield history. In the USA, western Washington (WWA) and the Alaska (AK) Interior are two regions where maritime and continental climates, high latitude and cropping systems necessitate early maturing spring wheat (Triticum aestivum L.). Both regions aim to increase the production of hard spring bread wheat for human consumption to support regional agriculture and food systems. The Nordic region of Europe has a history of breeding for early maturing spring wheat and also experiences long daylengths with mixed maritime and continental climates. Nordic wheat also carries wildtype (wt) NAM-B1 , an allele associated with accelerated senescence and increased grain protein and micronutrient content, at a higher frequency than global germplasm. Time to senescence, yield, protein and mineral content were evaluated on 42 accessions of Nordic hard red spring wheat containing wt NAM-B1 over 2 years on experimental stations in WWA and the AK Interior. Significant variation was found by location and accession for time to senescence, suggesting potential parental lines for breeding programmes targeting early maturity. Additionally, multiple regression analysis showed that decreased time to senescence correlated negatively with grain yield and positively with grain protein, iron and zinc content. Breeding for early maturity in these regions will need to account for this potential trade-off in yield. Nordic wt NAM-B1 accessions with early senescence yet with yields similar to regional checks are reported.
