Purpose The use of face masks has been proposed to cause or exacerbate the symptoms of dry eye disease (DED), which has been widely discussed under the term mask-associated dry eye (MADE). However, no studies have systematically investigated tear film parameters during the use of different face masks. Therefore, the objective of the present study was to investigate clinically relevant parameters of the tear film before and during the short-time use of face masks in dry and normal eyes. Methods In a prospective study, the tear film parameters of 42 DED patients and 42 healthy volunteers were examined while wearing different types of face masks in a randomized order. This included measurements of non-invasive tear break-up time (NIBUT), lipid layer thickness, tear meniscus height, and bulbar redness after 30 min of wearing no mask, a surgical face mask or an FFP2/K95 mask. The equivalence of the means was assessed using the two one-sided t-test (TOST) method. Results In healthy volunteers’ lipid layer thickness, NIBUT and tear meniscus height were not significantly altered by 30 min of surgical or FFP2 mask wear (p > 0.016). The use of either type of mask was significantly associated with decreased bulbar redness (p p > 0.016). Conclusions Based on these results, the short-term wearing of face masks, regardless of type, did not produce a significant difference in tear film parameters of lipid layer thickness, NIBUT, and tear meniscus in healthy or dry eyes, while bulbar redness was reduced after mask wear only in healthy volunteers.

Zusammenfassung Hintergrund Medizinische Register haben ihr Potenzial zur Versorgungsforschung in unterschiedlichen Bereichen des Gesundheitswesens bereits in der Vergangenheit zeigen können. Die Vereinigten Staaten von Amerika (USA) verfügen über das bekannte nationale ophthalmologische Register IRIS® (Intelligent-Research-In-Sight), auf dessen Grundlage Erkenntnisse zur ophthalmologischen Versorgungslage in den USA gewonnen werden können. Vergleichbare Projekte existieren in Deutschland gegenwärtig nicht. oregis ist das erste krankheitsübergreifende Register, das umfassende Daten auf dem Gebiet der Augenheilkunde in Deutschland zum Zweck der Versorgungsforschung sammelt. Methoden oregis trägt derzeit anonymisierte, klinische Patienteninformationen aus allen Teilbereichen der Augenheilkunde zusammen und stellt diese zu Forschungszwecken zur Verfügung. Teilnehmende Zentren lesen ihre Daten unentgeltlich und vorzugsweise automatisiert in das Register ein. Derzeit zählt die oregis-Parameterliste ca. 600 Parameter, wovon etwa 100 in der aktuellen Pilotphase eingesetzt werden. Ergebnisse Das deutsche ophthalmologische Register oregis hat seit seiner Initiierung Daten zu über 1,75 Mio. Arztbesuchen von über 400.000 Patientinnen und Patienten aus deutschen ophthalmologischen Zentren zusammengetragen. Bis zum Spätsommer 2022 wurden über 2,8 Mio. Visus- und gleich viele Tensiomessungen erfasst. Diskussion Ziel des Projektes ist die Einrichtung einer bundesweiten ophthalmologischen Datenbank, die die Grundlage für ein lernendes Gesundheitssystem auf dem Gebiet der Augenheilkunde bildet. Mithilfe einer fortwährenden Aktualisierung kann oregis auch kurzfristige Neuerungen in der augenärztlichen Versorgung abbilden.

Purpose To evaluate the standard of care, in particular the use of topical or subconjunctival interferon-α2b, in treating ocular surface squamous neoplasia or melanocytic tumours in tertiary eye centres in Germany. Methods A survey containing 14 questions was sent to 43 tertiary eye centres in Germany. The questions addressed the surgical and medical management of ocular surface squamous neoplasia and melanocytic tumours (primary acquired melanosis and malignant melanoma), as well as the clinical experiences and difficulties in prescribing off-label interferon-α2b eye drops and subconjunctival injections. Results Twenty-four tertiary eye centres responded to the survey. Eighty-three percent of centres had used interferon-α2b in their clinical practice and 25% prescribed it as the first-line cytostatic agent following surgical excision of ocular surface squamous neoplasia, while 10% would do so for melanocytic tumours. Correspondingly, the majority of respondents selected mitomycin C as their first-line agent. Side effects were uncommon with topical interferon-α2b eye drops but were more frequently reported after subconjunctival interferon-α2b injections. In total, eight centres had experience with interferon-α2b injections. The most significant obstacles perceived by ophthalmologists when prescribing interferon-α2b were its high cost and the reimbursement thereof. Conclusion Off-label mitomycin C was the preferred adjuvant therapy for epithelial and melanocytic tumours, with interferon-α2b being the standard second-line option. Interferon-α2b has predominantly been used to treat ocular surface squamous neoplasia and, to a lesser extent, melanocytic tumours at German tertiary eye centres. Following its market withdrawal, supply shortages of interferon-α2b are likely to have a profound impact on patient care and their quality of life.

Zusammenfassung Hintergrund Intravitreale operative Medikamenteneingaben (IVOM) gehören zu den häufigsten medizinischen Prozeduren überhaupt mit ca. 1,5 Mio. Eingriffen in Deutschland pro Jahr. Für diese enorme Versorgungsaufgabe gibt es nur wenige empirische Daten über den Versorgungsprozess und seine klinische und subjektive Wirkung. Material und Methoden Es wird die Entwicklung und der Aufbau des Hamburger Registers für intravitreale Injektionstherapien (QIVOM) detailliert beschrieben. IVOM-Patienten der drei großen Augenkliniken Hamburgs (Asklepios Kliniken Nord-Hamburg und Barmbek, Universitätsklinikum Hamburg-Eppendorf) werden im Rahmen der Routineversorgung mit IVOM zur Studienteilnahme eingeladen. Es werden subjektive Patientenangaben zum Krankheits- und Behandlungserleben erhoben und diese mit den patientenbezogenen medizinischen Parametern elektronisch ergänzt, pseudonymisiert und in einer elektronischen Datenbank erfasst. Ergebnisse Von den ersten 162 Studienpatienten (Alter 41–95 Jahre) war die Behandlungsindikation bei 64% exsudative altersbedingte Makuladegeneration (AMD), bei 22% retinaler Venenverschluss und bei 11% diabetisches Makulaödem. Es ergibt sich ein heterogenes Bild der Erkrankungsschwere und der subjektiven Beeinträchtigung. Eine Sehschärfe von 0,5 und besser wiesen 31,8% der Patienten am behandelten Auge im Vergleich zu 79,1% am Partnerauge auf. Die größte praktische Einschränkung betraf die Lesefähigkeit, bei der über ein Drittel erhebliche Einschränkungen erlebte. Autofahren war immerhin für 62% möglich. Eine Verbesserung der Sehfähigkeit durch IVOM erreichten 45% der Patienten. Schlussfolgerung Die Datenerfassung von patientenseitigen Daten und medizinischer Versorgungsdaten aus den Kliniken stellt den großen Vorteil des neu geschaffenen Registers dar. Die Erweiterung der Datenbasis soll in Zukunft vielfältige weiterführende Erkenntnisse erbringen und zur Qualitätssicherung in der IVOM-Therapie beitragen.

BACKGROUND/AIMS: Intravitreal rituximab is an off-label treatment option for primary vitreoretinal lymphoma (PVRL). The objective of this study was to monitor the therapeutic response and safety profile of intravitreal rituximab in a cohort of PVRL patients. METHODS: In this retrospective, uncontrolled, open label, multicentre study, 20 eyes from 15 consecutive patients diagnosed with PRVL received at least one intravitreal injection of 1mg in 0.1ml rituximab. Biodata of the PVRL patients was recorded as well as visual acuity and vitreous haze score immediately before rituximab intravitreal injection and at follow-up examinations. Intravitreal rituximab safety data was also recorded. Additional rituximab injections were made during control visits on a pro re nata (PRN) regime using increased vitreous haze to indicate recurrence. RESULTS: There was significant vitreous haze reduction (p=0.0002) followed by significant improvement of visual acuity (mean best visual acuity before therapy 0.57 logMAR, after therapy 0.20 logMAR (p=0.0228) during the follow-up time up to 4 years. Only mild ocular side effects were reported. Median follow-up time was 565 days (range, 7-1253 days). CONCLUSION: Intravitreal rituximab therapy shows promising PVRL regression without any severe side effects. Although our clinical data support rituximab as intravitreal therapy in PVRL disease, further study is warranted.

Das Spektrum der anasthesiologischen Verfahren in der Augenheilkunde hat sich in den letzten Jahrzehnten nicht wesentlich verandert, wobei die Lokalanasthesie deutlich in den Vordergrund geruckt ist. Die Entwicklung von minimal-invasiven Operationstechniken benotigt deutlich weniger Analgesie und okonomische Zwange zur Verkurzung der Operationszeiten, und der zunehmende Anteil ambulanter Augenoperationen fordert eine kurze und moglichst komplikationsarme Analgesie des Operationsgebietes. Trotzdem muss die Auswahl der Verfahren, die hier ausfuhrlich beschrieben werden, patientenindividuell geschehen. Die Allgemeinanasthesie hat weiterhin ihren Stellenwert insbesondere bei reduzierter Compliance und schwierigen Operationsverhaltnissen. Durch die enge Zusammenarbeit von Anasthesisten und Ophthalmochirurgen z. B. bei Analgosedierungen im ambulanten Bereich kann ein fur den Patienten sehr angenehmes Operationserleben geschaffen werden. Wichtiges Ziel sollte immer eine ausreichende Anxiolyse, Analgesie und bei speziellen Operationen ausreichende Akinesie sein.

Background/aim Conjunctival malignant melanoma (CMM) is a rare, but very aggressive tumor with a high metastasis rate. Not much is known about the CMM metastasis mechanisms. So far, epidermal growth factor (EGF) and its receptor (EGF-R) as well as macrophages and matrix metalloproteinase 9 (MMP-9) have been reported to lead to metastasis by epithelial-mesenchymal-transition and tumor migration in different solid tumors. Therefore, we evaluated whether EGF and EGF-R, CD68 and MMP-9 are altered in CMM samples in comparison to conjunctival nevi and healthy conjunctiva. Patients and methods EGF, EGF-R, the macrophage marker CD68 and MMP-9 expression were analyzed in human conjunctival melanoma (CMM, n=16), human conjunctival nevi (n=13) and disease-free human conjunctiva (controls, n=14) by immunohistology. Staining of each sample was evaluated using a standardized score ranging from negative (0) to triple positive (3). The groups were then compared by ANOVA, followed by Tukey's post-hoc test. Results A statistically significant increase of EGF was seen in CMM samples in comparison to conjunctival nevi (p=0.03). In contrast, no statistically significant differences in EGF-R expression were noted between the three groups. A statistically significant increase of CD68 was only seen in conjunctival nevi compared to controls (p=0.04). MMP-9 expression was similar in all groups. Conclusion In CMM, the study data demonstrated an up-regulation of EGF in comparison to conjunctival nevi. Hence, EGF might promote proliferation of CMM cells and induce the epithelial-mesenchymal transition. Therefore, our data suggest that an interplay between EGF and CMM might have a critical role in the developing CMM tumors and metastasis.

Zusammenfassung Hintergrund Im Bereich des Pflegedienstes wurden und werden Mindestpersonalbemessungen gefordert und umgesetzt. Die Bundesärztekammer hat die Deutschen Fachgesellschaften im Dezember 2019 nach ihren Erfahrungen zu Personalvorgaben für Ärztinnen und Ärzte im Krankenhaus befragt. Da hierzu in der Augenheilkunde bislang keine strukturiert erhobenen Daten vorlagen, gründete die DOG im März 2020 einen Arbeitskreis zur Bearbeitung dieser Anfrage. Methode Im Rahmen mehrerer Abstimmungsrunden erstellte der „Arbeitskreis Personalvorgaben für Ärztinnen und Ärzte im Krankenhaus“ der DOG einen Fragebogen zu den bisherigen Erfahrungen in der Personalausstattung und -planung von Augenkliniken, der im Mai 2020 an die Chefärzte und Direktoren aller deutschen Universitätsaugenkliniken und Hauptabteilungen für Augenheilkunde verschickt wurde. Der Fragenbogen beinhaltete Einzelfragen mit vorgegebenen und freien Antwortmöglichkeiten zur Vorgehensweise und Ist-Situation bei der Personalausstattung sowie zum Aufgaben- und Organisationsspektrum des Ärztlichen Dienstes der Abteilungen. Ergebnisse Von 104 angeschriebenen Leitern von Hauptabteilungen antworteten 53 (51 %). Davon entfielen 25 Antworten auf universitäre Abteilungen (64 % Antwortquote), 23 auf Abteilungen von öffentlichen oder frei-gemeinnützigen Trägern (46 %) und 5 auf private Klinikträger (33 %); 49 % gaben an, dass es keine transparente Berechnungsgrundlage der Anzahl ärztlichen Stellen ihrer Klinik gibt. Nur bei 47 % der befragten Abteilungen werden die ambulanten Aufgaben, Leistungen und Erlöse im Stellenschlüssel berücksichtigt. Es zeigte sich ein statistisch signifikant höheres Verhältnis an Vollzeitkräften pro Bettenanzahl in den Universitätsaugenkliniken gegenüber den privaten und öffentlich/frei-gemeinnützig geführten Augenkliniken (p Schlussfolgerung Das Ergebnis zeigt, wie heterogen und intransparent die Personalbemessung in den deutschen Hauptabteilungen für Augenheilkunde festgelegt wird. Die universitären Abteilungen haben im Schnitt höhere Personalressourcen in Bezug auf die Bettenkapazität für die Bewältigung zusätzlicher Aufgaben in Forschung und Lehre. In der Personalberechnung werden vielfältige Aufgaben, darunter auch ambulante Aufgaben, die deutlich zum Erlös einer Augenklinik beitragen, oft nicht berücksichtigt. Eine transparente und einheitliche Berechnungsgrundlage für das ärztliche Personal der Abteilungen ist zukünftig wichtig, um eine ausreichende Personalausstattung für ein patienten- und mitarbeiterorientiertes Arbeitsumfeld zu gewährleisten.

Purpose Dry eye symptoms after conventional cataract surgery are a very common problem. Until now, only few data are available on objective tear film parameters in regard to femtosecond laser-assisted cataract surgery (LCS). Aim of this study was therefore to analyze and compare tear film parameter changes between LCS and conventional cataract surgery. Methods A consecutive group of 34 patients, scheduled for cataract surgery, were randomly selected for either LCS or conventional cataract surgery (17 patients/group). Tear film assessments including tear film osmolarity, Schirmer test, MMP-9 analysis via quantitative ELISA, corneal sensitivity, corneal fluorescein staining, and conjunctival fluorescein staining were sequentially evaluated pre- as well as 1 and 3 months postoperatively. Results Both groups showed no significant difference in baseline characteristics. All surgeries were performed without any complications. After 1 and 3 months, there was no statistically significant difference in regard to tear film osmolarity (1 month: p = 0.81, 3 months: p = 1.0), Schirmer test (1 month: p = 0.35, 3 month: p = 0.08), and MMP-9 concentration (1 month: p = 0.36, 3 month: p = 0.28) between the two groups. Conclusions Neither LCS nor conventional cataract surgery affected objective tear film parameters significantly during our 3-month postoperative observation period. Hence, both surgical techniques can be equally used to treat patients without prior dry eye symptoms.

Zusammenfassung Hintergrund Bei der Behandlung von Glaukompatienten sind die Hauptziele die Bewahrung der Sehfähigkeit und Aufrechterhaltung einer möglichst hohen Lebensqualität bei volkswirtschaftlich akzeptablen Kosten. Daher ist es wichtig, jeden einzelnen Patienten sorgfältig zu evaluieren, um einen individuellen Behandlungsansatz zu erarbeiten. Material/Methoden Basierend auf den aktuellen Erkenntnissen, werden in der Übersicht die Vor- und Nachteile der medikamentösen Glaukomtherapie sowie der gängigen glaukomchirurgischen Methoden zusammengefasst. Die verschiedenen Wirkansätze der neuen minimal-invasiven Verfahren werden erläutert, die derzeit in Deutschland am häufigsten genutzten Verfahren vorgestellt sowie Empfehlungen für Vor- und Nachsorge gegeben. Ergebnisse und Diskussion Seit einigen Jahren stehen außer verschiedenen medikamentösen Tropftherapien und den klassischen operativen Verfahren, auch neue minimal-invasive Therapiealternativen zur Verfügung. Letztere eröffnen die Möglichkeit einer früheren chirurgischen Intervention, besonders für Patienten, die bereits initial oder im Laufe der Therapie für einen medikamentösen Ansatz ungeeignet erscheinen.

Hidradenitis suppurativa (HS) and psoriasis vulgaris (PSO) are chronic inflammatory dermatoses in which proinflammatory cytokines, such as IL-17, play a central role. The prevalence of keratoconjunctivitis sicca (KCS) is commonly higher in PSO than in healthy individuals. This study was thus set up to investigate the prevalence of KCS among patients with HS.In a cross-sectional study standardized tear film parameters and symptom-oriented questionnaires (OSDI, SPEED) were analyzed in a total of 71 subjects (HS n = 20, PSO n = 20, healthy controls n = 31). Additionally, IL-17 and MMP-9 in the tear film were analyzed. These parameters were correlated to the clinical severity of the skin disease. PSO patients served as inflammatory control group.There were statistically significant differences in OSDI (Data show that subjective complaints of KCS may be associated with HS and correlate with the severity of the respective Hurley stage, but do not involve alterations of tear film MMP-9 and IL-17. Clinicians should remain mindful that ocular complications in HS are often more vague than in psoriatic patients, but dry eye symptoms might be detrimental for the patients' quality of life.

Wir berichten uber einen 40-jahrigen Patienten mit seit 2 Wochen anhaltenden isolierten Metamorphopsien und visuellen Pseudohalluzinationen. Bei ophthalmologisch nicht wegweisenden Befunden erfolgte eine weiterfuhrende neurologische Diagnostik. Nach Diagnosestellung einer Anti-N-Methyl-D-Aspartat-(NMDA)Rezeptor-Enzephalitis, welche sich auf unterschiedlichste Weise psychiatrisch, neurologisch sowie ophthalmologisch ausern kann, erhielt der oligosymptomatische Patient eine Hochdosiskortikosteroidtherapie kombiniert mit intravenoser Immunglobulintherapie, worunter die Symptome sistierten.

Beim Einsatz von glaukomchirurgischen subkonjunktivalen/subtenonalen Ab-interno-Verfahren sind die Selektion der Patienten sowie deren praoperatives Management essenziell zur Verminderung des postoperativen Vernarbungsprozesses des Sickerkissens. Ziel der Arbeit war das Zusammenstellen der Risikofaktoren, die sich ungunstig auf den Vernarbungsprozess auswirken konnen. Literatursuche in PubMed mit Bezug auf den Vernarbungsprozess beim Gelimplantat XEN® (Allergan GmbH, Frankfurt am Main, Deutschland). Ophthalmologische Risikofaktoren fur den konjunktivoskleralen/tenonalen Wundheilungsprozess sind unter anderem das trockene Auge, Allergien, die Verwendung von konservierungsmittelhaltigen Augentropfen sowie vorangegangene Operationen am Auge. Die praoperative Gabe von kunstlichen Tranenersatzmitteln, Kortikosteroiden sowie die grundsatzliche Umstellung auf konservierungsmittelfreie Augentropfen konnen die Normalisierung einer chronisch entzundlich veranderten Bindehaut unterstutzen und somit den Wundheilungsprozess nach dem Eingriff positiv beeinflussen.

Purpose To provide an update of summary of risk factors and side effects of long-term use and contamination of intravitreal anti-VEGF injections. Methods We reviewed relevant publications of the topic of contamination of anti-VEGF injections and long-term side effects due to this reason. Results Due to the long-term use of anti-VEGF drugs and the higher number of injections worldwide, various studies have shown side effects in recent years, ranging from increased intraocular pressure to visual disturbing silicone oil vesicles in the vitreous cavity. Several studies have demonstrated that both the drug and the processing, storage, environmental factors and the material and design of the syringes have a decisive influence on these side effects. Conclusion The risks of deposits from syringes in the eye can be significantly reduced by various optimizations in transport, storage and syringe and cannula selection.

A CyPass®-microstent (Alcon, Fort Worth, TX, USA) extending too far into the anterior chamber should be trimmed as close as possible to the scleral spur to avoid progression of endothelial cell reduction.After CyPass implantation, if there is a significant loss of corneal endothelial cells due to the Cypass microstent extending too far into the anterior chamber, trimming or removal of the CyPass stent is necessary.There are no contraindications.As shown in our video, which is available online, a special cutter (19 Gauge Ahmed Micro Stent Cutter, MicroSurgical Technology Inc, Redmond, WA, USA) is inserted into the anterior chamber via a 1.5-mm wide corneal paracentesis made directly opposite to the CyPass stent. It is then possible to trim the anterior part of the stent. The severed fragment is removed using the head of the forceps. Finally, the previously inserted viscoelastic agent can be aspirated and the paracentesis can be hydrated.After the surgery vision testing as well as control of intraocular pressure and location of the stent are carried out. Antibiotic eye drops and ointment are postoperatively applied.There is still no standardized protocol for the procedure to trim the CyPass stent. Performing the trimming in our clinic using the procedure described here has so far not led to any complications. Long-term data about the development of the endothelial cell measurement after CyPass trimming are not yet available.OPERATIONSZIEL: Der zu weit in die Vorderkammer ragende CyPass®-Mikrostent (Alcon, Fort Worth, TX, USA) wird möglichst nah am Sklerasporn gekürzt, um einen fortschreitenden Endothelzellverlust zu verhindern.Kommt es nach CyPass-Implantation zu einem signifikanten Hornhautendothelzellverlust bei weit in die Vorderkammer ragendem CyPass-Mikrostent, besteht die Indikation zur Kürzung oder Entfernung.Es bestehen keine Kontraindikationen.Wie in dem von uns gezeigten Video, das online zur Verfügung steht, wird über eine korneale 1,5 mm breite Parazentese gegenüber dem CyPass-Stent ein spezieller Cutter (19 Gauge Ahmed Micro Stent Cutter, MicroSurgical Technology Inc, Redmond, WA, USA) in die Vorderkammer eingeführt. Dieser ermöglicht die Kürzung des anterioren Anteils des Stents. Das abgetrennte Stück wird mithilfe des Zangenkopfs geborgen. Zum Schluss wird das vorher eingebrachte Viskoelastikum abgesaugt, und die Parazentesen werden hydriert.Nach der Operation werden eine Visusprüfung sowie eine Tensions- und Lagekontrolle des Stents durchgeführt. Es werden postoperativ antibiotische Augentropfen und Augensalbe appliziert.Zum Verfahren der CyPass-Kürzung besteht noch kein standardisiertes Protokoll. Bei den bislang in unserer Klinik durchgeführten CyPass-Kürzungen mit dem hier beschriebenen Verfahren kam es zu keinerlei Komplikationen. Langzeitdaten über den Verlauf der Endothelzellmessungen nach CyPass-Kürzung liegen noch nicht vor.

Zusammenfassung Hintergrund In Deutschland sind 2 intravitreal anzuwendende Kortikosteroide (Dexamethason und Fluocinolon) für die Behandlung des diabetischen Makulaödems (DMÖ) zugelassen. Der Einsatz von DEX-Implantat bei DMÖ ist in der täglichen Praxis bisher nicht im Detail definiert. Mithilfe eines Delphi-Panels und einer Runde von Retinaspezialisten sollte ein Konsens für die Anwendung von DEX-Implantat erarbeitet werden. Material und Methoden Aus der Literatur wurden internationale und nationale Behandlungsempfehlungen identifiziert. Eine Steuerungsgruppe erarbeitete einen Katalog von 72 Aussagen zur Ätiologie und Pathogenese des DMÖ, Therapie mit dem DEX-Implantat, Einsatz bei mit VEGF-Hemmern vorbehandelten Patienten, Verwendung von DEX-Implantat in der Kombination mit anderen Therapien, Sicherheit von DMÖ-Therapien sowie Therapiebelastung des Patienten. 22 niedergelassene Ophthalmologen und 6 Ophthalmologen aus der Klinik gaben über Survey Monkey ihre Bewertung zu den Aussagen ab. Die Definition des Konsenses zu einer Aussage galt als erfüllt, wenn mehr als 75% der Befragten einer Aussage zustimmten bzw. nicht zustimmten. Über nicht konsensfähige Aussagen wurde nach erneuter Diskussion in der Konsensrunde nochmals abgestimmt. In der Folge sollte auch ein Behandlungsschema für DMÖ mit fovealer Beteiligung vorgeschlagen werden. Ergebnisse Sofern ein Patient unter VEGF-Hemmern unzureichend anspricht (Visusgewinn Schlussfolgerungen Der hier vorgestellte Algorithmus entspricht den Ergebnissen des Delphi-Prozesses und weicht u. U. von den Empfehlungen der Fachgesellschaft ab. Die im Rahmen der Befragung und Expertenrunde erarbeiteten Konsensempfehlungen für die Therapie von DMÖ können für den Einsatz des DEX-Implantats in der täglichen Praxis dienen.

Purpose Impairment of corneal nerves can lead to neurotrophic keratopathy accompanied with severe ocular surface damage, which due to limited treatment options, can result in severe visual deterioration. This study evaluates a possible new treatment by enhancing the corneal nerve regeneration using a Rho Kinase inhibitor (Y27632). ROCK is known to play an important role in regulating cell morphology, adhesion and motility but little is known about its role in corneal nerve regeneration. Methods Effects of ROCK inhibition on murine peripheral nerves was assessed in single cell- and wound healing assays as well as a 3D in vitro model. Furthermore, Sholl analysis evaluating neuronal branching and life-death assays evaluating toxicity of the inhibitor were performed. An in vivo mouse model was established, with monitoring weekly corneal nerve regrowth using confocal microscopy. Additionally, corneal nerve fiber length was evaluated by immunofluorescence staining. Underlying pathways were examined by qrtPCR. Results ROCK inhibition leads to a significant enhancement of fiber growth in vitro. Sholl analysis revealed a higher degree of branching of treated fibers. Cytotoxicity assay showed no influence of Y27632 on cellular survival. In vivo measurement revealed significant enhanced regeneration after injury in the treated group. QrtPCR of trigeminal ganglia confirmed ROCK knock-down as well as altered pathways. Conclusion The inhibition of ROCK after corneal nerve injury resulted in an enhanced regrowth of fibers in vitro and in vivo. This might be a step towards a new therapeutic concept for the treatment of impaired corneal nerves in diseases such as neurotrophic keratopathy.

Intravitreal drug delivery belongs to the most common medical procedures, with approximately 1.5 million treatments per year in Germany. However, for this enormous challenge to provision of proper care, there is a lack of empirical data regarding the treatment process and its clinical and subjective effect.This publication presents the development and structure of the Hamburg register for intravitreal therapies (QIVOM). Patients undergoing intravitreal injection therapy were invited to join the register study. Patients were recruited from the Hamburg eye clinics in Heidberg-Nord and Barmbek as well as from the University Medical Center Hamburg-Eppendorf (UKE). Both subjective patient-reported information about the perception of their eye disease and its treatment as well as medical parameters were stored in a central electronic database.Of the first 162 study patients (aged 41-95 years), 64% suffered from wet age-related macular degeneration (AMD), 22% had a retinal vein occlusion, and 11% a diabetic macula edema. Disease severity and subjective impairment were heterogenous. Among these patients, 31.8% had a visual acuity above 10/20 on the treated eye compared to 79.1% on the other eye. The reduced ability to read was the most relevant limitation for more than one third of patients. However, 62% were able to drive a vehicle. An improvement of vision through intravitreal therapy was experienced by 45%.Collection of patient-reported as well as treatment-related data comprises the advantage of the newly created register. This extension of the data basis should deliver new findings in the future and contribute to quality assurance in intravitreal care.HINTERGRUND: Intravitreale operative Medikamenteneingaben (IVOM) gehören zu den häufigsten medizinischen Prozeduren überhaupt mit ca. 1,5 Mio. Eingriffen in Deutschland pro Jahr. Für diese enorme Versorgungsaufgabe gibt es nur wenige empirische Daten über den Versorgungsprozess und seine klinische und subjektive Wirkung.Es wird die Entwicklung und der Aufbau des Hamburger Registers für intravitreale Injektionstherapien (QIVOM) detailliert beschrieben. IVOM-Patienten der drei großen Augenkliniken Hamburgs (Asklepios Kliniken Nord-Hamburg und Barmbek, Universitätsklinikum Hamburg-Eppendorf) werden im Rahmen der Routineversorgung mit IVOM zur Studienteilnahme eingeladen. Es werden subjektive Patientenangaben zum Krankheits- und Behandlungserleben erhoben und diese mit den patientenbezogenen medizinischen Parametern elektronisch ergänzt, pseudonymisiert und in einer elektronischen Datenbank erfasst.Von den ersten 162 Studienpatienten (Alter 41–95 Jahre) war die Behandlungsindikation bei 64% exsudative altersbedingte Makuladegeneration (AMD), bei 22% retinaler Venenverschluss und bei 11% diabetisches Makulaödem. Es ergibt sich ein heterogenes Bild der Erkrankungsschwere und der subjektiven Beeinträchtigung. Eine Sehschärfe von 0,5 und besser wiesen 31,8% der Patienten am behandelten Auge im Vergleich zu 79,1% am Partnerauge auf. Die größte praktische Einschränkung betraf die Lesefähigkeit, bei der über ein Drittel erhebliche Einschränkungen erlebte. Autofahren war immerhin für 62% möglich. Eine Verbesserung der Sehfähigkeit durch IVOM erreichten 45% der Patienten.Die Datenerfassung von patientenseitigen Daten und medizinischer Versorgungsdaten aus den Kliniken stellt den großen Vorteil des neu geschaffenen Registers dar. Die Erweiterung der Datenbasis soll in Zukunft vielfältige weiterführende Erkenntnisse erbringen und zur Qualitätssicherung in der IVOM-Therapie beitragen.

The range of anesthesiological procedures in ophthalmology has not changed significantly over the last decades, with local anesthesia clearly coming to the fore. The development of minimally invasive surgical techniques requires significantly less analgesia and economic constraints to shorten operating times. The increasing proportion of outpatient ophthalmic surgery requires a short analgesia of the surgical area with as few complications as possible. Nevertheless, the selection of the procedures, which are described here in detail, must be made individually for each patient. General anesthesia continues to have its place, especially in cases of reduced compliance and difficult surgical conditions. The close cooperation between anesthetists and ophthalmic surgeons, e.g. for analgosedation in the outpatient area, can create a very pleasant surgical experience for the patient. An important goal should always be sufficient anxiolysis, analgesia and, in special operations, sufficient akinesia.Das Spektrum der anästhesiologischen Verfahren in der Augenheilkunde hat sich in den letzten Jahrzehnten nicht wesentlich verändert, wobei die Lokalanästhesie deutlich in den Vordergrund gerückt ist. Die Entwicklung von minimal-invasiven Operationstechniken benötigt deutlich weniger Analgesie und ökonomische Zwänge zur Verkürzung der Operationszeiten, und der zunehmende Anteil ambulanter Augenoperationen fordert eine kurze und möglichst komplikationsarme Analgesie des Operationsgebietes. Trotzdem muss die Auswahl der Verfahren, die hier ausführlich beschrieben werden, patientenindividuell geschehen. Die Allgemeinanästhesie hat weiterhin ihren Stellenwert insbesondere bei reduzierter Compliance und schwierigen Operationsverhältnissen. Durch die enge Zusammenarbeit von Anästhesisten und Ophthalmochirurgen z. B. bei Analgosedierungen im ambulanten Bereich kann ein für den Patienten sehr angenehmes Operationserleben geschaffen werden. Wichtiges Ziel sollte immer eine ausreichende Anxiolyse, Analgesie und bei speziellen Operationen ausreichende Akinesie sein.

The aim of this study was to compare the efficacy and safety profile of XEN microstent implantation with trabeculectomy (TET) in a comparable group of open-angle glaucoma cases in a retrospective, monocentric, single-surgeon setting. The main goals of glaucoma treatment are to preserve the visual function and maintain as high a quality of life as possible at a cost acceptable to society. Therefore, it is crucial to carefully observe each individual patient in order to determine an individual and personalized treatment approach.This article summarizes the advantages and disadvantages of medicinal glaucoma treatment as well as traditional methods of glaucoma surgery, based on the current state of knowledge. The article explains the various mechanisms of action of new minimally invasive procedures, introduces the methods mostly commonly used in Germany and gives recommendations for preoperative care and postoperative follow-up.In addition to the plethora of medicinal glaucoma treatments and classical surgical procedures, new minimally invasive treatment alternatives have become available in the past few years. The latter are an option for an earlier surgical intervention, especially in naïve or previously treated patients who appear to be unsuitable for medicinal treatment.HINTERGRUND: Bei der Behandlung von Glaukompatienten sind die Hauptziele die Bewahrung der Sehfähigkeit und Aufrechterhaltung einer möglichst hohen Lebensqualität bei volkswirtschaftlich akzeptablen Kosten. Daher ist es wichtig, jeden einzelnen Patienten sorgfältig zu evaluieren, um einen individuellen Behandlungsansatz zu erarbeiten.Basierend auf den aktuellen Erkenntnissen, werden in der Übersicht die Vor- und Nachteile der medikamentösen Glaukomtherapie sowie der gängigen glaukomchirurgischen Methoden zusammengefasst. Die verschiedenen Wirkansätze der neuen minimal-invasiven Verfahren werden erläutert, die derzeit in Deutschland am häufigsten genutzten Verfahren vorgestellt sowie Empfehlungen für Vor- und Nachsorge gegeben.Seit einigen Jahren stehen außer verschiedenen medikamentösen Tropftherapien und den klassischen operativen Verfahren, auch neue minimal-invasive Therapiealternativen zur Verfügung. Letztere eröffnen die Möglichkeit einer früheren chirurgischen Intervention, besonders für Patienten, die bereits initial oder im Laufe der Therapie für einen medikamentösen Ansatz ungeeignet erscheinen.

Purpose: Quantification of corneal confocal microscopy (CCM) images has shown a significant reduction in corneal nerve fiber length (CNFL) in a range of peripheral neuropathies. We assessed whether corneal nerve fractal dimension (CNFrD) analysis, a novel metric to quantify the topological complexity of corneal subbasal nerves, can differentiate peripheral neuropathies of different etiology. Methods: Ninety patients with peripheral neuropathy, including 29 with diabetic peripheral neuropathy (DPN), 34 with chronic inflammatory demyelinating polyneuropathy (CIDP), 13 with chemotherapy-induced peripheral neuropathy (CIPN), 14 with human immunodeficiency virus-associated sensory neuropathy (HIV-SN), and 20 healthy controls (HCs), underwent CCM for estimation of corneal nerve fiber density (CNFD), CNFL, corneal nerve branch density (CNBD), CNFrD, and CNFrD adjusted for CNFL (ACNFrD). Results: In patients with DPN, CIDP, CIPN, or HIV-SN compared to HCs, CNFD (P = 0.004–0.0001) and CNFL (P = 0.05–0.0001) were significantly lower, with a further significant reduction among subgroups. CNFrD was significantly lower in patients with CIDP compared to HCs and patients with HIV-SN (P = 0.02–0.0009) and in patients with DPN compared to HCs and patients with HIV-SN, CIPN, or CIDP (P = 0.001–0.0001). ACNFrD was lower in patients with CIPN, CIDP, or DPN compared to HCs (P = 0.03–0.0001) and in patients with DPN compared to those with HIV-SN, CIPN, or CIDP (P = 0.01–0.005). Conclusions: CNFrD can detect a distinct pattern of corneal nerve loss in patients with DPN or CIDP compared to those with CIPN or HIV-SN and controls. Translational Relevance: Various peripheral neuropathies are characterized by a comparable degree of corneal nerve loss. We analyzed and quantified the 100 most frequently cited papers in macular imaging using optical coherence tomography (OCT). The databases of the Institute for Scientific Information databases utilized for identification of articles in macular imaging using OCT over the last 25 years since the introduction of OCT in ophthalmology. All articles containing “optical coherence tomography” and “retina” without “glaucoma”. The publication dates of the 100 most-cited articles ranged from 1991 to 2015, with the greatest number of articles published in the 2000s, while the total number of citations per article was greatest for articles published in the 1990s. Most studies were conducted in the U.S. and represented Level-IV clinical outcome studies indicating that also small case series or cohort studies could gain attention. The top 5 senior authors contributed to 39 of the 100 selected publications and accrued a total of 66.0% of all citations of those articles.

Purpose Citation analysis represents one of the best currently available methods for quantifying the impact of articles. Bibliometric studies list the ‘‘best sellers'' in a single field of interest. The purpose of the present study was to identify and analyze the most frequently cited papers in dry eye research that may be of high interest for researchers and clinicians. Methods We reviewed the database of the Institute for Scientific Information to identify articles published from 1900 to September 2016. All dry eye articles published in 59 ophthalmology journals were identified. The top 100 articles were selected for further analysis of authorship, source journal, number of citations, citation rate, geographic origin, article type, and level of evidence. Results The 100 most-cited articles were published between 1983 and 2011, with most of them in the 2000s. The number of citations per article ranged from 96 to 610, and was greatest for articles published in the 2000s. Each of these articles was published in one of 15 journals. Most articles represented Level-III evidence, followed by Levels II and I. Conclusions The present study focusing on dry eye research revealed that 55% of the most-cited articles came from the U.S. and 18% from Japan. Diagnostics and therapy were the areas of focus of most of the clinical articles; 13% of the most cited papers were review articles. This analysis provides researchers and clinicians with a detailed overview on the most cited dry eye papers over the past decades.

Zusammenfassung Hintergrund Bibliometrische Studien sind statistische und quantitative Analysen, die Merkmale von Publikationen in einem Forschungsfeld darstellen und deren Auswirkungen analysieren. Die 100 meistzitierten Studien der Glaukomforschung werden in dieser Arbeit bibliometrisch untersucht. Material und Methoden Alle themenrelevanten, zwischen 1900 und Dezember 2016 publizierten, Studien zur Glaukomforschung, wurden mithilfe der Datenbanken des Instituts für wissenschaftliche Information identifiziert. Bei den 100 meistzitierten Publikationen wurden folgende Parameter erfasst: Name der Zeitschrift, 1. Erscheinungsjahr der Zeitschrift, Erst- und Letztautor der Studie, Publikationsdatum, geografische Lokalisation der Studie, Gesamtzahl der Zitierungen, Gesamtzitationsrate (Gesamtzitationen/Alter der Publikation), aktuelle Zitationsrate (Anzahl der Zitierungen im Jahr 2015), Hauptforschungsgegenstand (Glaukomchirurgie, medizinische Behandlung, Diagnostik, Epidemiologie oder Grundlagenwissenschaft) und Evidenzgrad der Studie. Ergebnisse Die Publikationsdaten der 100 meistzitierten Publikationen reichte von 1966 – 2011, die meisten dieser Studien wurden in den 1990er-Jahren veröffentlicht. Die Gesamtzahl der Zitierungen pro Studie betrug 258 – 1908 und alle Studien wurden in 18 ophthalmologischen Fachzeitschriften veröffentlicht. Die führenden Herkunftsländer waren die USA, gefolgt vom UK. Hauptforschungsgegenstand der klinischen Studien waren Diagnostik und Epidemiologie. Die Anzahl der Zitierungen war am größten für Publikationen aus den 2000er-Jahren. Die meisten Studien hatten ein Level-III-Evidenzniveau, gefolgt von einem Level-I- und Level-II-Evidenzniveau. Schlussfolgerung Die meistzitierten Studien der Glaukomforschung wurden in nur 3 Fachzeitschriften veröffentlicht. Epidemiologie und Diagnostik waren am häufigsten klinischer Hauptforschungsgegenstand und Autoren mit mehreren Publikationen zeigten einen Fokus auf diese beiden Bereiche. Die meisten Studien wurden in den USA durchgeführt, in amerikanischen Zeitschriften publiziert und waren klinische Studien mit Evidenzniveau Level III. Dies deutet darauf hin, dass auch kleine Fallreihen oder Kohortenstudien Aufmerksamkeit erregen konnten.

To report a case of amiodarone-induced vortex keratopathy-associated anatomical findings and subjective visual perception before and after treatment with topical heparin eye drops.Case report.A 76-year-old man complained of halos in his vision in both his eyes due to prominent bilateral cornea verticillata. For treatment of cornea verticillata, we prescribed unpreserved eye drops of a sterile, phosphate-free solution of 0.1% sodium hyaluronate with 1300 IU/mL heparin sodium 3 times daily to the left eye, whereas the other side served as the control. The area of corneal deposits was measured by 2 examiners before and at the 1- and 3-month examination. At last follow-up, cornea verticillata had been reduced from 6 to 2 mm in area by approximately 66% from grade-III to grade-II amiodarone keratopathy.In patients using amiodarone, clearing of cornea verticillata may be achieved by topical use of unpreserved eye drops of a sterile, phosphate-free solution of 0.1% sodium hyaluronate with 1300 IU/mL heparin sodium.

ZusammenfassungDie Keratoconjunctivitis sicca ist eine der häufigsten Diagnosen in der Augenheilkunde. Eine Vielzahl von subjektiven und objektiven Testverfahren mit ungenauen Grenzbereichen und unterschiedlich hohen Sensitivitäten und Spezifitäten erschwert die Diagnostik. Gerade geringe bis moderate pathologische Befunde können so fehldiagnostiziert werden. In den letzten Jahren haben sich durch die Entwicklung neuer, klinisch einfach einsetzbarer Diagnostikgeräte bspw. für die Analyse der Tränenfilmosmolarität oder der Matrixmetalloproteinasen (MMP-9) neue Optionen ergeben. Der Stellenwert der Tränenfilmosmolarität und der Bestimmung der MMP-9-Werte in der Diagnostik des trockenen Auges und seiner Subformen wird diskutiert und anhand publizierter Studienergebnisse objektiv zu bewerten versucht.

Purpose. To determine the efficacy, safety, and predictive outcome factors for intravitreal dexamethasone implant (DEX) in pseudophakic cystoid macular edema (PCME). Methods. Retrospective, interventional, controlled study. Patients included had to have clinically significant PCME and have been treated with the DEX between 2012 and 2015. Charts and one-year data were selected consecutively, and efficacy and safety were abstracted. Visual acuity (VA) and central foveal thickness (CFT) were analysed. Results. Nineteen patient data sets were analysed. After treatment with DEX, mean VA increased significantly by 0.2 logMAR (p=0.034), while the mean CFT was reduced significantly by 162.79 μm (p<0.001). Five patients receiving a combination of DEX/bevacizumab have not experienced a higher mean VA gain or CFT reduction compared to fourteen patients receiving DEX alone. Decision rules, when to combine DEX with bevacizumab, have not been defined before the study. Only posttreatment VA gains in the nonhypertensive subgroup (n=11) were significantly better (p=0.026). Analysis of data from diabetes patients (n=4) versus nondiabetics yielded no significant differences in efficacy. There have been no adverse events within follow-up time. Conclusion. The use of DEX in PCME showed significant improvements in VA and CFT. The VA seems to show greater improvements in patients without hypertension.

Currently two intravitreally applied corticosteroids (dexamethasone and fluocinolone) are licensed in Germany for treatment of diabetic macular oedema (DME). The use of DEX implant for DME in daily clinical practice has not been defined in detail. Following a Delphi panel survey, a group of retina experts set out to come up with a consensus for use of the DEX implant in DME.International and national treatment recommendations were identified from the literature. A steering group generated a catalogue of 72 statements on the aetiology and pathogenesis of DME, therapy with DEX implant, use of DEX implant in patients previously treated with VEGF-inhibitors, use of DEX implant in combination therapy, safety of DME therapies as well as patients' burden of treatment. Twenty-two ophthalmologists from private practice and 6 hospital ophthalmologists participated in the Delphi panel via Survey Monkey. Consensus was reached if at least 75% of participants agreed or disagreed with a statement. Statements for which consensus was not reached were discussed once more during the expert consensus meeting and a vote was taken. Based on these results a treatment algorithm for foveal DME was proposed.If a patient does not show sufficient response after 3 - 6 months of anti-VEGF treatment (visual acuity gain of 5 ETDRS letters or reduction of central retinal thickness ≤ 20%), a switch to DEX implant should take place. DEX implant is also suitable in eyes with longer presentation of DME, showing e.g. massive lipid exudates. DEX implant is suitable as first-line therapy especially in pseudophakic patients, patients unwilling or able to comply with tight anti-VEGF injection intervals or patients with known vascular diseases. With fixed control visits every 4 - 8 weeks, use of DEX implant is flexible and individual. Decision parameters for repeated use should be visual acuity, retinal thickness and intraocular pressure. Treatment of both eyes on the same day should not take place.The algorithm presented reflects survey as well as expert discussion results and may differ from recommendations issued by the German professional society. The consensus recommendations for the treatment of DME generated during the survey and meeting of retina experts are intended to guide use of DEX implant in daily practice.In Deutschland sind 2 intravitreal anzuwendende Kortikosteroide (Dexamethason und Fluocinolon) für die Behandlung des diabetischen Makulaödems (DMÖ) zugelassen. Mithilfe eines Delphi-Panels und einer Runde von Retinaspezialisten sollte ein Konsens für die Anwendung von DEX-Implantat erarbeitet werden.Aus der Literatur wurden internationale und nationale Behandlungsempfehlungen identifiziert. Eine Steuerungsgruppe erarbeitete einen Katalog von 72 Aussagen zur Ätiologie und Pathogenese des DMÖ, Therapie mit dem DEX-Implantat, Einsatz bei mit VEGF-Hemmern vorbehandelten Patienten, Verwendung von DEX-Implantat in der Kombination mit anderen Therapien, Sicherheit von DMÖ-Therapien sowie Therapiebelastung des Patienten. 22 niedergelassene Ophthalmologen und 6 Ophthalmologen aus der Klinik gaben über Survey Monkey ihre Bewertung zu den Aussagen ab. Die Definition des Konsenses zu einer Aussage galt als erfüllt, wenn mehr als 75% der Befragten einer Aussage zustimmten bzw. nicht zustimmten. Über nicht konsensfähige Aussagen wurde nach erneuter Diskussion in der Konsensrunde nochmals abgestimmt. In der Folge sollte auch ein Behandlungsschema für DMÖ mit fovealer Beteiligung vorgeschlagen werden.Sofern ein Patient unter VEGF-Hemmern unzureichend anspricht (Visusgewinn 5 ETDRS-Buchstaben oder Reduktion der zentralen Netzhautdicke ≤ 20%), sollte nach 3 – 6 Monaten auf das DEX-Implantat umgestellt werden. DEX-Implantat ist ebenfalls geeignet für Augen mit länger bestehendem DMÖ, bei denen z. B. massive Lipidexsudate vorhanden sind. DEX-Implantat eignet sich als Ersttherapie insbesondere bei Pseudophaken, Patienten, die nicht willens oder in der Lage sind, enge Injektionsintervalle unter Anti-VEGF-Therapie einzuhalten oder für Patienten mit vaskulären Vorerkrankungen. Bei festgelegten Kontrollintervallen von 4 – 8 Wochen kann die Anwendung des DEX-Implantats flexibel und individuell erfolgen. Als Entscheidungsparameter gelten hierbei neben Visus und Netzhautdicke auch der Augeninnendruck. Eine Behandlung von beiden Augen am selben Tag sollte nicht stattfinden.Der hier vorgestellte Algorithmus entspricht den Ergebnissen des Delphi-Prozesses und weicht u. U. von den Empfehlungen der Fachgesellschaft ab. In glaucoma, studies revealed an involvement of the complement system. In an experimental autoimmune glaucoma model, immunization with an optic nerve homogenate antigen (ONA) led to retinal ganglion cell (RGC) loss, while intraocular pressure (IOP) remained unchanged. Here, we investigated the therapeutic effect of a complement system inhibition in this model. Hence, rats were immunized with ONA and compared to controls. In one eye of the ONA animals, an antibody against complement factor C5 was intravitreally injected (15 μmol: ONA+C5-I or 25 μmol: ONA+C5-II) before immunization and then every two weeks. IOP was measured weekly. After 6 weeks, spectral-domain optical coherence tomographies (SD-OCT), electroretinograms (ERG), immunohistochemistry, and quantitative real-time PCR analyses were performed. IOP and retinal thickness remained unchanged within all groups. The a-wave amplitudes were not altered in the ONA and ONA+C5-I groups, whereas a decrease was noted in ONA+C5-II animals (p < 0.05). ONA immunization provoked a significant decrease of the b-wave amplitude (p < 0.05), which could be preserved in ONA+C5-I, but not in ONA+C5-II animals. ONA animals showed a loss of RGCs (p = 0.001), while ONA+C5-I and ONA+C5-II retinae had similar cell counts as controls. A significant downregulation of apoptotic Bax/Bcl2 mRNA was noted in ONA+C5-I retinae (p = 0.02). Significantly more C3+ and MAC+ cells were observed in ONA animals (p < 0.001). The amount of C3+ cells in both treatment groups was significantly increased (p < 0.01), while the number of MAC+ cells in the treated retinas did not differ from controls. The number of activated microglia cells remained unchanged in ONA animals, but was increased in the treatment groups (p < 0.05). Recoverin+ cells were diminished in ONA animals (p = 0.049), but not in treated ones. Rho mRNA was downregulated in ONA and in ONA+C5-II retinas (both p = 0.014). Less opsin+ cones were observed in ONA animals (p = 0.009), but not in the treated groups. Our results indicate that the C5 antibody inhibits activation of the complement system, preventing the loss of retinal function as well as RGC, cone bipolar, and photoreceptor loss. Therefore, this approach might be a suitable new treatment for glaucoma patients, in which immune dysregulation plays an important factor for the development and progression of glaucoma.

PURPOSE: Corneal confocal microscopy (CCM) is an imaging method to detect loss of nerve fibers in the cornea. The impact of image quality on the CCM parameters has not been investigated. We developed a quality index (QI) with 3 stages for CCM images and compared the influence of the image quality on the quantification of corneal nerve parameters using 2 modes of analysis in healthy volunteers and patients with known peripheral neuropathy. METHODS: Images of 75 participants were a posteriori analyzed, including 25 each in 3 image quality groups (QI 1-QI 3). Corneal nerve fiber length (CNFL) was analyzed using automated and semiautomated software, and corneal nerve fiber density and corneal nerve branch density were quantified using automated image analysis. Three masked raters assessed CCM image quality (QI) independently and categorized images into groups QI 1-QI 3. In addition, statistical analysis was used to compare interrater reliability. Analysis of variance was used for analysis between the groups. Interrater reliability analysis between the image ratings was performed by calculating Fleiss' kappa and its 95% confidence interval. RESULTS: CNFL, corneal nerve fiber density, and corneal nerve branch density increased significantly with QI (P < 0.001, all post hoc tests P < 0.05). CNFL was higher using semiautomated compared with automated nerve analysis, independent of QI. Fleiss kappa coefficient for interrater reliability of QI was 0.72. CONCLUSIONS: The quantification of corneal nerve parameters depends on image quality, and poorer quality images are associated with lower values for corneal nerve parameters. We propose the QI as a tool to reduce variability in quantification of corneal nerve parameters.

Changes in the subbasal corneal plexus detected by confocal cornea microscopy (CCM) have been described for various types of neuropathy. An involvement of these nerves within light-chain (AL) amyloid neuropathy (a rare cause of polyneuropathy) has never been shown. Here, we report on a case of a patient suffering from neuropathy caused by AL amyloidosis and underlying multiple myeloma. Small-fiber damage was detected by CCM.

Introduction: Macular edema after cataract surgery (Irvine-Gass syndrome) or pars plana vitrectomy is a postoperative complication which can lead to permanent visual loss. Increased inflammatory substances, such as prostaglandins and cytokines, are discussed to be causative. Currently, there are no evidence-based guidelines for the treatment of postoperative macular edema. Intravitreal dexamethasone (DEX) could be effective by its anti-inflammatory effect. We examined the functional and morphological results of treatment with 0.7 mg intravitreal DEX implant (Ozurdex®). Methods: In an observational study, we analyzed visual acuity (logMAR), intraocular pressure (IOP), clinical findings, and the central macular thickness (CMT, optical coherence tomography [OCT] Spectralis®, Heidelberg Engineering, 30° macular scan, 19 scans) of 12 eyes before and 1 month after the last DEX implantation (off-label use, Ozurdex®, Allergan, Inc., Irvine, CA, USA) for macular edema after cataract surgery or vitrectomy. Re-implantation was performed when OCT showed new intraretinal fluid along with a decrease in the patient's visual acuity. The mean follow-up was 14.4 ± 10.6 months. Results: Twelve eyes of 12 patients (4 female, 8 male) with a mean age of 62.6 ± 11.9 years were treated with a mean of 2.5 ± 1.6 intravitreal DEX implant injections. Prior to injection, the visual acuity was 0.74 ± 0.34 logMAR and the CMT was 608 ± 129 µm. One month after the last injection (after a mean of 437 ± 322 days), the CMT normalized (300 ± 90 µm, p < 0.01) in all cases with a visual acuity of 0.49 ± 0.43 logMAR (p < 0.01). After 8.1 ± 5.3 months, recurring macular edema could be completely reduced by re-injection in 66% (8 patients). Four patients had no recurrence. Postinjection, the mean IOP was 17.4 ± 6.8 mm Hg. Postinjection, 7 patients required topical antiglaucomatous therapy. Conclusions: Treatment with an intravitreal DEX implant is an effective therapy for postoperative macular edema. Each injection leads to a complete resorption of the edema with a significant increase in visual acuity.

Purpose: To assess the natural disease progression of high myopia in Caucasians considered at risk for the development of myopic choroidal neovascularization (mCNV). Methods: Subjects were recruited in 25 clinical sites between June 2014 and June 2016. Main inclusion criteria included axial length of ≥26 mm, best-corrected visual acuity ≥0.05 decimal equivalent and presence of at least one out of five predefined morphological disease risk criteria. These were (1) subfoveal choroidal thinning < 50 µm, (2) enhanced choroidal curvature length > 6,300 µm, (3) lacquer cracks, (4) patchy atrophy > 5 mm2 and (5) preexisting mCNV in the fellow eye (German Clinical Trial Register DRKS00007761). Results: A total of 150 participants (66% females) with a mean age of 57.2 (±12.7) years (range 21.9–86.2 years) were included. The disease criteria most frequently encountered were choroidal thinning (33.3%) and lacquer cracks (32.7%). Enhanced choroidal curvature length was detected in only 8 subjects and always occurred in combination with other disease criteria. Presence of patchy atrophy was found to be more common in older subjects (p = 0.0012) and also associated with a more severe disease manifestation. Conclusion: The baseline data of this study indicate that enhanced choroidal curvature might be less common in Caucasians than in Asian populations. Further, disease severity in patients with high myopia is relatively high in the presence of patchy atrophy.

PURPOSE: To compare femtosecond laser-assisted cataract surgery without the use of ophthalmic viscosurgical device (OVD) with standard phacoemulsification using OVD. METHODS: This was a prospective, randomized, single-center trial of 37 patients (74 eyes) diagnosed as having significant cataract in both eyes. Randomly, one eye underwent standard phacoemulsification with OVD (the OVD group); this group acted as controls. The other eye was treated with the femtosecond laser; the subsequent manual part of the procedure was performed without OVD (non-OVD group). Time of surgery, amount of fluid used during phacoemulsification, central corneal thickness, intraocular pressure, endothelial cell count, and visual acuity were documented over the 6-month follow-up. RESULTS: There were no major complications and no significant difference in overall surgery time (non-OVD: 375 ± 81 seconds; OVD: 362 ± 43 seconds; P = .713) and in the quantity of fluid passing through the eye (non-OVD: 187 ± 35 mL; OVD: 186 ± 27 mL; P = .952) between groups. Endothelial cell loss after 6 months was not significantly different between groups (non-OVD: −2.4%; OVD: −2.7%; P = .880). Central corneal thickness was not different at 1 week postoperatively between groups (non-OVD: 575 ± 45 µ m; OVD: 573 ± 46 µ m; P = .820). Three patients in the OVD group and one patient in the non-OVD group experienced intraocular pressure greater than 25 mm Hg at 1 day postoperatively. There were no significant differences in corrected distance visual acuity between groups (logMAR non-OVD: 0.024; OVD: 0.038; P = .461). CONCLUSIONS: Femtosecond laser-assisted treatment allows the cataract surgeon to perform phacoemulsification and intraocular lens implantation without the use of OVD at no additional risk to the corneal endothelium. Furthermore, there was a tendency toward fewer increases in intraocular pressure in patients treated with the femtosecond laser. [ J Refract Surg . 2015;31(3):146–152.]

The aim of this study was to evaluate the reliability of tear film osmolarity as a diagnostic tool for detecting dry eye disease (DED) in patients with rheumatoid arthritis (RA). The second aim was to determine whether there is a relationship between tear film osmolarity, a commonly used RA score, and diagnostic blood tests for RA.A group of 74 patients with RA but without any previously established diagnosis of DED at University Hospital Wurzburg was examined. A Modified Dry Eye Severity Score was calculated based on tear film osmolarity, Schirmer test, tear film break-up time, ocular surface disease index questionnaire, corneal fluorescein staining, conjunctival lissamine green staining, and meibomian gland grading score. RA assessment included disease activity score (DAS) 28-erythrocyte sedimentation rate (ESR) scoring and serum levels of C-reactive protein (CRP) and rheumatoid factor (RF).Tear film osmolarity showed a high coefficient of determination and correlation when regressed to Modified Dry Eye Severity Score (R = 0.727, P0.001). Using a cutoff point of 316 mOsmol/L, there was significant correlation between DAS28-ESR values and tear film osmolarity (P = 0.016). Spearman correlation coefficients between osmolarity and CRP or RF were not significant (CRP: r = -0.104, P = 0.353; RF: r = 0.138, P = 0.226).Tear film osmolarity highly correlates with a modified dry eye severity scale in patients with RA with previously unknown DED. Tear film hyperosmolarity316 mOsmol/L is correlated significantly with a commonly used DAS of RA (DAS28-ESR) but not with serum markers such as CRP and RF. Patients with RA with a high DAS28-ESR score show higher prevalence of tear film hyperosmolarity.

The particle counts and the nature of particles of three different antivascular endothelial growth factor agents (VEGF) in different containers in a laboratory setting were compared.Original prefilled ranibizumab glass syringes, original vials with aflibercept, and repacked ready-to-use plastic syringes with bevacizumab from a compounding pharmacy and a compounding company (CC) were analyzed. Particle counts and size distributions were quantified by different particle characterization methods (nephelometry, light obscuration, Micro-Flow Imaging, nanotracking analysis, resonant mass measurement). Using high-performance size-exclusion chromatography (HP-SEC), levels of protein drug monomer and soluble aggregates were determined.Nearly all samples showed similar product quality. Light obscuration and Micro-Flow Imaging showed a 4-fold to 9-fold higher total particle count in compounding company bevacizumab (other samples up to 42,000 particles/mL). Nanotracking analysis revealed highest values for compounding company bevacizumab (6,375 million particles/mL). All containers showed similar amounts of silicone oil microdroplets. Ranibizumab showed lowest particle count of all tested agents with only one monomer peak in HP-SEC. Repackaged bevacizumab from different suppliers showed varying product quality.All three tested agents are available in similar quality regarding particulate purity and silicone oil microdroplet count. Repackaging can have a major impact on the quality.
