Asthma often remains uncontrolled, despite the fact that the pharmacological treatment has undergone large changes. We studied changes in the treatment of asthma over a 20-year period and identified factors associated with the regular use of inhaled corticosteroid (ICS) treatment. Changes in the use of medication were determined in 4617 randomly selected subjects, while changes in adults with persistent asthma were analysed in 369 participants. The study compares data from three surveys in 24 centres in 11 countries. The use of ICSs increased from 1.7% to 5.9% in the general population and the regular use of ICSs increased from 19% to 34% among persistent asthmatic subjects. The proportion of asthmatic subjects reporting asthma attacks in the last 12 months decreased, while the proportion that had seen a doctor in the last 12 months remained unchanged (42%). Subjects with asthma who had experienced attacks or had seen a doctor were more likely to use ICSs on a regular basis. Although ICS use has increased, only one-third of subjects with persistent asthma take ICSs on a regular basis. Less than half had seen a doctor during the last year. This indicates that underuse of ICSs and lack of regular healthcare contacts remains a problem in the management of asthma.

BACKGROUND AND OBJECTIVES: The textile industry is known to produce large amounts of dyes and other harmful contaminants. This issue is of great importance as it adversely affects both water resources and the well-being of organisms. To address this issue, biochar is frequently used as a sustainable and environmentally friendly material for removing chemical contaminants during wastewater treatment. The aim of this study was to evaluate the viability of utilizing biochar obtained from banana peels as a promising bioadsorbent for reducing environmental pollution caused by direct navy blue dye. The research investigated various factors such as temperature, potential of hydrogen levels, particle size, and concentrations to determine the effectiveness of biochar in dye removal.METHODS: The biochar obtained was separated into powdered and granular forms based on particle sizes of 425 and 850 micrometer, respectively. The biochar's textural characteristics were assessed through nitrogen adsorption/desorption isotherms. Fourier transform infrared spectroscopy and the Boehm method were employed to analyze and measure organic functional groups, specifically acidic groups, for identification and quantification purposes. Batch experiments were performed to ascertain the effects of the initial concentration and potential of hydrogen on the adsorption capacity and removal percentage.FINDINGS: The results indicated that the powdered biochar obtained at 500 degrees Celsius had the higher surface area, with a value of 80.4 square meter per gram. The biochar demonstrated remarkable removal percentages, achieving 97 percent at the lowest concentration and 89 percent at the highest concentration, when the potential of hydrogen was adjusted to a value of 6. The Freundlich model gave the best fit to the experimental data for this biochar and obeyed pseudo second order kinetics, with correlation coefficients of 0.93 and 0.99, respectively. CONCLUSION: This study provides evidence of the high removal efficiency achieved by biochar derived from banana peel waste in the removal of direct navy blue dye. Precise temperature control during the calcination process is essential to ensure its favorable chemical and textural properties. The unique attributes of banana peel biochar position it as an exceptionally promising adsorbent material. Not only is it cost-effective and environmentally friendly, but it also outperforms current wastewater treatment technologies in terms of competitiveness. Its remarkable ability to reduce contaminants, particularly the removal of dyes, further solidifies its potential as a highly effective solution.

International audience; Introduction (contexte de la recherche) :L’exposition répétée des patients asthmatiques aux allergènes intradomiciliaires contribue aux exacerbations de l’asthme. Plusieurs stratégies d’intervention sont proposées, parmi lesquelles, la visite domiciliaire de conseillers en environnement intérieur (CEI). Les CEI établissent un état des lieu au domicile du patient et proposent des mesures de remédiation afin d’améliorer les fonctions respiratoires. Plusieurs études monocentriques ont montré l’intérêt d’une telle démarche.ObjectifL’étude ECENVIR est un essai clinique, multicentrique, prospectif et randomisé qui vise à évaluer l’impact des CEI sur le niveau de contrôle de l’asthme.Méthodes :Au total, 104 patients ont été inclus dont 85 patients ont été suivis médicalement et randomisés dans 2 groupes : le Groupe Intervention (GI) a bénéficié d’une visite de CEI avec prélèvements pour mesures d’allergènes et conseils à l’inclusion et après 12 mois de suivi ; et le groupe témoin (GT) a bénéficié d’une même visite de contrôle uniquement à M12.Résultats :Le critère de jugement principal a été analysé chez 71/85 patients : 34 dans le GI et 37 dans le GT. A M12, une amélioration du contrôle de l’asthme était notée dans le GI avec une diminution du nombre d’asthme sévère et une augmentation d’asthme intermittent (p = 0,05) comparativement au GT (p = 0,24). En comparaison intergroupe, 50 % (17/34) des patients du GI ont montré une amélioration clinique comparativement au GT (p = 0,43). Bien que non statistiquement significative probablement dû à un manque de puissance, les patients du GI avaient 1,5 fois plus de chance de s’améliorer cliniquement en analyse multivariée (OR 1,47 CI95 % [0,57–3,75]. Les analyses secondaires montrent que : (i) dans la population des asthmes sévères, 12/16 patients (75 %) du GI se sont améliorés contre 7/16 du GT(44 %)(p = 0,06) ; (ii) la consommation des corticoïdes oraux et le taux d’hospitalisation sont significativement diminués dans le groupe ayant bénéficié de conseils (p = 0,029 et p = 0,042, respectivement).Conclusions :L’intervention des CEI est bénéfique cliniquement sur le niveau du contrôle de l’asthme et permet sur le plan économique une diminution de la consommation de soins, en cours d’investigation via les données d’Assurance maladie.

Las inundaciones son los desastres naturales que afectan a más personas en el Área Atlántica. En este sentido, el proyecto AA-Floods tiene el objetivo de desarrollar y poner en valor nuevas herramientas, planes y regulaciones que mejoren la gestión y la respuesta a las inundaciones. La importancia de la información altimétrica de calidad para la evaluación del peligro de inundación se basa en el conocimiento lo más detallado y preciso posible del relieve y de las alturas de los elementos situados sobre el suelo. Para ello se ha llevado a cabo una cobertura LiDAR de densidad 21 ptos/m² y fotogramétrica de resolución 0,10 m para la generación de productos altimétricos de gran exactitud posicional y alta precisión en los ámbitos de estudio de la cuenca del río Genil. Así mismo, se ha realizado el levantamiento batimétrico del cauce del río Genil y la obtención de Modelo final Topobatimétrico. Además, para el desarrollo adecuado de la modelización hidráulica y determinación de zonas inundables es necesario tomar en consideración todos aquellos elementos estructurales del cauce que afectan a la inundabilidad. Se ha realizado análisis de zonas inundables con estas fuentes de información que mejoran las utilizadas por el SNCZI. Los resultados se han contrastado con las láminas de inundación de 2010 cartografiadas mediante observación directa mediante satélite Radar.

Is generational diversity really “bad” for collective decision-making effectiveness in the family business? We argue that generational diversity reflects collective knowledge heterogeneity, and that through overconfidence as an important collective psychological mechanism, generational diversity may counter-intuitively benefit organizational consequences. Data from sixty decision-making teams in Indonesian family businesses with Chinese kinship relations suggested: first, generational diversity has positive effect on decision-making; second, generational diversity influences on group overconfidence negatively; third, overconfidence influences decision-making negatively; and fourth, overconfidence mediates in the relationship between diversity and decision effectiveness. All theoretical argument and empirical discussions grounded on knowledge heterogeneity perspective generated implications for theory and practices of decision-making in family businesses context.

A healthcare information system allows patients and other users to remotely login to medical services to access health data through the Internet. To protect the privacy of patients and security over the public network, secure communication is required. Therefore, the security of data in transmission has been attracting increasing attention. In recent years, blockchain technology has also attracted more attention. Relevant research has been published at a high rate. Most methods of satisfying relevant security-related regulations use modular and exponential calculation. This study proposes a medical care information preservation mechanism that considers the entire process of data storage in devices from wearable devices to mobile devices to medical center servers. The entire process is protected and complies with HIPAA privacy and security regulations. The proposed scheme uses extended chaotic map technology to develop ID-based key negotiation for wearable devices, and thereby reduces the amount of computing that must be carried out by wearable devices and achieve lightness quantify. It also uses the non-tamperability of the blockchain to ensure that the data have not been tampered with, improving data security. The proposed mechanism can resist a variety of attacks and is computationally lighter than the elliptic curve point multiplication that has been used elsewhere, while retaining its security characteristics.

Resume Sous l’egide de la Societe francaise de recherche et de medecine du sommeil (SFRMS), une journee de travail a ete consacree au syndrome d’apnees obstructives du sommeil (SAOS) de l’enfant. Un groupe multidisciplinaire de praticiens specialistes de cette pathologie (pneumologues, pediatres, otorhinolaryngologues, neurophysiologistes, specialistes du sommeil) a fait le point a travers leurs experiences et les donnees de la litterature sur la place des enregistrements electro-physiologiques dans le diagnostic du SAOS. Les criteres diagnostiques recommandes par l’American Academy of Sleep Medicine (AASM), revises en 2012 et en 2014, sont exposes : methodologie des enregistrements de sommeil, definitions des evenements respiratoires et regles de codage visuel. Les criteres diagnostiques des troubles respiratoires du sommeil chez l’enfant sur les enregistrements polysomnographiques (PSG) et les recommandations de la Haute Autorite de sante sur les indications de la polysomnographie sont egalement rapportes. L’interet et les limites de la PSG en unite de sommeil, de la PSG ambulatoire et de la polygraphie ventilatoire (PV) sont decrits. Un arbre decisionnel a ete elabore dans le but de repondre au mieux au besoin diagnostique et de suivi de ces enfants.

Introduction Il existe un lien entre allergie alimentaire et desordres emotionnels et comportementaux, en particulier alexithymie (difficulte a identifier, differencier et exprimer ses emotions avec un mode de pensee guide par des regles strictes), depression, anxiete et troubles du deficit de l’attention avec hyperactivite, surtout parmi les adolescents. Nous decrivons 2 cas de tentative de suicide (TS) par anaphylaxie alimentaire chez des adolescents vivants en famille d’accueil. Methodes Cas 1 : Une fille de 11 ans atteinte de troubles du comportement avec crises clastiques et idees suicidaires traites par Risperidone et Cyamemazine, d’eczema, d’asthme et de polyallergie alimentaire au lait, œuf, arachide, fruits a coque et moutarde a presente une anaphylaxie de grade 2 avec malaise, urticaire et vomissements apres ingestion volontaire de bonbons M&M's a l’arachide. Cas 2 : Un garcon de 17 ans atteint de troubles de comportement avec TS par arme blanche, traite par Risperidone, d’eczema, d’asthme et de polyallergie alimentaire a l’arachide, noisette et noix a fait une anaphylaxie de grade 2 avec dyspnee, œdeme pharynge et urticaire apres consommation volontaire et importante de cacahuetes et de gâteaux chocolates a la noisette. Resultats Les 2 enfants presentent une sensibilisation importante aux allergenes de stockage des fruits a coque et arachide. Discussion L’anaphylaxie alimentaire peut representer un traumatisme psychologique responsable d’anxiete chez le patient et ses proches. Une anxiete excessive peut conduire a des evictions alimentaires demesurees et limitations sociales, responsables d’une diminution de la qualite de vie et parfois de symptomes depressifs avec idees suicidaires. Un traumatisme affectif a contribue aux troubles de comportement chez ces 2 enfants. Dans la litterature, seulement deux cas de TS et un cas de suicide acheve par anaphylaxie alimentaire volontaire ont ete decrits. La TS par anaphylaxie volontaire serait probablement sous-estimee, car non avouee par les patients ou non reconnue par les soignants. Conclusion Il est important de connaitre ce moyen de TS, de rechercher un desordre emotionnel et de proposer un soutien psychologique a tout patient allergique alimentaire.

Security is a critical issue for medical and health care systems. Password-based group-authenticated key agreement for wireless sensor networks (WSNs) allows a group of sensor nodes to negotiate a common session key by using password authentication and to establish a secure channel by this session key. Many group key agreement protocols use the public key infrastructure, modular exponential computations on an elliptic curve to provide high security, and thus increase sensor nodes&rsquo, overhead and require extra equipment for storing long-term secret keys. This work develops a novel group key agreement protocol using password authentication for WSNs, which is based on extended chaotic maps and does not require time-consuming modular exponential computations or scalar multiplications on an elliptic curve. Additionally, the proposed protocol is suitable for multiple independent groups and ensures that the real identities of group members cannot be revealed. The proposed protocol is not only more secure than related group key agreement protocols but also more efficient.

Fixed-bed studies were performed to evaluate the removal efficiency of copper (Cu(II)) from aqueous solution using chitosan-coated bentonite (CCB), chitosan-coated sand (CCS), and chitosan-coated kaolinite (CCK). The thermal and morphological properties of CCB, CCK, and CCS were characterized using thermogravimetric analysis, Fourier transform infrared spectroscopy, and the Brunauer-Emmett-Teller method. Dynamic experiments were carried out to investigate the effect of solution pH (3.0 to 5.0) and initial Cu(II) concentration (200 to 1000 mg/L) on the time to reach breakthrough (tb), total volume of treated effluent (Veff), and adsorption capacity at breakthrough (qb). Results show that increasing the initial Cu(II) concentration inhibits the column performance where lower Veff, tb, and qb were obtained. Decreasing the pH from 5.0 to 3.0 led to improved removal efficiency with higher values of Veff, tb, and qb. Under pH 3.0 and 200 mg/L, the maximum removal efficiency of 68.60%, 56.10%, and 58.90% for Cu(II) was attained using CCB, CCS, and CCK, respectively. The Thomas model was determined to adequately predict the breakthrough curves based on high values of coefficient of determination (R2 ≥ 0.8503). Regeneration studies were carried out using 0.1 M HCl and 0.1 M NaOH solution in the saturated column of CCB, CCK, and CCS.

In water treatment plants, oxidizing agents (e.g., NaOCl) are commonly added prior to coagulation. This reduces algae activity and enhances coagulation effectiveness. However, oxidants generate harmful by-products (e.g., trihalomethanes, THMs). Landfill sites are scarce and sludge treatment processes are costly. Therefore, effective utilization of wasted sludge will lower removal and treatment costs significantly. magnetic iron and manganese oxides (MIMO) recovered from water treatment plant sludge (WTP sludge) were utilized for algae removal. This reduces sludge handling costs and waste materials are effectively recycled and reused.

Allergic Rhinitis and its Impact on Asthma (ARIA) has evolved from a guideline by using the best approach to integrated care pathways using mobile technology in patients with allergic rhinitis (AR) and asthma multimorbidity. The proposed next phase of ARIA is change management, with the aim of providing an active and healthy life to patients with rhinitis and to those with asthma multimorbidity across the lifecycle irrespective of their sex or socioeconomic status to reduce health and social inequities incurred by the disease. ARIA has followed the 8-step model of Kotter to assess and implement the effect of rhinitis on asthma multimorbidity and to propose multimorbid guidelines. A second change management strategy is proposed by ARIA Phase 4 to increase self-medication and shared decision making in rhinitis and asthma multimorbidity. Summary Food allergy to pumpkin seed is considered very rare, and only some isolated case reports have so far been published. We report here a case of food anaphylaxis to pumpkin seed in an eight-year-old boy, who tolerated all other edible seeds, peanut and tree nuts, as well as pulp of different kinds of pumpkins and other fruits of the Cucurbitaceae family. From this observation, a review of the botanical, historical, medicinal and allergenic aspects of pumpkin and its seeds is proposed. With the advent of diets rich in omega-3 and omega-6 polyunsaturated fatty acids, edible seeds like pumpkin seed have been incorporated in the modern diet. Their incremental use in the food-processing industry might contribute to an increase in food allergy to pumpkin seed in the future.

Knee osteoarthritis (OA) often leads to altered balance control, joint motion and loading patterns during gait. Acupuncture has been proven to be effective in pain relief but its effects on inter-joint load-sharing for body balance have not been reported. The current study bridged the gap by quantifying the immediate effects of acupuncture on the inter-joint and inter-limb load-sharing in patients with knee OA during level walking in terms of the total support moment (Ms) and the contribution of individual joints to the total support moment. Gait analysis was performed on fifteen healthy controls and on fifteen patients with mild to moderate bilateral medial knee OA. After acupuncture treatment the patients with bilateral knee OA walked with significantly increased speed ([Formula: see text]), and were able to resist greater and closer-to-normal knee flexion moments ([Formula: see text]). They also changed the inter-joint sharing of the support moments with increased knee ([Formula: see text]) contributions, but reduced hip contributions ([Formula: see text]) to the sagittal Ms during single-limb support. They showed an asymmetric inter-limb load-sharing similar to the normal controls, with increased sharing of the time integral of both the sagittal and frontal whole body support moment by the leading limb during double-limb support ([Formula: see text]). The altered intra- and inter-limb contributions to the demand of supporting the body during gait suggests that acupuncture treatment led to immediate changes in the control strategy toward a normal pattern. However, the effects of these changes on the progression of the disease in the long term would require further investigation.

Introduction Si l’allergie alimentaire aux laits de chevre et de brebis avec tolerance au lait de vache est rare mais connue, l’allergie au lait de bufflonne est exceptionnellement rapportee. Nous presentons un cas associant allergie alimentaire aux laits de brebis, de chevre et de bufflonne avec tolerance au lait de vache. Methodes Un garcon de 6 ans aux antecedents d’eczema, d’asthme, d’allergie alimentaire a l’arachide et aux laits de chevre et de brebis a presente 2 reactions allergiques de grade 2 apres consommation de mozzarella di Bufala Campana avec prurit, vomissement, dysphagie, dysphonie et asthme la 1re fois, et urticaire generalisee associee a des douleurs abdominales 6 mois plus tard. Le lait de vache est tolere sous toutes formes. Un test de reintroduction a ce fromage, propose 3 mois auparavant devant une suspicion d’allergie et un prick-test douteux, avait ete negatif a 56 g. La survenue progressive d’un prurit des oreilles ou pharynge a l’ingestion de cette mozzarella avait conduit a une consommation tres irreguliere. Resultats L’allergie est confirmee par un prick-test devenu positif a ce fromage associe a un prick-test negatif au lait de vache. Discussion Seulement 2 cas associant allergie au lait de brebis, de chevre et de bufflonne sans allergie au lait de vache, un cas d’allergie isolee au lait de bufflonne et un cas associant tolerance a ce lait et allergie au lait de vache ont ete rapportes dans la litterature. L’allergie aux laits de bufflonne, de chevre et de brebis sans allergie au lait de vache serait due a une sensibilisation a une β-caseine presentant une forte homologie entre les 3 premiers laits. Plus precisement, il pourrait s’agir d’une γ-caseine, une fraction de la β-caseine absente dans le lait de vache et dont la quantite augmente au cours de la fabrication de fromage a partir du lait frais. Une lactoglobuline specifique du lait de bufflonne pourrait egalement etre impliquee. Conclusion Devant une allergie aux laits de chevre et de brebis avec tolerance au lait de vache une sensibilisation au lait de bufflonne devrait systematiquement etre recherchee. L’inclusion de tous les laits de mammiferes dans la liste des 14 allergenes a declaration obligatoire serait souhaitable.

The French Society of Research and Sleep Medicine (SFRMS) organized a meeting on obstructive sleep apnea syndrome (OSAS) in children. A multidisciplinary group of specialists (pulmonologists, ENT surgeons, pediatricians, neurophysiologists, sleep specialists) drew up a consensus document on the value of electrophysiological recordings in the diagnosis of OSAS in children. Technical considerations and recommended sensors, respiratory event definitions, and scoring criteria are presented according to the 2012 and 2014 recommendations of the American Academy of Sleep Medicine (AASM). Polysomnographic criteria for sleep-disordered breathing in children and the French National Authority for Health guidelines for indications of polysomnographic studies were reported. Relevance and limits of in-lab PSG, home PSG, and respiratory polygraphy were presented and guidelines were proposed to improve the diagnosis and follow-up of these children.

Aspergillomas occur due to colonization of a pre-existing pulmonary, bronchial or pleural cavity by Aspergillus spp. Often asymptomatic, this pathology can reveal itself by recurrent haemoptysis or when bacterial superinfections occur. Aspergillomas occurring in post-traumatic cavities are rare and their management is poorly codified.A child suffered from a chest wound at the age of 13 years. Two years later, investigation of recurrent haemoptysis revealed a residual pneumatocele in the right lower lobe colonized by Aspergillus spp. Initial treatment with systemic azole antifungals was unsuccessful because of digestive and ophthalmological intolerance. Surgical treatment by right lower lobectomy was finally decided on by the multidisciplinary team. This revealed an intrabronchial foreign body of vegetal type with cellulosic reinforcement, causing a polymorphic granulomatous reaction around, and associated with a proliferation of filamentous fungi including Aspergillus fumigatus. Surgery was followed by liposomal amphotericin B treatment for three weeks with a favourable outcome.This clinical case illustrates the benefits of surgical management of post-traumatic aspergillomas, even in children, in order to eradicate the aspergillus implant and to remove any foreign body to prevent recurrence.

Security is a critical issue for medical and health care systems. Password-based group-authenticated key agreement for wireless sensor networks (WSNs) allows a group of sensor nodes to negotiate a common session key by using password authentication and to establish a secure channel by this session key. Many group key agreement protocols use the public key infrastructure, modular exponential computations on an elliptic curve to provide high security, and thus increase sensor nodes' overhead and require extra equipment for storing long-term secret keys. This work develops a novel group key agreement protocol using password authentication for WSNs, which is based on extended chaotic maps and does not require time-consuming modular exponential computations or scalar multiplications on an elliptic curve. Additionally, the proposed protocol is suitable for multiple independent groups and ensures that the real identities of group members cannot be revealed. The proposed protocol is not only more secure than related group key agreement protocols but also more efficient. [ABSTRACT FROM AUTHOR]

Introduction L’allergie alimentaire a la graine de courge est une allergie rarissime. Nous rapportons un cas pediatrique d’anaphylaxie a la graine de courge. Methodes Un garcon de 8 ans, aux ATCD d’asthme et d’allergies alimentaires gueries au lait de vache et a la noix de cajou et d’allergie persistante aux laits de brebis et de chevre, a presente une anaphylaxie de grade 3 apres la consommation de pain a la graine de tournesol et de courge. L’enfant consomme sans probleme les differentes cucurbitacees (differentes varietes de courge, courgette, concombre, melon et pasteque), l’arachide et tous les fruits a coque. Resultats Les prick-tests sont positifs aux graines des differentes especes de courge mais non a la chair. Ils sont douteux pour les autres graines comestibles, mais negatifs pour les huiles de pepin de courge, de lin, de sesame, les fruits a coque, l’arachide, pour le melon, le concombre, la pasteque, ainsi que pour les differentes farines. Les IgE specifiques sont positives a 3,75 KU/L pour la graine de citrouille, negatives pour la citrouille (chair) et pour les differentes graines. Un test de reintroduction aux graines de tournesol, de lin, de sesame et de pavot, propose du fait d’une faible sensibilisation cutanee a ces graines, est negatif a doses significatives. Discussion Utilisee en medecine traditionnelle depuis l’antiquite, la graine de courge a trouve une place de choix dans l’alimentation moderne avec l’avenement des regimes riches en acides gras polyinsatures. Mis a part des profilines et des proteines de 36, 48, 69, 77 et 87 kDa, les allergenes potentiels de la graine de courge pourraient correspondre a des albumines 2S et des globulines 7S ou 11S. Leur structure compacte, leur taille importante, leur abondance dans les graines et leur resistance a la denaturation thermique et a la digestion enzymatique seraient responsables de leurs proprietes allergeniques importantes. Conclusion L’utilisation croissante de la graine de courge dans l’alimentation moderne, liee aux effets therapeutiques potentiels de la graine de courge dans la prevention des maladies cardiovasculaires, des tumeurs et du vieillissement risque de conduire a une augmentation d’allergie alimentaire a la graine de courge.

Introduction Stroop test iteration performances and metabolism of tryptophan (TRP) via serotonin (5-HT) and kynurenine (KYN) have both been associated with suicidal behaviors. This study aims to probe their possible interactions. Objectives We explored the association of the performances on the Emotion Inhibition Subtask (EIS) of the Brief Assessment of Cognition for Affective Disorder and the plasmatic levels of 5-hydroxytryptophan (5-HTP), 5-HT, KYN, melatonin (MLT) among subjects with Lifetime Suicidal Ideation (LSI) vs non-LSI, and with Lifetime Suicide Attempts (LSA) vs non-LSA. Methods Using R studio, we employed: 1) the t-test for parametric data and the Wilcoxon test for non-parametric data; 2) Linear Modeling to probe the associations of EIS performances with MLT, KYN, 5-HTP or 5-HT plasmatic levels. Results In a sample comprising 45 individuals affected by Bipolar Disorder, we found a statistically significant difference for the Color Naming (CN, image 1) and the Neutral words (NW) subtasks among LSA vs non-LSA. In LSI vs non-LSI, only the NW retained significance, but not the CN. A significant association emerged between CN and 5-HTP in LSI but not in non-LSI (image 2). Similarly, in LSA, an association was found between CN and 5-HTP, but not in non-LSA (image 3). No statistically significant difference emerged among groups regarding gender composition, age, pharmacological therapy, Body Mass Index, Hamilton Depression Rating Scale, Young Mania Rating Scale, or Clinical Global Impression scale - Severity. Introduction During the COVID-19 pandemic, people with mental disorders were exposed to a common and prolonged source of stress. Studies focusing on the consequences of the pandemic on individuals with a history of mental disorder are scarce, but they suggest a higher vulnerability as compared to the general population. Objectives We aimed at identifying predictors of stress resilience maintained over time among these people during the first two years of the pandemic. Methods The presented study is part of a larger 2-year, 5-wave international longitudinal online survey. The Patient Health Questionnaire, the Generalized Anxiety Disorder scale and the PTSD Checklist DSM-5 were used as latent class indicators for a proxy measure of distress. Specifically, a Latent-Class Analysis was performed to identify a group that showed resilient outcomes across all waves. We investigated socio-demographic characteristics, economic and housing status, lifestyle and habits, pandemic-related issues, and chronic disease. Adherence to and approval of the restrictions imposed, trust in governments and the scientific community during the pandemic were also assessed. Social support, fear of contamination and personal values were investigated respectively through the Oslo Social Support Scale, the Padua Inventory, and the Portrait Values Questionnaire. The aforementioned characteristics were used to predict sustained resilience through a logistic regression. Results A total of 1711 participants out of the total sample (8011 participants from 13 different countries) reported a diagnosis of mental disorder before the pandemic. Nine hundred forty-three participants completed at least three of the five versions of the survey and were included in the analysis. A latent class of participants with resilience maintained over time (sustained resilience) was identified, with an estimated probability of 24.8%. The demographic and clinical variables associated with a higher chance of sustained resilience were older age, maintaining a job during the pandemic, and having a larger number of people in the household. In contrast, female gender, losing job during the pandemic, having difficulty meeting basic needs, greater fear of contamination, a stronger focus on hedonism, less social support and feeling lonely resulted in a lower likelihood of being sustained resilient. Conclusions This study identified a number of factors that may help predict resilient outcomes maintained over time in people with mental disorders. In the aspect of longitudinal beam bunching, the bunching strength can be controlled by the rf cavity phase and voltage. However, these machine parameters are different from those that interact with the beam itself. In order to gain control of the beam-cavity interaction, cavity calibration must be performed. Furthermore, it relies on fitting the beam energy gain versus cavity phase to a calibration function. Under the conventional assumption of relativistic beam conditions, the calibration function is a first harmonic sinusoidal function (a sinusoidal function with a period of 2π). However, this expression is insufficient for a high-voltage bunching cavity. Due to beam acceleration inside the cavity, an energy bias and a second harmonic function should be included to modify the conventional calibration function, even for a relativistic electron beam. In this paper, we will derive this modification and provide a comparison to both the Coherent Electron Cooling Experiment and the impact-t simulation, respectively.

48. Accident anaphylactique sévère à l’arachide après test de réintroduction négatif

Introduction Le test de reintroduction alimentaire (TPO) represente l’etalon-or afin d’affirmer ou d’infirmer une allergie alimentaire. Nous rapportons ici un cas d’anaphylaxie a l’arachide survenue apres un TPO negatif. Methodes Une jeune fille de 6 ans et demi, aux antecedents d’eczema atopique actif, d’asthme et d’allergie alimentaire a l’œuf, a presente une reaction anaphylactique de grade 3, comprenant œdeme du visage, douleurs abdominales, vomissements, crise d’asthme et asthenie, immediatement apres avoir mange 2 M&M's (environ 800 mg d’arachide). Le prick-test a l’arachide etait positif a 13,5 mm, les IgE specifiques arachide et arah2 > 100 kU/L, confirmant le diagnostic d’hypersensibilite immediate a l’arachide. Cette reaction est survenue 18 mois apres un TPO a l’arachide negatif jusqu’a la dose cumulee de 10 g, qui avait ete propose en raison d’une faible sensibilisation avec un arah2 A noter que l’enfant n’avait jamais consomme d’arachide avant le TPO, ni entre le TPO et le choc anaphylactique, cette eviction complete ayant ete pratiquee dans un contexte d’allergie a l’arachide chez le frere. Discussion Il s’agit d’un cas rare d’anaphylaxie par sensibilisation a l’arachide survenue 18 mois apres un TPO negatif a des doses significatives. On peut s’interroger sur le role de l’eviction complete dans la rupture de tolerance et faire en complement l’hypothese que le TPO chez cette enfant faiblement sensibilisee a l’arachide ait pu induire la survenue d’allergie, d’autant qu’il s’agissait de la premiere ingestion de cet aliment. Plusieurs etudes font etat du role deletere d’un regime d’eviction au long cours dans la survenue de reactions allergiques parfois severes chez des patients sensibilises, mais asymptomatiques, alors que la consommation reguliere d’un aliment sensibilisant favoriserait sa tolerance. Conclusion A notre connaissance, il s’agit du premier cas publie d’allergie alimentaire possiblement induite par un TPO.

Abstract High brightness, high charge electron beams are critical for a number of advanced accelerator applications. The initial emittance of the electron beam, which is determined by the mean transverse energy (MTE) and laser spot size, is one of the most important parameters determining the beam quality. The bialkali photocathodes illuminated by a visible laser have the advantages of high quantum efficiency (QE) and low MTE. Furthermore, Superconducting Radio Frequency (SRF) guns can operate in the continuous wave (CW) mode at high accelerating gradients, e.g. with significant reduction of the laser spot size at the photocathode. Combining the bialkali photocathode with the SRF gun enables generation of high charge, high brightness, and possibly high average current electron beams. However, integrating the high QE semiconductor photocathode into the SRF guns has been challenging. In this article, we report on the development of bialkali photocathodes for successful operation in the SRF gun with months-long lifetime while delivering CW beams with nano-coulomb charge per bunch. This achievement opens a new era for high charge, high brightness CW electron beams.

Introduction Aripiprazole (ARI) is an atypical antipsychotic drug with D2 partial agonist properties, usually prescribed to treat mood disorders (major depression or bipolar disorder) and schizophrenic disorder (schizophrenia or schizoaffective disorder). Dopamine receptor agonists, as is ARI, have been implicated in some cases of impulse-control problems, such as gambling disorder (GD), increased spending, hypersexuality and compulsive eating. Objectives Currently, it is hypothesized that aripiprazole may cause impulse-control problems because it can produce a hyperdopaminergic state in the mesolimbic pathway (reward system) through its predominant action on dopamine D3 receptors. We intend to do a non-systematic review of the scientific information regarding this subject. Methods The authors revised the published literature about this topic, selecting relevant articles, systematic reviews and case reports, with the topic words: “aripiprazol”, “gambling disorder” and “dopamine receptor” in scientific data base. Results Overall, a few cases of ARI-induced pathological gambling as well as ARI-induced hypersexuality have been reported. In one study it was verified that comorbid psychiatric and substance use disorders were common among those who have experienced GD or worsened GD after beginning ARI treatment. In another study, it was verified that the group of patients who reported this alleged side-effect were mostly young (mean age, 33.6 years), mostly men (88.2%) and most lived alone. Conclusions Attributing to dopamine agonists the only factor that can explain the onset of GD is simplistic and dangerous. Many other potential risk factors, including individual vulnerability factors (temperament, genetics) as well as environmental factors, must be considered. Disclosure No significant relationships.
