Il tessuto adiposo epicardico (EAT) è un fattore di rischio cardiovascolare in quanto promuove la progressione della fibrillazione atriale, della malattia coronarica e dell’insufficienza cardiaca. EAT si caratterizza per rapido metabolismo, misurabilità clinica e facile modificabilità e rappresenta un bersaglio terapeutico peculiare per farmaci innovativi, quali gli agonisti del recettore del peptide glucagone-simile 1 e gli inibitori del co-trasportatore sodio-glucosio 2, che appaiono salutari dal punto di vista cardiometabolico ben oltre i loro effetti sul glucosio e sul peso corporeo (Materiale Supplementare).

3. Incorporation of intermediate wheatgrass (Thinopyrum intermedium) into bread wheat doughs: Effects on pasting and rheological properties

Abstract

Intermediate wheatgrass (IWG), a resilient perennial grain with adaptability to difficult climates, is emerging as a compelling candidate for sustainable food solutions in bakery product applications. This study evaluated the pasting and rheological properties of doughs made from bread wheat flour and blended bread wheat–IWG flours (IWG content: 15%, 30%, 45% and 60%), with the goal of exploring the potential of IWG flour as a nutritious and sustainable ingredient in cereal‐based products. The RVA peak, trough, and final viscosity values significantly (p< .05) decreased, and the pasting temperature increased as the IWG substitution level increased. IWG‐containing flour blends had a lower retrogradation tendency compared to the control. As the IWG flour substitution level increased, both empirical (Mixograph, Kieffer dough and gluten extensibility, Glutograph) and fundamental (linear oscillatory frequency sweep and creep–recovery) rheological measurements indicated relatively weaker dough properties imparted by the addition of the IWG flour. The outcomes of this research will shed light on the impacts of utilizing alternative grains, such as intermediate wheatgrass, in bakery applications. This will provide insights that could inform the development of nutritious and sustainable baked products to meet the demands of health‐conscious consumers. The dough weakening effect of the IWG flour was tolerated at the 15% IWG flour substitution level. At lower substitution levels, the IWG‐containing flour blends exhibited a lower retrogradation tendency compared to bread wheat flour, which could indicate lower staling of loaves of bread during bread storage without compromising the physical quality of bakery products. Finally, the present research fills a critical gap in the literature as it represents the first application of fundamental rheological methods at known and controlled strain and stresses to evaluate the physical properties of doughs prepared from IWG flour blends.

7. Phosphonium-based ionic liquid immobilised in sodium alginate as a novel adsorbent: Kinetics, thermodynamics, and toxicity study on 2,4-dichlorophenol removal

Abstract

This study aimed to immobilise a phosphonium-based ionic liquid (IL) (trihexyl(tetradecyl)phosphonium decanoate) in sodium alginate capsules, and its adsorption capacity was evaluated using 2,4-dichlorophenol (2,4-DCP) as pollutant target. This novel adsorbent was characterized in terms of equivalent diameter, moisture content, impregnated IL percentage, and by Fourier Transform Infrared Spectroscopy (FT-IR), Scanning Electron Microscopy (SEM), Thermogravimetric Analysis (TGA), and Brunauer, Emmett and Teller (BET) Surface Area Analysis. The effect of pH, temperature, contact time, and salt concentration on the adsorption efficiency was examined. In addition, the phytotoxicity and cytotoxicity of the capsules were also determined. Adsorption was hindered in alkaline and strongly acidic environments and favoured at pHs between 6 and 7, as well as at temperatures close to 15 °C. The pseudo-second-order kinetic model best suited the data, and equilibrium was reached with a contact time close to 50 minutes. After several batch cycles, the cumulative capacity resulted in 172.01–217.55 mg g−1. Also, the phenol extraction efficiency of immobilised IL on sodium alginate was similar to the free IL used on liquid-liquid extraction. The IL-impregnated capsules proved to be non-toxic for lettuce seeds and onion bulbs. Trihexyl(tetradecyl)phosphonium decanoate could be efficiently impregnated into sodium alginate capsules and potentially used as an adsorbent.

Abstract

ABSTRACTIn the context of increased demand for timber, forest managers must consider best practices in forest management and harvesting operations. Depending on the harvesting systems adopted, the uncertainties of the cost components may reflect the volatility of productivity and the expected value of the project. We investigated whether the uncertainties of the cost components have an influence on the productivity volatility and the expected value of the mechanized timber harvesting investment project with a feller buncher. Monthly feller buncher productivity volatilities were measured based on the absolute value of the percentage variation in the deviation of the cost and productivity components. We modeled productivity and cost components using geometric Brownian motion to estimate project value. Productivity showed a strong positive correlation with the cost components of economic depreciation, supervisor labor, fuel, and operator labor. Lubricating oil and economic depreciation were the most volatile cost components. Considering the deterministic economic analysis, the net present value was USD 3,849,619. When considering the probabilistic economic analysis, the average net present value was USD 187,319 ± USD 1,275,033. We verified that economic depreciation and fuel influenced feller buncher productivity. The uncertainties of these components affect the economic viability of the feller buncher in Eucalyptusplantations.

Abstract

Background: This study aimed at describing the epidemiology of (neuro)cysticercosis as well as its clinical and radiological characteristics in a Taenia soliumendemic district of Zambia. Methods: This was part of a cross-sectional community-based study conducted in Sinda district to evaluate an antibody-detecting T. soliumpoint-of-care (TS POC) test for taeniosis and (neuro)cysticercosis. All TS POC cysticercosis positive (CC+) participants and a subset of the TS POC cysticercosis negative (CC-) received a clinical evaluation and cerebral computed tomography (CT) examination for neurocysticercosis (NCC) diagnosis and staging. Results: Of the 1249 participants with a valid TS POC test result, 177 (14%) were TS POC CC+ . Cysticercosis sero-prevalence was estimated to be 20.1% (95% confidence intervals [CI] 14.6–27.0%). In total, 233 participants received a CT examination (151 TS POC CC+ , 82 TS POC CC-). Typical NCC lesions were present in 35/151 (23%) TS POC CC+ , and in 10/82 (12%) TS POC CC- participants. NCC prevalence was 13.5% (95% CI 8.4–21.1%) in the study population and 38.0% (95% CI 5.2–87.4%) among people reporting epileptic seizures. Participants with NCC were more likely to experience epileptic seizures (OR = 3.98, 95% CI 1.34–11.78, p= 0.01) than those without NCC, although only 7/45 (16%) people with NCC ever experienced epileptic seizures. The number of lesions did not differ by TS POC CC status (median: 3 [IQR 1–6] versus 2.5 [IQR 1–5.3], p= 0.64). Eight (23%) of the 35 TS POC CC+ participants with NCC had active stage lesions; in contrast none of the TS POC CC- participants was diagnosed with active NCC. Conclusion: NCC is common in communities in the Eastern province of Zambia, but a large proportion of people remain asymptomatic.

Abstract

Despite the benefits resulting from the use of Industry 4.0 technologies in the agri-food sector, the adoption of digital technologies for preventing and/or reducing food loss and waste (FLW) across the agri-food supply chain is still under investigation. In fact, enhancing and optimizing agri-food supply chain operations through digital technologies would just represent a partial effort if FLW prevention and reduction are not effectively addressed. Although companies are starting to adopt digital technologies for eliminating FLW from their operations, the implementation process and the achieved results are generally presented at a superficial level and practical guidance is still missing. This systematic literature review contributes to theory by developing a framework analyzing the state-of-the-art of adoption of each Industry 4.0 technology across the agri-food supply chain, and providing a research agenda structured around the main themes of research design, digital technologies, contextual differences, governance, and sustainability. Eventually, the study also informs managers in the agri-food industry about the potential implementation of digital technologies for preventing and reducing FLW in across the agri-food supply chain.

Abstract

Cognitive dysfunctions are core-enduring symptoms of schizophrenia, with important sex-related differences. Genetic variants of the DTBPN1gene associated with reduced dysbindin-1 protein (Dys) expression negatively impact cognitive functions in schizophrenia through a functional epistatic interaction with Catechol-O-methyltransferase (COMT). Dys is involved in the trafficking of dopaminergic receptors, crucial for prefrontal cortex (PFC) signaling regulation. Moreover, dopamine signaling is modulated by estrogens via inhibition of COMT expression. We hypothesized a sex dimorphism in Dys-related cognitive functions dependent on COMT and estrogen levels. Our multidisciplinary approach combined behavioral-molecular findings on genetically modified mice, human postmortemDys expression data, and in vivo fMRI during a working memory task performance. We found cognitive impairments in male mice related to genetic variants characterized by reduced Dys protein expression (pBonferroni= 0.0001), as well as in male humans through a COMT/Dys functional epistatic interaction involving PFC brain activity during working memory (t(23) = −3.21; pFDR= 0.004). Dorsolateral PFC activity was associated with lower working memory performance in males only (p= 0.04). Also, male humans showed decreased Dys expression in dorsolateral PFC during adulthood (pFDR= 0.05). Female Dys mice showed preserved cognitive performances with deficits only with a lack of estrogen tested in an ovariectomy model (pBonferroni= 0.0001), suggesting that genetic variants reducing Dys protein expression could probably become functional in females when the protective effect of estrogens is attenuated, i.e., during menopause. Overall, our results show the differential impact of functional variants of the DTBPN1gene interacting with COMT on cognitive functions across sexes in mice and humans, underlying the importance of considering sex as a target for patient stratification and precision medicine in schizophrenia.

Abstract

Living (stained) benthic foraminifera from the continental shelf of Santos Basin, southeastern Brazilian continental margin, were investigated to understand their distribution patterns and their relationship with chlorophyll-a and particulate organic matter contents. Sediments were collected from 44 to 203 m of water depth. This study analysis benthic foraminifera and abiotic data, including grain size, total organic matter, distributions of ocean chlorophyll-a and particulate organic matter. Foraminiferal diversity and richness were positively correlated to the chlorophyll-a and particulate organic carbon contents. Stations located on the southern portion of the study area and sampled during austral winter are probably under the seasonal influence of the Brazilian Coastal Current intrusion, and Rio de la Plata and Lagoa dos Patos plumes. Ammonia rolshauseni, Textularia? sp. and Gyroidina umbonataare present within the foraminifera assemblages at these stations. On the other hand, stations in the north, are characterized by the presence of Uvigerina peregrina, Quinqueloculina atlantica, Hanzawaia concentricaand Bulimina marginata, and they are influence of Cabo Frio’s southward-moving plume.

AbstractThe present study focused on investigating the antioxidant, antiglycation activity, digestive enzymes inhibition, bioaccessibility and hypoglycemic effect of C. arabicaleaves extracts. The extracts deactivated the O2•-, ROO•, H2O2, HOCl reactive oxygen species. Coffee leaves showed strong inhibition of α-glucosidase (IC50= 40.30 μg mL−1) greater than the isolated metabolites and acarbose. There was also inhibition of pancreatic lipase (IC50= 56.43 μg mL−1) in addition to a hypoglycemic effect in zebrafish similar to acarbose and metformin. With the exception of rutin, all biocompounds were detected at all stages of in vitrodigestion. Finally, these results suggest that C. arabicaleaf extracts possess antidiabetic and anti-obesity properties that can be attributed to the main metabolites and the synergistic action between them.Communicated by Ramaswamy H. 25. Effect of different concentrations of phosphorus and nitrogen on the growth of the microalgae Chlorella vulgaris

Abstract

The growth curve is an important characteristic to estimate microalgae biomass production for biofuel generation, since they can measure the variation between concentrations of limiting factors of culture medium. Therefore, this work aimed to evaluate the development of Chlorella vulgariswith triplicate cultivation of three different concentrations of nitrogen and phosphorus (Treatment 1: 0.50 g L−1Ca(NO3)2·4H2O and 0.13 g L−1KH2PO4, Treatment 2: 0.50 g L−1Ca(NO3)2·4H2O and 0.39 g L−1KH2PO4, Treatment 3: 1.50 g L−1Ca(NO3)2·4H2O and 0.13 g L−1KH2PO4,). Growth curve using Gompertz model presented high R2(0.96 ≤ R2≤ 0.99) in the three studied treatments. In the thirteenth day, turbidity in the treatment with higher nitrogen concentration (203.67 NTU) was 2.15 times higher than the first treatment (94.56 NTU) and 1.78 times higher than the treatment with higher level of phosphorus (113.9 NTU). We therefore observed a major biomass production, chlorophylls and carotenoids in the treatment with higher concentration of nitrogen, while in high levels of phosphorus the growth is not statistically significant from the first treatment with lower nitrogen and phosphorus concentration (pvalue > 0.05). In the end of cultivation, there was an increase of 203.12% in chlorophyll-a in the third treatment compared to the first treatment and of 246.42% in comparison with the second treatment. For carotenoids, the highest increase was seen compared to the first treatment (192%) than for the second treatment (137.5%). Therefore the treatment with lower phosphorous concentration in the cultivation medium presented slightly higher chlorophyll concentration and smaller carotenoids with the treatment with higher phosphorus concentration. The ash content demonstrated that this microalgae have a great potential for energy use.

Abstract

Background: The prevalence of hypothyroidism among older patients hospitalized for COVID-19 and its association with mortality is unclear. This study aims to investigate the prevalence of hypothyroidism in older COVID-19 inpatients and verify if this comorbidity is associated with a specific pattern of onset symptoms and a worse prognosis. Methods: COVID-19 inpatients aged ≥ 60 years, participating in the GeroCovid acute wards cohort, were included. The history of hypothyroidism was derived from medical records and the use of thyroid hormones. Sociodemographic data, comorbidities, symptoms/signs at the disease onset and inflammatory markers at ward admission were compared between people with vs without history of hypothyroidism. The association between hypothyroidism and in-hospital mortality was tested through Cox regression. Results: Of the 1245 patients included, 8.5% had a history of hypothyroidism. These patients were more likely to present arterial hypertension and obesity compared with those without an history of hypothyroidism. Concerning COVID-19 clinical presentation, patients with hypothyroidism had less frequently low oxygen saturation and anorexia but reported muscle pain and loss of smell more commonly than those without hypothyroidism. Among the inflammatory markers, patients with hypothyroidism had higher lymphocytes values. At Cox regression, hypothyroidism was associated with reduced in-hospital mortality only in the univariable model (HR = 0.66, 95% CI 0.45–0.96, p = 0.03); conversely, no significant result were observed after adjusting for potential confounders (HR = 0.69, 95% CI 0.47–1.03, p = 0.07). Conclusions: Hypothyroidism does not seem to substantially influence the prognosis of COVID-19 in older people, although it may be associated with peculiar clinical and biochemical features at the disease onset.

32. Duration of androgen deprivation with radiotherapy for high-risk or locally advanced prostate cancer: A network meta-analysis

Abstract

Objective: To evaluate various outcomes of different lengths of androgen deprivation therapy in high- and very-high-risk prostate cancer, we conducted a network meta-analysis of randomized trials. The treatment of high-risk PC comprises the use of radical radiotherapy associated with various durations of androgen deprivation therapy, with luteinizing hormone releasing hormone analogues initiated during or immediately before the beginning of radiation.Methods and Materials: This study followed the PRISMA extension statement to report network meta-analyses. We systematically searched online databases, including MEDLINE, Embase, and the Cochrane Central Register of Controlled Trials, for all randomized trials published up to April 2022. The primary outcomes were overall survival, prostate cancer-specific mortality, and metastasis-free survival. Network meta-analyses were performed under a Bayesian framework using the “gemtc” package (https://gemtc.drugis.org).Results: The network meta-analysis included 12 studies (10 treatments) on overall survival outcomes. None of the arms showed superiority to radiotherapy alone with respect to overall deaths. Nine studies and 10 treatment arms had prostate cancer-specific mortality data. Overall, 36 months of adjuvant androgen deprivation therapy resulted in a better outcome than radiotherapy alone, three months of neoadjuvant androgen deprivation therapy, or 12 or 24 months of adjuvant androgen reprivation therapy, and it was the better treatment (73%) in terms of cancer mortality. Treatment involving luteinizing hormone releasing hormone analogues for 6 months before and during radiotherapy ranked the highest in reducing distant metastases (42%).Conclusions: We found that 36 months of adjuvant androgen deprivation therapy after radiotherapy was the optimal duration of endocrine treatment with regard to cancer mortality for high-risk and locally advanced prostate cancer.

Abstract

AbstractNoni, also known as Morinda citrifolia, is a plant that has been widely used for treating various pathologies due to its allegedly anti-inflammatory and immunomodulating properties. Macrophages and monocytes are critical immune cells that initiate inflammation and generate an immune response against various aggressors, regardless of their causative agent. However, it is not well investigated how noni fruit components affect the behavior or phenotype of these cells. Hence, studies in this direction could potentially demonstrate the therapeutic effects of this fruit, which is consumed worldwide. This study aimed to investigate the therapeutic properties of noni fruit juice in modulating the behavior and phenotype of human monocytes and macrophage cells in vitro. The results show that noni fruit juice inhibited the production of nitric oxide (NO) and inflammatory cytokines such as tumor necrosis factor (TNF)-α, interleukin (IL)-1β, and IL-4, while stimulating the production of the anti-inflammatory cytokine IL-10 in monocytes. In addition, it negatively modulated the expression of toll-like receptor 4 (TLR4). In macrophages, noni fruit juice enhanced the secretion of all cytokines and NO, and inhibited the expression of TLR4. The contrasting results observed could be attributed to the joint stimuli of PMA (Phorbol 12-myristate 13-acetate), which was used to induce macrophage differentiation and noni Juice. Overall, the data indicate that noni may act as an anti-inflammatory agent and may aid in the treatment of uncontrolled inflammatory diseases as a complementary treatment to conventional therapies, under medical supervision.

42. User needs in reverse logistics of electro medical equipment in a product-service system

Abstract

Product-service systems (PSS) and circular economy (CE) require reverse logistics (RL) networks to recover and maximise product lifespan. Those three systems (PSS, CE, and RL) must be designed from the consumer's perspective to succeed. However, few studies highlighted users' relevance in RL in the PSS context. In this sense, this research seeks to understand the user's needs to perform RL in a PSS. An initial case study was carried out with a company that manufactures and sells medical devices in Brazil. Then, multiple case studies were carried out in hospitals to collect on-site needs related to the RL of medical devices. The identified needs were organised into seven categories. The results indicate that lack of agility in the RL process can lead to critical situations within hospitals (e.g., lack of equipment to use), as well as the creation of stressful situations for all stakeholders involved in the process.

43. Treatment Satisfaction, Efficacy, and Tolerability of Low-Dose Diclofenac Epolamine Soft Capsules in Acute, Mild, or Moderate Musculoskeletal Pain: A Prospective Open-Label, Single-Arm Interventional Study

Abstract

Introduction: The use of oral nonsteroidal anti-inflammatory drugs (NSAIDs) for acute musculoskeletal pain should be at the lowest effective dosage and for the shortest duration to minimize potential adverse effects. This study evaluated treatment satisfaction, effectiveness, and tolerability of a low-dose diclofenac epolamine 12.5-mg soft capsule formulation (DHEP 12.5-mg capsules) using patient-reported outcome measures in a real-life setting over a short period (3 days) in subjects with mild-to-moderate acute musculoskeletal pain. Methods: A prospective, open-label, phase IV clinical study in adult outpatients at hospital clinic departments/general practitioner’s clinics at eight sites in Italy. The primary efficacy variable was the degree of satisfaction with treatment at 72 ± 7 h after initiation of treatment, assessed using the Overall Satisfaction Question of the Pain Treatment Satisfaction Scale (PTSS) and described by classic descriptive statistics. Secondary objectives were to evaluate the analgesic effect after the first administration and over time; the time to and satisfaction with the onset of pain relief, amount of and duration of pain relief; pain intensity differences over time; and safety and tolerability. The investigator’s satisfaction with the treatment was also assessed. Subjects initially took 1–2 capsules of the study treatment and then one or two soft capsules every 4–6 h according to their needs. Not more than six soft capsules were to be taken in any 24-h period. Results: A total of 182 subjects (mean age, 56.2 years; 54.4% female) took ≥ 1 dose of DHEP capsule and were included in the full analysis set. The most common musculoskeletal conditions were arthralgia (39.0%) and low back pain (23.1%). All subjects completed the study, and 165/182 (90.7%, 95% CI 0.86, 0.95) were satisfied or very satisfied with the treatment at 72 ± 7 h after the first dose (primary efficacy variable). Similar percentages were recorded for treatment satisfaction concerning other efficacy parameters. The onset of the analgesic effect was rapid, with complete pain relief reached after a mean of 49.45 min. Investigators rated their overall treatment satisfaction as 92.9%. Treatment was well tolerated. Conclusions: The low-dose (12.5 or 25 mg) oral diclofenac epolamine soft capsules formulation exerted rapid, effective, and safe analgesic activity in patients with mild-to-moderate musculoskeletal pain, with subjects’ overall satisfaction with treatment more than 90%. 44. Assessment of IEEE 1588-Based Timing System of the ITER Neutral Beam Test Facility

Abstract

MITICA is one of the two ongoing experiments at the ITER neutral beam test facility (NBTF) located in Padua (Italy). MITICA aims to develop the full-size neutral beam injector of ITER and, as such, its Control and Data Acquisition System will adhere to ITER CODAC directives. In particular, its timing system will be based on the IEEE1588 PTPv2 protocol and it will use the ITER time communication network (TCN) on a dedicated Ethernet local-area network. Following the ITER device catalog, the National Instruments PXI-6683H precision time protocol (PTP) timing modules will be used to generate triggers and clocks synchronized with a PTP grandmaster clock (GMC). The “Lazy Trigger” technique, then, will be adopted to implement event-driven data acquisition without the need for any hardware link in addition to the Ethernet connections used to transfer data and timing synchronization. To evaluate the accuracy over time that can be achieved with different network topologies and configurations, a test system has been set up consisting of a GMC, two PXI-6683H devices, and two PTP-aware network switches. In addition, a detailed simulation of the network and the involved devices has been performed using the OMNET++ discrete event simulator. The simulation parameters include not only the network and switches configuration, but also the PID parameters used in the clock servo controllers. A comparison between simulated and measured statistics is reported, together with a discussion of the possible optimal configuration strategies.

Abstract

Safety aspects regarding free asparagine concentration in wheat arise because of its acrylamide formation potential in baked wheat products. Free asparagine concentration in wheat is affected by environment, genotype, and nitrogen and sulfur fertilization, which also affect wheat protein content and composition. Changing protein content and composition affects gluten strength, altering product quality. This study investigated how gluten strength was affected by environment, genotype, and fertilization treatments grown under commercially relevant conditions. Environment predominated in affecting dough extensibility and genotype primarily affected resistance to extension, with minimal effects from fertilization treatments. Differences in extensigraph parameters at two resting times segregated genotypes according to gluten strength. Factors affecting free asparagine accumulation in wheat also affect gluten strength. Genotype and environment require primary attention since they play an important role in both wheat safety and quality. The lack of studies investigating the impact of factors affecting free asparagine concentration on the gluten strength of wheat has implications for the global wheat market. From a novel perspective on dough extensibility and resistance to extension, the effects of environment, genotype, and fertilization on the gluten strength of commercially important wheat varieties are considered in a food safety context.

Abstract

The presence of acrylamide, a probable carcinogen, is a global concern for the baking industry. The reduction of free asparagine in wheat is an effective strategy to mitigate acrylamide formation. However, field‐based strategies for this purpose also affect gluten strength and bread quality. Therefore, we investigated the relationship between wheat free asparagine concentration, gluten strength and whole wheat bread quality for an extensive set of hard red spring wheat samples. Gluten strength parameters negatively correlated to wheat free asparagine concentration and showed strong inverse correlations to wheat free asparagine concentration per unit mass of protein. Reducing free asparagine concentration in the wheat did not affect the quality of the bread. In efforts to deliver wheat grains with a low acrylamide‐formation potential, wheat producers can apply strategies, for example, variety selection, that will reduce free asparagine without worries that gluten strength and bread quality will be impaired. Safety concerns regarding free asparagine in wheat have elicited much attention from cereal scientists and the breadmaking industry. However, the effects of reducing free asparagine on gluten strength have not been systematically studied in the literature. This comprehensive study shows that there is an inverse relationship between gluten strength and wheat free asparagine concentration.

47. Frailty and post-operative delirium influence on functional status in patients with hip fracture: the GIOG 2.0 study

Abstract

Background: This study analyzes the effect of frailty and Post-Operative Delirium (POD) on the functional status at hospital discharge and at 4-month follow-up in patients with hip fracture (HF). Methods: Multicenter prospective observational study of older patients with HF admitted to 12 Italian Orthogeriatric centers (July 2019-August 2022). POD was assessed using the 4AT. A 26-item Frailty Index (FI) was created using data collected on admission. The outcome measures were Cumulated Ambulation Score (CAS) ≤ 2 at discharge and a telephone-administered CAS ≤ 2 after 4 months. Poisson regression models were used to assess the effect of frailty and POD on outcomes. Results: 984 patients (median age 84 years, IQR = 79–89) were recruited: 480 (48.7%) were frail at admission, 311 (31.6%) developed POD, and 158 (15.6%) had both frailty and POD. In a robust Poisson regression, frailty alone (Relative Risk, RR = 1.56, 95% Confidence Intervals, CI 1.19–2.04, p = 0.001) and its combination with POD (RR = 2.57, 95% CI 2.02–3.26, p &lt; 0.001) were associated with poor functional status at discharge. At 4-month follow-up, the combination of frailty with POD (RR 3.65, 95% CI 1.85–7.2, p &lt; 0.001) increased the risk of poor outcome more than frailty alone (RR 2.38, 95% CI 1.21–4.66, p &lt; 0.001). Conclusions: POD development exacerbates the negative effect that frailty exerts on functional outcomes in HF patients.

Abstract

AbstractTurnera subulataSm. belongs to the family Turneraceae and is found in Brazil. The present study evaluated the antinociceptive, anti-inflammatory, and hypoglycemic potential of T. subulataflower extract (EtFloTsu) in zebrafish (Danio rerio). The total phenol and flavonoid contents of EtFloTsu were determined and identified using the Folin Ciocalteu reagent and aluminum chloride (AlCl3), respectively. The constituents of the extract were identified by HPLC-DAD, and the in vitroantioxidant activity (DPPH) was determined, toxicity in brine shrimp, and acute toxicity of 96 h in adult zebrafish. In addition, adult zebrafish (n = 6/fish) were treated orally with EtFloTsu (4, 20, or 40 mg/kg; vo) and subjected to formalin-induced nociception tests (with its possible mechanism of action with camphor), carrageenan-induced inflammation, and D-glucose-induced hyperglycemia (111 mM). Oxidative stress in the liver and brain tissues was assessed. EtFloTsu showed high levels of phenolic and flavonoid compounds with antioxidant activity. The phytochemicals chlorogenic acid, luteolin-7-o-glucoside, vitexin, and apigenin-7-o-glucoside were also identified in EtFloTsu. The synergism between these constituents was possibly responsible for the antinociceptive (via TRPA1), anti-inflammatory, and hypoglycemic effects of EtFloTsu in adult zebrafish, without causing toxicity in animals. Therefore, T. subulataflowers have therapeutic agents that could treat pain, inflammation, diabetes, and related complications.Communicated by Ramaswamy H. Sarma
