Background: In 2018, the French government introduced two mutually exclusive financial incentives to increase etanercept and insulin glargine biosimilar use. The general case redirects 20% of the price difference between the reference product and its biosimilars to hospitals for every biosimilar dispensed in community pharmacies from hospital prescriptions. The experimental case redirects 30% to prescribing clinical units after hospital selection. Adalimumab was added to these incentives in 2019, after its first biosimilar was launched. Objective: This retrospective observational study aimed to compare both general and experimental incentives after 19 months to assess the impact of directly incentivizing clinical units for adalimumab biosimilars. Method: The monthly number of adalimumab boxes dispensed in community pharmacies was linked to the corresponding hospital’s prescription using IQVIA Xponent data from November 2017 to October 2020. The monthly mean rate of adalimumab biosimilars was compared between incentive groups and subgroups depending on hospitals’ prior experience with the etanercept experimental case. Results: General case hospitals had a significantly lower mean biosimilar uptake (16.7% vs 23.2%, p= 0.029) than those included in the experimental case. Twenty-three of the 40 hospitals in the experimental case and ten out of the 91 hospitals studied in the general case had already taken part in the etanercept experiment. Biosimilar uptake was higher, but not statistically significant, for hospitals with prior experience in the adalimumab general case group (p= 0.086). Conclusions: This study confirmed that incentivizing close to physicians was more effective in increasing the biosimilar rate. It also suggested that previous incentive experience positively influenced biosimilar penetration.

Ces dernières années, de nombreuses innovations thérapeutiques ont été approuvées et commercialisées en France, dans un contexte financier contraint. Des dispositions législatives ont permis des accords conventionnels entre le les entreprises du médicament (LEEM) et le comité économique des produits de santé (CEPS) pour développer une maîtrise médicalisée des dépenses médicamenteuses au travers de différents contrats confidentiels. L’objectif de cet article est de définir et de décrire ces contrats d’accès au marché, puis de s’interroger sur leur applicabilité pour le financement des médicaments d’immunothérapie. Les contrats financiers ont engendré des économies majeures pour le payeur public, mais n’ont pas répondu totalement à la problématique de financement des innovations thérapeutiques. Les contrats de performance (financement selon des données en vie réelle) constituent un espoir de maîtrise des dépenses de santé, mais les difficultés méthodologiques majeures pour leur utilisation courante les restreignent actuellement à de rares cas. Bien qu’un certain nombre de contrats financiers puissent répondre en partie à la problématique soulevée par les prix des immunothérapies, les contrats de performance semblent constituer une option intéressante.

Aim:Erythropoiesis-stimulating agents (ESAs) are particularly used to treat dialysis patients suffering from anemia due to renal failure. Since March 2014, ESAs have no longer been funded on top-of-diagnosis-related groups (DRGs) in French hospitals and are funded via DRGs. There are two ways to fund dialysis in French hospitals: the ‘DRG for dialysis in session’ and the ‘off-dialysis DRG’, which is not a DRG and consists in a supplement tariff specific to dialysis for patients hospitalized for another main reason than dialysis. The aim of this study is to assess the impact of this funding change on the dialysis activity and on the budget of the 37 University Public University Hospitals in Paris (10 of the hospitalizations in France). Materials & methods:A before–after study (March–September 2013 vs March–September 2014) was conducted. Medical activity data (and ESAs consumption data) were used to assess the number and costs of DRGs associated with ESAs use. As we do not have access to the whole dialysis activity over the period studied, two hypotheses were considered: the proportion of the dialysis activity was constant between the two periods (Hypothesis A); the dialysis activity was correlated to ESA consumptions delivered by hospital pharmacies to healthcare units (Hypothesis B). A budget impact analysis was also conducted taking into account the evolution of DRG costs and ESA prices. Results:The number of dialysis ‘DRG for dialysis in session’ with ESA consumption have increased by 5 (Hypothesis A) and decreased by 9 (Hypothesis B) between the two periods while the volume of ‘off-dialysis DRG’ with ESA consumption increased by 2 (Hypothesis A) and by 9 (Hypothesis B). The budget impact was -€1.02 million (Hypothesis A) and -€0.7 million (Hypothesis B) leading to a loss for our hospitals. Conclusion:There is no significant impact of the change of funding of ESAs on the hospital activity. The DRG-based payment is negative for the budget of our hospitals but is positive for the French National Health Insurance. Indeed, with the price decrease, the ESAs are more costly for hospitals (not funded on top-of-DRG), but less costly for the society.

Les guides personnalisés à usage unique ou patient-specific instruments(PSI) pour prothèses totales de genou sont présentés par leurs fournisseurs comme des innovations face aux ancillaires traditionnels et à la navigation, améliorant la précision sur l’alignement des axes mécaniques. Une revue exhaustive a été réalisée afin de dégager l’intérêt porté par fournisseur, avec leurs avantages et inconvénients.

Depuis 2009, des ancillaires usage unique (UU) pour prothèses de genou (PTG) et de hanche (PTH) sont commercialisés pour se substituer aux ancillaires réutilisables. Le concept n’est pas novateur mais leur utilisation en orthopédie est encore exceptionnelle. Une réflexion a été initiée sur l’évaluation des besoins en ancillaires UU à l’Assistance publique–hôpitaux de Paris et sur les conséquences d’utilisation pour le patient, le chirurgien et l’hôpital.

La traçabilité des dispositifs médicaux implantables (DMI) inclut l’information aux patients. Sa mise en application peut s’avérer difficile en l’absence de « cartes patients » (CP) reprenant les éléments de traçabilité obligatoires. Un état des lieux a été réalisé afin d’évaluer leur disponibilité auprès des industriels.

Dans un contexte d’évolution constante du cadre réglementaire, le travail mené au centre hospitalier d’Argenteuil propose une méthodologie, un support et les moyens mis en œuvre pour évaluer et améliorer la traçabilité des dispositifs médicaux implantables.

Évaluer l’impact économique des futures prescriptions de biosimilaires d’étanercept et d’adalimumab à l’échelle des territoires de santé couverts par le groupement de commandes des produits de santé PharmAlp’Ain.

En France, la CNEDiMTS est chargée d’émettre des avis sur l’intérêt du remboursement pour les dispositifs médicaux. L’évaluation repose en partie sur l’analyse des données cliniques fournies par le fabricant et permet de caractériser son service attendu/rendu (SA/R). Si celui-ci est jugé suffisant et sous réserve de tarification, le produit sera inscrit sous nom de marque, ou à défaut sous description générique, sur la liste des produits et prestations remboursables. Ce travail présente l’analyse des avis rendus par la CNEDiMTS pour les pansements entre 2007 et 2016.

L’impact économique des innovations thérapeutiques sur la prise en charge des patients à l’hôpital est difficilement quantifiable. L’objectif de cette étude est d’illustrer l’usage d’une enquête Delphi comme outil d’aide à l’analyse des modifications de prise en charge suite à l’utilisation d’idarucizumab chez des patients traités par dabigatran nécessitant une chirurgie en urgence ou une procédure urgente d’hémostase avec saignements incontrôlés menaçant le pronostic vital.

La gestion des préparations pour nourrissons à l’hôpital fait partie des missions dites « optionnelles » du pharmacien hospitalier. L’objectif de ce travail est de faire un état des lieux des diverses réglementations, de décrire les critères pertinents conduisant au choix des préparations à visée médicale et enfin, d’expliciter le rôle du pharmacien hospitalier dans leur référencement.

Les anticancéreux représentaient en 2005 le premier poste de dépenses des médicaments utilisés en hospitalisation. Dix ans après, on peut constater que la mise sur le marché des médicaments génériques a entraîné une forte diminution de leur prix. Afin de déterminer si cette baisse s’est faite au détriment de la qualité des produits, une étude de l’évolution du rapport qualité/prix de cinq anticancéreux a été réalisée à l’AP–HP.

Introduction:In the event of a major event or disaster, the intensive care unit (ICU) should be able to offer an adapted and proportional response, within a limited time frame, to the influx of patients who can benefit from the technical facilities of the University Hospitals of Geneva. We developed an innovative approach to ICU care aimed to guarantee continuity by protecting healthcare staff from excessive fatigue and by tailoring the care provided from individualized care to the best care for the most people.Method:A modified organizational and systematic investigation method (MINOS, Paries - 2013) was used to elaborate an ICU security model; threats to activity shut down were identified and their prevention, recovery, and mitigation were planned. These actions were updated following the evolution of the crisis. Crew resources management (CRM) and bedside simulations were used in the implementation phase.Results:The ICU security model pillars were staff protection and patient management; the identified threats to activity continuity were lack of human resources, activity overload, medical errors, pressure sores and healthcare acquired infections; they were evaluated at intermediate or high risk to patients’ safety. The prioritized care plan was developed to control, recover, and mitigate these threats. It consisted in: adaptable level of ICU care, modular organization by cell, huddles, matrix for activities prioritization and controlled delegation method. Before implementation, 55 nurses and 46 doctors were trained by CRM courses and simulations. The pilot phase was deployed in one cell, from December 2021 to January 2022; 67 patients were admitted in the period; 13 adaptations to the original plan were introduced. No critical safety issues were reported.Conclusion:The prioritized care could be an adapted and proportional ICU response to a major event allowing the continuity of the activity while protecting staff from overload. Further tests are needed.

Certains patients nécessitent le recours à des traitements non approuvés par les autorités. L’objectif était de décrire et de comparer les autorisations temporaires d’utilisation (ATU) en France et le programme d’accès spécial (PAS) au Canada et leurs conséquences sur la prise en charge des patients. Nous avons comparé les données de l’Agence nationale de sécurité du médicament (ANSM) et du ministère de la santé en France et de Santé-Canada, ainsi que les listes de médicaments ayant bénéficié de ces systèmes d’accès précoce au marché en 2014 (ATU nominatives et cohorte – ATUn et ATUc en France et liste des médicaments du PAS de l’Ontario Pharmacists Association au Canada). En France, après accord, le médicament peut être obtenu et est pris en charge, contrairement au Canada où le laboratoire accepte ou non la distribution, sans prise en charge. L’implication du pharmacien hospitalier est plus soutenue en France (coresponsabilité avec le médecin). En 2014, 41 médicaments ont bénéficié d’une ATUc (durée médiane de 4,5 mois) et 231 d’une ATUn (25 521 demandes). Au Canada, 166 médicaments étaient présents sur la liste (seule donnée accessible). Le système français semble plus souple et plus avantageux pour les patients comparé au PAS. Toutefois, d’autres études sont nécessaires pour s’assurer que ces accès précoces ne constituent pas un moyen de contournement de l’évaluation même par les autorités.

Introduction: The active clinical research programme of trabectedin continues to improve knowledge on the therapeutic activity and toxicity of the drug in the treatment of soft tissue sarcomas (STS). In contrast, limited number of data is available on its use outside of clinical trials.Patients and Methods: We retrospectively analysed efficacy and safety of trabectedin when given in daily practice to patients with advanced/recurrent STS. Outcomes were compared with previously published works including clinical and retrospective studies.Results: Forty-five patients received trabectedin between January 2005 and May 2014. Sarcomas were histologically heterogeneous in our cohort (37.9% of other types of sarcomas than L-sarcomas). Our patients had poor baseline health status (ECOG ≥ 2 [17.8%]) and had received multiple previous lines of chemotherapy. Patients received a median of five cycles of treatment (1–22). The objective response rate was statistically superior in our study (37.8%) compared to the other works. However, median PFS was similar. Trabectedin-related serious adverse events (AEs) induced hospitalizations and treatment discontinuation in 22 and 15% of patients.Conclusion: This analysis confirms the efficacy of trabectedin in clinical practice (with a third of patients experiencing prolonged disease control) and highlighted the importance of its administration as early line therapy to allow the best management of serious AEs.

Le parcours de soins des patients en cancérologie est complexe et par conséquent difficile à définir. Les anticancéreux oraux (AKPO) ont montré leurs bénéfices pour les patients et les professionnels de santé, cependant les risques entraînés sur le parcours restent méconnus. L’objectif de l’étude est de définir, quantifier les risques induits par les AKPO et leurs effets sur le parcours (déstructuration [Ds], rupture [Rt]). À partir du modèle de parcours de soins proposé, la méthode AMDEC est utilisée pour analyser les risques. Pour les 3 processus identifiés (1 monothérapie, 2 bithérapies : 2 AKPO ou 1 AKPO/1 AKIV), l’analyse révèle en moyenne 91 risques, 173 Ds, 147 Rt, majorés en cas de bithérapie. L’administration et la dispensation sont les étapes les plus à risques. Le défaut de formation et d’information engendre 80 % des Ds et Rt. Ce modèle confirme le caractère complexe, variable, multifactoriel du parcours. Le développement d’actions visant à améliorer la coordination ville–hôpital et les échanges d’informations sont nécessaires pour optimiser et sécuriser le parcours, confirmant ainsi les objectifs du plan cancer 3.

Despite the fact that this neuropsychiatric disease appears to be related to several different causes, common cellular and molecular pathways have emerged and point to synaptic dysfunction or cellular growth. Several studies have indicated the importance of the ubiquitin pathway in synaptic function and the aetiology of autism. Here, we focused on the ring finger protein 135 (RNF135) gene, encoding an E3 ubiquitin ligase expressed in the cortex and cerebellum, and located in the NF1gene locus in 17q11.2, a region linked to autism. We carried out a genetic analysis of the coding sequence of RFN135in a French cohort of patients with autism and observed a significantly increased frequency of genotypes carrying the rare allele of the rs111902263 (p.R115K) missense variant in patients (P=0.0019, odds ratio: 4.23, 95% confidence interval: 1.87–9.57). Particularly, three unrelated patients showed a homozygous genotype for K115, a situation not observed in the 1812 control individuals. Further cellular and molecular studies are required to elucidate the role of this gene and the variant K115 in brain development and neuronal function.

Les antipsychotiques (AP) conventionnels (APC) ou atypiques (APA) représentent le traitement médicamenteux de référence de la schizophrénie. Leur mécanisme d’action par blocage des récepteurs D2 est responsable d’effets indésirables dont les troubles neurologiques relativement fréquents, invalidants et susceptibles d’entraîner un arrêt du traitement. Dans le cadre de la certification, il est proposé de mesurer l’évolution des prescriptions des correcteurs des troubles neurologiques (antiparkinsoniens [APK]), quatre ans après une précédente étude afin d’améliorer leur prise en charge et par-là même la compliance du patient au traitement.

The Laser‐Diffraction Technique (LDT) based on the analysis of the light diffraction pattern forwardly scattered by droplets going through a laser optical probe, reports a drop‐diameter distribution. As reported by previous investigations this drop‐diameter distribution is sensitive to the shape of the drops and LDT measures an equivalent‐diameter whose definition has not been established so far. The experimental investigation presented in this paper tackles this very point. It has been motivated by the recent development of the surface‐scale distribution to characterize liquid drops. Being measured from Image Analysis Technique (IAT), the scale distribution contains information on the shape of the drops of the spray. The experimental work consists in analyzing a series of sprays with LDT and IAT to measure the surface‐based scale distribution. It is found that the virtual spray composed of spherical drops and whose diameter distribution is the one measured by the LDT has the same scale distribution as the actual spray. This result allows a relationship between the mean‐diameter series and the mean‐scale series to be established. This relationship is satisfied by the measurements and indicates that information on the droplet shape is available in the mean‐diameter series reported by the LDT. The analysis of the LDT mean‐diameter series confirms this point. The results reported by this experimental work participate to a better identification of the distribution measured by the LDT.

Administration of cyamemazine, an antipsychotic drug with anxiolytic properties, together with other antipsychotic agents is common in patients with schizophrenia. This retrospective study investigated the effects of cyamemazine on the steady-state plasma concentrations of risperidone and 9-hydroxyrisperidone in 47 patients treated with 1 to 12 mg/day of risperidone. Of these 47 patients, 24 were receiving cyamemazine comedication (“cyamemazine” group) and 23 patients were treated with risperidone alone (“control” group). Plasma concentrations were measured using a high-performance liquid chromatographic method with photodiode-array ultraviolet detection. The median plasma concentration of risperidone was significantly higher in the cyamemazine group (31.5 ng/mL) than in the control group (5.0 ng/mL), whereas the 9-hydroxyrisperidone median concentration was significantly lower in the cyamemazine group (16.5 ng/mL versus 39.0 ng/mL in the control group). However, the sum of risperidone plus 9-hydroxyrisperidone (active moiety) plasma concentration was not significantly affected by cyamemazine comedication. A combination with cyamemazine resulted in an inverted metabolic ratio (risperidone/9-hydroxyrisperidone). These findings suggest that cyamemazine inhibits the 9-hydroxylation of risperidone and is probably an inhibitor of cytochrome P450 2D6 as are many other phenothiazine drugs.

BACKGROUND Mutations in the angiogenin gene, ANG, have been associated recently with familial and sporadic forms of amyotrophic lateral sclerosis (ALS). However, the cellular and molecular mechanisms that link ANG, a multidomain protein, to ALS are still unknown. OBJECTIVE To assess the frequency of ANG gene mutations in 855 French patients with sporadic ALS. DESIGN We analyzed by direct sequencing the full coding region of the ANG gene in a cohort of French patients with sporadic ALS. The clinical characteristics of patients carrying ANG mutations are detailed. SETTING French ALS Study Group. PATIENTS A total of 855 patients with sporadic ALS. MAIN OUTCOME MEASURES Results of genetic analyses. RESULTS We observed a previously identified mutation (pI46V) in 2 patients with ALS without a known family link and found a novel mutation (pR121H) in 1 patient who developed ALS with rapid progression. We did not observe an association between patients with ALS and the rs11701 polymorphism, as previously reported in certain ALS populations of other ethnic origins. CONCLUSION Overall, our findings support the implication of ANG gene mutations as a rare but widespread cause of ALS.Arch Neurol. 2008;65(10):1333-1336--

Levetiracetam is a new antiepileptic drug prescribed for the treatment of patients with refractory partial seizures with or without secondary generalization as well as for the treatment of juvenile myoclonic epilepsy. A rapid and specific method by high-performance liquid chromatography diode array detection was developed to measure the concentration of levetiracetam in human plasma. The trough plasma concentrations measured in 69 epileptic patients treated with 500 to 3000 mg/d of levetiracetam ranged from 1.1 to 33.5 μg/mL. The mean (range) levetiracetam plasma concentrations in responders and nonresponders were 12.9 μg/mL (4.6-21 μg/mL) and 9.5 μg/mL (1.1-20.9 μg/mL), respectively. A wide variability in concentration-response relationships was observed in patients. Using a receiver operating characteristic curve, the threshold levetiracetam concentration for a therapeutic response was 11 μg/mL. The sensitivity and specificity for this threshold levetiracetam concentration were 73% and 71%, respectively. According to χ2analysis, this finding was not significant probably because of the small number of patients and because of their refractory seizure type. Nevertheless, the levetiracetam plasma concentration could be used to help clinicians detect severe intoxication or to verify compliance by repeating the measurement in patients.

This study enabled us, to define the quantitative parameters of the interactions which give rise to the formation of complexes between five phenylindanediones and albumin or polymethacryate. The existence of three attachment sites were demonstrated, by which these ligands are bound to the macromolecule. The results from the thermodynamic study of this phenomenon, together with the information supplied by; various spectroscopic methods; the formation of a complex between a synthetic polymer carrying amine functions, and the study of ligands in the presence of surfactant, were all in agreement. Furthermore, a significant correlation between the association constant and physiochemical parameters lipophilic character and acidity was obtained, leading to the conclusion that; the interactions permitting the formation of the complexes between albumin and the five anionic ligands seem to be principally hydrophobic and, to a lesser degree, electrostatic. The latter only occurred in a particular medium at sites which were mainly hydrophobic.

La prise en charge pharmacothérapeutique prend aujourd’hui de l’ampleur en établissements publics de santé mentale (EPSM). Dans de telles structures où les durées de séjours sont longues, le financement par dotation annuelle fait souvent obstacle à l’accès à l’innovation thérapeutique. A l’aube du plan « Ma Santé 2022 », le risque de rupture du continuum de soin est bien présent au regard de la complexité du financement de certaines thérapies à haute valeur ajoutée. Malgré une volonté d’adaptation du système pour répondre aux besoins sanitaires en constante évolution, contrairement aux structures de soins de suite et de réadaptation (SSR), aucune démarche relative aux médicaments onéreux (MO) n’a été entreprise à ce jour en EPSM à notre connaissance. Ceci renforce le décalage avec l’évolution de la recherche, et creuse davantage le fossé des inégalités entre secteurs sanitaires. Optimiser le financement des MO en psychiatrie octroierait la possibilité de diminuer l’étau des dotations en vigueur. A l’instar des réformes récentes en SSR, une prise en charge en sus des thérapeutiques coûteuses permettrait de réduire les arbitrages médicaux complexes: de la prévention à la réinsertion, la continuité de la prise en charge du patient en serait davantage garantie.

Deux indications de Avastin® (bevacizumab) ont été radiées de la liste en sus (LES) le 01/09/16 : cancer (K) du sein (à l’exception du triple négatif (TN) en 1re ligne en association au paclitaxel qui reste financé par un système dérogatoire) et K du rein. Selon un groupe d’experts de l’AP-HP, son utilisation dans le K du rein est marginale. Néanmoins, un besoin médical persiste dans le K du sein TN en association à la capécitabine en échec ou contre indiqué à un taxane et dans le K du sein HER2-/RH faiblement+en association au paclitaxel. L’objectif de ce travail est d’évaluer l’évolution des pratiques deux ans après la radiation de ces indications de la LES à l’AP–HP.

Les dispositifs médicaux (DM) s’inscrivent dans un contexte réglementaire en cours de révision au niveau européen. De nombreuses évolutions sont attendues, impliquant tous les acteurs du circuit du DM. L’objectif de ce travail était de dresser un état des lieux de la traçabilité des dispositifs médicaux implantables (DMI) et de mettre en place les actions correctives nécessaires.

L’utilisation croissante des dispositifs de traitement par pression négative (TPN) et l’élargissement de l’offre du marché, associés au manque de données cliniques (HAS, 2016) ont conduit le Comité des dispositifs médicaux stériles (CODIMS) de notre établissement à réunir un groupe d’experts (pharmaciens, infirmiers, médecins) pour mener une réflexion sur la place du TPN dans la stratégie de prise en charge des plaies et sur l’intérêt et le besoin en dispositifs de TPN.
