Although building preservice teachers’ racial literacy is a key goal to reducing racial inequity in K-12 settings, little attention has been paid to how literacy teacher educators are prepared to conduct this work. This is unsurprising given the neoliberal logic underpinning universities today. In this multiduo autoethnographic study, six literacy teacher educators utilized reflection on critical incidents to examine their own racial literacy development. By forming cross-racial pairs and utilizing a combination of writing and dialogue, these six participant/researchers interrogated their own views and experiences of race and racism. Findings suggest that the examination of critical incidents can support teacher educators’ racial literacy growth. Specifically, participants reflected on the vulnerability necessary to support preservice teachers’ racial literacy development and utilized ethnographic methods to build the practice of sharing about their own racial literacy learning. They also used critical incidents to come to a deeper understanding of the construct of racial literacy itself. These results imply that for professional organizations such as the Literacy Research Association to deliver on their espoused commitments to addressing ongoing institutional racism, they must intentionally create professional spaces for coreflection and dialog about racial literacy. Further, we consider the implications of this work for literacy doctoral programs and teacher educator preparation within contexts of college and university-based teacher education programs.

A systematic review was conducted to describe, and evaluate the effectiveness and cost-effectiveness of, intermediary services to support people with a disability to implement individualized funding plans. We included six records, including one subanalysis of randomized trial data, three qualitative studies, and two systematic reviews (reporting on 73 and 18 studies, respectively). No studies directly compared “consumer-directed plan plus intermediary services” to “consumer-directed plan with no/alternative intermediary,” so effectiveness of these interventions is uncertain. There is qualitative evidence from the perspective of disability planners and workers that intermediary interventions are important enablers of successful plan implementation. There is also qualitative evidence from consumer and family perspectives that external support is required to successfully navigate self-directed systems and that strong, trusting, and collaborative relationships with both paid and unpaid individuals in the person’s support network were facilitators of successful plan implementation. There was evidence of disabling practices and attitudes among some support agencies, resulting in coordinators being very risk averse in order to safeguard their clients. Suggestions for future research include carefully planned and ethically robust comparative trial designs, clear description and consistent delivery of interventions, and long-term evaluation of impact. Science texts tend to privilege the voices, perspectives, and practices of White males (e.g., Ford, 2006), reinforcing widely‐held perceptions of scientists as White men. This narrow portrayal of who “does science” discourages BIPOC or female students from building a science identity (Archer et al., 2015), with many such students also viewing science as difficult or undesirable (Brickhouse et al., 2000). We argue that intentional positioning of science text written by or about BIPOC or female figures in science and literacy instruction not only helps allstudents develop science and literacy skills and knowledge, but leads to more expansive views of who “does science” and fosters students’ science identities. Our purpose in this article is to help teachers identify and incorporate expansive informational texts and position these texts within sound science and literacy instruction. We draw from the Next Generation Science Standards to describe ways to develop students’ science identities and then provide teachers guidance for selecting and intentionally positioning expansive science texts in their instruction, with examples of text sets that teachers can use in their classrooms.

This qualitative study examined the interplay between teacher facilitation, children’s uptake of vocabulary and reasoning strategies, and the roles children assumed as learners as they experienced instruction grounded in Connected Teaching and Learning ([CTL] an interdisciplinary instructional framework that leverages key practices from culturally responsive pedagogies and meaningful use of multimodal text sets. Analyses suggest (1) students assumed more active roles in their learning as they “enacted” the work of scientists and (2) varied teacher facilitation practices and children’s vocabulary and reasoning uptake were key factors in children’s shift to more active roles. Although findings suggest CTL is a promising instructional framework, findings also underscore the significance of how teachers act on the instructional framework.

Femoroacetabular impingement (FAI) can present with debilitating symptoms in an athlete, affecting both their level of performance and activities of daily life. The prevalence of FAI in the athletic population is greater than in the general population, but the reasons are unknown. The rate of return to sport following surgery for FAI has been a popular research topic, and results have been promising. However, there are still a significant number of athletes who do not return to their sport. Additionally, the rate of return to previous level of competition has largely been neglected in the literature, which is arguably more important than the rate of return to sport. The literature is varied regarding radiographic assessment of FAI, patient reported outcome measures, time to follow-up, and overall methodological quality. This chapter aims to review risk factors for failure of return to sport, and predictors affecting the return to previous level of competition in the athlete after FAI treatment. Replicating the original bony anatomy during radial head replacement or fracture repair is important for both joint stability and prevention of arthritis, but can be difficult to achieve. An important landmark for determining appropriate radial head height that is often overlooked is the proximal radioulnar joint (PRUJ). We propose a methodology to use an en face view of the lesser sigmoid notch as a fluoroscopic reference point to establish appropriate height for the proximal border of the prosthesis in radial head replacement. 5 fresh frozen cadaveric elbows were dissected medially to assess the relationship of the radial head to the lesser sigmoid notch of the ulna while maintaining structural integrity of the lateral stabilizers. Plain radiographs of each elbow were obtained to define a true anteroposterior view of the PRUJ. The radial head and proximal ulna were resected to measure their cartilage thickness. The angle of the lesser sigmoid notch was found to be an average of 28.2 degrees, which correlated to a 30-degree external oblique radiograph of the elbow as providing an en face view of the PRUJ. The average thickness of the radial head cartilage rim in the coronal plane was 1.95 mm, while the middle aspect of the lesser sigmoid notch was 2.5 mm. A 30-degree lateral oblique radiograph of the elbow provides a true anteroposterior view of the PRUJ, which can be a useful adjunct for restoring radial length in situations such as radial head replacement or fracture repair. Four decades of research in the field of Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) have yielded lessons that may be instructive for those devising criteria to better comprehend Post-Acute Sequelae of SARS CoV-2 Infection (PASC) and Long COVID. For instance, substantial effort has been devoted to defining classification systems, operationalizing methods, and developing instruments with adequate reliability and validity in the ME/CFS field. The current article provides guidelines for developing a case definition for Long COVID and discusses the significance of psychometric issues and criterion variance, including how to specify symptoms, develop thresholds, subtypes, and exclusionary conditions. ME/CFS research could enhance our knowledge of Long COVID pathophysiology, early diagnosis, prognosis, and the identification of effective treatments.

Supplemental Digital Content is Available in the Text.This review examines the effects of psychosocial/vocational interventions compared with others for improving work outcomes in patients within 3 mo of acute myocardial infarction (AMI). A systematic review and grading of quality of literature was undertaken for 18 studies. Although these interventions show promise in post-AMI patients, compelling evidence is limited for improved outcomes.

Case report.

Optimal patient-physician communication in the outpatient clinical setting is critical for safe and effective patient care. Keeping track of multiple patient telephone messages can be difficult and hazardous if a structured system is not in place. A multidisciplinary group at Hershey Medical Center developed a standardized approach for addressing patient telephone calls at their outpatient surgical clinics. This program was designed to improve the patient experience by providing a realistic time frame for phone calls to be returned and requests fulfilled. Additionally, this system permitted phone calls to be tracked and documented appropriately and allowed for prioritization of urgent and emergent messages. Our intent for this program was to close potential gaps within the communication chain at our outpatient surgical clinics, improve overall communication between clinicians and their patients, and improve both patient and employee satisfaction.

Scoping review.

Introduction: The aim of this study was to compare the effectiveness of 2 interventions in prompting patients to obtain osteoporosis follow-up after a fracture. Our hypothesis was that a phone call plus letter would yield greater response toward osteoporosis evaluation versus a letter alone to patients after sustaining a fragility fracture.Materials and Methods: Prospective study randomized 141 patients age 50 years and older with a fragility fracture into 3 groups for comparison. Group 1 (letter only) patients received a letter 3 months after their diagnosis of fracture indicating their risk for osteoporosis and urging them to follow-up for evaluation. Group 2 (phone call plus letter) patients were contacted via phone 3 months after their diagnosis of fracture. A letter followed the phone call. Group 3 (control) patients were neither contacted via phone nor sent a letter. All groups were contacted via phone 6 months after their initial visit to determine if they followed up for evaluation.Results: In group 1, 23 (52.27%) of 44 had follow-up, and 21 (47.73%) of 44 did not follow-up. In group 2, 30 (62.5%) of 48 had follow-up, and 18 (37.50%) of 48 did not follow-up. In group 3, 6 (12.24%) of 49 had some sort of follow-up, and 43 (87.76%) of 49 did not have any follow-up. A statistical significance was achieved between group 3 (control) and both groups 1 and 2 with regard to follow-up (P< .0001). The results did not show a statistically significant difference between Groups 1 and 2, however, there was a trend toward improved response with a phone call plus letter (P= .321).Conclusion: A more personalized approach with a phone call plus follow-up letter to patients increased osteoporosis follow-up care by an additional 10%, however, this was not a statistically significant difference from just sending out a letter alone.

This mixed-methods study explored joining use of increasing-complex text with sound instructional practices on English Learners’ (ELs) academic language and conceptual knowledge. Findings showed one-week postintervention, ELs achieved significant academic vocabulary gains such that there were no differences between ELs and general education (GE) students. Moreover, six weeks postintervention, ELs’ academic language and conceptual knowledge closely approximated that of their GE peers. Findings suggest that carefully scaffolded use of increasingly-complex text holds promise in advancing ELs’ conceptual knowledge and academic language. Given the widespread use of simplified text as ELs’ primary reading curriculum—a practice that unintentionally delimits language and knowledge learning opportunities, and in turn, contributes to persistent achievement gaps—efforts aimed at identifying practices that build academic language and conceptual knowledge and in turn, enable access to complex text are of particular importance. If replicated in larger studies, teachers will have a clear alternative to such practices.

Purpose:A genetic diagnosis is an extremely useful tool in the management and care of families with inherited heart diseases, particularly in allowing clarification of risk status of asymptomatic family members. The psychosocial consequences of genetic testing in this group are poorly understood. This longitudinal pilot study sought to determine changes in health-related quality of life in patients and asymptomatic family members undergoing genetic testing for inherited heart diseases.Methods:Individuals attending two specialized multidisciplinary cardiac genetic clinics in Australia were invited to participate. Patients undergoing proband or predictive genetic testing for an inherited cardiomyopathy or primary arrhythmogenic disorder were eligible. The Medical Outcomes Short Form-36 (version 2) was administered before the genetic result was given, and follow-up surveys were completed 1–3, 6, and 12 months after the result was given.Results:A total of 54 individuals with hypertrophic cardiomyopathy, familial dilated cardiomyopathy, arrhythmogenic right ventricular cardiomyopathy, and long QT syndrome completed baseline and at least one follow-up survey, including 33 probands and 21 asymptomatic relatives. Physical and mental component scores analyzed at baseline and 1–3 months were found to be unchanged in all groups. Furthermore, no significant differences were observed up to 12 months after result.Conclusion:In this longitudinal pilot study, no change in health-related quality of life was observed up to 12 months after the result was given in patients and their asymptomatic family members undergoing genetic testing for an inherited heart disease.Genet Med 2012:14(8):749–752.

The characteristic level of occupational functioning among injured workers with musculoskeletal disorders needs to be explored to provide efficient and quality services. To date, English version of Occupational Self Assessment version 2.2 has not been translated and validated in Malaysian language. Therefore, this study aims to translate the 21-item Occupational Self Assessment and to measure its psychometric properties. A total of 35 participants were assessed using the finalised Malaysian version. 23 of them were re-assessed 7 to 14 days later. The results suggest that it is reliable and valid in assessing occupational functioning for injured worker with musculoskeletal disorders.

Introduction: It has been difficult for occupational therapists in Asia to use occupational therapy assessments, such as the Occupational Self Assessment (OSA) version 2.2, that are not validated in the local language. This study aimed to produce and assess the psychometric properties of a Malaysian version of the 21-item OSA.Method: The translation process involved the forward and back translation from the original English version into the Malaysian version, followed by field tests conducted with a small group of occupational therapists and workers to confirm the translation's equivalence and appropriateness. A total of 35 Malaysian injured workers with musculoskeletal disorders who were not involved in a return to work programme were assessed using the Malaysian version of the OSA. Twenty-three were reassessed 7–14 days later to determine test-retest reliability.Results: The Malaysian version of the OSA showed acceptable reliability (internal consistency, corrected item correlation and test-retest reliability) and validity (convergent and discriminant validity).Conclusion: The results suggest that the OSA is reliable and valid in assessing occupational functioning for injured Asian workers with musculoskeletal disorders.

The study design is qualitative phenomenological and grounded theory. Intraarticular fractures of the finger joints can severely limit function due to stiffness and pain. Distraction with early movement is thought to deliver the best results and this has been used to treat these types of injuries at The Alfred Hospital for eight years. Qualitative data from patient interviews were used to describe patients' own experiences of treatment with distraction splinting and identify key issues in patient adherence. The key finding was a disconnect between perceived complexity of injury and treatment. Those who adhered with the treatment regime felt that they were well informed of the reasoning behind it. The hand surgery and therapy team must be aware of the patient experience of complex finger injuries and should ensure patients are well supported with education about their injury and treatment. Early preemptive pain control may help optimize adherence to the splint and exercise regime. Findings can be applied to other acute conditions requiring cumbersome splinting and potentially uncomfortable early exercise routines.

Objective: Depression is a major public health concern that results in a wide range of economic costs to people, their families, and the health care system. Our study sought to determine the direct medical costs incurred by the Ontario government owing to cessation of antidepressant therapy during pregnancy.Methods: We conducted an economic evaluation by making assumptions based on data obtained from Statistics Canada, federal and provincial government reports, and relevant depression literature. The analysis included the number of pregnant women with depression residing in Ontario and, subsequently, the number of those women who experienced depressive relapse during pregnancy owing to discontinuation of antidepressant medication. The cost of physician services, hospitalizations, and the birth of preterm and low birth weight infants (2 adverse outcomes associated with untreated depression during pregnancy) were also taken into consideration.Results: An estimated 2953 pregnant women with depression in Ontario annually discontinue antidepressant therapy and subsequently have a depressive relapse. An estimated $20 546 982 is spent annually in Ontario on untreated maternal depression in pregnancy; this is the total after subtracting the cost of risks associated with treated depression during pregnancy ($3 144 053).Conclusions: Safe treatment options for the management of depression during pregnancy should be actively explored as treated depression translates into cost savings for the Ontario government and society as a whole. Beyond this cost, depression interferes with the quality of childrearing, maternal responsiveness to infants, and other determinants essential for optimal child development.
