This research examined the effect of regular home- and hospital-based cardiac rehabilitation exercise on state and trait hopelessness levels in 324 patients with coronary heart disease. For individuals with moderate to severe baseline hopelessness, home exercise was significantly associated with decreased state hopelessness, even after adjusting for other key variables. Baseline hopelessness was a predictor of attrition in this cohort, especially for home exercisers, but this effect was mediated in hospital-based exercise programs.

Recently, a genome-wide screening identified a functional single-nucleotide polymorphism in dual-specificity phosphatase 14 gene (DUSP14), which was associated with pulmonary tuberculosis (TB) in a West African study. DUSP14 regulates T-cell proliferation and cytokine production in a negative way via dephosphorylation and inactivation of key signaling molecules. The aim of this study is to further explore the possible significance of the DUSP14polymorphism. Total RNA was extracted from the whole blood of 109 healthcare workers (HCWs) in Vietnam and subjected to quantitative reverse-transcription PCR for DUSP14and 20 immune-related genes. DUSP14rs1051838 was genotyped in 502 new pulmonary TB patients and 506 healthy controls. Among disease-free individuals (HCWs), T-helper type-1 (Th1)-related genes, interferon-gamma receptor 2 (IFNGR2) and signal transducer and activator of transcription-1 (STAT1) mRNA levels significantly increased as the number of A alleles of rs1051838 increased, whereas the DUSP14mRNA level tended to decrease. The AA genotype was associated with protection against active TB in younger patients (?45 years old, OR=0.63, 95% CI 0.44–0.90). Our results suggest that a low-expression genotype of DUSP14accompanied by high transcript levels of Th1 immune-related genes may confer protection against early TB development.

A 45 nm microprocessor integrates an all-digital dynamic variation monitor (DVM) to continuously measure the impact of dynamic parameter variations on circuit-level performance to enhance silicon debug and adaptive clock control. The DVM consists of a tunable replica circuit, a time-to-digital converter, and multiplexers to measure circuit delay or frequency changes with less than a 1% measured resolution error while capturing clock-to-data correlations. In validating the DVM with microprocessor maximum clock frequency (FMAX) measurements, an on-die noise injector circuit induces a supply voltage (VCC) droop at a particular cycle in the test program. The FMAX measurement is then repeated for over a thousand iterations while shifting the droop placement to a different cycle per iteration. Silicon measurements demonstrate the DVM capability of tracking the worst case FMAX reduction to within 1% for a wide range of VCC droop profiles. Furthermore, silicon measurements reveal that FMAX is highly sensitive to the placement and magnitude of a high-frequency VCC droop during program execution, thus highlighting the value of the DVM for silicon debug. In addition, the DVM interfaces with an adaptive clock control circuit to dynamically adjust the clock frequency by changing the divide ratio in the phase-locked loop in response to persistent variations, enabling the microprocessor to adapt to the operating environment for maximum efficiency. The micronization of an asthma active pharmaceutical ingredient, fluticasone propionate, using the supercritical antisolvent (SAS) process was investigated in this study. The most commonly used supercritical carbon dioxide was employed as the antisolvent. The effects of five process parameters, including operating temperature, operating pressure, solution flow rate, solution concentration, and nozzle diameter, were compared and discussed. The physical properties of micronized fluticasone propionate were examined by the particle size analyzer, the scanning electron microscope and X‐ray diffractometer. The cross interaction effect of operating temperature and pressure was observed in the SAS treatment of fluticasone propionate and verified by the method of calculated mixture critical point (MCP), using the predictive Soave‐Redlich‐Kwong equation of state. Operation slightly above the MCP was suggested for successful micronization of fluticasone propionate.

Advancements in reconfigurable technologies, specifically FPGAs, have yielded faster, more power-efficient reconfigurable devices with enormous capacities. In our work, we provide testament to the impressive capacity of recent FPGAs by hosting a complete Pentium® in a single FPGA chip. In addition we demonstrate how FPGAs can be used for microprocessor design space exploration while overcoming the tension between simulation speed, model accuracy, and model completeness found in traditional software simulator environments. Specifically, we perform preliminary experimentation/prototyping with an original Socket 7 based desktop processor system with typical hardware peripherals running modern operating systems such as Fedora Core 4 and Windows XP; however we have inserted a Xilinx Virtex-4 in place of the processor that should sit in the motherboard and have used the Virtex-4 to host a complete version of the Pentium® microprocessor (which consumes less than half its resources). We can therefore apply architectural changes to the processor and evaluate their effects on the complete desktop system. We use this FPGA-based emulation system to conduct preliminary architectural experiments including growing the branch target buffer and the level 1 caches. In addition, we experimented with interfacing hardware accelerators such as DES and AES engines which resulted in a 27x speedup.

Objectives. Tophi deposits are a well-known cause of joint destruction, gouty nephropathy and spinal cord compression. This study reports another major complication of gout, namely tophi deposition causing limited knee joint excursion.Methods. Seven gout patients with limited knee joint excursion owing to tophi deposition were studied to reveal clinical features and magnetic resonance imaging (MRI) findings. None of the patients were able to assume a full squatting posture.Results. No patients had visible subcutaneous tophi over the knee joints, except for one case in which a pea-sized subcutaneous tophus was noted. Additionally, two patients even lacked visible tophi elsewhere. All knee problems in the study group were initially regarded as being due to degenerative or other internal derangements, but MRI unexpectedly revealed multiple tophaceous depositions within and around the joint.Conclusions. Intra-articular and periarticular tophi limiting knee joint range of motion are a rare but important cause of walking disability in gout patients. Although most patients do not display visible subcutaneous tophi over the knee on physical examination, the differential diagnosis should consider intra-articular tophi and MRI is valuable in this clinical setting.

Introduction:Dyssynchrony indices based on two-dimensional speckle tracking echocardiography have demonstrated added value in identifying patients with mitral valve prolapse (MVP) with a higher prevalence of arrhythmic complications and the potential for sudden cardiac death, but measurements are restricted to a single plane, and complex left ventricular (LV) dyssynchrony patterns may be overlooked.Hypothesis:The purpose of this study was to investigate whether three-dimensional speckle-tracking echocardiography helps in detecting MVP at higher arrhythmic risk.Methods:We studied 21 arrhythmic MVP patients (group 1) with a history of complex ventricular ectopy on holter and/or event monitor (n=17) or defibrillator implant (n=2), 21 MVPs with no arrhythmic complications (group 2) and 21 healthy controls (group 3). 3D LV longitudinal strain (3DE-LVLS) and area strain (3DE-LVAS) were determined (17 segments). 3D LV dyssynchrony index (3DE-LVDI) was obtained as the standard deviation of the times to peak area strain in 17 segments, normalized to RR interval.Results:MVP patients had significantly higher LV dyssynchrony index compared to controls (9.4±3.9% vs 4.3±2.5%, p=0.002) although they had similar LV ejection fraction (64% vs 61%, p=0.51). Group 1 and group 2 had similar LV ejection fraction and clinical data (p>0.05). Bileaflet prolapse was more frequent in group 1 pts (52% vs 24%, p=0.03). Moderate mitral regurgitation was present in 9/21 group 1 pts and 3/21 group 2 pts (p=0.04). Group 1 pts had greater LV dyssynchrony index when compared with group 2 (10.7±4.1% vs 5.1±2.9%, p=0.0001). By ROC (receiver operating characteristics) curves, 3DE-LVDI and 3DE-LVAS cutoff values of 10.2% and -23.8%, respectively, had 89% and 85% sensitivity and 79% and 76% specificity in identifying the presence of significant arrhythmias with areas under the curve 0.87 and 0.84.Conclusions:Impairment of LV deformation parameters determined by three-dimensional speckle tracking echocardiography may help detect increased arrhythmic risk in patients with MVP.

The childhood spinal muscular atrophies (SMAs) are the most common, serious neuromuscular disorders of childhood second to Duchenne muscular dystrophy. A single locus for these disorders has been mapped by recombination events to a region of 0.7 centimorgan (range, 0.1-2.1 centimorgans) between loci D5S435 and MAP1B on chromosome 5q11.2-13.3. By using PCR amplification to screen yeast artificial chromosome (YAC) DNA pools and the PCR-vectorette method to amplify YAC ends, a YAC contig was constructed across the disease gene region. Nine walk steps identified 32 YACs, including a minimum of seven overlapping YAC clones (average size, 460 kb) that span the SMA region. The contig is characterized by a collection of 30 YAC-end sequence tag sites together with seven genetic markers. The entire YAC contig spans a minimum of 3.2 Mb; the SMA locus is confined to roughly half of this region. Microsatellite markers generated along the YAC contig segregate with the SMA locus in all families where the flanking markers (D5S435 and MAP1B) recombine. Construction of a YAC contig across the disease gene region is an essential step in isolation of the SMA-encoding gene.

A polyclonal antiserum directed against the C-terminal domain of dystrophin was used to isolate a cDNA clone encoding an antigenically cross-reactive protein, microtubule-associated protein 1B (MAP-1B). Physical mapping of the human MAP-1B locus places its chromosomal location at 5q13, in proximity to the spinal muscular atrophy (SMA) locus. SMA is a degenerative disorder primarily affecting motor neurons. Genetic linkage analysis of SMA families using a human dinucleotide repeat polymorphism just 3' of the MAP-1B gene has shown tight linkage to SMA mutations. These mapping data together with the postulated role of MAP-1B in neuronal morphogenesis and its localization in anterior horn motor neurons suggest a possible association with SMA.

Identifying the full repertoire of cis elements required for gene expression in mammalian cells (or animals) is challenging, given the moderate sizes of many loci. To study how the human gp91-phox gene is expressed specifically in myeloid hematopoietic cells, we introduced yeast artificial chromosome (YAC) clones and derivatives generated in yeast into mouse embryonic stem cells competent to differentiate to myeloid cells in vitro or into mouse chimeras. Fully appropriate regulation was recapitulated with a 130-kb YAC containing 60 and 30 kb of 5' and 3' flanking sequences, respectively. Immunodetection of human gp91-phox protein revealed uniform expression in individual myeloid cells. The removal of upstream sequences led to decreased overall expression which reflected largely a variegated pattern of expression, such that cells were either "on" or "off," rather than pancellular loss of expression. The proportion of clones displaying marked variegation increased with progressive deletion. DNase I mapping of chromatin identified two hypersensitive clusters, consistent with the presence of multiple regulatory elements. Our findings point to cooperative interactions of complex regulatory elements and suggest that the presence of an incomplete set of elements reduces the probability that an open chromatin domain (or active transcriptional complex) may form or be maintained in the face of repressive influences of neighboring chromatin.

The thermomechanical history of the melt under injection compression molding follows a quadrilateral-shaped path, rather than conventional triangular-shaped path, over the P-v-T diagram. The control capacity and flexibility over post-filling is significantly enhanced. It thus can produce parts with better dimensional control with less frozen-in orientation than those from conventional injection molding. This paper is devoted to study its technological application on the precision molding of parts with precision contours. A modified injection molding machine incorporating compression mechanism, and a mold with movable wall and shut-off mechanism, are used. The quality of injection-compression-molded polysytrene convex lenses is defined and compared with those injection molded. With the same cooling time, injection compression can produce lenses with better quality than those without compression
