We sought to report the effectiveness of infliximab and adalimumab over the first 3 years of treatment and to define the factors that predict anti-TNF treatment failure and the strategies that prevent or mitigate loss of response.

Aim:Community stakeholder engagement in research (CSER) can improve research relevance and efficiency as well as prevent harmful practices, particularly for vulnerable populations. Despite potential benefits, researchers lack familiarity with CSER methods. Methods:We describe CSER strategies used across the research continuum, including proposal development, study planning and the first years of a comparative effectiveness study of care for pregnant women with opioid use disorder. Results:We highlight successful strategies, grounded in principles of engagement, to establish and maintain stakeholder relationships, foster bidirectional communication and trust and support active participation of women with opioid use disorder in the research process. Conclusion:CSER methods support research with a disenfranchised population. Future work will evaluate the impact of CSER strategies on study outcomes and dissemination.

Anti-TNF drugs are effective treatments for the management of Crohn's disease but treatment failure is common. We aimed to identify clinical and pharmacokinetic factors that predict primary non-response at week 14 after starting treatment, non-remission at week 54, and adverse events leading to drug withdrawal.

ABSTRACTBackgroundThe Accreditation Council for Graduate Medical Education (ACGME) Clinical Learning Environment Review (CLER) is an innovative element of the ACGME's new accreditation system. To date, little information has been collected regarding the value of CLER.ObjectiveThe purpose of this study is to collect information on designated institutional officials' (DIOs') perspectives about the initial CLER visits conducted at their institutions.MethodsThe authors created and distributed a survey to DIOs about their initial CLER visits. Demographic data were compared across survey responses with Spearman's rank correlation and the Kruskal-Wallis test.ResultsThe authors received responses from 63 of DIOs (186 of 297) at institutions that participated in the initial CLER visits, with 88 (164 of 186) having served as DIO during the visit. Seventy-two percent (114 of 158) reported institutional changes to address CLER focus areas prior to the visit, yet only 32 (51 of 157) reported that additional resources were allocated to these areas after the site visit. Sixty-five percent (102 of 156) reported institutional executive leadership was positive about participating in CLER; 85 (134 of 158) reported that ACGME conducted the visits efficiently; 84 (133 of 158) reported that the site visit accurately assessed the institution's performance in the CLER focus areas; and 60 (93 of 156) reported CLER provided high-value information.ConclusionSurvey results from DIOs suggest that CLER is an effective mechanism to improve the learning environment. Common concerns included limited advance notice for the site visit and disruptions of clinical practice.

This paper reports the quantitative findings of a study on conceptions of positive career outcomes held by Chinese students who returned home to work after gaining higher education qualifications in Australia. The study explores what positive career outcomes mean to Chinese returnees, the actual positive career outcomes Chinese returnees have experienced and contributing factors to the positive career outcomes achieved. Both quantitative and qualitative research methods are used and here the analysis of the quantitative findings is presented. The survey data show that Chinese returnees assign greater importance to ‘soft outcomes’ relating to personal and life fulfilment than ‘hard outcomes’ such as employment results. Also, the discrepancy between the returnees’ ideal and actual outcomes is greater in the soft outcomes than the hard outcomes. Statistical analysis indicates three emergent, distinct groups of conceptions of positive career outcomes. The qualitative findings will be presented separately.
