Unattachment to a regular primary care professional can affect children’s and adolescents’ well-being, considering their unique health needs. Having no alternative, many turn to emergency departments for non-urgent conditions. To help unattached patients access healthcare services while on waitlists, Quebec’s government implemented single access points in each administrative region across the province. Our study aimed to describe the paediatric population using single access points and identify associations between their characteristics and need for a medical appointment. Clinical-administrative data of 1,323 paediatric access point users in the Montérégie region from November 2022 to March 2023 were utilized to conduct bivariate and multivariable regression analyses. Our study showed that young age, assessment trajectory, and specific reasons for calling were more likely to necessitate a medical appointment. While access points improve accessibility to doctors, questions remain regarding the relevance of medical consultations, inequities, and possible security issues resulting from the overall process.

Objectives: To address global aging, a paradigm shift is needed from disease prevention and treatment towards active aging, i.e., optimizing opportunities for health, participation, and security as people age. Little is known about how age-friendly environments promote active aging. This study thus aimed to explore how (through which mechanismsand in what contexts) environments can promote active aging and, specifically, positive health, social participation, and health equity. Methods: Using a realist approach and semi-structured focus groups, a case study was used in two Quebec municipalities known for best fostering active aging. Data also included participants’ logbooks, sociodemographic questionnaires, municipalities’ sociodemographic profiles, and policy documents. A conceptual framework and thematic content analysis were carried out. Results: A total of 24 participants (9 older adults, 4 health professionals, 3 community-based actors, 5 municipal employees, and 3 elected officials) took part in 5 focus groups. Regarding contexts, both cases were midsize municipalities having an income and education level higher to Quebec’s averages with supportive active aging policies. Two main themes explained how the environments promoted active aging: (1) by ensuring proximitythrough built (urban planning), social (network structures), services (variety and availability of local and outreach resources), and organizational (active listening to older adults’ needs for active aging) environments; and (2) by fostering transversalitythrough built (universal accessibility, intergenerational spaces), social (intergenerational opportunities for social participation), and political/organizational (unified and complementary policies) environments. Conclusion: To better promote active aging through age-friendly environments, practices should focus on fostering proximity and transversality, and act simultaneously on multiple environments.

STUDY DESIGN: Messenger ribonucleic acid from amnion, chorion, endometrium, endocervix, myometrium, placenta, small intestine, peripheral blood lymphocytes, and cervical, endometrial, and trophoblast cell lines was reverse transcribed with a 3'-RACE adapter. 3'-RACE polymerase chain reaction was conducted with an upstream human defensin 5 primer and 3'-RACE adapter primer. Polymerase chain reaction products hybridizing to a human defensin 5 probe were cloned for sequence analysis. Sequence data were compared against a nucleotide sequence database, and secondary structure predictions were made. RESULTS: Chorionic tissue, endocervical tissue, endometrial tissue, and an endometrial cell line all demonstrated a single hybridizing 362 bp polymerase chain reaction product. Sequence analysis of all clones demonstrated near-perfect identity with human defensin 5. CONCLUSIONS: Human endocervix, endometrium, and chorion express defensin 5 at the level of transcription. While this increased energy demand during capacitation is well established, it remains unclear how mouse sperm modify their metabolism to meet this need. We now show that capacitating mouse sperm enhance glucose uptake, identifying glucose uptake as a functional marker of capacitation. Using an extracellular flux analyzer, we show that glycolysis and oxidative phosphorylation increase during capacitation. Furthermore, this increase in oxidative phosphorylation is dependent on glycolysis, providing experimental evidence for a link between glycolysis and oxidative phosphorylation in mouse sperm.

The German energy transition towards more sustainable forms of energy production has been characterized as a large-scale or real-world experiment. Whereas experiments are open-ended processes set up explicitly to allow (or even generate) surprises, by contrast sustainability implies the pursuit of clearly defined, normative ends. Whereas much of the literature on system transformation builds on the concept of innovation, our hypothesis is that focusing on the “natural” flipside of innovation—called here “exnovation,” i.e., departing from unsustainable pathways—should also be seen as a valuable conceptual strategy for coping with the tension between the unavoidable indeterminacy resulting from unknown risks and the necessary amendment and redefinition of goals and rules. In this paper the German energy transition (Energiewende) is used to exemplify the recursive processes of experimentation that make it possible to accommodate surprise, and, thus, to conceptualize the unavoidable tension between innovation and the maintenance of older, unsustainable structures.
