Actigraphy has been used for more than 60 years to objectively measure sleep–wake rhythms. Improved modern devices are increasingly employed to diagnose sleep medicine disorders in the clinical setting. Although less accurate than polysomnography, the chief advantage of actigraphs lies in the cost-effective collection of objective data over prolonged periods of time under everyday conditions. Since the cost of wrist actigraphy is not currently reimbursed, this method has not enjoyed wide acceptance to date. The present article provides an overview of the main clinical applications of actigraphy, including the recommendations of specialist societies.

Hintergrund: Ein Teil der Bevölkerung befürchtet negative Auswirkungen niederfrequenter Magnetfelder (NF-MF), die z. B. von Hochspannungsleitungen und anderen Anlagen und Geräten mit der Frequenz 50 Hz bzw. 16 2/3 Hz ausgehen. Manche Studien zeigen einen Zusammenhang zwischen einer NF-MF-Exposition und einem erhöhten Risiko für neurodegenerative Erkrankungen, u. a. für die Alzheimer-Demenz (AD). Ziel der Arbeit und Methode: In dem vorliegenden narrativen Review wird der aktuelle Stand der Forschung zu NF-MF und möglichen Auswirkungen auf das AD-Risiko und den Schlaf anhand epidemiologischer und experimenteller Studien zusammengefasst und methodenkritisch diskutiert. Ergebnisse und Diskussion: In epidemiologischen Studien konnte sowohl in Untersuchungen zur beruflichen Exposition gegenüber niederfrequenten Magnetfeldern als auch in Untersuchungen zur häuslichen Exposition durch Hochspannungsleitungen ein geringfügig erhöhtes Risiko für Alzheimer-Demenz beobachtet werden. Als signifikant erwies sich dieses Risiko allerdings nur in den Metaanalysen zur beruflichen Exposition. Die Studien zeichnen sich durch eine große Heterogenität aus, weshalb offen bleibt, ob den Beobachtungen ein ursächlicher Zusammenhang zugrunde liegt. Während ein Wirkmechanismus noch nicht bekannt ist, könnte der Schlaf bei der Suche nach einem solchen eine Schlüsselrolle spielen. Da ein gestörter Schlaf nachweislich zu einer erhöhten Konzentration der Biomarker der Alzheimer-Demenz (Amyloid und Tau und deren Ablagerungen) führt, ist eine Störung durch externe Faktoren als Auslöser oder Verstärker denkbar. In epidemiologischen Studien kann der Schlaf nur sehr ungenau gemessen werden und ist anfällig für Confounder, die im Rahmen von experimentellen Studien besser kontrolliert werden können.

Seit mehr als 60 Jahren wird die Aktigraphie eingesetzt, um den Schlaf-Wach-Rhythmus objektiv zu erfassen. Zunehmend werden verbesserte moderne Geräte angewendet, um schlafmedizinische Erkrankungen im klinischen Rahmen zu diagnostizieren. Aktigraphen sind zwar weniger genau als die Polysomnographie, aber ihr größter Vorteil liegt in der kostengünstigen Sammlung objektiver Daten über längere Zeiträume unter Alltagsbedingungen. Da die Kosten der Handgelenksaktigraphie derzeit nicht erstattet werden, hat dieses Verfahren bisher keine breite Akzeptanz erlangt. In der vorliegenden Arbeit wird ein Überblick über die wesentlichen klinischen Anwendungen der Aktigraphie sowie über die Empfehlungen der Fachgesellschaften gegeben.

Background: GHB (gammahydroxybutyrate) and its precursors are popular recreational drugs due to their sedative, anxiolytic and sexually stimulating effects. Their use has been steadily increasing in recent years. The detoxification process is complex and prone to high rates of complications while little is known about the pathophysiology. This study aims to elucidate the characteristics of GHB-addicted patients and to evaluate the risks and complications of GHB withdrawal treatment. Methods: This observational study describes prospectively the socioeconomic status, clinical history and course of inpatient detoxification treatment of a group of 39 patients suffering from GHB substance use disorder. Detoxification treatment took place in a highly specialized psychiatric inpatient unit for substance use disorders. Results: GHB patients were characterised by being young, well-educated and by living alone. More than 50% of the patients had no regular income. The patients were male and female in equal numbers. Detoxification treatment was complicated, with high rates of delirium (30.8%) and high need for intensive care (20.5%). Conclusions: In our sample, GHB users were young, well-educated people and male and female in equal number. Detoxification proved to be dangerous for GHB-addicted patients. The presence of delirium and the need for transfer to an intensive care unit during detoxification treatment was extraordinarily high, even with appropriate clinical treatment. The reasons for this remain unknown. Therefore an intensive care unit should be available for GHB detoxification treatment. Further studies are needed to evaluate the options for prophylactic treatment of delirium during detoxification.

In the present study, the benefits of a short-term sleep coaching intervention embedded in the workplace health promotion (WHP) program of the German Armed Forces was investigated. Well-being, self-care awareness, self-care behavior and irritation of participants attending the sleep coaching (n= 24) and participants attending other courses offered in the WHP program (n= 24) were assessed before and after the intervention. Participants attending the sleep coaching had a significant increase in well-being, self-care behavior and self-care awareness and a significant decrease in their irritation score after attending the sleep coaching. In the control group, no significant change in the dependent variables was observed; furthermore, participants’ evaluation of the sleep coaching was positive on both affective reactions and utility judgements. Results show that the sleep coaching intervention evaluated is practically feasible and able to improve important indicators of employees’ well-being and self-care in WHP programs.

Zusammenfassung.Chronische Alpträume verursachen im Rahmen der kriegsbedingten Posttraumatischen Belastungsstörung (PTBS) bei Soldaten einen hohen Leidensdruck. Ihre Behandlung wurde bisher bei traumatisierten, deutschen Soldaten nicht untersucht. Die vorliegende Pilotstudie untersucht die Wirksamkeit einer adaptierten, standardisierten Imagery Rehearsal Therapy (IRT) unter stationären Bedingungen im komplementären Setting. 14 kriegstraumatisierte Soldaten mit Alptraumstörung und chronischer PTBS wurden im Rahmen von 10 Einzelsitzungen behandelt. Die Soldaten wurden vor der Intervention (t1) sowie 2 Wochen (t2) und 3 Monate (t3) danach hinsichtlich verschiedener Parameter der Schlaf- und Alptraumsymptomatik, der Traumasymptomatik, der Depression und der allgemeinen psychischen Belastung untersucht. Der Fokus der imaginativen Modifikation lag auf der gezielten Entwicklung von „compassionate imagery“. Die Alptraumhäufigkeit reduzierte sich kurz- und langfristig signifikant (t1–t2, Effektstärke d: 1.8; t1–t3, Effektstärke d: 1.4). Die Schlafqualität und deren Auswirkungen auf verschiedene Lebensbereiche, die Depressionssymptomatik, die allgemeine psychische Beeinträchtigung und die Traumasymptomatik zeigten signifikante Effekte. Die adaptierte IRT mit spezifischer imaginativer Modifikation von traumabedingten Alpträumen kann eine wertvolle Erweiterung des komplementären Versorgungsangebotes für chronisch kriegstraumatisierte Soldaten mit PTBS darstellen.

The Diagnostic and Statistical Manual of Mental Disorders, Fourth Editionborderline personality disorder (BPD) seems to constitute a very heterogeneous category. Therefore, pharmacological therapy is symptom-oriented or targets comorbid conditions. A high comorbidity exists between BPD and posttraumatic stress disorder (PTSD). In a double-blind, randomized, placebo-controlled crossover study, we sought to determine whether the antinoradrenergic agent clonidine was effective in reducing hyperarousal and measures of BPD-specific and general psychopathology in a sample of 18 patients with BPD, with or without comorbid PTSD, and with a prominent hyperarousal syndrome. Hyperarousal as measured by the Clinician Administered PTSD scale improved significantly compared with placebo (P= 0.003) irrespective of PTSD comorbidity. Improvements in general and BPD-typical psychopathology were mainly seen in the PTSD-positive subgroup, whereas the subjective sleep latency (P= 0.005) and the restorative qualities of the sleep (P= 0.014) improved in the whole sample. Improvements, despite the small sample size of this pilot study, lead us to conclude that clonidine might be a useful adjunct to pharmacotherapy in patients with BPD who have marked hyperarousal and/or sleep problems and, in particular, in patients with BPD who have a PTSD comorbidity.

To investigate differences between visual sleep scoring according to the classification developed by Rechtschaffen and Kales (R&K, 1968) and scoring based on the new guidelines of the American Academy of Sleep Medicine (AASM, 2007).

Recent data suggest that inhibitory pathways may be involved in the pathophysiology of depression and in the mode of action of some antidepressant interventions. The aim of the present study was to test whether vagus nerve stimulation (VNS) can affect motor cortex excitability. Measures of motor cortical excitability were probed by using single-pulse and paired-pulse transcranial magnetic stimulation at baseline, after 10 weeks of left VNS, and additionally, in an on-off paradigm in 10 patients with treatment-resistant unipolar depression. Ten weeks of VNS was associated with a selective and pronounced increase in intracortical inhibition, whereas no changes occurred in the on-off paradigm. These results suggest that VNS is capable of changing motor cortical excitability in patients with depression.

Zusammenfassung: Auf Anregung der Deutschen Narkolepsie Gesellschaft (DNG) Berlin/Brandenburg wurde eine Studie durchgeführt, um zu untersuchen, ob mit Hilfe der Psychophonie eine Milderung der Tagesmüdigkeit bei Narkolepsie-Kranken erreicht werden kann. Bei der Psychophonie werden digital aufgezeichnete EEG-Signale mit einem mathematischen Algorithmus in hörbare Tonfolgen umgewandelt. Die so hergestellten Musikkassetten wurden von den Teilnehmern über einen Zeitraum von 46 Tagen mehrfach täglich für mindestens 8 bis 10 min gehört. Es wurden subjektive Einschätzungen von Müdigkeit. Befindlichkeit und Schlafverhalten vor und unter der Anwendung der Psychophonie mittels etablierter Meβinstrumente an insgesamt neun Patienten erhoben. Die Ergebnisse, lieβen keinen eindeutigen Effekt erkennen.

SummarySerum samples from eight endogamous Indian tribal populations of Madhya Pradesh (Dhurwa, Halba, Bhatra, Muria, Maria) and Orissa (Deshia Khond, Binjhal, Kisan) with a total of n=731 unrelated individuals were typed for G1M (1,2,3,17), G3M (5,10,11,13,14,15,16,21,26), and KM (1). In seven of these populations five different GM haplotypes were found:GM*1,17;21,26; GM*1,17;10,11,13,15,16; GM*1,2,17;21,26; GM*1,3;5,10,11,13,14,26; and GM*3;5,10,11,13,14,26. In the Kisan sample the haplotype GM*1,2,17; 21,26 is absent. The intergroup variability in the distribution of these haplotypes is considerable and statistically highly significant. The reasons for that can be attributed to the ethnohistory and to the genetic isolation of these eight endogamous tribal populations. The GM haplotype distribution pattern of all these groups is quite different from that of the non-tribal populations of India, whereas it is in good agreement with that of the so far tested other tribal populations from India. This can be explained by different origin and history of the Indian tribal and non-tribal populations. In the KM system, too, remarkable variability is seen in the distribution of phenotype and allele frequencies among the eight tribal populations under study.

Anhand der AB0-Blutgruppendaten von insgesamt 600954 Individuen aus 112 Populationsstichproben wird die ethnische Variabilität der AB0-Allelenfrequenzen im Iran untersucht. Diese ist beträchtlich und wird unter bevölkerungshistorischen Aspekten diskutiert. Bemerkenswert sind insbesondere die AB0-Allelenfrequenzen der assyrischen, armenischen und zoroastrischen Bevölkerungskomponenten, die ganz erheblich von denen aller anderen bisher untersuchten ethnischen Gruppen abweichen. Aber auch die Yazdis sowie die im Iran lebenden Turkmenen und Araber fallen durch abweichende AB0-Allelenfrequenzen auf. Und schließlich zeigen auch die iranischen Juden deutliche Frequenzunterschiede gegenüber den anderen Bevölke­ rungsgruppen des Iran. Es wird angenommen, daß die spezifischen AB0-Allelenfrequenzen der genannten ethnischen Gruppen sowohl mit ihrer verschiedenen geographischen Herkunft als auch mit ihrer Endogamie in Zusammenhang stehen.

Summary: QUISI has been developed as a one-channel, ambulatory EEG recording device with 3 electrodes placed on the forehead close to Fp1, Fz and Fp2. While Fz serves as ground electrode, the channel Fp1−Fp2 was recorded, filtered and analyzed by Fourier-Transform with the subsequent determination of power spectrum estimates. These variables served as input variables for neural network technique classification, taking 12 input variables, 2 hidden layers and 7 outcome variables for 0=Movement Time, 1=Wake, 2=REM, 3=S1, 4=S2, 5=S3 and 6=S4. Out of 118 primary values, 12 were selected using evolutionary and genetic algorithms. 8 neural networks were established using 8 different patients. Every 30 s sleep EEG epoch (segment) was subsequently classified 8 times. The final decision for each epoch of 30 s was made taking the median of the 8 classifications. Subsequently smoothing rules were applied in analogy to the Rechtschaffen and Kales smoothing rules. The analysis is fully automated. There is no artefact rejection and no removal of any sleep epoch. Once the rule was established, it was validated by forward classification of 38 sleep disturbed patients, none of whom had been used for the establishment of the classification rule. Every patient had been recorded with polysomnography (PSG) as well according to the standards of the German Sleep Research Society and with the QUISI equipment. The results show an acceptable agreement between PSG with visual scoring and QUISI with automatic scoring. While SPT, TST and SOL showed high correlations, REM and SWS were generally overestimated. Subsequently we evaluated the pathologies in QUISI and PSG. Out of the 38 patients, only 1 patient had a normal profile both in QUISI and PSG. Out of the 37 patients with pathologies, 17 showed exactly the same pathologies in the PSG and QUISI profile. In 1 patient PSG and QUISI showed completely different pathologies. In 10 patients QUISI did not find at least one pathology seen in the PSG, in 4 patients QUISI found more pathologies than were seen in the PSG, in 5 patients both applied. In its current version QUISI often over-estimates SWS and REM in certain patients, as known from other automated scoring methods described earlier in the literature. QUISI is considered as the beginning of a development with the aim of obtaining an affordable and easily self-applicable sleep EEG recording device, which can be applied before a therapy control and after adminssion to a sleep laboratory. QUISI however can in no way replace sleep laboratory diagnostics.

An 13 Populationen aus Sikkhim (Nord-Indien) — Lepchas (2), Bhutias (2), Sherpas, Tamangs, Gurungs, Rais, Limboos (Subbas), Pradhans (Newars), Brahmins, Chhetris, Scheduled Castes — wurde die Verteilung von 17 genetischen Polymorphismen des Blutes untersucht: A1A2B0, MNSs, Rhesus (C, c, D, E, e), Kell, Duffy, Kidd, Haptoglobin, Transferrin subtypes, Gc subtypes, Gm (1, 2, 5), Km (1), saure Erythrocytenphosphatase (aP), Phosphoglucomutase (PGM1, 6-Phosphogluconatdehydrogenase (6-PGD), Esterase D (EsD), Adenylatkinase (AK) und Hämoglobin-Varianten. Außerdem konnten an den Lepchas und Bhutias aus Nord-Sikkim noch die Lutheran- und Xg-Blutgruppen sowie die ABH-Sekretion im Speichel bestimmt werden. Phänotyp- und Allelenfrequenzen zeigen eine beträchtliche Intergruppenvariabilität, die unter Berücksichtigung von Geschichte und Heiratsbeziehungen dieser Populationen diskutiert wird. Der mittlere Koeffizient der gene diversity GST beträgt 0.0351; Wright’s FST = 0.0257. Diese Werte unterscheiden sich in gewissem Ausmaß von denen in anderen indischen Populationen ermittelten. Die Analyse der genetischen Abstände ergab ein Cluster-Muster, das zu einem großen Teil die ethnohistorischen Beziehungen zwischen den 13 untersuchten Sikkim-Populationen widerspiegelt.

In den Jahren 1978—79 wurden an insgesamt 13 assamesischen Bevölkerungsgruppen — zwei Muslimgruppen, fünf mongoliden Gruppen und sechs Hindu-Kastenbevölkerungen — Untersuchungen über die Verteilung anthropometrischer, anthroposkopischer und daktyloskopischer Merkmale sowie der AB0-Blutgruppen und PTC-Schmeckfähigkeit durchgeführt. Mit Ausnahme der Fingerbeerenmuster ließen alle untersuchten Merkmale eine ausgesprochene Intergruppenvariabilität erkennen. Für die anthropometrischen, anthroposkopischen und daktyloskopischen Merkmale konnte eine deutliche Differenzierung in mongolide Bevölkerungen einerseits und Kastenbevölkerungen andererseits beobachtet werden, was durch die Ergebnisse entsprechender Abstandsanalysen bestätigt wurde. Im Hinblick auf die Muslims muß zwischen den Marias, die den mongoliden Populationen ähnlicher sind und den Sheikhs, die in ihrer phänotypischen Erscheinung mehr den Hindu-Kastenbevölkerungen ähneln, unterschieden werden. Die Ergebnisse werden diskutiert.

In mehreren Untersuchungen konnte gezeigt werden, daß die Verteilung des Menarchealters europäischer Mädchen einen Nordwest-Südost-Gradienten erkennen läßt. Bei Mädchen aus den südlichen Teilen Europas tritt die Menarche durchschnittlich ein Jahr früher ein als bei solchen aus den nord- bzw. nordwesteuropäischen Ländern. Eveleth & Tanner (1976) sowie Danker-Hopfe (1986a) nehmen an, daß dieser Gradient im wesentlichen genetische Unterschiede widerspiegelt. Klimatische Faktoren sowie Ernährungsunterschiede sind zur Erklärung dieses Verteilungsmusters vermutlich weniger geeignet. Zur Überprüfung dieser Hypothese wurde das Menarchealter türkischer Mädchen analysiert, das per Probitanalyse aus Status quo-Daten von n = 494 Mädchen im Alter von 9,0 bis 16,5 Jahren geschätzt wurde, beträgt 12,90 ± 1,21 Jahre. Dieser Wert zeigt eine gute Übereinstimmung mit den von Neyzi et al. (1975) für Mädchen aus Istanbul mitgeteilten Daten. Andererseits ist das Menarchealter türkischer Mädchen in Bremen deutlich niedriger als das deutscher Mädchen im selben Bundesland. Zusammenfassend unterstützen die Ergebnisse der vorliegenden Untersuchung die Hypothese, daß die Ursachen des beobachteten Nordwest-Südost-Gradienten im Menarchealter vermutlich vorwiegend genetischer Natur sind.

22. Tagung der Gesellschaft für Anthropologie und Humangenetik: Unter der Leitung von H. Preuschoft fand vom 10.-12.10.1991 in Bochum die 22. Tagung der Gesellschaft für Anthropologie und Humangenetik statt. Die 21. Tagung der Gesellschaft, die im September 1989 in Bremen stattfinden sollte, hatte kurzfristig abgesagt werden müssen. Der erste Tag der Bochumer Tagung war der Thematik „Anthropologie im Spannungsfeld von Politik, Ideologien und anderen Interessen - Rückschau und Gegenwart" gewidmet.

7th Congress of the European Anthropological Association, 26.-30. August 1990, Wroclaw, Polen: Am 27. August wurde in der Aula Leopoldina der Universität von Wroclaw der 7th Congress of the European Anthropological Association in einer feierlichen Sitzung eröffnet. Der Einladung des Tagungspräsidenten T. Bielicki zu dieser Tagung, die vom 26.-30. August bei strahlendem Sommerwetter in Wroclaw stattfand, waren ca. 250 Kolleginnen und Kollegen aus mehr als 20 - nicht nur europäischen - Ländern gefolgt; 50 Jahre Institut für Anthropologie der Josef Attila-Universität in Szeged (Ungarn): Aus Anlaß des 50jährigen Bestehens des Instituts für Anthropologie der Jósef Attila-Universität in Szeged (Ungarn) fand vom 1.-3.10.1990 ein von Gy. Farkas organisiertes wissenschaftliches Symposium statt, an dem nicht nur zahlreiche Fachkolleginnen und -kollegen aus Ungarn selbst teilnahmen, sondern auch solche aus anderen europäischen Ländern.

Drei Bevölkerungsgruppen aus Assam — Hindus, Muslims, Mongolide — wurden hinsichtlich verschiedener soziobiologischer Parameter untersucht, nämlich Größe und Zusammensetzung der Familie, Geschlechtsproportion, Menarchealter, Heiratsalter, geographische Entfernung der Geburtsorte der Ehepartner, Konzeptionshäufigkeit, Häufigkeit von Aborten und Totgeburten und Fertilität. Die Ergebnisse werden vergleichend analysiert, und es wird versucht, die möglichen Ursachen für die zwischen diesen drei Bevölkerungsgruppen vorliegenden Unterschiede herauszuarbeiten.

Ergebnisse verschiedener Studien zeigten, daß die Nahrungsaufnahme und die Nahrungsauswahl innerhalb des Menstruationszyklus variiert. Die präsentierten Ergebnisse basieren auf Ernährungstagebüchern von 27 Frauen, die alle einen regelmäßigen Menstruationszyklus hatten und keine oralen Kontrazeptiva nahmen. Über einen Menstruationszyklus hinweg zeichneten die Frauen ihre ad libitum-Nahrungsaufnahme auf, wobei sie Menge und Art der Lebensmittel genau klassifizierten. Mit Hilfe des Bundeslebensmittelschlüssels des Bundesgesundheitsamtes wurden die Tagebücher ausgewertet und in die tägliche Aufnahme von Kohlehydraten, Protein, Fett und Energie auch im Hinblick auf die verschiedenen Zyklusphasen, die durch die Aufzeichnung der Basaltemperatur kontrolliert wurden, aufgeschlüsselt. Die Ergebnisse deuten darauf hin, daß die Menge der aufgenommenen Energie in der Lutealphase höher ist als in der Follikularphase. Die Kohlenhydrataufnahme ist in der periovulatorischen Phase am niedrigsten und erreicht ihr Minimum, ebenso wie die Fett- und die Energieaufnahme, zwei Tage nach der Ovulation. Außerdem konnte gezeigt werden, daß sich die subjektive Empfindung von "süß" in den Zyklusphasen verändert. Die Ergebnisse suggerieren, daß sowohl die Nahrungsaufnahme als auch die Nahrungsauswahl durch neurochemische, hormonelle, physiologische und psychologische Faktoren beeinflußt werden.

An 14 Bevölkerungsgruppen aus Sikkim (Indien) — Lepchas (2), Bhutias (2), Sherpas, Tamangs, Gurungs, Mangars, Rais, Limboos/Subbas, Pradhans (Newars), Brahmans, Chhetris, Scheduled Castes — wurden Untersuchungen über die Intra- und Intergruppenvariabilität der Farbenblindheit, Ohrläppchenverwachsung, Mittelphalangelabehaarung sowie einiger anthroposkopischer Merkmale (Zungenfalten, Händefalten, Verschränken der Arme und Beine, Händigkeit) durchgeführt. Einige dieser Variablen lassen eine beträchtliche Verteilungsheterogenität erkennen, die unter Berücksichtigung historischer Gegebenheiten sowie der Heiratsbeziehungen dieser Populationen diskutiert werden. Da die meisten von ihnen in hohem Maße endogam sind, kann man vermuten, daß diese Heterogenität Ergebnis lokal wirksamer Faktoren wie Drift und/oder Foundereffekt ist, welche wegen des praktisch fehlenden Genflusses zwischen diesen Populationen sich erhalten konnten. Außerdem werden die Sikkimdaten kurz mit denen für andere indische bzw. asiatische Populationen mitgeteilten verglichen.

ABSTRACTIncreasing evidence supports a close link between REM sleep and the consolidation of emotionally toned memories such as traumatic experiences. In order to investigate the role of sleep for the development of symptoms related to traumatic experiences, beyond experimental models in the laboratory, sleep of acutely traumatised individuals may be examined on the first night after trauma. This might allow us to identify EEG variables predicting the development of posttraumatic stress disorder (PTSD) symptoms, and guide the way to novel sleep interventions to prevent PTSD. Based on our experience, patients’ acceptance of polysomnography in the first hours after treatment in an emergency room poses obstacles to such a strategy. Wearable, self-applicable sleep recorders might be an option for the investigation of sleep in the aftermath of trauma. They would considerably decrease the perceived burden for patients and thus increase the likelihood of successful patient recruitment. As one potential sleep intervention, sleep deprivation directly after trauma has been suggested to reduce the consolidation of traumatic memories and hence act as a secondary preventive measure. However, experimental data from sleep deprivation studies in healthy volunteers with the trauma film paradigm have been inconclusive regarding the beneficial or detrimental effects of sleep on traumatic memory processing. Depending on further insights into the role of sleep in traumatic memory consolidation through observational and experimental studies, several options for therapeutic sleep interventions are conceivable: besides behavioural sleep deprivation, selective REM sleep suppression or enhancement by a pharmacological intervention into the serotonergic, noradrenergic or cholinergic systems might provide novel therapeutic options. While REM-modulating drugs have been used with some success for the prevention of PTSD after trauma, they have never been tried before the first night of sleep. In conclusion, more experimental and observational research is needed before sleep interventions are performed in actual trauma victims.

Electroconvulsive therapy (ECT) and repetitive transcranial magnetic stimulation (rTMS) of the dorsolateral prefrontal cortex are brain stimulation techniques that are used as therapeutic interventions in major depression. However, the exact therapeutic mode of action needs further clarification. In this case report, we describe the impact of these stimulation techniques on motor cortical excitability, as revealed by transcranial magnetic stimulation-elicited motor-evoked potentials in 2 patients who received consecutively both rTMS and ECT. Both patients showed a decrease in motor cortical excitability after response to antidepressant brain stimulation, whereas parameters of motor cortical excitability remained unchanged after the first non-successful intervention. These results suggest that both ECT and rTMS may have an impact on parameters of motor cortical neuronal excitability. Furthermore, measurement of motor cortical excitability may be a useful tool for investigating and monitoring inhibitory brain effects of different antidepressant stimulation techniques.

Abstract: The deposition of βA4-amyloid in senile plaques in the brain and small cerebral vessels is one of the pathological hallmarks of Alzheimer’s disease (AD). Serine protease inhibitors (serpins) such as α1-antitrypsin and α1-antichymotrypsin have been found to be associated with β-amyloid deposits; interest in their role in the pathogenesis of AD has therefore recently increased. We have analyzed α1-antitrypsin phenotypes in a sample of 29 Polish patients with probable Alzheimer’s disease. We have found an increased frequency of the PI*M3 allele (0.1897) in patients in comparison with the general population control (0.0563).

Tagungspräsident war Prof. Dr. G. Röhrborn aus Düsseldorf. In schon traditioneller Weise waren die Vormittage jeweils Symposien vorbehalten, deren Beiträge überwiegend von eingeladenen ausländischen Referenten bestritten wurden. 7th International Congress of Auxology 26. -30. Juni 1994, Szombathely, Ungarn: Vom 26. -30. Juni 1994 fand in Szombathely, Ungarn, bei höchst sommerlichen Temperaturen der 7th International Congress of Auxology unter Leitung des Kongreß-Präsidenten Prof. Dr. O. G. Eiben, Budapest, statt. Der Kongreß wurde in einer feierlichen Sitzung mit Grußadressen, musikalischer Umrahmung und einem wissenschaftlichen Vortrag eröffnet. Mehr als 270 an wachstumsbiologischen Problemen und klinischen Aspekten des Wachstums interessierte Fachkollegen/innen aus allen Teilen der Welt hatten sich eingefunden, um Forschungsergebnisse vorzustellen, zu diskutieren und Erfahrungen auszutauschen.

Aims. Sleep disturbances are common in addiction and withdrawal. This study examined the course of sleep quality in a population of alcohol dependent patients during qualified detoxification treatment in a psychiatric hospital. Methods. The Pittsburgh Sleep Quality Index (PSQI) was administered to 77 electively admitted alcohol dependent patients hospitalized for qualified detoxification treatment. Sleep quality was measured at admission and at discharge. Results. The prevalence of bad sleep as measured by a PSQI-score > 5 was 70.1% at admission. During detoxification, male and female patients were equally affected by sleep disturbances and improvement of sleep was not significantly different between males and females. The PSQI score at admission predicted the change of the PSQI score during qualified detoxification treatment. After inpatient detoxification, sleep disturbances persisted in 59.7% of the patients. Conclusions. Contrary to our expectations, the average patient’s sleep quality improved in our study after two weeks of detoxification treatment. Sleep disturbances nevertheless persisted in almost two-thirds of the patients. In the view of that finding, patients may require individual evaluation of sleep quality and insomnia-specific treatment in the course of detoxification therapy.

Aims. This prospective study aims to identify patient characteristics as predictors for treatment outcome during inpatient detoxification treatment for drug and alcohol dependent patients. Methods. A mixed gender sample of 832 consecutively admitted drug and alcohol dependent patients were interviewed by an experienced physician. The impact of a variety of factors concerning social environment, therapy motivation, impulsivity related variables, medical history, and addiction severity on treatment outcome was examined. Results. 525 (63.1%) of the patients completed detoxification treatment whereas 307 (36.9%) dropped out prematurely. Being female, living in a partnership, having children, being employed, and having good education were predictive for a positive outcome. Family, health, the fear of losing the job, prosecution, and emergency admission were significant motivational predictors for treatment outcome. Being younger, history of imprisonment, and the number of previous drop-outs were predictive for a negative outcome. Conclusions. Variables concerning social environment and the number of previous drop-outs have been identified as best predictors for treatment outcome. Socially stable patients benefit from the current treatment setting and treatment shall be adapted for patients with negative predictors. Treatment may consequently be tailored with respect to intervention type, duration, and intensity to improve the outcome for those patients that fulfil criteria with negative impact on treatment retention.

We previously analysed clinical and immunological parameters under Trichuris suisova (TSO) therapy in four patients with secondary progressive multiple sclerosis. The serum Brain-derived neurotrophic factor (BDNF) levels of these four patients were assessed before, during and after therapy with TSO and showed significant decrease of BDNF during TSO therapy (p< 0.05).

Februar 1990 fand in Lund/Schweden das Second Seminar of Nordic Physical Anthropology zum Thema Populations of the Nordic countries - Human population biology from the present to the Mesolithic statt. Es handelte sich dabei um eine Nachfolgeveranstaltung des First Seminar of Nordic Physical Anthropology, das im Jahr 1984 ebenfalls in Lund abgehalten wurde.

VIIIth Congress of the European Anthropological Association 7.-10. September 1992, Madrid, Spain: Vom 7. bis 10. September fand in Madrid, Spanien, unter der Präsidentschaft und wissenschaftlichen Leitung von Maria Dolores Garralda der VIIIth Congress der European Anthropological Association statt. Das wissenschaftliche Programm, das die verschiedensten Arbeitsgebiete der Anthropologie (Paläoanthrpologie, Neue Perspektiven der Osteologie, Wachstums- und Reifungsprozesse, Biodemographie, Historische Populationen, Forensische Anthropologie, Humangenetik und Kinanthropologie) berücksichtigte, wurde in der Universidad Complutense de Madrid, Facultad de Ciencias Biológicas abgehalten. Es war in Plenarveranstaltungen mit eingeladenen Vorträgen, Postersitzungen und Parallelveranstaltungen mit insgesamt etwa 80 Kurzvorträgen gegliedert.

BackgroundSleep disturbance is a common characteristic of patients with post-traumatic stress disorder (PTSD). Besides the clinical descriptions of nightmares and insomnia, periodic limb movements (PLMs) are reported to co-occur in PTSD. Although the causal relationship between sleep disturbance and PTSD is not fully understood, sleep disturbance is an independent risk factor for the development and reactivation of PTSD. In contrast, the link between PTSD and REM sleep behaviour disorder (RBD) is less clear.MethodA case report is presented to illustrate differential diagnosis and time course of sleep disturbance in the context of PTSD.ResultA 63-year-old man who had been successfully treated for PTSD but who suffered the re-occurrence of disturbed sleep due to RBD and PLM, which was misdiagnosed as reactivation of PTSD.ConclusionsRBD can mimic PTSD-related nightmares. Accurate diagnosis of sleep disturbance in PTSD is relevant for treatment and prognostic evaluation.
