Background Peritonitis is a major complication of peritoneal dialysis (PD) and is associated with significant morbidity and mortality. Early empirical antibiotic therapy is recommended, with the choice of agents guided by local resistance patterns. As routine use of specific antimicrobial agents can drive resistance, regular assessment of causative organisms and their susceptibility to empirical therapy is essential.Methods We conducted a retrospective review of all PD peritonitis cases and positive PD fluid cultures obtained over a 5-year period in Western Australia following the introduction of a statewide protocol for the initial management of PD peritonitis with intraperitoneal vancomycin and gentamicin.Results The incidence of PD peritonitis decreased from 1 in 16 patient months (0.75/year at risk) to 1 in 29 patient months (0.41/year at risk) over the 5 years. There were 1,319 culture-positive samples and 1,069 unique isolates identified. Gram-positive bacteria accounted for 69.9% of positive cultures, with vancomycin resistance averaging 2% over the study period. Gram-negative bacteria accounted for 25.4% of positive cultures, with gentamicin resistance identified in an average of 8% of organisms. No increase in antimicrobial resistance to vancomycin or gentamicin occurred over the 5 years and there was no change in the proportion of gram-positive (69.9%), gram-negative (25.4%) or fungal (4.4%) organisms causing PD peritonitis.Conclusions Over time, the peritonitis rates have dramatically improved although the profile of causative organisms remains similar. Empirical treatment of PD peritonitis with intraperitoneal vancomycin and gentamicin remains efficacious, with high levels of susceptibility and no evidence that the introduction of this statewide empirical PD peritonitis treatment protocol is driving resistance to these agents.

This study examined the effect of subinhibitory Melaleuca alternifolia (tea tree) essential oil on the development of antibiotic resistance in Staphylococcus aureus and Escherichia coli. Frequencies of single-step antibiotic-resistant mutants were determined by inoculating bacteria cultured with or without subinhibitory tea tree oil onto agar containing 2 to 8 times the MIC of each antibiotic and with or without tea tree oil. Whereas most differences in resistance frequencies were relatively minor, the combination of kanamycin and tea tree oil yielded approximately 10-fold fewer resistant E. coli mutants than kanamycin alone. The development of multistep antibiotic resistance in the presence of tea tree oil or terpinen-4-ol was examined by culturing S. aureus and E. coli isolates daily with antibiotic alone, antibiotic with tea tree oil, and antibiotic with terpinen-4-ol for 6 days. Median MICs for each antibiotic alone increased 4- to 16-fold by day 6. Subinhibitory tea tree oil or terpinen-4-ol did not greatly alter results, with day 6 median MICs being either the same as or one concentration different from those for antibiotic alone. For tea tree oil and terpinen-4-ol alone, day 6 median MICs had increased 4-fold for S. aureus (n = 18) and 2-fold for E. coli (n = 18) from baseline values. Lastly, few significant changes in antimicrobial susceptibility were seen for S. aureus and S. epidermidis isolates that had been serially subcultured 14 to 22 times with subinhibitory terpinen-4-ol. Overall, these data indicate that tea tree oil and terpinen-4-ol have little impact on the development of antimicrobial resistance and susceptibility.

Accurate methods for diagnosing active Helicobacter pyloriinfection in children have been limited to invasive or time-consuming techniques. Recently, fecal antigen testing has been used successfully for the diagnosis of H pyloriinfection in the pediatric population. We compared 2 monoclonal fecal antigen diagnostic methods in a population of children with a suspected high prevalence of H pyloriinfection. We also assessed the diagnostic performance of H pyloriimmunoglobulin G serology. In a cross-sectional study of African refugee children (<16 years) we compared an immunochromatographic technique (ICT) and serology with a monoclonal fecal antigen enzyme immunoassay (MFAT) method for the detection of active H pyloriinfection. Following the manufacturer's instructions, an optical density of =0.190 was used as a cutoff for MFAT. Sensitivity, specificity, and positive and negative predictive values were calculated. Of the 193 eligible children enrolled, active H pyloriinfection was detected in 149 of 182 (81.9%) in whom MFAT was performed. The prevalence of active infection increased with age; children with active infection were significantly older, and there were no sex differences. ICT and serology underperformed in comparison with MFAT (ICT sensitivity 74.6%, specificity 63.6%, positive predictive value 89.8%, negative predictive value 36.8%; and serology sensitivity 57.9%, specificity 77.4%, positive predictive value 92.0%, negative predictive value 29.9%). Monoclonal enzyme immunoassay fecal antigen testing is a practical and feasible alternative to traditional invasive diagnostic methods in high-prevalence pediatric populations. Neither immunochromatography nor serology is useful for the diagnosis of active H pyloriinfection in these children.

The murine olfactory epithelium (OE) generates olfactory receptor neurons (ORNs) throughout development and into adulthood, but only a few of the factors regulating olfactory neuro‐ and glio‐genesis have been delineated. Notch receptors maintain CNS neuronal progenitors and drive glial differentiation, and the Notch effectors Hes 1 and 5 are expressed in the OE, but the Notch receptors that stimulate Hes gene activation in defined lineages during OE development have not been determined. Here, we first use RT‐PCR to reveal which Notch receptors and ligands are expressed in the developing and adult OE. This is followed by immunofluorescent detection, combined with lineage‐specific markers to define the stage‐specific developmental expression of different Notch family members. We show that throughout development, Notch 1 and 3 are expressed in cells retained within the lamina propria, where Notch 3 is expressed in olfactory ensheathing cells (OECs). In contrast, Notch 2 is expressed in apical embryonic and early postnatal OE neuronal progenitors. In postnatal and adult OE, Notch 1 is expressed predominantly in Bowman's glands, and Notch 2 in sustentacular cells. Notch 2 and Notch 1/3 may, therefore, have different roles in the commitment and differentiation of neuronal and glial lineages of the OE during development, and the maintenance of non‐neuronal phenotypes postnatally. Due to the lack of antiseptics with sporicidal properties, the activity of tea tree oil (TTO)-containing products was investigated according to the EN 14347 European suspension method. The activity of different concentrations of TTO in 0.001% Tween 80, as well as an alcoholic hygienic skin wash (AHSW) and an alcoholic handrub (AHR), was compared to the sporicidal disinfectants sodium hypochlorite (SH) and hydrogen peroxide (HP), and the antiseptic povidone iodine (PVI). These formulations were assessed after 30, 60 and 120 minutes against two strains of spore-forming bacteria: Bacillus cereus and Bacillus subtilis. The TTO formulated products and PVI did not achieve 4 log10 reduction in spore counts, thus did not meet the European standard requirements. Nevertheless, 5%, 10% and 15% TTO in 0.001% Tween 80 significantly reduced the concentration of bacterial spores when compared with the water control, and were generally more effective than PVI. At a concentration of 15%, TTO was more active than 0.05% SH against spores of both strains after 30 minutes' contact time, while 5% TTO in Tween 80 was generally more active than the AHSW that also contained 5% TTO. These findings emphasise the fact that it is necessary to assess final formulations rather than the active component by itself as excipients may interfere with the activity. The alcohol-containing AHSW and AHR did not demonstrate any sporicidal properties, confirming previous studies showing that alcohol is not sporicidal and suggesting no synergy between TTO and alcohol against spores. Hence these should not be used for hand hygiene or surface decontamination if spores are involved. Further studies should be carried out to investigate the potential sporicidal activity of TTO directly on human skin in vivo with volunteers and/or by using an 'ex vivo' test that uses freshly excised human skin and which would allow testing of any biocide as well as any spore-forming bacterial strain, including pathogens.

ABSTRACTThe essential oil of Melaleuca alternifolia(tea tree) has broad-spectrum antimicrobial activity. The mechanisms of action of tea tree oil and three of its components, 1,8-cineole, terpinen-4-ol, and α-terpineol, against Staphylococcus aureusATCC 9144 were investigated. Treatment with these agents at their MICs and two times their MICs, particularly treatment with terpinen-4-ol and α-terpineol, reduced the viability of S. aureus. None of the agents caused lysis, as determined by measurement of the optical density at 620 nm, although cells became disproportionately sensitive to subsequent autolysis. Loss of 260-nm-absorbing material occurred after treatment with concentrations equivalent to the MIC, particularly after treatment with 1,8-cineole and α-terpineol. S. aureusorganisms treated with tea tree oil or its components at the MIC or two times the MIC showed a significant loss of tolerance to NaCl. When the agents were tested at one-half the MIC, only 1,8-cineole significantly reduced the tolerance of S. aureusto NaCl. Electron microscopy of terpinen-4-ol-treated cells showed the formation of mesosomes and the loss of cytoplasmic contents. The predisposition to lysis, the loss of 260-nm-absorbing material, the loss of tolerance to NaCl, and the altered morphology seen by electron microscopy all suggest that tea tree oil and its components compromise the cytoplasmic membrane.

In this paper, we show that there is a two‐step process of DNA fragmentation in apoptosis; DNA is first cleaved to large fragments of 50–300 kb that are subsequently cleaved to smaller oligonucleosomes in some, but not all cells. Significantly, only the first stage is considered essential for cell death since some cells, for example human MCF7 breast carcinoma cells and human NT2 neuronal cells, do not show this behavior but still display normal nuclear morphological apoptotic changes. In cells that usually produce small fragments blocking the second (internucleosomal) stage of DNA fragmentation prevents neither nuclear condensation nor apoptosis. We are beginning to understand why the extent of DNA fragmentation during apoptosis varies enormously and why it appears to be a function of the cell type not the inducer. Presumably, this reflects the content of not only endonuclease activit(ies) but also on the ability of the cells to activate caspases, particularly caspase‐3, and other proteases that may be involved in endonuclease activation. Since NT2 cells activate caspase‐3, but do not correctly process DFF45, bother factors must also impinge on the inevitability of that process.

In order to establish what role members of the activating protein-1 (AP-1) gene families, i.e., c-fos,c-jun, junB,andjunD,play in thymic apoptosis, we have analyzed changes in their expression in response to three different agents: a glucocorticoid analog dexamethasone, an inhibitor of topoisomerase II teniposide VM26, and gamma radiation. All three agents induced thymic death at a similar rate and with the same morphological and biochemical features. There was a rapid and transient increase in the steady-state mRNA level ofjunBand c-fosgenes in all treatments, including control cultures, reminiscent rather of cellular stress response to the environmental changes than to the apoptotic stimuli. On the other hand, treatments with the DNA-damaging agents, VM26 and gamma radiation, resulted in superinduction of the c-junmRNA and in the activation of the stress response signaling pathway of c-Jun N-terminal kinase. Gene transcription ceased completely in cells with fragmented DNA and the down-regulation of genes such asjunDandtubulinwas reflective of the thymocytes’ commitment to apoptosis. The DNA-binding activities of the serum response factors, cyclic AMP response element binding proteins, and AP-1 factors, indicative of their transcriptional competence, were compromised shortly after induction of apoptosis regardless of the agent employed, consistent with previously reported enhancement in cellular proteolysis which is an essential component of the apoptotic cell death.

In dexamethasone-treated thymocyte cultures an increase in nuclear proteolytic activity paralleled chromatin fragmentation and the appearance of small apoptotic cells. The elevation of nuclear proteolytic activity was accompanied by site-specific degradation of nuclear mitotic apparatus protein and lamin B, two essential components of the nuclear matrix. Nuclear mitotic apparatus protein phosphorylation and cleavage into 200 and 48 kDa fragments occured within 30 minutes of dexamethasone treatment. Cleavage of lamin B, which generated a fragment of 46 kDa consistent with the central rod domain of the protein, was also detected after 30 minutes of exposure to the steroid hormone. The level of lamin B phosphorylation did not change as a result of the dexamethasone treatment and the lamina did not solubilize until the later stages of apoptosis. Initial DNA breaks, detected by the terminal transferasemediated dUTP-biotin nick end labeling assay, occurred throughout the nuclei and solubilization of lamina was not required for this process to commence. The data presented in this paper support a model of apoptotic nuclear destruction brought about by the site-specific proteolysis of key structural proteins. Both the nuclear mitotic apparatus protein and lamin B were specifically targeted by protease(s) at early stages of the cell death pathway, which possibly initiate the cascade of degradative events in apoptosis.

Objectives: Thirty isolates of Pseudomonas aeruginosa, 15 isolates of Pseudomonas putida and 11 isolates of Pseudomonas fluorescens were tested for susceptibility to tea tree oil (TTO), the essential oil of Melaleuca alternifolia, and the components terpinen-4-ol, α-terpineol, cineole, γ-terpinene and ρ-cymene.Methods: MICs were determined by broth microdilution in Mueller–Hinton medium supplemented with 0.002% (v/v) Tween 80.Results: The MIC90 of TTO for all isolates tested was 4% (v/v) or less. Susceptibility to components tested varied between species.Conclusions: Pseudomonas spp. are susceptible to TTO and some of its components although they are less susceptible than many other bacteria tested previously.

Complementary and alternative medicines occupy a privileged marketing position in many countries, including Australia, being able to carry low-level health claims while being relatively unfettered by regulatory requirements. Consequently, many enjoy widespread use for therapeutic purposes in the absence of definitive efficacy or safety data. Occasionally, the therapeutic properties of complementary and alternative medicines are scrutinised more closely and more thoroughly than their conventional counterparts, leading to the confirmation or discrediting of their properties.

&lt;it>Objectives&lt;/it>: The essential oil of &lt;it>Melaleuca alternifolia&lt;/it> (tea tree oil) and its components have antimicrobial activity against a wide range of Gram-positive and Gram-negative bacteria, fungi and viruses. The mechanism(s) by which &lt;it>Pseudomonas aeruginosa&lt;/it> NCTC 10662 maintains a decreased susceptibility to tea tree oil and components was investigated. &lt;it>Results&lt;/it>: Ethylene diamine tetraacetic acid enhanced the antimicrobial activity of tea tree oil and terpinen-4-ol against stationary phase &lt;it>P. aeruginosa&lt;/it> while polymyxin B nonapeptide enhanced the activity of tea tree oil and γ-terpinene. Pre-treatment with the protonophore carbonyl cyanide &lt;it>m&lt;/it>-chlorophenylhydrazone increased the susceptibility of exponential phase cells to sub-inhibitory concentrations of tea tree oil, terpinen-4-ol and γ-terpinene, indicating that intrinsic tolerance to tea tree oil and components is substantially energy dependent. &lt;it>Conclusions&lt;/it>: Increased tolerance to tea tree oil in &lt;it>P. aeruginosa&lt;/it> is directly related to the barrier and energy functions of the outer membrane, and may involve efflux systems.
