Objective. Analyze health preparedness policies in Chile and identify their strengths and weaknesses. No other studies to date provide an analysis of the country's preparedness policies. Methods. A desk review and semi-structured interviews with experts in emergency preparedness and response were conducted to identify the regulatory framework, key actors, and the strengths and weaknesses of health preparedness policies. Results. The researchers identified 103 standards and interviewed seven preparedness experts. The reviewed standards and interviews show that Chile is in a transitional phase between the old National Civil Protection System and the new National Disaster Prevention and Response System. Only three standards were directly related to health, but the preparedness regulations provide for a multidisciplinary set of actors to address any threat. The experts gave a positive assessment of the Chilean system, although they agreed that certain weaknesses must be corrected. The country's main strength is its disaster response experience, along with its coordination mechanisms. The main shortcomings include risk communication, mitigation, preparedness and assessment, and human resources. Conclusions. Chile has a solid regulatory framework with an all-hazards approach and a set of multisectoral institutions. The new National Disaster Prevention and Response System must build on its strengths to correct the weaknesses that limit its emergency preparedness and response capacity. [ABSTRACT FROM AUTHOR]

To date, the majority of research into a good death has focused on the experience of the person who is dying. Taking the perspective of bereaved individuals, this qualitative study explores which elements of the end of life experience constitute a good death and how these elements influence the process of bereavement adjustment. Following interviews with ten bereaved adults four themes were identified which together define a good death: a lack of physical distress, emotional resolution, 'naming death as death' and death at 'the right time'. The value of open communication prior to death is highlighted. For those working with bereaved individuals these results support an understanding of the impact of the manner of death on bereavement adjustment. For those in palliative care settings, potentially modifiable elements of the end of life experience which may support better bereavement are suggested. [ABSTRACT FROM AUTHOR]

This report shares the challenges and opportunities encountered by a binational project that examined the availability of environmental and public health information for the United States-Mexico border area. The researchers interviewed numerous national and binational agencies on both sides of the border, endeavoring to develop a framework to advance the knowledge of academic and public health professionals in the area of environmental border health. However, the lack of standardized indicators and metrics in both countries validates the emergent need to establish a viable framework for the collection, analysis, and dissemination of environmental information. Objective. To identify 1) the main determinants of persistent Yersinia pestis circulation and the associated threat of plague at Hermelinda Market--a large farmers' market in the city of Trujillo, La Libertad, Peru--and the main actions taken against it, as perceived by local stakeholders; 2) the level of plague risk perception among local actors; and 3) recommended actions to solve the plague threat at the market. Methods. A conceptual framework was developed combining a social determinants approach with a complex systems-thinking framework and a knowledge management perspective. A four-step qualitative protocol was carried out (literature review; stakeholder mapping; 37 semi-structured interviews; and coding/analysis). In the fourth step, the data collected in the semi-structured interviews were coded for eight social determinants of health (SDH) variables and analyzed with ATLAS.ti®, and an emerging category analysis was performed to identify risk perception levels. Results. Based on analysis by SDH variable, the three main determinants of the plague threat at Hermelinda Market were: 1) local (Trujillo City) governance, 2) infrastructure and basic services, and 3) local culture. According to the same analysis, actions most frequently undertaken against plague involved 1) infrastructure and basic services, 2) social vigilance, and 3) communication. The emerging category analysis indicated local risk perception levels were low, with most of the data pointing to "unhygienic" ("naturalized") lifestyles and a general lack of awareness about the disease prior to plague-related health concerns at the market as the cause. Conclusions. The results indicate that the persistent circulation of Yersinia pestis at Hermelinda Market is not simply a technical matter but more of a managerial and cultural problem. As local governance was found to be a main factor in the persistence of this public health threat, future efforts against it should focus on sustainable inter-sectoral planning and education. Actions taken exclusively by the health sector and the improvement of infrastructure and basic services alone will not be enough to reduce the threat of plague at the market. [ABSTRACT FROM AUTHOR]

Objective. To identify perceived barriers to accessing reproductive health care according to the women of Ocotal, Nicaragua; describe their understanding of their reproductive rights; and document their opinions about Nicaragua's total ban on abortion. Methods. From May to June 2014, three focus group discussions were held in Spanish with 17 women from two different neighborhoods (barrios) in the city of Ocotal, Nicaragua. A semi-structured discussion guide with open-ended questions was employed to elucidate local perspectives regarding the focus group discussions themes. Results. Serious obstacles including 1) violence against women, 2) machismo, 3) criticism from others, and 4) lack of communication and education limit women's ability to make their own reproductive health decisions. Women had a pervasive lack of knowledge about reproductive rights and the international human rights documents that define them. In addition, due to religious and cultural ideologies, most women supported the country's total ban on abortion in most circumstances, with the possible exception of rape. Conclusions. Both men and women in Ocotal should be encouraged to participate in community-level programs designed to reduce the impact of the following obstacles to receiving reproductive health care: 1) violence against women and machismo; 2) insufficient, non-standardized sexual education and information about reproductive rights; and 3) poor communication within families and the community at large. Any future public health campaigns to address women's reproductive health needs in Ocotal should implement these types of programs, at the neighborhood level, to reduce stigma surrounding sexual health and activity. When an employee suffers a bereavement, the responses of colleagues and line managers can have long lasting effects. Critical elements in shaping this response may be Human Resource policies and their interpretation; these can influence not only the process of grief for the individual, but can also have a long term impact on the health of organisations themselves. This small interview study with recently bereaved employees indicates that the ways in which they perceive they are treated following the death of a loved one can deeply influence their ongoing relationship within the organisation. Handled well, these difficult episodes can instil a commitment from the employee towards work and aid in the grieving process. Inconsistent or poor support can, conversely, have long term negative consequences. It is imperative that bereaved employees are encouraged and supported by open dialogue with their colleagues and their line manager; the value of open and clear communication within the organisation cannot be overstated. Few studies have been conducted into the impact on parents’ sexual activity following the death of a child. In this study, 285 bereaved parents representing 175 couples completed a questionnaire and ten couples took part in detailed face-to-face interviews. The majority of parents reported reduced frequency of sexual activity, at least initially, although most resumed sexual relations within three months. There were clear gender differences. Women were more likely than men to report reduced sexual activity and loss of sexual desire, primarily linked to feelings of guilt, sadness, depression and exhaustion. A primary reason for recommencing sexual activity was to conceive another child, which could of itself be a cause of complex and mixed feelings. The findings suggest a need for health professionals to pay more attention to these issues when offering structured follow-up to bereaved parents. [ABSTRACT FROM AUTHOR]

Supervisors must develop the skills necessary to form questions that bring out full responses, and the ability to hear total messages. Every aspect of a supervisor's role relies on the ability to listen and communicate effectively.
