INTRODUCTION: Coronavirus disease 2019 (COVID-19) can lead to cardiovascular complications. We aimed to understand the trends in admission for COVID-19 and the incidence of various cardiovascular events. MATERIAL AND METHODS: The 2020 and 2021 National Inpatient Sample (NIS) was studied for cases of COVID-19 between April 2020 and December 2021 in the United States. Linear-by-linear association helped us understand the trends of various events. RESULTS: The number of cases of COVID-19 was highest in January 2021 (261,469 patients). The incidence of acute pulmonary embolism rose from 2.08% in April 2020 to 4.82% in November 2021, while deep vein thrombosis cases rose from 1.74% in April 2020 to 2.63% in December 2021. The incidence of cardiac arrest varied, with a maximum of 3.00% in August 2021. Similarly, acute ischemic stroke cases experienced their highest incidence in January 2021 (0.91%). The incidence of myocarditis was highest in April and May 2020 (0.42% each). Peak takotsubo cases were seen between October and December 2021. The highest overall all-cause mortality among COVID-19 cases was seen in April 2020 (16.74%). CONCLUSIONS: Throughout the 21 months of our analysis, various trends in COVID-19 cases and incidence of cardiac events were noticed. This could relate to the different variants of COVID-19, their direct and indirect impact on coagulation pathways and the myocardial tissues, and the protective roles of the vaccines.

Objectives: Apical ballooning shares features with acute coronary syndromes. Recently, atypical forms have been reported without apical involvement. Usually, the prognostic reports have compared them with ST-segment elevation infarction. Left ventricular transient dyskinesias (LVTD), however, frequently occur without ST-segment elevation and when present, these patients always have open arteries. Our aim was to assess the baseline features, clinical presentation, natural history and compare long-term prognosis in an LVTD-cohort with a first non-ST-segment elevation acute coronary syndrome (NSTEMI) group. Methods: We performed a prospective observational study including consecutive patients in two groups: (i) LVTD group: 62 patients with this syndrome between 2003 and 2007. Inclusion criteria were LV segmental transient motion abnormalities; ECG new alterations and elevated troponin; absence of recent significant head trauma or obstructive coronary artery lesions. (ii) Control group: 169 patients admitted for a first NSTEMI in 2004. Results: Median follow-up was 35 months. Mean age was 65 years. LVTD group included 83.9% females. NSTEMI group was predominantly males. Eleven in-hospital deaths happened in NSTEMI cohort and none in LVTD. Four patients in the LVTD group required readmission and two patients died. In the NSTEMI group, heart failure, unstable angina, myocardial infarction (P<0.001) and death (P=0.11) were more frequent. Cox regression showed that diabetes mellitus, significant onset mitral regurgitation and NSTEMI versus LVTD were found as event-independent predictors. Purpose: Takotsubo cardiomyopathy (TTC) is a transient but severe acute myocardial dysfunction with a wide range of outcomes from favorable to life-threatening. The current risk stratification scores of TTC patients do not include cardiac magnetic resonance (CMR) parameters. To date, it is still unknown whether and how clinical, trans-thoracic echocardiography (TTE), and CMR data can be integrated to improve risk stratification. Methods: EVOLUTION (Exploring the eVolution in prognOstic capabiLity of mUlti-sequence cardiac magneTIc resOnance in patieNts affected by Takotsubo cardiomyopathy) is a multicenter, international registry of TTC patients who will undergo a clinical, TTE, and CMR evaluation. Clinical data including demographics, risk factors, comorbidities, laboratory values, ECG, and results from TTE and CMR analysis will be collected, and each patient will be followed-up for in-hospital and long-term outcomes. Clinical outcome measures during hospitalization will include cardiovascular death, pulmonary edema, arrhythmias, stroke, or transient ischemic attack. Clinical long-term outcome measures will include cardiovascular death, pulmonary edema, heart failure, arrhythmias, sudden cardiac death, and major adverse cardiac and cerebrovascular events defined as a composite endpoint of death from any cause, myocardial infarction, recurrence of TTC, transient ischemic attack, and stroke. We will develop a comprehensive clinical and imaging score that predicts TTC outcomes and test the value of machine learning models, incorporating clinical and imaging parameters to predict prognosis. Conclusions: The main goal of the study is to develop a comprehensive clinical and imaging score, that includes TTE and CMR data, in a large cohort of TTC patients for risk stratification and outcome prediction as a basis for possible changes in patient management.

Background: Cardiac magnetic resonance (CMR) with late gadolinium enhancement (LGE) is a key diagnostic tool in the differential diagnosis between non-ischemic cause of cardiac chest pain. Some patients are not eligible for a gadolinium contrast-enhanced CMR; in this scenario, the diagnosis remains challenging without invasive examination. Our purpose was to derive a machine learning model integrating some non-contrast CMR parameters and demographic factors to identify Takotsubo cardiomyopathy (TTC) in subjects with cardiac chest pain. Material and methods: Three groups of patients were retrospectively studied: TTC, acute myocarditis, and healthy controls. Global and regional left ventricular longitudinal, circumferential, and radial strain (RS) analysis included were assessed. Reservoir, conduit, and booster bi-atrial functions were evaluated by tissue-tracking. Parametric mapping values were also assessed in all the patients. Five different tree-based ensemble learning algorithms were tested concerning their ability in recognizing TTC in a fully cross-validated framework. Results: The CMR-based machine learning (ML) ensemble model, by using the Extremely Randomized Trees algorithm with Elastic Net feature selection, showed a sensitivity of 92% (95% CI 78–100), specificity of 86% (95% CI 80–92) and area under the ROC of 0.94 (95% CI 0.90–0.99) in diagnosing TTC. Among non-contrast CMR parameters, the Shapley additive explanations analysis revealed that left atrial (LA) strain and strain rate were the top imaging markers in identifying TTC patients. Conclusions: Our study demonstrated that using a tree-based ensemble learning algorithm on non-contrast CMR parameters and demographic factors enables the identification of subjects with TTC with good diagnostic accuracy. Mental stress induces endothelial dysfunction through increased activity of the sympathetic nervous system, release of corticotropin-releasing hormone from the hypothalamus, inhibition of nitric oxide (NO) synthesis by cortisol, and increased levels of pro-inflammatory cytokines. Mental-stress-induced increased output of the sympathetic nervous system and concomitant withdrawal of the parasympathetic inflammatory reflex results in systemic inflammation and activation of a neural–hematopoietic–arterial axis. This includes the brainstem and subcortical regions network, bone marrow activation, release of leukocytes into the circulation and their migration to the arterial wall and atherosclerotic plaques. Low-grade, sterile inflammation is involved in all steps of atherogenesis, from coronary plaque formation to destabilisation and rupture. Takotsubo cardiomyopathy (TC) is associated with significant short-term morbidity and mortality. Several risk factors for poor outcomes have been identified; however, the prognostic implications of pre-existing comorbidity in TC are poorly delineated. We sought to assess the association of aggregate pre-existing comorbidity with short-term outcomes in TC. We performed a retrospective observational study of adult subjects diagnosed with TC at two academic tertiary care hospitals between 2005 and 2018. Overall burden of medical comorbidity was estimated using the Charlson comorbidity index (CCI). Multivariable logistic regression was used to test for independent association of CCI with 30-day mortality and severe shock at index presentation. Multivariable poisson regression was performed to assess the association of CCI with duration of hospitalization. Five-hundred and thirty-eight subjects were diagnosed with TC during the study period. The median CCI score of all subjects was 2 (IQR 1-4). Among subjects with physical triggers of TC, the median CCI score was 2 (IQR 1-4) compared to a median CCI score of 1 (IQR 0-1) in subjects with non-physical triggers of TC (P < 0.001). Seventy-six (14%) subjects died within 30 days of index diagnosis and 185 (34%) subjects experienced severe shock. The median duration of hospitalization was 7 days (IQR 3-14 days). In multivariable logistic regression, CCI was not associated with 30-day mortality or severe shock. In multivariable Poisson regression, CCI (IRR 1.17, 95% CI 1.16-1.18, P < 0.001) was associated with duration of hospitalization. Increased burden of pre-existing medical comorbidity was not independently associated with 30-day mortality or severe shock at index presentation, but was associated with increased duration of hospitalization after diagnosis of TC.

AIMS: To investigate predictors of the occurrence of subacute ventricular arrhythmias (VAs), defined as any VAs presenting after 48 h from admission in patients with Takotsubo Syndrome (TTS), and to evaluate the related in-hospital mortality.METHODS: This is a retrospective single-center study enrolling patients admitted between 2012 and 2017 with TTS according to International Takotsubo diagnostic criteria. Data collection included ECG on admission and at 48 h, telemetry monitoring and transthoracic echocardiogram.RESULTS: We enrolled 93 patients; during in-hospital stay (mean 14 ± 16 days) subacute VAs occurred in 25% of patients (VAs group). Life-threatening VAs occurred in 6% of patients (3 sustained ventricular tachycardia, 1 torsade de pointes, 1 ventricular fibrillation) and not life-threatening VAs in 19% (6 non-sustained ventricular tachycardia and 12 premature ventricular contractions > 2000 in 24 h). Mortality was higher in the VAs than in the non-VAs group (P = 0.03), without differences in terms of life-threatening and not life-threatening subacute VAs (P = 0.65) and VAs on admission (P = 0.25). Logistic regression identified the following independent predictors of subacute VAs occurrence: VAs on admission {odds ratio [OR] 22.5 (3.9-131.8), P = 0.001]}, New York Heart Association (NYHA) class III-IV on admission [OR 6.7 (1.3- 34.0), P = 0.021] and QTc at 48 h [OR 1.01 (1.00-1.03), P = 0.046].CONCLUSION: TTS patients with VAs and NYHA class III-IV on admission and higher QTc at 48 h are at increased risk of subacute VAs occurrence, associated with higher in-hospital mortality. Awareness of this potential complication is critical for proper patients management.

Acute exacerbation of psychiatric illness, rapid uptitration or overdose of certain psychotropic agents, and electroconvulsive therapy may trigger TC. A 71-year-old woman with no history of cardiovascular disease or risk factors presented to the emergency department for acute dyspnoea and chest pain after intense emotional stress. At initial evaluation, the blood pressure was 110/70 mmHg and physical examination showed third heart sound and pulmonary bilateral crepitation. The electrocardiogram demonstrated T-wave inversion on the inferior and anterior precordial leads (FIGURE A), accompanied with cardiac serum markers elevation (Troponin I 0,97 ng/dL). A chest x-ray revealed enlarged cardiac silhouette, pulmonary vascular congestion and slight pleural effusion. The patient was admitted to the cardiac care unit. Transthoracic echocardiography revealed akinesia of the left ventricular mid-apical segments with an ejection fraction of 35%, as well as few moveable masses affixed to left ventricular apex recesses compatible with intraventricular thrombi (FIGURE B-C-D-E). Antithrombotic therapy with low molecular weight heparin was started. She underwent a coronary angiography with no evidence of obstructive disease. Forty-eight hours after, a cardiac magnetic resonance was performed, showing apical myocardial inflammation (oedema), no late gadolinium enhancement and the disappearance of the ventricular thrombi. The patient was discharged with transient apical ballooning syndrome diagnosis (Tako-tsubo cardiomyopathy) under acenocumarol and betablocker treatment. Three months later, echocardiography control showed left ventricle contractile and systolic function recovery. Intraventricular thrombus formation in Tako-tsubo cardiomyopathy is an early and infrequent complication that may be detected in 2% to 8% of patients. It usually appears isolated, and in relation with the myocardial contractile dysfunction. Duration of antithrombotic therapy or its prophylaxis is not well established. It is frequently recommended three months of anticoagulation if intraventricular thrombus is detected. La ecocardiografía mostró disfunción sistólica con acinesia de toda la zona apical del v.izquierdo y presencia de varias masas bamboleantes de bordes nítidos (ancladas en los recesos de ápex y tercio distal de septo) compatible con trombos intraventriculares (PANELES B-C-D-E). Se inició tratamiento anticoagulante con HBPM. A las 48 horas se realizó una resonancia magnética cardiaca que mostró edema en ápex y ausencia de realce tardío con desaparición de los trombos. La paciente fue dada de alta con diagnóstico de Sd de takotsubo o discinesia apical transitoria tratado con betabloqueantes y anticoagulantes orales. En el control ecocardiográfico realizado a los 3 meses se observó normalización de la función sistólica y de la contractilidad de los segmentos apicales. La trombosis intraventricular en el sd de Takotsubo es una complicación temprana e infrecuente en relación con la alteración de la contractilidad miocárdica (2-8% de los casos), aunque puede darse también taras recuperar la contractilidad. Stress cardiomyopathy (SCM) typically presents similar symptoms to acute myocardial infarction (AMI). However, these symptoms differ when it comes to a transient and completely reversible myocardial dysfunction, which is frequently precipitated by acute stressful events, occurring in the absence of plaque rupture and coronary thrombosis. Findings suggest that, despite the more favourable medical prognosis of SCM patients, their cardiac condition being transient and resolving completely in few weeks, the psychological impact associated with their condition is more negative 1 year later than in the case of AMI patients whose medical prognosis is less favorable, and this difference is independent of type of trigger event. The purpose of this Chapter is to describe health-related quality of life (HRQL) and emotional burden subsequent to cardiac events in SCM patients.

Introduction Takotsubo cardiomyopathy is a syndrome characterized by transient cardiac ischemia-like symptoms, such as chest pain, increase of myocardial necrosis markers, electrocardiographic changes, and temporary left ventricular apical ballooning without significant coronary artery disease, often triggered by a particularly stressful situation. The association between Takotsubo and hip fracture surgery has been rarely reported in the literature. Presentation of case An 81-year-old woman was hospitalized with a diagnosis of right femoral neck fracture. During the surgical procedure, she displayed acute coronary symptoms a few minutes after the injection of bone cement, in the absence of coronary lesions. Due to the time relationship, bone cement implantation syndrome - not uncommon to observe - was considered in the differential diagnosis. However, the instrumental findings and the transient nature of the abnormalities guided us toward a diagnosis of Takotsubo. The treatment with Levosimendan, Amiodarone, and Metoprolol allowed gradual and satisfactory recovery of the cardiac function within a few days. The follow-up performed two and six months after surgery revealed complete cardiac recovery, and ability to walk at home comparable to the pre-fracture situation. Discussion Takotsubo cardiomyopathy is more common in women during the postmenopausal phase, especially if undergoing stressful physical or emotional stimuli. In this case, the sequence of hip fracture, pain, hospitalization, and surgery could easily be intended as a strong stressful event with high physical/psychological burden. Conclusion Despite the good prognosis associated with early recognition, Takotsubo represents a life-threatening adverse event. Considering its possible pathogenesis, a "gentle care" approach and the optimization of pain control must be pursued in elderly subjects with hip fracture, aiming at reducing the stress of the hospitalization and related procedures.

Due to its elusive natural cause and the lack of clarified pathology, further studies are needed. Thirteen patients presented with an episode of TTC, and referred to Örebro University Hospital (USÖ), were prospectively included and investigated by comparisons made at onset (acute phase) against at follow-up three months later (recovery phase). Including echocardiographic measurements, focused on biventricular systolic long-axis function and conventional diastolic function (DF) variables. Systolic improvement was shown, while most DF data were unchanged, suggesting that TTC is mainly a systolic disease affecting both ventricles. Diagnosis should include multidisciplinary engagement, as TTC associates both with emotional stress and pathological markers of physiological stress. In this thesis, such approach was offered to the aforementioned patients; to see if a common denominator could be found, thus, contributing to better handling. Emotional state was assessed, along with an array of cardiac investigations in addition to echocardiography. Acutely, imbalance in the autonomic cardiac control was shown, as well as a trend toward posttraumatic stress, but specific findings allowing conclusions on differential diagnosis could not be demonstrated. By adding another 15 TTC patients (i.e. 28 in total), through collaboration with observers from USA, a retrospective echocardiographic analysis could be done to further study DF; concluding that TTC associates with impairment of conventional DF variables which tends to parallel the systolic recovery, in contrary to the initial result but in line with other causesof LV dysfunction. Se presentan dos casos de mujeres postmenopáusicas que consultan al servicio de urgencias presentando su primer episodio de dolor torácico desencadenado después de una situación de estrés emocional; sus EKG mostraban cambios compatibles con eventos coronarios agudos y sus niveles de Troponina T fueron francamente positivos. Al ser llevadas a coronariografía, se encontró coronarias sanas y ventriculografía con fracción de eyección disminuida. Los ecocardiogramas iniciales mostraron alteración de la contractilidad segmentaria principalmente de los segmentos apicales con confirmación de la disfunción sistólica; los de control mostraron reversión de estos hallazgos, con mejoría de la fracción de eyección; uno de los casos presentó recurrencia. El diagnóstico final de ambas pacientes fue cardiomiopatía de Tako-tsubo.

The transient apical ballooning or Takotsubo is a recently described syndrome (in 1990). Occurs more often in postmenopausal women after a traumatic pychosocial or physical stressor. These signs and symptoms are suggestive of AMI: chest pain, ST- segment changes and the release of cardiac biomarkers. Coronary angiography reveals no significant coronary artery disease and the Echocardiographic image presents dyskinesis apical transient in the acute phase but left ventricular function improves days later. Prompt recognition of apical ballooning prevents the unnecessary administration of fibrinolytic agents to patients with apical ballooning who have ST-segment elevation. Goals of care are similar to those for patients with AMI: alleviating pain, reducing anxiety and preventing and alert of complications. We report a case with diagnosis and nursing interventions., La discinesia apical transitoria o Takotsubo es un síndrome descrito recientemente (año 1990). Predomina en mujeres posmenopáusicas y con antecedentes de estrés. Su sintomatología es muy similar a la del IAM: dolor precordial, cambios en el segmento ST del ECG y ligeras alteraciones enzimáticas. El cateterismo precoz muestra arterias coronarias normales y la imagen ecocardiográfica presenta discinesia apical transitoria en la fase aguda con normalización de la función ventricular días más tarde. El diagnóstico precoz del Takotsubo evita la administración innecesaria de agentes fibrinolíticos en pacientes con discinesia apical transitoria con elevación del segmento ST del ECG. Los cuidados de enfermería serán parecidos a los administrados a los pacientes con IAM e irán encaminados a aliviar el dolor, reducir la ansiedad y prevenir y alertar en caso de complicaciones. Presentamos un caso clínico junto con los Diagnósticos e Intervenciones de Enfermería que se pueden aplicar en este síndrome.

La miocardiopatía de estrés o síndrome de takotsubo, son variantes de una forma aguda y reversible de compromiso funcional cardiaco caracterizado por un inicio abrupto, usualmente relacionado con un evento emocional o físico estresante, acompañado de dolor torácico o disnea, y signos de lesión miocárdica (síntomas o signos compatibles con falla cardiaca, miocarditis, anormalidades electrocardiográficas y elevación de biomarcadores como troponina y péptidos natriuréticos), que hacen indistinguible la miocardiopatía de estrés del usual síndrome coronario agudo (SCA) en cualquier variedad o síndrome de falla cardiaca aguda (SFCA). Su diagnóstico, requiere alta sospecha clínica y el uso racional de diferentes ayudas imagenológicas, conocido como abordaje multimodal, en cuyo caso, el ecocardiograma transtorácico es la herramienta de mayor uso y facilidad para la aproximación inicial e identificación del patrón típico de compromiso apical y transitorio de esta entidad; sin embargo, se hace necesaria la combinación con otros métodos diagnósticos para completar los criterios diagnósticos, realizar seguimiento hemodinámico en la fase aguda, detectar complicaciones y evaluar la recuperación durante el seguimiento ambulatorio, convirtiéndose en un gran reto diagnóstico y terapéutico.
