We provide a general framework for computing upper bounds on mixing times of finite Markov chains when its minimal ideal is left zero. Our analysis is based on combining results by Brown and Diaconis with our previous work on stationary distributions of finite Markov chains. Stationary distributions can be computed from the Karnofsky--Rhodes and McCammond expansion of the right Cayley graph of the finite semigroup underlying the Markov chain. Using loop graphs, which are planar graphs consisting of a straight line with attached loops, there are rational expressions for the stationary distribution in the probabilities. From these we obtain bounds on the mixing time. In addition, we provide a new Markov chain on linear extension of a poset with $n$ vertices, inspired by but different from the promotion Markov chain of Ayyer, Klee and the last author. The mixing time of this Markov chain is $O(n \log n)$.

Objectives: To describe the occurrence of postprocedural atrial fibrillation (AF) among patients with cryptogenic stroke undergoing patent foramen ovale (PFO) closure in the REDUCE clinical study and analyze for potential risk factors for the development of postprocedural AF. Background: AF is an adverse event that might potentially counterbalance the stroke prevention benefit from PFO closure. Data on AF after transcatheter PFO closure are sparse. Methods: We evaluated data from patients having PFO closure (Gore HELEX or Gore Cardioform Septal Occluder) in the REDUCE clinical trial (n = 408) in at post hoc explorative analysis. Median follow-up was 5.0 years. Results: AF occurred in 30 patients (7.4%) after PFO closure with a total of 34 AF events. Most were reported as non-serious (68%), detected within 45 days post-procedure (79%), and resolved within 2 weeks of onset (63%). One subject with AF had recurrent stroke. Postprocedural AF occurred more frequently among subjects with higher age and large device sizes. Male sex was the only independent predictor of postprocedural AF. We found no association between the type of occluder (HELEX or Gore Cardioform Septal Occluder) or PFO anatomical characteristics and post-procedural AF. Conclusion: In the REDUCE clinical study, postprocedural atrial fibrillation was mostly early onset, transient and with no later recurrence. Postprocedural AF occurred more frequently among patients with higher age and larger devices. Male sex was the only independent predictor of postprocedural AF.

Background and Purpose: Randomized patent foramen ovale closure trials have used open-label end point ascertainment which increases the risk of bias and undermines confidence in the conclusions. The Gore REDUCE trial prospectively performed baseline and follow-up magnetic resonance imaging (MRIs) for all subjects providing an objective measure of the effectiveness of closure. Methods: We performed blinded evaluations of the presence, location, and volume of new infarct on diffusion-weighted imaging of recurrent clinical stroke or new infarct (>3 mm) on T2/fluid attenuated inversion recovery from baseline to follow-up MRI at 2 years, comparing closure to medical therapy alone. We also examined the effect of shunt size and the development of atrial fibrillation on infarct burden at follow-up. Results: At follow-up, new clinical stroke or silent MRI infarct occurred in 18/383 (4.7%) patients who underwent closure and 19/177 (10.7%) medication-only patients (relative risk, 0.44 [95% CI, 0.24-0.81], P=0.02). Clinical strokes were less common in closure patients compared with medically treated patients, 5 (1.3%) versus 12 (6.8%), P=0.001, while silent MRI infarcts were similar, 13 (3.4%) versus 7 (4.0%), P=0.81. There were no differences in number, volumes, and distribution of new infarct comparing closure patients to those treated with medication alone. There were also no differences of number, volumes, and distribution comparing silent infarcts to clinical strokes. Infarct burden was also similar for patients who developed atrial fibrillation and for those with large shunts. Conclusions: The REDUCE trial demonstrates that patent foramen ovale closure prevents recurrent brain infarction based on the objective outcome of new infarcts on MRI. Only clinical strokes were reduced by closure while silent infarctions were similar between study arms, and there were no differences in infarct volume or location comparing silent infarcts to clinical strokes. Aims: The REDUCE study demonstrated a reduction in the risk of recurrent stroke with patent foramen ovale closure and antiplatelet therapy compared to antiplatelet therapy alone. The clinicians were allowed to choose among aspirin, clopidogrel, or aspirin/dipyridamole with the expectation that all antiplatelet therapies would have similar efficacy in this population. We tested that presumption by comparing recurrent stroke rates among antiplatelet agents within the control arm of the trial. Methods: We evaluated patients in REDUCE study who were randomized to the medical arm. The primary endpoint for this analysis was freedom from clinical ischemic stroke through at least 2 years of follow-up, to a maximum of 5 years. In the primary analysis, antiplatelet treatment was defined as the agent during the week prior to a recurrent stroke or last known contact. Results: Of 223 patients in the medical treatment arm, the initial agent was aspirin 52%, clopidogrel 30%, and aspirin/dipyridamole 12%. Patients treated with aspirin were similar to those treated with alternatives, but were more likely to be enrolled in the United States. The last reported agent was aspirin alone in 55%, clopidogrel alone in 31%, aspirin/dipyridamole in 7%, and other/nothing/missing in 7%. Recurrent stroke rates were similar for all 3 antiplatelet regimens in unadjusted and adjusted analyses, with no overall difference among agents (P=.17). Conclusions: Among patients with patent foramen ovale-associated stroke who were managed medically, there were no differences among antiplatelet agents in the risk of recurrent stroke, though confidence intervals were wide.

The prop names a category of ubiquity: props are everywhere in cinema. The term, short for property, is defined by the Oxford English Dictionary as ‘any portable object (now usually other than an article of costume) used in a play, film, etc., as required by the action’. The straightforwardness of this definition, however, belies the strangeness of the prop. The prop begs questions of scale, ambience, contingency, commodification, objecthood, and narration. In the context of narrative cinema, props seem as necessary as actors, sets, and locations. Regarding cinema through the lens of the prop — which is the lens of property — helps us to see how an ontological instrumentality courses through the very nature of the cinematic medium. This talk, which will veer from theory to history to questions of close reading, emerges from Rhodes’s recent book on domestic architecture and cinema, Spectacle of Property: The House in American Film (2017). It will show how foregrounding cinema’s prop-ness summons into view some of the medium’s most curious and most unsettling features. Aims: Percutaneous closure of a patent foramen ovale (PFO) is known to lower the risk of recurrent stroke in patients with a cryptogenic stroke. However, the economic implications of transcatheter PFO closure are less well known. From a UK payer perspective, a detailed economic appraisal of PFO closure was performed for prevention of recurrent ischemic stroke in patients with a PFO who had experienced a cryptogenic stroke. Materials and methods: A Markov cohort model was constructed using a 5-year time-horizon with a patient mean age of 45.2 years, reflecting the characteristics reported in the REDUCE trial. Transition probabilities, clinical inputs, costs, and utility values were ascertained from published and national costing sources. Total costs, incremental costs, quality-adjusted life-years (QALYs), and incremental cost-effectiveness ratios were calculated, utilizing a discount rate of 3.5%. A range of univariate and probabilistic sensitivity analyses were also performed. Results: When applying a willingness-to-pay (WTP) threshold of £20,000/QALY in accordance with NICE guidelines, PFO closure compared with antiplatelet therapy alone showed a beneficial cost/QALY of £18,584, attained at 4 years. Applying discount rates of 0% and 6% had a negligible effect on the base-case model findings. PFO closure demonstrated a 76.9% probability of being cost-effective at a WTP threshold of £20,000/QALY at a 5-year time-horizon. Limitations: This model focused specifically on UK stroke patients and typically enrolled young (mean age <65 years old) patients. Hence, caution should be taken when comparing data vs non-UK populations, and it remains unclear how older patients might have affected cost-effectiveness findings, as the risk of paradoxical embolism can persist as patients age. Aims: To evaluate the cost-effectiveness of percutaneous patent foramen ovale (PFO) closure, from a US payer perspective. Lower rates of recurrent ischemic stroke have been documented following percutaneous PFO closure in properly selected patients. Stroke in patients aged <60 years is particularly interesting because this population is typically at peak economic productivity and vulnerable to prolonged disability. Materials and methods: A Markov model comprising six health states (Stable after index stroke, Transient ischemic attack, Post-Transient Ischemic Attack, Clinical ischemic stroke, Post-clinical ischemic stroke, and Death) was constructed to evaluate the cost-effectiveness of PFO closure in combination with medical management versus medical management alone. The base-case model employed a 5-year time-horizon, with transition probabilities, clinical inputs, costs, and utility values ascertained from published and national costing sources. Incremental cost-effectiveness ratio (ICER) was evaluated per US guidelines, utilizing a discount rate of 3.0%. Results: At 5 years, overall costs and quality-adjusted life-years (QALYs) obtained from PFO closure compared with medical management were $16,323 vs $7,670 and 4.18 vs 3.77, respectively. At 5 years, PFO closure achieved an ICER of $21,049, beneficially lower than the conventional threshold of $50,000. PFO closure reached cost-effectiveness at 2.3 years (ICER = $47,145). Applying discount rates of 0% and 6% had a negligible impact on base-case model findings. Furthermore, PFO closure was 95.4% likely to be cost-effective, with a willingness-to-pay (WTP) threshold of $50,000 and a 5-year time horizon. Limitations: From a cost perspective, our economic model employed a US patient sub-population, so cost data may not extrapolate to other non-US stroke populations. The presence of a patent foramen ovale (PFO) is implicated in the pathogenesis of a number of medical conditions; however, the subject remains controversial and no official statements have been published. This interdisciplinary paper, prepared with involvement of eight European scientific societies, aims to review the available trial evidence and to define the principles needed to guide decision making in patients with PFO. In order to guarantee a strict process, position statements were developed with the use of a modified grading of recommendations assessment, development, and evaluation (GRADE) methodology. A critical qualitative and quantitative evaluation of diagnostic and therapeutic procedures was performed, including assessment of the risk/benefit ratio. The level of evidence and the strength of the position statements of particular management options were weighed and graded according to predefined scales. Despite being based often on limited and non-randomised data, while waiting for more conclusive evidence, it was possible to conclude on a number of position statements regarding a rational general approach to PFO management and to specific considerations regarding left circulation thromboembolism. For some therapeutic aspects, it was possible to express stricter position statements based on randomised trials. This position paper provides the first largely shared, interdisciplinary approach for a rational PFO management based on the best available evidence.

Until recently, evidence to support Patent Foramen Ovale (PFO) closure for secondary prevention of recurrent stroke has been controversial. Publication of high-quality evidence from randomized clinical trials and the subsequent FDA approval of two devices for percutaneous PFO closure is expected to increase the volume of PFO closure procedures not only in the United States but worldwide. As this technology is disseminated broadly to the public, ensuring the safe and efficacious performance of PFO closure is essential to mitigate risk and avoid unnecessary procedures. This document, prepared by a multi-disciplinary writing group convened by the Society for Cardiovascular Angiography and Interventions and including representatives from the American Academy of Neurology, makes recommendations for institutional infrastructure and individual skills necessary to initiate and maintain an active PFO/stroke program, with emphasis on shared decision making and patient-centered care.

We introduce semaphore codes associated to a Turing machine via resets. Semaphore codes provide an approximation theory for resets. In this paper we generalize the set-up of our previous paper "Random walks on semaphore codes and delay de Bruijn semigroups" to the infinite case by taking the profinite limit of $k$-resets to obtain $(-\omega)$-resets. We mention how this opens new avenues to attack the P versus NP problem.

To those outside the field, and even to some focused on empirical applications, phylogenetics may appear to have little to do with algebra. Probability and statistics are clearly important ingredients, as modeling and inferring evolutionary relationships motivate the field. Combinatorics is also an obvious component, as the graph-theoretic notions of trees, and more recently networks, are used to describe the relationships. But where does the algebra arise? The models used in phylogenetics are necessarily complex. At the simplest, they depend on a tree structure, as well as Markov matrices describing changes in nucleotide sequences along the edges. We develop a new approach to random walks on de Bruijn graphs over the alphabet $A$ through right congruences on $A^k$, defined using the natural right action of $A^+$. A major role is played by special right congruences, which correspond to semaphore codes and allow an easier computation of the hitting time. We show how right congruences can be approximated by special right congruences.

The continuing education unit (CEU) has been designed to facilitate the accumulation and exchange of standardized information about individual participation in noncredit continuing education. The CEU is to be applied only after content, format, and methodology have been determined to avoid creating stereotypes in terms of program length, methods, or formats used in meeting educational objectives. One CEU is defined as 10 contact hours of participation in an organized continuing education experience under responsible sponsorship, capable direction, and qualified instruction. It has several uses for the individual, professional organizations, and educational administration. In awarding the CEU, administrative and program criteria must be considered. Appropriate use of the CEU is determined by Task Force guidelines. Rationale The utility of patent foramen ovale (PFO) closure for secondary prevention in patients with prior cryptogenic stroke is uncertain despite multiple randomized trials completed to date. Aims The Gore REDUCE Clinical Study (REDUCE) aims to establish superiority of patent foramen ovale closure in conjunction with antiplatelet therapy over antiplatelet therapy alone in reducing the risk of recurrent clinical ischemic stroke or new silent brain infarct in patients who have had a cryptogenic stroke. Methods and design This controlled, open-label trial randomized 664 subjects with cryptogenic stroke at 63 multinational sites in a 2:1 ratio to either antiplatelet therapy plus patent foramen ovale closure (with GORE® HELEX® Septal Occluder or GORE® CARDIOFORM Septal Occluder) or antiplatelet therapy alone. Subjects will be prospectively followed for up to five years. Neuroimaging is required for all subjects at baseline and at two years or study exit. Study outcomes The two co-primary endpoints for the study are freedom from recurrent clinical ischemic stroke through at least 24 months post-randomization and incidence of new brain infarct (defined as clinical ischemic stroke or silent brain infarct) through 24 months. The primary analyses are an unadjusted log-rank test and a binomial test of subject-based proportions, respectively, both on the intent-to-treat population, with adjustment for testing multiplicity. Discussion The REDUCE trial aims to target a patient population with truly cryptogenic strokes. Medical therapy is limited to antiplatelet agents in both arms thereby reducing confounding. The trial should determine whether patent foramen ovale closure with the Gore septal occluders is safe and more effective than medical therapy alone for the prevention of recurrent clinical ischemic stroke or new silent brain infarct; the neuroimaging data will provide an opportunity to further support the proof of concept. BACKGROUND: The efficacy of closure of a patent foramen ovale (PFO) in the prevention of recurrent stroke after cryptogenic stroke is uncertain. We investigated the effect of PFO closure combined with antiplatelet therapy versus antiplatelet therapy alone on the risks of recurrent stroke and new brain infarctions.METHODS: In this multinational trial involving patients with a PFO who had had a cryptogenic stroke, we randomly assigned patients, in a 2:1 ratio, to undergo PFO closure plus antiplatelet therapy (PFO closure group) or to receive antiplatelet therapy alone (antiplatelet-only group). Imaging of the brain was performed at the baseline screening and at 24 months. The coprimary end points were freedom from clinical evidence of ischemic stroke (reported here as the percentage of patients who had a recurrence of stroke) through at least 24 months after randomization and the 24-month incidence of new brain infarction, which was a composite of clinical ischemic stroke or silent brain infarction detected on imaging.RESULTS: We enrolled 664 patients (mean age, 45.2 years), of whom 81% had moderate or large interatrial shunts. During a median follow-up of 3.2 years, clinical ischemic stroke occurred in 6 of 441 patients (1.4%) in the PFO closure group and in 12 of 223 patients (5.4%) in the antiplatelet-only group (hazard ratio, 0.23; 95% confidence interval [CI], 0.09 to 0.62; P=0.002). The incidence of new brain infarctions was significantly lower in the PFO closure group than in the antiplatelet-only group (22 patients [5.7%] vs. 20 patients [11.3%]; relative risk, 0.51; 95% CI, 0.29 to 0.91; P=0.04), but the incidence of silent brain infarction did not differ significantly between the study groups (P=0.97). Serious adverse events occurred in 23.1% of the patients in the PFO closure group and in 27.8% of the patients in the antiplatelet-only group (P=0.22). We begin a systematic study of finite semigroups that generate join irreducible members of the lattice of pseudovarieties of finite semigroups, which are important for the spectral theory of this lattice. Finite semigroups $S$ that generate join irreducible pseudovarieties are characterized as follows: whenever $S$ divides a direct product $A \times B$ of finite semigroups, then $S$ divides either $A^n$ or $B^n$ for some $n \geq 1$. We present a new operator ${ \mathbf{V} \mapsto \mathbf{V}^\mathsf{bar} }$ that preserves the property of join irreducibility, as does the dual operator, and show that iteration of these operators on any nontrivial join irreducible pseudovariety leads to an infinite hierarchy of join irreducible pseudovarieties. We also describe all join irreducible pseudovarieties generated by a semigroup of order up to five. It turns out that there are $30$ such pseudovarieties, and there is a relatively easy way to remember them. We explore the size of the largest (permuted) triangular submatrix of a random matrix, and more precisely its asymptotical behavior as the size of the ambient matrix tends to infinity. The importance of such permuted triangular submatrices arises when dealing with certain combinatorial algebraic settings in which these submatrices determine the rank of the ambient matrix and thus attract special attention.

BACKGROUND: Right ventricular outflow tract (RVOT) conduit dysfunction is a limitation of the Ross procedure. Transcatheter pulmonary valve replacement (TPVR) could alter the impact of conduit dysfunction and the risk-benefit balance for the Ross procedure.METHODS: Retrospective review of databases from 3 prospective Melody TPV (Medtronic Inc, Minneapolis, MN) trials.RESULTS: Among 358 patients who were catheterized with the intent to implant a Melody TPV for RVOT conduit stenosis or regurgitation (PR) as part of 3 prospective multicenter studies, 67 (19%) had a prior Ross procedure. Of these, 56 (84%) received a Melody valve; in 5 of the 11 patients who did not, the implant was aborted due to concern for coronary artery compression, and 1 implanted patient required emergent surgery for left coronary compression. The RVOT gradient decreased from a median 38 mm Hg to 13.5 mm Hg (p < 0.001). There was no or trivial PR in all but 4 patients, in whom it was mild. At a median follow-up of 4.0 years, 1 patient died from sepsis. Twelve patients underwent 14 transcatheter (n = 8) or surgical (n = 6) TPV reinterventions for obstruction with stent fracture (n = 9), endocarditis with conduit obstruction (n = 3), or reoperation (n = 2). Freedom from TPV explant was 89% ± 5% at 4 years. Among patients who did not undergo reintervention for obstruction, there was no change in RVOT gradient over time, and all but 1 patient had mild or less PR at last follow-up.CONCLUSIONS: The TPVR with the Melody valve provides acceptable early outcomes and durable valve function in the majority of Ross patients. Recurrent RVOT obstruction associated with stent fracture was the main reason for reintervention. Coronary compression is not uncommon in Ross patients and should be assessed prior to TPVR.

Atherosclerotic plaque localization correlates with regions of disturbed flow in which endothelial cells (ECs) align poorly, whereas sustained laminar flow correlates with cell alignment in the direction of flow and resistance to atherosclerosis. We now report that in hypercholesterolemic mice, deletion of syndecan 4 (S4(-/-)) drastically increased atherosclerotic plaque burden with the appearance of plaque in normally resistant locations. Strikingly, ECs from the thoracic aortas of S4(-/-) mice were poorly aligned in the direction of the flow. Depletion of S4 in human umbilical vein endothelial cells (HUVECs) using shRNA also inhibited flow-induced alignment in vitro, which was rescued by re-expression of S4. This effect was highly specific, as flow activation of VEGF receptor 2 and NF-κB was normal. S4-depleted ECs aligned in cyclic stretch and even elongated under flow, although nondirectionally. EC alignment was previously found to have a causal role in modulating activation of inflammatory versus antiinflammatory pathways by flow. Consistent with these results, S4-depleted HUVECs in long-term laminar flow showed increased activation of proinflammatory NF-κB and decreased induction of antiinflammatory kruppel-like factor (KLF) 2 and KLF4. The point of departure for this premiere performance was the obliteration, literally the blotting-out, of the cinematic object of projection. Projection performance as the art par excellence of obstructing, concealing, and masking absolutely nothing but itself. Obliteration: in which the spectacle of the apparatus is to be screened, or better screened-out, in the negative relief of a self-cancellation vis-à-vis nothing but the disciplinary application of a cinematic standard.Sandra Gibson and Luis Recoder stage the scene of film as orphaned object through the temporal labor of moving image installation. Collaborators since 2000, Gibson and Recoder unite the rich traditions of the experimental film, particularly its structuralist and materialist strands, and the multi-modal sensibility of expanded cinema that emerged in the 1960s, in which the moving image was woven into the labile space of performance, sound and audience interaction. Pasolini and Crane both refer to practices of melding different styles and traditions in order to produce fresh and unusual aesthetic experiences. The discussion focused on the importance of physical locations and the specificity of geography in their work. Place was a heuristic for Pasolini’s practice and politics as it is for Cathy Lee Crane. The conversation also engaged with Pasolini’s concept of ‘Cinema of Poetry’ and the stylistic freedom and intensity that it proposes. It grounded the viewing of Crane’s film Pasolini’s Last Words on 26 September 2014 in the Pasolini retrospective at the Cinema Arsenal in the context of the Pasolini Roma exhibit at the Martin-Gropius-Bau. Cathy Lee Crane has been making hybrid films on 16mm since 1994. Her first feature Pasolini’s Last Words (2012) was supported by a grant from the New York State Council on the Arts in 2010 and a 2009 New York Foundation for the Arts Fellowship in Film, and premiered at Festival du Nouveau Cinema in Montreal as a ‘gem of world cinema’. She received the prestigious Guggenheim Fellowship in 2013 for her lyrical re-combinations of archival and staged material. Currently she is associate professor in the Department of Cinema, Photography, and Media Arts at Ithaca College. John David Rhodes is a university lecturer in film studies in the Department of Italian, University of Cambridge. His research focuses on cinema’s engagement with other aesthetic forms and on the mutual implication of theory and material history. He is he author of Stupendous, Miserable City: Pasolini’s Rome (2007) where he places the city of Rome at the center of this in-depth examination of the work of Pasolini. Manuele Gragnolati (Oxford University/ICI Berlin) is professor of Italian at Oxford University and Fellow of Somerville College. His latest book is entitled Amor che move. Linguaggio del corpo e formal del desiderio in Dante, Pasolini e Morante (2013). Keynote by Laurence Rickels Live performance by Sandra Gibson and Luis Recoder The term abandon encompasses radical renunciation and immersive indulgence in its oscillation between abandonment of and abandonment to, between restraint and luxury, mindfulness and neglect. When we speak of abandonment we indicate a situation in which we take leave of something, or disband a collective entity, or else act in a way that suggests a disaggregation of certain protocols of behaviour, or belonging (as when we ‘laugh with abandon’). Discourses and scenes of media and politics are generally highly invested in ideas of taking-leave, breaking apart or away, acting with abandon. In the present moment, we believe the term resonates in manifold ways. For instance: with often painful choices between theoretical and political models that have outlasted their effectiveness but to which there seem to be no alternatives; with turns to abandoned objects as new sources of ontologies in which the turn itself is a mode of abandoning an established political-theoretical project; with the obdurate ‘problem’ of pleasure in aesthetics and aesthetic theory as either the obstacle or the medium of the aesthetic’s interface with the political; with the cathexis of the body and its phenomenology as an instrument and medium of political and aesthetic experimentation; with attempts to relinquish the human, and its attendant association with agency, as a category of experience; with contemporary experiences/fantasies of control and resistance to control; with theatricalizations of abjuration and gratification., Abandon: World Picture Conference, conference, ICI Berlin, 7–8 November 2014

We present a review of the symbol map, a mathematical tool that can be useful in simplifying expressions among multiple polylogarithms, and recall its main properties. A recipe is given for how to obtain the symbol of a multiple polylogarithm in terms of the combinatorial properties of an associated rooted decorated polygon. We also outline a systematic approach to constructing a function corresponding to a given symbol, and illustrate it in the particular case of harmonic polylogarithms up to weight four. Furthermore, part of the ambiguity of this process is highlighted by exhibiting a family of non-trivial elements in the kernel of the symbol map for arbitrary weight.

ObjectivesIn the Single Ventricle Reconstruction trial, infants undergoing the Norwood procedure were randomly allocated to undergo a right ventricle-to-pulmonary artery shunt or a modified Blalock-Taussig shunt. Apart from shunt type, subjects received the local standard of care. We evaluated variation in perioperative care during the Norwood hospitalization across 14 trial sites.MethodsData on preoperative, operative, and postoperative variables for 546 enrolled subjects who underwent the Norwood procedure were collected prospectively on standardized case report forms, and variation across the centers was described.ResultsGestational age, birth weight, and proportion with hypoplastic left heart syndrome were similar across sites. In contrast, all recorded variables related to preoperative care varied across centers, including fetal diagnosis (range, 55%-85%), preoperative intubation (range, 29%-91%), and enteral feeding. Perioperative and operative factors were also variable across sites, including median total support time (range, 74-189 minutes) and other perfusion variables, arch reconstruction technique, intraoperative medication use, and use of modified ultrafiltration (range, 48%-100%). Additional variation across centers was seen in variables related to postoperative care, including proportion with an open sternum (range, 35%-100%), median intensive care unit stay (range, 9-44 days), type of feeding at discharge, and enrollment in a home monitoring program (range, 1%-100%; 5 sites did not have a program). Overall, in-hospital death or transplant occurred in 18% (range across sites, 7%-39%).ConclusionsPerioperative care during the Norwood hospitalization varies across centers. Further analysis evaluating the underlying causes and relationship of this variation to outcome is needed to inform future studies and quality improvement efforts.

Geometric semigroup theory is the systematic investigation of finitely-generated semigroups using the topology and geometry of their associated automata. In this article we show how a number of easily-defined expansions on finite semigroups and automata lead to simplifications of the graphs on which the corresponding finite semigroups act. We show in particular that every finite semigroup can be finitely expanded so that the expansion acts on a labeled directed graph which resembles the right Cayley graph of a free Burnside semigroup in many respects.

Background : We report a multiinstitutional study on intermediate-term outcome of intravascular stenting for treatment of coarctation of the aorta using integrated arch imaging (IAI) techniques. Methods and Results : Medical records of 578 patients from 17 institutions were reviewed. A total of 588 procedures were performed between May 1989 and Aug 2005. About 27% (160/588) procedures were followed up by further IAI of their aorta (MRI/CT/repeat cardiac catheterization) after initial stent procedures. Abnormal imaging studies included: the presence of dissection or aneurysm formation, stent fracture, or the presence of reobstruction within the stent (instent restenosis or significant intimal build-up within the stent). Forty-one abnormal imaging studies were reported in the intermediate follow-up at median 12 months (0.5–92 months). Smaller postintervention of the aorta (CoA) diameter and an increased persistent systolic pressure gradient were associated with encountering abnormal follow-up imaging studies. Aortic wall abnormalities included dissections ( n = 5) and aneurysm ( n = 13). The risk of encountering aortic wall abnormalities increased with larger percent increase in CoA diameter poststent implant, increasing balloon/coarc ratio, and performing prestent angioplasty. Stent restenosis was observed in 5/6 parts encountering stent fracture and neointimal buildup ( n = 16). Small CoA diameter poststent implant and increased poststent residual pressure gradient increased the likelihood of encountering instent restenosis at intermediate follow-up. Conclusions : Abnormalities were observed at intermediate follow-up following IS placement for treatment of native and recurrent coarctation of the aorta. Not exceeding a balloon:coarctation ratio of 3.5 and avoidance of prestent angioplasty decreased the likelihood of encountering an abnormal follow-up imaging study in patients undergoing intravascular stent placement for the treatment of coarctation of the aorta. A mixed mode cooling system is one which operates in either the open, closed, or helper (once-through but with the use of the cooling towers) modes. Such systems may be particularly economical where the need for supplementary cooling to meet environmental constraints on induced water temperature changes is seasonal or dependent upon other transient factors such as stream- flow. The issues involved in the use of mixed mode systems include the design of the open cycle and closed cycle portions of the cooling system, the specification of the environmental standard to be met, and the monitoring system and associated decision rules used to determine when mode changes are necessary. These issues have been examined in the context of a case study of TVA's Browns Ferry Nuclear Plant which utilizes the large quantity of site specific data reflecting conditions both with and without plant operation. The most important findings of this study are: (1) The natural temperature differences in the Tennessee River are of the same order of magnitude (50F) as the maximum allowed induced temperature increase. (2) Predictive estimates based on local hydrological and meteorological data are capable of accounting for 40% of the observed natural variability. (3) Available algorithms for plant induced temperature increases provide estimates within 1F of observed values except during periods of strong stratification. (4) A mixed mode system experiences only 10% of the capacity losses experienced by a totally closed system, (5) The capacity loss is relatively more sensitive to the environmental standard than to changes in cooling system design. (6) About one third of the capacity loss incurred using the mixed mode system is the result of natural temperature variations. This unnecessary loss may be halved by the use of predictive estimates for natural temperature differences.
