Abstract: Background Chronic cough is a common respiratory symptom with an impact on daily activities and quality of life. Global prevalence data are scarce and derive mainly from European and Asian countries and studies with outcomes other than chronic cough. In this study, we aimed to estimate the prevalence of chronic cough across a large number of study sites as well as to identify its main risk factors using a standardised protocol and definition. Methods We analysed cross-sectional data from 33,983 adults (>= 40 years), recruited between Jan 2, 2003 and Dec 26, 2016, in 41 sites (34 countries) from the Burden of Obstructive Lung Disease (BOLD) study. We estimated the prevalence of chronic cough for each site accounting for sampling design. To identify risk factors, we conducted multivariable logistic regression analysis within each site and then pooled estimates using random -effects metaanalysis. We also calculated the population attributable risk (PAR) associated with each of the identifed risk factors. Findings The prevalence of chronic cough varied from 3% in India (rural Pune) to 24% in the United States of America (Lexington,KY). Chronic cough was more common among females, both current and passive smokers, those working in a dusty job, those with a history of tuberculosis, those who were obese, those with a low level of education and those with hypertension or airflow limitation. The most influential risk factors were current smoking and working in a dusty job. Interpretation Our findings suggested that the prevalence of chronic cough varies widely across sites in different world regions. Cigarette smoking and exposure to dust in the workplace are its major risk factors.

Abstract: Item does not contain fulltext, OBJECTIVE: Identifying biomarkers of transition to psychosis in individuals at clinical high risk for psychosis (CHR-P) is essential to understanding the mechanisms underlying the disease. Although cross-sectional abnormalities in cortical surface area (CSA) have been demonstrated in individuals at CHR-P who transition to psychosis (CHR-P-T) compared with those who do not (CHR-P-NT), how CSA longitudinally develops remains unclear, especially in younger individuals. We set out to compare CSA in adolescents at CHR-P and healthy controls (HC) over 2 points in time. METHOD: A longitudinal multicenter study was performed in adolescents at CHR-P in comparison to HC and according to transition to psychosis. Magnetic resonance imaging scans were acquired at baseline, at 18-month follow-up, or at the time of transition. Images were pre-processed and hemisphere and regional CSA were computed using FreeSurfer. Between-group analyses were performed with linear mixed-effects models. RESULTS: A total of 313 scans (107 CHR-P and 102 HC) were included in the analysis. At 18 months, the rate of transition to psychosis in CHR-P was 23.4%. Adolescents at CHR-P-T presented greater age-related decrease in CSA in the left parietal and occipital lobes compared with HC, and in the bilateral parietal lobe and right frontal lobe relative to CHR-P-NT. These results were not influenced by antipsychotic treatment, cannabis use, or intelligence quotient (IQ). CONCLUSION: Adolescents at CHR-P that developed a psychotic disorder presented different developmental trajectories of CSA relative to those who did not. A relatively greater decrease in CSA in the parietal and frontal lobes may index clinical transition to psychosis in adolescents at CHR-P.

Abstract: Modelling complex interactions involving climatic features, socio-economic vulnerability or responses, and long impact transmissions is associated with substantial uncertainty. Physical climate storylines are proposed as an approach to explore complex impact transmission pathways and possible alternative unfoldings of event cascades under future climate conditions. These storylines are particularly useful for climate risk assessment for complex domains, including event cascades crossing multiple disciplinary or geographical borders. For an effective role in climate risks assessments, development guidelines are needed to consistently develop and interpret the storyline event analyses. This paper elaborates on the suitability of physical climate storyline approaches involving climate event induced shocks propagating into societal impacts. It proposes a set of common elements to construct the event storylines. In addition, criteria for their application for climate risk assessment are given, referring to the need for storylines to be physically plausible, relevant for the specific context, and risk-informative. Apart from an illustrative gallery of storyline examples found in literature, three examples of varying scope and complexity are presented in detail, all involving the potential impact on European socio-economic sectors induced by remote climate change features occurring far outside the geographical domain of the European mainland. The storyline examples illustrate the application of the proposed storyline components and evaluate the suitability of the criteria defined in this paper. It thereby contributes to a rigorous design and application of event-based climate storyline approaches.

4. Migratory patterns of Patagonian toothfish (Dissostichus eleginoides) in the southwestern Atlantic Ocean

Abstract: The Patagonian toothfish (Dissostichus eleginoides) is a commercially valuable demersal fish present in the southern hemisphere. Therefore, understanding movements of the species across time and spatial scales would enhance our understanding of its behavior within the Argentine and Chilean Patagonian platform. The Instituto Nacional de Investigación y Desarrollo Pesquero (INIDEP, Argentina) initiated a tag-recapture program for D. eleginoides in 2004. A total of 5,907 specimens, mostly juveniles (< 82 cm total length), have been tagged and released in waters off the edge of the Argentine shelf and slope between 37° S and 47° S (northern Sector of the Argentine fishing ground), and east of De los Estados Island and south of Tierra del Fuego (54° S-57° S -southern Sector of the Argentine fishing ground). A total of 121 specimens were recaptured: 25 (20.7%) were recovered in the northern Sector, 84 (69.4%) in the southern Sector and 12 (9.9%) in waters of the Pacific Ocean in Chile. A total of 67.5% were recaptured within 20 nm (37 km) of the release site and 15% traveled distances of less than 120 nm. A smaller fraction (5%) traveled distances between 120 and 400 nm and only 12.5% were recaptured at more than 400 nm. Using a Generalized Additive Model, it was determined that variables Days at liberty, Sector and Time of tagging were influential in the distance traveled by specimens. From the present work, it seems evident that this species possesses high site fidelity and lacks cyclic migratory movements involving a substantial component of the stock in the American southern cone., La merluza negra (Dissostichus eleginoides) es un pez demersal presente en el hemisferio sur, muy valioso comercialmente. Por ello, comprender los movimientos en diferentes escalas temporales y espaciales contribuiría a conocer más acerca del comportamiento que presenta la especie en la plataforma patagónica argentina y chilena. Desde 2004, el Instituto Nacional de Investigación y Desarrollo Pesquero (INIDEP, Argentina), inició un programa de marcado y recaptura de D. eleginoides. Un total de 5.907 ejemplares, en su mayoría juveniles (< 82 cm de largo total), fueron marcados y liberados en sectores ubicados en aguas del borde de la plataforma y talud de la Argentina entre 37° S y 47° S (Sector norte del caladero argentino), y al este de la Isla de los Estados y sur de Tierra del Fuego (54° S-57° S -Sector sur del caladero argentino). Actualmente, fueron recapturados 121 ejemplares, 25 (20,7%) se recuperaron en el Sector norte, 84 (69,4%) en el Sector sur y 12 (9,9%) en aguas del Océano Pacífico en Chile. El 67,5% fue recapturado a menos de 20 mn (37 km) del lugar de liberación y 15% recorrió distancias inferiores a las 120 mn. Una fracción menor (5%) recorrió distancias entre 120 y 400 mn y solo 12,5% se recapturó a más de 400 mn. Mediante un Modelo Aditivo Generalizado se determinó que las variables Días en libertad, Sector y Época de marcado influyeron en la distancia recorrida de los ejemplares. A partir del presente trabajo, parece evidente que la especie en el cono sur americano carece de desplazamientos migratorios regulares que involucren a una parte significativa del stock.

5. Effects of the establishment of marine protected areas on the Argentine Patagonian toothfish (Dissostichus eleginoides) fisheries

Abstract: At the beginning of the 1990s, the Argentine toothfish fishery gained prominence as a consequence of the rapid expansion of trawler and longliner fleets targeting this species. This fishing area covers the slope and shelf from 60° S to 37° S in the Argentine Exclusive Economic Zone. The main fishing ground is located in the southern area, bordering Namuncurá-Burdwood Bank II (NBBII) and Yaganes (Y) marine protected areas (MPA) established in 2018. In order to determine the impact generated by MPAs on effort distribution, 308 fishing trips carried out between 2010 and 2020, which reported 82% of the total fishing effort of Argentine toothfish declared in that period, were spatially analyzed. The Y-MPA sector categorized as National Marine Reserve and located to the south of Tierra del Fuego reported more than half (58%) of the toothfish catch recorded throughout that period, while the NBBII-MPA located to the east of Tierra del Fuego and south of De los Estados Island represented 17%. The NBBII-MPA sector established as a Strict National Marine Reserve and located to the south of the Burdwood Bank represented 25%. With the establishment of the MPAs, 7.11% of the international requirement has been met. At the moment, effects resulting from the creation of MPAs can only be speculated upon qualitatively, but should be quantified in the near future., A principios de la década de 1990, la pesquería de merluza negra argentina ganó protagonismo gracias a la rápida expansión de las flotas de arrastre y palangre que se dirigían a esta especie. Esta zona de pesca cubre el talud y plataforma desde los 60° S hasta los 37° S en la Zona Económica Exclusiva argentina. El principal caladero se encuentra en la zona sur, colindando con las áreas marinas protegidas (AMP) Banco Namuncurá-Burdwood II (NBBII) y Yaganes (Y), establecidas en 2018. Para determinar el impacto que generan las AMP en la distribución del esfuerzo, se analizaron espacialmente 308 viajes de pesca realizados entre 2010 y 2020, que reportaron 82% del total del esfuerzo pesquero declarado de merluza negra argentina en ese período. El sector Y-AMP categorizado como Reserva Nacional Marina y ubicado al sur de Tierra del Fuego, reportó más de la mitad (58%) de la captura de merluza negra registrada durante ese período, mientras que el NBBII-AMP ubicado al este de Tierra del Fuego y sur de la Isla de los Estados representaron 17%. El sector NBBII-AMP establecido como Reserva Nacional Marina Estricta y ubicado al sur del Banco Burdwood representó el 25%. Con el establecimiento de las AMP se ha cumplido 7,11% del requerimiento internacional. En la actualidad, los efectos resultantes de la creación de AMP solo pueden especularse cualitativamente, pero deberían cuantificarse en un futuro próximo.

Abstract: Cardiovascular diseases (CVDs), principally ischemic heart disease (IHD) and stroke, are the leading cause of global mortality and a major contributor to disability. This paper reviews the magnitude of total CVD burden, including 13 underlying causes of cardiovascular death and 9 related risk factors, using estimates from the Global Burden of Disease (GBD) Study 2019. GBD, an ongoing multinational collaboration to provide comparable and consistent estimates of population health over time, used all available population-level data sources on incidence, prevalence, case fatality, mortality, and health risks to produce estimates for 204 countries and territories from 1990 to 2019. Prevalent cases of total CVD nearly doubled from 271 million (95% uncertainty interval [UI]: 257 to 285 million) in 1990 to 523 million (95% UI: 497 to 550 million) in 2019, and the number of CVD deaths steadily increased from 12.1 million (95% UI:11.4 to 12.6 million) in 1990, reaching 18.6 million (95% UI: 17.1 to 19.7 million) in 2019. The global trends for disability-adjusted life years (DALYs) and years of life lost also increased significantly, and years lived with disability doubled from 17.7 million (95% UI: 12.9 to 22.5 million) to 34.4 million (95% UI:24.9 to 43.6 million) over that period. The total number of DALYs due to IHD has risen steadily since 1990, reaching 182 million (95% UI: 170 to 194 million) DALYs, 9.14 million (95% UI: 8.40 to 9.74 million) deaths in the year 2019, and 197 million (95% UI: 178 to 220 million) prevalent cases of IHD in 2019. The total number of DALYs due to stroke has risen steadily since 1990, reaching 143 million (95% UI: 133 to 153 million) DALYs, 6.55 million (95% UI: 6.00 to 7.02 million) deaths in the year 2019, and 101 million (95% UI: 93.2 to 111 million) prevalent cases of stroke in 2019. Abstract: Background: Adverse cardiac remodeling is a major risk factor for the development of post myocardial infarction (MI) heart failure (HF). This study investigates the effects of the chymase inhibitor fulacimstat on adverse cardiac remodeling after acute ST-segment-elevation myocardial infarction (STEMI). Methods: In this double-blind, randomized, placebo-controlled trial patients with first STEMI were eligible. To preferentially enrich patients at high risk of adverse remodeling, main inclusion criteria were a left-ventricular ejection fraction (LVEF) ≤45% and an infarct size >10% on day 5 to 9 post MI as measured by cardiac MRI. Patients were then randomized to 6 months treatment with either 25 mg fulacimstat (n = 54) or placebo (n = 53) twice daily on top of standard of care starting day 6 to 12 post MI. The changes in LVEF, LV end-diastolic volume index (LVEDVI), and LV end-systolic volume index (LVESVI) from baseline to 6 months were analyzed by a central blinded cardiac MRI core laboratory. Results: Fulacimstat was safe and well tolerated and achieved mean total trough concentrations that were approximately tenfold higher than those predicted to be required for minimal therapeutic activity. Comparable changes in LVEF (fulacimstat: 3.5% ± 5.4%, placebo: 4.0% ± 5.0%, P = .69), LVEDVI (fulacimstat: 7.3 ± 13.3 mL/m2, placebo: 5.1 ± 18.9 mL/m2, P = .54), and LVESVI (fulacimstat: 2.3 ± 11.2 mL/m2, placebo: 0.6 ± 14.8 mL/m2, P = .56) were observed in both treatment arms. Conclusion: Fulacimstat was safe and well tolerated in patients with left-ventricular dysfunction (LVD) after first STEMI but had no effect on cardiac remodeling.

Abstract: Complex interactions involving climatic features, socio-economic vulnerability or responses, and long impact transmissions are associated with substantial uncertainty. Physical climate storylines are proposed as approach to explore complex impact transmission pathways and possible alternative unfolding of event cascades under future climate conditions. These storylines are particularly useful for climate risk assessment for complex domains, including event cascades crossing multiple disciplinary or geographical borders. For an effective role in climate risks assessments, practical guidelines are needed to consistently develop and interpret the storyline event analyses.This paper elaborates on the suitability of physical climate storyline approaches involving climate event induced shocks propagating into societal impacts. It proposes a set of common elements to construct the event storylines. In addition, criteria for their application for climate risk assessment are given, referring to the need for storylines to be physically plausible, relevant for the specific context, and risk-informative.Six examples of varying scope and complexity are presented, all involving the potential climate change impact on European socio-economic sectors induced by remote climate change features occurring far outside the geographical domain of the European mainland. The storyline examples illustrate the application of the proposed storyline components and evaluates the suitability criteria defined in this paper. It thereby contributes to the standardization of the design and application of event-based climate storyline approaches.

Abstract: Background: During the past century, socioeconomic and scientific advances have resulted in changes in the health and physique of European populations. Accompanying improvements in lung function, if unrecognised, could result in the misclassification of lung function measurements and misdiagnosis of lung diseases. We therefore investigated changes in population lung function with birth year across the past century, accounting for increasing population height, and examined how such changes might influence the interpretation of lung function measurements. Methods: In our analyses of cross-sectional data from ten European population-based studies, we included individuals aged 20–94 years who were born between 1884 and 1996, regardless of previous respiratory diagnoses or symptoms. FEV1, forced vital capacity (FVC), height, weight, and smoking behaviour were measured between 1965 and 2016. We used meta-regression to investigate how FEV1 and FVC (adjusting for age, study, height, sex, smoking status, smoking pack-years, and weight) and the FEV1/FVC ratio (adjusting for age, study, sex, and smoking status) changed with birth year. Using estimates from these models, we graphically explored how mean lung function values would be expected to progressively deviate from predicted values. To substantiate our findings, we used linear regression to investigate how the FEV1 and FVC values predicted by 32 reference equations published between 1961 and 2015 changed with estimated birth year. Findings: Across the ten included studies, we included 243 465 European participants (mean age 51·4 years, 95% CI 51·4–51·5) in our analysis, of whom 136 275 (56·0%) were female and 107 190 (44·0%) were male. After full adjustment, FEV1 increased by 4·8 mL/birth year (95% CI 2·6–7·0; p<0·0001) and FVC increased by 8·8 mL/birth year (5·7–12·0; p<0·0001). Abstract: Background: During the past century, socioeconomic and scientific advances have resulted in changes in the health and physique of European populations. Accompanying improvements in lung function, if unrecognised, could result in the misclassification of lung function measurements and misdiagnosis of lung diseases. We therefore investigated changes in population lung function with birth year across the past century, accounting for increasing population height, and examined how such changes might influence the interpretation of lung function measurements. Methods: In our analyses of cross-sectional data from ten European population-based studies, we included individuals aged 20–94 years who were born between 1884 and 1996, regardless of previous respiratory diagnoses or symptoms. FEV1, forced vital capacity (FVC), height, weight, and smoking behaviour were measured between 1965 and 2016. We used meta-regression to investigate how FEV1 and FVC (adjusting for age, study, height, sex, smoking status, smoking pack-years, and weight) and the FEV1/FVC ratio (adjusting for age, study, sex, and smoking status) changed with birth year. Using estimates from these models, we graphically explored how mean lung function values would be expected to progressively deviate from predicted values. To substantiate our findings, we used linear regression to investigate how the FEV1 and FVC values predicted by 32 reference equations published between 1961 and 2015 changed with estimated birth year. Findings: Across the ten included studies, we included 243 465 European participants (mean age 51·4 years, 95% CI 51·4–51·5) in our analysis, of whom 136 275 (56·0%) were female and 107 190 (44·0%) were male. 14. Quantum Origin of (Newtonian) Mass and Symmetry for Lorentz Covariant Physics

Abstract: The Galilean symmetry and the Poincare symmetry are usually taken as the fundamental (relativity) symmetries for `nonrelativistic' and `relativistic' physics, respectively, quantum or classical. Our fully group theoretical formulation approach to the theories, together with its natural companion of mechanics from symplectic geometry, ask for different perspectives. We present a sketch of the full picture here, emphasizing aspects which are different from the more familiar picture. The letter summarizes our earlier presented formulation while focusing on the part beyond, with an adjusted, or corrected, identification of the basic representations having the (Newtonian) mass as a Casimir invariant. 15. Quantum Frames of Reference and the Noncommutative Values of Observables

Abstract: Based on a recent relational formulation of quantum reference frame transformations, especially with a case of quantum spatial translations in particular, we analyzed how the `value' of an observable for a fixed state change. That is the exact analog of the classical description, for example, of the value of the $x$-coordinate for a particle decrease by 2 units when we perform a translation of the reference frame putting the new origin at $x=2$. The essence of the quantum reference frame transformations is to have the quantum fluctuations, and even entanglement, of the physical object which serves as the (new) reference frame, taken into account. We illustrate how the recently introduced notion of the noncommutative values of quantum observables gives such a definite description successfully. Formulations, and an analysis of a case example in qubit systems, of analog transformations for observables with a discrete or finite spectrum is also presented. Abstract: The chymase inhibitor fulacimstat is developed as a first-in-class treatment option for the inhibition of adverse cardiac remodeling in patients with left ventricular dysfunction (LVD) after acute myocardial infarction (MI). The aim of the study was to examine the safety and tolerability of fulacimstat in patients with LVD after remote MI. A multicenter, multinational randomized, placebo-controlled study was performed in clinically stable patients (40–79 years of age, left ventricular ejection fraction ≤ 45% because of MI in medical history) who were on stable evidence-based standard-of-care therapies for LVD post-MI including an angiotensin converting enzyme inhibitor or angiotensin receptor blocker at doses of at least half the recommended target dose. Patients were treated for 2 weeks with either placebo (n = 12) or 4 different doses of fulacimstat (5 mg twice daily, n = 9; 10 mg twice daily, n = 9; 25 mg twice daily, n = 10; 50 mg once daily, n = 9). Fulacimstat was safe and well tolerated at all examined doses. There were no clinically relevant effects on vital signs or potassium levels compared with placebo treatment. Mean plasma concentrations of fulacimstat increased with the administered dose and reached exposures predicted to be therapeutically active. The safety profile and the absence of effects on blood pressure or heart rate in a chronic patient population having similar comorbidities and receiving similar comedication as patients after acute MI support future clinical trials with fulacimstat in patients after acute MI.

Abstract: Extracellular vesicles (EVs) have great potential as biomarkers since their composition and concentration in biofluids are disease state dependent and their cargo can contain disease-related information. Large tumor-derived EVs (tdEVs, >1 μm) in blood from cancer patients are associated with poor outcome, and changes in their number can be used to monitor therapy effectiveness. Whereas, small tumor-derived EVs (<1 μm) are likely to outnumber their larger counterparts, thereby offering better statistical significance, identification and quantification of small tdEVs are more challenging. In the blood of cancer patients, a subpopulation of EVs originate from tumor cells, but these EVs are outnumbered by non-EV particles and EVs from other origin. In the Dutch NWO Perspectief Cancer-ID program, we developed and evaluated detection and characterization techniques to distinguish EVs from non-EV particles and other EVs. Despite low signal amplitudes, we identified characteristics of these small tdEVs that may enable the enumeration of small tdEVs and extract relevant information. The insights obtained from Cancer-ID can help to explore the full potential of tdEVs in the clinic.

18. Covariant Quantum Mechanics and Quantum Spacetime

Abstract: We present in the article the formulation of a version of Lorentz covariant quantum mechanics based on a group theoretical construction from a Heisenberg-Weyl symmetry with position and momentum operators transforming as Minkowski four-vectors under the Lorentz symmetry. The basic representation is identified as a coherent state representation, essentially an irreducible component of the regular representation, with the matching representation of an extension of the group $C^*$-algebra giving the algebra of observables. The key feature of the formulation is that it is not unitary but pseudo-unitary, exactly in the same sense as the Minkowski spacetime representation. Explicit wavefunction description is given without any restriction of the variable domains, yet with a finite integral inner product. The associated covariant harmonic oscillator Fock state basis has all the standard properties in exact analog to those of a harmonic oscillator with Euclidean position and momentum operators of any `dimension'. Galilean limit of the Lorentz symmetry and the classical limit of the Lorentz covariant framework are retrieved rigorously through appropriate symmetry contractions of the algebra and its representation, including the dynamics described through the symmetry of the phase space, given both in terms of real/complex number coordinates and noncommutative operator coordinates. 19. Analysis on Complete Set of Fock States with Explicit Wavefunctions for the Covariant Harmonic Oscillator Problem

Abstract: The earlier treatments of Lorentz covariant harmonic oscillator have brought to light various difficulties, such as reconciling Lorentz symmetry with the full Fock space, and divergence issues with their functional representations. We present here a full solution avoiding those problems. The complete set of Fock states is obtained, together with the corresponding explicit wavefunction and their inner product integrals free from any divergence problem and the Lorentz symmetry fully maintained without additional constraints imposed. By a simple choice of the pseudo-unitary representation of the underlying symmetry group, motivated from the perspective of the Minkowski spacetime as a representation for the Lorentz group, we obtain the natural non-unitary Fock space picture commonly considered though not formulated and presented in the careful details given here. From a direct derivation of the appropriate basis state wavefunctions of the finite-dimensional irreducible representations of the Lorentz symmetry, the relation between the latter and the Fock state wavefunctions is also explicitly shown. Moreover, the full picture including the states with non-positive norm may give consistent physics picture as a version of Lorentz covariant quantum mechanics. Probability interpretation for the usual von Neumann measurements is not a problem as all wavefunctions restricted to a definite value for the `time' variable are just like those of the usual time independent quantum mechanics. Abstract: Cardiovascular diseases (CVDs), principally ischemic heart disease (IHD) and stroke, are the leading cause of global mortality and a major contributor to disability. This paper reviews the magnitude of total CVD burden, including 13 underlying causes of cardiovascular death and 9 related risk factors, using estimates from the Global Burden of Disease (GBD) Study 2019. GBD, an ongoing multinational collaboration to provide comparable and consistent estimates of population health over time, used all available population-level data sources on incidence, prevalence, case fatality, mortality, and health risks to produce estimates for 204 countries and territories from 1990 to 2019.Prevalent cases of total CVD nearly doubled from 271 million (95% uncertainty interval [UI]: 257 to 285 million) in 1990 to 523 million (95% UI: 497 to 550 million) in 2019, and the number of CVD deaths steadily increased from 12.1 million (95% UI:11.4 to 12.6 million) in 1990, reaching 18.6 million (95% UI: 17.1 to 19.7 million) in 2019. The global trends for disability-adjusted life years (DALYs) and years of life lost also increased significantly, and years lived with disability doubled from 17.7 million (95% UI: 12.9 to 22.5 million) to 34.4 million (95% UI:24.9 to 43.6 million) over that period. The total number of DALYs due to IHD has risen steadily since 1990, reaching 182 million (95% UI: 170 to 194 million) DALYs, 9.14 million (95% UI: 8.40 to 9.74 million) deaths in the year 2019, and 197 million (95% UI: 178 to 220 million) prevalent cases of IHD in 2019. The total number of DALYs due to stroke has risen steadily since 1990, reaching 143 million (95% UI: 133 to 153 million) DALYs, 6.55 million (95% UI: 6.00 to 7.02 million) deaths in the year 2019, and 101 million (95% UI: 93.2 to 111 million) prevalent cases of stroke in 2019.Cardiovascular diseases remain the leading cause of disease burden in the world. 22. Group Theoretical Approach to Pseudo-Hermitian Quantum Mechanics with Lorentz Covariance and $c \rightarrow \infty $ Limit

Abstract: We present in the article the formulation of a version of Lorentz covariant quantum mechanics based on a group theoretical construction from a Heisenberg-Weyl symmetry with position and momentum operators transforming as Minkowski four-vectors under the Lorentz symmetry. The basic representation is identified as a coherent state representation, essentially an irreducible component of the regular representation, with the matching representation of an extension of the group $C^*$-algebra giving the algebra of observables. The key feature of the formulation is that it is not unitary but pseudo-unitary, exactly in the same sense as the Minkowski spacetime representation. The language of pseudo-Hermitian quantum mechanics is adopted for a clear illustration of the aspect, with a metric operator obtained as really the manifestation of the Minkowski metric on the space of the state vectors. Explicit wavefunction description is given without any restriction of the variable domains, yet with a finite integral inner product. The associated covariant harmonic oscillator Fock state basis has all the standard properties in exact analog to those of a harmonic oscillator with Euclidean position and momentum operators of any `dimension'. Abstract: Background: European population cohorts continue to expand our understanding of chronic airways disease and inter-study collaboration may help address the inevitable limitations of study size, duration, era and geography. Towards this aim, CADSET has collated data from ten major general population European cohorts: Asklepios; Copenhagen City Heart Study; Copenhagen General Population Study; ECRHS; HUNT; LEAD; Lifelines, OLIN, Rotterdam Study and WSAS. We included males and females aged 20 to 95 years with baseline demographic and spirometry data. Results: Data from 262,829 individuals (44% male) from multiple European countries provided good coverage across all adult ages (Fig.1A). Recruitment occurred in every year from 1976 through 2020. 23% were current-smokers and 42% were never-smokers, a pattern varying with advancing age (Fig.1B). The prevalence of airflow limitation varied according to whether lower limit of normal (LLN) or <0.70 thresholds were applied, increasing with age if the latter was used (Fig.1C). Interpretation: These results fit with previous reports, however the size, geographical reach and span of recruitment provided by this collaboration provides a unique opportunity to explore chronic airways disease development. Together, we are now pursuing research questions previously beyond the scope of individual cohort studies.

Abstract: Hypoxia-induced carbonic anhydrase IX (CAIX) expression is a prognostic marker in solid tumors. In recent years many radiotracers have been developed, but a fair comparison of these compounds is not possible because of the diversity in tumor models and other experimental parameters. In this study we performed a direct in vivo comparison of three promising CAIX targeting radiotracers in xenografted head and neck cancer models. The biodistribution of [In-111]In-DOTA-ZCAIX:2 was directly compared with [In-111]In-DTPA-G250-F(ab')(2) and [In-111] In-DTPA-G250 in female BALB/C nu/nu mice bearing two HNSCC xenografts with different levels of CAIX expression. In vivo biodistribution was quantified by means of microSPECT/CT scans and ex vivo biodistribution was determined with the use of gamma-counter. Tumors were snap frozen and sections were stained for CAIX expression, vessels, hypoxia (pimonidazole) and tumor blood perfusion. Tracer uptake was significantly higher in SSCNij153 tumors compared to SCCNij185 tumors for [In-111]In-DOTA-HE3-ZCAIX:2: 0.32 +/- 0.03 versus 0.18 +/- 0.01%ID/g,(p = 0.003) 4 h p.i., for [In-111]In-DTPA-girentuximab-F(ab')(2): 3.0 +/- 0.5%ID/g and 1.2 +/- 0.1%ID/g (p = 0.03), 24 h p.i. and for [In-111]In-DTPA-girentuximab: 30 +/- 2.1%ID/g and 7.0 +/- 1.0%ID/g (p = 0.0002) 72 h p.i. SPECT imaging with both [In-111]In-DTPA-girentuximab-F(ab') 2 and [111In]In-DTPA-girentuximab showed a clear difference in tracer distribution between the two tumor models. The whole IgG, i.e. [In-111]In-DTPA-girentuximab, showed the highest tumor-to-muscle ratio. We showed that different CAIX-targeting radiotracers can discriminate a low CAIX-expressing tumor from a high CAIX-expressing head and neck cancer xenografts model. In these hypoxic head and neck xenograft models [In-111]In-DTPA-girentuximab showed the most promising results.

Abstract: Plasmonic sensitization of semiconductors is an attractive approach to increase light-induced photocatalytic performance; one method is to use plasmonic nanostructures in core@shell geometry. The occurrence and mechanism of synergetic effects in photocatalysis of such geometries are under intense debate and proposed to occur either through light-induced charge transfer (CT) or through thermal effects. This study focuses on the relation between the dimensions of Ag@CeO2 nanocubes, the wavelength-dependent efficiency, and the mechanism of light-induced direct CT. A 4-mercaptobenzoic acid (4-MBA) linker between core and shell acts as a Raman probe for CT. For all Ag@CeO2 nanocubes, CT increases with decreasing excitation wavelength, with notable increase at and below 514 nm. This is fully explainable by CT from silver to the 4-MBA LUMO, with the increase for excitation wavelengths that exceed the Ag/4-MBA LUMO gap of 2.28 eV (543 nm). A second general trend observed is an increase in CT yield with ceria shell thickness, which is assigned to relaxation of the excited electron further into the ceria conduction band, potentially producing defects.

Abstract: We discuss the notion about physical quantities as having values represented by real numbers, and its limiting to describe nature to be understood in relation to our appreciation that the quantum theory is a better theory of natural phenomena than its classical analog. Getting from the algebra of physical observables to their values on a fixed state is, at least for classical physics, really a homomorphic map from the algebra into the real number algebra. The limitation of the latter to represent the values of quantum observables with noncommutating algebraic relation is obvious. Abstract: We illustrate an isomorphic representation of the observable algebra for quantum mechanics in terms of the functions on the projective Hilbert space, and its Hilbert space analog, with a noncommutative product in terms of explicit coordinates and discuss the physical and dynamical picture. The isomorphism is then used as a base for the translation of the differential symplectic geometry of the infinite dimensional manifolds onto the observable algebra as a noncommutative geometry. Hence, we obtain the latter from the physical theory itself. We have essentially an extended formalism of the Schrodinger versus Heisenberg picture which we describe mathematically as like a coordinate map from the phase space, for which we have presented argument to be seen as the quantum model of the physical space, to the noncommutative geometry coordinated by the six position and momentum operators. The observable algebra is taken essentially as an algebra of formal functions on the latter operators. Abstract: Polyaromatic hydrocarbons (PAH) are common pollutants of water ecosystems originating from incineration processes and contamination with mineral oil. Water solubility of PAHs is generally low; for toxicity tests with aquatic organisms, they are therefore usually dissolved in organic solvents. Here we examined the effects of a typical model PAH, phenanthrene, and a solvent, acetone, on amphipods as relevant aquatic invertebrate models. Two of these species, Eulimnogammarus verrucosus and Eulimnogammarus cyaneus, are common endemics of the oligotrophic and pristine Lake Baikal, while one, Gammarus lacustris, is widespread throughout the Holarctic and inhabits smaller and more eutrophic water bodies in the Baikal area. Neither solvent nor phenanthrene caused mortality at the applied concentrations, but both substances affected gene expression in all species. Differential gene expression was more profound in the species from Lake Baikal than in the Holarctic species. Moreover, in one of the Baikal species, E. cyaneus, we found that many known components of the cellular xenobiotic detoxification system reacted to the treatments. Finally, we detected a negative relationship between changes in transcript abundances in response to the solvent and phenanthrene. This mixture effect, weaker than the impact by a single mixture component, needs further exploration.

Abstract: Background: Lake Baikal is one of the oldest freshwater lakes and has constituted a stable environment for millions of years, in stark contrast to small, transient bodies of water in its immediate vicinity. A highly diverse endemic endemic amphipod fauna is found in one, but not the other habitat. We ask here whether differences in stress response can explain the immiscibility barrier between Lake Baikal and non-Baikal faunas. To this end, we conducted exposure experiments to increased temperature and the toxic heavy metal cadmium as stressors.Results: Here we obtained high-quality de novo transcriptome assemblies, covering mutiple conditions, of three amphipod s pecies, and compared their transcriptomic stress responses. Two of these species, Eulimnogammarus verrucosus and E. cyaneus, are endemic to Lake Baikal, while the Holarctic Gammarus lacustris is a potential invader.Conclusions: Both Baikal species possess intact stress response systems and respond to elevated temperature with relatively similar changes in their expression profiles. G. lacustris reacts less strongly to the same stressors, possibly because its transcriptome is already perturbed by acclimation conditions.

Abstract: In this article, we utilize the insights gleaned from our recent formulation of space(-time), as well as dynamical picture of quantum mechanics and its classical approximation, from the relativity symmetry perspective in order to push further into the realm of the proposed fundamental relativity symmetry $SO(2,4)$ of our quantum relativity project. We explicitly trace how the diverse actors in this story change through various contraction limits, paying careful attention to the relevant physical units, in order to place all known relativity theories -- quantum and classical -- within a single framework. More specifically, we explore both of the possible contractions of $SO(2,4)$ and its coset spaces in order to determine how best to recover the lower-level theories. These include both new models and all familiar theories, as well as quantum and classical dynamics with and without Einsteinian special relativity. Along the way, we also find connections with covariant quantum mechanics. The emphasis of this article rests on the ability of this language to not only encompass all known physical theories, but to also provide a path for extensions. Abstract: Objective Cardiac disease is the leading cause of indirect maternal mortality. The aim of this study was to analyse to what extent socioeconomic factors influence the outcome of pregnancy in women with heart disease. Methods The Registry of Pregnancy and Cardiac disease is a global prospective registry. For this analysis, countries that enrolled ≥10 patients were included. A combined cardiac endpoint included maternal cardiac death, arrhythmia requiring treatment, heart failure, thromboembolic event, aortic dissection, endocarditis, acute coronary syndrome, hospitalisation for cardiac reason or intervention. Associations between patient characteristics, country characteristics (income inequality expressed as Gini coefficient, health expenditure, schooling, gross domestic product, birth rate and hospital beds) and cardiac endpoints were checked in a three-level model (patient-centre-country). Results A total of 30 countries enrolled 2924 patients from 89 centres. At least one endpoint occurred in 645 women (22.1%). Maternal age, New York Heart Association classification and modified WHO risk classification were associated with the combined endpoint and explained 37% of variance in outcome. Gini coefficient and country-specific birth rate explained an additional 4%. There were large differences between the individual countries, but the need for multilevel modelling to account for these differences disappeared after adjustment for patient characteristics, Gini and country-specific birth rate. Conclusion While there are definite interregional differences in pregnancy outcome in women with cardiac disease, these differences seem to be mainly driven by individual patient characteristics. Adjustment for country characteristics refined the results to a limited extent, but maternal condition seems to be the main determinant of outcome.

44. Using Test Results to Improve Instruction.

Abstract: A model for summarizing test scores and using them to modify instructional programs is presented. The proposed model consists of two types of summaries of the data gathered through standardized tests. The first summary contains individual and single class results. Information in a "Class Item Response Record" chart provides individual student responses for each item on the test, class percent correct for each item, and national percent correct. This chart enables teachers to determine the skills that individual students have or have not attained, the concepts learned by a majority of students in the class, how the class differs from the national norms, and the strengths and weaknesses of the instructional program. The second summary of data contains a response record chart for all students taking the test in a given school. It contains identifying information pertaining to school, grade level and test, item numbers, item description, and the percent of students responding to each choice for each question. Percent of correct response for the city and national testing population are also included. Suggestions for using the charts and activities for principals to use test results to improve instruction are presented. (PN)

Abstract: Objective: Schizophrenia may be the result of a failure of the normal lateralization process of the brain. However, whole-brain asymmetry has not been assessed up to date. Here we propose a novel measure of global brain asymmetry based on the Dice coefficient in order to quantify similarity between brain hemispheres. Method: Global gray and white matter asymmetry was calculated from high-resolution T1 structural images acquired from 24 patients with schizophrenia and 26 healthy controls, age- and sex-matched. Some of the analyses were replicated in a much larger sample (n = 759) obtained from open-access online databases. Results: Patients with schizophrenia had more global gray matter asymmetry than controls. Additionally, increased gray matter asymmetry was associated with avolition, whereas the inverse relationship was found for anxiety. These analyses were replicated in a larger sample and confirmed previous results. Conclusion: Our findings suggest that global gray matter asymmetry is related to the concept of developmental stability and is a useful indicator of perturbations during neurodevelopment.

Abstract: The DUct aCoustic Test rig with Rectangular cross section (DUCT-R) is designed at the German Aerospace Center (DLR), Engine Acoustics Department, to test commercial aircraft engine nacelle liner samples. An air flow with speed up to Mach 0.3 can be achieved in the liner section. An overall number of 106 microphone positions are spread over the rig, making it capable of investigating the scattering of the fundamental and higher order acoustic modes above the liner samples. In this paper, the DUCT-R facility is described. The measurement setups currently being used at DLR are introduced. In addition, a methodology for decomposing measured sound-field data with fundamental and higher order modes is described. The methodology is applied to data sets obtained from a “zero measurement” (a measurement without liner samples) with and without mean flows for validation purposes. 48. Analysis on a Nambu--Jona-Lasinio Model of Dynamical Supersymmetry Breaking

Abstract: This is a report on our newly proposed model of dynamical supersymmetry breaking with some details of the analysis involved. The model in the simplest version has only a chiral superfield (multiplet), with a strong four-superfield interaction in the K\"ahler potential that induces a real two-superfield composite with vacuum condensate. The latter has supersymmetry breaking parts, which we show to bear nontrivial solution following basically a standard nonperturbative analysis for a Nambu--Jona-Lasinio type model on a superfield setting. The real composite superfield has a spin one component but is otherwise quite unconventional. We discuss also the parallel analysis for the effective theory with the composite. Plausible vacuum solutions are illustrated and analyzed. The supersymmetry breaking solutions have generated soft mass(es) for the scalar avoiding the vanishing supertrace condition for the squared-masses of the superfield components. We also present some analysis of the resulted low energy effective theory with components of the composite become dynamical. The determinant of the fermionic modes is shown to be zero illustrating the presence of the expected Goldstino. The model gives the possibility of constructing a supersymmetric standard model with all (super)symmetry breaking masses generated dynamically and directly without the necessity of complicated hidden or mediating sectors., Comment: 40 pages, 17 figures

49. Anti-tumor effects of ketogenic diets in mice: A meta-analysis

Abstract: Background: Currently ketogenic diets (KDs) are hyped as an anti-tumor intervention aimed at exploiting the metabolic abnormalities of cancer cells. However, while data in humans is sparse, translation of murine tumor models to the clinic is further hampered by small sample sizes, heterogeneous settings and mixed results concerning tumor growth retardation. The aim was therefore to synthesize the evidence for a growth inhibiting effect of KDs when used as a monotherapy in mice. Methods: We conducted a Bayesian random effects meta-analysis on all studies assessing the survival (defined as the time to reach a pre-defined endpoint such as tumor volume) of mice on an unrestricted KD compared to a high carbohydrate standard diet (SD). For 12 studies meeting the inclusion criteria either a mean survival time ratio (MR) or hazard ratio (HR) between the KD and SD groups could be obtained. The posterior estimates for the MR and HR averaged over four priors on the between-study heterogeneity τ2 were MR = 0.85 (95% highest posterior density interval (HPDI) = [0.73, 0.97]) and HR = 0.55 (95% HPDI = [0.26, 0.87]), indicating a significant overall benefit of the KD in terms of prolonged mean survival times and reduced hazard rate. All studies that used a brain tumor model also chose a late starting point for the KD (at least one day after tumor initiation) which accounted for 26% of the heterogeneity. In this subgroup the KD was less effective (MR = 0.89, 95% HPDI = [0.76, 1.04]). Conclusions: There was an overall tumor growth delaying effect of unrestricted KDs in mice. Future experiments should aim at differentiating the effects of KD timing versus tumor location, since external evidence is currently consistent with an influence of both of these factors.
