2. Factors for Good Text Legibility : Eye-tracking in Virtual Reality

- Abstract
Living with a hearing impairment can have a large impact on a person’slife. There already exists many different aids to help in their life, butas technology advances new solutions can be created to further improvethe life quality for everyone. Two technologies that have advanced andbecome more affordable are Virtual Reality (VR) and Augmented Reality(AR). A potential aid for those with a hearing impairment could bea system where speech is converted to text and presented in AR to theuser. Such a system must have an easily read and legible text. In this master thesis legibility and user perception are studied for differenttext presentation in VR. The VR enables a more controlled environmentthan AR. Reading speed, subjective scoring and eye-movementdata are used to analyze the presentations. Lastly, some design recommendationsbased on the findings are presented. The result showed that the legibility was affected by many factors.Middle-layers (layer between the fore- and background) improved thelegibility, especially over complex backgrounds. The size of the textalso affected legibility where the larger text performed the worst. Theoptimal number of lines of text seems to be two. There were variationsbetween the preferred presentations, indicating that a future systemmight seek to accommodate this by some level of customization.

3. Increased Beta Activity Links to Impaired Emotional Control in ADHD Adults With High IQ

Objective: The present study investigated the neuropathology of everyday-life executive function (EF) deficits in adults with ADHD with high IQ. Method: Forty adults with ADHD with an IQ ≥ 120 and 40 controls were recruited. Ecological EFs were measured, and eyes-closed Electroencephalograph (EEG) signals were recorded during a resting-state condition; EEG power and correlations with impaired EFs were analyzed. Results: Compared with controls, the ADHD group showed higher scores on all clusters of EF. The ADHD group showed globally increased theta, globally decreased alpha, and increased central beta activity. In the ADHD group, central beta power was significantly related to emotional control ratings, while no such correlation was evident in the control group. Conclusion: The results suggest that resting-state beta activity might be involved in the neuropathology of emotional control in adults with ADHD with high IQ.

4. Der stumme Schmerz : Ein Literaturreview zu Schmerz-Assessmentinstrumenten für geistig behinderte Kinder im postoperativen Setting

Ausgangslage: Geistig behinderte Kinder leiden häufiger an Schmerzen als kognitiv intakte Kinder. Die Schmerzen von geistig behinderten Kindern sind schwer zu erfassen, da sie nicht in der Lage sind adäquate Schmerz-Äusserungen zumachen. IhreSchmerzen müssen daher anhand ihres Verhaltens analysiert werden. Fragestellung und Ziel: Diese Arbeit befasst sich mit drei Assessmentinstrumenten für behinderte Kinder im postoperativen Setting (NCCPC-PV, (r)FLACC, PPP). Die Fragestellung der Arbeit lautet: „Welche Schmerz-Assessmentinstrumente sind für Kinder im postoperativen Setting im Alter von 4 bis 18 Jahren mit einer schweren geistigen Behinderung geeignet?“. Die Arbeit hat zum Ziel eine Empfehlung für die Praxis abgeben zu können. Methode: Anhand einer Literaturrecherche wurden die drei häufigsten Assessmentinstrumente eruiert und es zeigten sich pro Assessmentinstrument zwei geeignete evidenzbasierte Studien. Relevante Ergebnisse: Alle drei Assessmentinstrumente sind valide und reliabel. Die Assessmentinstrumente haben einen ähnlichen Aufbau. In vielen Studien wirdeine Schwierigkeit bezüglich des Assessments anhand der Extremitäten und ein gute Übereinstimmung bezüglich des Assessments anhand des Gesichts festgestellt. Einige Studien zeigen auf, dass viele behinderte Kinder individuelle Schmerz-Verhaltensmuster aufweisen. Schlussfolgerung: Alle analysierten Assessmentinstrumentesind für das postoperative Setting geeignet. Für die Praxisempfiehlt sich ein Assessmentinstrument, welches über eine Standardisierung verfügt undindividuelle Schmerz-Verhaltensmuster integrieren kann.

5. VirtuNav: A virtual reality indoor navigation simulator with haptic and audio feedback for the visually impaired

VirtuNav provides a haptic-enabled Virtual Reality (VR) environment that facilitates persons with visual impairment to explore a 3D computerized model of a real-life indoor location, such as a classroom or hospital. For administrative purposes, the screen displays a 2D overhead view of the map to monitor user progress and location as well as the 3D reconstructed environment. The system offers two unique interfaces: a freeroam interface where the user can navigate and interact with the environment, and an edit mode where the administrator can manage test users, maps and retrieve test data. VirtuNav is a practical application offering several unique features including map design, semi-automatic 3D map reconstruction and object classification from 2D map data. Visual and haptic rendering of real-time 3D map navigation are provided as well as automated administrative functions including shortest path calculation, actual path traversal, and assessment of performance indicators including time taken for exploration and collision data. VirtuNav is a research tool for investigation of user familiarity developed after repeated exposure to the indoor location, to determine the extent to which haptic and/or sound cues improve a visually impaired user's ability to navigate a room or building with or without occlusion. System testing reveals that spatial awareness and memory mapping improve with user iterations within the VirtuNav environment. This application aims to promote confidence by independent exploration, for the sight impaired.

The reading process is one of the cognitive abilities that enable humans to unravel the message provided by the authors, through graphic signs in a given text. Apprehend those signs represent an important intellectual act, which is initiated by the receiving through exteroceptors; such as nerve endings respond to stimuli from abroad. The sensations are perceived by the corpuscles, mainly those of Meissner receptors which are conjunctive tissue capsule and distributed between the different layers of the skin. 7. Impaired perception of facial motion in autism spectrum disorder

Facial motion is a special type of biological motion that transmits cues for socio-emotional communication and enables the discrimination of properties such as gender and identity. We used animated average faces to examine the ability of adults with autism spectrum disorders (ASD) to perceive facial motion. Participants completed increasingly difficult tasks involving the discrimination of (1) sequences of facial motion, (2) the identity of individuals based on their facial motion and (3) the gender of individuals. Stimuli were presented in both upright and upside-down orientations to test for the difference in inversion effects often found when comparing ASD with controls in face perception. The ASD group's performance was impaired relative to the control group in all three tasks and unlike the control group, the individuals with ASD failed to show an inversion effect. These results point to a deficit in facial biological motion processing in people with autism, which we suggest is linked to deficits in lower level motion processing we have previously reported.

8. Pragmatic language development in language impaired and typically developing children: incorrect answers in context

- Abstract
This study focussed on young children's incorrect answers to pragmatically demanding questions. Children with specific language impairment (SLI), including a subgroup with pragmatic language difficulties (PLD) and typically developing children answered questions targeting implicatures, based on a storybook and short verbal scenarios. Ninety-seven children participated in this study: 30 children with SLI of whom 12 had PLD, 32 typically developing children aged 5-6 years and 35 aged 7-11 years. The incorrect answers produced by the children with SLI were similar in their use of context to those of the 5-6 year old, suggesting developmental delay. The children with PLD produced significantly more irrelevant answers than both the language impaired children without PLD and the typically developing groups and had most difficulty when the context was presented solely verbally. Results are discussed in relation to a cognitive theory of communication and the clinical implications.

9. Diet quality is prospectively associated with incident impaired fasting glucose in older adults

- Abstract
Aims Dietary modifications may play an important role in the prevention of diabetes. We aimed to assess the temporal association between diet quality and both impaired fasting glucose and Type 2 diabetes among older adults. Methods A total of 2564 participants aged 49+ years at baseline were examined between 1992 and 1994 and 2002–2004 and had their fasting blood glucose measured. Dietary data were collected using a semi-quantitative food frequency questionnaire. A modified version of the Healthy Eating Index for Australians was developed to determine Total Diet Score. Incident diabetes (or impaired fasting glucose) was defined in participants at risk who were newly diagnosed by a physician during the follow-up or found to have a fasting blood glucose level ≥ 7.0 mmol/l (or 6.1–6.9 mmol/l). Results After adjusting for age, sex, current smoking, body mass index, hypertension and serum triglycerides, comparing highest with lowest tertile of total diet score, a significant 75% decrease in risk of incident impaired fasting glucose was observed in men (Ptrend = 0.02). Also, in men, each two-point increase in Total Diet Score was associated with a 52% reduction in the 10-year incidence of impaired fasting glucose, (OR 0.48, 95% CI 0.33–0.69). No significant associations were observed among women or with the 10-year incidence of diabetes. Conclusions Greater compliance with published dietary guidelines (better diet quality) was associated with a reduced risk of pre-diabetes in men, but not women.

10. Pedestrian lane detection for the visually impaired

- Abstract
Traveling safely in outdoor environments is one of the most challenging activities for vision-disabled people. To improve the mobility of these people, an assistive navigation system is necessary. This paper is a step towards developing this system. We propose a robust method to detect the pedestrian marked lane at traffic junctions. The proposed method involves three stages: patches of interest (POI) extraction, lane marker detection and lane detection. The POI extraction is first performed to detect image patches located on the lane marker boundaries using template matching. Potential lane markers are then formed by using shape and intensity information. Finally, the walkable lane region is determined by verifying pairs of detected potential makers. A probabilistic framework using multiple geometric cues is proposed for the verification step. We create a new dataset collected from diverse environments and evaluate the proposed method on this dataset. The experimental results under challenging conditions have illustrated the efficiency and robustness of the proposed method compared to conventional techniques. © 2012 IEEE.

11. Assessing clinical trial informed consent comprehension in non-cognitively-impaired adults: a systematic review of instruments

This systematic review identifies and critically evaluates instruments that have been developed to measure clinical trial informed consent comprehension in non-cognitively-impaired adults. Literature searches were carried out on Medline (Ovid), PsycInfo, CINHAL, ERIC, ScienceDirect, and Cochrane Library for English language articles published between January 1980 and September 2008. Instruments were excluded if they focused on consent onto paediatric trials, the construct under study was primarily capacity or competency, or the instrument was developed specifically for psychiatric or cognitively-impaired populations. Instruments selected for review were evaluated against the following criteria: (1) method of item generation; (2) type and format of test items; (3) administration and interpretation of test results; and (4) psychometric properties. Three instruments met our defined inclusion criteria: the Deaconess Informed Consent Comprehension Test (DICCT), the Quality of Informed Consent (QuIC) questionnaire and the Brief Informed Consent Protocol (BICEP). Each instrument varied in terms of content measured. Significantly, these are the first standardized instruments developed to assess comprehension in non-cognitively-impaired adults. Yet, each instrument had its own set of limitations such as the lack of generalizability and the absence of details pertaining to how test results should be used to guide clinical decision-making. Standardized clinical trial informed consent comprehension assessments have been developed to identify gaps in research participants' understanding and ensure that respect for patient autonomy is satisfied.

A care planning guidance tool (CPGT) was developed as part of a local Palliative Care Planning Project. The Guidelines to a Palliative Approach in Residential Aged Care was utilized to provide the underpinning knowledge framework. Prior to piloting implementation, two preliminary steps needed to be undertaken: (i) a pre-implementation documentation audit across the three participating facilities, and (ii) education to all staff about the CPGT and its use.

Issue addressed: To evaluate the effectiveness of a brief intervention using a pedometer and step-recording diary on promoting physical activity in people with type 2 diabetes or impaired glucose tolerance (IGT). Methods: People with type 2 diabetes or IGT who attended the Illawarra Diabetes Service were invited to participate. Participants in the intervention group received a pedometer and a diary to record their daily steps for a two-week period. Both the intervention and comparison group received advice on physical activity. Physical activity levels were measured using the Active Australia Survey at baseline, and at two and 20 weeks. Results: A total of 226 participants were recruited. At two-week follow-up the mean self-reported minutes of walking was significantly higher in the intervention group than the comparison group (223 minutes versus 164 minutes; p=0.01), as was the percentage of intervention participants achieving recommended levels of moderate-intensity physical activity (63.5% versus 41.8%, p=0.02) and the percentage of intervention participants achieving adequate levels of total physical activity (68.9% versus 48.0%, p=0.04). There were no differences between study groups for any physical activity measure at 20-week follow-up. Conclusions: A pedometer and a step-recording diary were useful tools to promote short-term increase in physical activity in people diagnosed with type 2 diabetes or IGT. Future studies need to examine whether a longer intervention, individualised physical activity counselling and support for achieving step goals could result in increasing physical activity over the long term.

The main idea in this rehabilitation program, that we wont to present, is it imitate the development of the usual children's population. Since 1978. this program was apply during the rehabilitation process in ORL Clinic at Department for Audiology rehabilitation. This program have three phases adaptation, development of the communication and speech development. Authors wants to introduce concept that based on the verbal method, by conception very similar to John Tracy Clinic and Galoudet College in USA (individual treatment, parent as active member of the rehabilitation teem...) and some elements of Piaget's Theory of Senzorimotori (Cognitive) Development. The results are related on 600 hearing impaired children's, at 7 year old rehabilitated by this program. We are going to show a number of children which they start education in regular schools (which we are observed as criteria for successful results at this age) with normal hearing population., Osnovna ideja na kojoj je bazirana rana rehabilitacija dece oštećenog sluha prema konceptu koji želimo da prikažemo u ovom radu, je oponašanje razvoja dece opšte populacije. Proces rahabilitacije prema konceptu koji prikazujemo i koji se koristi od 1979. god na ORL Klinici, odeljenje za Audiološku rehabilitaciju, (tj. u modifikovanoj formi, od 1967. g i formiranja Centra za sluh i govor Sveti Sava, i prvih projekata na temu karakteristika i aspekata gluvoće i nagluvosti). Prema ovom programu rana rehabilitacija odvija se u tri faze: adaptacija, razvoj komunikacije i razvoj govora i jezika. Cilj ovog rada je da podsetimo i ukažemo na inovacije u teorijskom i programskom konceptu sistema rehabilitacije, verbalnom metodom, koja se konceptualno oslanja na način rada Džon Trejsi klinike i Galodet Koledža (individualan rad, roditelj aktivan učesnik, itd), a zasnovana je na elementima Pijažeovoe teorije senzomotornog razvoja. Osim toga, ukazaćemo na program, ciljeve i trajanje svake faze u rehabilitaciji i na promene do kojih je dovelo pomeranje uzrasta detekcije i dijagnostike patologije sluha. Rezultati se odnose na oko 300 do sada rehabilitovane dece oštećenog sluha po ovom programu, na uzrastu do sedme godine starosti. Osim toga biće prikazani i podaci o učestalosti upisa ove dece u redovnu školu, što se posmatra kao stepen uspešnosti rehabilitacije na ovom uzrastu.

We agree with Ikeda et al that the absence of systolic hypertension may provide some discriminatory power towards exclusion of brain lesions, be they ischaemic, haemorrhagic, or space occupying in nature.1 However, we disagree with them that neurological examination of patients with impaired consciousness is often a waste of time and resources and can delay diagnosis.

18. Normal and language-impaired children's use of reference: Syntactic versus pragmatic processing

The present study investigates children's syntactic and pragmatic processing when specifying referents presented in short video clips. Within Relevance theory, the assumption of 'optimal relevance' implies that utterances are intended to involve the least processing effort on the part of the listener. In the present context, lexically specified NPs are assumed to be more in line with optimal relevance than pronouns. Subjects were 48 normally developing children aged 3;4-8;10 and 30 SLI children aged 5;1-8;9, divided into a low and a normal MLU group. Children's responses were coded according to levels of pragmatic processing and syntactic positions. Normally developing children's referent specifications were found to be increasingly relevant with increasing age. Differences between SLI and normal children were only found for the low MLU group with SLI who used fewer pronouns than the younger children, thereby showing that syntactic limitations alone cannot account for children's specification of referents.

19. The use of context in pragmatic comprehension by specifically language-impaired and control children

The purpose of this paper is to explore how a theory of pragmatic comprehension, relevance theory (Sperber and Wilson 1995), can be used to examine contextual processing in pragmatic comprehension. It will present data from a task that explores pragmatic comprehension skills, within this theoretical framework, of 17 SLI children (5; 11-10; 10), 11 older controls (7;11-9;11) and 30 younger controls (4;0-5;11). The children were asked questions based on a storybook, that required different degrees of contextual processing, as defined by the theory. Reference assignment questions required the children to provide a referent that is verbally/pictorially given, hence the least demanding in terms of contextual processing. Questions that require the resolution of semantic underdeterminations via the process of enrichment are contextually more demanding than reference-assignment question. Questions requiring the resolution of an implicature require the greatest contextual processing, hence these are deemed pragmatically the most demanding questions. A study by Ryder and Leinonen (i.p.) of three-to five-year-old normally developing children has shown that the theory and the characterization of questions in terms of pragmatic complexity have developmental validity. This finding is further supported by the current study. It was found that the pattern of performance across the three question types was very similar for the SLI group and the younger controls and that this reflected the contextual complexity of the question. The older controls had the greatest number of correct answers to all question types when compared to the younger controls and the SLI group, and this difference was particularly evident in relation to the pragmatically more demanding question types (enrichment and implicature questions). When the SLI group was divided into "younger" (5;11-7;11) and "older" (8;0-10;0) groups, it became evident that the older SLI children performed worse than their chronological

20. Comprehension of inferential meaning in language-impaired and language normal children

Three groups of language-normal (LN) 6, 8 and 16/17 year olds, and a group of language-impaired (LI) children were given a task answering questions about pictures that involved inferential meaning. A developmental progression in the types of responses given is seen, with the LI children performing like the youngest LN children. A similar progression is seen in the ability to justify the answers given to inferential questions with the young adult group giving the fewest justifications that were problematical in some way. Larger numbers of problematical justifications in the LI group can be related to some extent to non-pragmatic aspects of their impairment, but overall this group also gave more pragmatically irrelevant responses when asked to justify answers given. It was not possible to identify any major differences between subgroups of children within the LI group, identified as pragmatically impaired (PI) and non pragmatically impaired (NPI), in terms of either answers given to inferential questions or in terms of problematical justifications for these answers. Possible reasons for these findings are discussed.

- Abstract
Two groups of children, a pragmatically impaired (PI) group and a group of language-normal (LN) age-matched peers, were compared by use of a referential communication task. Experimenter and child both played the roles of listener and instructor during the task and, in addition, the experimenter sometimes failed to give adequate information when in the role of instructor. Lexical content and structural complexity were controlled, and it was hypothesised that difficulties for the PI group would arise when in the role of instructor, as a result of failing to specify necessary information in order for the experimenter to respond appropriately. In fact, the main difference between the two groups arose when the experimenter failed to give adequate information to the child; the LN children were quick to realise this and to request clarification, whereas the PI children requested clarification to a lesser extent and appeared less aware of the need to do so. Possible explanations for this pattern of results are explored.

33. Impaired glucose tolerance in Pima Indians

More than 50% of Pima Indians develop NIDDM. This disorder is preceded by impaired glucose tolerance (IGT) and we tested the hypothesis that the elevated glucose levels in IGT must be due to reduced ??-cell function. We first determined the plasma glucose/plasma insulin and plasma insulin/insulin resistance relationships in individuals with NGT, relationships which by definition must be normal, and determined if these relationships were intact in individuals with IGT. We also compared Pimas and Caucasians with NGT or IGT. Subjects were assessed with an OGTT, an IVGTT, underwater weighing (for body composition), and a euglycaemic clamp. The results showed that insulin concentrations in Pimas with IGT were not lower than the levels predicted by the relationships found in subjects with NGT. Compared to Caucasians, Pima Indians had elevated insulin concentrations at the same degree of insulin resistance. These studies indicate that insulin resistance, and not ??-cell failure, is the principal lesion determining IGT in Pimas. NIDDM occurs when ??-cell failure develops in the presence of insulin resistance. In some individuals of other races, ??-cell function may be less able to withstand insulin resistance, and presumably in these individuals ??-cell failure assumes a greater importance in the evolution to NIDDM.

- Abstract
Impaired glucose tolerance often presages the development of non-insulin-dependent diabetes mellitus. We have studied insulin action and secretion in 24 Pima Indians before and after the development of impaired glucose tolerance and in 254 other subjects representing the whole spectrum of glucose tolerance, including subjects with overt non-insulin-dependent diabetes. The transition from normal to impaired glucose tolerance was associated with a decrease in glucose uptake during hyperinsulinemia, from 0.018 to 0.016 mmol per minute (from 3.3 to 2.8 mg per kilogram of fat-free body mass per minute) (P 0.0003). Mean plasma insulin concentrations increased during an oral glucose-tolerance test, from 1200 to 1770 pmol per liter (from 167 to 247 ??U per milliliter). In 151 subjects with normal glucose tolerance, the insulin concentration measured during an oral glucose-tolerance test correlated with the plasma glucose concentration (r = 0.48, P ??? 0.0001). This relation was used to predict an insulin concentration of 1550 pmol per liter (216 ??U per milliliter) in subjects with impaired glucose tolerance (actual value, 1590 pmol per liter [222 ??U per milliliter]; P not significant), suggesting that these subjects had normal secretion of insulin. In contrast, plasma insulin concentrations in the diabetics decreased as glucose concentrations increased (r = -0.75, P ??? 0.0001), suggesting deficient secretion of insulin. This relative insulin deficiency first appears at the lower end of the second (diabetic) mode seen in population frequency distributions of plasma glucose concentrations. Our data show that impaired glucose tolerance in our study population is primarily due to impaired insulin action. In patients with non-insulin-dependent diabetes mellitus, by contrast, impaired insulin action and insulin secretory failure are both present.
