Abstract

The advent of chimeric antigen receptor (CAR) T‐cell therapy has changed the therapeutic landscape of relapsed/refractory aggressive B‐cell lymphomas. Cytokine release syndrome and immune effector cell‐associated neurotoxicity syndrome (ICANS) are the typical adverse events associated with this therapy. Cardiovascular toxicities have also been reported in this setting. However, there is scarce data regarding the development of sinus bradycardia after CAR T‐cell therapy. Here, we detail the clinical course of 4 patients with aggressive B‐cell malignancies who received CAR T‐cells and developed transient and reversible sinus bradycardia in the context of ICANS. We also discuss several hypotheses behind the pathophysiology of this potential new adverse event. [ABSTRACT FROM AUTHOR]

Abstract

These patients presented with common characteristics of MCL, with PB involvement and lymphocytosis (2/3) and BM infiltration (2/3), and their clinical course was heterogeneous, indolent in three cases and aggressive, as in our patient, in the other two. Mantle cell lymphoma (MCL) is a mature B-cell neoplasm accounting for 5-10% of all non-Hodgkin lymphomas [[1]]. On the other hand, four cases of lymphoma with the t(2;11)(p11;q13) have been previously reported: three of them were classified as leukemic small-cell B-non-Hodgkin lymphoma [[7]], and the other one described as MCL [[14]]. In conclusion, the variant t(11;22)(q13;q11.2) in MCL is very uncommon and it is not associated with differentiated clinical features in MCL, but rather presents with the clinical characteristics of a MCL. Abstract

Whereas 38 patients (67.8% of the patients who did not receive CAR-T cells) were excluded at first (remote) evaluation, 10 (17.9%) and eight (14%) patients dropped out after first visit and after leukapheresis, respectively. Chimeric antigen receptor T-cells (CAR-T) have recently emerged as an active therapy for patients with relapse or refractory diffuse large B-cell lymphoma (R/R DLBCL) [[1]]. Among the evaluated patients, age was the only factor associated with not finally receiving the therapy, presumably because elderly patients have more comorbidities and less treatment options during the bridging period. Abstract

Background: Chimeric antigen receptor (CAR) T-cell therapy is increasingly used in patients affected by B-cell lymphoma and acute lymphoblastic leukemia. For logistical reasons, initial apheresis products may be cryopreserved for shipment to manufacturing centers. Due to the characteristics of these patients, cells are often collected in large volumes, meaning more bags must be cryopreserved. This requires increased storage, time and monetary costs. In this context, we aimed to evaluate a high cell concentration cryopreservation protocol by centrifugation to standardize the initial CAR-T manufacturing procedure., Materials and Methods: Sixty-eight processes of leukapheresis of 57 patients affected by refractory/relapsed B cell lymphoma and 9 patients affected by acute lymphoblastic leukemia who were eligible for anti-CD19 CAR-T cell treatment performed between June 2019 and October 2022 were analyzed. Whole blood count, percentage and number of T cells were assessed on the apheresis final product. The apheresis product, which was alternatively stored overnight at 4°C, was centrifuged, adjusting the volume to approximately 40 mL. The product was immediately cryopreserved to achieve a final cell concentration of 50-200×10 6 cells/ml for cryopreservation., Results: Leukapheresis volume was reduced by almost fivefold (median: 185 to 40 mL), resulting in a higher product concentration in one bag. In addition, the number of non-target cells (monocytes, platelets and erythrocytes) was also reduced during the development of CAR-T cell therapy, thereby maintaining T lymphocyte levels and providing a purer starting material., Discussion: The advantages of the protocol include reducing economic costs, saving storage space, simplifying the manufacturing process, and facilitating shipping logistics. In conclusion, we present a validated, simple, and cost-effective cell enrichment processing protocol that provides high-quality cryopreserved products as starting material for the CAR-T cell manufacturing process.
