Reciprocal inhibition and coactivation are strategies of the central nervous system used to perform various daily tasks. In automatic postural responses (APR), coactivation is widely investigated in the ankle joint muscles, however reciprocal inhibition, although clear in manipulative motor actions, has not been investigated in the context of APRs. The aim was to identify whether reciprocal inhibition can be observed as a strategy in the recruitment of gastrocnemius Medialis (GM), Soleus (So) and Tibialis Anterior (TA) muscles in low- and high-velocity forward and backward perturbations. We applied two balance perturbations with a low and a high velocity of displacement of the movable platform in forward and backward conditions and we evaluated the magnitude and latency time of TA, GM and So activation latency, measured by electromyography (EMG). In forward perturbations, coactivation of the three muscles was observed, with greater activation amplitude of the GM and lesser amplitude of the So and TA muscles. For backward, the pattern of response observed was activation of the TA muscle, a decrease in the EMG signal, which characterizes reciprocal inhibition of the GM muscle and maintenance of the basal state of the So muscle. People with degenerative cervical myelopathy are known to have impaired standing balance and walking abilities, but less is known about balance responses during walking. The aim of this project was to assess reactive balance impairments during walking in people with degenerative cervical myelopathy (PwDCM). We hypothesized that center of mass motion following perturbations would be larger in PwDCM and gluteus medius electromyographic amplitude responses would be decreased in PwDCM. Reactive balance responses were quantified during unanticipated lateral pulls to the waist while treadmill walking. Walking biomechanics data were collected from 10 PwDCM (F=6) and 10 non-myelopathic controls (F=7) using an 8 camera Vicon System (Vicon MX T-Series). Electromyography was collected from lower limb muscles. Participants walked on an instrumented treadmill and received lateral pulls at random intervals and in randomized direction at 5% and 2.5% body mass. Participants walked at 3 prescribed foot placements to control for effects of the size of base of support. As compared with controls, the perturbation-related positional change of the center of mass motion (ΔCOM) was increased in PwDCM (p=0.001) with similar changes in foot placement (p>0.05). Change in gluteus medius electromyography, however, was less in PwDCM than in controls (p<0.001). After experimentally controlling step width, people with mild-to-moderate degenerative cervical myelopathy at least 3 months following cervical spine surgery have impaired reactive balance during walking likely coupled with reduced gluteus medius electromyographic responses. Background: Previous work suggests that proprioceptive information from ankle and hip are crucial in maintaining balance during upright standing; however, the contribution of these proprioceptive information during stepping balance recovery in not clear. The goal of the current study was to assess the role of ankle and hip proprioceptive information on balance recovery performance by manipulating type 1a afferent in muscle spindles using vibratory stimulation. Methods: Twenty healthy young participants were recruited (age = 22.2 ± 2.7 years) and were randomly assigned to balance recovery sessions with either ankle or hip stimulation. Trip-like perturbations were imposed using a modified treadmill setup with a protecting harness. Vibratory stimulation was imposed bilaterally on ankle and hip muscles to expose participants to three condition of no-vibration, 40Hz vibration, and 80Hz vibration. Kinematics of the trunk and lower-extremities were measured using wearable sensors to characterize balance recovery performance. Outcomes were response time, recovery step length, trunk angle during toe-off and heel-strike of recovery stepping, and required time for full recovery. BACKGROUND: Analysis of performance asymmetries has indicated that the right and left cerebral hemispheres are specialized for specific functions of motor control. AIM: In the current investigation, we aimed to evaluate the effect of leg dominance on electrical activation of the plantar flexor muscles in responses to unanticipated stance perturbations in a state muscular fatigue of the dominant leg. METHOD: Fatigue was induced through ankle isometric contraction targeting 40% of maximum voluntary contraction. Muscle activation of the triceps surae of the dominant and non-dominant legs were compared in reactive responses to unanticipated load released from the trunk, leading to forward body sway. Muscular responses were analyzed in two states: pre-fatigue and fatigue of the triceps surae muscles of the dominant leg only. RESULTS: Analysis of magnitude of muscular activation for balance recovery following perturbations revealed fatigue-related compensatory activation in the lateral gastrocnemius muscle, as indicated by over-activation of the non-fatigued/non-dominant leg to compensate for the low muscular activation of the dominant/fatigued leg in the unilateral fatigue state. CONCLUSION: Compensatory behavior between the legs was not evident in the medial gastrocnemius and soleus muscles. While the literature has characterized balance control during quasi-static and/or dynamic tasks, comparatively few studies have examined relationships across paradigms. This study investigated whether quiet-stance postural steadiness metrics were associated with reactive control parameters (during both stepping and restabilization phases) following a lean-and-release perturbation. A total of 40 older adults participated. Postural steadiness (center of the pressure range, root mean square, velocity, and frequency) was evaluated in "feet together" and "tandem stance" positions. During the reactive control trials, the step length, step width, movement time, and reaction time were measured, in addition to the postural steadiness variables measured during the restabilization phase following the stepping response. Out of 64 comparisons, only 10 moderate correlations were observed between postural steadiness and reactive spatio-temporal stepping parameters (P ≤.05, r = −.312 to −.534). However, postural steadiness metrics were associated with the center of pressure velocity and frequency during the restabilization phase of the reactive control trials (P ≤.02, r =.383 to.775 for velocity and P ≤.01, r =.386 to.550 for frequency). Although some elements of quasi-static center of pressure control demonstrated moderate associations with dynamic stepping responses, relationships were stronger for restabilization phase dynamics after foot-contact. Low back pain (LBP) is a painful manifestation in the lower part of the spine which causes disabilities changing the sensitivity of sensory neurons resulting in weakness of postural muscles interfering with the balance. It is not already clear if LBP people's muscle changes affect the centre of pressure (CoP) recovery in challenging stance perturbations. The aim of this study was to identify differences in the muscle reactions of people with and without LBP to control CoP in challenging stance perturbations. We applied low (Vel1) and high (Vel2) magnitude stance perturbation by a movable platform and evaluated (a) the magnitude and latency time of trunk and lower limb muscle activation, (b) and the displacement and the latency time of the first CoP peak. The latency of trunk and hip muscle activation on Vel2 was lower for LBP group. The CoP latency and displacement did not change between groups and velocities indicating that the muscles took the same time to overcome the external forces. In conclusion, the maintenance of CoP latency at both velocities was not affected on Vel2, suggesting that such alterations have protective action and preservation of the pain on the LPB group in challenging stance perturbations. This study aimed to investigate how, especially how rapidly, the multiple lower-limb muscles and joints would respond to the unexpected standing balance perturbations. Unexpected waist-pull perturbations with small, medium and large magnitudes were delivered to twelve healthy young adults from the anterior, posterior, medial and lateral directions. Electromyographical (EMG) and mechanomyographical (MMG) responses of eight dominant-leg muscles (i.e., hip abductor/adductors, hip flexor/extensor, knee flexor/extensor, and ankle dorsiflexor/plantarflexors) together with the lower-limb joint angle, moment, and power data were recorded. The onset latencies, time to peak, peak values, and/or rate of change of these signals were analyzed. Statistical analysis revealed that: (1) agonist muscles resisting the delivered perturbation had faster activation than the antagonist muscles; (2) ankle muscles showed the largest rate of activation among eight muscles following both anteroposterior and mediolateral perturbations; (3) lower-limb joint moments that complied with the perturbation had faster increase; and (4) larger perturbation magnitude tended to evoke a faster response in muscle activities, but not necessarily in joint kinetics/kinematics. Background: Declines in muscular function may hinder our ability to properly respond balance perturbations during walking. Examining age-related differences in muscle activation during balance-perturbed walking could be an important summary of literature to guide future clinical or scientific research.Research Question: Are there differences in lower limb muscle activation between young and older adults when responding to balance perturbations during walking?Methods: A literature search was conducted in October 2020 to identify relevant articles using Pubmed, Scopus, Web of Science, Ovid EMBASE, and CINAHL. Inclusion criteria were defined to identify studies investigating lower limb muscle activation in healthy older adults during balance-perturbed walking. Data extraction was independently performed by both authors. Outcome measures included key findings of lower limb muscle activations during walking and balance-related tasks (e.g. multidirectional perturbations, different speeds, cognitive tasks, slippery/slopes, and obstacles).Results: This article reviewed fourteen studies including 230 older adults (age: 70 ± 4.5, females: 124 [53.9%]) and 230 young adults (age: 23 ± 2.0, females: 113 [49.1%]). The overall quality of included studies was fair, with a mean score of 76%. Twelve lower limb muscles were assessed during balance-perturbed walking. All studies reported electromyographic measurements, including magnitude, timing, co-contraction indices, and variability of activation.Significance: Compared to young adults, older adults demonstrated different adaptations in lower limb muscle activation during balance-perturbed walking. Co-contraction of ankle and knee joint muscles had more conclusive results, with the majority reporting an increased co-contraction in older adults, especially when balance is perturbed by a physical task. Traumatic brain injury (TBI) often results in balance impairment, increasing the risk of falls, and the chances of further injuries. However, the underlying neural mechanisms of postural control after TBI are not well understood. To this end, we conducted a pilot study to explore the neural mechanisms of unpredictable balance perturbations in 17 chronic TBI participants and 15 matched healthy controls (HC) using the EEG, MRI, and diffusion tensor imaging (DTI) data. As quantitative measures of the functional integration and segregation of the brain networks during the postural task, we computed the global graph‐theoretic network measures (global efficiency and modularity) of brain functional connectivity derived from source‐space EEG in different frequency bands. We observed that the TBI group showed a lower balance performance as measured by the center of pressure displacement during the task, and the Berg Balance Scale (BBS). They also showed reduced brain activation and connectivity during the balance task. Furthermore, the decrease in brain network segregation in alpha‐band from baseline to task was smaller in TBI than HC. The DTI findings revealed widespread structural damage. In terms of the neural correlates, we observed a distinct role played by different frequency bands: theta‐band modularity during the task was negatively correlated with the BBS in the TBI group; lower beta‐band network connectivity was associated with the reduction in white matter structural integrity. Background: Postural strategies are enabled by rapid muscle activation sequences to prevent a fall. Intralimb muscular couplings underlie these postural strategies are likely impaired after incomplete spinal cord injury (iSCI), leading to inappropriate postural reactions and increased fall risk; yet, the nature of these changes is unknown.Research Question: Identify changes occurring in intralimb coupling following a perturbation in individuals with iSCI.Methods: Ten men with iSCI and eight age-matched controls (CTRL) stood on a force-platform that was randomly tilted forward or backward. Electromyographic (EMG) activity of the lower limb muscles was recorded, and coactivation or simultaneous facilitation/suppression between pairs of muscles was analyzed. Onset and duration of coupling latency, intralimb coupling delay, and amplitude ratios were measured in the distal (soleus [SOL]/tibialis anterior [TA]), proximal (biceps femoris [BF]/vastus lateralis [VL]), anterior (TA-VL), and posterior (SOL-BF) muscle couplings.Results: In forward tilt, the main coupling was TA-SOL co-contraction for both groups, but the latency was longer and the duration shorter in SCI participants. In backward tilt, the TA-VL co-activation was the main coupling in CTRL (88 %), although it was also expressed by 60 % of SCI participant with a delayed latency. The facilitation/suppression of TA-SOL was the main coupling in SCI group (80 % vs 63 % in CTRL). Delayed coupling latencies were more pronounced in individuals with cervical iSCI and were correlated with the strength of lower limbs.Significance: Similar muscular couplings are present in both groups but are delayed, which might contribute to postural reaction deficits in individuals with iSCI. Interfering with or temporarily eliminating foot-sole tactile sensations causes postural adjustments. Furthermore, individuals with impaired or missing foot-sole sensation, such as lower-limb amputees, exhibit greater postural instability than those with intact sensation. Our group has developed a method of providing tactile feedback sensations projected to the missing foot of lower-limb amputees via electrical peripheral nerve stimulation (PNS) using implanted nerve cuff electrodes. As a step toward effective implementation of the system in rehabilitation and everyday use, we compared postural adjustments made in response to tactile sensations on the missing foot elicited by our system, vibration on the intact foot-sole, and a control condition in which no additional sensory input was applied. Three transtibial amputees with at least a year of experience with tactile sensations provided by our PNS system participated in the study. Participants stood quietly with their eyes closed on their everyday prosthesis while electrically elicited, vibratory, or no additional sensory input was administered for 20 s. Early and steady-state postural adjustments were quantified by center of pressure location, path length, and average angle over the course of each trial. Electrically elicited tactile sensations and vibration both caused shifts in center of pressure location compared to the control condition. Initial (first 3 s) shifts in center of pressure location with electrically elicited or vibratory sensory inputs often differed from shifts measured over the full 20 s trial. Over the full trial, participants generally shifted toward the foot receiving additional sensory input, regardless of stimulation type. Similarities between responses to electrically elicited tactile sensations projected to the missing foot and responses to vibration in analogous regions on the intact foot suggest that the motor control system treats electrically elicited tactile inputs similarly to native tactile inputs. The ability of electrically elicited tactile inputs to cause postural adjustments suggests that these inputs are incorporated into sensorimotor control, despite arising from artificial nerve stimulation. These results are encouraging for application of neural stimulation in restoring missing sensory feedback after limb loss and suggest PNS could provide an alternate method to perturb foot-sole tactile information for investigating integration of tactile feedback with other sensory modalities. Challenging quiet standing tasks for the ankle, knee, hip, and neck joints were used to test the effect of pre-cueing on the reaction time of articular stabilization strategies. We assessed balance recovery in 15 subjects who were exposed to backward translations on a movable platform during two conditions: alerted, where participants gazed at a countdown light that pre-cued the onset of the platform translation; and unalerted, without onset time pre-cue. We compared the angular variation of the ankle, knee, hip, and neck joints between the two conditions over four window epochs (0–50 ms; 50–100 ms; 100–150 ms; 150–200 ms). When comparing the window epochs of the ankle and hip joints between conditions, a significant effect was observed in the first and second windows. In the second and third windows, a significant effect was only observed in the alerted group, thereby indicating a faster stability for alerted condition. No significant effect was observed for the knee joint, while a significant effect was observed in the neck joint during the third and fourth windows of the alerted condition. Pre-cueing can modify the synergies of the automatic reactive postural responses, prioritizing the stabilization of main joints such as the ankle and hip over that of the neck and knee. It was suggested that joint stabilization in the hip occurred due to the involuntary anticipation preprogrammed in the cortex by the visual pre-cue. • Precueing time makes faster stabilization of ankle and hip joints. • Knee joint has orthostatic function in balance recovery. • Neck joint stabilization does not appear to be a priority for postural control. Purpose: Postural reactions (PR) of standing subjects have been mostly investigated in response to platform displacements or body perturbations of fixed magnitude. The objective of this study was to investigate the relationship between PR and the peak force and impulse of the perturbation.Methods: In ten healthy young men, standing balance was challenged by anteriorly directed perturbations (peak force: 20-60 N) delivered to the back, at the lumbar (L) or inter-scapular (IS) level, by means of a manual perturbator equipped with a force sensor. Postural reactions as expressed by the displacement of the center of pressure (CoP) were recorded using a force platform. Two sets of 20 randomly ordered perturbations (10 to each site) were delivered in two separate testing sessions.Results: The magnitude of CoP response (∆CoP) was better correlated with the impulse (I) than with the peak force of the perturbation. The normalized response, ∆CoPn = ∆CoP/I, exhibited good reliability (ICCs of 0.93 for IS and 0.82 for L), was higher with IS than with L perturbations (p < 0.01), and was significantly correlated with the latency of CoP response: r = 0.69 and 0.71 for IS and L, respectively.Conclusion: These preliminary findings support the concept that manually delivered perturbations can be used to reliably assess individual PR and that ∆CoPn may effectively express a relevant aspect of postural control. The study proposed to identify balance strategies used by younger and older adults during gait under proprioceptive, visual, and simultaneous proprioceptive-visual challenges. Participants ambulated under 4 conditions: consistent, noncompliant surface; inconsistent, compliant surface (C); consistent, noncompliant surface with vision obscured (NCVO); and inconsistent, compliant surface with vision obscured (CVO). Balance adaptations were measured as changes in gait velocity, cadence, and gait-stability ratio (GSR). Participants were 5 younger (mean age = 27.2) and 5 older (mean age = 68) healthy adults. Significant age differences were found for GSR (p = .03) on all surfaces. Older adults adopted a more stable gait pattern than younger adults regardless of the challenge presented by surface. Significant condition differences were found for velocity (p < .001) and cadence (p = .001). All participants exhibited significantly decreased velocity and increased cadence on surfaces C and CVO. Gait speed and cadence did not significantly change in NCVO. Younger and older adults exhibited similar adaptive balance strategies, slowing and increasing steps/s, under proprioceptive and proprioceptive-visual challenges to dynamic balance. Maintaining balance is a complex process requiring multisensory processing and coordinated muscle activation. Previous studies have indicated that the cortex is directly involved in balance control, but less information is known about cortical flow of signals for balance. We studied source-localized electrocortical effective connectivity dynamics of healthy young subjects (29 subjects: 14 male and 15 female) walking and standing with both visual and physical perturbations to their balance. The goal of this study was to quantify differences in group-level corticomuscular connectivity responses to sensorimotor perturbations during walking and standing. We hypothesized that perturbed visual input during balance would transiently decrease connectivity between occipital and parietal areas due to disruptive visual input during sensory processing. We also hypothesized that physical pull perturbations would increase cortical connections to central sensorimotor areas because of higher sensorimotor integration demands. Our findings show decreased occipito-parietal connectivity during visual rotations and widespread increases in connectivity during pull perturbations focused on central areas, as expected. We also found evidence for communication from cortex to muscles during perturbed balance. These results show that sensorimotor perturbations to balance alter cortical networks and can be quantified using effective connectivity estimation. • Cortical connectivity during perturbed balance has not been well studied. • We computed multi-subject, source-localized EEG connectivity across frequency bands. • Altered occipito-parietal connectivity reflects changes to sensorimotor integration. • Supplementary motor area was a cortical hub during pull perturbations. • Perturbed balance alters cortical connectivity, which can be studied across subjects. Processing of predictive contextual cues of an impending perturbation is thought to induce adaptive postural responses. Cueing in previous research has been provided through repeated perturbations with a constant foreperiod. This experimental strategy confounds explicit predictive cueing with adaptation and non-specific properties of temporal cueing. Two experiments were performed to assess those factors separately. To perturb upright balance, the base of support was suddenly displaced backwards in three amplitudes: 5, 10 and 15 cm. In Experiment 1, we tested the effect of cueing the amplitude of the impending postural perturbation by means of visual signals, and the effect of adaptation to repeated exposures by comparing block versus random sequences of perturbation. In Experiment 2, we evaluated separately the effects of cueing the characteristics of an impending balance perturbation and cueing the timing of perturbation onset. Results from Experiment 1 showed that the block sequence of perturbations led to increased stability of automatic postural responses, and modulation of magnitude and onset latency of muscular responses. Results from Experiment 2 showed that only the condition cueing timing of platform translation onset led to increased balance stability and modulation of onset latency of muscular responses. Conversely, cueing platform displacement amplitude failed to induce any effects on automatic postural responses in both experiments. Our findings support the interpretation of improved postural responses via optimized sensorimotor processes, at the same time that cast doubt on the notion that cognitive processing of explicit contextual cues advancing the magnitude of an impending perturbation can preset adaptive postural responses. In this study, we evaluated the effect of precueing characteristics of an impending perturbation to upright stance on reactive responses of distal leg muscles. Young and older individuals were compared in a task of recovering stable upright stance following rotation of the supporting platform to induce anterior or posterior body sway. Directions of the supporting platform rotation were randomized across trials. Immediately before postural perturbation participants were cued about direction and/or time of platform rotation, or performed the task under directional and temporal uncertainty of the impending perturbation. Results showed that precueing time of perturbation led to earlier muscular activation onset, while precueing perturbation direction did not modulate either latency or magnitude of muscular activation. Those effects were similar between age groups. Our findings suggest that awareness of the perturbation time favored shorter response latencies in both the young and older individuals. Abstract: The present study compared neuromuscular adaptations to 12weeks of plyometric (PLY) or pneumatic (PNE) power training and their effects on dynamic balance control. Twenty-two older adults aged 60–70 (PLY n =9, PNE n =11) participated in the study. Measurements were conducted at Pre, 4, 8 and 12weeks. Dynamic balance was assessed as anterior–posterior center of pressure (COP) displacement in response to sudden perturbations. Explosive isometric knee extension and plantar flexion maximal voluntary contractions (MVCs) were performed. Maximal drop jump performance from optimal dropping height was measured in a sledge ergometer. Increases in knee extensor and ankle plantar flexor torque and muscle activity were higher and occurred sooner in PNE, whereas in drop jumping, PLY showed a clearer increase in optimal drop height (24%, p <0.01) after 8weeks of training and soleus muscle activity after 12weeks of training. In spite of these training mode specific adaptations, both groups showed similar improvements in dynamic balance control after 4weeks of training (PLY 38%, p <0.001; PNE 31%, p <0.001) and no change thereafter. These results show that although power and plyometric training may involve different neural adaptation mechanisms, both training modes can produce similar improvements in dynamic balance control in older individuals. As COP displacement was negatively correlated with rapid knee extension torque in both groups (PLY r =−0.775, p <0.05; PNE r =−0.734, p <0.05) after training, the results also highlight the importance of targeting rapid force production when training older adults to improve dynamic balance. Abstract: Anticipatory postural adjustments (APAs) prior to step execution are thought to be immutable once released. Here we challenge this assumption by testing whether APAs can be modified online if a body perturbation occurs during execution. Two directions of perturbation (resisting and assisting) relative to the body weight transfer were used during the execution of APAs. We found that APAs are modified online (increase in both ground pressure and muscle activity) to compensate for resisting perturbations. The outcomes of a biomechanical model confirmed that the early changes in the APAs resulted from an active control of the APAs and were not merely mechanical consequences of the perturbation. However, no modification of the initial feedforward command was observed for assisting perturbations. The motor command changes for the resisting perturbation may originate from the mismatch between passively originated forces and those actively specified by the central command when acting in the opposite direction. The absence of a mismatch in the assisting perturbation might explain why the central nervous system was not prompted to modify the APAs in this condition. [Copyright &y& Elsevier]

Abstract: The aim of the study was to examine how individuals of different ages react to forward balance perturbations. Thirty-six volunteers, divided into four groups [young (YA), middle-age (MA40 and MA50), and old (OA) adults], stood on a platform that was either kept stationary, moved backward, or moved forward. EMG onset, EMG time-to-peak, iEMG, and agonist–antagonist co-activation, as well as cumulative angular excursion, maximum center of mass (CM) backward displacement, and CM time-to-reversal were assessed after forward translations. Postural synergies were assessed using principal component analysis (PCA). The results showed that OA activated their muscles later than YA [TA=25ms, RF=17ms] and OA and MA50 reached the peak of activation later than YA [MA50:TA=23ms, RF=32ms, OA:TA=28ms, RF=21ms]. Moreover, OA kept a higher level of activation longer than all younger groups. No differences among groups were observed in co-activation, kinematic, and PCA variables. We conclude that changes in temporal EMG patterns can be seen in the fifth decade. However, such changes have no effect on the CM horizontal displacement and CM time-to-reversal after perturbation, which cannot be justified by the use of different postural synergies, suggesting that temporal aspects of muscle activation could play a minor role in controlling excessive CM displacements after perturbations. Abstract: An effective stepping response is often critical in avoiding a fall. Our objective was to study the effects of age and balance impairment on anterior and posterior compensatory stepping strategies in response to waist pull perturbations of 1–5% body weight (BW). Based on maximal unipedal stance time (UST), we tested 15 balance-impaired old (BI, UST<10s, mean age=76 years), 12 healthy old (O, UST>30s, mean age=71 years), and 13 healthy young women (Y, UST>30s, mean age=23 years). Randomized anterior and posterior pulls of 1–5% body weight (BW) were applied to the waist while kinematic and kinetic recovery responses were recorded. Results show that O and BI required 0.5 more steps than Y to recover balance for posterior pulls of 4–5% BW (P <0.01). For anterior pulls of 4–5% BW, only BI had a greater probability of step initiation (P ≤0.05 or <0.02) and mean number of steps required to recover balance (P <0.03). The Y used 93% greater torso extension and 24% greater torso flexion in responding to anterior and posterior pulls, respectively. In the posterior, but not anterior direction, O employed smaller (P <0.007), but more laterally-directed (P <0.03) steps than the Y. The BI were less able to attenuate their momentum during the step landing in both directions. We conclude that the additional steps required by the BI in both directions occurred because their initial step failed to properly arrest their momentum. Controlling torso inclination before step liftoff and linear momentum after step landing are critical components of successful compensatory stepping. [Copyright &y& Elsevier]

Background: Impaired balance regulation after stroke puts patients and therapists at risk of injury during rehabilitation. Body weight support systems (BWSSs) minimize this risk and allow patients to safely practice balance activities during therapy. Treadmill-based balance perturbation systems with BWSSs are known to improve balance in patients with age- or disease-related impairments. However, these stationary systems are unable to accommodate complex exercises that require more freedom of movement., Objective: This study aims to evaluate the effect of a new balance perturbation module, which is directly integrated into a track-mounted BWSS, on balance impairments secondary to acute stroke., Methods: This unblinded quasi-randomized controlled preliminary study was conducted in a rehabilitation-focused long-term acute care hospital. Participants were recruited from stroke rehabilitation inpatients with an admission Berg Balance Scale (BBS) score of 21 (out of 56) or greater. Over a 2-week period, consented participants completed 8 BWSS or BWSS with perturbation (BWSS-P) treatment sessions; study activities were incorporated into regular treatment to avoid disruption of their normal care. Although both groups conducted the same balance and gait activities during their treatment sessions, the BWSS-P sessions included lateral, anterior, and posterior balance perturbations. Pre- and postintervention BBS and Activities-Specific Balance Confidence (ABC) assessments were the primary outcome measures collected. Institutional BBS data from the year before installation of the track-mounted BWSS were retrospectively included as a post hoc historical standard of care comparison., Results: The improved postintervention BBS and ABC assessment scores showed that all participants benefited from therapy (P<.001 for all pre- and postintervention comparisons). The average BBS percent change for the BWSS-P sample (n=14) was 66.95% (SD 43.78%) and that for the BWSS control sample (n=15) was 53.29% (SD 24.13%). These values were greater than those for the standard of care group (n=30; mean 28.31%, SD 17.25%; P=.02 and P=.005 respectively), with no difference among the BWSS groups (P=.67). ABC score changes were also similar among the preintervention and postintervention BWSS groups (P=.94 and P=.92, respectively)., Conclusions: Both BWSS groups demonstrated similar BBS and ABC score improvements, indicating that balance perturbations were not detrimental to postacute stroke rehabilitation and were safe to use. Mobility has been one of the most impacted aspects of human life due to the spread of the COVID-19 pandemic. Home confinement, the lack of access to physical rehabilitation, and prolonged immobilization of COVID-19-positive patients within hospitals are three major factors that affected the mobility of the general population world-wide. Balance is one key indicator to monitor the possible movement disorders that may arise both during the COVID-19 pandemic and in the coming future post-COVID-19. A systematic quantification of the balance performance in the general population is essential for preventing the appearance and progression of certain diseases (e.g., cardiovascular, neurodegenerative, and musculoskeletal), as well as for assessing the therapeutic outcomes of prescribed physical exercises for elderly and pathological patients. Current research on clinical exercises and associated outcome measures of balance is still far from reaching a consensus on a "golden standard" practice. Moreover, patients are often reluctant or unable to follow prescribed exercises, because of overcrowded facilities, lack of reliable and safe transportation, or stay-at-home orders due to the current pandemic. A novel balance assessment methodology, in combination with a home-care technology, can overcome these limitations. This paper presents a computational framework for the in-home quantitative assessment of balance control skills. Novel outcome measures of balance performance are implemented in the design of rehabilitation exercises with customized and quantifiable training goals. Using this framework in conjunction with a portable technology, physicians can treat and diagnose patients remotely, with reduced time and costs and a highly customized approach. Background: Age-related difficulty in controlling lateral stability is of crucial importance because lateral falls increase risk of debilitating hip-fracture injury. This study examined whether a small increase in footwear sole width can improve ability of older adults to regain lateral stability subsequent to balance perturbation., Methods: The study involved sixteen healthy, ambulatory, community-dwelling older adults (aged 65-78). Widened base-of-support (WBOS) footwear was simulated by affixing polystyrene-foam blocks (20mm wide) on the medial and lateral sides of rubber overshoes; unaltered overshoes were worn in normal (NBOS) trials. Balance perturbations were applied using a motion platform., Results: Gait, mobility and agility tests revealed no adverse effects of wearing the WBOS footwear. Lateral-perturbation tests showed that the WBOS footwear improved ability to stabilize the body without stepping (p=0.002). Depending on the perturbation magnitude, the frequency of stepping was reduced by up to 25% (64% of NBOS trials vs 39% of WBOS trials). In addition, the WBOS footwear appeared to improve ability to maintain lateral stability during forward-step reactions, as evidenced by reduced incidence of additional lateral steps (p=0.04) after stepping over an obstacle in response to a forward-fall perturbation., Conclusions: A small increase in sole width can improve certain aspects of lateral stability in older adults, without compromising mobility and agility. This finding supports the viability of WBOS footwear as an intervention to improve balance. Further research is needed to test populations with more severe balance impairments, examine user compliance, and determine if WBOS footwear actually reduces falling risk in daily life.
