In many cases sarcoidosis is a multisystemic disease that requires interdisciplinary medical cooperation in the diagnostics, treatment and medical care during follow-up. Due to the often chronic course, it is of utmost importance to include patients with their priorities and wishes at an early stage and extensively in disease management and to establish a shared decision making whenever possible. In the process of writing this joint position paper, the expert group on interstitial and orphan lung diseases of the Austrian Society for Pulmonology and the working group on rheumatological lung disorders of the Austrian Society for Rheumatology and Rehabilitation sought to include patient advocacy groups as well as experts for rare organ manifestations of sarcoidosis. This position paper is not only meant to reflect current scientific and clinical standards but should also focus the national expertise and by networking and exchange to be a first step to strengthen cooperation between stakeholders to ultimately improve care for patients with sarcoidosis., (© 2024. People living with diabetes mellitus can be supported in the daily management by diabetes technology with automated insulin delivery (AID) systems to reduce the risk of hypoglycemia and improve glycemic control as well as the quality of life. Due to barriers in the availability of AID-systems, the use and development of open-source AID-systems have internationally increased. This technology provides a necessary alternative to commercially available products, especially when approved systems are inaccessible or insufficiently adapted to the specific needs of the users. Open-source technology is characterized by worldwide free availability of codes on the internet, is not officially approved and therefore the use is on the individual's own responsibility. In the clinical practice a lack of expertise with open-source AID technology and concerns about legal consequences, lead to conflict situations for health-care professionals (HCP), sometimes resulting in the refusal of care of people living with diabetes mellitus. This position paper provides an overview of the available evidence and practical guidance for HCP to minimize uncertainties and barriers. People living with diabetes mellitus must continue to be supported in education and diabetes management, independent of the chosen diabetes technology including open-source technology. Simple Summary: The heat-loving paper wasp Polistes dominula builds small nests in sheltered places in quite differing habitats, providing a favorable microclimate for brood development. Their careful choice of the nesting site enables them to keep the nest and brood temperature above the ambient air temperature. In temperate Austrian (Central European) climates, the cooling efforts of the adult wasps keep the mean comb temperature from exceeding approximately 39 °C, which seems to be a threshold for optimal development. Measurement of the nest and the body temperature of the wasps at typical nesting sites revealed a close relationship with the air temperature at the nests but a suboptimal correlation with climate data from a local standard weather station or model-generated macroclimate data. The comparison emphasizes the importance of microclimate measurements if it is to judge the vulnerability of insects to climate change. The paper wasp Polistes dominula is a thermophilic species originating from the Mediterranean climate, but is now widely spread in Europe. They live in quite differing habitats; and as synanthropic species, they have been established in human settlement areas. They build a single small comb at protected places with a favorable microclimate. We measured the temperature of the wasps, the nests and their environment at typical nesting sides in Austria (Europe) in the temperate climate, in order to reveal relationships between nest and body temperature and the habitats' microclimate. The temperatures of the comb and of the wasps' body were in a wide range (~20–37 °C) above the ambient air temperature at the nest. This is an advantage as higher temperatures accelerate the development speed of the brood. However, the mean comb temperature did not exceed approximately 38.6 °C. This was managed by cooling efforts of the adult wasps. The ambient air temperature near the nest (~1–2 cm) was always clearly elevated above the ambient air temperature at a local standard weather station in the habitat. A comparison with climate-model-generated macroclimate data revealed the necessity of measuring microclimate data for a reliable description of the insects' thermal environment. Simple Summary: During overwintering diapause, the gynes of paper wasps (Polistes sp.) are mainly dormant in sheltered hibernacles, protecting them against predators and adverse weather conditions but hardly against low temperatures. By measuring the temperature inside hibernacles occupied by species from both Mediterranean (Italian; P. dominula, P. gallicus) and temperate (Austrian; P. dominula) climates (mean hibernacle temperatures: 8.5 °C and 3.2 °C, respectively), we were able to calculate the energetic demand of overwintering. The cumulative energetic costs differed between the populations. Costs were lowest for the P. dominula population from the cooler Austrian winter climate and significantly higher in P. dominula and P. gallicus from the warmer Italian climate. The lower costs of the temperate species were a result of the lower winter temperature and physiological acclimation processes. Energetic calculations with an assumed temperature increase of up to 3 °C due to climate change predict a dramatic increase of up to 40% in overwintering costs in all species. Gynes of paper wasps (Polistes sp.) spend the cold season in sheltered hibernacles. These hibernacles protect against predators and adverse weather conditions but offer only limited protection against low temperatures. We investigated the hibernacles' microclimate conditions of species from the Mediterranean (Italy, P. dominula, P. gallicus) and temperate (Austria, P. dominula) climates in order to describe the environmental conditions and calculate the energetic demand of overwintering according to standard metabolic rate functions. The temperatures at the hibernacles differed significantly between the Mediterranean and temperate habitats (average in Austria: 3.2 ± 5.71 °C, in Italy: 8.5 ± 5.29 °C). In both habitats, the hibernacle temperatures showed variance, but the mean hibernacle temperature corresponded closely to the meteorological climate data. Cumulative mass-specific energetic costs over the studied period were the lowest for the temperate P. dominula population compared with both Mediterranean species. The lower costs of the temperate species were a result of the lower hibernacle temperature and acclimation to lower environmental temperatures. Background: Cannabis is the illegal drug most frequently used by Minors in Austria. Due to the gradual decriminalization and legalization that has taken place in many European countries in recent years, the ÖGKJP would like to take a balanced and scientifically based stand on the complex issue of cannabis use and abuse among young people., Methods: The authors searched the medline for current studies using searches tailored to each specific subtopic. Furthermore, recognized compendiums were quoted., Results: While occasional recreational use of cannabis in adults with completed brain maturation and no risk profile for mental disorders is likely to be relatively harmless, early initiation of use with regular use and the increasingly available, highly potent cannabis varieties can lead to explicit and sometimes irreversible neurocognitive brain dysfunction., Conclusion: Legalisation of cannabis consumption for minors needs to be objected to due to the risks of the expected damage in the area of brain development. At the same time, however, it is important to establish sensible legal regulations in order to be able to adequately counteract the fact that over 30% of all European young people occasionally consume cannabis. The humanized monoclonal anti-α4β7-integrin-antibody vedolizumab is one of several biologic therapeutic options in moderate-to-severe ulcerative colitis and Crohn's disease. Within the VISIBLE trial program, a novel subcutaneous application route was evaluated in addition to the already established intravenous form. Objective: This work aims to analyze the 100 most cited papers in radiotherapy or chemoradiotherapy for cervical cancer. Methods: The 100 most cited papers in radiotherapy or chemoradiotherapy between 1990 and 2020 were identified with Thompson Reuters Web of Science citation indexing on August 24, 2020. The publication years, source titles, countries/regions, total citations, and average citations per year were extracted from the Web of Science. The research type and research domain were classified by the authors. Results: These 100 papers were cited a total of 28,714 times, and the median number of citations was 169.5 (ranging from 116 to 1,700 times). The most cited paper was "Concurrent cisplatin-based radiotherapy and chemotherapy for locally advanced cervical cancer" by Rose et al., with a total citation of 1,700 times. The International Journal of Radiation Oncology Biology Physics (40 papers), Radiotherapy and Oncology (14 papers), and the Journal of Clinical Oncology (12 papers) published the largest number of papers. USA (47 papers), Austria (18 papers), Canada (13 papers), and England (13 papers) contributed the largest number of papers. Image-guided adaptive brachytherapy (IGABT) had the largest number of papers (25 papers). Concurrent chemotherapy was the most cited research domain, with 10,663 total citations and 592.4 citations per paper. Conclusion: In the present study, we conducted a bibliometric analysis of the 100 most cited papers in radiotherapy or concurrent chemoradiotherapy for cervical cancer in the past 30 years. It highlights the significance of the Heat Exchanger Fouling and Cleaning Conference, which brings together experts from around the world to share research and advancements in fouling science, mitigation strategies, and cleaning technologies. The 14th conference took place in Wagrain, Austria, in 2022, after a hiatus due to the global pandemic. The treatment of peripheral nerve pathologies requires a rapid and precise diagnosis. However, the correct identification of nerve pathologies is often difficult and valuable time is lost in the process. In this position paper of the German-Speaking Group for Microsurgery of Peripheral Nerves and Vessels (DAM), we describe the current evidence for various perioperative diagnostics for the detection of traumatic peripheral nerve lesions or compression syndromes. In detail, we evaluated the importance of clinical examinations, electrophysiology, nerve ultrasound and magnetic resonance neurography. Additionally, we surveyed our members for their diagnostic approach in this regard. Background and Objectives: In March 2020, the WHO declared the SARS-CoV-2 corona virus a pandemic which caused a great disruption to global society and had a pronounced effect on the worldwide supply of blood., Materials and Methods: In 2022 an on-line meeting was organised with experts from Austria, Canada, Germany, Greece, Netherlands and United States to explore the opportunities for increasing preparedness within blood systems for a potential future pandemic with similar, or more devastating, consequences. Ulcerative colitis (UC) is one of the main forms of inflammatory bowel disease (IBD). Despite the widening range of drug treatment options, primary nonresponse, secondary loss of response as well as adverse events call for additional treatment alternatives.Tofacitinib is an oral small-molecule drug of the class of Janus kinase inhibitors which, in the European Union, was approved for the treatment of moderate to severe active UC in August 2018. This position paper, drawn up by the IBD Working Group of the Austrian Society of Gastroenterology and Hepatology, summarizes the mechanism of action, clinical development, marketing authorization status, efficacy and safety of tofacitinib. Also, by providing a synopsis of available data from both pivotal and post-marketing studies, clinical aspects of specific interest are highlighted and discussed.The available body of evidence indicates that tofacitinib is an additional effective medication for the treatment of UC that exhibits a good safety profile. Public safety (prevention of accidents) is the primary objective in assessing fitness to drive a motor vehicle. However, general access to mobility should not be restricted if there is no particular risk to public safety. For people with diabetes mellitus, the Führerscheingesetz (Driving Licence Legislation) and the Führerscheingesetz-Gesundheitsverordnung (Driving Licence Legislation Health enactment) regulate important aspects of driving safety in connection with acute and chronic complications of the disease. Critical complications that may be relevant to road safety include severe hypoglycemia, pronounced hyperglycemia and hypoglycemia perception disorder as well as severe retinopathy and neuropathy, endstage renal disease and certain cardiovascular manifestations. If there is a suspicion of the presence of one of these complications, a detailed evaluation is required.In addition, the individual antihyperglycemic medication should be checked for existing potential for hypoglycemia. Sulfonylureas, glinides and insulin belong to this group and are therefore associated with the requirement of a 5-year limitation of the driver's license. Other antihyperglycemic drugs without potential for hypoglycemia such as Metformin, SGLT‑2 inhibitors (Sodium-dependent-glucose-transporter‑2 inhibitors, gliflozins), DPP-4-inhibitors (Dipeptidyl-Peptidase inhibitors, gliptins), and GLP‑1 analogues (GLP‑1 rezeptor agonists) are not associated with such a time limitation.The relevant laws which regulate driving safety give room for interpretation, so that specific topics on driving safety for people with diabetes mellitus are elaborated from a medical and traffic-relevant point of view. Epidemiological investigations have shown that approximately 2-3% of all Austrians have diabetes mellitus with renal involvement, leaving 250,000 people in Austria affected. Survival in cancer is continuously improving due to evolving oncological treatment. Therefore, cardiovascular short-term and long-term side effects gain crucial importance for overall outcome. Cardiotoxicity not only presents as heart failure, but also as treatment-resistant hypertension, acute coronary ischemia with plaque rupture or vasospasm, thromboembolism, arrhythmia, pulmonary hypertension, diastolic dysfunction, acute myocarditis and others. Recent recommendations have proposed baseline cardiac risk assessment and surveillance strategies. Major challenges are the availability of monitoring and imaging resources, including echocardiography with speckle tracking longitudinal strain (GLS), serum biomarkers such as natriuretic peptides (NT-proBNP) and highly sensitive cardiac troponins. This Austrian consensus encompasses cardiotoxicity occurrence in frequent antiproliferative cancer drugs, radiotherapy, immune checkpoint inhibitors and cardiac follow-up considerations in cancer survivors in the context of the Austrian healthcare setting. It is important to optimize cardiovascular risk factors and pre-existing cardiac diseases without delaying oncological treatment. If left ventricular ejection fraction (LVEF) deteriorates during cancer treatment (from >10% to <50%), or myocardial strain decreases (>15% change in GLS), early initiation of cardioprotective therapies (angiotensin-converting enzyme inhibitors, angiotensin or beta receptor blockers) is recommended, and LVEF should be reassessed before discontinuation. Lower LVEF cut-offs were recently shown to be feasible in breast cancer patients to enable optimal anticancer treatment. It mentions that optimizations were also implemented on the generators, and the result is an increase in performance in the upper single-digit percentage range and a significantly improved environmental compatibility of the system.

In the wake of local initiatives and developmental funding programs, interprofessionality is now included in national curricula in the German-speaking countries. Based on the 3P model (presage, process, product), this position paper presents the development of interprofessional education in recent years in Germany, Austria and Switzerland and places it in an international context. Core aspects as legal frameworks, including amendments to occupational regulations as well as the formation of networks and faculty development are basic requirements for interprofessional education. New topics and educational settings take shape in the process of interprofessional education: patient perspectives and teaching formats, such as online courses, become more important or are newly established. The influence of the COVID-19 pandemic on interprofessional education is explored as well. Among many new interprofessional courses, particularly the implementation of interprofessional training wards in Germany and Switzerland are positive examples of successful interprofessional education. The objective of interprofessional education continues to be the acquisition of interprofessional competencies. The main focus is now centered on evaluating this educational format and testing for the corresponding competencies. In the future, more capacities will be required for interprofessional continuing education and post-graduate education. Structured research programs are essential to ascertain the effects of interprofessional education in the German-speaking countries. In this position paper the GMA committee on interprofessional education encourages further advancement of this topic and expresses the aim to continue cooperating with other networks to strengthen and intensify interprofessional education and collaboration in healthcare. Bisphenol A (BPA) a synthetic, high production volume chemical identified as endocrine disruptor and toxic to reproduction is mainly used in the production of polycarbonate plastics, in epoxy resins, polyvinylchloride, thermal papers as color developer, and is present in a wide range of consumer goods such as food packaging materials, storage containers, and cash receipts. Due to its effects on health and legal restrictions, BPA is increasingly replaced by other bisphenols. In this study, BPA and 13 alternatives including BPS, Bisphenol F (BPF), Bisphenol B (BPB), Bisphenol C (BPC), Bisphenol Z (BPZ), Bisphenol M (BPM), Bisphenol P (BPP), Bisphenol AF (BPAF), Bisphenol FL (BPFL), Bisphenol C12 (BPC12), Tetramethylbisphenol A (tmBPA), 4,4-bisphenol (BP-4,4), and p,p-oxybisphenol were analyzed in thermal paper cash receipts (content) and migration studies were carried out in BPA-free labelled reusable plastic drinking bottles using a sensitive UHPLC-MS/MS method. The receipts contained almost only BPA and BPS, whereas BPS was found in all samples ranging at levels up to 38 μg/g. BPA was detected at low concentrations, only in one sample 11,000 μg/g were found, exceeding the EU limit of BPA in thermal paper of 0.02% per weight. In leaching solutions from the drinking bottles BPA, BPS and BPF were found at concentrations up to 0.047 μg/L BPA, 0.043 μg/L BPS, and <0.01 μg/L BPF. No other analogues were detected. However, these levels identified are far below the legal limits. Winter in climatic regions with low temperatures is a challenge for overwintering insects. They are exposed to temperature extremes, which directly cause mortality or lead to energy depletion. The winter hibernaculum of paper wasp gynes protects from predators and rain, but only poorly from ambient temperature. In order to detect physiological adaptations to differing climates, we compared the respiratory metabolism of overwintering gynes of two polistine species from the Mediterranean climate in Italy (Polistes dominula and Polistes gallicus), and of one species from the temperate climate in Austria (Polistes dominula). The wasps' CO2 emission was measured with stop‐flow respirometry in a temperature range from 2.5 to 20 °C. The mass‐specific standard (resting) metabolic rate, the main type of metabolism of the dormant insects during overwintering, increased exponentially with ambient temperature but was suppressed in comparison to individuals measured in the summer, which conserves the energy stores. In addition, it was lower in the Mediterranean species (P. dominula and P. gallicus) in comparison to the temperate species (P. dominula), especially at higher temperatures. The active metabolic rate was suppressed to a similar amount. The suppressed metabolism of the overwintering Mediterranean gynes could be an adaptation to the higher winter temperatures to prevent a premature depletion of the energy resources. The paper mill “SCA GRAPHIC LAAKIRCHEN AG” is situated in Austria and produces 486,000 to of graphic paper per year. A yearly amount of 6,923,000 m3 of wastewater with a COD-load of 7,819,000 kg is treated on site (2005). The trend towards brighter paper grades requires more intense bleaching of mechanical pulp and increased usage of chelating agents and optical brighteners. These chemicals are difficult to degrade with biological waste water treatment. Higher bleaching dissolves more lignin, which is hard to degrade as well. As a result the COD removal efficiency in the biological stage of the wastewater treatment plant drops and additional measures for enhanced COD reduction have to be implemented in order to maintain given discharge limits. Radiofrequency denervation has been established for many years as an important minimally invasive procedure for the treatment of chronic pain conditions. Positive experiences of many users for various indications are contrasted by a nonuniform evidence. With meticulous patient selection and correct assessment of the indications a longer term reduction of pain, a reduced need for analgesics and an improvement in the quality of life can be achieved. The aim of this interdisciplinary position paper is to present the value of radiofrequency denervation in the treatment of chronic pain. The summarized recommendations of the expert group are based on the available evidence and on the clinical experiences of Austrian centers that frequently implement the procedure. The position paper contains recommendations on patient selection and proven indications. We discribe safety aspects, complications, side effects and contraindications.

Older adults are particularly affected by the current COVID-19 (SARS-CoV-2) pandemic. The risk of dying from COVID-19 increases with age and is often associated with pre-existing health conditions. Globally, more than 50 million-in Austria currently approximately 140,000 people-suffer from dementia. The co-occurrence of dementia as a "pandemic of old age" together with the COVID-19 pandemic has a double impact on persons living with dementia and their caregivers. The COVID-19 pandemic poses major challenges for individuals with dementia and their caregivers: (1) People with dementia have limited access to information on COVID-19, may have difficulties with protective measures such as wearing masks and in remembering safety regulations. (2) People with dementia live alone or with their family, or are institutionalized. To reduce the chance of infection among older people in nursing homes, Austrian local authorities have banned visitors to nursing homes and long-term care facilities and implemented strict social-distancing measures. As a result, older people lost face-to-face contact with their family members, became isolated and social activities stopped. Consequently, anxiety, stress and serious concerns about infections among staff in nursing homes increased and they developed signs of exhaustion and burnout during the full lockdown of the facilities. Thus, due to the emerging COVID-19 crisis, the Austrian Alzheimer Association (Österreichische Alzheimer Gesellschaft, ÖAG) and international societies developed recommendations to support people living with dementia and their caregivers on various issues of physical and mental health.

The pulp and paper (P&P) sector is a dynamic manufacturing industry and plays an essential role in the Austrian economy. However, the sector, which consumes about 20 TWh of final energy, is responsible for 7% of Austria's industrial CO2 emissions. This study, intending to assess the potential for improving energy efficiency and reducing emissions in the Austrian context in the P&P sector, uses a bottom-up approach model. The model is applied to analyze the energy consumption (heat and electricity) and CO2 emissions in the main processes, related to the P&P production from virgin or recycled fibers. Afterward, technological options to reduce energy consumption and fossil CO2 emissions for P&P production are investigated, and various low-carbon technologies are applied to the model. For each of the selected technologies, the potential of emission reduction and energy savings up to 2050 is estimated. Finally, a series of low-carbon technology-based scenarios are developed and evaluated. These scenarios' content is based on the improvement potential associated with the various processes of different paper grades. The results reveal that the investigated technologies applied in the production process (chemical pulping and paper drying) have a minor impact on CO2 emission reduction (maximum 10% due to applying an impulse dryer). In contrast, steam supply electrification, by replacing fossil fuel boilers with direct heat supply (such as commercial electric boilers or heat pumps), enables reducing emissions by up to 75%. This means that the goal of 100% CO2 emission reduction by 2050 cannot be reached with one method alone. Consequently, a combination of technologies, particularly with the electrification of the steam supply, along with the use of carbon-free electricity generated by renewable energy, appears to be essential. Background: 5-Fluorouracil (FU) is one of the most commonly used cytostatic drugs in the systemic treatment of cancer. Treatment with FU may cause severe or life-threatening side effects and the treatment-related mortality rate is 0.2-1.0%., Summary: Among other risk factors associated with increased toxicity, a genetic deficiency in dihydropyrimidine dehydrogenase (DPD), an enzyme responsible for the metabolism of FU, is well known. This is due to variants in the DPD gene (DPYD). Up to 9% of European patients carry a DPD gene variant that decreases enzyme activity, and DPD is completely lacking in approximately 0.5% of patients. Here we describe the clinical and genetic background and summarize recommendations for the genetic testing and tailoring of treatment with 5-FU derivatives. The statement was developed as a consensus statement organized by the German Society for Hematology and Medical Oncology in cooperation with 13 medical associations from Austria, Germany, and Switzerland. Key Messages: (i) Patients should be tested for the 4 most common genetic DPYD variants before treatment with drugs containing FU. (ii) Testing forms the basis for a differentiated, risk-adapted algorithm with recommendations for treatment with FU-containing drugs. The grey silverfish Ctenolepisma longicaudatum is a new and serious pest spreading rapidly across Europe. Compared to the silverfish Lepisma saccharinum, it does not depend on relative humidity above 70% and thrives also in new museum buildings, modern storage depositories, apartments, and office buildings. Especially collections of graphic art, modern art on paper, photographs, but also archives and libraries are increasingly affected by this pest. Damage of paper objects has been reported by different authors and institutions across Europe. As these animals live in inaccessible cracks and crevices inside the buildings, we investigated the spread of microorganisms on living grey silverfish, to show a potential additional threat posed by this pest. In 8 locations in Austria living animals were collected with traps and placed for 10 min on agar plates which were incubated for 2 weeks. After incubation of 50 agar plates, between 2 and 20 colonies of fungi were growing on each plate. The most dominant fungal genera were Cladosporium (Cladosporium cladosporioides clade) and Penicillium (Penicillium glabrum, Penicillium decumbens); also, species of the genera Aspergillus, Alternaria, Botrytis, Bartalinia, Byssochlamis, Chaetomium and Mycothypha were found. The diversity of fungi reflects the common fungal community of museums and archives. The spores and mycelial fragments of the fungi are obviously travelling on the legs and bodies of the silverfish. While the digitization of medical data within electronic health records has been introduced in some areas, massive amounts of paper-based health records are still produced on a daily basis. This data has to be stored for decades due to legal reasons but is of no benefit for research organizations, as the unstructured medical data in paper-based health records cannot be efficiently used for clinical studies. This paper presents a system for the recognition and privacy preservation of personal data in paper-based health records with the aim to provide clinical studies with medical data gained from existing paper-based health records.

Recent epidemiological investigations have shown that approximately 2-3% of all Austrians suffer from diabetes with renal involvement, i. e. 250,000 people in Austria are affected. The risk of occurrence and progression of this disease can be ameliorated by life style interventions as well as optimization of blood pressure, blood glucose levels and special drug classes. The present article represents the joint recommendations of the Austrian Diabetes Association and the Austrian Society for Nephrology for the diagnostics and treatment strategies of diabetic kidney disease.

The middle of the 19th century marks the beginning of a global process of science and knowledge transfer from -Europe to the rest of the world. During the phase of globali-zation, Austrian physician and ethnographer Jacob E. Polak (1818-1891) played a key role in the transmission of practical and scientific reasoning, leading to the development of medical science and clinical care in Persia. In 1851, Polak was commissioned by the Persian court to work as an academic teacher at the first secular institution of higher learning in Teheran, the Dār al-Fonūn. After 4 years of teaching and working as a doctor and surgeon, Polak was appointed personal physician to the Qājār king, Nāṣer-ad-Dīn Shāh (r. 1848-1896). During Polak's 9 year stay in Persia, he performed numerous surgical operations with specific focus on lithotomies on men and women of all ages. He documented each operation and collected samples of bladder calculi for the purpose of chemical analysis. After his return to Austria, he published a detailed report on his practice of lithotomy in Persia. This extensive documentation is, we believe, the only known historical evidence that currently exists of the introduction of modern urology to Persia. This study will present Polak's role as a pioneer of modern medicine and lithotomy, and will examine how he introduced the latest achievements of Viennese medicine in the field of operative urology to Persia., (© 2018 S. This paper gives a proposal for a standardised agar diffusion susceptibility testing method with 25 micrograms fluconazole discs. The methodology compiles the results of several years of work to develop a reliable and reproducible routine-method for the microbiology laboratory. In this proposal, in addition, the critics and experiences of a collaborative study for susceptibility testing of fluconazole with 21 laboratories from Germany and Austria are included.

This paper is general statement about the psychosocial care of refugees, developed by the of the Austrian Society for Psychiatry, Psychotherapy and Psychosomatics as well as by the Austrian Society for Social Psychiatry. Reasons of recent wave of refugees and the usual mental disorders among refugees are described. The increasing number of refugees, asylum seekers and irregular migrants poses a challenge for mental health services in Europe. As a consequence, several expert recommendations were given. As a first step, basic needs of refugees such as regular housing facilities, sufficient nutrition and safety in everyday life should be met. Fast decisions about the applications for asylum and support of social integration through education and employment are essential for mental health. Provision of mental health care should be needs based as described by the intervention pyramid for mental health and psychosocial support as given by the Inter-Agency Standing Committee of the World Health Organization. In addition, culture-specific aspects must be considered in everyday clinical work and the training of health professionals.

In some cases terminally ill patients fear of prolonged dying and suffering can manifest itself in the voluntary refusal of food and fluids, aiming to accelerate the dying process. This represents a considerable area of conflict, because of the ethical responsibility to not aid a person's death but also to respect a patients autonomy.There is a clear separation between an assisted suicide and following a patient's wishes. Not to accept the voluntary refusal of FVNF would have to be considered as forced treatment of patients while they are capable of self-determination.Several symptoms associated with or caused by voluntary refusal of food and fluids do require palliative care. It is important to be aware, that caring for dying patients refusing food and fluids and accepting their choice is not synonymous with assisted suicide. Rather is it part of medical and nursing care for patients during their dying-process.An interprofessional working group of the Austrian Palliative Society (OPG) intends to shed light on the legal, ethical, medical and nursing related aspects concerning this subject of growing public and professional interest.

Despite many positive developments, postoperative pain and its treatment is still not always given the necessary attention. Severe pain after surgical procedures affects a significant proportion of patients. This very fact is not only detrimental to the immediate recovery process, but can also form the basis for the development of chronic pain conditions.An adequate and effective management of perioperative pain requires appropriate organizational structures. This multidisciplinary paper which was initiated by the Austrian Society for Anaesthesiology and Intensive Care and the Austrian Pain Society and developed together with numerous specialist and professional societies dealing with the subject aims at supporting the organization of perioperative pain management structures and to make best use of proven concepts. Additional recommendations describe specific interventions for selected types of intervention.

As a part of our research in the field of thermochemical energy storage, this study aims to investigate the potential of three fly ash samples derived from the fluidized bed reactors of three different pulp and paper plants in Austria for their use as thermochemical energy (TCES) and CO2 storage materials. The selected samples were analyzed by different physical and chemical analytical techniques such as X-ray fluorescence spectroscopy (XRF), X-ray diffraction (XRD), particle size distribution (PSD), scanning electron microscopy (SEM), inductively coupled plasma atomic emission spectroscopy (ICP-OES), and simultaneous thermal analysis (STA) under different atmospheres (N2, CO2, and H2O/CO2). To evaluate the environmental impact, leaching tests were also performed. The amount of CaO as a promising candidate for TCES was verified by XRF analysis, which was in the range of 25–63% (w/w). XRD results indicate that the CaO lies as free lime (3–32%), calcite (21–29%), and silicate in all fly ash samples. The results of STA show that all fly ash samples could fulfill the requirements for TCES (i.e., charging and discharging). A cycling stability test of three cycles was demonstrated for all samples which indicates a reduction of conversion in the first three reaction cycles. The energy content of the examined samples was up to 504 kJ/kg according to the STA results. More energy (~1090 kJ/kg) in the first discharging step in the CO2/H2O atmosphere could be released through two kinds of fly ash samples due to the already existing free lime (CaO) in those samples. The CO2 storage capacity of these fly ash samples ranged between 18 and 110 kg per ton of fly ash, based on the direct and dry method. The leaching tests showed that all heavy metals were below the limit values of the Austrian landfill ordinance. It is viable to say that the valorization of fly ash from the pulp and paper industries via TCES and CO2 storage is plausible. Abstract: Introduction: All hospitals in the province of Styria (Austria) are well equipped with sophisticated Information Technology, which provides all-encompassing on-screen patient information. Previous research made on the theoretical properties, advantages and disadvantages, of reading from paper vs. reading from a screen has resulted in the assumption that reading from a screen is slower, less accurate and more tiring. However, recent flat screen technology, especially on the basis of LCD, is of such high quality that obviously this assumption should now be challenged. As the electronic storage and presentation of information has many advantages in addition to a faster transfer and processing of the information, the usage of electronic screens in clinics should outperform the traditional hardcopy in both execution and preference ratings. This study took part in a County hospital Styria, Austria, with 111 medical professionals, working in a real-life setting. They were each asked to read original and authentic diagnosis reports, a gynecological report and an internal medical document, on both screen and paper in a randomly assigned order. Reading comprehension was measured by the Chunked Reading Test, and speed and accuracy of reading performance was quantified. In order to get a full understanding of the clinicians'' preferences, subjective ratings were also collected. Results: Wilcoxon Signed Rank Tests showed no significant differences on reading performance between paper vs. screen. However, medical professionals showed a significant (90%) preference for reading from paper. Despite the high quality and the benefits of electronic media, paper still has some qualities which cannot provided electronically do date. [Copyright &y& Elsevier]

Purpose: More than 20 years ago we reported an analysis of a case series of elderly critically ill patients with acute kidney injury (AKI)--then termed acute renal failure. At that time, AKI was regarded as a "simple" complication, but has since undergone a fundamental change and actually has become one of the central syndromes in the critically ill patient., Methods: We have analyzed elderly patients above 65 years of age with an AKI defined as serum creatinine above 3 mg/dl corresponding to modern KDIGO stage 3, most of them requiring renal replacement therapy (RRT). Using an extremely complete data set the diagnosis differentiated the underlying disease entity, the dominant cause of AKI, acute and chronic risk factors (comorbidities). Special aspects such as severity of disease, early AKI at admission versus late AKI, early versus later start of RRT, AKI not treated by RRT in spite of indication for RRT, various measures of short-term and long-term prognosis, renal outcome, patients dying with resolved AKI, and causes of death were evaluated., Results: Crude mortality was 61% which corresponds to modern studies with gross variation among the different subgroups. Age per se was not a determinant of survival either within the group of elderly patients or as compared to younger age groups. Despite an increase in mean age and disease severity during the observation period prognosis improved. A total of 17% of patients developed a chronic kidney disease. Long-term survival as compared to the general population was low., Conclusions: A look back at the last two decades illustrates a remarkable evolution or rather metamorphosis of a syndrome. AKI has evolved as a central syndrome in intensive care patients, a systemic disease process associated with multiple systemic sequels and extra-renal organ injury and exerting a pronounced effect on the course of disease and short- and long-term prognosis not only of the patient but also of the kidney. Moreover, the "non-renal-naïve" elderly patient with multiple comorbidities has become the most frequent ICU patient in industrialized nations.

The use of ECMO to stabilize critically ill patients with severely depressed cardiac function and hemodynamics increased in the last years due to broader availability, better performance and easier implantation of the devices. The present guidelines of the Austrian Society of Cardiology focus on the use of ECMO in adult non-operated patients with cardiac diseases. Not only indications and contraindications are highlighted, but also the equally important issues of monitoring, complication management, measures during implantation and operation, and weaning of the devices are treated in detail. Thereby the present guidelines aim to optimize the use of ECMO in the individual centers, and aim to help current non-ECMO centers in developing a local ECMO-program or to contact ECMO-centers for discussion of individual patients.

The article highlights the discussion regarding the influx of refugees from Hungary, measures to maintain neutrality, coordination with international organizations, and potential appeals to other countries for assistance. It mentions rumors of Austria's involvement in violating neutrality by supporting the Hungarian uprising through the smuggling of military personnel and volunteers.

The approach to the support of gifted and talented children and youth has changed considerably in the past twenty years. While, until the 2000s, provision programmes predominantly focused on extracurricular activities for pupils, gifted education now follows a systemic and inclusive approach, including all (educational) institutions--kindergarten, school, college and university--as well as the family, the economy, the working world and the community. Furthermore, there have been considerable efforts to provide for gifted children within the regular classroom by way of differentiation and individualised learning. This new approach to talent support was first outlined in 2011 in the "White Paper Promoting Talent and Excellence," which the Austrian Research and Support Centre for the Gifted and Talented published in cooperation with the interministerial Giftedness Research and Gifted Education Task Force. The present article outlines the reasons that led to this new strategy, explains its main features and attempts an analysis of its strengths and weaknesses.

The current White paper summarizes the discussions and exchange of experiences during the first European Interdisciplinary Summit on Cell-Based ATMPs held in Vienna, Austria, May 02-03, 2013. The meeting was supported by the Research Networking Programme REMEDIC (regenerative medicine) funded by the European Science Foundation and by the British Medical Research Council. To improve the competitiveness of Europe in the field of cell-based Advanced Medicinal Therapy Products (ATMPs), the following key issues were identified during the meeting: removal of national hurdles in the European Union, harmonization of national and subnational differences in Hospital Exemption rules, improved treatment algorithms for reimbursement, better knowledge on the mode of action, predictive preclinical efficacy and safety testing, need for innovative systems for preclinical testing, appropriate product characterization, manufacturing with cost of goods in mind, and appropriate design of clinical trials.

In the 1920s, experiments with apparatus-supported lie detection and registration of expression were conducted at the Criminological Institute of the University of Graz in order to establish a sound methodological basis for testimony research. For this purpose, the criminologist Ernst Seelig used a method of lie detection developed by the psychologist Vittorio Benussi, which focuses on the analysis of breathing. Benussi had stated that the expiration after telling a lie was faster than after telling the truth, but Seelig could not verify this rule in forensic practice. Consequently, this method of lie detection was of no practical use for criminology. Seelig also carried out experiments with the method of registration of expression developed by the psychiatrist Otto Lowenstein. He registered the examinee's thoracic and abdominal breathing and the movements of the extremities with the help of a kymograph. By interpretation of the curves recorded on soot blackened paper, conclusions concerning the mental elements of an offence as well as the existence of certain dispositions and of amnesia should have been made possible. Seelig was convinced of the efficiency of this method. These experiments can be regarded as early attempts at finding not only simple facts but also answers to quasi-metaphysical questions concerning the "true nature" of man with the help of methods based on natural science and modern technology. Thus they are precursors of present-day neuroscience and neuro-imaging.

Summary: Objectives: In Austria, the general practitioner (GP) is the first point of contact for persons with health problems. Depending on the severity of the person''s medical condition, a GP may refer her or him to a secondary care hospital consultant, who reports findings back to the GP in form of a paper-based discharge letter. Researchers report that paper-based communication of medical documents between different health care providers is insufficient in quality, error prone and too slow in many cases. Our aim was to develop and to realise a strategy for a stepwise replacement of the paper-based transmission of medical documents with a distributed, shared medical record. Methods: In the first step of a three-steps strategy for development of a consistent, comprehensive and secure regional health care network, an electronic communication of discharge letters and diagnostic results between existing information systems of different health care providers in Tyrol, Austria, has been established: in the form of cryptographically signed S/MIME e-mail messages and, additionally, via a secure web portal system. In two further steps, an extension of the system by a bi-directional communication and by improvements of the web portal system is planned, leading to a comprehensive electronic patient record for shared care. Results: After realisation of step 1, in October 2004, about 3500 electronic discharge letters were sent out from the Innsbruck University Hospital (IUH), which represents about 8% of the total number of discharge letters of the IUH. In addition, a lot of feedback was received and legal, organisational, financial and methodical difficulties were overcome. Discussion: The stepwise approach to replace paper-based with electronic communication in the first step was helpful, since knowledge has been gained and cooperations were formed. For the realisation of a distributed, shared medical record (steps 2 and 3), it will not be sufficient only to replace paper-based transmission of medical documents with electronic communication technologies, but in the further steps, organisational changes will become necessary. As well, legal ambiguities must be resolved before a distributed medical record for cooperative care, used by several institutions as well as by patients, could be established. The article offers the authors insight on their reply to the comment by Fügenschuh and colleagues on their paper titled "Estimating displacement along the Brenner Fault and orogen-parallel extension in the Eastern Alps." The authors note that the zircon fission track ages in the footwall of Brenner Fault (BF) in Eastern Alps reflect the antiform of Tauern Window (TW). They cite that there is a link between ductile mylonites and brittle faulting. They say that the trend of ductile mylonitic belt continues into the Austroalpine Units as exhumation was insufficient to attain brittle/ductile transition. They mention the tectonic omissions and the new estimate of extension based on stratigraphic omissions at Brenner Pass in Austria.

This article provides an overview of the selected papers of the 1st Joint European Stable Isotope Users Group Meeting (JESIUM) in Vienna, Austria from August 30 through September 3, 2004. The intention of JESIUM 2004 was to harness the spirit of the established national meetings and scale it up to a European level, to broaden the scope beyond national borders in order to discuss ideas and methodologies, foster new collaborations and provide a platform to air future ideas and applications in stable isotope research.
